Studies are being conducted in our laboratory to test the concept of introducing an elastomer to attenuate and damp forces applied to the bone interface in a major weightbearing joint replacement prosthesis. An analogue of a fully constrained intramedullary stem type prosthesis has been developed in a segmental femoral replacement prosthesis of the dog. The layer of silastic was introduced to damp forces at the bone-prosthesis interface. This paper describes the response of this elastomer prosthesis to torsional and bending loads, and defines the upper limits of elastomer strain. The low modulus silastic displayed surprisingly low strain for applied loads, particularly in bending tests, in this prosthetic configuration. The results of these mechanical studies serve as a bench mark for the eventual design and material selection of an elastomer for human prosthetic use.  相似文献

Analysis of a femoral hip prosthesis designed to reduce stress shielding   总被引:11,自引:0,他引:11  
The natural stress distribution in the femur is significantly altered after total hip arthroplasty (THA). When an implant is introduced, it will carry a portion of the load, causing a reduction of stress in some regions of the remaining bone. This phenomenon is commonly known as stress shielding. In response to the changed mechanical environment the shielded bone will remodel according to Wolff's law, resulting in a loss of bone mass through the biological process called resorption. Resorption can, in turn, cause or contribute to loosening of the prosthesis. The problem is particularly common among younger THA recipients. This study explores the hypothesis that through redesign, a total hip prosthesis can be developed to substantially reduce stress shielding. First, we describe the development of a new femoral hip prosthesis designed to alleviate this problem through a new geometry and system of proximal fixation. A numerical comparison with a conventional intramedullary prosthesis as well as another proximally fixed prosthesis, recently developed by Munting and Verhelpen (1995. Finite element stress analyses were conducted of the canine femoral head before and after implantation of various surface replacement-type components. The femoral head was replaced by four implant geometries; (a) shell, (b) shell with peg, (c) shell with rod, and (d) a new epiphyseal replacement design. All implants were modelled to simulate bony ingrowth along the underside of the shell and along the surfaces of the peg and rod. The results indicated that in the normal femur the forces are transferred from the articular surface through the femoral head cancellous bone to the inferior cortical shell of the femoral neck. After shell-type surface replacement, forces were transferred more distally at the rim of the shell and at the end of the peg or rod, thereby reducing the stresses in the proximal head cancellous bone. Computer simulation of bone remodelling due to proximal bone stress reduction was shown to accentuate the abnormality of the stress fields. Surface replacement with a lower modulus material created a less abnormal redistribution of bone stresses. The new epiphyseal replacement design resulted in stress distributions similar to those in the normal femoral head and minimal shear stresses at the implant/bone interface. These findings suggest that the epiphyseal replacement concept may provide better initial mechanical integrity and create a more benign milieu for adaptive bone remodelling than conventional, shell-type surface replacement components.  相似文献

Previous attempts aimed at reproducing clinically observed bone adaptations in response to the implant have not recreated the full set of common radiographic changes, so a modified bone adaptation algorithm was developed in an attempt to replicate more closely the effects of the prosthesis on the host bone. The algorithm features combined implant–bone interface healing and continuum bone remodelling. It was observed that remodelling simulations that accounted for progressive gap filling at the implant–bone interface predicted the closest periprosthetic bone density changes to clinical X-rays and DEXA data. This model may contribute to improved understanding of clinical failure mechanisms with traditional hip resurfacing designs and enable more detailed pre-clinical analysis of new designs.  相似文献

The presence of a femoral prosthesis superior to a shaft fracture severely complicates fixation and treatment. This study uses two-dimensional, multithickness, plane stress finite-element models of a femur with prosthesis to investigate the stresses developed with the application of three popular fixation techniques: revision to a long stem prosthesis, lateral plating with a cortical bone allograft strut and cerclage wires, and custom plate application with proximal Parham band fixation with distal cortical screws (Ogden plate). The plate and bone contact as well as the fracture site contact were modelled by using orthotropic elements with custom-fit moduli so that only the normal stress to the interface was significant. A thermal analogy was used to model the cerclage and Parham band preloads so that representative preloads in the proximal fixation of the two types of plate treatments could be modelled. A parametric study was performed with the long-prosthesis model to show variations in stem lengths of one, two and three femoral diameters distal to the fracture site. The Ogden plate model showed a transfer of tensile stress near the proximal-most band, with the highest tensile stress being at the fracture site with evidence of stress shielding of the proximal lateral cortex. The cortical bone strut model showed a transfer of tensile stress to the bone strut but showed less shielding of the proximal cortex. The cerclage wires at the base of the bone strut showed the highest changes in load with the distalmost wire increasing to almost four times its original preload.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

The adaptation of bone around intramedullary fixated prostheses, such as the femoral component of the hip joint or the radial component of the elbow joint, is well documented in follow-up studies. Bone adaptation takes the form of proximal bone atrophy accompanied, in some cases, by distal bone hypertrophy. A mechanistic model has been formulated to predict bone adaptation based on the concept that the continuous processes of damage and repair regulate bone adaptation. We apply the model to investigate the significance of two features of intramedullary prosthesis design on bone adaptation: prosthesis Young's modulus and the presence of a prosthesis collar. Results, as well as indicating some characteristics of accumulative-damage stimulated bone adaptation, predict that a low Young's modulus stem will very much reduce the extent of bone loss whereas the presence of a collar will have no significant effect. The results predict that a collarless low stiffness prosthesis is one possible approach for improving the secondary stability of intramedullary-fixated orthopaedic implants.  相似文献

All hip replacement prostheses alter the load transfer from the hip joint into the femur by changing the mechanical loading of the proximal femur from an externally to an internally loaded system. This alteration of the load transfer causes stress shielding and might lead to severe bone loss, especially with uncemented prostheses. To minimize these effects, load transfer to the femur should occur as proximal as possible. In order to support sufficient primary stability, however, directly post operative (PO) distal stabilization is reasonable. Consequently, the prostheses have to alter its mechanical characteristics after implantation. This concept is referred to as load-shift concept. Primary stability during the early PO state is provided by a prosthesis shaft, which is widened at the tip by a biodegradable pin. After resorption of the pin load transfer occurs no longer distally. The objective of this study was the numerical evaluation of the load-shift concept. The analysis was performed with a finite element model. Three-dimensional non-linear dynamic gait analyses data were used to evaluate whether the load transfer during walking can be altered effectively by insertion and resorption of a distal pin. Directly PO 38% of the transverse load is transferred through the prosthesis shaft and micromotion of the proximal prostheses tip is below 55 microm. After resorption of the pin, no transverse loads are transferred through the prosthesis shaft. Therefore, the loading of the proximal bone tissue is far more pronounced than in the case of a standard prosthesis, demonstrating the feasibility of the load-shift concept. A balanced degradation of the pin simultaneously with the ingrowth of the prosthesis is expected to reduce hip replacement complications.  相似文献

The objective of this paper is to identify the effects of materials of cementless femoral stem on the functional adaptive behaviors of bone.The remodeling behaviors of a two-dimensional simplified model of cementless hip prosthesis with stiff stem,flexible 'iso-elastic' stem,one-dimensional Functionally Graded Material (FGM) stem and two-dimensional FGM stem for the period of four years after prosthesis replacement were quantified by incorporating the bone remodeling algorithm with finite element analysis.The distributions of bone density,von Mises stress,and interface shear stress were obtained.The results show that two-dimensional FGM stem may produce more mechanical stimuli and more uniform interface shear stress compared with the stems made of other materials,thus the host bone is well preserved.Accordingly,the two-dimensional FGM stem is an appropriate femoral implant from a biomechanical point of view.The numerical simulation in this paper can provide a quantitative computational paradigm for the changes of bone morphology caused by implants,which can help to improve the design of implant to reduce stress shielding and the risk of bone-prosthesis interface failure.  相似文献

One proximal femoral replacement (PFR) and two distal femoral replacements (DFR) used in tumoural limb salvage were instrumented to measure axial force in the femoral shaft and at the tip of the intramedullary stem in vivo. The prostheses were implanted in suitable subjects and regular recording sessions held over 2 years for each subject. The ratio of the tip force to the shaft force increased with time in all 3 subjects, albeit at different rates, indicating that a greater proportion of the load was being transmitted along the prosthesis stem. The aim of this study was to correlate these changes with bone remodelling observed radiographically over the same period. Radiographs were analysed using a computer-assisted method to quantify bone width, radiolucent line width and relative bone density. The analysis showed the development of radiolucent lines at the bone-cement interface and a decrease in the lateral to medial ratio of bone density near the transection site. For the PFR, the mean width of the radiolucent area alongside the proximal fifth of the stem on the lateral side increased from 0 to 0.67 mm over the 2 years. The tip to shaft ratio correlated significantly with radiolucency on the lateral side of the stem. Both the DFRs became loose within 2 years but the PFR has remained in situ for 13 years to date. The correlations found thus far suggest that it may be possible to establish relationships between load transfer (and therefore loosening in the fixation) and radiographic data, with possible application to advance warning of clinical loosening in stemmed implants.  相似文献

Femoral head apparent density distribution predicted from bone stresses   总被引:4,自引:1,他引:3  
A new theory relating bone morphology to applied stress is used to predict the apparent density distribution in the femoral head and neck. Cancellous bone is modeled as a self-optimizing material and cortical bone as a saturated (maximum possible bone density) response to stress in the bone tissue. Three different approaches are implemented relating bone apparent density to: (1) the von Mises stress, (2) the strain energy density in the mineralized tissue and (3) a defined closed effective stress (spherical stress). An iterative nonlinear three-dimensional finite element model is used to predict the apparent density distribution in the femoral head and neck for each of the three approaches. It is shown that the von Mises stress (an open effective stress) cannot accurately predict bone apparent density. It is shown that strain energy density and the defined closed effective stress can predict apparent density and that they give predictions consistent with the observed density pattern in the femoral head and neck.  相似文献

目的:探讨人工髋关节置换术在治疗股骨头缺血性坏死(ANFH)中的临床疗效。方法:选择2007年2月-2011年2月我院收治的320例(340髋)股骨头缺血性坏死患者,均采用人工髋关节置换术对患者进行治疗,其中160例(172髋)患者应用骨水泥型假体进行治疗,另外160例(168)患者采用非骨水泥型假体进行治疗。采用Harris评分对患者手术前后的髋关节功能情况进行评价,并比较骨水泥治疗组和非骨水泥治疗组的临床疗效。结果:患者均获得随访,随访时间为3~18个月。全部患者手术后的Harris评分明显高于手术前,差异有统计学意义(P〈0.05)。骨水泥治疗组和非骨水泥治疗组在术后出血量、术后Harris评分及住院时间方面的差异无统计学意义(P〉0.05),但非骨水泥治疗组的并发症发生率明显低于骨水泥治疗组(P〈0.05)。结论:采用人工髋关节置换术治疗ANFH疗效显著,能明显改善患者的生活质量,骨水泥型假体与非骨水泥型假体的治疗效果相当,应根据患者的具体情况进行合理的选择。  相似文献

The aim of this paper is to describe a new numerical-experimental method to determine the stiffness of a conceptual proximal femoral prototype. The methodology consists of the comparison of the numerical and experimental displacement distributions of the prosthesis loaded as a cantilever beam to validate a design concept: controlled stiffness prosthesis. The manufactured prototype used to test the applicability of the numerical-experimental procedure integrates a stiff metal core bonded to a composite material made of an epoxy resin reinforced with carbon-glass braided pre-forms. The prosthesis with an embedded controlled stiffness concept was obtained by varying the geometry of the core with the composite layer thickness.  相似文献

The femoral element of a total hip replacement is a composite structure of two, or perhaps three, components — the endoprosthesis, the bone and, where present, the cement. The interfacial conditions are such that complete structural continuity does not necessarily obtain. That this is so has often been suspected due to the observed loosening which can occur in vivo. In modelling the system, typically for finite element analysis, it has usually been considered to be monolithic, such that tensile and shear stresses could be transmitted across the interfaces as well as the normal compressive stress. Here the femoral component of a Freeman hip replacement is considered, implanted without bone cement, and analyses are carried out under monolithic, i.e. fully bonded, and non-bonded assumptions. Simultaneously the effect of retaining the neck of the femur, one of the features of using this particular prosthesis, is also examined.  相似文献

Total hip replacement has evolved over the past 30 years utilising a variety of biocompatible materials, geometric shapes and fixation techniques. The main objective of this study is to investigate the long-term effects of strain adaptive bone remodelling due to the influence of a novel titanium cementless femoral hip replacement. The period of on-growth has been taken into account and the simulation has been run to predict the remodelling behaviour for a 36-month period. The main conclusion from this analysis is that the implant does shield the calcar to a similar degree as other cementless femoral hip designs. It does, however, tend to cause bone to be laid down along its length. This may, in part, be due to the novel geometry of the implant interlocking with and loading the bone.  相似文献

In this work, a new model for internal anisotropic bone remodelling is applied to the study of the remodelling behaviour of the proximal femur before and after total hip replacement (THR). This model considers bone remodelling under the scope of a general damage-repair theory following the principles of continuum damage mechanics. A "damage-repair" tensor is defined in terms of the apparent density and Cowin's "fabric tensor", respectively, associated with porosity and directionality of the trabeculae. The different elements of a thermodynamically consistent damage theory are established, including resorption and apposition criteria, evolution law and rate of remodelling. All of these elements were introduced and discussed in detail in a previous paper (García, J. M., Martinez, M. A., Doblaré, M., 2001. An anisotrophic internal-external bone adaptation model based on a combination of CAO and continuum damage mechanics technologies. Computer Methods in Biomechanics and Biomedical Engineering 4(4), 355-378.), including the definition of the proposed mechanical stimulus and the qualitative properties of the model. In this paper, the fundamentals of the proposed model are briefly reviewed and the computational aspects of its implementation are discussed. This model is then applied to the analysis of the remodelling behaviour of the intact femur obtaining densities and mass principal values and directions very close to the experimental data. The second application involved the proximal femoral extremity after THR and the inclusion of an Exeter prosthesis. As a result of the simulation process, some well-known features previously detected in medical clinics were recovered, such as the stress yielding effect in the proximal part of the implant or the enlargement of the cortical layer at the distal part of the implant. With respect to the anisotropic properties, bone microstructure and local stiffness are known to tend to align with the stress principal directions. This experimental fact is mathematically proved in the framework of this remodelling model and clearly shown in the results corresponding to the intact femur. After THR the degree of anisotropy decreases tending, specifically in the proximal femur, to a more isotropic behaviour.  相似文献

Patellar resurfacing during knee replacement is still under debate, with several studies reporting higher incidence of anterior knee pain in unresurfaced patellae. Congruency between patella and femur impacts the mechanics of the patellar cartilage and strain in the underlying bone, with higher stresses and strains potentially contributing to cartilage wear and anterior knee pain. The material properties of the articulating surfaces will also affect load transfer between femur and patella. The purpose of this study was to evaluate the mechanics of the unresurfaced patella and compare with natural and resurfaced conditions in a series of finite element models of the patellofemoral joint. In the unresurfaced analyses, three commercially available implants were compared, in addition to an 'ideal' femoral component which replicated the geometry, but not the material properties, of the natural femur. Hence, the contribution of femoral component material properties could be assessed independently from geometry changes. The ideal component tracked the kinematics and patellar bone strain of the natural knee, but had consistently inferior contact mechanics. In later flexion, compressive patellar bone strain in unresurfaced conditions was substantially higher than in resurfaced conditions. Understanding how femoral component geometry and material properties in unresurfaced knee replacement alters cartilage contact mechanics and bone strain may aid in explaining why the incidence of anterior knee pain is higher in the unresurfaced population, and ultimately contribute to identifying criteria to pre-operatively predict which patients are suited to an unresurfaced procedure and reducing the incidence of anterior knee pain in the unresurfaced patient population.  相似文献
