Elderly people with peripheral neuropathy of the lower limbs (PNLL) demonstrate a typical balance and gait impairment because of sensory ataxia. There is evidence that rehabilitation produces important gains on balance and gait. However, responsiveness to rehabilitation of balance and gait measures is unknown in PNLL. Aim of the current work is to evaluate the responsiveness to rehabilitation of balance, gait and sensory ataxia measures in elderly with PNLL.Twenty-five elderly with PNLL attending physiotherapy and occupational therapy during inpatient rehabilitation were recruited. Balance and gait measures (including static posturography, TUG test and the 10 m walking test) were administered on admission and discharge. An accelerometer secured to the trunk was used for TUG recording and static balance assessment. Static balance was tested with open and closed eyes, so as to assess sensory ataxia.Following rehabilitation, patients improved gait [admission vs discharge, mean(SD): 0.86(0.33) vs 0.98(0.32) m/s], TUG [18.7(7.8) vs 15.1(5.2) s] and turning [46.2(15.3) vs 53.3(15.3) °/s]. However, none of 12 static balance parameters derived from trunk acceleration significantly changed. Principal component analysis showed that before training, eyes closed and eyes open balance correlated with orthogonal components (one and two vs. three and four). After training, eyes open and eyes closed balance were more similar to each other being both correlated with component one.Responsiveness to rehabilitation is larger for gait than static balance measured by trunk acceleration. However, exercise can also have a beneficial effect on sensory ataxia by making eyes closed balance more similar to eyes open balance.  相似文献

The prevalence of sustaining a fall is particularly high in children. Deficits in postural control and muscle strength are important intrinsic fall risk factors. Thus, the purpose of this study was to investigate the impact of balance training (BT) followed by detraining on postural control, plantar flexor strength, and jumping height in prepubertal children. Thirty grade 1 school children participated in this study and were assigned to either an intervention class (INT, n = 15, age 6.7 ± 0.5 years) or a control class (n = 15, age 6.6 ± 0.5 years). The INT participated in 4 weeks of BT (3 per week) integrated in their physical education lessons. Pre, post, and follow-up tests included the measurements of postural sway on a balance platform, maximal torque and rate of force development of the plantar flexors on an isokinetic device, and jumping height on a force platform. The significance level was set at p < 0.05. Balance training resulted in tendencies in terms of small to medium interaction effects yet not statistically significant improvements in postural sway (f = 0.14; p > 0.05), force production of the plantar flexors (f = 0.18; p> 0.05), and jumping height (f = 0.25; p> 0.05). Immaturity of the postural control system and deficits in attentional focus during practice of balance exercises could be responsible for the nonsignificant findings in this study. Thus, other training regimen (e.g., resistance training) should be applied alone or in combination with BT, which may have the potential to promote balance and strength in children.  相似文献

Maintenance of human upright stance requires the acquisition and integration of sensory inputs. Conventional measures of sway have had success in identifying age- and some disease-related changes, but remain unable to address the complexities and dynamics associated with postural control. We investigated the effects of vision, surface compliance, age, and gender on the spectral content of center of pressure (COP) time series. Sixteen healthy young (age 18-24) and older participants (age 55-65) performed trials of quiet, upright stance under different vision (eyes open vs. closed) and surface (hard vs. compliant) conditions. Spectral analyses were conducted to describe COP mean normalized power in discretized bands. Effects of the two sensory modalities and age were distinct in the antero-posterior and medio-lateral directions, and a reorganization of spectral content was evident with increasing task difficulty (eyes open vs. closed and hard vs. compliant surface) and among older adults. These results indicate that vision and surface compliance are predominantly associated with responses from musculature associated with antero-posterior and medio-lateral directions of sway, respectively. Finally, distinguishing between the contributions of different afferent systems to the postural control system using the spectral content of sway bi-directionally may help in diagnosing individuals with balance impairments.  相似文献

Athletes experienced in maximal-power and power-endurance training performed 1 set of 2 common power training exercises in an effort to determine the effects of moderately high repetitions upon power output levels throughout the set. Twenty-four and 15 athletes, respectively, performed a set of 10 repetitions in both the bench throw (BT P60) and jump squat exercise (JS P60) with a resistance of 60 kg. For both exercises, power output was highest on either the second (JS P60) or the third repetition (BT P60) and was then maintained until the fifth repetition. Significant declines in power output occurred from the sixth repetition onwards until the 10th repetition (11.2% for BT P60 and 5% for JS P60 by the 10th repetition). These findings suggest that athletes attempting to increase maximal power limit their repetitions to 2 to 5 when using resistances of 35 to 45% 1RM in these exercises.  相似文献

The purpose of this study was to compare the peak force and force curve characteristics during a traditional bench press (BP) and a ballistic bench throw (BT). Eight (age = 21.0 +/- 2.3 years, height = 182.3 +/- 7.4 cm, body mass = 85.9 +/- 5.5 kg) semi-professional rugby league players with resistance and power training experience performed both BP and BT exercises at loads of 55 and 80% of their predicted one-repetition maximum. The force curves for each test were then divided into three intensity levels, set at low to moderate (0-75%), high (75-95%), and near-maximal force (95-100%). These values were obtained by determining the percentage of the range of motion (ROM) in which the force produced during each test was within these thresholds. The BT exercise produced significantly (p < 0.05) higher peak force than BP under both loading conditions. A significantly greater portion of the ROM during the 80% BT was at a high intensity in comparison with the BP. No significant differences were found between force intensity conditions at 55% loads. It can be concluded that performing the BT exercise results in a greater peak force output when compared with the traditional BP movement under both resistance training and maximal power loading conditions. Furthermore, performing the BT exercise with heavy loads results in a more efficient training method for maintaining high force levels throughout the ROM.  相似文献

To study the effect of exercise and dehydration on the postural sensory-motor strategies, 10 sportsmen performed a 45 min-exercise on a cycle ergometer at intensity just below the ventilatory threshold without fluid intake. They performed, before, immediately and 20 min after exercise, a sensory organization test to evaluate balance control in six different sensory situations, that combine three visual conditions (eyes open, eyes closed and sway-referenced visual surround motion) with two platform conditions (stable platform, sway-referenced platform motion). Blood samples were collected before and after exercise. Exercise induced a mild dehydration, characterized by body mass loss and increase in proteinemia. Postural performances decreased immediately after exercise, mainly in the standard situation (eyes open, stable visual surround and platform) and when only the vestibular cue was reliable (eyes closed and sway-referenced platform). Moreover, the decreased use of vestibular input was correlated with the dehydration level. Finally, postural performances normalized 20 min after exercise. Even though muscular fatigue could explain the decrease in postural performances, vestibular fluid modifications may also be involved by its influence on the intralabyrinthine homeostasis, lowering thus the contribution of vestibular information on balance control.  相似文献

The objective of the study was to examine the relationship between balance and pitching error in college baseball pitchers. Sixteen college baseball pitchers, 9 National Association of Intercollegiate Athletics (NAIA) and 7 National Collegiate Athletic Association (NCAA) Division III, participated in the study. Balance ability, expressed as average sway velocity (deg.s(-1)), during dominant leg unilateral stance with eyes open and eyes closed was quantified for each subject utilizing the Balance Master System 7.04 (long force plate). Additionally, each subject underwent sensory organization testing on the SMART EquiTest System providing information regarding the effective use of the somatosensory, visual, and vestibular inputs. Pitching error was assessed with a high-speed video camera recorder during spring practice. A JUGS radar gun measured pitch velocity. The mean pitching error was 37.50 cm with a mean pitch velocity of 78 miles.h(-1) (35 m.s(-1)). No significant correlation was demonstrated between unilateral stance eyes open and pitching error (r = -0.24; p = 0.36) or unilateral stance eyes closed and pitching error (r = -0.29; p = 0.27). A significant negative correlation was demonstrated between sensory organization test 5 and pitching error (r = -0.50; p = 0.05) and between sensory organization test 5/1 and pitching error (r = -0.50; p = 0.05). Additionally, unilateral stance eyes closed demonstrated a positive correlation with pitch velocity (r = 0.52; p = 0.04). The results reveal that low levels of vestibular input utilization may lead to high levels of pitching error in college baseball pitchers.  相似文献

目的:评定持续性军事职业活动对平衡能力的影响及视觉系统在其中的作用,为精准军事运动训练提供依据。方法:54名健康男性受试者,年龄(20.28±3.72)岁;身高(173.21±5.67)cm;体重(64.29±5.12) kg,在36 h的时间内完成多项军事科目,记录全程运动负荷(随机抽取11人),军事活动结束后进行睁眼平衡能力测试(54人)和闭眼平衡能力测试(随机抽取27人)。结果:内部负荷,与安静状态相比,军事职业活动中的心率(HR)、运动后的过氧消耗(EPOC)和运动冲量(TRIMP)值均显著增加(P<0.05);平衡能力,与睁眼安静值相比,军事职业活动后睁眼状态下整体、前后和左右方向的重心动摇距离和速度均显著增加(P<0.05),而整体、前后和左右动摇面积,前后和左右方向的最大动摇径,以及运动椭圆面积无显著变化(P>0.05);与闭眼安静值相比,军事活动后闭眼状态下平衡能力所有指标均显著增加(P<0.05)。结果表明视觉可控制持续性军事职业活动后人体重心动摇的幅度和面积。结论:长时间军事职业活动会损害人体的平衡能力,持续性的军事职业活动后,闭眼状态下平衡能力的破坏程度较睁眼状态大,表现为重心动摇幅度和范围的增加,表明视觉系统在控制姿态稳定方面起一定作用。  相似文献

The efficient coordination of agonist and antagonist muscles is one of the important early adaptations in resistance training responsible for large increases in strength. Weak antagonist muscles may limit speed of movement; consequently, strengthening them leads to an increase in agonist muscle movement speed. However, the effect of combining agonist and antagonist muscle exercises into a power training session has been largely unexplored. The purpose of this study was to determine if a training complex consisting of contrasting agonist and antagonist muscle exercises would result in an acute increase in power output in the agonist power exercise. Twenty-four college-aged rugby league players who were experienced in combined strength and power training served as subjects for this study. They were equally assigned to an experimental (Antag) or control (Con) group and were no different in age, height, body mass, strength, or maximal power. Power output was assessed during bench press throws with a 40-kg resistance (BT P40) with the Plyometric Power System training device. After warming up, the Con group performed the BT P40 tests 3 minutes apart to determine if any acute augmentation to power output could occur without intervention. The Antag group also performed the BT P40 tests; however, an intervention strategy of a set of bench pulls, which is an antagonistic action to the bench throw, was performed between tests to determine if this would acutely affect power output during the second BT P40 test. Although the power output for the Con group remained unaltered between test occasions, the significant 4.7% increase for the Antag group indicates that a strategy of alternating agonist and antagonist muscle exercises may acutely increase power output during complex power training. This result may affect power training and specific warm-up strategies used in ballistic sports activities, with increased emphasis placed upon the antagonist muscle groups.  相似文献

Motor skills during sport activity are influenced by practice-related constraints and leads to the development of appropriate postural sensorimotor strategies. Fencing is highly requiring visual monitoring and high-speed motor skills while retaining efficient balance control. Conversely, pistol shooting is a static activity requiring a high control of body sway. This study aimed to evaluate balance control and the related neurosensory organisation through reproducible postural tasks with and without sensory conflict. Twelve expert fencers, 10 expert shooters and 10 sedentary controls have performed a static posturographic test and a sensory organisation test (in 6 different sensory situations based upon sway-referenced vision and support surface, C1 to C6). Shooters yielded a better balance control during C1 (eyes open) and C2 (eyes closed) than fencers and controls. Fencers showed a better balance control in C5 (eyes closed with sway-referenced support surface) than shooters and controls. While this study confirms the beneficial effects of physical activities on balance control, a differential effect on balance characteristics due to the acquired specific motor skills was also noted. In addition to high proprioceptive sensitivity in sportsmen, dynamic constraints in fencing force fencers to permanently select the most relevant information to manage better sensory conflicting situations.  相似文献

Given its tri-planar action at the hip, strengthening of gluteus maximus (GMAX) has been advocated as part of rehabilitation and injury prevention protocols for various musculoskeletal conditions. However, recruitment of GMAX during weight-bearing strengthening exercises can be challenging owing to the muscular redundancy at the hip for a given joint motion. The current study sought to determine if a 1-week activation program could result in greater GMAX recruitment during functional strengthening exercises. Pre- and post-training surface electromyography were collected from 12 healthy participants as they performed double- and single-leg squats. Between testing sessions, participants completed a GMAX activation training program consisting of isometric exercises with band resistance (twice per day for 7 days). Following the 1-week activation program, GMAX recruitment was found to increase by 57% during the double-leg squat (p = 0.005, Cohen’s r = 0.73) and 53% during the single-leg squat (p = 0.006, Cohen’s r = 0.70). Implementation of an initial GMAX activation program should be considered to facilitate neuromuscular adaptations that facilitate utilization of GMAX during hip strengthening exercises.  相似文献

Cellulose degradation to methane under continuous fermentation conditions was compared using fully mixed, fully mixed with solids return, sludge-blanket, and fixed-film fermenters. In fully mixed fermenters, a decrease in hydraulic retention time (HRT) of two weeks or less caused the wash out of anaerobes capable of converting volatile fatty acids to methane, while at increased feeding rates over 1 g/L day the rapid growth of cellulolytic anaerobes upset the balance between acid formation and its conversion to CH(4). Circulation of cellulose and difficulty in settling of cellulose with attached bacteria imposed problems in the use of other types of fermenters. On the basis of information obtained from this study, a fermenter which combined a fully mixed phase for cellulose degradation and a fixed-film phase with pre-immobilized bacteria for converting fatty acids to CH(4) in one vessel, was devised. Using this fermenter, a mixed culture converted cellulose to CH(4) at 4.8 g/L day at a HRT of six days as compared to 0.7 g/L day at a HRT of 28 days in the fully mixed fermenter.  相似文献

The purpose of this study was to determine the effect of Power Balance? bands on strength, flexibility, and balance. Strength and flexibility were measured using the MicroFit system. Strength was measured via a bicep curl and flexibility via the sit-and-reach method. Balance was measured by the BIODEX System SD. There were 4 different conditions for the balance test: eyes open on a firm surface (EOFS), eyes closed on a firm surface (ECFS), eyes open on a foam surface (EOFoS), and eyes closed on a foam surface (ECFoS). There were 24 subjects in the study (10 men and 14 women). A counterbalance, double-blind, placebo, controlled within-subject design was used. Each of the subjects participated in 3 treatment sessions, consisting of Power Balance?, placebo band, and no band. An alpha level of p ≤ 0.05 was set a priori. There were no significant differences in strength, flexibility, or balance with regard to the treatments used. There was a significant difference between the conditions in the balance test (p = 0.000): EOFS (0.51), ECFS (0.68), EOFoS (0.99), and ECFoS (2.18); however, these were independent of the treatment conditions. The results indicate that the Power Balance? bands did not have an effect on strength, flexibility, or balance.  相似文献

Serum cortisol concentrations and testosterone:cortisol concentration ratios of eight prepubescent elite male gymnasts (mean age 10 years 11 months) and 11 controls (mean age 11 years 1 month) were examined during 5 consecutive training days. During this period, the gymnasts trained 3 h each day with moderate intensity mobility, strength and skill exercises while the controls were relatively sedentary. Blood samples were taken from all the boys in both groups before (1630 hours) and 30 min after (2000 hours) training on 4 days. Serum cortisol concentrations of the gymnasts were not significantly different from those of the controls throughout the experiment. Serum cortisol concentrations of both groups were significantly larger (P < 0.05) at 1630 hours than at 2000 hours, indicating that cortisol secretion followed the typical adult circadian change, seemingly unaltered by training. However, there was a significant decrease (P < 0.05) in the testosterone:cortisol ratio of the gymnasts when compared with controls from day 1 to day 3. After a rest on day 4 the testosterone: cortisol ratio of the gymnasts significantly increased (P < 0.05) but the ratio of the control group also increased indicating that there may have been some day-to-day change by factor(s) other than training. The most obvious factor which may have accounted for the unresponsiveness of serum cortisol concentration to the gymnastics training was that the exercise intensity was too low. However, several days of the training seemed to reduce the anabolic to catabolic balance but further experiments are needed to confirm this finding.  相似文献

Dyslexic children, besides difficulties in mastering literacy, also show poor postural control that might be related to how sensory cues coming from different sensory channels are integrated into proper motor activity. Therefore, the aim of this study was to examine the relationship between sensory information and body sway, with visual and somatosensory information manipulated independent and concurrently, in dyslexic children. Thirty dyslexic and 30 non-dyslexic children were asked to stand as still as possible inside of a moving room either with eyes closed or open and either lightly touching a moveable surface or not for 60 seconds under five experimental conditions: (1) no vision and no touch; (2) moving room; (3) moving bar; (4) moving room and stationary touch; and (5) stationary room and moving bar. Body sway magnitude and the relationship between room/bar movement and body sway were examined. Results showed that dyslexic children swayed more than non-dyslexic children in all sensory condition. Moreover, in those trials with conflicting vision and touch manipulation, dyslexic children swayed less coherent with the stimulus manipulation compared to non-dyslexic children. Finally, dyslexic children showed higher body sway variability and applied higher force while touching the bar compared to non-dyslexic children. Based upon these results, we can suggest that dyslexic children are able to use visual and somatosensory information to control their posture and use the same underlying neural control processes as non-dyslexic children. However, dyslexic children show poorer performance and more variability while relating visual and somatosensory information and motor action even during a task that does not require an active cognitive and motor involvement. Further, in sensory conflict conditions, dyslexic children showed less coherent and more variable body sway. These results suggest that dyslexic children have difficulties in multisensory integration because they may suffer from integrating sensory cues coming from multiple sources.  相似文献

Objectives

System identification techniques have the potential to assess the contribution of the underlying systems involved in standing balance by applying well-known disturbances. We investigated the reliability of standing balance parameters obtained with multivariate closed loop system identification techniques.

Methods

In twelve healthy elderly balance tests were performed twice a day during three days. Body sway was measured during two minutes of standing with eyes closed and the Balance test Room (BalRoom) was used to apply four disturbances simultaneously: two sensory disturbances, to the proprioceptive and the visual system, and two mechanical disturbances applied at the leg and trunk segment. Using system identification techniques, sensitivity functions of the sensory disturbances and the neuromuscular controller were estimated. Based on the generalizability theory (G theory), systematic errors and sources of variability were assessed using linear mixed models and reliability was assessed by computing indexes of dependability (ID), standard error of measurement (SEM) and minimal detectable change (MDC).

Results

A systematic error was found between the first and second trial in the sensitivity functions. No systematic error was found in the neuromuscular controller and body sway. The reliability of 15 of 25 parameters and body sway were moderate to excellent when the results of two trials on three days were averaged. To reach an excellent reliability on one day in 7 out of 25 parameters, it was predicted that at least seven trials must be averaged.

Conclusion

This study shows that system identification techniques are a promising method to assess the underlying systems involved in standing balance in elderly. However, most of the parameters do not appear to be reliable unless a large number of trials are collected across multiple days. To reach an excellent reliability in one third of the parameters, a training session for participants is needed and at least seven trials of two minutes must be performed on one day.  相似文献

Laboratory training and testing of auditory recognition skills in animals is important for understanding animal communication systems that depend on auditory cues. Songbirds are commonly studied because of their exceptional ability to learn complex vocalizations. In recent years, mounting interest in the perceptual abilities of songbirds has increased the demand for laboratory behavioural training and testing paradigms. Here, we describe and demonstrate the success of a method for auditory discrimination experiments, including all the necessary hardware, training procedures and freely-available, versatile software. The system can run several behavioural training and testing paradigms, including operant (go-nogo, stimulus preference, and two-alternative forced choice) and classical conditioning tasks. The software and some hardware components can be used with any laboratory animal that learns and responds to sensory cues. The peripheral hardware and training procedures are designed for use with songbirds and auditory stimuli. Using the go-nogo paradigm of the training system, we show that adult zebra finches learn to recognize and correctly classify individual female calls and male songs. We also show that learning the task generalizes to new stimulus classes; birds that learned the task with calls subsequently learned to recognize songs faster than did birds that learned the task and songs at the same time.  相似文献

Abstract

Purpose/background: Multiscale entropy (MSE) is a nonlinear measure of postural control that quantifies how complex the postural sway is by assigning a complexity index to the center of pressure (COP) oscillations. While complexity has been shown to be task dependent, the relationship between sway complexity and level of task challenge is currently unclear. This study tested whether MSE can detect short-term changes in postural control in response to increased standing balance task difficulty in healthy young adults and compared this response to that of a traditional measure of postural steadiness, root mean square of velocity (VRMS).

Methods: COP data from 20?s of quiet stance were analyzed when 30 healthy young adults stood on the following surfaces: on floor and foam with eyes open and closed and on the compliant side of a Both Sides Up (BOSU) ball with eyes open. Complexity index (CompI) was derived from MSE curves.

Results: Repeated measures analysis of variance across standing conditions showed a statistically significant effect of condition (p?<?0.001) in both the anterior–posterior and medio-lateral directions for both CompI and VRMS. In the medio-lateral direction there was a gradual increase in CompI and VRMS with increased standing challenge. In the anterior–posterior direction, VRMS showed a gradual increase whereas CompI showed significant differences between the BOSU and all other conditions. CompI was moderately and significantly correlated with VRMS.

Conclusions: Both nonlinear and traditional measures of postural control were sensitive to the task and increased with increasing difficulty of standing balance tasks in healthy young adults.  相似文献

Melanin-concentrating hormone (MCH) is a hypothalamic peptide that plays a critical role in the regulation of food intake and energy metabolism. In this study, we investigated the potential role of dense hippocampal MCH innervation in the spatially oriented food-seeking component of feeding behavior. Rats were trained for eight sessions to seek food buried in an arena using the working memory version of the food-seeking behavior (FSB) task. The testing day involved a bilateral anti-MCH injection into the hippocampal formation followed by two trials. The anti-MCH injection did not interfere with the performance during the first trial on the testing day, which was similar to the training trials. However, during the second testing trial, when no food was presented in the arena, the control subjects exhibited a dramatic increase in the latency to initiate digging. Treatment with an anti-MCH antibody did not interfere with either the food-seeking behavior or the spatial orientation of the subjects, but the increase in the latency to start digging observed in the control subjects was prevented. These results are discussed in terms of a potential MCH-mediated hippocampal role in the integration of the sensory information necessary for decision-making in the pre-ingestive component of feeding behavior.  相似文献

The present study was designed to assess the effects of experimentally-induced plantar pain on the displacement of centre of foot pressure during unperturbed upright stance in different sensory conditions of availability and/or reliability of visual input and somatosensory input from the vestibular system and neck. To achieve this goal, fourteen young healthy adults were asked to stand as still as possible in three sensory conditions: (1) No-vision, (2) Vision, and (3) No-vision – Head tilted backward, during two experimental conditions: (1) a No-pain condition, and (2) a condition when a painful stimulation was applied to the plantar surfaces of both feet (Plantar-pain condition). Centre of foot pressure (CoP) displacements were recorded using a force platform. Results showed that (1) experimentally-induced plantar pain increased CoP displacements in the absence of vision (No-vision condition), (2) this deleterious effect was more accentuated when somatosensory information from the vestibular and neck was altered (No-vision – Head tilted backward condition) and (3) this deleterious effect was suppressed when visual information was available (Vision condition). From a fundamental point of view, these results lend support to the sensory re-weighting hypothesis whereby the central nervous system dynamically and selectively adjusts the relative contributions of sensory inputs (i.e. the sensory weightings) in order to maintain balance when one or more sensory channels are altered by the task (novel or challenging), environmental or individual conditions. From a clinical point of view, the present findings further suggest that prevention and treatment of plantar pain may be relevant for the preservation or improvement of balance control, particularly in situations (or individuals) in which information provided by the visual, neck proprioceptive and vestibular systems is unavailable or disrupted.  相似文献
