Visuo-spatial training is considered a promising approach to provide young children with a sound foundation for later mathematical learning. We developed and implemented a tablet-based visuo-spatial intervention in kindergarten classrooms aiming to foster the development of children’s visuo-spatial and numerical abilities. A sample of N?=?125 children participated in the present study, 68 children were part of the intervention group and participated in 20 training sessions of 20?min over a 10-week period, 57 children formed a business as usual control group. Results show that, at this young age, visuo-spatial and early math skills are already strongly interlinked. However, the training effects were domain-specific as they only improved visuo-spatial skills, but did not transfer to early math performance in the present setting.  相似文献

This pilot study tested the efficacy of an audio–visual stimulation (AVS) program for the promotion of sleep in individuals with chronic pain. Insomnia and chronic pain are common comorbid conditions and their relationship has been viewed as bidirectional. Recent studies suggest a relatively dominant role of sleep in this dyad. The premise of this pilot study was that AVS enhances low frequency while reducing high frequency brain activity resulting in decreased hyperarousal and improved sleep with potential consequent reduction in pain. We conducted a pilot intervention study of AVS using a pre–post design. Participants self-administered a 30-min AVS program nightly at bedtime for 1 month. Sleep and pain were assessed at baseline and at the conclusion of the 4-week intervention phase. Nine adults (mean age 33 ± 15.8 years; female, 89 %) completed the study. After using the AVS device for 4 weeks, significant improvement was seen in reported insomnia (ISI, p = 0.003), pain severity (BPI, p = 0.005), and pain interference with functioning (BPI, p = 0.001). Large effect sizes (Partial η2 0.20–0.94) (Cohen’s d 0.44–1.45) were observed. The results of this pilot study suggest that the AVS program may be efficacious in decreasing both insomnia and pain symptoms. In order to better assess the efficacy of AVS for sleep promotion and possible pain reduction, more definitive randomized controlled trials will be needed. These should include appropriate sample sizes, objective measures of sleep and pain, and longitudinal follow-up.  相似文献

Studies suggest that there may be an association between sleep and growth; however, the relationship is not well understood. Changes in biology and external factors such as school schedule heavily impact the sleep of adolescents, during a critical phase for growth. This study assessed the changes in sleep across school days, weekends and school holidays, while also measuring height and weight changes, and self-reported alterations in food intake and physical activity. The impact of morningness–eveningness (M-E) on height change and weight gain was also investigated. In a sample of 63 adolescents (mean age = 13.13, SD = 0.33, 31 males) from two independent schools in South Australia, height and weight were measured weekly for 4 weeks prior to the school holidays and 4 weeks after the school holidays. Participants also completed a Morningness/Eveningness Scale and 7-day sleep, diet and physical activity diaries prior to, during and after the school holidays. Participants at one school had earlier wake times during the weekends than participants attending the other school, leading to a significantly shorter sleep duration on weekends for those participants. Regardless of school, sleep was significantly later and longer during the holidays (< 0.001) and those with a stronger morning preference fell asleep (F18,36 = 3.4, = 0.001) and woke (F18,44 = 2.0, = 0.027) earlier than evening types. Growth rate was lower during the holiday weeks. For those attending the school with limited sleep in opportunities, growth after the holidays was lower for those with greater evening preference, whereas for those at the other school, growth was greater for those with greater evening preference. The increase in average weight from pre- to post-holidays was greater for those attending the school with limited opportunities to sleep longer. Participants reported greater food intake during the holidays compared to school days and greater physical activity levels on weekends compared to school days, and school days compared to holidays. Results suggest that time of day preference may impact growth, with evening types who cannot sleep in growing at a slower rate than evening types who can or morning types. This may be related to sleep restriction. Despite sleep being both later and longer during the school holidays, participants’ growth slowed during the holiday period. It is possible that this may be a reflection of other behavioural changes in the holidays (increased food intake and reduced physical activity), as sleep timing during the school period was related to growth.  相似文献

ABSTRACT

The purpose of the study was to evaluate the recuperative efficacy of pre-exercise napping on physical capacity after military sustained operations (SUSOPS) with partial sleep deprivation. Before and after a 2-day SUSOPS, 61 cadets completed a battery of questionnaires, and performed a 2-min lunges trial and a 3,000-m running time-trial. After the completion of SUSOPS, subjects were randomized to either a control [without pre-exercise nap (CON); n = 32] or a nap [with a 30-min pre-exercise nap (NAP); n = 29] group. SUSOPS enhanced perceived sleepiness and degraded mood in both groups. Following SUSOPS, the repetitions of lunges, in the CON group, were reduced by ~ 2.3%, albeit the difference was not statistically significant (p = 0.62). In the NAP group, however, the repetitions of lunges were increased by ~ 7.1% (p = 0.01). SUSOPS impaired the 3,000-m running performance in the CON group (~ 2.3%; p = 0.02), but not in the NAP group (0.3%; p = 0.71). Present results indicate, therefore, that a relatively brief pre-exercise nap may mitigate physical performance impairments ensued by short-term SUSOPS.  相似文献

This study investigated the effect of using an artificial bright light on the entrainment of the sleep/wake cycle as well as the reaction times of athletes before the Rio 2016 Olympic Games. A total of 22 athletes from the Brazilian Olympic Swimming Team were evaluated, with the aim of preparing them to compete at a time when they would normally be about to go to bed for the night. During the 8-day acclimatization period, their sleep/wake cycles were assessed by actigraphy, with all the athletes being treated with artificial light therapy for between 30 and 45 min (starting at day 3). In addition, other recommendations to improve sleep hygiene were made to the athletes. In order to assess reaction times, the Psychomotor Vigilance Test was performed before (day 1) and after (day 8) the bright light therapy. As a result of the intervention, the athletes slept later on the third (p = 0.01), seventh (p = 0.01) and eighth (p = 0.01) days after starting bright light therapy. Regarding reaction times, when tested in the morning the athletes showed improved average (p = 0.01) and minimum reaction time (p = 0.03) when comparing day 8 to day 1. When tested in the evening, they showed improved average (p = 0.04), minimum (p = 0.03) and maximum reaction time (p = 0.02) when comparing day 8 to day 1. Light therapy treatment delayed the sleep/wake cycles and improved reaction times of members of the swimming team. The use of bright light therapy was shown to be effective in modulating the sleep/wake cycles of athletes who had to perform in competitions that took place late at night.  相似文献

Backround: Enuresis implies severe stress in affected children, and impairs quality of life and sleep. Children with enuresis experience difficulties in their arousal from sleep, possibly associated with disturbances of the circadian rhythm. In this study, we aimed to evaluate the sleep–wake cycle and sleep disturbances in children with monosymptomatic enuresis nocturna (MEN). Method: The study comprised 70 children with MEN who were admitted to the pediatrics and urology outpatients department and 94 age-matched healthy controls. Parents completed “Strengths and Difficulties Questionnaire,” Children’s Sleep Habits Questionnaire (CSHQ), Children’s Chronotype Questionnaire scale. Results: Children with enuresis had significantly more sleep and psychological problem. Enuresis group reported higher bedtime resistance, parasomnias, breathing-related problems, and daytime sleepiness in CHSQ (p < 0.05). Although circadian preference did not differ statistically between the groups (p > 0.05), sleep duration on school days and awakening and mid-sleep points, both on scheduled and free days, were found to be significantly different in the enuretic group (p < 0.05). In logistic regression analysis, age, sleep period on scheduled days, sleep inertia on scheduled and free days were significant predictor for enuresis. Discussion: Children with enuresis were more likely to experience problematic sleep. This may reflect that enuretic children have impaired sleep–wake cycles, leading to dysregulation of daily functional changes of bladder capacity and related hormones such as ADH. These findings might imply a sleep–wake disturbance in enuresis.  相似文献

This study evaluated sleep duration, travelling habits and jet lag effects in kite surfers according to their competition level. Ninety-four male kite surfers (34.3 ± 8.8 years) were evaluated through an online questionnaire in order to collect information on training volume, clinical history, anthropometric profile, sleep habits, fluid and fruit intake and jet lag effects on athletic performance. Mean sleep duration was 07 h 19 min ± 01 h 12 min on weekdays with 82.3% sleeping less than 8 h/night. Sleep duration was less on weekdays (p = 0.002) and weekends (p = 0.011) in kite surfers‘ squad members (SM) compared to no squad members (NoSM). Greater jet lag symptoms were reported following west-to-east flights. Kite surfers with SM arrived earlier at the competition destination (p = 0.019), were more likely to implement strategies to minimize travel effects (p = 0.003), but reported more symptoms of jet lag than did NoSM (p = 0.041). Travel effects were positively correlated with the distance travelled and negatively correlated with sleep duration on weekdays and water intake during travel to an international kite surfing competition. Jet lag negatively influenced kite surfers’ athletic performance with greater symptoms following west-to-east travel in both SM and NoSM.  相似文献

ABSTRACT

The present study aimed to explore the effects of a single 10-mg dose of melatonin (MEL) administration after exhaustive late-evening exercise on sleep quality and quantity, and short-term physical and cognitive performances in healthy teenagers. Ten male adolescent athletes (mean ± SD, age = 15.4 ± 0.3 years, body-mass = 60.68 ± 5.7 kg, height = 167.9 ± 6.9 cm and BMI = 21.21 ± 2.5) performed two test sessions separated by at least one week. During each session, participants completed the Yo-Yo intermittent-recovery-test level-1 (YYIRT-1) at ~20:00 h. Then, sleep polysomnography was recorded from 22:15 min to 07:00 h, after a double blind randomized order administration of a single 10-mg tablet of MEL (MEL-10 mg) or Placebo (PLA). The following morning, Hooper wellness index was administered and the participants performed the Choice Reaction Time (CRT) test, the Zazzo test and some short-term physical exercises (YYIRT-1, vertical and horizontal Jumps (VJ; HJ), Hand grip strength (HG), and five-jump test (5-JT)). Evening total distance covered in the YYIRT-1 did not change during the two conditions (p > 0.05). Total sleep time (Δ = 24.55 mn; p < 0.001), sleep efficiency (Δ = 4.47%; p < 0.001), stage-3 sleep (N3 sleep) (Δ = 1.73%; p < 0.05) and rapid-eye-movement sleep (Δ = 2.15%; p < 0.001) were significantly higher with MEL in comparison with PLA. Moreover, sleep-onset-latency (Δ = –8.45mn; p < 0.001), total time of nocturnal awakenings after sleep-onset (NA) (Δ = –11 mn; p < 0.001), stage-1 sleep (N1 sleep) (Δ = –1.7%; p < 0.001) and stage-2 sleep (N2 sleep) (Δ = ?1.9%; p < 0.05) durations were lower with MEL. The Hooper index showed a better subjective sleep quality, a decrease of the subjective perception of fatigue and a reduced level of muscle soreness with MEL. Moreover, MEL improved speed and performance but not inaccuracy during the Zazzo test. CRT was faster with MEL. Morning YYIRT-1 (Δ = 82 m; p < 0.001) and 5-JT (Δ = 0.08 m; p < 0.05) performances were significantly higher with MEL in comparison with PLA. In contrast, HG, VJ and HJ performances did not change during the two conditions (p > 0.05). The administration of a single dose of MEL-10 mg after strenuous late-evening exercise improved sleep quality and quantity, selective attention, subjective assessment of the general wellness state, and some short-term physical performances the following morning in healthy teenagers.  相似文献

Schedule changes associated with rotating shifts can interfere with the circadian rhythms of nurses and thereby affect their sleep duration, sleep quality, work efficiency, and work performance. The objectives of this study was to investigate differences in workday sleep fragmentation, rest-activity cycle, sleep quality, and activity level among nurses working different shifts. After filling out a basic information questionnaire and completing the Pittsburgh Sleep Quality Index (PSQI) questionnaire, participants were asked to wear an actigraph and keep sleep records for seven consecutive days. Data pertaining to wake after sleep onset (WASO), 24-hour autocorrelation coefficient (r24), and daytime activity mean was collected in order to investigate workday sleep fragmentation, rest-activity cycle, and daytime activity level. We obtained complete questionnaires and data from 191 nurses. Day- and evening-shift nurses had more regular workday rest-activity cycles than did night-shift nurses (F = 51.26, p < .001). After controlling for r24 coefficients, we determined that nurses who experienced greater workday sleep fragmentation had higher PSQI scores (β = .18, p = .008). After controlling for WASO times, we determined that nurses who had more regular rest-activity cycles on workdays had lower PSQI scores (β = – .16, p = .036). After controlling for shift type and WASO times, we determined that nurses with higher PSQI scores displayed lower activity levels (β = – .21, p = .015) and those with higher r24 coefficients displayed higher activity levels (β = .18, p = .040) on workdays. We then examined the causal path relationships. Among the shifts, only the day-shift nurses had a higher r24 (β = ?.59, p < .001) than did the night-shift nurses; WASO exerted a significant impact on PSQI scores (β = .20, p = .002); r24 had a significant and negative influence on PSQI scores (β = ?.38, p < .001), and PSQI scores significantly and negatively influenced workday activity levels (β = ?.20, p = .006). This study determined that day- and evening-shift nurses enjoyed more regular and consistent rest-activity cycles than did night-shift nurses; nurses with greater workday sleep fragmentation and/or more irregular rest-activity cycles experienced poorer sleep quality; and nurses suffering from poorer sleep quality displayed lower daytime activity levels on workdays.  相似文献

Men are more evening oriented than women. These sex difference might be a result of gonadal hormones, especially of testosterone. Here, we tested for a causal relationship by shifting the sleep timing of young men to one hour later and assessed the influence on saliva testosterone. 50 men participated in our study (31 experimental, 19 control group). Sleep timing was measured with actigraphy. One week served as baseline and afterwards, participants were randomly assigned to either the control or the experimental group. In the experimental group, men were asked to go to bed about one hour later and to sleep one hour longer in the morning. Participants in the control group experienced no significant changes in sleep timing. In the experimental group, participants went to bed later and woke up later; therefore, mid-point of sleep was shifted to a later time and sleep duration decreased, because he men went to bed about one hour later but woke up only about 30 min later. The control group experienced a decline in testosterone, but the experimental group not. Changes in mid-point of sleep between baseline week and experimental week were correlated with the change in testosterone (r = 0.285).  相似文献

Across a wide range of tasks, cognitive functioning is affected by circadian fluctuations. In this study, we investigated diurnal variations of working memory performance, taking into account not only hits and errors rates, but also sensitivity (d’) and response bias (c) indexes (established by signal detection theory). Fifty-two healthy volunteers performed four experimental tasks twice – in the morning and in the evening (approximately 1 and 10 h after awakening). All tasks were based on Deese–Roediger–McDermott paradigm version dedicated to study working/short-term memory distortions. Participants were to memorize sets of stimuli characterized by either conceptual or perceptual similarity, and to answer if they recognized subsequent stimulus (probe) as an “old” one (i.e. presented in the preceding memory set). The probe was of three types: positive, negative or related lure. In two verbal tasks, memory sets were characterized by semantic or phonological similarity. In two visual tasks, abstract objects were characterized by a number of overlapping similarities or differed in only one detail. The type of experimental material and the participants’ diurnal preference were taken into account. The analysis showed significant effect of time of day on false alarms rate (F(1,50) = 5.29, p = 0.03, ηp2 = 0.1) and response bias (F(1,50) = 11.16, = 0.002, ηp2 = 0.18). In other words, in the evening participants responded in more liberal way than in the morning (answering “yes” more often). As the link between variations in false alarms rate, response bias and locus coeruleus activity was indicated in literature before, we believe that our data may be interpreted as supporting the hypothesis that diurnal fluctuations in norepinephrine release have effect on cognitive functioning in terms of decision threshold.  相似文献

The nature of on-call work is such that workers can be called and required to respond immediately after being woken. However, due to sleep inertia, impaired performance immediately upon waking is typical. We investigated the impact of a preceding stressor (an alarm/mobilisation procedure) on sleepiness and performance upon waking. Healthy, adult males (n = 16) attended the sleep laboratory for four consecutive nights which included two, counterbalanced on-call sleeps where participants were woken at 04:00 h by (a) an alarm/mobilisation procedure (Alarm) or (b) gently by a researcher (Control). Following waking was a 2-h testing session comprising the repeated administration of the Karolinska Sleepiness Scale (KSS) and 5-min Psychomotor Vigilance Task (PVT). Results from within-subjects analysis of variance in both the Control and Alarm conditions showed that for subjective sleepiness (KSS) there was a significant fixed effect of time (p = 0.012), with participants becoming less sleepy as time post-wake increased. In terms of PVT performance outcomes, in neither the Alarm or Control conditions were there measurable signs of sleep inertia with performance remaining stable across the 2-h testing period. Based on previous research measuring impact of sleep inertia when woken near the circadian nadir, performance findings in particular were unexpected. We propose that stress caused by study procedures (i.e. finger pricks using lancets) unrelated to the simulated wake-up protocols may have countered any impact of sleep inertia on performance.  相似文献

A chronotype is an individual trait that determines circadian rhythm (dark/light cycle) characteristics, associated with bedtime, waking, and other daily activities. A chronotype is classified as morning, intermediate, and evening. The objective is to associate chronotypes with academic performance in university students. A cross-sectional study was performed to evaluate the chronotype of university students (n = 703) by Horne-Ostberg questionnaire and associated with academic performance. The group with higher GPAs had higher chronotype scores (p = 0.002). Morning and intermediate chronotypes exhibited better academic performance; however, more studies are necessary to determine the underlying causes, which could influence cognitive aspects.  相似文献

This study examined the sleep intra-individual variability (IIV) of rugby league athletes across senior and junior levels during one week of the competitive season. Forty-five rugby league athletes across elite senior, sub-elite senior and elite junior levels each wore actigraphy monitors for seven days during the competitive season, and completed a subjective sleep diary each morning upon waking. Linear mixed models were used to assess differences in sleep measures between playing levels. Intra-individual standard deviations for each sleep measure were calculated for each athlete to determine their respective IIV, allowing differences in IIV between groups to be assessed. Elite junior athletes went to bed later (ES = 0.94 ± 0.50, < 0.05) and woke later than elite senior athletes (ES = 0.94 ± 0.40, < 0.05), while also displaying greater IIV when considering time in bed (ES = 1.14 ± 0.60, < 0.05) and sleep duration (ES = 1.53 ± 0.64, < 0.05) compared with elite senior athletes. Similarly, IIV was more pronounced in elite junior players for time in bed (ES = 0.88 ± 0.60, < 0.05) and sleep duration (ES = 1.03 ± 0.64, < 0.05) compared with sub-elite senior athletes. Despite this, elite junior athletes still obtained sufficient sleep duration, efficiency and quality. The outcomes of this investigation show a distinct difference in the habitual sleep-wake patterns of senior and junior rugby league athletes, and show how sleep IIV differs between playing levels in rugby league.  相似文献

One of the most important issues nowadays is memory disorders. The exact circadian modulation of learning and memory is still under investigation. The present study was carried out to evaluate the probable enhancing and neuroprotective effects of the Egyptian Moringa peregrina; administered at two different Zeitgeber times (ZT), on learning and memory in mice. M. peregrina dried leaves were aqueously extracted and intranasally (IN) administered at four doses (viz. 12.5, 25, 50, and 100 mg/kg). Twenty animal groups (n = 8/group) were treated with M. peregrina at ZT 3:00 vs. 15:00 representing the rest (day) vs. active (night) phases, respectively. Additionally, sub-chronic effect of daily IN administration of the 25 mg/kg dose for two weeks was examined at ZT 3:00 vs. 15:00. Memory performance was assessed after 1, 2 weeks of treatment and a 3rd recovery week. Memory test was performed at ZT 17:00 by measuring the percentage of time spent in novel arm of Y-maze. Dose-response curve revealed that both 25 and 50 mg/kg doses administered at ZT 3:00 significantly improved memory efficiency. Rest phase administration significantly enhanced memory functions after 1, 2 weeks as well as after the recovery week. Moreover, IN pretreatment with M. peregrina (25 mg/kg) revealed a protective effect against scopolamine-induced amnesia. In conclusion, these findings clearly indicated that M. peregrina possesses valuable enhancing and protective effects on learning and memory processes in mice which is circadian-phase dependent.  相似文献

The present study investigated social jetlag in school students of the Raipur district of Chhattisgarh. The sample consisted of 919 school students from six different schools, age range 12–19 year, comprising of both males (n = 513) and females (n = 406). Two inventories, the 19-item MEQ and MCTQ (Munich Chronotype Questionnaire) were employed. Data analysis (SPSS) used t-test, Chi-square test, ANOVA, and correlation. The results revealed that 73.9% of the sample had social jetlag. There were significant effects upon social jetlag of chronotype, gender, and the use of an alarm clock. Owls were at a higher risk of having social jetlag. There was a significant positive correlation between the amount of social jetlag and the time spent watching television. From these results, it is concluded that there is prevalence of social jetlag, particularly among boys and those who use an alarm clock. The results also lead to the prediction that those school students with social jetlag will be predisposed to health hazards associated with it in the future. Thus it is recommended that school students must be provided with sleep education so that they understand the role of sleep on health and do not become sleep deprived.  相似文献

When shift nurses change shifts, it is likely to affect the cortisol patterns of their bodies and sleep quality. The objectives of this study was to verify the influence of monthly rotating day, evening and night shifts on the sleep quality of female nurses and determine whether the cortisol awakening response (CAR) mediates this relationship. A total of 132 female shift nurses were recruited, and ultimately 128 complete questionnaires and samples were obtained (subject loss rate = 3.0%) from 45 day-shift nurses, 44 evening-shift nurses and 39 night-shift nurses at a teaching hospital in Northern Taiwan. The Pittsburgh sleep quality index served as the research instrument that nurses used to collect saliva samples at home every day after waking and 30?min after waking so as to calculate the net increases in cortisol levels (CARi). Hierarchical multiple regression was employed to examine the influence of shift type on the sleep quality of the female nurses and the mediating effect of CARi. The results of this study indicate that shift type significantly influenced CARi (F = 19.66, p < 0.001) and that the regression coefficients of evening versus day shifts and night versus day shifts are both negative. Shift type also significantly influenced sleep quality (F = 15.13, p < 0.001), and the regression coefficients of evening versus day shifts and night versus day shifts are both positive. After controlling for the influence of shift type, CARi remained significantly correlated with sleep quality (ΔF = 5.17, p = 0.025). The results show that female evening-shift or night-shift nurses display significantly lower CARi and experience significantly poorer sleep quality than day-shift nurses. A greater CARi in the female shift nurses represents better sleep quality. Furthermore, the results prove that CARi is a mediating variable influencing the sleep quality of female shiftwork nurses.  相似文献

The medical career is considered highly stressful, especially during internships when academic and clinical demands, combined with changes in sleep patterns, increase students’ likelihood to develop depression. Resilience, which is considered as opposite vulnerability to stress and, along with another protective factor, namely morningness, may cause a student to be less reactive to stimuli and, therefore, less prone to depression. The objective of this study was to evaluate the role of resilience and morningness facing to sleep quality and main risk factors, on the development of depression symptoms in a group of students with sleep pattern alterations. To this end, an observational and longitudinal study was performed with 30 undergraduate interns, with an average age of 22.63 years (SE ± 0.13), 33% men and 67% women. A survey was conducted in three different times during the year of internship: at the beginning (T1), in the middle (T2) and the end (T3). The instruments were the Brief Resilience Scale, Composite Scale of Morningness, Pittsburgh Sleep Quality Index and Patient Health Questionnaire. The path analysis examined the roles of morningness, sleep quality and resilience as potential mediators between family history of depression and depression symptoms at different times. The results showed that resilience had a protective effect on depression symptoms at T2 (β = ?0.18, p < 0.05) and with greater power at T3 (β = ?0.41, p < 0.05), as did morningness, although less strongly, on the symptoms at T3 (β = ?0.13, p < 0.05). A relationship between these two mediating variables was also observed (β = 0.30, p < 0.05). The initial sleep quality had an effect on the increase of depression symptoms at T1 (β = 0.61, p < 0.05) and T3 (β = 0.21, p < 0.05), while family history of depression had a direct effect on the measures of depression at T2 (β = 0.49, p < 0.05) and T3 (β = 0.19, p < 0.05). Aside from personal risk factors, it is possible to conclude that the levels of resilience, morningness and sleep quality manifested by students at the beginning of their internship may explain the decrease in depression symptoms at the end of the course.  相似文献

To investigate the effect of an eight-week, home-based, personalized, computerized cognitive training program on sleep quality and cognitive performance among older adults with insomnia.

Design

Participants (n = 51) were randomly allocated to a cognitive training group (n = 34) or to an active control group (n = 17). The participants in the cognitive training group completed an eight-week, home-based, personalized, computerized cognitive training program, while the participants in the active control group completed an eight-week, home-based program involving computerized tasks that do not engage high-level cognitive functioning. Before and after training, all participants'' sleep was monitored for one week by an actigraph and their cognitive performance was evaluated.

Setting

Community setting: residential sleep/performance testing facility.

Participants

Fifty-one older adults with insomnia (aged 65–85).

Interventions

Eight weeks of computerized cognitive training for older adults with insomnia.

Results

Mixed models for repeated measures analysis showed between-group improvements for the cognitive training group on both sleep quality (sleep onset latency and sleep efficiency) and cognitive performance (avoiding distractions, working memory, visual memory, general memory and naming). Hierarchical linear regressions analysis in the cognitive training group indicated that improved visual scanning is associated with earlier advent of sleep, while improved naming is associated with the reduction in wake after sleep onset and with the reduction in number of awakenings. Likewise the results indicate that improved “avoiding distractions” is associated with an increase in the duration of sleep. Moreover, the results indicate that in the active control group cognitive decline observed in working memory is associated with an increase in the time required to fall asleep.

Conclusions

New learning is instrumental in promoting initiation and maintenance of sleep in older adults with insomnia. Lasting and personalized cognitive training is particularly indicated to generate the type of learning necessary for combined cognitive and sleep enhancements in this population.

ABSTRACT

Individuals with an evening chronotype are at increased risk of experiencing emotional problems, including depressive symptoms. However, the mechanisms underlying these associations remain unclear. The present study aimed to determine whether poor sleep quality, substance use and cognitive emotion regulation difficulties – which have been implicated in the etiology of depression – mediate the relationship between chronotype and depressive symptoms in a student sample, which was assessed cross-sectionally and after 1 year. A total of 742 Dutch students (75% women, mean age 21.4 ± 2.9 years) completed the Quick Inventory of Depressive Symptomatology, the Morningness-Eveningness Questionnaire, the Pittsburgh Sleep Quality Index, a questionnaire assessing alcohol, caffeine, tobacco and cannabis use, the Cognitive Emotion Regulation Questionnaire and the Behavioral Inhibition/Activation Scale. A subsample (n = 115) was assessed 1 year later with the same questionnaires. Cross-sectional analyses showed that evening chronotype was associated with more depressive symptoms, adjusted for age and gender (β = ?0.082, p = 0.028). The relationship between eveningness and depressive symptoms was mediated by sleep quality, alcohol consumption and the cognitive emotion regulation strategies of self-blame and positive reappraisal. In longitudinal analyses, eveningness at baseline predicted more depressive symptoms at follow-up, adjusted for age and gender (β = ?0.29, p = 0.002); after additional adjustment for baseline depressive symptoms, chronotype remained a significant predictor of depressive symptoms at T2 (β = ?0.16, t = ?2.01, p = 0.047). Only poor sleep quality at follow-up was a significant mediator of this relationship. Even though the effect is small in terms of explained variance, eveningness is related to depressive symptoms and this relationship is mediated by poor sleep quality, also in a prospective design. Self-blame and reduced positive reappraisal are correlated with eveningness. Further research is needed to assess the efficacy of chronotherapeutic interventions for the prevention of depression, in addition to sleep education and cognitive approaches.  相似文献
