目的:探讨观察儿童朗格汉斯细胞组织增生症临床治疗方案和效果。方法:选取我院2007年4月-2014年11月收治的儿童朗格汉斯细胞组织细胞增生症65例,按随机数字表法分为观察组(32例)和对照组(33例)。对照组给予朗格汉斯细胞组织细胞增生症-Ⅲ(LCH-Ⅲ)治疗方案,观察组给予难治性2008方案。观察两组患者临床疗效、复发率、并发症、生存率。结果:观察组完全缓解率显著高于对照组(P0.05),而两组患者的复发率和总有效率之间的差异无统计学意义(P0.05);两组治疗后9个月、12个月、24个月生存率差异无统计学意义(P0.05);观察组不良反应发生率为9.38%,对照组为24.24%,观察组稍低于对照组,但两组差异无统计学意义(P0.05)。结论:采用难治性2008方案治疗儿童朗格汉斯细胞组织细胞增生症较LCH-Ⅲ方案疗效更佳,且远期生存率明显改善,还可减少不良反应,值得在临床治疗中推广应用。  相似文献

目的:探讨朗格罕细胞组织细胞增生症累及垂体的MR表现及相关临床表现。方法:搜集了6例确诊为朗格罕细胞组织细胞增生症并垂体表现异常的患儿,男5例,女1例,年龄2~11岁,平均(6±3)岁,对其影像及临床表现进行回顾性分析。结果:临床患儿主要以头面部包块,多饮、多尿等就诊。头颅MR平扫(T1WI)表现6例患儿神经垂体高信号全部消失,垂体柄增粗5例,垂体柄著征1例,垂体饱满1例,其中3例治疗后复查垂体及垂体柄均有变化。结论:神经垂体高信号消失,垂体柄增粗或著征为朗格罕细胞组织细胞增生症累及垂体的头颅MR表现。累及垂体者临床几乎都有多饮、多尿表现。目前,MRI是诊断朗格罕细胞组织细胞增生症累及垂体的唯一可靠的影像学检查方法,并对治疗后病情随访有重要作用。  相似文献

Langerhans cell histiocytosis (LCH) is a rare disease of the reticuloendothelial system which consists in various clinical manifestations from a single lytic bone lesion to multisystemic lesions with organ dysfunction. One single bone lesion, the most frequent presentation, has a good prognosis. We relate here the case of a 4-year-old child who had a pain of his right hip with lameness and fever. The bone scintigraphy (BS) highlighted an increased uptake of the right iliac wing. Biopsy of the lesion led to the diagnosis of LCH. The progression was spontaneously favourable with relief of pain after the biopsy. This case report allows us to highlight the most recent guidelines regarding the classification of the disease and the indication of imaging. Staging is currently based on chest radiograph, abdominal ultrasound and skeletal radiograph survey. The role of BS, MRI and FDG PET/CT for staging remains to be clarified.  相似文献

Langerhans cell histiocytosis is a rare disorder characterized by abnormal proliferation of Langerhans cells that can affect various organ systems. The disease usually presents as a unifocal lytic bone lesion and can affect any age group. Less frequently it presents as a disseminated disease with multisystem involvement. Hepatic manifestation in Langerhans cell histiocytosis is relatively rare and usually presents as a part of a disseminated process. We report a case of Langerhans cell histiocytosis involving only the liver in a 9-years-old child.  相似文献

摘要 目的:探讨无创性皮肤屏障功能检测在朗格汉斯细胞组织细胞增生症(Langerhans cell histiocytosis,LCH)中的应用价值。方法:研究时间为2017年1月到2020年12月,选择在本院诊治的朗格汉斯细胞组织细胞增多症患者72例作为LCH组,同期选择健康体检者72例作为对照组。采用无创性皮肤屏障功能检测皮肤水分、经皮水分丢失(Transdermal water loss,TEWL)、油脂水平,同时检测所有入选者的免疫功能、皮肤菌群并进行相关性分析。结果:LCH组的皮肤水分低于对照组(P<0.05),TEWL、油脂水平高于对照组(P<0.05)。LCH组的乳酸杆菌(La)阳性率低于对照组(P<0.05),表皮葡萄球菌(Se)、痤疮丙酸杆菌(Pa)、金黄色葡萄球菌(Sa)阳性率高于对照组(P<0.05)。LCH组的CD163、ki-67表达阳性率分别为77.8 %、52.8 %,高于对照组的19.4 %和6.9 %(P<0.05)。在LCH组中,Pearson相关性分析显示皮肤水分与乳酸杆菌呈现正相关性(P<0.05),TEWL、油脂与表皮葡萄球菌、痤疮丙酸杆菌、金黄色葡萄球菌、CD163、ki-67呈现正相关性(P<0.05)。结论:无创性皮肤屏障功能检测在朗格汉斯细胞组织细胞增生症中的应用可反映患者的皮肤水分与油脂状况,也可间接反映患者的皮肤微生态与免疫功能状况。  相似文献

Osteopontin (OPN) acts as an osteoclast activator, a proinflammatory cytokine, and a chemokine attracting histiocytes/monocytes and is abundantly expressed in Langerhans cell histiocytosis (LCH). We investigated whether serum OPN levels are related to disease types in LCH. Fifty-eight newly diagnosed LCH patients were studied; eight with risk organ (liver, spleen and/or hematopoietic) involvements positive multisystem (MS+) disease, 27 with risk organ involvement negative multisystem (MS−) disease and 23 with single system (SS) disease. Pediatric patients with non-inflammatory disease (n = 27) were used as controls. All of patients with MS+ disease were younger than 3 years. Serum OPN levels and 44 kinds of humoral factors were measured by ELISA and Bio-Plex suspension array system, respectively. In the patients younger than 3 years, the median serum OPN level (interquartile range) was 240.3 ng/ml (137.6–456.0) in MS+ (n = 8); 92.7 ng/ml (62.0–213.8) in MS− (n = 14) and 72.5 ng/ml (55.6–94.0) in SS (n = 9) and 74.4 ng/ml (42.2–100.0) in control (n = 12). The OPN values were significantly higher in the MS+ group than the MS−, SS and control groups (p = 0.044, p = 0.001 and p = 0.002, respectively), but not different between the MS−, SS and control groups. In the patients older than 3 years, the median level of serum OPN (IQR) was 56.2 ng/ml (22.9–77.5) in MS− (n = 13), 58.9 ng/ml (31.0–78.7) in SS (n = 14) and 41.9 (28.9–54.1) in control (n = 15). These values did not differ significantly between each group. The serum OPN levels were positively correlated with the serum IL-6, CCL2, IL-18, IL-8 and IL-2 receptor concentration. OPN may be involved in risk organ dissemination and poor prognosis of LCH through the function as inflammatory cytokine/chemokine.  相似文献

Airborne trace elements are implicated in the etio-pathogenesis of a large number of pulmonary diseases. The aim of this study was to evaluate the reliability and effectiveness of direct determination of Cd, Cr, Cu, Fe, Mn, Ni, Pb, V, and Zn concentrations in bronchoalveolar lavage (BAL) samples from patients with sarcoidosis, idiopathic pulmonary fibrosis, and Langerhans cell histiocytosis and healthy (smoking and non-smoking) controls. A total of 44 individuals were recruited among sarcoidosis, idiopathic pulmonary fibrosis, and Langerhans cell histiocytosis patients and healthy (smoking and non-smoking) controls. Average Mn concentrations in BAL from patients were 45% lower than in controls (p < 0.01) and remarkable decreases in average concentrations of Cr, Ni and Zn were also found in BAL from patients with idiopathic pulmonary fibrosis and Langerhans cell histiocytosis. As these diseases are characterized by the enhanced activation of certain immunomodulatory cells and by generation of free radicals, the depressed Mn, Zn, Cr and Ni concentrations in BAL from patients may be due to oxidative stress. These preliminary results indicate that assessment of the elemental composition of BAL is a promising approach to study the pathogenesis of diffuse lung diseases and Langerhans cell histiocytosis.  相似文献

Summary The islets of Langerhans of the equine pancreas were examined with the electron microscope after immersion or perfusion fixation. Five cell types could be distinguished after fixation by either technique: 1. A-cells, situated at the center of the islets, 2. B-cells, containing mostly pale granules and constituting the principal cell type of the periphery of the islets, 3. D-cells, also located mainly at the periphery of the islets, 4. G-cells, found at the edge of the islets and in the exocrine pancreas, and 5. S-cells, (small granule cells), which are relatively few in number and occur only in the islets. The function and age-dependent modifications of these cells are discussed. The formation of light and dark cells and of mixed cells are regarded as artifact, since cells of this type occur only under the condition of immersion fixation.Dedicated to Professor Dr. Drs. h.c. Objective Dental granulomas (DGs) and radicular cysts (RCs) are chronic periapical lesions frequently involving the jaws. Langerhans cells (LCs) are dendritic cells responsible for the presentation of antigens to T lymphocytes. This study examined the expression of LCs in DG and RCs by immunohistochemical staining. Study Design Eighteen cases of DGs and 26 cases of RCs were analyzed using anti-CD1a marker. Results CD1a-labeled LCs were observed in 11.1% of DGs and in 69.2% of RCs, showing a significant correlation (P < 0.0001; Fisher’s test). In DGs, LCs were only observed in granulation tissue, showing discrete immunostaining density. In RCs, LCs exhibited both a round and a dendritic shape in all epithelial layers. Although a correlation was observed between immunostaining density and epithelial thickness, as well as between immunostaining and inflammatory intensity, the differences were not significant in radicular cysts. Conclusion Langerhans cells provide important insight into the immunopathogenesis of chronic periapical lesions.  相似文献

Epidermal Langerhans cells (LC) are the first sentinels of the skin immune system. To study immigration of human LC precursor cells into the skin, we established a two-compartmental skin model consisting of a dermal matrix and an epidermal sheet of keratinocytes. We tested the individual components of the skin model for their influence on phenotype and function of LC precursors. A time window at day 5/6 of differentiation was determined, during which in vitro generated LC precursors expressed adhesion molecules and chemokine receptors required for transmigration across endothelial cell layers and the dermis towards the epidermis. They expressed L-selectin, integrins, platelet endothelial cell adhesion molecule-1, E-cadherin and CC-chemokine receptor 6 and were thus fitted out for transendothelial migration and immigration into the dermis. In a transwell system, these LC precursors migrated towards the chemokine MIP3alpha, demonstrating functional integrity of chemokine receptor 6. For the in vitro reconstituted skin, keratinocytes were grown on a de-epidermized dermis for one to three weeks and formed an epidermal sheet. We allowed LC precursor cells to migrate into this two-compartmental model from the dermal side and examined the presence of CD1alpha--positive cells. LC precursors migrated through the dermal matrix towards the layer of keratinocytes representing the epidermis and could be identified by immunohistology. Experiments designed to investigate the influence of signals provided by both the skin components and by the LC precursors on LC immigration into the skin are in progress.  相似文献

本实验用酶细胞化学和示踪细胞化学方法观察了睾丸间质细胞中多泡体的形成过程及其与溶酶体的关系。实验结果表明,睾丸间质细胞中多泡体的形成可分三个阶段:首先,一些含内吞物质的泡状结构进入高尔基体区域,与那里的小泡融合,形成内含少量小泡的前多泡体;然后,前多泡体互相融合,形成体积较大、基质电子密度低、内含小泡排列稀疏的低电子密度多泡体;最后,低电子密度多泡体通过表面长出微绒毛样结构并不断断裂的方式去除多余的界膜,形成体积较小、基质电子密度高、内含小泡排列紧密的高电子密度多泡体。因此,多泡体的形成既与内吞活动有关,又与高尔基体区域小泡有关。前多泡体和低电子密度多泡体不含溶酶体酶。在多泡体形成过程中,只有到高电子密度多泡体阶段,才与溶酶体发生关系,从溶酶体中获取溶酶体酶。多泡体形成后,常与自体吞噬泡靠近,可能参与睾丸间质细胞的自体吞噬活动。  相似文献

Cytopathology of Follicular Tumours of the Thyroid With Clear Cell Change   总被引:1,自引:0,他引:1  
A retrospective cytological study of nine follicular tumours of the thyroid with clear cell change was undertaken. In five clear cell adenomas and one moderately differentiated clear cell follicular carcinoma the epithelial cells occurred singly or in sheets and clusters; they sometimes assumed a trabecular or follicular pattern. The cells usually had pale diffusely vacuolated cytoplasm with ill-defined boundaries, a variable degree of anisonucleosis, nucleolar enlargement, and nuclear overlapping. Smears from a signet-ring cell adenoma contained in addition a few cells with large cytoplasmic vacuoles and compressed eccentric nuclei. In these cases a cytological diagnosis of 'follicular lesion' (or follicular neoplasia), clear cell type or signet-ring cell type, was given. A cytodiagnosis of 'carcinoma' was made only in the poorly differentiated follicular carcinoma-clear cell variant studied which showed unequivocal features of malignancy. Features suggestive of thyroid cyst, nodular goitre, Hashimoto's thyroiditis, and cell hyperactivity (marginal vacuoles, 'fire flare') were also found in the aspirated specimens of these cases of clear cell tumour of the thyroid.  相似文献

A simple method to enhance the staining of cell wall components for fluorescence microscopy is described. Pigment producing in vitro cells of Vaccinium pahalae (ohelo) were tested for their ability to survive cryopreservation and retain pigment-production capacity after encapsulation-dehydration. Preculture of cells for 6 to 8 days in a medium containing 1.0 M sucrose was essential before dehydration. Reduction of bead water content before quenching in liquid nitrogen was highly correlated (r = 0.94) with increased survival rate in cells after cryopreservation. Dehydration of beads for 4 h was satisfactory for survival of cells. After quenching in liquid nitrogen, colored cells became pale, but pigment content was recovered once cells resumed growth. After three subcultures, cells regained their maximum capacity for pigment accumulation. The percentage of colored-to-total cell volume was not influenced by cryopreservation. Encapsulation-dehydration and cryopreservation did not diminish the capacity of cells to produce anthocyanins and other flavonoids. Summary The development and maturation of Langerhans cells during the differentiation of skin was studied in mice from fetal day 13 to adult using 3 indices: (1) ATPase activity; (2) ultrastructure; and (3) quantitative evaluation of the cell population.ATPase-positive Langerhans cells appeared in the epidermis at first at fetal day 16, and they increased in number in the differentiating epidermis during the late fetal period. The earliest appearance of Birbeck granules was at postnatal day 4. Cored tubules were also formed in the Langerhans cells in the dermis at around the same age. The cells containing Birbeck granules or cored tubules are considered to be mature Langerhans cells. In the Langerhans-cell lineage, those cells in the epidermis at stages earlier than postnatal day 4 and not yet containing specific organelles are considered to be immature Langerhans cells. These immature Langerhans cells can be identified ultrastructurally in the epidermis at fetal day 16, coinciding with the appearance of ATPase-positive cells. The increase in the number of immature Langerhans cells during the perinatal period was shown by quantitative analysis of nuclear density and relative Langerhans-cell area on the electron micrographs.It is concluded that ATPase is a marker of the Langerhans-cell lineage from the early development stages, while Birbeck granules and cored tubules are markers that identify mature Langerhans cells in electron micrographs.  相似文献

在寻找新型抗真菌抗生素的过程中,从海洋放线菌FIM03-1149发酵液中分离到抗真菌抗生素新抑锈病素(Neorustmicin)。菌株FIM03-1149在多数培养基上生长良好,淡黄-橙黄色,无气生菌丝,不产生可溶性色素。形态特征、化学分类特征和生理生化特性等研究表明菌株FIM03-1149属于小单孢菌属。16SrRNA基因序列分析也表明菌株FIM03-1149属于小单孢菌属,但与最接近已知种Micromonospora siamensisTT2-4T处于不同的进化分支,2个菌株在某些生理生化特性上也有不同。基于上述数据,菌株FIM03-1149可能是小单孢菌属的1个新种。  相似文献

1. Protective coloration in insects may be aposematic or cryptic, and some species change defensive strategy between instars. In Sweden, the adult striated shieldbugs Graphosoma lineatum (Heteroptera: Pentatomidae) undergo a seasonal colour change from pale brown and black striation in the pre‐hibernating adults, to red and black striation in the same post‐hibernating individuals. To the human eye the pre‐hibernating adults appear cryptic against the withered late summer vegetation, whereas the red and black post‐hibernating adults appear aposematic. This suggests a possibility of a functional colour change. However, what is cryptic to the human eye is not necessarily cryptic to a potential predator. 2. Therefore we tested the effect of coloration in adult G. lineatum on their detectability for avian predators. Great tits (Parus major) were trained to eat sunflower seeds hidden inside the emptied exoskeletons of pale or red G. lineatum. Then the detection time for both colour forms was measured in a dry vegetation environment. 3. The birds required a longer time to find the pale form of G. lineatum than the red one. The pale form appears more cryptic on withered late summer vegetation than the red form, not only to the human eye but also to avian predators. The result supports the idea that the adult individuals of G. lineatum undergo a functional change from a cryptic protective coloration to an aposematic one.  相似文献

N-methyl-2-pyrrolidone (NMP) is known for its multi-solvent properties. However, its biological, especially immunological significance still remains to be elucidated. In this study, we show for the first time that NMP stimulates the skin immune system by activating epidermal Langerhans cells (LCs). In contrast with the placebo tape, when the NMP-containing adhesive tape was applied on murine skin, LCs were stimulated immediately. Activated LCs not only exhibited enhanced expression of major histocompatibility complex class II and morphological changes, including the loss of dendrites, but also migrated effectively to draining lymph nodes. In addition, application of the tyrosine-related protein-2 peptide, which is the cytotoxic T lymphocyte (CTL) epitope against B16 melanoma, in combination with the NMP tape, resulted in explosive expansion of specific CTLs in mouse spleens. Taken together, these results demonstrate a novel role of NMP as an adjuvant in percutaneous peptide immunization.  相似文献

朗格罕细胞(langerhanscel,LC)是一种骨髓来源的免疫活性细胞,具有呈递抗原,诱导免疫反应的功能。口腔粘膜的复层鳞状上皮中亦有LC分布,它们是口腔粘膜防御体系的组成部分。本研究运用ATP酶组织化学方法和电镜技术,对25例人的口腔粘膜标本中的LC的形态、结构及数量分布进行观察,结果提示,在口腔粘膜的上皮中,LC主要分布于上皮的基底部,并且趋于环绕结缔组织乳头排列,LC的数量也因部位不同而有显著差异  相似文献
