目的:探讨关节镜下胫骨嵌入术治疗膝关节后交叉韧带损伤患者的临床疗效及安全性。方法:回顾性分析2013 年2 月至 2014 年2 月在我院接受关节镜下胫骨嵌入术治疗的膝关节膝关节后交叉韧带损伤患者36 例的临床资料,比较手术前后 Lysholm 膝关节评分、KT-2000 关节测量值、后抽屉试验与Lachman 征的差异。结果:与手术前比较,手术后6 个月患膝Lysholm 膝关节评分、KT-2000 关节测量值及物理检查(后抽屉试验试验与Lachman 征)均明显改善,差异具有统计学意义(P<0.05)。全部患 者术后无韧带断裂、免疫排斥反应等现象。结论:关节镜下胫骨Inlay技术对膝关节PCL损伤患者的临床疗效显著,安全性高,值 得临床推广应用。  相似文献

目的:比较老年膝关节骨性关节炎予以保守治疗以及外科膝关节置换手术的治疗效果,以探讨其合理的干预方法。方法:选 取我院2009 年1 月-2013 年12 月间收治的50 例临床资料完整的老年膝关节骨性关节炎疾病患者,将其根据治疗方式的不同分 为保守治疗组17 例以及外科手术治疗组33 例,手术方法为膝关节置换术。比较术后的KSS 评分情况,HSS评分、膝关节优良率。 结果:治疗后手术治疗组患者的KSS 评分较治疗前明显改善,P<0.05。治疗后外科手术组患者的HSS 各项评分均明显优于保守治 疗组,治疗后手术组患者的膝关节功能优良率达到了100 %(33/33),明显优于保守治疗组35.29 %(6/17)的优良率,组间比较有统 计学差异,P<0.05。结论:对于老年膝关节骨性关节炎进行保守治疗可能不会对疾病的进展起到干预作用,而明确诊断的患者行全 膝关节置换术可以有效改善膝关节功能,提高生活质量。  相似文献

The aim of this study was to investigate the effect of high-intensity locomotor training on knee extensor and flexor muscle activation and adaptability to increased body-weight (BW) support during walking in patients with Parkinson’s disease (PD). Thirteen male patients with idiopathic PD and eight healthy participants were included. The PD patients completed an 8-week training program on a lower-body, positive-pressure treadmill. Knee extensor and flexor muscles activation during steady treadmill walking (3 km/h) were measured before, at the mid-point, and after training. Increasing BW support decreased knee extensor muscle activation (normalization) and increased knee flexor muscle activation (abnormal) in PD patients when compared to healthy participants. Training improved flexor peak muscle activation adaptability to increased (BW) support during walking in PD patients. During walking without BW support shorter knee extensor muscle off-activation time and increased relative peak muscle activation was observed in PD patients and did not improve with 8 weeks of training. In conclusion, patients with PD walked with excessive activation of the knee extensor and flexor muscles when compared to healthy participants. Specialized locomotor training may facilitate adaptive processes related to motor control of walking in PD patients.  相似文献

目的:探究膝关节单髁置换术(UKA)与全膝关节置换术(TKA)治疗膝关节内侧单间室骨性关节炎的临床治疗效果。方法:将2011年4月-2015年7月期间因膝关节单间室骨性关节炎入院接受治疗的89例患者纳入本研究,随机分为研究组和对照组,研究组44例,行UKA手术,对照组45例,采用TKA手术方式治疗。对两组患者进行术后随访,对比临床治疗效果。结果:两组术前均有明显膝关节疼痛,术后膝关节疼痛均明显改善,组间差别无显著统计学意义(X2=1.323,P=0.2500.05);术后膝关节屈曲角度、HSS评分相对于术前均显著改善,研究组术后膝关节屈曲角度(111.2±18.8)度高于对照组的(98.6±14.7)度,差异有统计学意义(P0.05);HSS评分(87.6±13.7)分高于对照组的(73.2±16.8)分,差异有统计学意义(P0.05);研究组膝关节屈曲至90度时间比对照组短,数据差异有统计学差异(t=-2.303,P=0.0240.05)。结论:膝关节内侧单间室骨性关节炎采用UKA与TKA均能取得一定临床效果,减轻患者痛苦,改善膝关节功能,但UKA临床疗效较好,手术创伤较小,术后恢复较快。  相似文献

Arthrofibrosis, which causes joint motion restrictions, is a common complication following total knee arthroplasty (TKA). Key features associated with arthrofibrosis include myofibroblast activation, knee stiffness, and excessive scar tissue formation. We previously demonstrated that adiponectin levels are suppressed within the knee tissues of patients affected by arthrofibrosis and showed that AdipoRon, an adiponectin receptor agonist, exhibited anti-fibrotic properties in human mesenchymal stem cells. In this study, the therapeutic potential of AdipoRon was evaluated on TGFβ1-mediated myofibroblast differentiation of primary human knee fibroblasts and in a mouse model of knee stiffness. Picrosirius red staining revealed that AdipoRon reduced TGFβ1-induced collagen deposition in primary knee fibroblasts derived from patients undergoing primary TKA and revision TKA for arthrofibrosis. AdipoRon also reduced mRNA and protein levels of ACTA2, a key myofibroblast marker. RNA-seq analysis corroborated the anti-myofibrogenic effects of AdipoRon. In our knee stiffness mouse model, 6 weeks of knee immobilization, to induce a knee contracture, in conjunction with daily vehicle (DMSO) or AdipoRon (1, 5, and 25 mg/kg) via intraperitoneal injections were well tolerated based on animal behavior and weight measurements. Biomechanical testing demonstrated that passive extension angles (PEAs) of experimental knees were similar between vehicle and AdipoRon treatment groups in mice evaluated immediately following immobilization. Interestingly, relative to vehicle-treated mice, 5 mg/kg AdipoRon therapy improved the PEA of the experimental knees in mice that underwent 4 weeks of knee remobilization following the immobilization and therapy. Together, these studies revealed that AdipoRon may be an effective therapeutic modality for arthrofibrosis.  相似文献

目的:探讨严重骨质疏松患者在进行人工全膝关节置换术时的特点和围手术期处理方法。方法:2005年至2007年期间,我科进行伴有严重骨质疏松症的人工全膝关节置换术患者共计36例41膝,其中单膝关节置换31例,双膝置换5例。所有病例入院前均采用双光能X线骨密度仪(DEXA)检测骨密度。所有病例均采用后稳定型膝关节假体。患者在术前给予钙尔奇D片及鲑鱼降钙素等抗骨质疏松治疗1月,术后继续给予抗骨质疏松治疗2~3月。结果:术后切口均一期愈合,无并发症。所有病例随访时间36~60个月,平均49个月。膝关节功能评分(HSS评分法)术前平均为37.2±6.4分,术后平均评分94.6±8.8分,随访期间未见假体早期松动影像学改变,骨密度复查较术前有明显改善。结论:对于伴有严重骨质疏松的病例,骨质已受到不同程度的破坏,进行假体选择的个体化,手术操作的精细化以及术后康复治疗具体化等对临床疗效至关重要。  相似文献

Patients with a total of 112 chronic knee effusions unresponsive to the usual conservative methods of therapy were treated with intra-articular injections of radioactive gold and followed from 6 months to 5 years. Most patients in the study had rheumatoid arthritis but others had ankylosing spondylitis, psoriatic arthritis, intermittent hydrathrosis and undiagnosed synovitis. After 6 months 81% showed improvement. This figure diminished to approximately 70% at 1 and 2 years, 60% at 3 and 4 years and 50% at 5 years. The mose beneficial results were seen in patients with intermittent hydrarthrosis. Twelve patients with a synovial cyst and/or rupture improved. Patients with thin synovia and anatomic stage I or II disease did best, but those with stage III disease also did well. After the injection 33% of the patients had a reactive synovitis. A variable amount of radioactive material escaped from the knee to the regional lymph nodes and general circulation.  相似文献

The knee kinematics and kinetics of 139 patients (47 males and 92 females) with Grade II knee osteoarthritis (OA) were measured during level walking, stair ascent and stair descent. There was no significant difference in knee motion between the patients and normal subjects. The patients with knee OA had a significantly reduced internal knee extensor moment compared to normal subjects. This difference reflects the patient's compensation to reduce the knee joint loading. Further, subjects with OA and a higher body mass index have a lower knee extensor moment. The female subjects had significantly greater knee flexion and a greater knee extensor moment. This gender difference may partially explain the increased prevalence of OA in females. Most tests of OA treatments are assessed by criteria that do not reflect functional activities. This study demonstrates that objective gait analysis can be used to document gait adaptations used by patients with knee OA.  相似文献

目的:研究膝关节镜下有限清理与广泛清理术治疗膝关节骨关节炎的临床疗效。方法:选择2012年2月至2013年2月我院收治的80例膝关节骨关节炎患者,按随机数字表法平均分为研究组及对照组各40例,研究组行膝关节镜下有限清理术,对照组行广泛清理术;比较两组患者治疗优良率、手术前及术后1年膝关节功能评分及手术时间、住院时间等。结果:研究组患者治疗优良率为80.00%(32/40)高于对照组的75.00%(30/40),但差异无统计学意义(P0.05);研究组手术时间及住院时间分别为(30.4±14.8)h及(8.9±4.3)d,明显低于对照组的(60.6±16.9)h及(15.6±6.8)d,比较差异具有统计学意义(P0.05);两组术后1年膝关节功能评分较治疗前均明显改善,研究组术后Lysholm评分为(73.2±12.3)分,与对照组的(73.7±11.9)分比较差异无统计学意义(P0.05);两组患者均未出现术后感染等并发症,对照组2例出现术后下肢静脉血栓,研究组无严重并发症发生,两组比较差异无统计学意义(P0.05)。结论:膝关节镜下有限清理术治疗膝关节骨关节炎与广泛清理术疗效相当,但可明显缩短手术时间及住院时间,患者恢复快,值得推广应用。  相似文献

Background

Clinical guidelines recommend knee muscle strengthening exercises to improve physical function. However, the amount of knee muscle strength increase needed for clinically relevant improvements in physical function is unclear. Understanding how much increase in knee muscle strength is associated with improved physical function could assist clinicians in providing appropriate strength gain targets for their patients in order to optimise outcomes from exercise. The aim of this study was to investigate whether an increase in knee muscle strength is associated with improved self-reported physical function following exercise; and whether the relationship differs according to physical function status at baseline.

Methods

Data from 100 participants with medial knee osteoarthritis enrolled in a 12-week randomised controlled trial comparing neuromuscular exercise to quadriceps strengthening exercise were pooled. Participants were categorised as having mild, moderate or severe physical dysfunction at baseline using the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC). Associations between 12-week changes in physical function (dependent variable) and peak isometric knee extensor and flexor strength (independent variables) were evaluated with and without accounting for baseline physical function status and covariates using linear regression models.

Results

In covariate-adjusted models without accounting for baseline physical function, every 1-unit (Nm/kg) increase in knee extensor strength was associated with physical function improvement of 17 WOMAC units (95% confidence interval (CI) ?29 to ?5). When accounting for baseline severity of physical function, every 1-unit increase in knee extensor strength was associated with physical function improvement of 24 WOMAC units (95% CI ?42 to ?7) in participants with severe physical dysfunction. There were no associations between change in strength and change in physical function in participants with mild or moderate physical dysfunction at baseline. The association between change in knee flexor strength and change in physical function was not significant, irrespective of baseline function status.

Conclusions

In patients with severe physical dysfunction, an increase in knee extensor strength and improved physical function were associated.

PurposeThis study was designed to evaluate the effects of botulinum toxin type-A (BoNTA) injection of the rectus femoris (RF) muscle on the electromyographic activity of the knee flexor and extensor and on knee and hip kinematics during gait in patients with hemiparesis exhibiting a stiff-knee gait.MethodTwo gait analyses were performed on fourteen patients: before and four weeks after BoNTA injection. Spatiotemporal, kinematic and electromyographic parameters were quantified for the paretic limb.ResultsBoNTA treatment improved gait velocity, stride length and cadence with an increase of knee angular velocity at toe-off and maximal knee flexion in the swing phase. Amplitude and activation time of the RF and co-activation duration between the RF and biceps femoris were significantly decreased. The instantaneous mean frequency of RF was predominantly lower in the pre-swing phase.ConclusionsThe results clearly show that BoNTA modified the EMG amplitude and frequency of the injected muscle (RF) but not of the synergist and antagonist muscles. The reduction in RF activation frequency could be related to increased activity of slow fibers. The frequency analysis of EMG signals during gait appears to be a relevant method for the evaluation of the effects of BoNTA in the injected muscle.  相似文献

In this study, we investigated the effect of walker type on gait pattern characteristics comparing normal gait (NG), gait with a regular walker (RW), and gait with a newly developed walker with vertical moveable handlebars, the Crosswalker (CW).Partial weight bearing (PWB) of the feet, peak joint angles and largest Lyapunov exponent (λmax) of the lower extremities (hip, knee, ankle) in the sagittal plane, and gait parameters (gait velocity, stride length, cadence, stride duration) were determined for 18 healthy young adults performing 10 walking trials for each walking condition. Assistive gait with the CW improved local dynamic stability in the lower extremities (hip, knee, ankle) compared with RW and was not significantly different from NG. However, peak joint angles and stride characteristics in CW were different from NG. The PWB on the feet was lower with the RW (70.3%) compared to NG (82.8%) and CW (80.9%). This improved stability may be beneficial for the elderly and patients with impaired gait. However, increased PWB is not beneficial for patients during the early stages of rehabilitation.  相似文献

Peripheral sensory feedback is believed to contribute significantly to maintaining walking stability. Patients with diabetic peripheral neuropathy have a greatly increased risk of falling. Previously, we demonstrated that slower walking speeds in neuropathic patients lead to improved local dynamic stability. However, all subjects exhibited significant local instability during walking, even though no subject fell or stumbled during testing. The present study was conducted to determine if and how significant changes in peripheral sensation and walking speed affect orbital stability during walking. Trunk and lower extremity kinematics were examined from two prior experiments that compared patients with significant neuropathy to healthy controls and walking at multiple different speeds in young healthy subjects. Maximum Floquet multipliers were computed for each time series to quantify the orbital stability of these movements. All subjects exhibited orbitally stable walking kinematics, even though these same kinematics were previously shown to be locally unstable. Differences in orbital stability between neuropathic and control subjects were small and, with the exception of knee joint movements (p=0.001), not statistically significant (0.380p0.946). Differences in knee orbital stability were not mediated by differences in walking speed. This was supported by our finding that although orbital stability improved slightly with slower walking speeds, the correlations between walking speed and orbital stability were generally weak (r(2)16.7%). Thus, neuropathic patients do not gain improved orbital stability as a result of slowing down and do not experience any loss of orbital stability because of their sensory deficits.  相似文献

For a successful completion of a movement task the motor control system has to observe a multitude of internal constraints that govern the coordination of its segments. The purpose of this study was to apply principal component (PC) analysis to detect differences in the segmental coordination between healthy subjects and patients with medial knee osteoarthritis (OA). It was hypothesized that (1) systematic differences in patterns of whole body movement would be identifiable with this method even in small sample sized groups and that (2) these differences will include compensatory movements in the OA patients in both the lower and upper body segments. Marker positions and ground reaction forces of three gait trials of 5 healthy and 5 OA participants with full body marker sets were analyzed using a principal component analysis. Group differences in the PC-scores were determined for the first 10 PC-vectors and a linear combination of those PC-vectors where differences were found defined a discriminant vector. Projecting the original trials onto this discriminant vector yielded significant group differences (t(d=8)=3.011; p=0.017) with greater upper body movement in patients with knee OA that was correlated with the medial–lateral ground reaction force. These results help to characterize the adaptation of whole-body gait patterns to knee OA in a relatively small population and may provide an improved basis for the development of interventions to modify knee load. The PC-based motion analysis offered a highly sensitive approach to identify characteristic whole body patterns of movement associated with pathological gait.  相似文献

In a study comparing two regimens of treatment after intra-articular irradiation of the knee with yttrium-90 one group of patients was allocated to bed rest for 48 hours in hospital and the other to mobilisation at home. Initially a Robert-Jones orthopaedic bandage was applied to the knee in all patients, serving as a semi-rigid splint, but as loss of isotope from the knee was appreciable in the mobilised patients, subsequent patients were sent home with the knee in a plaster-of-Paris cylinder. No difference in extra-articular spread or chromosomal damage was found between the patients sent home with their knee in a rigid splint and those treated by bed rest. Clinical outcome at three months was satisfactory in all three groups. These results show that rigid splinting is essential in reducing extra-articular spread of the isotope but that bed rest is not necessary. Increases in intra-articular pressure associated with quadriceps muscle activity combined with flexion of the knee may be the most important factor affecting extra-articular spread of isotope.  相似文献

Background

Knee osteoarthritis (OA) is the most common joint disease of adults worldwide. Since the treatments for advanced radiographic knee OA are limited, clinicians face a significant challenge of identifying patients who are at high risk of OA in a timely and appropriate way. Therefore, we developed a simple self-assessment scoring system and an improved artificial neural network (ANN) model for knee OA.

Methods

The Fifth Korea National Health and Nutrition Examination Surveys (KNHANES V-1) data were used to develop a scoring system and ANN for radiographic knee OA. A logistic regression analysis was used to determine the predictors of the scoring system. The ANN was constructed using 1777 participants and validated internally on 888 participants in the KNHANES V-1. The predictors of the scoring system were selected as the inputs of the ANN. External validation was performed using 4731 participants in the Osteoarthritis Initiative (OAI). Area under the curve (AUC) of the receiver operating characteristic was calculated to compare the prediction models.

Results

The scoring system and ANN were built using the independent predictors including sex, age, body mass index, educational status, hypertension, moderate physical activity, and knee pain. In the internal validation, both scoring system and ANN predicted radiographic knee OA (AUC 0.73 versus 0.81, p<0.001) and symptomatic knee OA (AUC 0.88 versus 0.94, p<0.001) with good discriminative ability. In the external validation, both scoring system and ANN showed lower discriminative ability in predicting radiographic knee OA (AUC 0.62 versus 0.67, p<0.001) and symptomatic knee OA (AUC 0.70 versus 0.76, p<0.001).

Conclusions

The self-assessment scoring system may be useful for identifying the adults at high risk for knee OA. The performance of the scoring system is improved significantly by the ANN. We provided an ANN calculator to simply predict the knee OA risk.  相似文献

Objective: The objective was to determine whether knee osteoarthritis (OA) reduces exercise ambulatory capacity and impairs quality of life (QOL) in obese individuals. Research Methods and Procedures: There were 56 subjects, with and without knee OA, who were obese. The subjects were evaluated with anthropometric measurements, a body composition assessment, maximal cardiopulmonary exercise test, 6‐minute walk test (6‐MWT), perceived exertion (RPE), self‐reported disability [Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC)], and the Medical Outcomes Study Short Form 36 (SF‐36). Results: VO2peak was significantly higher in the controls when compared with the patients (mean ± standard deviation, 1.584 ± 0.23 L/kg per min vs. 0.986 ± 0.20 L/kg per min; p < 0.001). Obese subjects without knee OA walked a significantly longer distance in the 6‐MWT than obese patients with knee OA (p < 0.001). We also observed significant negative correlation between Vo 2max and RPE, WOMAC pain and physical limitation, and bodily pain and general health domains of short‐form 36. Discussion: Knee OA reduces exercise and ambulatory capacity and impairs QOL in obese individuals. RPE, WOMAC pain, and SF‐36 items might provide information about exercise capacity in the obese subjects with knee OA. Our study confirms that exercise capacity and QOL might be improved by energetic and intensive treatment of pain resulting from knee OA.  相似文献

The diminished knee flexion associated with stiff-knee gait, a movement abnormality commonly observed in persons with cerebral palsy, is thought to be caused by an over-active rectus femoris muscle producing an excessive knee extension moment during the swing phase of gait. As a result, treatment for stiff-knee gait is aimed at altering swing-phase muscle function. Unfortunately, this treatment strategy does not consistently result in improved knee flexion. We believe this is because multiple factors contribute to stiff-knee gait. Specifically, we hypothesize that many individuals with stiff-knee gait exhibit diminished knee flexion not because they have an excessive knee extension moment during swing, but because they walk with insufficient knee flexion velocity at toe-off. We measured the knee flexion velocity at toe-off and computed the average knee extension moment from toe-off to peak flexion in 17 subjects (18 limbs) with stiff-knee gait and 15 subjects (15 limbs) without movement abnormalities. We used forward dynamic simulation to determine how adjusting each stiff-knee subject's knee flexion velocity at toe-off to normal levels would affect knee flexion during swing. We found that only one of the 18 stiff-knee limbs exhibited an average knee extension moment from toe-off to peak flexion that was larger than normal. However, 15 of the 18 limbs exhibited a knee flexion velocity at toe-off that was below normal. Simulating an increase in the knee flexion velocity at toe-off to normal levels resulted in a normal or greater than normal range of knee flexion for each of these limbs. These results suggest that the diminished knee flexion of many persons with stiff-knee gait may be caused by abnormally low knee flexion velocity at toe-off as opposed to excessive knee extension moments during swing.  相似文献

Background

Foot center of pressure (COP) manipulation has been associated with improved gait patterns. The purpose of this study was to determine lower limb muscle activation changes in knee osteoarthritis patients, both immediately after COP manipulation and when COP manipulation was combined with continuous gait therapy (AposTherapy).

Methods

Fourteen females with medial compartment knee osteoarthritis underwent EMG analyzes of key muscles of the leg. In the initial stage, trials were carried out at four COP positions. Following this, gait therapy was initiated for 3 months. The barefoot EMG was compared before and after therapy.

Results

The average EMG varied significantly with COP in at least one phase of stance in all examined muscles of the less symptomatic leg and in three muscles of the more symptomatic leg. After training, a significant increase in average EMG was observed in most muscles. Most muscles of the less symptomatic leg showed significantly increased peak EMG. Activity duration was shorter for all muscles of the less symptomatic leg (significant in the lateral gastrocnemius) and three muscles of the more symptomatic leg (significant in the biceps femoris). These results were associated with reduced pain, increased function and improved spatiotemporal parameters.

Conclusions

COP manipulation influences the muscle activation patterns of the leg in patients with knee osteoarthritis. When combined with a therapy program, muscle activity increases and activity duration decreases.  相似文献

摘要 目的:探讨经皮穴位电刺激联合下肢康复机器人在膝关节前交叉韧带损伤重建术后康复中的应用效果。方法:选取我院2019年1月到2022年12月收治的120例采取膝关节前交叉韧带损伤重建术的患者作为研究对象,分为观察组与对照组,每组60例。对照组采取常规术后康复治疗,观察组采取经皮穴位电刺激联合下肢康复机器人康复治疗,对比两组患者康复治疗效果,位置觉和运动觉,疼痛情况与肿胀情况,膝关节功能以及生活质量。结果:观察组治疗总有效率高于对照组(P<0.05);治疗前两组患者患肢肿胀值、视觉模拟量表(VAS)评分对比无差异(P>0.05),治疗后均降低,且观察组较对照组低(P<0.05);两组患者的治疗前膝关节75度、45度、15度位置觉,伸膝、屈膝运动觉对比无差异(P>0.05),治疗后两组患者膝关节75度、45度、15度位置觉,伸膝、屈膝运动觉均下降,且观察组较对照组低(P<0.05);治疗前两组患者膝关节主观(IKDC)评分、膝关节功能(Lysholm)评分对比无差异(P>0.05),治疗后均降低,且观察组较对照组低(P<0.05);两组患者治疗前生活质量相关评分对比无差异(P<0.05),治疗后均升高,且观察组较对照组高(P<0.05)。结论:经皮穴位电刺激联合下肢康复机器人在膝关节前交叉韧带损伤重建术后康复中应用效果显著,能够改善患者膝关节位置觉和运动觉,减轻远期疼痛情况与肿胀情况,提升膝关节功能,患者生活质量较好。  相似文献
