A prosthetic foot is a key element of a prosthetic leg, literally forming the basis for a stable and efficient amputee gait. We determined the roll-over characteristics of a broad range of prosthetic feet and examined the effect of a variety of shoes on these characteristics. The body weight of a person acting on a prosthetic foot during roll-over was emulated by means of an inverted pendulum-like apparatus. Parameters measured were the effective radius of curvature, the forward travel of the center of pressure, and the instantaneous radius of curvature of the prosthetic feet. Finally, we discuss how these parameters relate to amputee gait.  相似文献

The concealment of amputation through prosthesis usage can shield an amputee from social stigma and help improve the emotional healing process especially at the early stages of hand or finger loss. However, the traditional techniques in prosthesis fabrication defy this as the patients need numerous visits to the clinics for measurements, fitting and follow-ups. This paper presents a method for constructing a prosthetic finger through online collaboration with the designer. The main input from the amputee comes from the Computer Tomography (CT) data in the region of the affected and the non-affected fingers. These data are sent over the internet and the prosthesis is constructed using visualization, computer-aided design and manufacturing tools. The finished product is then shipped to the patient. A case study with a single patient having an amputated ring finger at the proximal interphalangeal joint shows that the proposed method has a potential to address the patient's psychosocial concerns and minimize the exposure of the finger loss to the public.  相似文献

In this study, we developed an approach for prosthetic foot design incorporating motion analysis, mechanical testing and computer analysis. Using computer modeling and finite element analysis, a three-dimensional (3D), numerical foot model of the solid ankle cushioned heel (SACH) foot was constructed and analyzed based upon loading conditions obtained from the gait analysis of an amputee and validated experimentally using mechanical testing. The model was then used to address effects of viscoelastic heel performance numerically. This is just one example of the type of parametric analysis and design enabled by this approach. More importantly, by incorporating the unique gait characteristics of the amputee, these parametric analyses may lead to prosthetic feet more appropriately representing a particular user's needs, comfort and activity level.  相似文献

Lower limb amputees have decreased balance during daily ambulation compared to nonamputees. An optimally compliant torsion adapter, which enables transverse plane rotation at the socket–pylon junction may reduce limb asymmetries and improve comfort leading to increased confidence and stability during gait. The purpose of this study was to determine if the presence of a torsion adapter affects amputee sensitivity to local perturbations (local dynamic stability) during straight-line walking and during a turning task. Ten unilateral transtibial amputees were fit with a torsion and rigid adapter in random order and blinded to the condition. After a 3-week acclimation period, kinematic data were collected while subjects walked in a straight-line on a treadmill and around a 1-m radius circular path at constant speed. Maximum finite-time Lyapunov exponents (λ), an estimator of local dynamic stability, were calculated for the amputee’s sagittal plane hip, knee and ankle angles for each condition. The prosthetic limb λ was greater during a turn compared to straight-line walking, suggesting amputees are less stable while turning. There were no statistically significant differences found in λ between adapters during both walking conditions, suggesting the torsion adapter had no effect on amputee stability; however, high inter-subject variability due to the examined population and turning task may have masked a small decrease in prosthetic limb hip and knee stability for the torsion adapter during straight-line gait. Therefore, the torsion adapter’s added degree of freedom may have a small adverse effect on prosthetic limb stability during straight-line walking and no effect on turning.  相似文献

The mobility of above-knee amputees (A/K) is limited, in part, due to the performance of A/K prostheses during the stance phase. Currently stance phase control of most conventional A/K prostheses can only be achieved through leg alignment and choice of the SACH (Solid Ankle Cushioned Heel) foot. This paper examines the role of the knee controller in relation to a SACH foot during the stance phase of level walking. The three-dimensional gait mechanics were measured under two stance phase conditions. In the first set of trials, the amputee used a prosthesis with a conventional knee controller that allowed the amputee to maintain the knee joint in full extension during the stance phase. In the second set of trials, the prosthetic knee, during stance, echoed the modified kinematics of the amputee's sound (intact) knee that had been recorded during the previous sound stance phase. Analysis and interpretation of the data indicate the following: (1) SACH foot design can strongly influence the walking mechanics independent of the knee controller; (2) knee controller design and SACH foot design are mutually interdependent; and (3) normal kinematics imposed on the prosthetic knee does not necessarily produce normal hip kinematics (e.g. reduce the abnormal rise in the prosthetic side hip trajectory). Future research is necessary to explore and exploit the interdependency of prosthetic knee control and foot design.  相似文献

This paper describes the initial test results obtained from a newly developed computer-aided socket design (CASD) and manufacturing (CASM) process for above-knee amputees. Anthropometric measures taken from an amputee provided input information to a CASD system. Using these measurements, data from a reference shape library stored in the computer were selected and modified to create a unique socket shape reflecting the particular characteristics of the amputation stump. The resultant shape was produced as a ‘primitive’ test socket by a CASM process. Numerical shape data were then transferred to a CNC milling machine to construct a negative cast, from which the primitive socket was produced by a vacuum-forming procedure. The resultant primitive socket shape was fitted and the amputee was able to load the socket without discomfort. Some shape discrepancies were identified and the shape data were modified interactively by the CASD system to create a final socket shape. The final socket shape was manufactured and worn by the amputee during a 35 min walking trial. Subjective evaluation was that the socket provided comfort and control comparable with that of the conventional socket, and proved to be acceptable to the amputee. This was followed by a 2-month home trial which was also successful. The CASD socket shapes were compared numerically in area, shape and volume with data taken from the original socket worn by the amputee, a new socket made by conventional methods and a topographic model of the amputation stump. The final CASD socket shape compared favourably with that of a socket manufactured by conventional methods. Results indicated that by the use of the CASD/CASM process, it was possible to produce an above-knee prosthetic socket which provided comfort and control for the amputee.  相似文献

Today, the prevalence of major amputation in France can be estimated between 90,000 and 100,000 and the incidence is about 8300 new amputations per year (according to French National Authority for Health estimation). This prevalence is expected to increase in the next decade due to the ageing of the population. Even if prosthetic fitting allows amputee people recovering the walking ability, their autonomy remains limited when crossing obstacles such as slopes, stairs or cross-slopes frequently encountered during outdoors displacements. The aim of the project APSIC was to complete scientific knowledge about adaptation strategies to situations of daily living compared to level walking through an extensive motion analysis study of transtibial and transfemoral amputee compared to non-amputee people. APSIC succeeded in identifying physiologic joint functions and current prosthetic joint limitations in the studied situations, which notably resulted in the design of a prototype of ankle-knee prosthesis adapted to multimodal locomotion of transfemoral amputee. Perspectives of the clinical use of motion analysis within the rehabilitation process were explored and proved to be relevant for personalized approach of motor learning.  相似文献

Despite walking with a wider step width, amputees remain 20% more likely to fall than non-amputees. Since mediolateral (ML) balance is critical for ambulation and contingent on ML foot placement, we used a ML disturbance to perturb walking balance and explore the influence of prosthetic foot stiffness on balance recovery. Ten transtibial amputees were fit with two commonly prescribed prosthetic feet with differing stiffness characteristics; 12 non-amputees also participated. A perturbation device that released an air burst just before heel strike imposed a repeatable medial or lateral disturbance in foot placement. After a medial disturbance, the first recovery step width was narrowed (p<0.0001) for the prosthetic limb (−103%), the sound limb (−51%) and non-amputees (−41%) and more than twice as variable. The ML inclination angle remained reduced (−109%) for the prosthetic limb, while the sound limb and non-amputees approached undisturbed levels (p<0.0004). Amputees required five steps to return to undisturbed step width after a prosthetic medial disturbance versus two steps for the sound limb and for non-amputees. After a lateral disturbance, the first recovery step was widened for the prosthetic limb (+82%), sound limb (+75%), and wider than non-amputees (+51%; p<0.0001), with all participants requiring three steps to return to undisturbed step width. Amputees also exhibited a similar upper torso response compared to the non-amputees for both disturbances. Prosthetic feet with different stiffness properties did not have a significant effect. In conclusion, amputee balance was particularly challenged by medial disturbances to the prosthetic limb implying a need for improved interventions that address these balance deficits.  相似文献

The initial trauma of a major amputation and the medical disorders secondary to it are being studied at the Biomechanics Laboratory. Problems that arise from amputation and from the replacement of the limb by a prosthetic device are complex but interrelated, and approaches to study on basic, applied and clinical levels must be integrated.Three studies carried on at the Biomechanics Laboratory are described in this paper.In the field of amputation operations, it is believed that certain osteoplastic and myoplastic techniques, along with protection for nerve stumps and measures to provide optimum circulatory conditions, may restore biological and mechanical function to a greater degree than is possible with present procedures.From studies of the expenditure of energy during locomotion with and without assistive devices and during therapeutic exercise, it was found that crutch-walking is metabolically much more costly in energy for the amputee than walking with the suction-socket prosthesis; through these studies, it will be possible to define criteria for maximum work loads for disabled persons.In dermatological studies, although there are still unsolved problems, successful methods of treatment have been developed for certain bacterial and fungus infections, contact dermatitides and disorders resulting from edema.  相似文献

Background  

Electromyography (EMG) is the study of muscle function through the inquiry of electrical signals that the muscles emanate. EMG signals collected from the surface of the skin (Surface Electromyogram: sEMG) can be used in different applications such as recognizing musculoskeletal neural based patterns intercepted for hand prosthesis movements. Current systems designed for controlling the prosthetic hands either have limited functions or can only be used to perform simple movements or use excessive amount of electrodes in order to achieve acceptable results. In an attempt to overcome these problems we have proposed an intelligent system to recognize hand movements and have provided a user assessment routine to evaluate the correctness of executed movements.  相似文献

Background

Processing and pattern recognition of myoelectric signals have been at the core of prosthetic control research in the last decade. Although most studies agree on reporting the accuracy of predicting predefined movements, there is a significant amount of study-dependent variables that make high-resolution inter-study comparison practically impossible. As an effort to provide a common research platform for the development and evaluation of algorithms in prosthetic control, we introduce BioPatRec as open source software. BioPatRec allows a seamless implementation of a variety of algorithms in the fields of (1) Signal processing; (2) Feature selection and extraction; (3) Pattern recognition; and, (4) Real-time control. Furthermore, since the platform is highly modular and customizable, researchers from different fields can seamlessly benchmark their algorithms by applying them in prosthetic control, without necessarily knowing how to obtain and process bioelectric signals, or how to produce and evaluate physically meaningful outputs.

Results

BioPatRec is demonstrated in this study by the implementation of a relatively new pattern recognition algorithm, namely Regulatory Feedback Networks (RFN). RFN produced comparable results to those of more sophisticated classifiers such as Linear Discriminant Analysis and Multi-Layer Perceptron. BioPatRec is released with these 3 fundamentally different classifiers, as well as all the necessary routines for the myoelectric control of a virtual hand; from data acquisition to real-time evaluations. All the required instructions for use and development are provided in the online project hosting platform, which includes issue tracking and an extensive “wiki”. This transparent implementation aims to facilitate collaboration and speed up utilization. Moreover, BioPatRec provides a publicly available repository of myoelectric signals that allow algorithms benchmarking on common data sets. This is particularly useful for researchers lacking of data acquisition hardware, or with limited access to patients.

Conclusions

BioPatRec has been made openly and freely available with the hope to accelerate, through the community contributions, the development of better algorithms that can potentially improve the patient’s quality of life. It is currently used in 3 different continents and by researchers of different disciplines, thus proving to be a useful tool for development and collaboration.

Three years'' experience as a doctor taking two clinics a week in an area health authority child health clinic was reviewed. A wide range of clinical conditions was seen, including: problems associated with feeding in breast- and bottle-fed infants; minor developmental abnormalities (mental, behavioural, and physical); surgical and orthopaedic conditions requiring treatment; medical conditions, mainly respiratory and alimentary infections, skin conditions, and problems of over-treatment for minor ailments; and minor genetic abnormalities. Mothers asked for advice on a wide range of topics, risks and benefits of immunisation being the most common. The clinic doctor needs a wide experience in paediatrics to deal with such problems. It is suggested that all lecturers in child health and paediatric and senior registrars should take one clinic a week for six months, and all medical students should attend some clinics as part of their paediatric training. Health visitors have an important role in helping the clinic doctor, but their training should be more realistic and appropriate facilities should be provided to keep them up to date in their work.  相似文献

The influence of limb alignment on the gait of above-knee amputees.   总被引:2,自引:0,他引:2  
Biomechanical gait tests on above-knee amputees were conducted in which the alignment of the prosthesis was changed systematically. An eight-segment biomechanical model of the above-knee amputee was developed to analyse and present the three-dimensional kinematic and kinetic data obtained. The effects of alignment changes on the above-knee amputees' gait were studied in terms of the angular displacements of the lower limbs, ground reactions and intersegmental moments. It was found that following the alignment changes the angular displacement at the hip joint on the prosthetic side showed compensatory actions by the amputee. The ground reaction force was sensitive to alignment changes, and in particular, the changes in the characteristics of the fore-aft component of the ground force could be related to the alignment changes. The antero-posterior intersegmental moments at the prosthetic ankle and knee joints were evidently influenced by alignment.  相似文献

Lack of temperature sensation of myoelectric prosthetic hand limits the daily activities of amputees.To this end,a non-invasive temperature sensation method is proposed to train amputees to sense temperature with psychophysical sensory substitution.In this study,22 healthy participants took part besides 5 amputee participants.The duration time of the study was 31 days with five test steps according to the Leitner technique.An adjustable temperature mug and a Peltier were used to change the temperature of the water/phantom digits to induce temperature to participants.Also,to isolate the surround-ings and show colors,a Virtual Reality(VR)glass was employed.The statistical results conducted are based on the response of participants with questionnaire method.Using Chi-square tests,it is concluded that participants answer the experiment significantly correctly using the Leitner technique(P value<0.05).Also,by applying the"Repeated Measures ANOVA",it is noticed that the time of numbness felt by participants had significant(P value<0.001)difference.Participants could remember lowest and highest temperatures significantly better than other temperatures(P value<0.001);furthermore,the well-trained amputee participant practically using the prosthesis with 72.58%could identify object's temperature with only once time experimenting the color temperature.  相似文献

The users of today's commercial prosthetic hands are not given any conscious sensory feedback. To overcome this deficiency in prosthetic hands we have recently proposed a sensory feedback system utilising a "tactile display" on the remaining amputation residual limb acting as man-machine interface. Our system uses the recorded pressure in a hand prosthesis and feeds back this pressure onto the forearm skin. Here we describe the design and technical solution of the sensory feedback system aimed at hand prostheses for trans-radial/humeral amputees. Amputation in the transfemoral amputee (TFA) results in loss of sensory feedback of the amputated limb and therefore results in the poor postural stability. To assess the postural stability, the limit of stability (LOS) is a reliable parameter. In this study, we have investigated the effect of vibrotactile feedback (VF) on the LOS during the weight shifting exercise (WSE) for a TFA. The data of centre of pressure (COP) during WSE was collected from five TFA and five healthy individuals using a zebris force plate. The VF was provided on the amputated/healthy limb’s anterior and posterior part of the stump/thigh during forward and backward WSE, respectively. A customized foot insole with 24 embedded dielectric sensors was used to drive the vibratory motor. The effect of VF was analyzed by pre and post-test. Results show that with the use of VF, TFA significantly improved (t-test, p?<?.05) the sound limb’s LOS during forward WSE. Also, ANOVA analysis between WSE divisions shows that the prosthetic limb does not follow the path of WSE. We further examine the spectral power using the Welch method to determine the dominant sway frequency of COP. It shows a decreased frequency between 0.5–2?Hz in the healthy and decreased frequency between 0–0.5?Hz and >2?Hz in the amputee with VF. It concluded that VF could improve the LOS of TFA during WSE which ultimately leads to postural stability enhancement.  相似文献
