A dynamic model for finger interphalangeal coordination   总被引:2,自引:1,他引:2  
In this paper a dynamic model to investigate interphalangeal coordination in the human finger is proposed. Suitable models which describe the relationship between the tendon displacement and the joint angles have been chosen and incorporated into the skeletal dynamic model. A kinematic and kinetic model for interphalangeal coordination is suggested. Digital computer simulations are carried out to study interphalangeal (IP) flexion. Moreover, the effect of two different optimization methods is contrasted. The two optimization algorithms are employed to obtain a set of feasible values for the forces in the tendons or muscles of the finger.  相似文献

In order to perform cadaveric biomechanical studies of the human forearm and distal radio-ulnar joint, a dynamic simulator has been constructed. The device is based upon a Plexiglas frame, to which the ulna is secured in a vertical orientation and the humerus in a horizontal orientation. The hand is secured in a sliding bar linkage to a stepper-motor that is used to rotate the forearm. The tendons to be loaded are connected to pneumatic actuators that provide agonist and antagonist muscle loading resulting in torque along the forearm axis. The muscle loading profiles and magnitudes are programmable as a function of the pronation-supination position and direction. A magnetic tracking system is used to collect three-dimensional kinematics data of up to four segments, in conjunction with the muscle tendon loads, forearm torque and other prescribed experimental measures. All functions are under PC control using custom software written with LabVIEW (National Instruments, Austin, TX). For the DRUJ testing, the validity of the tendon loading protocol to produce physiologic torque/rotation patterns was verified using in vivo data. The relationship of individual muscle forces to forearm torque was determined by a cadaveric study.  相似文献

Twenty-six nail deformities secondary to ganglions of the distal interphalangeal joint were retrospectively reviewed to assess the important aspects of their management. The patients' ages ranged from 41 to 79 years. The long and index fingers were most commonly involved. A depression or groove was present in 23 of 26 digits reviewed. Two had gross disruption of the nail. Fifty-eight percent of the cysts had spontaneously drained or had been drained by the patient or a physician preoperatively. Degenerative arthritic changes were seen in 87 percent of those with x-rays or a radiology report available. Most underwent surgical removal of the cyst and debridement of associated osteophytes of the distal interphalangeal joint. The cyst was located above the germinal matrix in all but two digits. Osteophytes were found in all 20 digits in which the joint was explored. No recurrences were seen in those available for postoperative follow-up (22 of 25). Normal nail growth was found in 14 of 22, although follow-up was short in one. All eight postoperative nail deformities were quite mild and of little concern to the patient. There was no correlation between preoperative cyst drainage and aesthetic postoperative nail growth. Nail removal at the time of surgery appeared to be unnecessary unless the nail was grossly disrupted.  相似文献

Traditionally, toe-to-hand transfers have been reserved for thumb amputations or for use after severe mutilating injuries. The authors report their experience with the use of second toe-for-finger amputations with preserved or reconstructible proximal interphalangeal joints in manual workers. The aim of the procedure was to reduce impairment and to upgrade the hand from a functional and cosmetic standpoint. Fifteen second-toe wrap-around or variations were carried out on 11 adults (18 to 41 years old). Four patients with two or more finger amputations received two sequential second toes; four patients with two finger amputations received one toe; and each of three patients with single-digit amputation received a single toe. All but one amputation were performed less than 3 weeks after the accident. All toes survived. Range of motion at the native proximal interphalangeal joint was more than 90 percent in all patients but one; however, it was minimal at the transplanted joints. Patient satisfaction was high from a cosmetic and functional standpoint. Ten of 11 laborers resumed their previous activity. On the basis of this experience, a classification with aesthetic and functional implications is proposed to help in the decision-making process when dealing with multidigital injuries. It is concluded that second-toe transfer is an excellent choice for finger amputation distal to the proximal interphalangeal joint in laborers. Its prime indication is for amputations of two fingers where at least one toe should be transferred, as required, to achieve an "acceptable hand" (three-fingered hand). Early transfer allows salvage of critical structures from the damaged finger, such as joints, tendons, and bone, that otherwise would be lost. Early transplantation is highly recommended.  相似文献

Data are reported for a series of 20 patients who were treated with the pins and rubbers traction system for fractures of the proximal interphalangeal joints of the long fingers. This technique allows fracture reduction with external dynamic traction and immediate active mobilization. Two patients in the series were lost to follow-up monitoring. For two others, the pins and rubbers traction system needed to be removed early (during the first week) because of intolerance or infection. Sixteen patients who were reexamined after minimal follow-up periods of 1 year demonstrated a mean active range of motion of 85.9 degrees for the injured joint; only one patient experienced intermittent pain.  相似文献

An instrumented,dynamic test for anterior laxity of the ankle joint complex   总被引:1,自引:0,他引:1  
Evaluation of anterior laxity of the ankle joint complex is a difficult clinical problem. Currently, the prime determinant for anterolateral ligament function is the subjective manual examination of anterior laxity of the ankle joint complex. An instrumented dynamic test was developed for objective measurement of anterior laxity of the ankle joint complex. The principle of the test was to apply a force-impulse to the calcaneus, within the muscle reflex time, and to measure anterior–posterior and mediolateral rotation. The test was performed on a cadaver specimen and on 15 volunteers of which five subjects suffered from chronic one-sided lateral ankle ligament instability.

In the cadaver test, anterior translation values increased from 5 to 11 mm, after cutting the anterior talofibular ligament and subsequently cutting the calcaneofibular ligament. In the 10 normal subjects, the mean anterior translation value was 6.7 mm (±1.9 mm). The relative variation of the test result within a measurement session was 2.5% (±1.6%). Between the sessions the relative laxity variation was 2.6% (±2.6%). In the ten normal subjects the mean right–left difference was not significantly different from zero. In four out of the five patients it was more than 2 mm. As in the cadaver test in all measurements, the mediolateral rotations were small (<2.5°). The volunteers complained about same pain at the heel after multiple test sessions.

In conclusion the dynamic, functional test appears to be capable of objectively measuring a value for anterior laxity of the ankle joint complex reflecting the functional status of the anterolateral ankle ligaments.  相似文献

With the aim to go deep into the knowledge of the morpho-functional anatomical characteristics of the temporo-mandibular joint in humans, a dynamic method of study by means of a computerized analyzer of images is suggested. The acquired advantages are the following: a) the accuracy of evaluation of the chosen morphometric parameters; b) the working speed, from which results: c) the possibility to increase adequately the number of cases and d) the possibility to easily investigate many parameters with a very high accuracy of the quantitative results. Both right and left temporo-mandibular joints of adult individuals aged from 18 to 53 have been studied utilizing lateral tomographies focused at 3.3 mm to the lateral surface of the condylar head. The evaluations were done both in the position of completely closed mouth and in extreme opening. From the barycentre of the condyle several straight lines were drawn according to the figure 2. The length of the segment a-b (distance of profiles of the condyle and mandibular fossa) were evaluated in all the lines counter-clockwise and the results submitted to a statistical analysis. The results furnish very good information on the normal or pathological anatomical characteristics, of the joint.  相似文献

The paper describes 42 intact, 14 damaged and 52 ruptured radioulnar joints. The joints width was measured, the radioulnar index calculated, x-ray pictures filtered. Variants of isolated injuries of the joints in different bone dislocation, changes in combined ruptures and injuries are described. The radioulnar index proved the best in quantitation of the joint damage.  相似文献

A detailed 3D anatomical model of the patellofemoral joint was developed to study the tracking, force, contact and stability characteristics of the joint. The quadriceps was considered to include six components represented by 15 force vectors. The patellar tendon was modeled using four bundles of viscoelastic tensile elements. Each of the lateral and medial retinaculum was modeled by a three-bundle nonlinear spring. The femur and patella were considered as rigid bodies with their articular cartilage layers represented by an isotropic viscoelastic material. The geometrical and tracking data needed for model simulation, as well as validation of its results, were obtained from an in vivo experiment, involving MR imaging of a normal knee while performing isometric leg press against a constant 140 N force. The model was formulated within the framework of a rigid body spring model and solved using forth-order Runge-Kutta, for knee flexion angles between zero and 50 degrees. Results indicated a good agreement between the model predictions for patellar tracking and the experimental results with RMS deviations of about 2 mm for translations (less than 0.7 mm for patellar mediolateral shift), and 4 degrees for rotations (less than 3 degrees for patellar tilt). The contact pattern predicted by the model was also consistent with the results of the experiment and the literature. The joint contact force increased linearly with progressive knee flexion from 80 N to 210 N. The medial retinaculum experienced a peak force of 18 N at full extension that decreased with knee flexion and disappeared entirely at 20 degrees flexion. Analysis of the patellar time response to the quadriceps contraction suggested that the muscle activation most affected the patellar shift and tilt. These results are consistent with the recent observations in the literature concerning the significance of retinaculum and quadriceps in the patellar stability.  相似文献

Experimental interruption of the root stele of Pisum sativum L. induces in the cortex tissue the development of wound-sieve tubes which bridge the wound and reconnect the vascular stumps. Outside the stele, sieve plates arise from primary pit fields. This origin is confirmed by the distribution of future sieve pores over the original parenchyma cell wall and by remnants of the pitfield cavity in developing sieve plates. Differentiation of wound-sieve elements is similar to that of bundle-sieve elements and includes the chromatolytic disintegration of nuclei as well as the development of typical sieve pores arising from pit-field plasmodesmata. The completion of first woundsieve tubes (indicated by a continuous chain of anilin-blue-positive sieve plates by-passing the wound) was observed 55–62 h after wounding. However, effective translocation, visualized with fluoresceine as a phloem-mobile marker, was not found until 10 h (on average) later. It is suggested that this time delay corresponds to the maturing of the last link within a chain of wound-sieve-tube members.
