目的探讨在B超或CT引导下经皮穿刺组织间植入125I放射性粒子治疗中晚期肝癌的护理方法。方法对2015年1月~2016年12月在我院行B超或CT引导下125I放射性粒子植入治疗的50例患者实施全程关护护理模式,包括术前评估及干预、术中护理、术后护理、营养干预、出院指导。结果 50例患者对放射防护相关知识完全知晓,均能顺利配合治疗及护理,护理满意度达98.0%;本组患者中主要并发症为穿刺部位轻度疼痛6例,穿刺口少量渗血3例。结论 B超或CT引导下经皮穿刺组织间植入125I放射性粒子治疗中晚期肝癌是一种安全、有效的微创介入法,并发症轻、发生率低,通过全程关护护理干预可提高患者对放射性粒子植入防护相关知识知晓率,提高防护意识,从而提高患者满意度及生活质量。  相似文献

目的:探讨老年颈内动脉系统急性脑梗死动脉溶栓的疗效。方法:对我院老年科收治的63例老年颈内动脉系统急性脑梗死随机分为股动脉组以及颈动脉组,其中股动脉组患者30例,对其采用经皮股动脉穿刺微导管介入尿激酶溶栓治疗,颈动脉组患者33例,对其采用经皮患侧颈动脉穿刺尿激酶溶栓治疗。结果:对两组患者治疗前、术后2h以及术后30d采用NIHSS评分比较,两组患者NIHSS评分在术后2h及术后30d均较术前有明显改善(P<0.05);对两组患者治疗相关情况比较,股动脉组患者术后2h血管再通率明显高于颈动脉组(P<0.05),两组颅内出血率未显示出统计学差异性;对两组患者术后30d牛津残障OHS评分、BI指数比较,股动脉组患者明显优于颈动脉组(P<0.05)。结论:对老年急性脑梗死患者采用经皮股动脉穿刺微导管介入或采用经皮患侧颈动脉穿刺尿激酶溶栓治疗,均可有效改善患者预后,提高患者的临床疗效,两组相比较,股动脉组更能显示出优良的治疗率。  相似文献

目的:评价介入技术治疗急性和亚急性门静脉(portal vein,PV)及肠系膜上静脉(superior mesenterie vein, SMV)血栓形成的临床疗效。方法:对28例治疗急性和亚急性(发病一周至一月)的PV及SMV血栓患者进行介入治疗。按介入治疗途径不同分为下述两组:选择经颈静脉穿刺门静脉(transjugular intrahepatic portosystemic shunt , TIPS途径)置管溶栓(19例)和经皮穿肝内门脉(percutaneous transhepatic)置管溶栓治疗(9例)。结果:所有患者随访一周至三个月,其中治疗成功24例,临床症状明显改善,无严重并发症。经TIPS途径治疗的患者组中,16例随访显示大部分血栓被清除,门静脉系统有血流通过,临床症状缓解。3例SMV及PV恢复部分血流,但临床症状无明显改善。经皮穿肝内门脉直接置管溶栓治疗组中,6例患者的PV及SMV内血栓大部分清除,血流基本改善,2例患者PV及SMV血流部分好转,临床症状无明显改善,严重并发症1例(术后两天死于腹腔出血)。结论:经TIPS途径介入技术和经皮穿肝内门脉直接置管溶栓治疗是治疗急性和亚急性PV及SMV血栓形成的有效方法,前者的疗效及安全性均好于后者。  相似文献

目前临床普遍采用功能与分子影像检测手段能来评价头颈部肿瘤的放射治疗计划和疗效,可指导个体化治疗从而提高疗效。文章概述了功能与分子影像技术CT,MRI,PET-CT,超声检测技术在头颈部肿瘤放射治疗计划制定和疗效评价中的应用进展。结果显示,不同分子影像检测方法如在检查时机的选择、诊断和鉴别诊断的价值、观察放射治疗后肿瘤的残存和复发、预测放射治疗效果、指导后续治疗等方面均可起到重要作用。采用图像融合技术进行联合应用,如PET-CT和MRI-CT等,可提高检测的准确率。临床医生需在常规影像学手段的基础上,根据头颈部肿瘤患者病情和治疗方法的不同选用正确的功能和分子影像检测手段,更好地指导制定放射治疗计划及综合评价放射治疗后的疗效。  相似文献

肝包虫病是畜牧地区常见的寄生虫病,是一种人畜共患病,非牧地区偶尔也可以见到,可以通过与狗等动物密切接触直接感染,还可以通过呼吸道吸入虫卵或经消化道误食含有虫卵感染的食物或水源等方式感染使人患病,并且可以寄生于人体内各个部位,但以肝脏最为多见,严重危害人体健康和畜牧业的发展。加强肝包虫病的诊断是防控本疾病的重要措施之一,在不能排除肝包虫病的情况下,应结合病史、化验室检查、B超、CT等影像学检查、免疫学检查进行综合分析,给出明确诊断,以防止误诊、漏诊。近年来随着分子生物学以及病原学等技术的发展,为包虫病的诊断提供更多的方法和信息,本文将从肝包虫病的病史及临床表现、影像学检查、免疫学检查、分子生物学检查及病原学检查等方面对其诊断现状及进展做一综述。  相似文献

Hepatocellular carcinoma (HCC) is the fifth most common malignant tumor and constitutes a major health threat globally. Intermediate HCC (Barcelona Clinic Liver Cancer Staging, stage B) encompasses a wide range of patients and is characterized by substantial heterogeneity with varying tumor burdens and liver functions. Therefore, it is paramount to evaluate the patient's overall conditions and to select the most appropriate therapy based on available evidence. Transarterial chemoembolization is the recommended first-line therapy for intermediate HCC patients. However, in clinical practice, other treatment options are also used as alternative therapies, such as hepatic resection, percutaneous thermal ablation, radiotherapy (RT), systemic treatment, immunotherapy, and so forth. In this review, we will introduce current treatment strategies for intermediate HCC, discuss their advantages and disadvantages, and propose future directions.  相似文献

Echinococcus granulosus, an intestinal tapeworm of dogs and other canids, infects humans in its larval stage and causes human echinococcosis or hydatid disease. In the Republic of Korea, 31 parasite-proven human echinococcosis cases have been reported, most of which were imported from the Middle East. We recently examined a 61-year-old Korean man who had a large cystic mass in his liver. ELISA was negative for tissue parasitic infections, including echinococcosis, cysticercosis, paragonimiasis, and sparganosis. The patient underwent surgery to remove the cyst, and the resected cyst was processed histopathologically for microscopic examinations. In sectioned cyst tissue, necrotizing protoscolices with disintegrated hooklets of E. granulosus were found. In some areas, only freed, fragmented hooklets were detected. The patient had traveled to western and central Europe in 1996, and had no other history of overseas travel. We report our patient as a hepatic echinococcosis case which was probably imported from Europe.  相似文献

Liver cancer is the sixth most common tumor in the world and the majority of patients with this disease usually die within 1 year. The effective treatment for end‐stage liver disease (also known as liver failure), including liver cancer or cirrhosis, is liver transplantation. However, there is a severe shortage of liver donors worldwide, which is the major handicap for the treatment of patients with liver failure. Scarcity of liver donors underscores the urgent need of using stem cell therapy to the end‐stage liver disease. Notably, hepatocytes have recently been generated from hepatic and extra‐hepatic stem cells. We have obtained mature and functional hepatocytes from rat hepatic stem cells. Here, we review the advancements on hepatic differentiation from various stem cells, including hepatic stem cells, embryonic stem cells, the induced pluripotent stem cells, hematopoietic stem cells, mesenchymal stem cells, and probably spermatogonial stem cells. The advantages, disadvantages, and concerns on differentiation of these stem cells into hepatic cells are highlighted. We further address the methodologies, phenotypes, and functional characterization on the differentiation of numerous stem cells into hepatic cells. Differentiation of stem cells into mature and functional hepatocytes, especially from an extra‐hepatic stem cell source, would circumvent the scarcity of liver donors and human hepatocytes, and most importantly it would offer an ideal and promising source of hepatocytes for cell therapy and tissue engineering in treating liver disease. BackgroundEchinococcosis is a zoonotic parasitic disease caused by larval stages of cestodes belonging to the genus Echinococcus. The infection affects people’s health and safety as well as agropastoral sector. In China, human echinococcosis is a major public health burden, especially in western China. Echinococcosis affects people health as well as agricultural and pastoral economy. Therefore, it is important to understand the prevalence status and spatial distribution of human echinococcosis in order to advance our knowledge of basic information for prevention and control measures reinforcement.MethodsReport data on echinococcosis were collected in 370 counties in China in 2018 and were used to assess prevalence and spatial distribution. SPSS 21.0 was used to obtain the prevalence rate for CE and AE. For statistical analyses and mapping, all data were processed using SPSS 21.0 and ArcGIS 10.4, respectively. Chi-square test and Exact probability method were used to assess spatial autocorrelation and spatial clustering.ResultsA total of 47,278 cases of echinococcosis were recorded in 2018 in 370 endemic counties in China. The prevalence rate of human echinococcosis was 10.57 per 10,000. Analysis of the disease prevalence showed obvious spatial positive autocorrelation in globle spatial autocorrelation with two aggregation modes in local spatial autocorrelation, namely high-high and low-high aggregation areas. The high-high gathering areas were mainly concentrated in northern Tibet, western Qinghai, and Ganzi in the Tibetan Autonomous Region and in Sichuan. The low-high clusters were concentrated in Gamba, Kangma and Yadong counties of Tibet. In addition, spatial scanning analysis revealed two spatial clusters. One type of spatial clusters included 71 counties in Tibet Autonomous Region, 22 counties in Qinghai, 11 counties in Sichuan, three counties in Xinjiang Uygur Autonomous Region, two counties in Yunnan, and one county in Gansu. In the second category, six types of spatial clusters were observed in the counties of Xinjiang Uygur Autonomous Region, and the Qinghai, Gansu, and Sichuan Provinces.ConclusionThis study showed a serious prevalence of human echinococcosis with obvious spatial aggregation of the disease prevalence in China. The Qinghai-Tibet Plateau is the "hot spot" area of human echinococcosis in China. Findings from this study indicate that there is an urgent need of joint strategies to strengthen efforts for the prevention and control of echinococcosis in China, especially in the Qinghai-Tibet Plateau.  相似文献

The lack of knowledge regarding polycystic hydatid disease results in delayed or even incorrect diagnosis. The lack of systematic information regarding treatment also makes it difficult to assess the results and prognosis in patients with peritoneal and hepatic lesions caused by Echinococcus vogeli. Here we describe the clinical features of patients, propose a radiological classification protocol and describe a therapeutic option for the treatment of hydatid disease that previously had only been used for cases of cystic echinococcosis (Echinococcus granulosus). A prospective cohort study was initiated in 1999 and by 2009 the study included 60 patients. These patients were classified according to the PNM classification (parasite lesion, neighbouring organ invasion and metastases) and placed in one of three therapeutic modalities: (i) chemotherapy with albendazole at a dose of 10 mg/kg/day, (ii) surgical removal of cysts or (iii) percutaneous puncture of the cysts via puncture, aspiration, injection and re-aspiration (PAIR). The results were stratified according to therapeutic outcome: "cure", "clinical improvement", "no improvement", "death" or "no information". The PNM classification was useful in indicating the appropriate therapy in cases of polycystic hydatid disease. In conclusion, surgical therapy produced the best clinical results of all the therapies studied based on "cure" and "clinical improvement" outcomes. The use of PAIR for treatment requires additional study.  相似文献

"Hemobilia," upper gastrointestinal tract bleeding that originates from within the biliary tract, has become widely recognized due to an increased clinical awareness of the disorder and to improvements in diagnostic techniques. In addition, the growing use of percutaneous liver puncture for the diagnosis of and therapy for hepatobiliary diseases and the increased incidence of both blunt and penetrating hepatic trauma have contributed to a rising incidence of hemobilia. We review the history, pathophysiology, and current approaches to the diagnosis and treatment of this disease.  相似文献

BackgroundSchistosomiasis (or bilharzia), a major parasitic disease, affects more than 260 million people worldwide. In chronic cases of intestinal schistosomiasis caused by trematodes of the Schistosoma genus, hepatic fibrosis develops as a host immune response to the helminth eggs, followed by potentially lethal portal hypertension. In this study, we characterized hepatic and splenic features of a murine model of intestinal schistosomiasis using in vivo magnetic resonance imaging (MRI) and evaluated the transverse relaxation time T2 as a non-invasive imaging biomarker for monitoring hepatic fibrogenesis.Conclusions/SignificanceOur multiparametric MRI approach confirms that this murine model replicates hepatic and splenic manifestations of human intestinal schistosomiasis. Quantitative T2 mapping proved sensitive to assess liver fibrogenesis non-invasively and may therefore constitute an objective imaging biomarker for treatment monitoring in diseases involving hepatic fibrosis.  相似文献

The main goal of the paper is to clarify anthelminthic treatment as an alternative hydatic cyst therapy, its indications and contraindications. Chemotherapy constitutes a non-invasive treatment and is less limited by the patient's status than surgery or PAIR. Many investigators have employed benzoimidazole carbonates for the management of human hydatid disease. Both, albendazole and mebendazole have, a favourable effect in patients suffering from multiorgan and multicystic disease, in inoperable primary liver or lung echinococcosis, and they can also prevent secondary echinococcosis. Chemotherapy is contraindicated for large cysts that are at risk to rupture and for inactive or calcified cysts. The main adverse events are related to changes in liver enzyme levels. The best efficacy is observed with liver, lung, and peritoneal cysts. Certain various factors influence the therapeutic results of medical treatment. The vast majority of the recurring cysts show good susceptibility to re-treatment.  相似文献

BackgroundLiver abscesses caused by Candida species are mainly found in immunocompromised hosts, associated with conditions (such as neutropenia and mucositis) that facilitate the spreading of microorganisms from the gastrointestinal tract.Case reportWe present the case of a non-immunocompromised 72-year-old woman with a liver abscess caused by Candida haemulonii var. vulnera, in whom potential associated conditions could be polycystic kidney disease and renal replacement therapy. The patient experienced clinical resolution after percutaneous drainage and treatment with caspofungin.ConclusionsTo our knowledge, this is the first case reported in Peru of a liver abscess due to Candida haemulonii var. vulnera, a clinical presentation that has not been described previously. This finding should prompt us to establish active surveillance of causal agents of systemic candidiasis.  相似文献
