Aspiration cytology of breast cysts   总被引:1,自引:0,他引:1  
A cytologic evaluation was made of 57 samples of fluid aspirated from cystic breast masses during the past 12 years in our hospital. The cytologic diagnosis of the cyst aspirates from 17 cancer cases was positive in 10 cases, suspicious in 2 and negative in 5. No false-positive cytologic diagnosis was made in 40 cases of benign cystic disease. Blood-tinged aspirates were observed in 14 of the 17 cases of malignant cystic disease but in only 2 of 40 benign cases. Ductal cell clusters were detected in aspiration smears from 16 of the cases of malignant cystic disease and 15 of the cases of benign cystic disease. Apocrine cells were seen in the aspiration smears from 12 of the cases of benign cystic disease but in none of the aspirates from the 17 cancer cases.  相似文献

In the past 12 years aspiration cytological examination was carried out in 310 cases of histologically verified breast cancer. A classification of mammary carcinomas is presented and the cytomorphological features of cells constituting the different types of tumour are described. The value of aspiration cytology in preoperative diagnosis of early cancerous proliferations is emphasized. In the light of clinical, mammographic and cytological investigations the accuracy is about 96%. The method is reliable, quick, has no hazard, and can be performed in out-patients. Its routine application is recommended to all institutions possessing adequate facilities and staff.  相似文献

The aspiration cytology of two cases of solitary schwannoma is documented. The first tumor was retroperitoneal and cystic; the aspirated fluid showed single, regular, spindle-shaped cells with cigar-shaped nuclei and scanty wavy cytoplasm in addition to lymphocytes and foamy histiocytes. The second tumor was solid and peripheral; its aspiration smears revealed spindle-shaped cells with cigar-shaped nuclei present singly or in clusters showing nuclei in a palisading pattern.  相似文献

The distinct cytologic picture of a fine needle aspirate from a case of adult rhabdomyoma is described. The presence of cytoplasmic cross striations was a diagnostic feature that could be demonstrated in some tumor cells with all routine stains used (Papanicolaou, May-Grünwald-Giemsa and hematoxylin and eosin).  相似文献

The fine needle aspiration biopsy findings in four cases of renal oncocytomas were reviewed. The cytologic smears contained many large, well-demarcated, eosinophilic cells with prominent nucleoli. Some of the cells were binucleated. The findings in these cases indicate that renal oncocytoma may be suggested preoperatively on the bases of renal angiography and aspiration biopsy cytology. The differential diagnosis between renal adenocarcinoma and carcinomas with oncocytic differentiation is discussed.  相似文献

OBJECTIVE: To evaluate a method of obtaining conjunctival cells that utilizes aspiration of the conjunctival surface. STUDY DESIGN: Eighteen eyes from 16 patients with epibulbar lesions underwent aspiration of the conjunctival surface using a tuberculin syringe without a needle. The aspirated cells were prepared by the cytocentrifuge or Millipore filter technique. The lesions were subsequently biopsied, and the cytologic and histologic diagnoses were compared. RESULTS: Diagnoses of the conjunctival lesions, in decreasing order of frequency, were conjunctival intraepithelial neoplasia (dysplasia, carcinoma in situ), squamous cell carcinoma, squamous metaplasia, nevus, lymphoma and inflammation. In this feasibility study, the cytologic diagnosis accurately correlated with the histologic diagnosis in most cases. There were two cases in which the cytologic diagnosis led to the correct histologic diagnosis. CONCLUSION: Aspiration cytology of the conjunctival surface is useful in obtaining diagnostic material and may be used as a guide to where a biopsy should be performed. Limitations include inadequate samples and lack of observer experience.  相似文献

The cytologic features in smears of fine needle aspirates from 34 tubular breast carcinomas were studied. Uniform and bland epithelium in cohesive clusters dominated all specimens; two-thirds also showed some degree of cellular dissociation. Characteristic (but not quite pathognomonic) angular epithelial clusters with a tubular structure occurred in about 50% of the smears. Sparsely occurring atypical nuclei and/or cytoplasmic vacuoles, similar to those seen in lobular cancer, were found in half of the cases. Although a lack of myoepithelial cells was apparent in the majority of the smears, one-fourth of them showed a quite prominent myoepithelial component. Various combinations of atypical features resulted in a picture that was considered diagnostic of malignancy in 50% of the cases; the remaining specimens showed less pronounced atypia. Other pitfalls associated with the cytologic diagnosis of tubular carcinoma are discussed, and the importance of basing the management of small breast lesions on the mammographic as well as on the cytologic findings is emphasized.  相似文献

BACKGROUND: Medigstinal seminoma ia an uncommon tumor that occurs primarily in young males. We present a case of mediastinal seminoma in an elderly male diagnosed on fine needle aspiration (FNA) tytology. CASE: A 62-year-old male was admitted to our hospital because of anterior chest pain for 2 weeks. Chest computed tomography revealed a huge, lobulated mass in the anterior mediastinum. Aspirate smears were highly cellular. The tumor cells appeared singly or in loose groups of a few cells in a markedly necrotic background without lymphocytic or tigroid characteristics. Well-preserved cells were relatively uniform, large and polygonal, with round or ovoid nuclei, 2 1 prominent nucleoli and a mild to moderate amount of clear, occasionally vacuolated cytoplasm. The chromatin wasfinely granular. Mitotic figures were frequently found. The cell block section showed solid nests of tumor cells with cytologic features similar to those in the smears, supported by loose, fibrous stroma infiltrated by some lymphocytes. Immunocytochemically the tumor showed diffuse reactivity for c-kit and appearedfocally positive for placentalike alkaline phosphatase and was uniformly negative for pancytokeratin, CAM 5.2 low-molecular-weight cytokeratin, CD5, CD30, alpha-fetoprotein and leukocyte common antigen. CONCLUSION: A confident diagnosis of mediastinal seminoma can be established by FNA cytology, cell block and immunocytochemistry.  相似文献

Histochemical methods play a vital role in the histology laboratory. The application of histochemistry to aspiration biopsy cytology (ABC) has shown equal promise in the demonstration of distinctive intracytoplasmic and extracytoplasmic substances. In our examination of more than 8,000 fine needle aspirates, we have found that certain histochemical studies may be of special diagnostic importance. These include mucicarmine, periodic acid-Schiff, oil red O, Fontana-Masson argentaffin and Gomori's iron stains. With the exception of the oil red O stain for neutral lipids (alcohol soluble), each may be applied to the destained smear initially prepared according to the method of Papanicolaou. Modifications of these histologic stains have been made to suit the "rapid service" motto of our busy cytology laboratory.  相似文献

BACKGROUND: Glomus tumor is a relatively uncommon soft tissue tumor that can occur at any age and anatomic site, with a predilection for the subungual region. CASE: A 24-year-old female presented with a gradually enlarging, painful swelling in the subungual region of the right index finger. Fine needle aspiration was performed under a ring block and yielded hemorrhagic material. The smears revealed clusters of uniform, round to oval cells admixed with wisps of magenta intercellular myxoid material and a few spindle-shaped cells crossing clusters of tumor cells. A diagnosis of benign vascular tumor consistent with glomus tumor was given. Subsequent histopathologic examination confirmed the diagnosis and showed alpha-SMA positivity on immunohistochemistry. CONCLUSION: This case report is the second on the cytologic features of glomus tumor in the English-language literature. Although the cytomorphologic features of glomus tumor are quite distinctive, an appropriate clinical history and immunohistochemical stains (e.g., alpha-SMA and vimentin positivity) can further help to ascertain the diagnosis.  相似文献

Unlike other gynecologic tumors, which are accessible to inspection and biopsy, ovarian neoplasms are located within the pelvis or abdomen and are reachable only by laparoscopy or exploratory laparotomy. Because of its simplicity, fine needle aspiration cytology may be used as an alternative to surgical procedures. However, because of the great variety of tumor types in the ovary, cytologic interpretation of aspirates from that organ represents one of the most challenging fields in diagnostic cytology. Nevertheless, as clinicians become more experienced in using this technique and pathologists gain confidence in their ability to evaluate samples correctly, aspiration cytology may prove to be one of the most valuable and acceptable tools in the diagnosis of tumors of the female pelvis.  相似文献

Fine needle aspiration (FNA) of a thyroid mass clinically suspected of being acute thyroiditis led to a cytologic diagnosis of hypernephroma metastatic to the thyroid and to the subsequent detection of the occult primary tumor. The FNA cytomorphologic findings were substantiated by cytochemical staining of FNA samples and confirmed by subsequent histopathologic examination of the resected thyroid. Postoperative studies revealed an expansive growth in the left kidney; analysis of the nephrectomy specimen showed an invasive renal-cell carcinoma. This case emphasizes the considerable value of FNA biopsy in making the frequently difficult preoperative differential diagnosis of primary and metastatic thyroid tumor and the importance of cytochemical analyses in making that distinction and in suggesting the site of the primary tumor.  相似文献

Eighty-seven fine needle aspiration cytological samples from 29 patients suffering from irregular perimenopausal uterine bleeding were evaluated. Aspiration cytology was performed prior to hysteroscopy, after distension of the uterine cavity and finally after uterine curettage. In this paper, cytological examination of fluid from the pelvic content in women with benign endometrial findings is compared with that of patients with adenocarcinoma. Endometrial curettage influenced the cell content of the cytologica specimens.  相似文献
