Background

Obesity and major depressive disorder (MDD)/anxiety disorders often co-occur and aggravate each other resulting in adverse health-related outcomes. As little is known about the potential effects of interaction between obesity and MDD and/or anxiety disorders on health-related quality of life (HR-QoL), this study was aimed at examining these combined effects.

Methods

We collected data among N = 89,332 participants from the LifeLines cohort study. We categorized body weight using body mass index (kg/m2) as normal weight (18.5–24.99), overweight (25–29.9), mild obesity (30–34.9) and moderate/severe obesity (≥ 35); we measured abdominal obesity using a waist circumference of ≥102 and ≥ 88 cm for males and females, respectively. MDD and anxiety disorders were diagnosed with the Mini-International Neuropsychiatric Interview. HR-QoL was assessed using the RAND-36 questionnaire to compute physical and mental quality of life scores. We used binary logistic and linear regression analyses.

Results

The combined effect of obesity and MDD and/or anxiety disorders on physical QoL was larger than the sum of their separate effects; regression coefficients, B (95%-confidence interval, 95%-CI) were: - 1.32 (-1.75; -0.90). However, the combined effect of obesity and major depression alone on mental QoL was less than the additive effect. With increasing body weight participants report poorer physical QoL; when they also have MDD and/or anxiety disorders participants report even poorer physical QoL. In persons without MDD and/or anxiety disorders, obesity was associated with a better mental QoL.

Conclusions

Obesity and MDD and/or anxiety disorders act synergistically on physical and mental QoL. The management of MDD and/or anxiety disorders and weight loss may be important routes to improve HR-QoL.  相似文献

Objective: To estimate the association between body mass index (BMI) and health‐related quality of life (HRQL) and examine whether joint pain and obesity‐related comorbidities mediate the BMI‐HRQL association. Research Methods and Procedures: Population‐based survey data from the 1999 Behavioral Risk Factor Surveillance Survey. Adults (N = 155, 989) were classified according to BMI as underweight (<18.5 kg/m2), desirable weight (18.5 to 24.9 kg/m2), overweight (25 to 29.9 kg/m2), obese class I (30 to 34.9 kg/m2), obese class II (35 to 39.9 kg/m2), and obese class III (≥40 kg/m2). Data including general health status, unhealthy days in the past 30 caused by physical problems and mental problems, and total unhealthy days in the past 30 were collected. Results: After adjusting for age, sex, race, smoking, education, and income, we observed J‐shaped associations between BMI and HRQL. Compared with desirable weight adults, underweight, overweight, and obesity classes I, II, and III adults [odds ratio (OR) = 1.57, 1.19, 1.95, 2.72, and 4.36, respectively] were significantly (p < 0.001) more likely to report fair/poor general health status. For unhealthy days caused by physical problems, the corresponding ORs were 1.51, 1.15, 1.66, 2.27, and 3.61 (p < 0.001). For unhealthy days caused by mental problems, the ORs were 1.35, 1.14 1.43, 1.57, and 2.25 (p < 0.001). For total unhealthy days, the corresponding ORs were 1.27, 1.09, 1.37, 1.73, and 2.46 (p < 0.01). Adding joint pain and obesity‐related comorbidities into models attenuated BMI‐HRQL associations. Discussion: Associations between BMI and HRQL indices were J‐shaped. Joint pain and comorbidities may mediate BMI‐HRQL associations.  相似文献

Objective: Research investigating obesity‐related quality of life (QOL) has shown that at increasing levels of overweight, individuals report more impaired QOL. Further, some research has indicated that white women suffer more impairment than men and African Americans. The current study sought to expand the existing literature by investigating an extreme subsample of the obese population. It was expected that participants in the current study would report more impaired obesity‐related QOL than in previous research conducted with less obese individuals. It was also hypothesized that race and gender groups would differ in obesity‐related QOL and that the relationship between degree of overweight and QOL would not be consistent across race and gender groups. Research Methods and Procedures: Impact of Weight on Quality of Life Questionnaire‐Lite Version data were collected from 512 individuals seeking gastric bypass surgery (mean BMI = 53.3) Results: Results confirmed the study hypotheses. In general, white women reported the most QOL impairment, despite having significantly lower BMI than other race/gender groups. Compared with previous studies, the observed relationships between BMI and QOL were somewhat attenuated. Discussion: Various domains of QOL may be differentially affected by degree of obesity; these relationships are not homogeneous throughout the obese population.  相似文献

Objective: To investigate the contribution of meal and snack replacements for long‐term weight maintenance and risk factor reduction in obese patients. Research Methods and Procedures: Prospective, randomized, two‐arm, parallel intervention for 12 weeks followed by a prospective single‐arm 4‐year trial in a University Hospital clinic. One hundred patients, >18 years old and with a body mass index > 25 and ≤ 40 kg/m2, were prescribed a 1200 to 1500 kcal/d control diet (Group A) or an isoenergetic diet, including two meal and snack replacements (vitamin‐ and mineral‐fortified shakes, soups, and bars) and one meal high in fruits and vegetables (Group B). Following a 3 months of weight loss, all patients were prescribed the same energy‐restricted diet (1200 to 1500 kcal) with one meal and one snack replacement for an additional 4 years. Results: All 100 patients were evaluated at 12 weeks. Mean percentage weight loss was 1.5 ± 0.4% and 7.8 ± 0.5% (mean ± SEM) for Groups A and B, respectively. At 12 weeks systolic blood pressure, plasma triacylglycerol, glucose, and insulin concentrations were significantly reduced in Group B, whereas no changes occurred in Group A. After 4 years, 75% of the patients were evaluated. Total mean weight loss was 3.2 ± 0.8% for Group A and 8.4 ± 0.8% (mean ± SEM) for Group B. Both groups showed significant improvement in blood glucose and insulin (p < 0.001), but only Group B showed significant improvement in triacylglycerol and systolic blood pressure compared to baseline values (p < 0.001). Discussion: Providing a structured meal plan via vitamin‐ and mineral‐fortified liquid meal replacements is a safe and effective dietary strategy for obese patients. Long‐term maintenance of weight loss with meal replacements can improve certain biomarkers of disease risk.  相似文献

Objective: Body fatness is partly under hypothalamic control with effector limbs, which include the endocrine system and the autonomic nervous system (ANS). In previous studies we have shown, in both obese and never‐obese subjects, that weight increase leads to increased sympathetic and decreased parasympathetic activity, whereas weight decrease leads to decreased sympathetic and increased parasympathetic activity. We now report on the involvement of such ANS mechanisms in the action of anti‐obesity drugs, independent of change in weight. Research Methods and Procedures: Normal weight males (ages 22 to 38 years) were fed a solid food diet, carefully measured to maintain body weight, for at least 2 weeks, as inpatients at the Rockefeller University General Clinical Research Center. In a single‐blind, placebo/drug/placebo design, eight subjects received dexfenfluramine, seven phentermine (PHE), and seven sibutramine (SIB). ANS measures of parasympathetic and sympathetic activity included: determination of amount of parasympathetic control (PC) and sympathetic control (SC) of heart period (interbeat interval), using sequential pharmacological blockade by intravenous administration of atropine and esmolol. These autonomic controls of heart period are used to estimate the overall level of parasympathetic and sympathetic activities. Norepinephrine, dopamine, and epinephrine levels in 24‐hour urine collections were measured and also resting metabolic rate (RMR). Results: Sufficient food intake maintained constant body weight in all groups. PHE and SIB produced significant increases in SC but no change in PC or in RMR. In contrast, dexfenfluramine produced marked decreases in SC, PC, and RMR. For all three drugs, the effects on urine catecholamines directly paralleled changes in cardiac measures of SC. Discussion: ANS responses to PHE and SIB were anticipated. The large, and unanticipated, response to dexfenfluramine suggests further study to determine whether there could be any relation of these ANS changes to the adverse cardiovascular effects of treatment with dexfenfluramine.  相似文献

We studied the effects of hand weeding of second‐year plants of the biennial garlic mustard (Alliaria petiolata) on first‐year plants (seedlings) and native ground layer vegetation. Garlic mustard is a Eurasian species that has invaded deciduous forest ground layers in eastern North America. Treatments consisted of a control and an early or late weeding of second‐year garlic mustard. The early treatment (early March) was applied before garlic mustard seeds had germinated and when most native species were dormant. The late treatment (mid‐May) occurred after plants had bolted, flowering was occurring, and most native species and new garlic mustard seedlings were actively growing. Pre‐treatment data were obtained in 2004 and treated and control plots were sampled in 2005, 2006, and 2007. No significant treatment effects were observed in 2004 or 2005. In 2006, mean cover of first‐year plants was higher in the early weeding treatment than in the late weeding treatment and control. In 2007, mean cover of first‐year garlic mustard was higher in the control than in either of the two weeding treatments. There were no significant treatment effects in any year on native vegetation cover, bare ground, or the five most abundant native species. Our data indicate that (1) late weeding of garlic mustard provided more effective control than early weeding because late weeding allows second‐year plants to compete with garlic mustard seedlings for a longer period of time and (2) competition between first‐ and second‐year plants is responsible for alternating dominance of first‐year and second‐year garlic mustard plants.  相似文献

Objective: This study evaluated gastric bypass surgery outcomes according to presurgical binge eating severity. Research Methods and Procedures: Adult patients completed assessment questionnaires including the Short Form‐36, Gormally Binge Eating Scale (BES), and Beck Depression Inventory (BDI) before and 12 months after surgery. Results: One hundred nine patients (18 men, 91 women) were recruited. Based on their baseline BES scores, patients were non‐ [n = 52 (48%)], moderate [n = 31 (28%)], or severe [n = 26 (24%)] binge eaters. Although the percentage of excess weight loss was greatest after 12 months in the severe binge eaters, the difference among groups was not significant. Severe binge eaters had higher baseline BDI scores than either non‐ or moderate binge eaters (p = 0.001). After surgery, BDI scores declined significantly in all groups from the baseline scores but remained higher postoperatively in the severe binge eaters (p = 0.018). BES scores declined significantly (p = 0.000) after surgery within all groups. There was no difference in the Short Form‐36 physical component summary scores at baseline among groups. Mental component summary scores were significantly lower in the severe binge eaters (p = 0.001). After surgery, there was no difference among groups in either physical or mental component summary scores. Discussion: In conclusion, data from the present study suggest that patients have similar outcomes in terms of improved depression scores, binge eating behavior, and health‐related quality of life regardless of their binge eating severity before surgery. Patients with the most severe binge eating behavior before surgery showed the most improvement when assessed 12 months after surgery.  相似文献

A hybrid approach combining life cycle assessment and input‐output analysis was used to demonstrate the economic and environmental benefits of current and future improvements in agricultural and industrial technologies for ethanol production in Brazilian biorefineries. In this article, three main scenarios were evaluated: first‐generation ethanol production with the average current technology; the improved current technology; and the integration of improved first‐ and second‐generation ethanol production. For the improved first‐generation scenario, a US$1 million increase in ethanol demand can give rise to US$2.5 million of total economic activity in the Brazilian economy when direct and indirect purchases of inputs are considered. This value is slightly higher than the economic activity (US$1.8 million) for an energy equivalent amount of gasoline. The integration of first‐ and second‐generation technologies significantly reduces the total greenhouse gas emissions of ethanol production: 14.6 versus 86.4 grams of carbon dioxide equivalent per megajoule (g CO2‐eq/MJ) for gasoline. Moreover, emissions of ethanol can be negative (–10.5 g CO2‐eq/MJ) when the system boundary is expanded to account for surplus bioelectricity by displacement of natural gas thermal electricity generation considering electricity produced in first‐generation optimized biorefineries.  相似文献

Objective: Obesity is an important etiologic factor in sleep‐disordered breathing (SDB), but the extent to which changes in obesity across adult life contribute independently to SDB in old age has not been studied. In this study, we examined the association between changes in obesity from midlife to late adulthood and overnight recording of respiration during sleep. Research Methods and Procedures: Subjects in this study are from the Western Collaborative Group Study, a longitudinal cardiovascular epidemiological study that began in 1960 through 1961. Overnight sleep recordings were obtained from 281 male participants in the 1995 through 1996 follow‐up of the Western Collaborative Group Study. Subjects were 75 to 91 years old when assessed for SDB as indexed by the respiratory disturbance index and an oxygen desaturation index (O2DI). Long‐term changes in anthropometrics were evaluated and examined in relation to SDB severity. Results: Over the 30 years of follow‐up, body mass index and waist circumference increased significantly for this sample and were associated with SDB severity as indexed by respiratory disturbance index and O2DI. Waist circumference at baseline and gain in waist circumference over the 30 years of follow‐up (both p = 0.01) were significantly and independently associated with SDB severity as assessed by O2DI. However, percentage of variance as accounted for by waist circumference was modest. Discussion: This study supports the hypothesis that gain in waist circumference over adult life is significantly associated with SDB severity in older men.  相似文献

Objective: Obesity researchers have a growing interest in measuring the impact of weight and weight reduction on quality of life. The Impact of Weight on Quality of Life questionnaire (IWQOL) was the first self‐report instrument specifically developed to assess the effect of obesity on quality of life. Although the IWQOL has demonstrated excellent psychometric properties, its length (74 items) makes it somewhat cumbersome as an outcome measure in clinical research. This report describes the development of a 31‐item version of the IWQOL (IWQOL‐Lite). Research Methods and Procedures: IWQOLs from 996 obese patients and controls were used to develop the IWQOL‐Lite. Psychometric properties of the IWQOL‐Lite were examined in a separate cross‐validation sample of 991 patients and controls. Results: Confirmatory factor analysis provided strong support for the adequacy of the scale structure. The five identified scales of the IWQOL‐Lite (Physical Function, Self‐Esteem, Sexual Life, Public Distress, and Work) and the total IWQOL‐Lite score demonstrated excellent psychometric properties. The reliability of the IWQOL‐Lite scales ranged from 0.90 to 0.94 and was 0.96 for the total score. Correlations between the IWQOL‐Lite and collateral measures supported the construct validity of the IWQOL‐Lite. Changes in IWQOL‐Lite scales over time correlated significantly with changes in weight, supporting its sensitivity to change. Significant differences in IWQOL‐Lite scale and total scores were found among groups differing in body mass index, supporting the utility of the IWQOL‐Lite across the body mass index spectrum. Discussion: The IWQOL‐Lite appears to be a psychometrically sound and clinically sensitive brief measure of quality of life in obese persons.  相似文献

Objective: To assess the cost‐effectiveness and cost‐benefit of Planet Health, a school‐based intervention designed to reduce obesity in youth of middle‐school age children. Research Methods and Procedures: Standard cost‐effectiveness analysis methods and a societal perspective were used in this study. Three categories of costs were measured: intervention costs, medical care costs associated with adulthood overweight, and costs of productivity loss associated with adulthood overweight. Health outcome was measured as cases of adulthood overweight prevented and quality‐adjusted life years (QALYs) saved. Cost‐effectiveness ratio was measured as the ratio of net intervention costs to the total number of QALYs saved, and net‐benefit was measured as costs averted by the intervention minus program costs. Results: Under base‐case assumptions, at an intervention cost of $33, 677 or $14 per student per year, the program would prevent an estimated 1.9% of the female students (5.8 of 310) from becoming overweight adults. As a result, an estimated 4.1 QALYs would be saved by the program, and society could expect to save an estimated $15, 887 in medical care costs and $25, 104 in loss of productivity costs. These findings translated to a cost of $4305 per QALY saved and a net saving of $7313 to society. Results remained cost‐effective under all scenarios considered and remained cost‐saving under most scenarios. Discussion: The Planet Health program is cost‐effective and cost‐saving as implemented. School‐based prevention programs of this type are likely to be cost‐effective uses of public funds and warrant careful consideration by policy makers and program planners.  相似文献

Objective: Few studies examining the relationship between obesity and health-related quality of life (HRQOL) have used a medical outpatient population or demonstrated a relationship in men. Furthermore, most studies have not adequately considered comorbid illness. The goal of this study was to examine the relationship between body mass index (BMI) and HRQOL in male outpatients while considering comorbid illness. Research Methods and Procedures: This cross-sectional study examined 1168 male outpatients from Durham Veterans’ Affairs Medical Center. Multiple linear regression was used to examine the relationship of BMI with each subscale from the Medical Outcomes Study Short Form 36 while adjusting for age, race, comorbid illness, depression, and physical activity. Results: Participants had a mean age of 54.7 ± 5.6 years; 69% were white and 29% were African American. The distribution for BMI was as follows: 18.5 to <25 kg/m2 (21%), 25 to <30 kg/m2 (43%), 30 to <35 kg/m2 (25%), 35 to <40 kg/m2 (8%), and ≥40 kg/m2 (3%). Mean Short Form 36 subscale scores were lower than U.S. norms by an average of 27%. Individuals with BMI ≥40 kg/m2 had significantly lower scores compared with normal weight individuals on the Role-Physical and Vitality subscales. On the Physical Functioning and Physical Component subscales, lower scores were observed at BMI ≥35 kg/m2. On the Bodily Pain subscale, lower scores were observed at BMI ≥25 kg/m2. Discussion: An inverse relationship between BMI and physical aspects of HRQOL exists in a population of male outpatients. Increased BMI was most prominently associated with bodily pain; this relationship should receive more attention in clinical care and research.  相似文献

目的:探究综合性心理干预对冠心病介入治疗患者焦虑、抑郁及生活质量的影响。方法:选择2014年6月~2015年6月期间我院收治冠心病性介入治疗患者3280例为研究对象,采用随机数字法将其分为观察组(1648例)和对照组(1632例),观察组患者给予常规治疗、抗抑郁治疗及综合心理干预,对照组给予常规治疗、抗抑郁治疗;采用焦虑自评量表(SAS)、抑郁自评量表(SDS)评价患者治疗前后焦虑、抑郁状态,生活质量评价量表QLQ-C30量表评价患者治疗前后生活质量的变化情况。结果:两组患者在入院时SAS和SDS得分不存在差异(P0.05);干预1月后两组组患者的SAS和SDS得分均出现显著降低(P0.05),且观察组患者的SAS和SDS评分均明显低于对照组(P0.05);干预前两组患者生存质量各维度的评分均不存在显著差异(P0.05);干预1个月后两组患者躯体功能、角色功能、社会功能、情绪功能及总体症状较干预前均出现明显改善(P0.05),且干预后观察组患者躯体功能、角色功能、社会功能、情绪功能及总体症状均显著优于对照组(P0.05)。结论:冠心病介入治疗患者进行综合性心理干预能够改善患者心理状态,降低患者焦虑、抑郁情绪,提高生活质量,对临床冠心病的治疗有重要的意义。  相似文献

Most major rivers in the southwestern United States have been hydrologically altered to meet human needs. Altered hydrological regimes have been associated with declines in native riparian forests. Today, many riparian areas have little or no regeneration of native riparian species and are now dominated by exotic Saltcedar (Tamarix chinensis Lour.). Success of riparian restoration efforts at least partially depends on the number of seedlings surviving the first growing season. Seedling survival is influenced by many abiotic and biotic factors including competition from other plants and available soil moisture, which is partially dependent on soil texture. In this study, we evaluated the relative importance of four soil categories (sandy loam, loam, silt, and clay), rate of soil moisture decline, salinity, beginning‐ and end‐season Saltcedar density, initial Cottonwood (Populus deltoides Marshall subsp. wislizenii (Wats.) Eckenw.) seedling density, percent vegetation cover by potential dominant competitors Pigweed (Amaranthus L.) and Barnyard grass (Echinochloa crusgalli L., Beauv.), and average total vegetation height to Cottonwood seedling survival. Factors influencing seedling survival differed among the four soil types. Rate of moisture decline was important in sandy soils, whereas vegetation height influenced seedling survival in loamy soils. Overall, models of seedling survival in all the four soil types indicated rate of moisture decline as the single most important variable influencing Cottonwood survival. High initial densities of Saltcedar were correlated to higher survival in Cottonwood seedlings. Therefore, it is important to identify soil texture and understand soil moisture decline rates when proposing riparian Cottonwood restoration.  相似文献

Nationally representative data on the quality of care for obese patients in US‐ambulatory care settings are limited. We conducted a cross‐sectional analysis of the 2005 and 2006 National Ambulatory Medical Care Survey (NAMCS). We examined obesity screening, diagnosis, and counseling during adult visits and associations with patient and provider characteristics. We also assessed performance on 15 previously published ambulatory quality indicators for obese vs. normal/overweight patients. Nearly 50% (95% confidence interval (CI): 46–54%) of visits lacked complete height and weight data needed to screen for obesity using BMI. Of visits by patients with clinical obesity (BMI ≥30.0 kg/m2), 70% (66–74%) were not diagnosed and 63% (59–68%) received no counseling for diet, exercise, or weight reduction. The percentage of visits not being screened (48%), diagnosed (66%), or counseled (54%) for obesity was also notably higher than expected even for patients with known obesity comorbidities. Performance (defined as the percentage of applicable visits receiving appropriate care) on the quality indicators was suboptimal overall. In particular, performance was no better than 50% for eight quality indicators, which are all related to the prevention and treatment of obesity comorbidities, e.g., coronary artery disease, hypertension, hyperlipidemia, asthma, and depression. Performance did not differ by weight status for any of the 15 quality indicators; however, poorer performance was consistently associated with lack of height and weight measurements. In conclusion, many opportunities are missed for obesity screening and diagnosis, as well as for the prevention and treatment of obesity comorbidities, in office‐based practices across the United States, regardless of patient and provider characteristics.  相似文献

The purpose of this study was to examine the correlates of participation in a childhood obesity prevention trial. We sampled parents of children recruited to participate in a randomized controlled trial. Eligible children were 2.0–6.9 years with BMI ≥95th percentile or 85th to <95th percentile if at least one parent was overweight. We attempted contact with parents of children who were potentially eligible. We recruited 475 parents via telephone following an introductory letter. We also interviewed 329 parents who refused participation. Parents who refused participation (n = 329) did not differ from those who participated (n = 475) by number of children at home (OR 0.94 per child; 95% CI: 0.77–1.15) or by child age (OR 1.07 per year; 95% CI: 0.95–1.20) or sex (OR 1.06 for females vs. males; 95% CI: 0.80–1.41). After multivariate adjustment, parents who were college graduates vs. <college graduates were less likely to participate (OR 0.62; 95% CI: 0.46–0.83). In addition, parents were less likely (OR 0.41; 95% CI: 0.31–0.56) to participate if their child was overweight vs. obese. Among the 115 refusers with obese children, 21% cited as a reason for refusal that their children did not have a weight problem, vs. 30% among the 214 refusers with overweight children. In conclusion, parents of preschool‐age children with a BMI 85–95th%ile are less likely to have their children participate in an obesity prevention trial than parents of children with BMI >95th%ile. One reason appears to be that they less frequently consider their children to have a weight problem.  相似文献
