Background

Acute respiratory infection (ARI) is a leading cause of morbidity and mortality in children worldwide. This study aimed to determine the viral and atypical bacterial causes of different severities and clinical manifestations of ARI in preschool children from low-income families in North-East Brazil.

Methods

Clinical/demographic data and nasopharyngeal aspirates (NPA) were prospectively collected from children <5 years presenting with ARI over one year to a paediatric A&E department. Disease severity was grouped according to presence of lower respiratory tract signs, need for hospital admission and need for oxygen. Clinical manifestation of ARI was based on discharge diagnosis from hospital with four conditions predominating: bronchiolitis, pneumonia, episodic viral wheeze/asthma and upper respiratory tract infection. Multiplex PCR was used to detect 17 common respiratory viral and atypical bacterial pathogens in NPA.

Findings

407 children with a median age of eight months were recruited. Pathogens were detected in 85·5% samples with co-infection being particularly common (39·5%). Respiratory Syncytial Virus (RSV; 37%), Adenoviruses (AdV; 25%), Rhinoviruses (hRV; 19%), Bocavirus (hBoV; 19%), human Meta-pneumovirus (hMPV; 10%) and Mycoplasma pneumoniae (Mpp; 10%) were most prevalent. Detection and co-infection rates were similar in all severities and clinical manifestations of ARI apart from RSV, which was associated with more severe disease and specifically more severe cases of bronchiolitis, and Mpp, which was associated with more severe cases of pneumonia. Mpp was detected in 17% of children admitted to hospital with pneumonia.

Interpretation

This study underlines the importance of viral and atypical bacterial pathogens in ARI in pre-school children and highlights the complex epidemiology of these pathogens in this age group. Generally, viruses and atypical bacteria were detected in all severities and clinical manifestations of ARI but RSV and Mpp were associated with more severe cases of bronchiolitis and pneumonia respectively.  相似文献

目的对2015年兰州单中心急性下呼吸道感染患儿中呼吸道合胞病毒(respiratory syncytial virus,RSV)进行分离鉴定及分析。方法收集临床急性下呼吸道感染患儿痰分泌物226份,在HEP-2细胞中连续盲传培养3代,收获的培养物用半巢式RT-PCR进行检测,选取强阳性样品进行测序以确定其型别,并对其流行病学特征进行分析。结果 226份样品中RSV阳性样本为76份(阳性率为34%)。RSV阳性患儿中,男女性别比为3∶1;年龄分布6月龄患儿占45%,6月龄~2岁占35%,2~5岁的占20%。冬季为发病高峰。76例RSV阳性患儿中,临床诊断依次为毛细支气管炎31例(40.79%)、喘息性支气管炎21例(27.36%)、支气管肺炎20例(26.32%)和支气管哮喘4例(5.26%)。40份强阳性RSV样品经测序确定为B亚型。结论 RSV是引起婴幼儿急性下呼吸道感染的重要病原之一,2015年RSV的流行株主要为B亚型。  相似文献

Adenoviruses (AdV) are commonly involved in acute respiratory infections (ARI), which cause high morbidity and mortality in children. AdV are grouped in six species (A-F), which are associated with a wide range of diseases. The aim of this study was to identify the AdV species infecting non-hospitalized Mexican children with ARI symptoms, attending to the same school. For that, a PCR/RFLP assay was designed for a region of the hexon gene, which was chosen, based on the bioinformatical analysis of AdV genomes obtained from GenBank. A total of 100 children's nasopharyngeal samples were collected from January to June, 2005, and used for viral isolation in A549 cells and PCR/RFLP analysis. Only 15 samples produced cytopathic effect, and in all of them AdV C was identified. AdV C was also identified in eight additional nasopharyngeal samples which were negative for viral isolation. In summary, this outpatient population showed a rate of AdV infection of 23%, and only AdV C was detected.  相似文献

With a view to obtaining fuller information on airborne pollen content in the city of Havana, pollen sampling was carried out using a volumetric capture method, for the first time in Cuba. The study was conducted during 2 years (2011 and 2015). An annual pollen integral of 3414 grains was registered during the first year of study, whereas 5120 grains were observed along the 2015. Monthly maximum concentrations were recorded during April, June and July with total values close to 800 pollen grains. Of the 45 pollen types identified, Cecropia (38% of the total pollen identified in both years), Poaceae (18%), Urera type (9%) and Casuarina (6%) were particularly abundant. Although the main pollen types differed in terms of intradiurnal distribution, the highest concentrations were in all cases recorded between 0900 and 1300 hours. Maximum temperature was the variable most influencing airborne pollen counts in the air, with the exception of Casuarina. This paper sought to establish a methodological basis for the further development of aerobiological research in Cuba, thus helping to enhance the prevention and diagnosis of pollen allergies in the affected island population.  相似文献

目的分析大连地区急性呼吸道感染患儿病原体检测结果及流行情况,为临床早期诊断、治疗及预防提供依据。方法调查2015年1月-2016年7月医院住院急性呼吸道感染患儿2 944例,采用间接免疫荧光法(IFA)对呼吸道感染患儿血液中的呼吸道合胞病毒(respiratory syncytial virus,RSV)、腺病毒(adenovirus,ADV)、流感病毒A型(influenza virus type A,IFA)、流感病毒B型(influenza virus type B,IFB)、副流感病毒(parainfluenza virus,PIV)、肺炎衣原体(Chlamydia pneumoniae,CP)、肺炎支原体(Mycoplasma pneumonia,MP)和嗜肺军团菌(Legionella pneumophila,LP)8种病原体进行检测。结果 2 944例患儿中血清IgM抗体阳性共1 280例,总阳性率为43.48%(1 280/2 944),主要为IFB和MP感染:IFB阳性率最高,为16.68%(491/2 944);其次为MP,阳性率为14.44%(425/2 944)。ADV各年龄组之间差异无统计学意义(χ~2=0.286,P=0.5930.05),其余各种病原体不同年龄组之间差异均有统计学意义。其中IFB、MP在3~6岁组中检出率最高;RSV在0~1岁组中检出率明显高于其他年龄组。MP男女患儿组间差异有统计学意义(χ~2=7.671,P=0.0060.05),其余各项差异均无统计学意义(P0.05)。IFB第二季度和第三季度检出率高于其他季度,MP在第三季度检出率最高,IFA、LP、CP也同样是第三季度检出率最高,PIV和ADV各季度检出率差异无统计学意义(P0.05),RSV在第一季度检出率略高于其他季度。结论大连地区引起患儿呼吸道感染的病原体主要是IFB和MP,针对不同年龄组、不同季度的感染情况,应做好重点预防工作。  相似文献

【目的】从湖南长沙市采集到的土样中分离假单胞菌并进行归类,研究各菌株抑菌和抗肿瘤生物活性,以丰富假单胞菌菌种资源并为微生物次级代谢物的挖掘奠定基础。【方法】采用大蜡螟诱集法诱集分离假单胞菌,结合形态观察、生理生化特征和16S rRNA基因序列同源性分析,鉴定并归类各细菌,通过平板扩散法、对峙培养法和肿瘤细胞毒性试验分别研究各菌株抑制细菌、拮抗真菌和抗肿瘤细胞等生物活性。【结果】从湖南长沙市郊区菜地、林地中分离得到5株假单胞菌,归类并命名为Pseudomonas protegens CY01、绿针假单胞菌CY02(Pseudomonas chlororaphis CY02)、栖稻假单胞菌CY04(Pseudomonas oryzihabitans CY04)、Pseudomonas sp.CY05和恶臭假单胞菌CY06(Pseudomonas putida CY06)。P.protegens CY01和P.chlororaphis CY02对枯草芽胞杆菌(Bacillus subtillis)和金黄色葡萄球菌(Staphylococcus aureus)具有较好的抑菌效果,P.chlororaphis CY02对水稻稻瘟病菌(Pyricularia oryzae)具有良好的拮抗作用,对小鼠黑色素瘤细胞B16具有较强的细胞毒性。【结论】分离得到的P.chlororaphis CY02,在抑制病原细菌、拮抗水稻稻瘟病菌和抗肿瘤细胞等方面具有显著效果。  相似文献

A locality in the district of Tlalpan, Mexico City, was selected in order to identify the viral agents in children younger than 5 years of age with acute respiratory infection (ARI). A total of 300 children were randomly selected and were included in this study for a period of 13 months. During this period nasopharyngeal exudates were collected for the isolation of viral agents. Monoclonal fluorescent antibodies were used for viral identification after cell culture. Viral infection was detected in 65% of the specimens. The respiratory syncytial virus (RSV) was the most common virus agent detected. Children required an average of two consultations during the study period. Two high incidence peaks were observed, one during the summer and the other during winter; the most frequent viruses during these seasons were influenza A and RSV, respectively. The largest number of viruses was isolated in the group of children between 1 and 2 years of age and in the group between 4 and 5 years of age. This study demonstrated the presence of ARI and of different viruses in a period of 13 months, as well as the most frequent viruses in children younger than 5 years of age from a community of Mexico City.  相似文献

The Havana aeromycobiota diversity was studied from November 2010 to October 2011 using two complementary volumetric methods. A total of 35 fungal genera were characterised, 26 of them were recognised only by non-viable methods, six with viable methodology and the other three with both sampling methods. Furthermore, 47 species were identified by cultivation and the spores collected with the non-viable methodology. These could not be included in a specific genus, and thus, were categorised into five fungal types. In general, the main, spread worldwide, mitosporic fungi also predominated the Havana atmosphere. The predominant species were Cladosporium cladosporioides, Aspergillus flavus and Penicillium citrinum. Moreover, several Zygomycetes (Syncephalastrum racemosum, Rhizopus stolonifer and Rhizopus oryzae), Ascomycetes (Chaetomium globosum) and Basidiomycetes such as Coprinus or Ganoderma were isolated. In the present paper, the review of the airborne fungi conducted in previous studies in Cuba was completed by the detection of two new genera and the first isolation of ten new records in the Cuban atmosphere. Most of the fungi detected showed a diurnal pattern with high spore peak at 11.00–12.00.  相似文献

目的 体外分离出猴腺病毒(simian adenovirus,SAdV)毒株,并对其进行鉴定,为SAdV的生物学特性、致病机制和诊断试剂等方面的研究提供基础.方法 采用PCR法对猕猴粪便样本进行SAdV筛查,将PCR检测为阳性的样本处理后接种BSC-1细胞,盲传3代后用PCR扩增测序并负染色电镜观察.结果 从广东地区食蟹猴粪便样本中成功分离出一株SAdV,细胞病变明显,PCR检测呈阳性,电镜下可观察到典型腺病毒样形态的病毒粒子,命名为GD株.经测序鉴定,发现GD株属HAdV-G亚属,与SAdV-1进化发生距离最近.结论 GD株的分离成功,使猴腺病毒基因转移载体的构建成为可能,有助于猴腺病毒替代载体的发展.  相似文献

The role of influenza and parainfluenza viruses, respiratory syncytial viruses and adenoviruses in the etiological structure of morbidity in acute respiratory virus infections (ARVI) in children hospitalized during the 19 year period is analyzed. As the result of examination of 56,287 patients by direct immunofluorescent test, respiratory viruses were detected in 21% of cases. The seasonal character, periodicity and level of ARVI morbidity were established. According to medical records, in the 1990s ARVI took a more severe course in children than that observed in the 1980s. In addition, the data on morbidity among children regarding rotavirus infection and ARVI were found to be similar.  相似文献

BackgroundChildhood diarrhoea, a major cause of morbidity and mortality in low-income regions, remains scarcely studied in many countries, such as Guinea-Bissau. Stool sample drying enables later qPCR analyses of pathogens without concern about electricity shortages.MethodsDried stool samples of children under five years treated at the Bandim Health Centre in Bissau, Guinea-Bissau were screened by qPCR for nine enteric bacteria, five viruses, and four parasites. The findings of children having and not having diarrhoea were compared in age groups 0–11 and 12–59 months.ResultsOf the 429 children– 228 with and 201 without diarrhoea– 96.9% and 93.5% had bacterial, 62.7% and 44.3% viral, and 52.6% and 48.3% parasitic pathogen findings, respectively. Enteroaggregarive Escherichia coli (EAEC; 60.5% versus 66.7%), enteropathogenic E. coli (EPEC; 61.4% versus 62.7%), Campylobacter (53.2% versus 51.8%), and enterotoxigenic E. coli (ETEC; 54.4% versus 44.3%) were the most common bacterial pathogens. Diarrhoea was associated with enteroinvasive E. coli (EIEC)/Shigella (63.3%), ETEC (54.4%), astrovirus (75.0%), norovirus GII (72.6%) and Cryptosporidium (71.2%). The only pathogen associated with severe diarrhoea was EIEC/Shigella (p<0.001). EAEC was found more frequent among the infants, and EIEC/Shigella, Giardia duodenalis and Dientamoeba fragilis among the older children.ConclusionsStool pathogens proved common among all the children regardless of them having diarrhoea or not.  相似文献

Although limited almost exclusively to military trainees, acute respiratory disease (ARD) caused by adenovirus types 4 and 7 had been the leading cause of hospitalization in U.S. Army personnel. This decrease which resembles influenza in clinical manifestations led to hospitalization of as many as 50% of military trainees in midwinter and imposed a heavy burden on military hospitals and training programs. In studies undertaken from 1965 to 1970, live adenovirus type 4 and subsequently type 7 vaccines were found to be safe and immunogenic and to confer protection against type specific adenovirus ARD. For the past 5 yr. military trainees have been immunized with both adenovirus vaccines during periods of expected adenovirus disease. Since 1966, use of adenovirus vaccines has been monitored through the adenovirus surveillance program which yields weekly data on incidence and etiology of ARD in basic combat trainees. Since 1973, stable adenovirus vaccines have resulted in excellent control of adenovirus ARD. Potential problems with this immunization program are discussed.  相似文献

目的 了解呼吸道感染住院患儿呼吸道病毒的分布情况。方法 选取2015年7月至2016年6月呼吸道感染的住院患儿病例,抽取鼻咽分泌物,采用直接免疫荧光法检测常见的7种病毒,即呼吸道合胞病毒(RSV)、甲型流感病毒(IVA)、乙型流感病毒(IVB)、副流感病毒1型(PIV1)、副流感病毒2型(PIV2)、副流感病毒3型(PIV3)、腺病毒(IVD)。结果 共有569例样本送检,193例病毒检测阳性(33.92%),其中有3例为2种病毒的混合感染。男性共369例,女性共200例。其中RSV、PIV3、ADV的感染率居前三位。呼吸道病毒的感染率随着患儿年龄的增长逐渐下降,除新生儿外,6个月内的婴幼儿呼吸道病毒检出率最高。RSV检测阳性率自11月开始呈增高趋势,12月最高,达55.86%。PIV3的检出高峰出现在7月,达13.64%。结论 RSV是本地区儿童呼吸道感染最常见的病毒。呼吸道病毒的检出率与年龄、季节等因素有关。  相似文献

Introduction: Children under five years of age living in poor areas and with low availability of healthy food have a higher risk of undernutrition-related mortality. However, this relationship has not been well established among older adults.Objective: To analyse socioeconomic inequality trends related to undernutrition mortality in children under five years of age and adults over 60 in Colombian municipalities during 2003-2009 and 2010-2016.Materials and methods: We conducted an ecological study of trends between 2003 and 2016. The study population consisted of children under five years of age and adults over 60 residing in the Colombian municipalities during the study period. We estimated smoothed and standardized mortality rates by fitting a hierarchical Bayesian model and explored their relationship with five socioeconomic area-level variables.Results: In most of the municipalities, undernutrition-related mortality was three times higher in older adults compared to children. Moreover, the difference in the risk of undernutrition-related mortality between municipalities showed a marked reduction. Finally, the poor and less developed municipalities had higher rates of undernutrition-related mortality in children; conversely, wealthier territories had higher rates in older adults.Conclusions: Although in most of the municipalities the mortality rates due to undernutrition in children under five and older adults have decreased, their socioeconomic conditions influence in different ways the risk of mortality for these two populations so there is the need to develop age-specific strategies to close social gaps considering the structural conditions of the areas.  相似文献

A forecast of nonepidemic morbidity due to acute respiratory infections were carry out by using time series analysis. The data consisted of the weekly reports of medical patient consultation from ambulatory facilities from the whole country. A version of regression model was fitted to the data. Using this approach, we were able to detect the starting data of the epidemic under routine surveillance conditions for various age groups. It will be necessary to improve the data reporting system in order to introduce these procedures at the local health center level, as well as on the provincial level.  相似文献

目的了解冬季儿童急性下呼吸道感染病原谱及临床流行病学特征,为临床抗感染及病原检测提供依据。方法对我院2006年12月~2007年2月急性下呼吸道感染住院患儿采用一次性无菌吸痰管经鼻腔插入7~8cm,达到咽部以下负压吸取1~2ml深部鼻咽分泌液送细菌培养,并对呼吸道合胞病毒(RSV),腺病毒(ADV),A、B型流感病毒(IFV),1、2及3型副流感病毒(PIV)等7种常见呼吸道病毒抗原进行检测及运用荧光定量聚合酶链反应(PCR)技术检测标本中支原体和衣原体DNA。结果①381份下呼吸道感染儿童痰标本中细菌培养阳性81份,病毒检测阳性133份,支原体和衣原体阳性分别12份与6份,混合感染标本44份,总标本病原学检出率为50.66%(193/381)。②RSV阳性标本112份,为最重要的感染病原,连续3个月RSV检出率均在30%左右,6月龄以下儿童占61.61%,2岁以下儿童占86.61%,阳性标本中78.57%的患儿有喘息表现。③大肠埃希菌(16株)、肺炎链球菌(14株)、肺炎克雷伯菌及金黄色葡萄球菌(各10株)、卡他莫拉菌布兰汉亚种(8株)、流感嗜血杆菌和副流感嗜血杆菌(各6株)表现为主要的致病菌。结论RSV感然仍为儿童冬季急性下呼吸道感染最主要的病原,尤其在2岁以下儿童,且易表现为喘息发作。仍有40%以上感染病原未明,儿童急性下呼吸道感染病原谱需进一步完善。  相似文献
