BackgroundIt is unclear whether weight change after middle adulthood influences the risk of thyroid cancer. The aim of this study was to investigate associations between the risk of papillary thyroid cancer (PTC) and body mass index (BMI) and weight change after middle adulthood (age 35).MethodsA matched case–control study based on three hospitals included 516 pairs of cases newly diagnosed with PTC and controls. Current height and weight after defecation in the morning were measured by trained nurses. During measurement, all subjects were requested to wear lightweight clothing and no shoes. Weight at age 35 was self-reported. BMI and weight change were modeled as continuous and categorical variables. Conditional and unconditional logistic regression models were used to estimate the odds ratio (OR) and 95% confidence interval (95%CI) for the association between BMI and weight change after middle adulthood and PTC.ResultsAfter adjustment for covariates, measured BMI at the time of current diagnosis was positively associated with PTC (OR 1.16, 95%CI 1.10–1.21). According to WHO BMI guidelines for Asia-Pacific populations, the OR (95%CI) for PTC risk in obesity was 2.99 (1.92–4.67) compared to normal weight (p-trend <0.001). Moreover, PTC was positively associated with BMI at age 35; the OR (95%CI) for PTC risk per unit increase in BMI was 1.06 (1.02–1.11). Compared to stable weight (changed <0.5 kg/year), weight gain ≥1.0 kg/year after middle adulthood was positively associated with PTC (OR 2.57, 95%CI 1.39–4.76, p-trend <0.001). Compared to maintaining non-overweight status, the PTC risk was significantly increased in those individuals who gained weight and became overweight after middle adulthood (OR 3.82, 95%CI 2.50–5.85).ConclusionThis study showed that high BMI and obesity were positively associated with increased risk of PTC, and weight gain after middle adulthood also could elevate the PTC risk.  相似文献

The relationship of resistin levels with obesity remains unclear. The aim of this study was to determine resistin levels in prepubertal children and adolescents and evaluate their association with anthropometric parameters and body composition. The study population included 420 randomly selected 6–8-year-old children and 712 children aged 12–16 years. Anthropometric data were measured and body mass index (BMI) and waist-to-hip and waist-to-height ratios were calculated. Body composition was assessed using an impedance body composition analyzer. Serum resistin levels were determined using a multiplexed bead immunoassay. Resistin levels were not significantly different between sexes. No significant differences in serum resistin concentrations were found between obese, overweight, and normal weight children at any age, and no significant correlations were observed between resistin concentrations and weight or BMI. However, resistin levels showed a significant positive correlation with fat mass in 12–16-year-old children, particularly in girls. In addition to describing serum resistin levels in prepubertal children and adolescents, our study suggests that resistin is related to body fat rather than to BMI in adolescents.  相似文献

We employ a unique dataset of Major League Baseball (MLB) players - a select, healthy population - to examine trends in height, weight, and body mass in birth cohorts from 1869 to 1983. Over that 115-year time period, U.S. born MLB players have gained, on average, approximately 3 in. (7.6 cm) in height and 27.0 lb (12.2 kg) in weight, which has contributed a 1.6-unit increase in the body mass index. Where comparable data are available, U.S. born MLB players are about 2.0 in. (5.1 cm) taller and 20.0 lb (9.1 kg) heavier but substantially less obese than males in the general U.S. population. But both groups exhibit similar height and weight trends; the majority of height and weight gains take place in cohorts that were born prior to World War II, followed by slower gains and occasional declines in height and weight for cohorts born in 1939 and later.  相似文献

Over the last two decades, the prevalence of obesity has risen worldwide. As obesity is a confirmed risk factor for a number of diseases, its increasing prevalence nurtures the supposition that obesity may present a growing and significant economic burden to society.The objective of this study is to analyse the correlation between body mass index (BMI) and future direct and indirect costs, as well as the correlation between changing BMI and future in(direct) costs. Health care utilisation and productivity losses were based on data from 2581 participants aged 25-65 years (1994/95) from two cross-sectional, population-representative health surveys (MONICA/KORA-survey-S3 1994/95 and follow-up KORA-survey-F3 2004/05) in Augsburg, Germany.The predicted average adjusted total direct costs per year and per user were estimated to be €1029-(healthy weight), €1093-(overweight) and €1040-(obesity). There are significantly greater future costs in the utilisation of general practitioners per user and per year at higher obesity levels (€72; €75; €96).The average total direct costs per person for those who stay in the same BMI class are €982, €1000 and €973. An overweight participant who becomes obese incurs significant costs of internists of €160 compared with those who remain overweight (€124). An overweight user incurs indirect costs of €2474, compared with €2136 for those who remain a healthy weight.There is a trend for higher predicted (in)direct costs when people are overweight or obese compared with healthy weight persons 10 years earlier. Potential cost savings could be attained if preventive programs effectively targeted these individuals.  相似文献

BackgroundWhile the breast cancer risk associated with increasing adult BMI in postmenopausal women can be explained by increases in concentrations of endogenous estrogens the biologic mechanisms behind the inverse association between adolescent BMI and breast cancer risk are still a subject of controversial debate.MethodsWe investigated the association of breast cancer with body size and changes in body size across life time estimated by age-specific BMI Z scores and changes in BMI Z scores from teenage years to middle age in an age-matched population-based case-control study of 2994 Australian women. Logistic regression adjusted for the matching factor age and further potential confounders was used.ResultsAdolescent body leanness in postmenopausal women and excess adult weight gain in all study participants were associated with an increased breast cancer risk with an odds ratio [95% confidence interval] of 1.29 [1.08,1.54] and 1.31 [1.09,1.59], respectively. Interaction analyses restricted to postmenopausal women revealed an increased risk of breast cancer in those who were lean during adolescence and gained excess weight during adulthood (odds ratio [95% confidence interval]: 1.52 [1.19,1.95]) but not in women who were lean during adolescence and did not gain excess weight during adulthood (1.20 [0.97,1.48]) and not in women who were not lean during adolescence and but gained excess weight during adulthood (1.10 [0.95,1.27]) compared to postmenopausal women who were neither lean during adolescence nor gained excess weight.ConclusionIn postmenopausal women adolescent leanness was only associated with increased breast cancer risk when excess weight was gained during adulthood.  相似文献

Recent studies have shown that SNPs in the FTO gene predispose to childhood and adult obesity. In this study, we examined the association between variants in FTO and KIAA1005, a gene that maps closely to FTO, and obesity, as well as obesity related traits among 450 well characterised severely obese children and 512 normal weight controls. FTO showed significant association with several obesity related traits while SNPs in KIAA1005 did not. When stratified by gender, the FTO variant rs9939609 showed association with obesity and BMI among girls (P = 0.006 and 0.004, respectively) but not among boys. Gender differences were also found in the associations of the FTO rs9939609 with obesity related traits such as insulin sensitivity and plasma glucose. This study suggests that FTO may have an important role for gender specific development of severe obesity and insulin resistance in children.  相似文献

This study uses logistic regressions to consider the effect of social factors, physical activity, meal frequency and BMI of parents on children's body mass index (BMI) values. Three thousand three hundred and sixty-two children aged 7-11 years were examined in 38 schools in the Czech Republic. Six anthropometric characteristics were assessed in 1999-2000. Most important factors influencing children's BMI values were: obesity of both or at least one of the parents, being an only child, residence in small communities, and irregular breakfasts.  相似文献

The proportion of obese population has been gradually increasing in the US over the past few decades. In this study I investigate how education is associated with Body Mass Index (BMI) in later stages of life. BMI, weight(kg)/height(m)2, is the principle measure used for classifying people as obese. Using sibling data and methods that take account of unobserved endowments and environment shared by siblings, I find that there is large variation in BMI between siblings and that education is negatively associated with BMI. One more year of schooling is associated with an estimated reduction of 0.15 in BMI. When considering different education levels, completing college education is associated with 0.7 reduction in BMI relative to high school graduation only. The significant effect of education on obesity that remains in the long-run has policy implications.  相似文献

Body segment parameters (BSPs) such as segment mass, center of mass, and radius of gyration are required in many ergonomic tools and biomechanical models to estimate injury risk, and quantify muscle and joint contact forces. Currently, the full effects of age and obesity have not been taken into account when predicting BSPs. The goal of this study is to quantify the impact of body mass index (BMI) and age on BSPs, in order to provide more representative measures necessary for modeling inputs. A whole body dual energy X-ray absorptiometry (DXA) scan was collected for 280 working men and women with a wide range of BMI and aged 21 to 70 years. Established DXA processing methods were used to determine in-vivo estimates of the mass, center of mass, and radius of gyration for the upper arm, forearm, torso, thigh, and shank for males and females. Regression models were used to determine if age and BMI terms, as well as their interactions, were associated with these BSPs. The variability in BSPs explained by BMI alone ranged from 4 to 51%, and age explained an additional 3–19%. Thus, BMI and age are significant correlates of BSPs, and need to be taken into account when predicting certain BSPs in order to obtain accurate and representative results in biomechanical models.  相似文献

Body segment parameters such as segment mass, center of mass, and radius of gyration are used as inputs in static and dynamic ergonomic and biomechanical models used to predict joint and muscle forces, and to assess risks of musculoskeletal injury. Previous work has predicted body segment parameters (BSPs) in the general population using age and obesity levels as statistical predictors (Merrill et al., 2017). Estimated errors in the prediction of BSPs can be as large as 40%, depending on age, and the prediction method employed (Durkin and Dowling, 2003). Thus, more accurate and representative segment parameter inputs are required for attempting to predict modeling outputs such as joint contact forces, muscle forces, and injury risk in individuals. This study aims to provide statistical models for predicting torso, thigh, shank, upper arm, and forearm segment parameters in working adults using whole body dual energy x-ray absorptiometry (DXA) scan data along with a set of anthropometric measurements. The statistical models were developed on a training data set, and independently validated on a separate test data set. The predicted BSPs in validation data were, on average, within 5% of the actual in vivo DXA-based BSPs, while previously developed predictions (de Leva, 1996) had average errors of up to 60%, indicating that the new models greatly increase the accuracy in predicting segment parameters. These final developed models can be used for calculating representative BSPs in individuals for use in modeling applications dependent on these parameters.  相似文献

The authors estimated the benchmark durations (BMDs) and their 95% lower confidence limit (BMDL) for the reference duration of shiftwork for weight gain. A 14-yr prospective cohort study was conducted in male workers at a Japanese steel company (n?=?7254) who had received annual health check-ups between 1991 and 2005. The endpoints in the study were either a 5%, 7.5%, or 10% increase in body mass index (BMI) during the period of observation, compared to the BMI at entry. The association between the duration of shiftwork and weight gain was investigated using multivariate pooled logistic regression analyses with stepwise selection of covariates, including age, BMI measured during the study, drinking and smoking habits, and habitual exercise. The BMDL/BMD for shiftwork in subjects aged in their 40s or ≥50 yrs was estimated using benchmark responses (BMRs) of 5% or 10% and parameters for the duration of shiftwork and other covariates. For workers aged in their 40s, the BMDL/BMD for shiftwork with a BMR of 5% was 18.6/23.0 yrs (≥7.5%) and 16.9/19.4 yrs (≥10%). For workers aged ≥50 yrs, the BMDL/BMD with a BMR of 5% was 22.9/28.2 yrs (≥7.5%) and 20.6/23.6 yrs (≥10%). The reference duration of shiftwork that associated with weight gain was shown to be at least 17 yrs in middle-aged workers. Since laws of physics exists in nature, their possible relationship to terrestrial growth is introduced. By considering the human body as a dynamic system of variable mass (and volume), growing under a gravity field, it is shown how natural laws may influence the vertical growth of humans. This approach makes sense because the non-linear percentile curves of different aspects of human physical growth from childhood to adolescence can be described in relation to physics laws independently of gender and nationality. Analytical relations for the dependence of stature, measured mass (weight), growth velocity (and their mix as the body mass index) on age are deduced with a set of common statistical parameters which could relate environmental, genetics and metabolism and different aspects of physical growth on earth. A relationship to the monotone smoothing using functional data analysis to estimate growth curves and its derivatives is established. A preliminary discussion is also presented on horizontal growth in an essentially weightless environment (i.e., aquatic) with a connection to the Laird-Gompertz formula for growth.  相似文献

BackgroundInfertility is a public health disorder affecting 10% of the population worldwide. Research on the impact of body mass index (BMI) on male fertility is very limited as compared to the multiple studies evaluating the impact of overweight in women's fertility. Although 25%-30% of the cases of couples consulting for infertility are attributable to male factors, studies evaluating the association between semen parameters and BMI are controversial.ObjectiveTo assess the impact of BMI on semen parameters in a selected group of men with unexplained infertility.MethodA retrospective analysis of 168 patients during the 2008-2010 period. They all had at least one semen analysis and related studies to rule out known causes of infertility. Median age of patients was 35 years (22-55), and they were divided into three groups: normal weight (BMI: 20-24.9 kg/m2), overweight (BMI 25-29.9 kg/m2), and obese (BMI ≥ 30 kg/m2).ResultsThere were no significant differences in semen parameters evaluated between the three groups, and no significant correlation was found between the same parameters and BMI.ConclusionsThere was no significant association between BMI and conventional semen parameters, but we cannot exclude an impairment in other semen parameters that are not routinely assessed, which could result in a lower potential fertility in these individuals.  相似文献

Loss of control over hedonically motivated actions is a defining component of impulse control disorders, such as drug dependence and the proposed ‘food addiction’ model of obesity. Devolution from goal‐directed to compulsively maintained behaviors is partially attributed to abnormalities in the orbitofrontal cortex, an area critical in reward valuation. In the current study, overlapping reductions in orbitofrontal gray matter volume relating to body mass index were seen in healthy control and cocaine‐dependent individuals, as well as in relation to duration of cocaine abuse, providing support for a shared neuropathology between the two conditions potentially related to dysfunctional reward‐seeking behavior.  相似文献

目的:回顾性分析我院同一组医生施行腹腔镜下直肠癌根治术患者的临床资料,探讨体重指数对于腹腔镜直肠癌根治术的影响,评估腹腔镜下直肠癌根治术在肥胖患者中的应用。方法:选择2009年1月至2010年12月因直肠癌在我院行腹腔镜直肠癌根治术的患者199例为研究对象,根据患者不同体重指数分为3组,正常体重组:BMI〈24.0kg/m2143例、超重组:24.0〈=BMI〈28.0kg/m^2 50例、肥胖组:BMI〉=28.0kg/m^2 6例,进而比较3组间患者围术期的各项指标,包括手术时间,估计术中出血量,术后住院时间,取检淋巴结数,对正常体重组,超重组及肥胖组三组患者围术期的各项数据进行比较。结果:3组患者均顺利完成手术,无术中死亡病例。1例患者中转开腹,中转开腹率为0.5%。1例患者术后3天死亡。分别比较三组间的手术时间、术中出血量、取检淋巴结数、术后住院天数,在正常体重组、超重组、肥胖组,手术时间分别为183.5±49.1min,191.2±54.3min,170.0±26.8min;术中出血量分别为65.6±66.4mL,59.9±61.1mL,83.3±20.4mL;取检淋巴结数分别为10.5±6.8枚、9.0±4.3枚、11.5±10.0枚;术后住院天数分别为13.1±3.8天、13.1±3.6天、12.7±1.0天,差异均无统计学意义(P〉0.05)。结论:腹腔镜下直肠癌根治术是一种安全、有效的手术方式,可安全地用于肥胖患者,可体现出其微创优势。  相似文献

No research has been done on the body mass index values of the 19th-century Mexican population. This paper introduces a new data source of 19th-century Mexican male inmates in American prisons and finds that the majority of Mexican body mass was in the normal range (18.5-24.9). Few Mexicans were underweight or obese by modern standards. Body mass varied little (0-0.9 units) by occupations possibly because criminals probably came from the lower end of the socioeconomic distribution; however, it did vary with crimes for which inmates were incarcerated.  相似文献

BackgroundEvidence suggests that birth weight may be associated with colorectal cancer (CRC) risk later in life. Whether the association is mediated by adult body size remains unexamined.MethodCox proportional hazards models (Hazard Ratio (HR) and 95 % Confidence Intervals (CI)) were used to evaluate the association between self-reported birth weight (<6 lbs, 6-<8 lbs, ≥8 lbs) and CRC risk among 70,397 postmenopausal women from the Women’s Health Initiative. Further, we assessed whether this association was mediated by adult body size using multiple mediation analyses.ResultsCompared with birth weights of 6-< 8 lbs, birth weight ≥ 8 lbs was associated with higher CRC risk in postmenopausal women (HR = 1.31, 95 % CI 1.16–1.48). This association was significantly mediated by adult height (proportion mediated =11.4 %), weight (11.2 %), waist circumference (10.9 %), and body mass index at baseline (4.0 %). The joint effect of adult height and weight explained 21.6 % of this positive association.ConclusionOur data support the hypothesis that the intrauterine environment and fetal development may be related to the risk of developing CRC later in life. While adult body size partially explains this association, further investigation is required to identify other factors that mediate the link between birth weight and CRC.  相似文献
