Background

Based on spatiotemporal clustering of human dengue virus (DENV) infections, transmission is thought to occur at fine spatiotemporal scales by horizontal transfer of virus between humans and mosquito vectors. To define the dimensions of local transmission and quantify the factors that support it, we examined relationships between infected humans and Aedes aegypti in Thai villages.

Methodology/Principal Findings

Geographic cluster investigations of 100-meter radius were conducted around DENV-positive and DENV-negative febrile “index” cases (positive and negative clusters, respectively) from a longitudinal cohort study in rural Thailand. Child contacts and Ae. aegypti from cluster houses were assessed for DENV infection. Spatiotemporal, demographic, and entomological parameters were evaluated. In positive clusters, the DENV infection rate among child contacts was 35.3% in index houses, 29.9% in houses within 20 meters, and decreased with distance from the index house to 6.2% in houses 80–100 meters away (p<0.001). Significantly more Ae. aegypti were DENV-infectious (i.e., DENV-positive in head/thorax) in positive clusters (23/1755; 1.3%) than negative clusters (1/1548; 0.1%). In positive clusters, 8.2% of mosquitoes were DENV-infectious in index houses, 4.2% in other houses with DENV-infected children, and 0.4% in houses without infected children (p<0.001). The DENV infection rate in contacts was 47.4% in houses with infectious mosquitoes, 28.7% in other houses in the same cluster, and 10.8% in positive clusters without infectious mosquitoes (p<0.001). Ae. aegypti pupae and adult females were more numerous only in houses containing infectious mosquitoes.

Conclusions/Significance

Human and mosquito infections are positively associated at the level of individual houses and neighboring residences. Certain houses with high transmission risk contribute disproportionately to DENV spread to neighboring houses. Small groups of houses with elevated transmission risk are consistent with over-dispersion of transmission (i.e., at a given point in time, people/mosquitoes from a small portion of houses are responsible for the majority of transmission).  相似文献

Background

African countries are scaling up malaria interventions, especially insecticide treated nets (ITN) and indoor residual spraying (IRS), for which ambitious coverage targets have been set. In spite of these efforts infection prevalence remains high in many parts of the continent. This study investigated risk factors for malaria infection in children using three malaria indicator surveys from Zambezia province, Mozambique. The impact of IRS and ITNs, the effects of keeping farm animals and of the construction material of roofs of houses and other potential risk factors associated with malaria infection in children were assessed.

Methods

Cross-sectional community-based surveys were conducted in October of 2006, 2007 and 2008. A total of 8338 children (ages 1–15 years) from 2748 households were included in the study. All children were screened for malaria by rapid diagnostic tests. Caregiver interviews were used to assess household demographic and wealth characteristics and ITN and IRS coverage. Associations between malaria infection, vector control interventions and potential risk factors were assessed.

Results

Overall, the prevalence of malaria infection was 47.8% (95%CI: 38.7%–57.1%) in children 1–15 years of age, less than a quarter of children (23.1%, 95%CI: 19.1%–27.6%) were sleeping under ITN and almost two thirds were living in IRS treated houses (coverage 65.4%, 95%CI: 51.5%–77.0%). Protective factors that were independently associated with malaria infection were: sleeping in an IRS house without sleeping under ITN (Odds Ratio (OR)  = 0.6; 95%CI: 0.4–0.9); additional protection due to sleeping under ITN in an IRS treated house (OR = 0.5; 95%CI: 0.3–0.7) versus sleeping in an unsprayed house without a ITN; and parental education (primary/secondary: OR = 0.6; 95%CI: 0.5–0.7) versus parents with no education. Increased risk of infection was associated with: current fever (OR = 1.2; 95%CI: 1.0–1.5) versus no fever; pig keeping (OR = 3.2; 95%CI: 2.1–4.9) versus not keeping pigs; living in houses with a grass roof (OR = 1.7; 95%CI: 1.3–2.4) versus other roofing materials and bigger household size (8–15 people: OR = 1.6; 95%CI: 1.3–2.1) versus small households (1–4 persons).

Conclusion

Malaria infection among children under 15 years of age in Zambezia remained high but conventional malaria vector control methods, in particular IRS, provided effective means of protection. Household ownership of farm animals, particularly pigs, and living in houses with a grass roof were independently associated with increased risk of infection, even after allowing for household wealth. To reduce the burden of malaria, national control programs need to ensure high coverage of effective IRS and promote the use of ITNs, particularly in households with elevated risks of infection, such as those keeping farm animals, and those with grass roofs.  相似文献

Restoration treatments were tested on the South Rim (ponderosa pine) and North Rim (mixed conifer) of Grand Canyon National Park, never-harvested forests where the historically frequent surface regime was interrupted in the late 19th century. Treatments were designed for "minimal impact" by limiting the size of trees to be thinned and minimizing mechanical equipment. Treatments included (1) thinning of small trees (diameter ≤ 12.7 cm) and prescribed fire; (2) thinning of small trees located close to large old trees and prescribed fire; (3) prescribed fire only; and (4) control. On the South Rim, density declined by 23–45% but basal area only by 9–14%. On the North Rim, density was reduced 32–68% and basal area declined 18–31%. Declines of 5–8% were observed in these same variables in the controls. Surface fuels were significantly reduced in all burned treatments (70–84% reduction in forest floor, 66–76% reduction in woody debris). However, canopy cover was nearly unchanged, and canopy fuel loads were not significantly reduced, although canopy base heights increased. The experiment accomplished the goal of minimally impacting the forest ecosystem but the effects of small-tree thinning plus burning were nearly indistinguishable from those of burning alone. All the treated units were left with modest gain in resistance to severe wildfires but in conditions still far removed from prefire-exclusion conditions.  相似文献

Previous investigations into the factors associated with house loss in wildfires have focused on the house construction and its immediate environment (e.g. gardens). Here, we examine how nearby native forest and other houses can influence house loss. Specifically, we used a sample of 3500 houses affected by the Victorian bushfires of February 7th 2009 to explore how the amount of forest, proportion of forest burned by crown fire and the number of nearby houses affected house loss and how far from the house this influence was exerted. These fires were the most destructive in Australian history and so represent the extreme of fire risk. Using generalized linear modeling we found that the probability of house loss increased with forest extent and the proportion burnt by crown fire and this relationship was strongest for forest measured 1 km from the houses. Houses were more likely to be destroyed if there were other houses within 50 m and if they were on a slope. A model containing these variables predicted house loss with 72% accuracy. Our findings have three important implications: i) management to change the occurrence of crown fire will be effective in reducing house loss; ii) this management may be required up to 1 km away from houses in some situations (a much larger zone than is currently used); iii) high density of houses may increase risk of loss. Given the potentially large width of this management zone and the hazard from nearby houses, it may be more sensible to concentrate on modification of buildings to reduce their vulnerability.  相似文献

Background

Millions of people are infected with Trypanosoma cruzi, the causative agent of Chagas disease in Latin America. Anti-trypanosomal drug therapy can cure infected individuals, but treatment efficacy is highest early in infection. Vector control campaigns disrupt transmission of T. cruzi, but without timely diagnosis, children infected prior to vector control often miss the window of opportunity for effective chemotherapy.

Methods and Findings

We performed a serological survey in children 2–18 years old living in a peri-urban community of Arequipa, Peru, and linked the results to entomologic, spatial and census data gathered during a vector control campaign. 23 of 433 (5.3% [95% CI 3.4–7.9]) children were confirmed seropositive for T. cruzi infection by two methods. Spatial analysis revealed that households with infected children were very tightly clustered within looser clusters of households with parasite-infected vectors. Bayesian hierarchical mixed models, which controlled for clustering of infection, showed that a child''s risk of being seropositive increased by 20% per year of age and 4% per vector captured within the child''s house. Receiver operator characteristic (ROC) plots of best-fit models suggest that more than 83% of infected children could be identified while testing only 22% of eligible children.

Conclusions

We found evidence of spatially-focal vector-borne T. cruzi transmission in peri-urban Arequipa. Ongoing vector control campaigns, in addition to preventing further parasite transmission, facilitate the collection of data essential to identifying children at high risk of T. cruzi infection. Targeted screening strategies could make integration of diagnosis and treatment of children into Chagas disease control programs feasible in lower-resource settings.  相似文献

Variation among flowering seasons in the time of flowering, synchrony and length of flowering, and fluctuations in the abundance of pollinators may cause a variation in pollen dispersal distance. In this study, we analyzed the temporal variation in pollen dispersal and breeding structure in the Neotropical tree species Tabebuia aurea (Bignoniaceae) and evaluated pollen dispersal between a population inside the reserve and a patch of isolated individuals on the edge of the reserve, and tested the hypothesis that isolated individuals are sinking for pollen. All adult trees (260) within a population of 40 ha and 9 isolated individuals on the edge of the reserve were sampled, and from these adults, 21 open-pollinated progeny arrays were analyzed in 2 flowering seasons (309 seeds in 2004 and 328 in 2005). Genetic analyses were based on the polymorphism at 10 microsatellite loci. A high proportion of self-pollination found in both flowering seasons indicated a mixed-mating system. The mean pollen dispersal distance differed significantly between the two flowering seasons (307.78 m in 2004 and 396.26 m in 2005). Maximum pollen dispersal was 2608 m, but most pollination events (65%) occurred at distances <300 m. Our results also showed that isolated individuals are sinking for pollen, with high pollen flow between the population inside the reserve and individuals on the edge. These results are most likely due to the large pollinator species, which can potentially fly long distances, and also due to temporal variation in individual fecundity and contribution to pollen dispersal.  相似文献

Background

The loading of atmospheric particulate matter (aerosol) in the eastern Himalaya is mainly regulated by the locally generated anthropogenic aerosols from the biomass burning and by the aerosols transported from the distance sources. These different types of aerosol loading not only affect the aerosol chemistry but also produce consequent signature on the radiative properties of aerosol.

Methodology/Principal Findings

An extensive study has been made to study the seasonal variations in aerosol components of fine and coarse mode aerosols and black carbon along with the simultaneous measurements of aerosol optical depth on clear sky days over Darjeeling, a high altitude station (2200 masl) at eastern Himalayas during the year 2008. We observed a heavy loading of fine mode dust component (Ca2+) during pre-monsoon (Apr – May) which was higher by 162% than its annual mean whereas during winter (Dec – Feb), the loading of anthropogenic aerosol components mainly from biomass burning (fine mode SO4 2− and black carbon) were higher (76% for black carbon and 96% for fine mode SO4 2−) from their annual means. These high increases in dust aerosols during pre-monsoon and anthropogenic aerosols during winter enhanced the aerosol optical depth by 25 and 40%, respectively. We observed that for every 1% increase in anthropogenic aerosols, AOD increased by 0.55% during winter whereas for every 1% increase in dust aerosols, AOD increased by 0.46% during pre-monsoon.

Conclusion/Significance

The natural dust transport process (during pre-monsoon) plays as important a role in the radiation effects as the anthropogenic biomass burning (during winter) and their differential effects (rate of increase of the AOD with that of the aerosol concentration) are also very similar. This should be taken into account in proper modeling of the atmospheric environment over eastern Himalayas.  相似文献

Background

The parasitic skin disease tungiasis (caused by the flea Tunga penetrans) affects resource-poor communities in Latin America, the Caribbean and sub-Saharan Africa. Prevalences in endemic areas are high, and severe pathology occurs commonly. However, risk factors for infestation have never been assessed in Africa.

Methods and Findings

A cross-sectional study was conducted in Erekiti, a rural community in Lagos State (Nigeria), where tungiasis is endemic. Individuals were examined clinically for the presence of tungiasis, and a questionnaire was applied. Data from 643 individuals (86.6% of the target population) were analyzed; 252 (42.5%) were infested with T. penetrans. In the multivariate logistic regression analysis, presence of pigs on the compounds (adjusted odds ratio = 17.98; 95% confidence interval: 5.55–58.23), sand or clay floor inside houses (9.33; 5.06–17.19), and having the common resting place outside the house (7.14; 4.0–14.29) were the most important risk factors identified. The regular use of closed footwear (0.34; 0.18–0.62) and the use of insecticides indoors (0.2; 0.05–0.83) were protective against infestation. The population attributable fractions associated with tungiasis were: sand or clay floor inside the house (73.7%), resting usually outside the house (65.5%), no regular use of closed footwear (51.1%), and pigs on the compound (37.9%).

Conclusion

The presence of tungiasis in Erekiti is determined to an important extent by a limited number of modifiable variables. Effective and sustainable intervention measures addressing these factors need to be implemented in this and other West African communities with high disease burden.  相似文献

Human activities are changing patterns of ecological disturbance globally. In North American deserts, wildfire is increasing in size and frequency due to fuel characteristics of invasive annual grasses. Fire reduces the abundance and cover of native vegetation in desert ecosystems. In this study, we sought to characterize stem growth and reproductive output of a dominant native shrub in the Mojave Desert, creosote bush (Larrea tridentata (DC.) Coville) following wildfires that occurred in 2005. We sampled 55 shrubs along burned and unburned transects 12 years after the fires (2017) and quantified age, stem diameter, stem number, radial and vertical growth rates, and fruit production for each shrub. The shrubs on the burn transects were most likely postfire resprouts based on stem age while stems from unburn transects dated from before the fire. Stem and vertical growth rates for shrubs on burned transects were 2.6 and 1.7 times higher than that observed for shrubs on unburned transects. Fruit production of shrubs along burned transects was 4.7‐fold more than shrubs along paired unburned transects. Growth rates and fruit production of shrubs in burned areas did not differ with increasing distance from the burn perimeter. Positive growth and reproduction responses of creosote following wildfires could be critical for soil stabilization and re‐establishment of native plant communities in this desert system. Additional research is needed to assess if repeat fires that are characteristic of invasive grass‐fire cycles may limit these benefits.  相似文献

The objective of this study was to estimate the possible travel distance of airborne bacteria emitted from a naturally ventilated broiler house by using staphylococci as indicator organisms. Air samples were taken during the second half of three fattening periods with Impinger (AGI-30) in the barn and simultaneously upwind and downwind from the building. Staphylococci concentrations varied between 1 × 106 and 1 × 107 cfu m−3 in the barn. No Staphylococci were detected in air samples at the upwind side. A strong exponential decrease of these bacteria was observed at three sampling heights (1.5, 4.0 and 9.5 m) in the main wind direction downwind of the barn. Staphylococci concentrations up to 5.9 × 103 cfu m−3 were detected at the farthest sampling point (333 m) downwind. Identification to the species level by means of a 16S–23S ITS PCR confirmed that Staphylococcus spp. from downwind samples originated from the barn. Staphylococci served as an useful indicator to demonstrate the travel distance of bacterial emissions originating from a naturally ventilated broiler house. These findings indicate that airborne transmission of viable bacteria from this type of housing system to adjacent residential dwellings or animal houses several hundred metres away is possible.  相似文献

Background

We compared the performance of two new commercial tests for the detection of dengue NS1 protein during the clinical phase of dengue virus (DENV) infection—an immunochromatographic test allowing rapid detection of the NS1 antigen, Dengue NS1 Ag STRIP (Bio-Rad Laboratories - Marnes La Coquette, France), and a two-step sandwich-format microplate enzyme-linked immunosorbent assay (ELISA), pan-E Dengue Early ELISA (Panbio - Brisbane, Australia)—with a one-step sandwich-format microplate ELISA, the Platelia Dengue NS1 Ag test (Bio-Rad).

Methods

We tested 272 serum samples from patients with dengue disease. Of these, 222 were from patients with acute infection of one of the four dengue serotypes, detected by RT-PCR and/or virus isolation. Forty-eight acute-phase serum samples from patients not infected with dengue virus were also included.

Results

The sensitivity of the Platelia Dengue NS1 Ag test on acute serum samples (n = 222) was 87.4% (95% confidence interval: 82.3% to 91.5%); that of Dengue NS1 Ag STRIP was 81.5% (95% CI: 75.8% to 86.4%) after 15 minutes and 82.4% (95% CI: 76.8% to 87.2%) after 30 minutes. Both tests had a specificity of 100% (97.5% CI, one-sided test: 92.6% to 100.0%). The pan-E Dengue Early ELISA had a sensitivity of 60.4% (95% CI: 53.4% to 66.8%) and a specificity of 97.9% (95% CI: 88.9% to 99.9%).

Conclusion

Our findings support the use of diagnostic tools based on the NS1 antigen detection for the diagnosis of acute DENV infection. The immunochromatographic test, Dengue NS1 Ag STRIP—the first rapid diagnostic test for DENV infection—was highly sensitive and specific, and would therefore be a suitable first-line test in the field. The pan-E Dengue Early ELISA was less sensitive than the Platelia test; this two-step ELISA should be combined with DENV IgM antibody detection for the diagnosis of DENV infection.  相似文献

Numbers of fecal and vomit spots deposited by house flies, Musca domestica L., on spot cards were about twice as high on cards placed on the downwind sides as on the upwind sides of building support posts in caged-layer poultry houses with tunnel ventilation in Brooksville, FL. This trend was stronger at the ends of the houses where airflow is faster than in the relatively still-air center of the houses. A similar evaluation conducted in a pullet house (Zephyrhills, FL) with an evaporative cooling ventilation system revealed significantly higher fly counts on spot cards and sticky cards in downwind compared with upwind orientations. Flies in the pullet house were concentrated in both ends of the house and in the center, with comparatively fewer flies in the intermediate regions. There was a high degree of correlation between spot card and sticky card counts in the pullet house.  相似文献

Background

Cardiovascular disease is the leading cause of mortality among patients with serious mental illness (SMI) and the prevalence of metabolic syndrome—a constellation of cardiovascular risk factors—is significantly higher in these patients than in the general population. Metabolic monitoring among patients using second generation antipsychotics (SGAs)—a risk factor for metabolic syndrome—has been shown to be inadequate despite the release of several guidelines. However, patients with SMI have several factors independent of medication use that predispose them to a higher prevalence of metabolic syndrome. Our study therefore examines monitoring and prevalence of metabolic syndrome in patients with SMI, including those not using SGAs.

Methods and Findings

We retrospectively identified all patients treated at a Veterans Affairs Medical Center with diagnoses of schizophrenia, schizoaffective disorder or bipolar disorder during 2005–2006 and obtained demographic and clinical data. Incomplete monitoring of metabolic syndrome was defined as being unable to determine the status of at least one of the syndrome components. Of the 1,401 patients included (bipolar disorder: 822; schizophrenia: 222; and schizoaffective disorder: 357), 21.4% were incompletely monitored. Only 54.8% of patients who were not prescribed SGAs and did not have previous diagnoses of hypertension or hypercholesterolemia were monitored for all metabolic syndrome components compared to 92.4% of patients who had all three of these characteristics. Among patients monitored for metabolic syndrome completely, age-adjusted prevalence of the syndrome was 48.4%, with no significant difference between the three psychiatric groups.

Conclusions

Only one half of patients with SMI not using SGAs or previously diagnosed with hypertension and hypercholesterolemia were completely monitored for metabolic syndrome components compared to greater than 90% of those with these characteristics. With the high prevalence of metabolic syndrome seen in this population, there appears to be a need to intensify efforts to reduce this monitoring gap.  相似文献

全球范围内森林片断化现象日益严重。与其他木本植物(乔木和灌木)相比, 木质藤本更趋向于分布在片断化森林的边缘, 因而了解木质藤本对边缘效应的响应对于进一步了解其对森林动态的影响极其必要。本文对哀牢山中山湿性常绿阔叶林林缘到林内环境梯度上木质藤本的变化进行了调查。在形成年龄分别为13年、35年和53年的3种类型的林缘, 设置从林缘向林内连续延伸的长方形样地(20 m × 50 m)各10个(总面积3 ha), 每个样地再划分为5个20 m × 10 m的样方。在每个样方内对胸径≥ 0.2 cm且长度≥ 2.0 m的木质藤本进行了每木调查。在3 ha的林缘样地中共记录到木质藤本植物2,426株, 隶属于14科19属31种。木质藤本的物种丰富度和多度均随距林缘距离的增加而降低, 边缘效应深度在35年林缘的边缘为30 m, 13和53年林缘的边缘则为20 m; 它们的胸高断面积在53年林缘的边缘效应深度为20 m, 但在13和35年林缘的不同距离上差异不显著。木质藤本对边缘效应的响应在物种水平上存在显著差异, 主要呈现正向和中性的响应格局, 包括只分布于林缘的物种, 和从林缘到林内环境梯度上密度逐渐降低的物种; 也有对边缘效应不敏感的物种。典范对应分析(CCA)表明, 林冠开度、边缘形成年龄和土壤水分是决定木质藤本在片断化森林边缘分布的重要影响因子。  相似文献

Background

We aimed to determine the representation of elderly people in published reports of randomized controlled trials (RCTs). We focused on trials of 4 medications—pioglitazone, rosuvastatin, risedronate, and valsartan—frequently used by elderly patients with chronic medical conditions.

Methods and Findings

We selected all reports of RCTs indexed in PubMed from 1966 to April 2008 evaluating one of the 4 medications of interest. Estimates of the community-based “on-treatment” population were from a national health insurance database (SNIIR-AM) covering approximately 86% of the population in France. From this database, we evaluated data claims from January 2006 to December 2007 for 1,958,716 patients who received one of the medications of interest for more than 6 months. Of the 155 RCT reports selected, only 3 studies were exclusively of elderly patients (2 assessing valsartan; 1 risedronate). In only 4 of 37 reports (10.8%) for pioglitazone, 4 of 22 (18.2%) for risedronate, 3 of 29 (10.3%) for rosuvastatine and 9 of 67 (13.4%) for valsartan, the proportion of patients aged 65 or older was within or above that treated in clinical practice. In 62.2% of the reports for pioglitazone, 40.9% for risedronate, 37.9% for rosuvastatine, and 70.2% for valsartan, the proportion of patients aged 65 or older was lower than half that in the treated population. The representation of elderly people did not differ by publication date or sample size.

Conclusions

Elderly patients are poorly represented in RCTs of drugs they are likely to receive.  相似文献

Changes in the cover and density of shrubs and trees were assessed from aerial photographs (1940, 1974 and 1998) as well as from fixed-point photographs taken in 1984 and 1996 in the Kruger National Park, South Africa. Woody cover (trees and shrubs combined) increased by 12% on granite substrates but decreased by 64% on basalt substrates over the past 58 years. Both these figures are expressed in terms of the initial values, respectively. The density of the large tree component of woody vegetation decreased on both substrates. Woody vegetation cover declined as fire return periods became shorter, but the relationship was weak. The increases in woody plant density and cover on granite are thought to be the result of decreased competition from grasses, which in turn is a result of overgrazing by wild herbivores whose numbers have been kept high through the provision of surface water. These effects were not seen on the relatively nutrient-rich basalts, where grasses can recover rapidly even after heavy grazing. The decline in overall woody cover on basalts is interpreted as a result of regular, short-interval prescribed burning over the past 40 years, while the universal decline in large trees seems to result from an interaction between regular, frequent fires and utilization by elephants. Background

HIV prevalence and incidence among sexually active women in peri-urban areas of Ladysmith, Edendale, and Pinetown, KwaZulu-Natal, South Africa, were assessed between October 2007 and February 2010 in preparation for vaginal microbicide trials.

Methodology/Principal Findings

Sexually active women 18–35 years, not known to be HIV-positive or pregnant were tested cross-sectionally to determine HIV and pregnancy prevalence (798 in Ladysmith, 1,084 in Edendale, and 891 in Pinetown). Out of these, approximately 300 confirmed non-pregnant, HIV-negative women were subsequently enrolled at each clinical research center (CRC) in a 12-month cohort study with quarterly study visits. Women in the cohort studies were required to use a condom plus a hormonal contraceptive method. HIV prevalence rates in the baseline cross-sectional surveys were high: 42% in Ladysmith, 46% in Edendale and 41% in Pinetown. Around 90% of study participants at each CRC reported one sex partner in the last 3 months, but only 14–30% stated that they were sure that none of their sex partners were HIV-positive. HIV incidence rates based on seroconversions over 12 months were 14.8/100 person-years (PY) (95% CI 9.7, 19.8) in Ladysmith, 6.3/100 PY (95% CI 3.2, 9.4) in Edendale, and 7.2/100 PY (95% CI 3.7, 10.7) in Pinetown. The 12-month pregnancy incidence rates (in the context of high reported contraceptive use) were: 5.7/100 PY (95% CI 2.6, 8.7) in Ladysmith, 3.1/100 PY (95% CI 0.9, 5.2) in Edendale and 6.3/100 PY (95% CI 3.0, 9.6) in Pinetown.

Conclusions/Significance

Background

Acute lower respiratory infection (ALRI) is a major cause of hospitalization for children in China, while the etiological diagnosis of ALRI remains a challenge. This study was performed to evaluate the utility of the blind Nasotracheal aspiration (NTA) in the pathogen detection in ALRI through an evaluation of the test''s specificity.

Methodology/Principal Findings

A hospital-based study of children ≤3 years was carried out from March 2006 through March 2007 in Suzhou University Affiliated Children''s Hospital, including 379 cases with ALRI from the respiratory wards, and 394 controls receiving elective surgery. Nasopharyngeal swabs (NPS) and NTA specimens were taken on admission. S. pneumoniae was isolated from 10.3% of NTA samples from ALRI children, H. influenzae from 15.3%, and M. catarrhalis from 4.7%. The false positive rate—the strains from NTA in control group children—was 8.4% (95% CI: 5.8%–11.4%) for S. pneumoniae, 27.2% (95% CI: 22.7–31.5%) for H. influenzae, and 22.1% (95% CI: 18.0%–26.2%) for M. catarrhalis. The agreement between NPS and NTA in the control group was over 70%.

Conclusion/Significance

The blind NTA test is not a useful test for etiologic diagnosis of ALRI.  相似文献

From April to July of 2003–2005, we investigated habitat selection of breeding brown eared-pheasants (Crossoptilon mantchuricum) in the Xiaowutaishan National Nature Reserve, Hebei Province. Our results show that brown eared-pheasants preferred deciduous-coniferous mixed or deciduous forests with altitudes ranging from 1600 to 2200 m, steep gradients of slopes and medium or high positions on slopes. Moreover, they preferred habitats with more tree species, larger diameters at the breast height, more density and cover of trees and habitats with more types of grasses, high density and cover of grasses, but with fewer types of shrubs and lower density and cover of shrubs. They had larger territories in the early breeding stage than in the late breeding stage. There were significant differences in diameters at the breast height of trees, average height of trees, cover of trees, density of shrubs, cover of shrubs, aspect, distance to path, distance to water, distance to forest edge (P < 0.01, respectively), density of trees, type of shrubs, height of shrubs, type of grasses, density of grasses and proportion of bareness (P < 0.05, respectively). The result of the principal component analysis suggested that the principal components among the 23 habitat factors were the average height of trees, density of trees, diameters at the breast height of trees, density of shrubs, cover of shrubs and density of grasses. The differences in habitat selection of brown eared-pheasants between early breeding and late breeding stages may correlate with the dynamics of ambient environmental conditions, the reproductive behavior and energy requirements in their sub-stages of breeding.  相似文献
