Cumulative risk assessments (CRAs) include the examination of risks posed by multiple stressors and include population-specific vulnerabilities and susceptibilities. In this case study, we assess potential hearing impairment hazard due to joint exposure from noise and volatile organic compounds (VOCs) in order to examine the strengths and limitations of using secondary data on exposure and health effects for a CRA. Block group-level noise categories were estimated using modeled street-level data. A quantile regression model of sociodemographic and personal predictors from the 1999–2000 U.S. National Health and Nutrition Examination Survey VOC dataset was used along with block group-level sociodemographic and personal variables to estimate VOC exposures. Hazard indices (HIs) for potential hearing impairment due to joint noise and VOC exposures were calculated. County-averaged HIs for hearing impairment ranged from 0.8 (10th total VOCs percentile and 45–60 dB) to 1.7 (90th total VOCs percentile and 71–75 dB). Limitations of the exposure and health effects data included issues combining heterogeneous data and a lack of established threshold levels for combined low-level exposures; yet, this case study illustrates that screening-level CRAs, including non-chemical stressors, can be accomplished with publicly available data and existing methods.

The relative risk model (RRM) used in Port Valdez, AK, and in Oregon's Willamette/McKenzie Watershed was applied to the Codorus Creek Watershed in south central Pennsylvania. The assessment evaluated the relative risk model for its applicability for ranking ecological risks within the Codorus Creek Watershed. The Codorus Creek Watershed approach included ranking stressors and habitats for regions within the watershed. Geographical Information Systems were vital in compiling and comparing stressor and habitat spatial data from regions in the watershed. The risk of ecological impacts to degrade assessment endpoints were calculated and ranked by quantitatively determining the interactions of the stressors and habitats as defined in the conceptual site model. Uncertainty assessment was conducted and the impact upon the relative ranks and risk conclusions evaluated. To determine regional risks, risk management information was gathered identifying areas to be protected, areas of high stress, and areas where additional information is needed. The results supported the applicability of the RRM and suggested areas and stressors for restoration efforts in the Codorus Creek Watershed. Two critical sets of conclusions were drawn from the assessment. First, in the Codorus Creek Watershed, the most significant stressor is agriculture land use, the most significantly impacted endpoint is water quality, and the most vulnerable habitats are those for macroinvertebrates and warm-water fish. Second, this risk assessment demonstrates the feasibility of using the RRM for assessing risk from multiple stressors on habitats with multiple assessment endpoints in an eastern watershed.

Hormesis is a widespread phenomenon across occurring many taxa and chemicals, and, at the single species level, issues regarding the application of hormesis to human health and ecological risk assessment are similar. However, interpreting the significance of hormesis for even a single species in an ecological risk assessment can be complicated by competition with other species, predation effects, etc. In addition, ecological risk assessments may involve communities of hundreds or thousands of species as well as a range of ecological processes. Applying hormetic adjustments to threshold effect levels for chemicals derived from sensitivity distributions for a large number of species is impractical. For ecological risks, chemical stressors are frequently of lessor concern than physical stressors (e.g., habitat alteration) or biological stressors (e.g., introduced species), but the relevance of hormesis to non‐chemical stressors is unclear. Although ecological theories such as the intermediate disturbance hypothesis offer some intriguing similarities between chemical hormesis and hormetic‐like responses resulting from physical disturbances, mechanistic explanations are lacking. While further exploration of the relevance of hormesis to ecological risk assessment is desirable, it is unlikely that hormesis is a critical factor in most ecological risk assessments, given the magnitude of other uncertainties inherent in the process.

We conducted an ecological risk assessment of the marine environment of Port Valdez, a fjord in south-central Alaska. Because the assessment was regional rather than site-specific and contained a large number of different stressors in a variety of environments, we required a nontraditional method to estimate risks. We created a Relative Risk Model to rank and sum individual risks numerically within each subarea, from each source, and to each habitat. Application of this model involved division of Port Valdez into 11 subareas containing specific ecological and anthropogenic structures and activities. Within each subarea, the stressor sources were analyzed to estimate exposure of receptors within habitats leading to effects relevant to the chosen assessment endpoints. The subareas were analyzed and compared to form a Port-wide perspective of ecological risk. Available chemical concentrations from sediment and mussels collected from the Port were compared to various toxicological benchmarks as a partial confirmation of the risk analysis. An estimation of the risk of polycyclic aromatic hydrocarbons (PAHs) to marine invertebrates indicated low risk. The municipal boat harbor had the highest estimate, which reflected our relative risk rankings. The Relative Risk Model approach appears robust and has potential for use in situations where multiple stressors are of concern and for assessments covering broad geographic areas. In the Port Valdez assessment the approach provided relative risk rankings for chemical and physical stressors from various sources. But data were available for confirmation of risk estimates only for the chemical stressors. The rankings are relative, and extrapolation beyond the scenario in which they were developed is not warranted. Uncertainty is large, and the numerical scores collapse a multidimensional space into a single value. Use of just the numerical score out of context is more valid than with other indexes. The value of the approach lies in the relative rankings and the accounting of the components of the relative risk score.

Several procedures for Ecological Risk Assessment (ERA) have been suggested. The use of these existing procedures often relies on availability of existing data and/or on large resources for acquisition of new ones. This paper presents a three-tiered procedure for retrospective evaluation of risks adapted to limited resources and scarce background information of relevance for risk assessments, such as in developing countries. The tiers require successively more detailed investigations. The approach assures that resources available for site-specific investigations are directed towards well-formulated questions raised during previous stages of the assessment. The first tier, the preliminary assessment, is a qualitative evaluation of existing information on anthropogenic stressors, sources of stressors and expected ecological effects. The second tier is a regional risk assessment; a semi-quantitative evaluation of ecological risks, over large geographical areas, which results in a ranking of sources and stressors having the greatest potential for ecological impact and ranking of subareas inside the study area more likely to be impacted. The final tier is a site-specific and quantitative risk assessment, at a smaller scale and requiring more resources, that incorporates methodologies for establishing causality between exposure to multiple stressors and effects on specific endpoints of ecological and societal relevance.

Assessing and managing the ecological risks from multiple stressors is becoming increasingly important as our environmental and regulatory focus moves from managing point sources to one of managing and trading risks from multiples sources over large geographic areas. There are important corollaries to this shift in focus and scale, one is the increased role and importance of non-chemical stressors in shaping and controlling ecological systems, the fact that these categories of stress are, for the most part, not regulated under the traditional legislative mandates, that we have limited knowledge regarding the interaction of chemical and non-chemical stressors, that at large scales we are faced with the integration of and trading of risks to multiple resource categories. This trend of increased attention to regional-scale environmental issues requires the development of new analysis and interpretive strategies and highlight the need for a systematic framework for addressing the ecological effects of multiple stressors at regional ecological scales. We propose that such a framework have three essential properties: it be risk-based; it be effects driven; and it have the flexibility to be used in both a retrospective and prospective manner. While several approaches have been proposed we believe that an ecological risk assessment framework satisfies these criteria, has been used successfully in regional assessments, and can readily be modified and adapted to serve a wide variety of problem settings.

Soldiers who deployed to Saudi Arabia in support of Operation Desert Shield were exposed to a wide variety of stressors. These stressors from the pre-combat phase of the deployment undoubtedly affect the current health of Gulf War veterans, but the exact mechanisms and linkages are not known. This article examines the nature of those stressors and possible effects on later health of veterans.

Extrapolation in risk assessment involves the use of data and information to estimate or predict something that has not been measured or observed. Reasons for extrapolation include that the number of combinations of environmental stressors and possible receptors is too large to characterize risks comprehensively, that direct characterization is sometimes impossible, and that the power to characterize risk in a particular situation can be enhanced by using information obtained in other similar situations. Three types of extrapolation are common in risk assessments: biological (including between taxa and across levels of biological organization), temporal, and spatial. They can be thought of conceptually as the axes of a 3-dimensional graph defining the state space of biological, temporal, and spatial scales within which extrapolations are made. Each of these types of extrapolation can introduce uncertainties into risk assessments. Such uncertainties may be reduced through synergistic research facilitated by the sharing of methods, models, and data used by human health and ecological scientists

Food allergies are emerging health risks in much of the Western world, and some evidence suggests prevalence is increasing. Despite lacking scientific consensus around prevalence and management, policies and regulations are being implemented in public spaces (e.g., schools). These policies have been criticized as extreme in the literature, in the media, and by the non-allergic population. Backlash appears to be resulting from different perceptions of risk between different groups. This article uses a recently assembled national dataset (n = 3,666) to explore how Canadians perceive the risks of food allergy. Analyses revealed that almost 20% self-report having an allergic person in the household, while the average respondent estimated the prevalence of food allergies in Canada to be 30%. Both of these measures overestimate the true clinically defined prevalence (7.5%), indicating an inflated public understanding of the risks of food allergies. Seventy percent reported food allergies to be substantial risks to the Canadian population. Multivariate logistic regression models revealed important determinants of risk perception including demographic, experience-based, attitudinal, and regional predictors. Results are discussed in terms of understanding emerging health risks in the post-industrial era, and implications for both policy and risk communication.

Approaches for Integrated Risk Assessment
Recognizing the need to enhance the effectiveness and efficiency of risk assessments globally, the World Health Organization's International Programme on Chemical Safety, the U.S. Environmental Protection Agency, the European Commission, and the Organization for Economic Cooperation and Development developed a collaborative partnership to foster integration of assessment approaches used to evaluate human health and ecological risks. The objectives of this effort included: improving understanding of the benefits of integration, identifying obstacles to the integration process, and engaging key agencies, organizations, and scientific societies to promote integration. A framework with supporting documentation was developed to describe an approach for integration. Four case studies were constructed to illustrate how integrated risk assessments might be conducted for chemical and nonchemical stressors. The concepts and approaches developed in the project were evaluated in an international workshop. The goal of this effort was international acceptance of guidance for integrated risk assessment.

Corosolic acid (CRA), a constituent of banaba leaves, has been reported to have anti-inflammatory and hypoglycemic activities. The aim of this study was to determine the effects of CRA on metabolic risk factors including obesity, hypertension, hyperinsulinemia, hyperglycemia, and hyperlipidemia together with oxidative stress and inflammation, all of which are characteristic of the SHR/NDmcr-cp (cp/cp) (SHR-cp) rat, an animal model of metabolic syndrome. Six-week-old male SHR-cp rats were fed a high fat diet containing 0.072% CRA for 14 weeks. Treatment with CRA lowered blood pressure, which was elevated in control animals, by 10% after 8 weeks, and serum free fatty acids by 21% after 2 weeks. CRA treatment resulted in decreases in the levels of the oxidative stress markers thiobarbituric acid-reactive substances and 8-hydroxydeoxyguanosine by 27% and 59%, respectively, after 2 weeks. CRA treatment also reduced the levels of myeloperoxidase markers, 3-nitrotyrosine and 3-chlorotyrosine by 38% and 39%, respectively, after 10 weeks, and tended to decrease the levels of high sensitivity C-reactive protein, a marker of inflammation, after 6 weeks. However, CRA had no effect on weight gain or hyperglycemia. These results demonstrate that CRA can ameliorate hypertension, abnormal lipid metabolism, and oxidative stress as well as the inflammatory state in SHR-cp rats. This implies that CRA can be beneficial for preventing atherosclerosis-related diseases that are an increasing health care problem worldwide.

Currently, no general measure of population health response to untestable doses of chemicals and microbes has been established that accounts for uncertainty quantitatively and indicates relative toxicity or virulence directly. Untestable doses include those corresponding to the 2.74 × 10?7 illnesses per exposure expressed in goals of the U.S. Environmental Protection Agency's Surface Water Treatment Rule, and doses of human bioterror agents. For example, it is shown that relative Benchmark Dose (BMD) values depend upon the level of confidence assumed. Because of the lack of scientific basis for this level, BMDs are not comparable among health stressors for untestable doses and stressors. In this paper a new predictive Bayesian method is proposed for absolute and relative dose-response assessment based on available information. Information may include toxicological judgment, epidemiological statistics, genetic information, related data, and numeric dose-response data. Results for rotavirus indicate a “safe dose” of 6.3 × 10?7 focus-forming units/exposure, approximately one-half log above the dose corresponding to the maximum risk for any pathogen assuming a 100% infection rate. The result further indicates the limited value of data in refining the assessment, due to the inability of data to reduce variability. The method is suggested for assessing risks of new and existing chemicals and pathogens, as a basis for prioritizing expenditures for protection against environmental and terrorist threats.

Environmental toxicants are ubiquitous, and many are known to cause harmful health effects. However, much of what we know or think we know concerning the targets and long-term effects of exposure to environmental stressors is sadly lacking.Toxicant exposure may have health effects that are currently mischaracterized or at least mechanistically incompletely understood. While much of the recent excitement about stem cells(SCs)focuses on their potential as therapeutic agents, they also offer a valuable resource to give us insight into the mechanisms and risks of toxicant effects. Not only as a response to the increasing ethical pressure to reduce animal testing,SC studies allow us valuable insight into the true effects of human exposure to environmental stressors under controlled conditions. We present a review of the history of publications on the effects of environmental stressors on SCs,followed by a consolidation of the literature over the past five years on a subset of key environmental stressors of importance to human health and their effects on both embryonic and tissue SCs.The review will make constructive suggestions as to areas of toxicant research where further studies are needed,as well as making indications of the potential utility for advancing knowledge and directing research on environmental toxicology.

The forests of North America provide a variety of benefits including water, recreation, wildlife habitat, timber, and other forest products. However, they continue to face many biotic and abiotic stressors including fires, native and invasive pests, fragmentation, and air pollution. Forest health specialists have been monitoring the health of forests for many years. This paper highlights some of the most damaging forest stressors affecting North American forests in recent years and provides some projections of future risks.

Background: Biomedical science is producing an avalanche of data about risk factors, often with a small predictive value, associated with a broad diversity of diseases. Prevention and screening are increasingly moving from public health into the clinic. Therefore, the question of which risk factors to investigate and disclose in the individual patient, becomes ethically increasingly urgent. In line with Wilson and Jungner's public health‐related 10 principles for screening, it seems crucial to distinguish important from unimportant health risks. Aim: to explore the ways in which clinicians distinguish important from unimportant health risks. Methods: We interviewed 36 respondents (gastroenterologists and gynaecologists/obstetrics) in 5 focus group interviews and 15 open in‐depth interviews on their interpretation of what makes a health risk important. Results: Physicians primarily interpreted importance as the severity of the possible harm, less often its probability. Possibilities of prevention or reassurance strongly influenced their judgment on importance. Discussion: It is not likely that interpreting ‘important’ as ‘severe’ will help in differentiating meaningful from meaningless risk knowledge. A more fundamental change in our ways of dealing with risk may be called for. We discuss existing literature on resilience as an alternative way to deal with risk. Balancing prevention and risk reduction with resilience could be a fruitful direction.

Ecological indicators can be defined as relatively simple measurements that relay scientific information about complex ecosystems. Such indicators are used to characterize risk in ecological risk assessment (ERA) and to mark progress toward resource management goals. In late 1997, scientists from the U.S. Environmental Protection Agency and from the Chemical Manufacturers Association (CMA) held a workshop to explore opportunities for collaborative research and scientific exchange on the development and application of ecological indicators. Several scientific challenges were identified as they relate to problem formulation, exposure and effects assessment, and risk characterization. Chief among these were a better understanding of multiple stressors (both chemical and non-chemical), characterization of reference sites and natural variability, extrapolation of measures to ecologically relevant scales, development of comprehensive, ecosystem-based models that incorporate multiple stressors and receptors, and a consistent system for evaluating ecological indicators.

This article examines the use of uncertainty factors (UFs) by the Agency for Toxic Substances and Disease Registry (ATSDR) when developing health guidance values known as minimal risk levels (MRLs) in environmental risk analysis as it applies to children. Improvements in the chemical-specific databases often reveal new information and thereby reduce uncertainty or alternatively raise new concerns. As a result, MRLs can and will change. Children, in particular, are not “small adults” and in some instances demonstrate greater risks of exposure to environmental toxicants and greater susceptibility for adverse health effects once exposed. Recent experience with risk analysis for three toxicants (organic mercury, dioxin, and manganese) is recounted to demonstrate how ATSDR has revised MRLs as the emerging science generates greater knowledge and awareness of children's special vulnerabilities to toxic substances in the environment.

Contemporary urban societies display in high relief the action of social stratification on human biology. Recent studies of biological responses to urban environments and of socioeconomically disadvantaged people indicate that culture allocates risks disproportionately to some individuals and groups within society through its constituent values and related patterns of behavior. Although risk allocation is present in all societies, it is very clear in urban environments within stratified societies where high exposure to harmful materials is many times more likely for some segments of society. In urban environments, culture may be seen as adding stressors to the environment by concentrating naturally occurring materials to levels that are toxic to humans and through the creation of new toxic materials. In stratified societies the risk of exposure to these new stressors is focused on the socioeconomically disadvantaged. This exposure has consequences that increase the likelihood of more exposure and more socioeconomic disadvantage, thereby increasing social stratification. This suggests that models of biocultural interaction include a feedback relationship in which biological factors influence the sociocultural system in addition to the usual action of the sociocultural system on biological features and responses. This model strongly reinforces the view that stressors can originate from cultural arrangements. Am J Phys Anthropol 102:67–77, 1997 © 1997 Wiley-Liss, Inc.  相似文献
