OBJECTIVE: To discover cancer patients'' views about disclosure of information to their family, their family''s influence over the information given them, and their preferences for doctors'' behaviour if they and their family disagree, as a complement to applied ethical theory. DESIGN: Semistructured interviews followed by qualitative content analysis. SETTING: Mainly urban British general practice. SUBJECTS: 30 patients in whom cancer, excluding basal and squamous cell skin carcinomas, was diagnosed 1-7 years earlier. RESULTS: All subjects wished doctors to respect their views rather than those of their family, should they differ. With their consent, subjects favoured close family receiving information about their illness, all but one mentioning advantage to their family. Without such consent, six unconditionally favoured disclosure of information to their family while seven unconditionally opposed disclosure. Seventeen participants restricted their approval for such disclosure to specific circumstances. Only two participants approved of their family influencing the information that they received about their illness; all but seven wished to receive full information, the exceptions relating to information about bad prognosis. CONCLUSIONS: Subjects favoured openness with their family but most rejected unconditional disclosure of information without their consent and their family influencing what information they would be given. They valued respect for their autonomy more highly than beneficence and considered that their own needs took priority over those of their family.  相似文献

The application of bioinformatics methodologies to proteins inherently lacking 3D structure has brought increased attention to these macromolecules. Here topics concerning these proteins are discussed, including their prediction from amino acid sequence, their enrichment in eukaryotes compared to prokaryotes, their more rapid evolution compared to structured proteins, their organization into specific groups, their structural preferences, their half-lives in cells, their contributions to signaling diversity (via high contents of multiple-partner binding sites, post-translational modifications, and alternative splicing), their distinct functional repertoire compared to that of structured proteins, and their involvement in diseases.  相似文献

The characterization and ultrastructure of two new strains of Butyrivibrio   总被引:1,自引:0,他引:1  
Strains B-385-1 and 2-33 are numerically important components rumen bacterial populations , but they have remained (taxonomically) undefined. In spite of some resemblance to Selenomonas ruminantium in their cell size and in their formation of tufts of flagella, they more closely resemble Butyrivibrio fibrisolvens in the subpolar location of their flagella, in their guanine + cytosine content, and in most biochemical characteristics, including butyrate formation. Cells of these strains stain Gram negative, as do both Selenomonas and Butyrivibrio, but their cell walls closely resemble those of Butyrivibrio in their Gram-positive type of molecular architecture and in their cleavage pattern in freeze-etching. Cells of these strains and of B. fibrisolvens have a very thin (ca. 12 nm) peptidoglycan cell wall; thus, they fail to retain the crystal violet complex of the Gram stain and stain Gram negative. This important structural characteristic of their cell walls places strains B-385-1 and 2-33 within the genus Butyrivibrio and certain morphological and biochemical characteristics distinguish them from B. fibrisolvens.  相似文献

To investigate the regulation of the biosynthesis of ergot alkaloids by end products, the effect of exogenous agroclavine-1 and epoxyagroclavine-1 on their accumulation byP. sizovae was studied. The added alkaloids stimulated considerably their own biosynthesis depending on their concentration and time of introduction. The stimulating effect of both alkaloids and products of their degradation was suggested. Exogenous agroclavine-1 and epoxyagroclavine-1 changed the relation between the quantities of intra-and extracellular alkaloids, thus pointing to their possible influence on the transport processes.  相似文献

A ‘Monte Carlo’ method was developed for characterizing matrilines of rhesus monkey infants (Macaca mulatta) born to their mothers within six similar captive social groups, according to behavioural measures, and for characterizing social groups according to their matrilines' median scores. Eight- and 16-week-old infants differed according to their groups, but not according to their matrilines, in measures of their activity in the vicinity of their mothers, and the frequencies with which social companions other than the mother initiated social contacts with them while they were out of contact with their mothers. Sixteen-week-olds differed according to their groups in the frequencies with which their mothers rejected them, and 8-week-olds in the frequencies of the social contacts they initiated with companions other than mother while off their mothers. When tested with their mothers, in a mildly disturbing situation away from other social companions, 52-week-olds differed according to their group of origin in time spent out of contact with their mothers. Different groups were seen as producing infants differing in ‘enterprise’ from their eighth week, in the sense of being ready to initiate social contacts with others while off their mothers at 8 weeks, and of being off their mothers in the test at 52 weeks. Cases where the 8-week measures did not reflect infant enterprise could be explained in terms of other aspects of the relationships involving the infants and their mothers in the social group. In particular, matrilines differed consistently in dominance status, and some mothers also received high levels of aggression from other adults. Mothers receiving high levels of aggression were more responsible for maintaining proximity with their 8-week-old infants, and their 8-week-olds were less often involved in social contacts with others.  相似文献

The Color of My Skin is an instrument developed to assess children's internalized idea (abstraction) of the color of their skin; their satisfaction with that color; the desire, if any, to change the color of their skin; and their affect regarding their skin color. The assessment is part of a questionnaire utilized in a 3-year longitudinal study that examines psychosocial development, physical health, and behavioral adjustment of Puerto Rican children (N = 257) reared in the Greater Boston area. The results demonstrate that children's internalized representation of their skin color is a construct that can be reliably and validly measured. The children's ratings of their skin color were not associated with their sex, school grade, ethnic identity, the child's or the parent's nativity, or the racial or ethnic compositions of 3 social contexts: their neighborhood, their classmates, and their closest friends. Puerto Rican children did not show a preference for light-colored skin. Moreover, there were no significant differences in self-esteem based on the child's self-reported skin color. The lack of association between self-esteem and skin color was interpreted in light of a developmental tendency prevalent in early to middle childhood to place a positive value on different aspects of one's self. Whereas almost all children (96%) reported being happy or very happy with their color, 16% of the children would like to change their skin color if they could (51% to a lighter and 46% to a darker color).  相似文献

The ionizing irradiation of rat fetuses during the last third of intrauterine development increased blood corticosterone level adulthood and decreased the open field locomotion of their adult offsprings of the next first nonirradiated generation. Treatment of the pregnant rats with glucocorticoids also decreased the offspring locomotion. Irradiation of fetuses in the middle of embryogenesis decreased blood corticosterone level in adulthood and shortened the open-field freezing reaction of their adult offsprings of the next first nonirradiated generation. Adrenalectomy of females before mating decreasing the blood corticosterone level had a similar effect on freezing duration of their adult offsprings. Irradiation of the ancestors within the last third of their intrauterine development had no effect on blood corticosterone level of their adult offsprings of the first generation and produced no behavioral alterations in their descendants of the next second nonirradiated generation. Irradiation of the ancestors in the middle of their embryogenesis decreased the stress-induced corticosterone response in their adult offsprings of the first generation and increased rearings and locomotion in their descendants of the next second nonirradiated generation. The data suggest that a single noxious treatment may have behavioral effects throughout two consequent generations of rats. Mother's glucocorticoid hormones may be one of the factors which transmit the effect.  相似文献

Studies of the glycosyltransferases (GTs) of small molecules have greatly increased in recent years as new approaches have been used to identify their genes and characterize their catalytic activities. These enzymes recognize diverse acceptors, including plant metabolites, phytotoxins and xenobiotics. Glycosylation alters the hydrophilicity of the acceptors, their stability and chemical properties, their subcellular localisation and often their bioactivity. Considerable progress has been made in understanding the role of GTs in the plant and the utility of GTs as biocatalysts, the latter arising from their regio- and enantioselectivity and their ability to recognize substrates that are not limited to plant metabolites.  相似文献

Plant cell wall-degrading enzymes, such as cellulases, hemicellulases and pectinases, have been extensively studied because of their well documented biotechnological potential, mainly in the food industry. In particular, lytic enzymes from filamentous fungi have been the subject of a vast number of studies due both to their advantages as models for enzyme production and their characteristics. The demand for such enzymes is rapidly increasing, as are the efforts to improve their production and to implement their use in several industrial processes, with the goal of making them more efficient and environment-friendly. The present review focuses mainly on pectinolytic enzymes of filamentous fungi, which are responsible for degradation of pectin, one of the major components of the plant cell wall. Also discussed are the past and current strategies for the production of cell wall-degrading enzymes and their present applications in a number of biotechnological areas.  相似文献

Many common devices can be used as biofeedback devices. The feedback use of the bathroom scale in rehabilitation medicine(physical therapy) has been explored with stroke patients. These patients used two scales, one under each foot, to relearn their sense of weight distribution. In addition, the scale was used to teach patients to shift their weight distribution from their knees to their hands as they learned to crawl, and from their feet to their buttocks and the chair as they learned to sit down.  相似文献

The case records of 100 patients over the age of 60 at the time of their admission to a state mental hospital in California were evaluated, using rigid criteria to determine whether the precipitating cause for their state hospitalization was due to a deterioration of their mental state or a change in their socio-environmental milieu. The results of the study indicated that 77 per cent of these patients were in hospital because of a deterioration in their own mental state while 23 per cent were there because of a deterioration in their socio-environmental milieu. The data also were indicative that the proportion of patients put in hospital because of a deterioration in their socio-environmental milieu might conceivably be higher than 23 per cent if subtle changes in family attitude towards the patient could be adequately evaluated.  相似文献

In U.S. nursing homes, it is the job of nursing assistants to tend to residents' basic bodily needs, including elimination and incontinence care. Given their frequent contact with pollutants, aides are very much at risk of becoming "polluted people." In this article, I investigate how nursing assistants' continual contact with contaminating substances impacts their status within the workplace, their relationships with others, and their attitudes toward their work and themselves as workers. I also explore how aides manage their encounters with pollutants and their stigmatized role as "dirty workers." In doing so, I hope to explicate the meaning of elimination and of incontinence caregiving in the United States.  相似文献

For the first time V. fluvialis strains were detected on the territory of the USSR. The taxonomic position of these vibrios was determined by their nucleotide DNA composition (the content of guanine + cytosine was 49.3-51.0 mole%) and the characteristic features of their phenotype. The individual features of the strains consisted in their capacity for agglutination with cholera antisera, groups 01 and Inaba, in diagnostic dilutions in the presence of differences in genomes and phenotypes with cholera vibrios. Molecular hybridization DNA-DNA also gave no confirmation of their relationship to cholera vibrios (23-26% homology). The comparative study of V. fluvialis strains from the USSR and other countries by a broader set of their phenotypical signs confirmed their identity.  相似文献

Mesenchymal stem cells (MSCs) are attractive candidates for inclusion in cell-based therapies by virtue of their abilities to home to wound sites. However, in-depth characterization of the specific effects of MSCs on their microenvironments is needed to realize their full therapeutic potentials. Furthermore, since MSCs of varying properties can be isolated from a diverse spectrum of tissues, a strategic and rational approach in MSC sourcing for a particular application has yet to be achieved. For example, MSCs that activate their proteolytic environments may promote tissue remodeling, while those from different tissue sources may inhibit proteases and promote tissue stabilization. This study attempts to address these issues by analyzing MSCs isolated from three adult tissue sources in terms of their effects on their proteolytic microenvironments. Human bone marrow, adipose, and traumatized muscle derived MSCs were compared in their soluble and cellular-associated MMP components and activity. For all types of MSCs, MMP activity associated with the cell surface, but activity levels and MMP profiles differed with tissue source. All MSC types bound exogenous active MMPs at their surfaces. MSCs were also able to activate exogenous proMMP-2 and proMMP-13. This is in marked contrast to the MSC soluble compartment, which strongly inhibited MMPs via endogenous TIMPs. The exact TIMP used to inhibit the exogenous MMP differed with MSC type. Thus, MSCs saturate their environment with both MMPs and TIMPs. Since they bind and activate MMPs at their surfaces, the net result is a very controlled pericellular localization of MMP activities by MSCs.  相似文献

Epigenetic and regulatory elements provide an additional layer of complexity to the heterogeneity of anxiety disorders. MicroRNAs (miRNAs) are a class of small, noncoding RNAs that have recently drawn interest as epigenetic modulators of gene expression in psychiatric disorders. miRNAs elicit their effects by binding to target messenger RNAs (mRNAs) and hindering translation or accelerating degradation. Considering their role in neuronal differentiation and synaptic plasticity, miRNAs have opened up new investigative avenues in the aetiology and treatment of anxiety disorders. In this review, we provide a thorough analysis of miRNAs, their targets and their functions in the central nervous system (CNS), focusing on their role in anxiety disorders. The involvement of miRNAs in CNS functions (such as neurogenesis, neurite outgrowth, synaptogenesis and synaptic and neural plasticity) and their intricate regulatory role under stressful conditions strongly support their importance in the aetiology of anxiety disorders. Furthermore, miRNAs could provide new avenues for the development of therapeutic targets in anxiety disorders.  相似文献

The Mikirs living in Karbi-Anglong district in northeastern India depend on the natural plant resources of their neighbouring forests for much of their food, all their medicine, and for other material cultural needs and recreation. They, of course, use many plants in their magico-religious ceremonies. The area has remained botanically and ethnobotanically unexplored until the recent studies were initiated in 1976. Many heretofore unknown or little-known uses of plants have been recorded. However, much remains to be learnt from this region regarding the use of plants for human welfare.  相似文献

Temporally long-ranging (=long-lived) taxa have been postulated to have unusual properties that aid their prolonged geologic survival. Past studies have examined dispersal capabilities, geographic ranges, and single-character morphological adaptations as factors that may contribute to geologic longevity. Here, I test whether long-lived fossil crinoid taxa are morphologically unusual using a whole suite of morphological characters. I define long-lived taxa in several explicit, comparative ways. I find that long-lived crinoid genera and families are often less distant from mean morphologies of their crinoid orders than their shorter-lived relatives; that is, they are relatively less specialized. I also compare the morphology of crinoid genera relative to basal members of their respective orders; mean morphological distances of long-lived genera from basal morphologies are seldom distinct from those of their shorter-lived relatives. I observe that long-lived crinoid genera are less distant from mean morphologies of their temporal cohorts compared with shorter-lived genera but not in a statistically significant manner. I conclude that long-lived crinoids are relatively unspecialized, in the sense that they are relatively closer to mean morphologies of their taxonomic groups.  相似文献

Two photosensitizing preparation for the photodynamic therapy of oncological diseases have been obtained. As revealed in this study, under experimental conditions their action suppresses the growth of Enterococcus faecalis and Staphylococcus aureus, which may be of great importance for the treatment of infections. And in contrast to antibiotic therapy, their use may not affect the development of normal intestinal microflora. The study has shown that their derivatives with the residues of carbonic (or muramic) acids more intensively penetrate microorganisms through their cell walls. This makes in possible to expand the spectrum of action of the preparations. The proposed method permits the rapid analysis of the photochemical activity of preparation for photodynamic therapy in the process of their development.  相似文献
