目的:观察个体化康复运动训练联合八段锦运动对冠心病患者经皮冠状动脉介入术(PCI)术后心功能、心境状态和生活质量的影响。方法:选取2017年9月-2018年9月期间来我院接受治疗的100例冠心病PCI术后患者,根据随机数字表法分为对照组和研究组,各50例。对照组患者在常规治疗的基础上接受个体化康复运动训练,研究组患者在对照组的基础上联合八段锦运动。观察两组心功能、生活质量和心境状态变化情况。统计两组6个月内心血管不良事件发生率。结果:干预后,两组患者6 min步行试验(6MWT)距离、左心室射血分数(LVEF)均较干预前升高,且研究组的变化程度优于对照组(P<0.05)。干预后,两组患者紧张-焦虑、抑郁-沮丧、愤怒-敌意、疲乏-迟钝、迷惑-混乱评分均较干预前下降,精力-活力、与自我有关的情绪评分较干预前升高,且研究组的变化程度优于对照组(P<0.05)。研究组6个月内的心血管不良事件发生率低于对照组,但是两组组间对比无统计学差异(P>0.05)。干预后,两组总体/精神健康、精力、情感/生理职能、躯体疼痛、生理/社会功能各维度评分升高,且研究组较对照组高(P<0.05)。结论:八段锦运动联合个体化康复运动训练可促进冠心病PCI术后患者心功能、生活质量和心境状态改善,同时还可控制心血管不良事件发生风险。  相似文献

Under resting conditions, the failing heart shifts fuel use toward greater glucose and lower free fatty acid (FFA) oxidation. We hypothesized that chronic metabolic abnormalities in patients with dilated cardiomyopathy (DCM) are associated with the absence of the normal increase in myocardial glucose uptake and maintenance of cardiac mechanical efficiency in response to pacing stress. In 10 DCM patients and 6 control subjects, we measured coronary flow by intravascular ultrasonometry and sampled arterial and coronary sinus blood. Myocardial metabolism was determined at baseline, during atrial pacing at 130 beats/min, and at 15 min of recovery by infusion of [(3)H]oleate and [(13)C]lactate and measurement of transmyocardial arteriovenous differences of oxygen and metabolites. At baseline, DCM patients showed depressed coronary flow, reduced uptake and oxidation of FFA, and preferential utilization of carbohydrates. During pacing, glucose uptake increased by 106% in control subjects but did not change from baseline in DCM patients. Lactate release increased by 122% in DCM patients but not in control subjects. Cardiac mechanical efficiency in DCM patients was not different compared with control subjects at baseline but was 34% lower during stress. Fatty acid uptake and oxidation did not change with pacing in either group. Our results show that in DCM there is preferential utilization of carbohydrates, which is associated with reduced flow and oxygen consumption at rest and an impaired ability to increase glucose uptake during stress. These metabolic abnormalities might contribute to progressive cardiac deterioration and represent a target for therapeutic strategies aimed at modulating cardiac substrate utilization.  相似文献

摘要 目的:探讨弹力带抗阻训练、中等强度有氧运动联合应用对稳定性冠心病患者心肺功能、血管内皮功能、运动耐力的影响。方法:选择2020年9月~2022年4月期间我院收治的稳定性冠心病患者70例,按照随机数字表法分为对照组(35例,中等强度有氧运动)和联合组(35例,对照组的基础上接受弹力带抗阻训练)。对比两组运动耐力、血管内皮功能指标[超声颈动脉内膜-中层厚度(IMT) 、内皮依赖性血管舒张功能(FMD)值、Crouse斑块积分]、心肺功能指标[左心室射血分数(LVEF)、心脏每搏输出量(SV)、最大摄氧量(VO2max)和氧脉搏(O2pulse)]、生活质量和心血管不良事件发生率。结果:干预后,两组6分钟步行试验(6MWT)升高,且联合组高于对照组(P<0.05)。干预后,两组LVEF、SV、VO2max、O2pulse升高,且联合组高于对照组(P<0.05)。干预后,两组生理/社会功能、躯体疼痛、生理/情感职能、精力、总体/精神健康各维度评分升高,且联合组高于对照组(P<0.05)。干预后,两组IMT缩小,FMD升高,Crouse斑块积分下降,且联合组的变化程度大于对照组(P<0.05)。两组心血管不良事件发生率对比无差异(P>0.05)。结论:稳定性冠心病患者采用弹力带抗阻训练和中等强度有氧运动联合干预可改善心肺功能和血管内皮功能,提高运动耐力及生活质量,具有较好的临床应用价值。  相似文献

本研究旨在探讨两种不同形式的间歇性低压低氧(intermittent hypobaric hypoxia,IHH)对发育大鼠心脏缺血,再灌注损伤的影响。雄性Sprague-Dawley(SD)新生大鼠72只,随机分为三组:对照组、IHH3000in组(IHH3000)、IHH5000m组(IHH5000)。低氧组大鼠出生后立即于低压氧舱分别接受28d、42d和56d(海拔5000m、每天6h:海拔3000m、每天5h)的低压低氧处理。应用Langendorff离体心脏灌流技术,给予心脏缺血(停灌30min)/再灌注(复灌60min)处理,分别在缺血前5min及复灌后l、5、10、20、30、60min记录心功能和冠状动脉流量变化,并测定乳酸脱氢酶(1actate dehydrogenase,LDH)活性。实验结束时测定心脏重量。结果显示:(1)IHH3000组大鼠体重增长与对照组无明显差异;IHH5000组大鼠体重增长明显慢于对照组及IHH3000组大鼠(P〈0.01)。(2)IHH3000组人鼠表现明显的心脏保护效应。与对照组相比较,在心脏停灌,再灌注60min时,心功能(LVDP、±LVdp/drmax)恢复增强(P〈0.05)、LDH活性降低(P〈0.05)、冠状动脉流量增多(P〈0.05);心脏重量与对照组大鼠无差异;IHH42d处理的大鼠心功能恢复明显好于IHH28d处理的大鼠(P〈0.05)。(3)IHH5000组大鼠表现出明显的心脏损伤效应,各项心功能指标(LVDP、±LVdp/dtmax)的恢复均低于对照组(P〈0.05),复灌过程中LDH活性明显高于相应对照组(P〈0.05),右心室重量明显高于对照组大鼠(P〈0.05)。结果表明,适当的IHH增强发育大鼠心脏对缺血,再灌注损伤的抵抗能力;间歇性低氧方式是影响其心脏保护作用的重要因素。  相似文献

Cardiac contractile function is dependent on the integrity and function of the sarcolemmal membrane. Swimming exercise training is known to increase cardiac contractile performance. The purpose of the present study was to examine whether a swimming exercise program would alter sarcolemmal enzyme activity, ion flux, and composition in rat hearts. After approximately 11 wk of exercise training, cardiac myosin and actomyosin Ca2+-adenosinetriphosphatase (ATPase) activity was significantly higher in exercised rat hearts than in sedentary control rat hearts. Glycogen content was increased in plantaris and gastrocnemius muscles from exercised animals as was succinic dehydrogenase activity in gastrocnemius muscle of exercised rats in comparison to sedentary rat preparations. Sarcolemmal vesicles were isolated from hearts of exercise-trained and control rats. Sarcolemmal Na+-K+-ATPase and K+-p-nitrophenylphosphatase activities, Na+-Ca2+ exchange, and passive Ca2+ binding did not differ between the two groups. ATP-dependent Ca2+ uptake and 5'-nucleotidase activity were elevated in the cardiac sarcolemmal vesicles isolated from exercised animals compared with sedentary control rats. Sarcolemmal phospholipid composition was not altered by the exercise training. Our results demonstrate that swimming training in rats does not affect most parameters of cardiac sarcolemmal function or composition. However, the elevated sarcolemmal Ca2+ pump activity in exercised rats may help to reduce intracellular Ca2+ and augment cardiac relaxation rates. The enhanced 5'-nucleotidase activity may stimulate adenosine production, which could affect myocardial blood flow. The present results further our knowledge on the subcellular response of the heart to swimming training in the rat.  相似文献

摘要 目的:探讨辅酶Q10联合替格瑞洛治疗冠心病不稳定型心绞痛疗效及对冠脉血流参数的影响。方法:选取我院2020年10月到2022年10月收治的128例冠心病不稳定型心绞痛患者作为研究对象,分为观察组与对照组,每组64例,对照组患者采取常规西医治疗,观察组在对照组基础上增加辅酶Q10联合替格瑞洛治疗,对比两组患者临床治疗效果,并应用视觉模拟疼痛量表(VAS)评定两组患者心绞痛疼痛情况,并对比其每日发作频率、每次持续时间、每日硝酸甘油用量,分别对两组患者治疗前与治疗后的心功能指标和冠脉血流参数。结果:观察组治疗总有效率高于对照组(P<0.05);观察组疼痛评分、每日发作频率、每次持续时间、每日硝酸甘油用量均明显低于对照组(P<0.05);治疗前两组患者6分钟步行距离、LVEF、BNP对比无差异(P>0.05),治疗后两组患者6分钟步行距离、LVEF均升高,且观察组较对照组高,BNP水平降低,观察组低于对照组(P<0.05);治疗前两组患者舒张期时间速度积分、左前降支收缩期峰值流速、左前降支舒张期峰值流速、冠脉血流速度储备值对比无明显差异(P>0.05),治疗后两组患者舒张期时间速度积分、左前降支收缩期峰值流速、左前降支舒张期峰值流速、冠脉血流速度储备值均升高,且观察组高于对照组(P<0.05)。结论:辅酶Q10联合替格瑞洛治疗冠心病不稳定型心绞痛疗效显著,可减少患者心绞痛疼痛程度,减少发作频率、持续时间和硝酸甘油用量,提升患者心功能水平,改善冠脉血流参数,值得临床应用推广。  相似文献

摘要 目的:探讨益心舒片联合美托洛尔对冠心病心绞痛患者心功能、血液流变学和血管内皮功能的影响。方法:纳入南京市中心医院2020年7月~2022年8月期间收治的冠心病心绞痛患者126例。采用随机数字表法分为对照组(n=63,接受美托洛尔治疗)和研究组(n=63,对照组基础上接受益心舒片治疗)。对比两组临床症状、心功能[左心室射血分数(LVEF)、心输出量(CO)、左室舒张末期内径(LVEDD)]、血液流变学[血浆黏度、全血高切黏度、全血低切黏度]和血管内皮功能[一氧化氮(NO)、内皮素-1(ET-1)]变化情况,同时观察两组不良反应发生率。结果:两组治疗12周后心绞痛发作次数减少,心绞痛持续时间缩短,研究组的改善幅度大于对照组(P<0.05)。两组治疗12周后CO、LVEF升高,LVEDD缩小,研究组的改善幅度大于对照组(P<0.05)。两组治疗12周后血浆黏度、全血高切黏度、全血低切黏度下降,研究组的改善幅度大于对照组(P<0.05)。两组治疗12周后ET-1下降,NO升高,研究组的改善幅度大于对照组(P<0.05)。对照组(4.76%)、研究组(6.35%)的不良反应发生率组间对比未见差异(P>0.05)。结论:益心舒片联合美托洛尔治疗冠心病心绞痛患者,可有效改善患者的血液流变学、心功能和血管内皮功能。  相似文献

目的:探讨有创-无创序贯机械通气对急性心源性肺水肿患者低氧血症及血流动力学的影响。方法:按照随机数字表法将2013年3月至2015年3月我院收治的49例急性心源性肺水肿患者分为两组,对照组行常规抗心衰治疗联合有创机械通气,观察组接受对照组治疗方案后,再给予无创鼻面罩双水平气道正压通气。比较两组患者治疗前后的动脉血气分析指标及血流动力学指标,以及有创机械通气时间、机械通气总时间、呼吸机相关肺炎(VAP)发生率、再发心衰率及死亡例数。结果:两组患者治疗过程中未出现死亡病例,治疗后,两组患者动脉血气分析指标、血流动力学指标较治疗前不同程度的改善,差异有统计学意义(P0.05),两组患者治疗后的动脉血气分析指标、血流动力学指标比较,差异无统计学意义(P0.05)。观察组患者有创通气时间、通气总时间少于对照组,VAP发生率低于对照组,差异有统计学意义(P0.05),两组患者再发心衰率差异无统计学意义(P0.05)。结论:有创-无创序贯机械通气能有效改善急性心源性肺水肿患者的低氧血症和血流动力学指标,安全有效,作为抢救急性心源性肺水肿的措施具有重要临床价值。  相似文献

目的:研究有氧运动联合破壁蛋白核小球藻对高脂膳食大鼠脂代谢某些指标的影响。方法:55只雄性Wistar大鼠经过适应性饲养4 d后进行3 d、20 min/d的无负重游泳训练,筛选淘汰5只不适应游泳训练的大鼠后,按体重以数字随机分组法分为5组:普通膳食+安静对照组(C组)、高脂膳食+安静对照组(H组)、高脂膳食+小球藻+安静对照组(HC组)、高脂膳食+有氧运动组(HM组)、高脂膳食+有氧运动+小球藻组(HMC),每组10只。HM组和HMC组进行6周每周6次60 min/d的无负重游泳训练。C组大鼠以普通饲料常规喂养;其余各组以高脂饲料喂养;HC组和HMC组大鼠灌胃破壁小球藻1次,灌胃剂量为3.9 g/(kg·d),灌胃体积为5 ml/kg,其余各组ig等量生理盐水。6周后,测定大鼠Lee’s指数,取血、肝脏检测相关生化指标。结果:与C组比较,H组Lee’s指数,血清FFA、IL-6、TNF-α、TC、TG、LDL-c,肝脏FFA、IL-10显著升高(P<0.01);血清HDL-c水平显著降低(P< 0.01)。与H组比较,HC、HM、HMC组Lee’s指数,血清FFA、IL-6、TNF-α、TC、TG、LDL-c,肝脏FFA、IL-10显著降低(P<0.05或P<0.01);血清HDL-c水平显著升高(P<0.05或P<0.01)。与HC、HM组比较,HMC组Lee’s指数,血清FFA、IL-6、TNF-α、TC、TG、LDL-c,肝脏FFA、IL-10显著降低(P<0.05);血清HDL-c水平显著升高(P<0.05)。结论:有氧运动和破壁蛋白核小球藻干预能够不同程度改善高脂膳食大鼠脂代谢,降低肥胖对肝脏造成的脂毒性。其中联合干预较单一干预更为有效。  相似文献

目的: 研究有氧运动联合黑果枸杞对高脂膳食大鼠心肌脂代谢某些指标的影响。方法: 55只雄性Wistar大鼠经过适应性饲养4 d后进行20 min/d的无负重游泳训练,连续3 d,筛选淘汰5只不适应游泳训练的大鼠后,按体重以数字随机分组法分为5组:普通膳食+安静组(RDC组)、高脂膳食+安静组(HDC组)、高脂膳食+黑果枸杞+安静组(HDLC组)、高脂膳食+有氧运动组(HDM组)、高脂膳食+黑果枸杞+有氧运动组(HDLM),每组10只。HDM组和HDLM组进行6周每周6次60 min/d的无负重游泳训练。RDC组大鼠以普通饲料常规喂养;其余各组以高脂饲料喂养;HDLC组和HDLM组大鼠灌胃黑果枸杞,灌胃剂量为4.48 g/(kg·d),灌胃体积为5 ml/kg,其余各组灌胃等量蒸馏水。6周后,测定大鼠Lee’s指数,取血、心肌检测相关生化指标。结果: 与RDC组比较,HDC组Lee’s指数,血清FFA、TNF-α、IL-6、TC、TG、LDL-C,心肌FFA、ICAM-1显著升高(P<0.01);血清HDL-C水平显著降低(P<0.01)。与HDC组比较,HDLC、HDM、HDLM组Lee’s指数,血清FFA、TNF-α、IL-6、TC、TG、LDL-C,心肌FFA、ICAM-1显著降低(P<0.05或P<0.01);血清HDL-C水平显著升高(P<0.05或P<0.01)。与HDLC、HDM组比较,HDLM组Lee’s指数,血清FFA、TNF-α、IL-6、TC、TG、LDL-C,心肌FFA、ICAM-1显著降低(P<0.05);血清HDL-C水平显著升高(P<0.05)。结论: 有氧运动和/或黑果枸杞干预能够不同程度改善高脂膳食大鼠脂代谢,降低肥胖引发的脂毒性。其中联合干预较单一干预更为有效。  相似文献

目的:研究木犀草素是否能改善心脏停搏保存液(UW液)对离体大鼠心脏的低温保存效果。方法:将40只成年SD大鼠随机分成4组(n=10):对照组(UW组)、7.5μmol/L木犀草素小剂量组,15μmol/L木犀草素中剂量组及30μmol/L木犀草素大剂量组。利用Langendorff离体心脏灌流法,观察心脏在4℃含或不含木犀草素的UW液中保存12 h复灌60 min后心脏功能及超微结构变化,比较心脏冠脉流量(CF)、心肌含水量及冠脉流出液中磷酸肌酸激酶(CK)的释放量。结果:与对照组比较,添加木犀草素后,复灌期心脏的收缩功能(LVPSP,+dp/dtmax)与心脏舒张功能(-dp/dtmax)、冠脉流量在多个复灌时间点均优于对照组,心率在复灌60 min时也显著优于对照组;复灌过程中磷酸肌酸激酶的漏出量及低温保存后心脏超微结构的损伤也均明显低于对照组;随灌注时间延长木犀草素组心脏结构和功能的改善有剂量依赖性趋势;木犀草素对心肌含水量没有影响。结论:木犀草素能显著改善UW液对离体大鼠心脏的低温保存效果,对心脏有明显的保护作用,以30μmol/L的木犀草素大剂量组作用最显著。  相似文献

To determine whether a prior chronic swimming program would alter the heart's response to chronic hypertension, female rats were made to swim for 10 wk, and then the left renal artery was stenosed. Heart perfusions were performed 10 wk later. The five groups studied were: control (C), normotensive swimmers (Sw), sedentary hypertensives (H), swimming rats made hypertensive and then allowed to be sedentary (Sw-H-Sd); and swimming animals made hypertensive and continued in a swimming program (Sw-H-Sw). Total heart and left ventricular weights were increased in increasing degrees in the sequence Sw, H, Sw-H-Sd, and Sw-H-Sw. Right ventricular weight was only increased in Sw and Sw-H-Sw. Swimming before the onset of hypertension enhanced total cardiac output and stroke work. Ejection fractions and mean velocity of circumferential fiber shortening (Vcf) were increased in Sw-H-Sd or Sw-H-Sw vs. controls. Myocardial O2 extraction was increased and coronary flow and myocardial O2 consumption were diminished in all hypertensive groups. However, lactate production was similar in all groups. Myosin adenosinetriphosphatase activity was increased in Sw but decreased in the three H groups. The percent of V1 myosin isozyme was greater and the percent of V3 less in Sw than in C; V1 was diminished and V3 increased in H and Sw-H-Sd; isozymes were normal in Sw-H-Sw.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

To evaluate the combined effects of cardiac overload imposed by hypertension and chronic swim training on coronary vascularity, female rats were made hypertensive by unilateral renal artery stenoses and were exercised in an 8- to 10-wk swimming program. Maximal coronary flow was assessed in isolated retrograde buffer-perfused hearts under conditions of minimal coronary resistance (15 microM adenosine or anoxia). Sedentary normotensive animals, sedentary hypertensive animals, and normotensive animals exposed to a swimming program were also studied. Swimming was associated with an 18% increase in heart weight and with increases in both absolute (ml/min) and relative (ml X g-1 X min-1) maximal coronary flow. Hypertension was associated with a 32% increase in heart weight but with a decrease in absolute and relative coronary flow compared with controls. The combined stimuli resulted in a 63% myocardial hypertrophy and a 19% increase in absolute flow. Relative coronary flow (g tissue-1) was similar in hearts from hypertensive sedentary animals and hypertensive swimmers. These data indicate that the coronary vascular deficit that accompanies the cardiac hypertrophy of hypertension is not worsened by the superimposition of an exercise load that exaggerates the hypertrophy.  相似文献

目的 :研究运动对高血压肥大心脏心肌初级和次级应答基因 (immediateearlygeneandlateresponsegene)表达的影响。方法 :采用Northern分子杂交方法对游泳运动 10周后自发性高血压大鼠 (spontaneouslyhypertensiverats ,SHR)心肌初级应答基因c fosmRNA和次级应答基因心钠素 (atrialnatriureticfactor ,ANF)mRNA的表达进行比较研究。结果 :游泳SHR收缩压和舒张压分别比安静SHR降低 2 2 %和 2 5 % (P <0 .0 1) ,但左心室重 /体重比值两组间无明显差异 (P >0 .0 5 )。SHR最后一次游泳 2 4h后 ,心肌c fosmRNA表达与安静SHR相比无明显差异 ,但两组大鼠比SHR的正常血压对照鼠WistarKyoto(WKY)分别提高 83 %和 80 %。游泳SHR心肌ANFmRNA表达比安静SHR降低 3 2 % ,但仍比WKY大鼠高 2 9%。结论 :SHR经过游泳运动后 ,出现心室肌ANF基因表达降低与c fos基因表达增强的不一致现象可能是运动改善高血压肥大心脏的分子机制之一。  相似文献

The effects of training were investigated in male Sprague Dawley rats group (N), fed ad libitum, by measuring the weight increase and food intake of the animals, biochemical parameters (myocardial triglycerides and glycogen levels), mechanical and metabolic properties of the heart, and adrenergic reactions to swimming stress. An other group of rats remaining sedentary served as control (T). Conditioned rats had been submitted to a one hour test swim 6 days a week for 9 weeks. Gradually additional weights were fixed to the thorax. At the end of training, the load reached 6% of the body weight. Both groups were sacrified by decapitation at the same time, thirty hours after the last swimming session. The study of mechanical performances and metabolic properties was achieved with isolated working heart preparation. Adrenergic reactions of swimming stress were evaluated from plasma, heart and adrenal catecholamine concentrations.  相似文献

目的:探讨高密度脂蛋白胆固醇(HDL-C)及其亚型与冠心病患者预后的关系。方法:选取2012年6月~2015年12月在我院检查的371例疑似冠心病患者为研究对象,其中确诊为冠心病患者274例为观察组,冠状动脉检查正常者97例为对照组。对两组患者的血脂指标进行检测,同时分析各终点事件与血脂各指标之间的相关性,并采用COX回归分析探讨HDL-C及其亚型与冠心病患者预后的关系。结果:与正常组比较,冠心病患者总胆固醇、低密度脂蛋白胆固醇等指标显著升高,而高密度脂蛋白胆固醇及其亚型HDL2-C显著降低。这种改变随冠状动脉病变支数的增加而更加显著。COX回归分析显示HDL2-C亚型与患者心源性死亡的发生风险存在一定的相关性。结论:HDL2-C亚型与冠心病的发生、发展,患者病变程度存在显著的相关性,可以用作对冠心病患者病情程度和终点事件预测的重要参考指标。  相似文献

目的:探讨麝香保心丸联合丹参多酚酸盐治疗冠心病心绞痛的临床疗效。方法:选择2014年5月-2016年5月我院收治的冠心病心绞痛患者94例,根据治疗方法不同分为研究组和对照组,每组47例。对照组患者给予丹参多酚酸盐注射液治疗,研究组患者在对照组基础上给予麝香保心丸口服治疗。观察并比较两组患者治疗前后心功能指标及血清同型半胱氨酸(Hcy)及C反应蛋白(CRP)水平。结果:与治疗前比较,两组患者治疗后Pro-BNP,LVEDD及E/A均降低,而LVEF均升高,差异具有统计学意义(P0.05);与对照组比较,研究组治疗后Pro-BNP,LVEDD及E/A较低,而LVEF较高,差异具有统计学意义(P0.05)。与治疗前比较,两组患者治疗后血清Hcy及CRP水平均升高,差异具有统计学意义(P0.05);与对照组比较,研究组患者治疗后血清Hcy及CRP水平较高,差异具有统计学意义(P0.05)。研究组患者临床总有效率显著高于对照组,差异具有统计学意义(P0.05)。结论:麝香保心丸联合丹参多酚酸盐治疗冠心病心绞痛具有显著的临床疗效,可以改善患者心功能,促进冠状动脉血流畅通,值得临床推广应用。  相似文献

目的:探讨不同动脉途径行经皮冠状动脉介入治疗高龄冠心病的临床疗效和安全性。方法:选取2014年1月~2017年1月我院收治的265例高龄冠心病患者为研究对象,根据就诊顺序将受试者分为对照组132例及研究组133例,对照组患者给予股动脉途径(TFI)行经皮冠状动脉介入治疗,研究组患者给予桡动脉途径(TRI)行经皮冠状动脉介入治疗,比较两组患者的手术情况、手术前后各心功能指标变化、心血管不良事件及并发症的发生情况。结果:两组患者的手术成功率、支架数量、造影剂用量比较差异无统计学意义(P0.05),但研究组患者动脉穿刺时间、导管插入时间及X线曝光时间均长于对照组,卧床时间及住院时间均明显短于对照组(P0.05)。治疗后,两组患者的左心射血分数(LVEF)、左室收缩末期内径(LVESD)及左室舒张末期内径(LVEDD)较治疗前均明显改善,且研究组优于对照组(P0.05),研究组患者心血管不良事件发生率及各并发症发生率均显著低于对照组(P0.05)。结论:TRI与TFI在经皮冠状动脉介入术治疗高龄冠心病患者的手术效果相当,但TRI在改善患者心功能、减少心血管不良事件的效果更好且安全性更高,可做为高龄冠心病患者PCI治疗的首选途径。  相似文献

Metabolic disturbances in diabetic cardiomyopathy   总被引:24,自引:0,他引:24  
It has been established that diabetes results in a cardiomyopathy, and increasing evidence suggests that an altered substrate supply and utilization by cardiac myocytes could be the primary injury in the pathogenesis of this specific heart muscle disease. For example, in diabetes, glucose utilization is insignificant, and energy production is shifted almost exclusively towards -oxidation of free fatty acids (FFA). FFA's are supplied to cardiac cells from two sources: lipolysis of endogenous cardiac triglyceride (TG) stores, or from exogenous sources in the blood (as free acid bound to albumin or as TG in lipoproteins). The approximate contribution of FFA from exogenous or endogenous sources towards -oxidation in the diabetic heart is unknown. In an insulin-deficient state, adipose tissue lipolysis is enhanced, resulting in an elevated circulating FFA. In addition, hydrolysis of the augmented myocardial TG stores could also lead to high tissue FFA. Whatever the source of FFA, their increased utilization may have deleterious effects on myocardial function and includes the abnormally high oxygen requirement during FFA metabolism, the intracellular accumulation of potentially toxic intermediates of FFA, a FFA-induced inhibition of glucose oxidation, and severe morphological changes. Therapies that target these metabolic aberrations in the heart during the early stages of diabetes could potentially delay or impede the progression of more permanent sequelae that could ensue from otherwise uncontrolled derangements in cardiac metabolism.  相似文献

目的:比较不同治疗方式对冠心病合并心脏瓣膜炎的临床指标、生活质量及血清炎性反应的影响。方法:选取2014年2月~2016年6月期间于本院进行治疗的64例冠心病合并心脏瓣膜炎患者为研究对象,将其随机分为对照组和观察组每组各32例,对照组进行常规的药物治疗,观察组则进行外科手术治疗,然后将两组患者治疗前后的心脏功能指标、生活质量及血清炎性指标进行比较。结果:治疗前两组患者的心脏功能指标、生活质量及血清炎性指标比较,P均0.05,而治疗后观察组的心脏功能指标、生活质量及血清炎性指标均显著地好于对照组,P均0.05,均有显著性差异。结论:外科手术对冠心病合并心脏瓣膜炎患者血清炎症因子水平及生活质量具有积极的改善作用,适于临床进一步推广应用。  相似文献
