267株肠球菌分布及耐药性分析   总被引:2,自引:0,他引:2  
目的 了解肠球菌的分布特征和耐药特点,指导临床合理用药.方法 采用美国Dade Behring Mi-croscan Walkaway40全自动细菌鉴定及药敏测试仪对267株肠球菌进行鉴定和药敏测试,用全国医院感染监测网软件和P检验进行分析.结果 267株肠球菌以屎肠球菌和粪肠球菌为主,分别占60.7%和31.5%,以尿液和脓液中分离出最多,其次为血液和胆汁,依次是56.6%、14.2%、10.9%、9.7%,占总标本的91.4%.肠球菌不同种间的耐药性差异存在显著性.屎肠球菌的耐药率相对较高,除对万古霉素的耐药率3.7%最低,四环素37.7%外,对其它抗菌药物的耐药率均超过了50%.粪肠球菌的耐药率虽比屎肠球菌低,但对红霉素、四环素、氨基糖苷类的耐药性仍高,在60%以上.屎肠球菌和粪肠球菌对高浓度庆大霉素和链霉素的耐药率均在55%以上.共检出耐万古霉素株11株,对万古霉素的耐药率为4.1%.结论 肠球菌可引起多种部位感染,呈多重和高耐药性,耐万古霉 素的菌株出现,增加了感染控制难度,应引起临床重视.  相似文献

了解2001~2003年肠球菌对常用抗菌药物的耐药性,为临床治疗肠球菌感染提供参考。采用Kirby-Bauer纸片扩散法进行药敏试验。结果显示:2001~2003年肠球菌属的耐药率呈上升趋势。万古霉素和替考拉宁依然是敏感性最高的抗生素。青霉素、氨苄西林和呋喃妥因对粪肠球菌具有较好的抗菌活性,敏感率分别为62.5%、67.1%和92.4%。屎肠球菌对除糖肽类抗生素以外的所有临床常用抗菌药物显示高水平耐药。可见,动态监测肠球菌的耐药状况对指导临床治疗具有重要意义。  相似文献

本文报告200株肠球菌系统生理生化、血清学及药敏试验研究。其结果为:粪肠球菌165株,屎肠球菌21株,鸟肠球菌和孤立肠球菌各3株,希拉肠球菌2株,假鸟肠球菌、鸡肠球菌、芒地肠球菌、坚韧肠球菌各1株,2株未定种。148株含D抗原,占74%。对其进行青霉素、氨苄青霉素、氟哌酸和呋喃妥因药敏试验,敏感率较高,并发现屎肠球菌比粪肠球菌更耐药。对严重的肠球菌感染,在测定肠球菌高浓度(≥2g/L)氨基糖昔类耐药性指导下,以联合用药为佳。  相似文献

【摘 要】 目的 了解2011年中国重庆市主要7所教学医院临床分离粪肠球菌和屎肠球菌对各类抗菌药物的耐药性。方法 重庆市主要7所教学医院(6所综合性医院,1所儿童医院)按统一方案、采用统一的材料、方法和判断标准(CLSI 2011年版)进行粪肠球菌和屎肠球菌的耐药性监测。数据用WHONET 5.5软件按照CLSI 2011年版折点进行分析。结果 共分离到非重复粪肠球菌589株、屎肠球菌675株,对利奈唑胺、万古霉素、替考拉宁仍极敏感,耐药率<2%,万古霉素耐药粪肠球菌和屎肠球菌检出率分别为0.3%、0.7%。粪肠球菌对青霉素、氨苄西林、呋喃妥因的耐药率较低,分别为14.8%、8.6%和5.1%,对高浓度庆大霉素的耐药率分别为46.9%;屎肠球菌耐药性明显高于粪肠球菌,对青霉素和氨苄西林耐药率接都在90%左右。儿童和成人耐药率存在一定差别。结论 本市医院肠球菌感染以屎肠球菌为主, 粪肠球菌次之,两者耐药性明显不同, 监测其耐药情况对指导临床用药具有重要意义。  相似文献

目的 监测我院肠球菌中粪肠球菌株和屎肠球菌株的耐药性,为临床合理应用抗菌药物提供依据。方法 采用法国生物梅里埃公司的GPI板进行细菌鉴定及药敏试验,应用whonet5软件统计粪肠球菌和屎肠球菌的耐药率。结果 粪肠球菌和屎肠球菌对氯霉素、呋喃妥因、万古霉素有较好体外抗菌活性,耐药率都在50%以下,对万古霉素的耐药率在1%以下。粪肠球菌对青霉素、高水平庆大霉素、环丙沙星、利福平、红霉素等大部分抗菌素的耐药率有逐年下降趋势,而屎肠球菌对环丙沙星、利福平、呋喃妥因等抗菌素的耐药率则有上升趋势,屎肠球菌对大多数抗菌素耐药率都高于粪肠球菌。结论 粪肠球菌和屎肠球菌呈多重耐药,临床用药应结合药敏试验结果合理选择抗菌药物。  相似文献

目的研究肝硬化患者肠球菌感染的现状及药敏特点,以加强对肝硬化肠球菌感染的认识,指导用药。方法留取标本进行细菌培养、鉴定及药敏试验。结果所有纳入研究范围的肝硬化患者共检出肠球菌112株,其中粪肠球菌89株,占79.5%,比例最高,屎肠球菌占14.3%,居第2位。腹水中检出肠球菌64株,占57.1%,其次为痰和尿液,分别为38.1%和14.4%。肠球菌对红霉素、奈替米星的耐药率超过80%,对青霉素G、氨苄西林、呋喃妥因的敏感率超过70%,对左氧氟沙星的敏感率为58.9%,对头孢唑啉、头孢派酮的敏感率分别只有25.0%和33.0%,检出高耐氨基糖苷类肠球菌(HLAR)58株,占51.8%,未检出耐万古霉素和替考拉宁肠球菌,但检出万古霉素中介肠球菌4株,占3.6%。结论肠球菌是肝硬化患者医院感染的重要致病菌,尤以粪肠球菌和屎肠球菌为主,屎肠球菌的耐药率高于粪肠球菌,万古霉素和替考拉宁是治疗肠球菌感染的首选药物。  相似文献

目的 :比较临床分离肠球菌与肠道肠球菌的不同特性。方法 :分别检测 4 1株临床分离肠球菌以及 38株健康人群粪便分离肠球菌的属种分布、β溶血素检出率及耐药性情况。 结果 :临床菌株以粪肠球菌为主 (75 6 % ) ,健康人群粪便分离肠球菌以屎肠球菌为主 (73 7% ) ;临床菌株的 β溶血素检出率高于健康人群粪便分离肠球菌 (P <0 0 1) ;临床分离肠球菌的抗生素耐药性明显高于肠道肠球菌 (P <0 0 1)。结论 ;引起医院感染的肠球菌与肠道球菌特性不同。  相似文献

目的了解温州医学院附属第一医院临床分离主要肠球菌的分布及其对常用抗菌药物的耐药现状,以指导临床合理用药。方法对2008年至2011年临床分离的635株粪肠球菌和屎肠球菌的标本来源和药敏结果进行回顾性分析。结果各种临床标本中两种肠球菌的分布比例存在差异,总体以尿液标本所占比例最多,且屎肠球菌的总体分离率高于粪肠球菌。粪肠球菌对利奈唑胺、氨苄西林、万古霉素、呋喃妥因和替考拉宁的耐药率都在5.0%以下,对莫西沙星和青霉素G的耐药率也仅为7.0%和6.7%;屎肠球菌对莫西沙星、左旋氧氟沙星、环丙沙星、氨苄西林、青霉素G和红霉素的耐药率都在90.0%以上,对利奈唑胺、万古霉素、替考拉宁和奎奴敏感。粪肠球菌的多重耐药株占总数的26.4%,屎肠球菌的多重耐药株占总数的78.2%。结论粪肠球菌和屎肠球菌对15种抗菌药物的耐药情况不同,屎肠球菌具有更高的耐药率和更广的耐药谱。临床应根据药敏试验的结果合理选择抗菌药物,以防止耐药菌株的产生和播散。  相似文献

目的 了解中国医科大学附属盛京医院肠球菌属细菌的临床分布及耐药情况,为指导临床制定合理有效治疗方案提供理论依据。方法 收集2017年1月1日至2017年12月31日我院门诊、住院患者肠球菌属细菌病原学资料,回顾性分析其菌种分布、标本分布、科室来源及其耐药特征。结果 共分离出466株肠球菌属细菌,其中粪肠球菌211株(45.28%)、屎肠球菌255株(54.72%)。肠球菌属细菌均以尿液标本为最主要的来源,其次为全血标本,并且来源于尿液标本的肠球菌属细菌对临床常用抗生素的耐药率普遍高于非尿液标本肠球菌属细菌耐药率。儿科为屎肠球菌最主要的分布科室,而粪肠球菌则主要分布于泌尿外科。耐药性方面,除四环素、利福平和喹努普汀/达福普汀外,屎肠球菌对受检的其余18种抗生素的耐药性均高于粪肠球菌,但对喹努普汀/达福普汀无耐药性,而粪肠球菌对此种抗生素的耐药率却为100.00%。二者对克林霉素的耐药率均为100.00%。共检出耐万古霉素肠球菌7株(1.50%),耐替考拉宁肠球菌6株(1.28%),未发现对替加环素、利奈唑胺耐药菌株。结论 屎肠球菌在肠球菌属细菌中占主要地位,且耐药率方面明显高于粪肠球菌,耐糖肽类抗生素菌株的出现需引起临床的高度警惕,应进一步加强耐药菌株的监测,并对临床应用抗生素加以规范。  相似文献

目的:了解我院临床分离肠球菌的分布特征及耐药现状,为临床合理用药提供依据。方法:对我院2010年1月至2012年12月期间所有临床分离的肠球菌分布情况及药敏结果进行回顾性分析。结果:临床共分离肠球菌242株,粪肠球菌分离率(55.0%)高于屎肠球菌(40.9%),屎肠球菌分离率有增高的趋势。标本来源以尿液(62.9%)、分泌物(10.3%)、血液(6.9%)为主。肠球菌对万古霉素、替考拉宁的敏感性最高,均高于90%。发现耐万古霉素的肠球菌(VRE)7株,其中5株同时耐高浓度的氨基糖苷类抗生素(HLAR);对克林霉素、复方磺胺、阿米卡星、庆大霉素、妥布霉素、苯唑西林耐、头孢西丁耐药率最高,均高于95%。屎肠球菌对青霉素类、氨苄西林、红霉素、呋喃妥因、环丙沙星耐药率均高于粪肠球菌;对四环素、奎努普丁/达福普汀耐药率低于粪肠球菌。结论:肠球菌是临床感染重要病原菌,且具有多重耐药性,屎肠球菌和粪肠球菌耐药水平差异较大,临床应根据药敏结果合理选择抗菌药物。  相似文献

目的了解肠球菌在重症监护病房的感染状况及耐药现状,指导临床合理用药。方法从重症监护病房送检的各类标本中分离到的316株肠球菌,采用VITEK-AMS微生物自动分析仪进行鉴定和药敏试验。结果316株中以粪肠球菌(54.8%)及屎肠球菌(43.0%)为主,屎肠球菌对β-内酰胺类抗生素的耐药率明显高于粪肠球菌,但粪肠球菌对喹奴普汀/达福普汀的耐药率明显高于屎肠球菌(P<0.01)。粪肠球菌对万古霉素耐药率为3.7%,力奈唑烷耐药率为0.7%,屎肠球菌对万古霉素耐药率为0.6%,力奈唑烷耐药率为0%。结论肠球菌为重症监护病房感染的主要致病菌,万古霉素合理正确的使用仍是当前治疗肠球菌感染的重要途径,VRE的出现对临床合理使用抗生素提出了更高的要求。  相似文献

目的波市妇女儿童医院肺炎链球菌的临床分布和耐药情况,为临床合理用药提供依据。方法对该院2009年6月1日至2011年3月31日期间分离的114株肺炎链球菌进行分析,菌株鉴定采用法国生物梅里埃公司的VITEK60分析仪,药敏试验采用K-B法,用参考菌株做质量控制。结果该院分离的肺炎链球菌主要来自儿童(84.21%),标本来源主要是痰液(61.4%),其次是血液(9.65%),其他(28.95%)。肺炎链球菌的耐药率:林可霉素92.19%,红霉素90.12%,青霉素G85.53%,左旋氧氟沙星20.24%,氨苄西林16.67%,头孢唑啉8.7%,头孢曲松5.8%,头孢噻肟4.11%,万古霉素0%,氨苄西林/舒巴坦0%,哌拉西林/他唑巴坦0%。结论宁波市妇女儿童医院肺炎链球菌对某些药物的耐药率很高,有必要对其进行耐药率监测,指导临床合理选择抗菌药物。  相似文献

回顾性分析上海市某三甲医院血培养阳性标本中粪肠球菌和屎肠球菌的临床分布及对抗菌药物的耐药特征,为临床治疗其所致感染奠定基础。收集上海市某三甲医院2012年2月—2016年9月血流感染患者血液标本中的粪肠球菌和屎肠球菌,采用法国生物梅里埃公司的VITEK 2Compact全自动细菌鉴定和药敏分析系统进行细菌鉴定及药敏测定,研究细菌临床分布特点及对常用抗菌药物的耐药特征。共分离获得30株粪肠球菌和17株屎肠球菌。粪肠球菌样本主要来自泌尿科、消化科和血液科,所占比例分别为13.33%、16.67%和10.00%。粪肠球菌对青霉素、氨苄西林、环丙沙星、左氧氟沙星、四环素和红霉素的耐药率分别为13.33%、10.00%、36.67%、33.33%、66.67%和60.00%。屎肠球菌样本主要来自消化科(29.41%),其对以上抗菌药物的耐药率分别为88.24%、82.35%、88.24%、76.47%、23.53%和70.59%。屎肠球菌对青霉素、氨苄西林、环丙沙星和左氧氟沙星的耐药率显著高于粪肠球菌,而对四环素的耐药率显著低于粪肠球菌。两者均对替加环素、利奈唑胺和万古霉素敏感,但万古霉素对屎肠球菌的最低抑菌浓度显著低于粪肠球菌。结果提示,屎肠球菌对青霉素、氨苄西林、环丙沙星、左氧氟沙星的耐药率高于屎肠球菌,对万古霉素敏感,且其万古霉素最低抑菌浓度低于粪肠球菌。本研究为治疗这两种细菌所致感染的经验性用药提供了数据支持。  相似文献

Aims:  The level of genetic diversity and relationships between the specific genotypes and the distribution of virulence determinants among Enterococcus faecalis strains isolated from patients hospitalized in different wards of two hospitals were investigated.
Methods and Results:  Fifty-six clinical strains of E. faecalis , isolated from patients hospitalized in the period of 1999–2004 in several wards in Wrocław (Poland), were analysed by multiple-locus variable-number tandem-repeat analysis (MLVA). Analysis of seven genomic loci identified 40 novel genotypes among the analysed E. faecalis strains, with two major genomic groups, designated I and II, distinguished at a cut-off of 35%. With a similarity cut-off of 85·7%, the genotypes could be combined into 12 clusters (C1–C12), containing at least two isolates. The remaining 18 MLVA types were represented by a single isolate.
Conclusions:  Based on the data obtained by MLVA, it was found that (i) many E. faecalis isolates recovered from patients from the wards whose location allowed the potential transmission of micro-organisms, belonged to closely related MLVA types and (ii) possible relationships between specific E. faecalis genotype and the virulence factors lipase, haemolysin and esp gene can exist.
Significance and Impact of the Study:  Our study confirms that MLVA is a suitable method for the epidemiological study of E. faecalis and for the first time shows possible relationships between specific genotypes and such virulence determinants, i.e. lipase, haemolysin and esp gene.  相似文献

AIMS: The aim of this study was to examine two VanA-type vancomycin-resistant Enterococcus faecium (VRE) strains that had been isolated from patients resident in mainland China. This is the first molecular analysis of clinical VRE strains being isolated in mainland China. METHODS AND RESULTS: Two VanA-type VRE isolates were isolated from in-patients at hospitals located in the Chinese cities Beijing and Dalian and were designated C264 and I125. The plasmids pC264V (40 kbp) and pI125V (370 kbp) that were isolated from C264 and I125, respectively, carried a Tn1546-like element encoding VanA resistance. The vancomycin-resistant plasmids pC264V and pI125V were transferred by filter mating at frequencies of 10(-7) and 10(-4) respectively. Sequence analysis of pC264V revealed that two IS1216V sequences and an IS1542 sequence were present within the Tn1546-like element. pI125V had two IS1216V insertions in the Tn1546-like element. CONCLUSIONS: The two VanA-type vancomycin-resistant E. faecium (VRE) strains C264 and I125 were isolated from in-patients in Chinese hospitals. The vancomycin-resistant conjugative plasmids pC264V and pI125V plasmids isolated from these strains carried the Tn1546-like element. The Tn1546-like element was found to contain the insertion sequences IS1216V and IS1542. SIGNIFICANCE AND IMPACT OF THE STUDY: This is the first molecular analysis of VanA-type VRE strains from patients resident in mainland China.  相似文献

Enterococci population from Argentinean artisanal dry fermented sausage was identified and their safety aspects were evaluated. Species-specific PCR was used to distinguish between Enterococcus faecium (56%) and Enterococcus faecalis (17%). Other isolates (27%) were identified as Enterococcus durans , Enterococcus casseliflavus and Enterococcus mundtii by using 16S RNA gene sequence. RAPD analyses showed different biotypes for Ent. faecium and Ent. faecalis species. Low incidence of antibiotic resistance and high virulence traits in Ent. casseliflavus and Ent. faecalis were found; the majority of the Ent. faecium strains were shown to be free of virulence factors. The absence of virulence/resistance traits and the anti-Listeria activity of Ent. faecium isolates may be exploited to enhance natural preservation thereby guaranteeing organoleptic/safety characteristics of artisanal fermented sausages.  相似文献

Vancomycin-resistant enterococci represent a large reservoir in animals because of the use of avoparcin as a growth promoter in Europe. These strains of animal origin enter the food chain and can either colonize the human gut or transfer their resistance genes to the human microbiota. In this study, we compared the transfer of vancomycin resistance from resistant animal Enterococcus faecium to sensitive human Enterococcus faecalis and E. faecium. We analysed these transfers in dibiotic mice and human faecal flora-associated mice. VanA transfer from animal E. faecium to human E. faecalis occurred in dibiotic mice. The transconjugants appeared rapidly and persisted at levels between 3.0 and 4.0 log10 colony-forming units g(-1) of faeces. In human faecal flora-associated mice, vanA gene transfer was not detected towards E. faecalis but was possible between E. faecium strains. Our experiments revealed the possibility of vanA transfer from animal E. faecium to human E. faecalis in vitro and in vivo in the intestine of dibiotic mice. However, intraspecies transfer of vanA gene seems more common than interspecies transfer among enterococci.  相似文献
