Background

The association between obesity in adults and excess morbidity and mortality is well established, but the health impact throughout adult life of being obese in early adulthood needs elucidation. We investigated somatic morbidity, including fatal morbidity, throughout adulthood in men starting adult life as obese.

Methods

Among 362,200 Danish young men, examined for military service between 1943 and 1977, all obese (defined as BMI≥31.0 kg/m2), and, as controls, a random 1% sample of the others was identified. In the age range of 18–25 years, there were 1,862 obese, which encompass the men above the 99.5 percentile, and 3,476 controls. Information on morbidity was obtained via national registers. Cox regression models were used to estimate the relative morbidity assessed as first incidence of disease, occurrence of disease in the year preceding death and prevalent disease at time of death.

Results

From age 18 through 80 years the obese had an increased risk of becoming diseased by or die from a broad range of diseases. Generally, the incidence of first event, occurrence in the year prior to death, and prevalence at time of death showed the same pattern. As an example, the relative hazard of type 2 diabetes was constant throughout life at 4.9 (95% confidence intervals [CI]: 4.1–5.9), 5.2 (95% CI: 3.6–7.5), and 6.8 (95% CI: 4.6–10.1), respectively.

Conclusions

Our findings strongly support the continued need to avoid beginning adult life as obese, as obese young men experience an increased morbidity, including fatal morbidity, from many diseases throughout life.  相似文献

There is an immense philosophical literature dealing with the notions of normativity and agency, as well as a sizeable and rapidly growing scientific literature on the topic of autonomous agents. However, there has been very little cross-fertilization between these two literatures. As a result, the philosophical literature tends to assume a somewhat outdated mechanistic image of living things, resulting in a quasi-dualistic picture in which only human beings, or the higher animals, can be normative agents properly speaking. From this perspective, the project of 'naturalizing normativity' becomes almost a contradiction in terms. At the same time, the scientific literature tends to misuse 'normativity,' 'agency,' and related terms, assuming that it is meaningful to ascribe these concepts to 'autonomous agents' conceived of as physical systems whose behavior is to be explained in terms of ordinary physical law. From this perspective, the true depth of the difficulty involved in understanding what makes living systems distinctive qua physical systems becomes occluded. In this essay, I begin the attempt to remedy this situation. After some preliminary discussion of terminology and situating of my project within the contemporary philosophical landscape, I make a distinction between two different aspects of the project of naturalizing normativity: (1) the 'Scope Problem,' which consists in saying how widely in nature our concept of normative agency may properly be applied; and (2) the 'Ground Problem,' which consists in rationalizing the phenomenon of normative agency in terms of the rest of our knowledge of nature. Then, in the remainder of this paper, I argue that the Scope Problem ought to be resolved in favor of attributing normative agency, in the proper sense of those words, to living things as such. Cold is the fiercest and most widespread enemy of life on earth. Natural cold adaptation and survival are discussed in terms of physicochemical and biochemical water management mechanisms, relying on thermodynamic or kinetic stabilization. Distinctions are drawn between general effects of low temperature (chill) and specific effects of freezing. Freeze tolerance is a misnomer because tolerance does not extend to the cell fluids. Freezing is confined to the extracellular spaces where it acts as a means of protecting the cytoplasm against freezing injury. Freeze resistance depends on the phenomenon of undercooling, a survival mechanism that relies on the long-term maintenance of a thermodynamically highly unstable state. Correct water management involves many factors, among them the control of membrane composition and transmembrane osmotic equilibrium, the biosynthesis of compounds able to afford protection against injury through freeze desiccation and the availability (or inactivation) of biogenic ice nucleation catalysts.  相似文献

Living organisms are unique in their ability to generate and replicate ordered systems from disordered components. Generation of order, replication of the individual, and evolution of the species all depend on the successful utilization of external energy derived from chemicals and light. The information for reproduction is encoded in nucleic acids, but evolution depends on a limited variability in replication, and proceeds through the selection of individuals with altered biochemistry. Essentially all biochemistry is catalyzed; therefore, altered biochemistry implies altered or new catalysts. In that sense catalysis is the medium of evolution. We propose that a basic property of enzymes, at least as fundamental as reaction rate enhancement, is to adjust the reaction path by altering and eventually optimizing the reversible interchange of chemical, electrical and mechanical energy among themselves and their reactants.  相似文献

No other member of the VEGF (vascular endothelial growth factor) family has been as mysterious as VEGF-B. Notwithstanding its name, VEGF-B can hardly be regarded as a growth factor because growth occurs fairly normally in Vegf-b deficient mice. Moreover, VEGF-B is barely angiogenic under most conditions, although it was expected to be an angiogenic factor for a long time. Under certain conditions, VEGF-B has been shown to be involved in blood vessel growth. Under other conditions, however, VEGF-B can act to inhibit tumor growth and angiogenesis. Given these contradictory findings, the biological function of VEGF-B appears enigmatic. In this review, we summarize recent advances in VEGF-B biology and discuss its multifaceted roles, the underlying mechanisms, and the potential therapeutic implications.  相似文献

This article introduces a new model of the relationship between growth and learning and tests a set of hypotheses related to the development of adult competency using time allocation, anthropometric, and experimental task performance data collected between 1992 and 1997 in a multiethnic community in the Okavango Delta, Botswana. Building on seminal work in life history theory by Hawkes, Blurton Jones and associates, and Kaplan and associates, the punctuated development model presented here incorporates the effects of both growth and learning constraints on age-specific task performance. In addition, the payoff to investment in two forms of embodied capital, growth-based and learning-based, are examined in relation to features of the socioecology, including subsistence economy and family composition. The three main findings are:
1.  The development of adult competency in specific tasks entails a steplike relationship between growth- and experience-based forms of embodied capital in the ontogeny of ability acquisition.
2.  There is a trade-off between the acquisition of experience-based embodied capital in the form of skills and knowledge and immediate productivity among children. Time allocation to these alternatives is primarily determined by the short- and long-term costs and benefits to parents of investment in children’s embodied capital.
3.  The availability of laborers and the overall labor requirements of the household are major determinants of investment in alternate forms of embodied capital and resulting variation in children’s time allocation. The value of children’s labor to their parents is dependent upon the opportunity costs to engaging in other activities not only for the child in question but also for potential substitute laborers.

In animal cell, mitochondria are the main sites of the synthesis of ATP required for cell functioning and survival. On the other hand, mitochondria play a key role in initiating cell programmed death (apoptosis). In addition, defects in the mitochondrial genome and in the nuclear genome encoding mitochondrial proteins may result in malfunctioning of these organelles and, as result, in diseases of the whole organism. This article contains basic information on the functioning of oxidative phosphorylation and on mitochondrial production of reactive oxygen species. A large body of literature argues that marriage promotes health and increases longevity. But do these benefits extend to maintaining a healthy body weight, as the economic theory of health investment suggests they should? They do not. Using the Health and Retirement Study (HRS), I find that entry into marriage among both men and women aged 51–70 is associated with weight gain and exit from marriage with weight loss. I evaluate three additional theories with respect to the cross-sectional and longitudinal variation in the data. First, it may be that a broader set of shared risk factors (such as social obligations regarding meals) raises body mass for married couples. However, the shared risk factor model predicts that the intra-couple correlation should increase with respect to marital duration. Instead, it declines. Second, scholars have recently promoted a “crisis” model of marriage in which marital transitions, not marital status, determine differences in body mass. The crisis model is consistent with short-term effects seen for divorce, but not for the persistent weight gains associated with marriage or the persistent weight loss following widowhood. And transition models, in general, cannot explain significant cross-sectional differences across marital states in a population that is no longer experiencing many transitions, nor can it account for the prominent gender differences (in late middle-age, the heaviest group is unmarried women and the lightest are unmarried men). Third, I argue that pressures of the marriage market, in combination with gendered preferences regarding partner BMI, can account for all the longitudinal and cross-sectional patterns found in the data.  相似文献
