Aims This study aimed to examine the effectiveness of a primary care adult mental health service operating within a stepped care model of service delivery.Methods Supervised by a principal psychologist manager, psychology graduate practitioners provided one-to-one brief cognitive behavioural therapy (CBT) to service users. The Clinical Outcomes in Routine Evaluation-Outcome Measure (CORE-OM) was used to assess service user treatment outcomes. Satisfaction questionnaires were administered to service users and referring general practitioners (GPs).Results A total of 43 individuals attended for an initial appointment, of whom 19 (44.2%) completed brief CBT treatment. Of the 13 service users who were in the clinical range pre-treatment, 11 (84.6%) achieved clinical and reliably significant improvement. Of the six service users who were in the non-clinical range pre-treatment, three (50%) achieved reliably significant improvement. Both service users and GPs indicated high levels of satisfaction with the service, although service accessibility was highlighted as needing improvement.Conclusion The service was effective in treating mild to moderate mental health problems in primary care. Stricter adherence to a stepped care model through the provision of low-intensity, high-throughput interventions would be desirable for future service provision.  相似文献

In cloud computing, service providers offer cost-effective and on-demand IT services to service users on the basis of Service Level Agreements (SLAs). However the effective management of SLAs in cloud computing is essential for the service users to ensure that they achieve the desired outcomes from the formed service. In this paper, we introduce a SLA management framework that will enable service users to select the best available service provider on the basis of its reputation and then monitor the run time performance of the service provider to determine whether or not it will fulfill its promise defined in the SLA. With the advances of network function virtualization and cloud computing technologies, a number of network services are implemented across data centers by creating a service chain using different virtual network functions (VNFs) running on virtual machines. Due to the complexity of network infrastructure, creating a service chain requires high operational cost especially in carrier-grade network service providers and supporting stringent QoS requirements from users is also a complicated task. There have been various research efforts to address these problems that only focus on one aspect of optimization goal either from users such as latency minimization and QoS based optimization, or from service providers such as resource optimization and cost minimization. However, meeting the requirements both from users and service providers efficiently is still a challenging issue. This paper proposes a VNF placement algorithm called VNF-EQ that allows users to meet their service latency requirements, while minimizing the energy consumption at the same time. The proposed algorithm is dynamic in a sense that the locations or the service chains of VNFs are reconfigured to minimize the energy consumption when the traffic passing through the chain falls below a pre-defined threshold. We use genetic algorithm to formulate this problem because it is a variation of the multi-constrained path selection problem known as NP-complete. The benchmarking results show that the proposed approach outperforms other heuristic algorithms by as much as 49% and reduces the energy consumptions by rearranging VNFs.  相似文献

The rapid growth of published cloud services in the Internet makes the service selection and recommendation a challenging task for both users and service providers. In cloud environments, software re services collaborate with other complementary services to provide complete solutions to end users. The service selection is performed based on QoS requirements submitted by end users. Software providers alone cannot guarantee users’ QoS requirements. These requirements must be end-to-end, representing all collaborating services in a cloud solution. In this paper, we propose a prediction model to compute end-to-end QoS values for vertically composed services which are composed of three types of cloud services: software (SaaS), infrastructure (IaaS) and data (DaaS) services. These values can be used during the service selection and recommendation process. Our model exploits historical QoS values and cloud service and user information to predict unknown end-to-end QoS values of composite services. The experiments demonstrate that our proposed model outperforms other prediction models in terms of the prediction accuracy. We also study the impact of different parameters on the prediction results. In the experiments, we used real cloud services’ QoS data collected using our developed QoS monitoring and collecting system.  相似文献

Objectives To understand how the policy of user involvement is interpreted in health service organisations and to identify factors that influence how user involvement is put into practice.Design Ethnographic study using participant observation, interviews, and collection of documentary evidence.Setting A multiagency modernisation programme to improve stroke services in two London boroughs.Participants Service users, National Health Service managers, and clinicians.Results User involvement in the programme was initiated and led by professionals. Professionals determined the areas of service improvement service users could participate in. A wide range of activities were considered “user involvement,” from patient satisfaction surveys to service users delivering peer support. Involvement tended to be most active in the least technical areas and areas with least input from clinicians. Factors that might explain this included organisational structure, the vagueness of the concept of user involvement, the value attributed to service users’ experiential knowledge, and variations in professional and service user understandings of and commitment to involvement. The gains of involvement were harder to identify in terms of impact on services. More evident were the personal gains for those involved: satisfaction of feeling listened to by professionals, social opportunities of meeting others in a similar situation, and increased knowledge about stroke and services available.Conclusions User involvement may not automatically lead to improved service quality. Healthcare professionals and service users understand and practise user involvement in different ways according to individual ideologies, circumstances, and needs. Given the resource implications of undertaking user involvement in service development there is a need for critical debate on the purpose of such involvement as well as better evidence of the benefits claimed for it.  相似文献

The National Childbirth Trust, along with other groups of health service users, is working with health professionals and researchers in planning clinical trials, setting priorities for research, systematically reviewing research reports, and getting research findings into practice. User groups may bridge the gap between the public and researchers by explaining research issues to a wide audience, presenting the needs and views of health service users to the research community, and suggesting how members of the public may be approached for their views directly. Service users recognise their need for training and support, and they call for development and evaluation of this work.  相似文献

Background

Poor service user experiences are often reported on mental health inpatient wards. Crisis houses are an alternative, but evidence is limited. This paper investigates therapeutic alliances in acute wards and crisis houses, exploring how far stronger therapeutic alliance may underlie greater client satisfaction in crisis houses.

Methods and Findings

Mixed methods were used. In the quantitative component, 108 crisis house and 247 acute ward service users responded to measures of satisfaction, therapeutic relationships, informal peer support, recovery and negative events experienced during the admission. Linear regressions were conducted to estimate the association between service setting and measures, and to model the factors associated with satisfaction. Qualitative interviews exploring therapeutic alliances were conducted with service users and staff in each setting and analysed thematically.

Results

We found that therapeutic alliances, service user satisfaction and informal peer support were greater in crisis houses than on acute wards, whilst self-rated recovery and numbers of negative events were lower. Adjusted multivariable analyses suggest that therapeutic relationships, informal peer support and negative experiences related to staff may be important factors in accounting for greater satisfaction in crisis houses. Qualitative results suggest factors that influence therapeutic alliances include service user perceptions of basic human qualities such as kindness and empathy in staff and, at service level, the extent of loss of liberty and autonomy.

Conclusions and Implications

We found that service users experience better therapeutic relationships and higher satisfaction in crisis houses compared to acute wards, although we cannot exclude the possibility that differences in service user characteristics contribute to this. This finding provides some support for the expansion of crisis house provision. Further research is needed to investigate why acute ward service users experience a lack of compassion and humanity from ward staff and how this could be changed.  相似文献

Optimizing and monitoring the data flow in high-throughput sequencing facilities is important for data input and output, for tracking the status of results for the users of the facility, and to guarantee a good, high-quality service. In a multi-user system environment with different throughputs, each user wants to access his/her data easily, track his/her sequencing history, analyze sequences and their quality, and apply some basic post-sequencing analysis, without the necessity of installing further software. Recently, Fiocruz established such a core facility as a "technological platform". Infrastructure includes a 48-capillary 3730 DNA Sequence Analyzer (Applied Biosystems) and supporting equipment. The service includes running samples for large-scale users, performing DNA sequencing reactions and runs for medium and small users, and participation in partial or full genome projects. We implemented a workflow that fulfills these requirements for small and high throughput users. Our implementation also includes the monitoring of data for continuous quality improvement (reports by plate, month and user) by the sequencing staff. For the user, different analyses of the chromatograms, such as visualization of good quality regions, as well as processing, such as comparisons or assemblies, are available. So far, 180 users have made use of the service, generating 155,000 sequences, 35% of which were produced for the BCG Moreau-RJ genome project. The narcotization level of the country is sufficiently high. According to some expert evaluations, the number of drug users varies between 2-4 million persons. Of these, about 100,000 are registered in narcotics service of the Ministry of Health of the Russian Federation and about 250,000 are registered in the system of the Ministry of Internal Affairs of the Russian Federation. The main annual increase in drug users is given by unemployed young people and students (more than 15% of them are minors). An important task standing at present before the specialists of the service responsible for the prophylaxis of HIV infection is the steady spread of information among executive and legislative authorities, as well as the preparation of public opinion, on the necessity for the state to choose and accept the optimum variant of the strategy for the prophylaxis of HIV infection among drug users.  相似文献

Due to the restrictions that most traditional scheduling strategies only cared about users’ quality of service (QoS) time or cost requirements, lacked the effective analysis of users’ real service demand and could not guarantee scheduling security, this paper added trust into workflow’s QoS target and proposed a novel customizable cloud workflow scheduling model. In order to better analyze different user’s service requirements and provide customizable services, the new model divided workflow scheduling into two stages: the macro multi-workflow scheduling as the unit of cloud user and the micro single workflow scheduling. It introduced trust mechanism into multi-workflow scheduling level. And in single workflow scheduling level, it classified workflows into time-sensitive, cost-sensitive and balance three types according to different workflow’s QoS demand parameters using fuzzy clustering method. Based on it, it customized different service strategies for different type. The simulation experiments show that the new schema has some advantages in shortening workflow’s final completion time, achieving relatively high execution success rate and user satisfaction compared to other kindred solutions.  相似文献

利用描述性综述的方法,总结近10年来医疗服务绩效信息透明对医疗质量促进效果,根据纳入排除标准检索Web of Knowledge、Pubmed等数据库,最终纳入31篇密切相关文献。结果表明,国外一般通过医疗报告卡或网站向公众公开医疗服务绩效信息,改善了医疗服务行为,但对病人选择服务提供者和临床结果的改善作用有限。我国应制定相关政策,推动医疗服务绩效信息向更广泛的使用者公开,引导病人利用医疗服务绩效信息选择医疗服务提供者。  相似文献

Recent years have witnessed the tremendous growth of the online social media. In China, Weibo, a Twitter-like service, has attracted more than 500 million users in less than five years. Connected by online social ties, different users might share similar affective states. We find that the correlation of anger among users is significantly higher than that of joy. While the correlation of sadness is surprisingly low. Moreover, there is a stronger sentiment correlation between a pair of users if they share more interactions. And users with larger number of friends possess more significant sentiment correlation with their neighborhoods. Our findings could provide insights for modeling sentiment influence and propagation in online social networks.  相似文献

Cloud computing environment came about in order to effectively manage and use enormous amount of data that have become available with the development of the Internet. Cloud computing service is widely used not only to manage the users’ IT resources, but also to use enterprise IT resources in an effective manner. Various security threats have occurred while using cloud computing and plans for reaction are much needed, since they will eventually elevate to security threats to enterprise information. Plans to strengthen the security of enterprise information by using cloud security will be proposed in this research. These cloud computing security measures must be supported by the governmental policies. Publications on guidelines to information protection will raise awareness among the users and service providers. System of reaction must be created in order to constantly monitor and to promptly respond to any security accident. Therefore, both technical countermeasures and governmental policy must be supported at the same time. In this paper, we discuss strategies for providing World Wide Web service users with adequate Quality of Service (QoS). We argue that QoS can be provided by distributing the service requests processing load among replicated Web servers (WSs), that can be geographically distributed across the Internet. In order to support our argument, we compare and contrast several load distribution strategies, and assess their effectivness when deployed within the context of a geographically replicated Web service; the principal figure of merit we use in this assessment is the response time experienced by the users of that service. As a result of this comparison, we propose a specific strategy, named QoS-based, that implements load distribution among WS replicas by binding a user to the replica that provides the shortest user response time. We examine several architectures that exploit our QoS-based strategy. Two of these architectures, named, respectively, Browser-based and Load Distribution-based, are described in detail as they are particularly appropriate for implementing our strategy.  相似文献

Seventy five consecutive users of the Leeds disabled living foundation were surveyed. Two thirds of these users were at least moderately severely disabled. Forty eight of the 65 people (74%) who completed the questionnaire were recommended aids, and 33 had received some at the end of the survey. Only 10 of 28 people had received recommended adaptations. Cloud computing, an on-demand computation model that consists of large data-centers (Clouds) managed by cloud providers, offers storage and computation needs for cloud users based on service level agreements (SLAs). Services in cloud computing are offered at relatively low cost. The model, therefore, forms a great target for many applications, such as startup businesses and e-commerce applications. The area of cloud computing has grown rapidly in the last few years; yet, it still faces some obstacles. For example, there is a lack of mechanisms that guarantee for cloud users the quality that they are actually getting, compared to the quality of service that is specified in SLAs. Another example is the concern of security, privacy and trust, since users lose control over their data and programs once they are sent to cloud providers. In this paper, we introduce a new architecture that aids the design and implementation of attestation services. The services monitor cloud-based applications to ensure software quality, such as security, privacy, trust and usability of cloud-based applications. Our approach is a user-centric approach through which users have more control on their own data/applications. Further, the proposed approach is a cloud-based approach where the powers of the clouds are utilized. Simulation results show that many services can be designed based on our architecture, with limited performance overhead.  相似文献

We describe two additional services now available as part of the previously described Dynamite protein dynamics web service. Dynatraj provides principle component analysis and visualization of modes of motion for a user's own ensemble of protein structures, e.g. from Molecular Dynamics, NMR or experimental ensembles. Dynapocket predicts probable configurations of a protein pocket from a single known structure.
