We evaluated the performance of a consumer multi-sensory wristband (Fitbit Charge 2?), against polysomnography (PSG) in measuring sleep/wake state and sleep stage composition in healthy adults.

In-lab PSG and Fitbit Charge 2? data were obtained from a single overnight recording at the SRI Human Sleep Research Laboratory in 44 adults (19—61 years; 26 women; 25 Caucasian). Participants were screened to be free from mental and medical conditions. Presence of sleep disorders was evaluated with clinical PSG. PSG findings indicated periodic limb movement of sleep (PLMS, > 15/h) in nine participants, who were analyzed separately from the main group (n = 35). PSG and Fitbit Charge 2? sleep data were compared using paired t-tests, Bland–Altman plots, and epoch-by-epoch (EBE) analysis.

In the main group, Fitbit Charge 2? showed 0.96 sensitivity (accuracy to detect sleep), 0.61 specificity (accuracy to detect wake), 0.81 accuracy in detecting N1+N2 sleep (“light sleep”), 0.49 accuracy in detecting N3 sleep (“deep sleep”), and 0.74 accuracy in detecting rapid-eye-movement (REM) sleep. Fitbit Charge 2? significantly (p < 0.05) overestimated PSG TST by 9 min, N1+N2 sleep by 34 min, and underestimated PSG SOL by 4 min and N3 sleep by 24 min. PSG and Fitbit Charge 2? outcomes did not differ for WASO and time spent in REM sleep. No more than two participants fell outside the Bland–Altman agreement limits for all sleep measures. Fitbit Charge 2? correctly identified 82% of PSG-defined non-REM–REM sleep cycles across the night. Similar outcomes were found for the PLMS group.

Fitbit Charge 2? shows promise in detecting sleep-wake states and sleep stage composition relative to gold standard PSG, particularly in the estimation of REM sleep, but with limitations in N3 detection. Fitbit Charge 2? accuracy and reliability need to be further investigated in different settings (at-home, multiple nights) and in different populations in which sleep composition is known to vary (adolescents, elderly, patients with sleep disorders).  相似文献

Circadian phase resetting is sensitive to visual short wavelengths (450–480?nm). Selectively filtering this range of wavelengths may reduce circadian misalignment and sleep impairment during irregular light-dark schedules associated with shiftwork. We examined the effects of filtering short wavelengths (<480?nm) during night shifts on sleep and performance in nine nurses (five females and four males; mean age?±?SD: 31.3?±?4.6 yrs). Participants were randomized to receive filtered light (intervention) or standard indoor light (baseline) on night shifts. Nighttime sleep after two night shifts and daytime sleep in between two night shifts was assessed by polysomnography (PSG). In addition, salivary melatonin levels and alertness were assessed every 2?h on the first night shift of each study period and on the middle night of a run of three night shifts in each study period. Sleep and performance under baseline and intervention conditions were compared with daytime performance on the seventh day shift, and nighttime sleep following the seventh daytime shift (comparator). On the baseline night PSG, total sleep time (TST) (p?<?0.01) and sleep efficiency (p?=?0.01) were significantly decreased and intervening wake times (wake after sleep onset [WASO]) (p?=?0.04) were significantly increased in relation to the comparator night sleep. In contrast, under intervention, TST was increased by a mean of 40?min compared with baseline, WASO was reduced and sleep efficiency was increased to levels similar to the comparator night. Daytime sleep was significantly impaired under both baseline and intervention conditions. Salivary melatonin levels were significantly higher on the first (p?<?0.05) and middle (p?<?0.01) night shifts under intervention compared with baseline. Subjective sleepiness increased throughout the night under both conditions (p?<?0.01). However, reaction time and throughput on vigilance tests were similar to daytime performance under intervention but impaired under baseline on the first night shift. By the middle night shift, the difference in performance was no longer significant between day shift and either of the two night shift conditions, suggesting some adaptation to the night shift had occurred under baseline conditions. These results suggest that both daytime and nighttime sleep are adversely affected in rotating-shift workers and that filtering short wavelengths may be an approach to reduce sleep disruption and improve performance in rotating-shift workers. 目的:对比分析药物干预和手术切除治疗儿童阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的临床疗效。方法:应用随机数字表法将2015年2月至2017年11月经本院确诊的100例OSAHS患儿分为药物组、手术组,每组50例。药物组采用孟鲁司特钠治疗6个月,手术组行腺样体和扁桃体切除术。6个月后比较两组患儿多导睡眠图监(PSG)的监测结果和生活质量情况,比较两组疗效评定情况,记录手术组无效及并发症的原因。结果:6个月后,药物组、手术组患儿呼吸暂停低通气指数(AHI)、阻塞性呼吸暂停指数(OAI)、微觉醒指数(MAI)和睡眠呼吸紊乱指数(RDI)较治疗前降低,且手术组患儿AHI低于药物(P0.05)。手术组患儿6个月后睡眠障碍、对监护人的影响、身体症状评分较治疗前降低,且低于药物组(P0.05),而药物组治疗前后OSA-18评分各指标比较差异无统计学意义(P0.05)。手术组患儿总有效率为90.00%(45/50),高于药物组的50.00%(25/50),差异有统计学意义(P0.05)。手术组患儿有出血现象的4例、伴舌后坠2例、上呼吸反复道感染6例和鼻炎5例,无效的5例患儿为伴有肥胖的重度OSAHS。结论:对于OSAHS患儿,药物干预和手术切除均可改善患儿PSG指标水平,但手术切除治疗可提高患儿生活质量和治疗有效率。  相似文献

We examined the effects of low-dose oral risperidone (RIS) on nocturnal sleep in healthy participants. This study was performed in a placebo-controlled manner in 10 healthy male volunteers (mean age, 23.6 years), with administration of 0.5 mg of RIS oral solution or a placebo in the morning or evening for 2 consecutive days. Each night, polysomnography (PSG) was performed, and PSG data during non-rapid-eye movement (REM) sleep were processed by power spectral analysis. An evening administration of 0.5 mg RIS significantly increased total sleep time, sleep efficiency and sleep stage 3, and significantly decreased total waking time and waking after sleep onset (P < 0.05). A morning administration of 0.5 mg RIS significantly increased sleep stage 3 (P < 0.05). According to power spectral analysis, the evening administration of RIS significantly increased the theta power (P < 0.05) and decreased the beta power (P < 0.05) during non-REM sleep. The administration of 0.5 mg oral RIS increases sleep stage 3 and increases total sleep time following evening administration.

We investigated sleep quality and melatonin in 12 adults who wore blue-light shield or control eyewear 2 hours before sleep while using a self-luminous portable device, and assessed visual quality for the two eyewear types. Overnight melatonin secretion was significantly higher after using the blue-light shield (P < 0.05) than with the control eyewear. Sleep efficacy and sleep latency were significantly superior for wearers of the blue-light shield (P < 0.05 for both), and this group reported greater sleepiness during portable device use compared to those using the control eyewear. Participants rated the blue-light shield as providing acceptable visual quality.  相似文献

The last 20 yrs have seen a marked increase in studies utilizing actigraphy in free-living environments. The aim of the present study is to directly compare two commercially available actigraph devices with concurrent polysomnography (PSG) during a daytime nap in healthy young adults. Thirty healthy young adults, ages 18–31 (mean 20.77 yrs, SD 3.14 yrs) simultaneously wore AW-64 and GT3X+ devices during a polysomnographically recorded nap. Mann-Whitney U (M-U) test, intraclass correlation coefficients, and Bland-Altman statistic were used to compare total sleep time (TST), sleep onset latency (SOL), wake after sleep onset (WASO), and sleep efficiency (SE) between the two actigraphs and PSG. Epoch-by-epoch (EBE) agreement was calculated to determine accuracy, sensitivity, specificity, predictive values for sleep (PVS) and wake (PVW), and kappa and prevalence- and bias-adjusted kappa (PABAK) coefficients. All frequency settings provided by the devices were examined. For both actigraphs, EBE analysis found accuracy, sensitivity, specificity, PVS, and PVW comparable to previous reports of other similar devices. Kappa and PABAK coefficients showed moderate to high agreement with PSG depending on device settings. The GT3X+ overestimated TST and SE, and underestimated SOL and WASO, whereas no significant difference was found between AW-64 and PSG. However, GT3X+ showed overall better EBE agreements to PSG than AW-64. We conclude that both actigraphs are valid and reliable devices for detecting sleep/wake diurnal patterns. The choice between devices should be based on several parameters as reliability, cost of the device, scoring algorithm, target population, experimental condition, and aims of the study (e.g., sleep and/or physical activity). Wearable fitness-tracker devices are becoming increasingly available. We evaluated the agreement between Jawbone UP and polysomnography (PSG) in assessing sleep in a sample of 28 midlife women. As shown previously, for standard actigraphy, Jawbone UP had high sensitivity in detecting sleep (0.97) and low specificity in detecting wake (0.37). However, it showed good overall agreement with PSG with a maximum of two women falling outside Bland–Altman plot agreement limits. Jawbone UP overestimated PSG total sleep time (26.6?±?35.3?min) and sleep onset latency (5.2?±?9.6?min), and underestimated wake after sleep onset (31.2?±?32.3?min) (p’s?<?0.05), with greater discrepancies in nights with more disrupted sleep. The low-cost and wide-availability of these fitness-tracker devices may make them an attractive alternative to standard actigraphy in monitoring daily sleep–wake rhythms over several days.  相似文献

ABSTRACT

The relevance of altered rest-activity rhythm (RAR) and light exposure rhythm (LER) in insomnia patients under natural conditions remains unclear. The aim of this study was to compare the parametric and nonparametric circadian variables of RAR and those of LER under natural conditions between insomnia patients and normal controls (NC) in a community-dwelling setting. The relationship of the nonparametric variables with sleep quality was also explored in both groups. Participants above 18 years old were recruited from three Public Health Centers in a rural area of Korea. Actigraphy (Actiwatch 2; Philips Respironics, Murrysville PA, USA) recording was conducted for 7 days. Subjects were eligible for our study if they had an insomnia disorder (ID) for at least 1 month. Actigraphy data of 78 normal control (NC) subjects (Age, 55.95 ± 13.22 years) and 104 patients with insomnia disorder (ID) (Age, 62.14 ± 12.34 years) were included for the analysis. Acrophases and amplitudes of RAR and LER were estimated using cosinor analysis. Interdaily stability (IS), intradaily variability (IV), and relative amplitude (RA) of these rhythms were determined using nonparametric methods. Parametric cosinor and nonparametric variables of RAR and LER were compared between the NC and ID groups. Generalized linear models (GLMs) were applied to evaluate the main effects of group and each nonparametric variable as well as a group by each variable interaction on the sleep onset latency (SOL), sleep efficiency (SE), and wake after sleep onset (WASO) reflecting sleep quality. Among sleep parameters, the ID group showed significantly lower SE and greater WASO than the NC group. There were no significant differences in the acrophase and amplitude of RAR and LER between the two groups. There were no significant differences in IV, IS, and RA of RAR and LER between the two groups either. GLMs for RAR revealed a significant interaction between the group and IS on the SOL (β = ?46.39, p < 0.01), indicating a negative relationship of the IS with SOL in ID unlike its positive relationship in NC. There were no significant main effects of IV on the SOL, SE, and WASO, but significant main effects of RA on the SE and WASO (β = 63.65 and β = ?221.43, respectively, p < 0.01). GLMs for LER revealed no significant main effects of IS, IV or RA on the SOL, SE, and WASO, but significant interactions between group and RA on the SE and WASO (β = 56.17 and β = ?171.93, respectively, p < 0.05), indicating a stronger positive relationship of the RA with SE in ID compared to NC, and a negative relationship of the RA with WASO in ID, unlike its positive relationship in NC. Although our study did not reveal group differences in circadian variables of RAR and LER, it suggested that the regularity of RAR could be positively associated with sleep initiation, while the robustness of LER could be positively associated with sleep maintenance in insomnia patients.  相似文献

Objective: To determine the prevalence of sleep apnea in morbidly obese patients and its relationship with cardiac arrhythmias. Research Methods and Procedures: Fifty‐two consecutive morbidly obese (body mass index ≥ 40 kg/m2) outpatients from the Obesity Clinic of the National Institute of Nutrition Salvador Zubirán underwent two nights of polysomnography with standard laboratory techniques. Electrocardiographic polysomnography signals (Lead II) were evaluated by two experienced cardiologists, and sleep complaints were measured with a standard sleep questionnaire (Sleep Disorders Questionnaire). In order to make comparisons between groups with different severities of sleep‐disordered breathing, we classified the patients in four groups using the apnea‐hypopnea index (AHI): Group 1, AHI 5 < 15 (n = 10); Group 2, AHI 15 < 30 (n = 10); Group 3, AHI 30 < 65 (n = 14); Group 4, AHI ≥ 65 (n = 17). Results: A wide range of sleep‐disordered breathing, ranging from AHI of 2.5 to 128.9 was found. Ninety‐eight percent of the sample (n = 51) had an AHI ≥ 5 (mean = 51 ± 37), and 33% had severe sleep apnea with AHI ≥ 65 with a mean nocturnal desaturation time of <65% over 135 minutes. Electrocardiographic abnormalities were present in 31% of the patients. Cardiac rhythm alterations showed an association with the level of sleep‐disordered breathing and oxygen desaturation. Discussion: We conclude that there is a high prevalence of sleep apnea in morbidly obese patients and that the risk for cardiac arrhythmias increases in this population in the presence of a severe sleep apnea (AHI ≥ 65) with severe oxygen desaturation (Sao 2 ≤ 65%).  相似文献

摘要 目的:探讨血脂、血小板参数、稳态模型胰岛素抵抗指数(HOMA-IR)与阻塞性睡眠呼吸暂停低通气综合征(OSAHS)合并高血压患者多导睡眠图(PSG)参数的相关性及其预测价值。方法:选择2020年1月至2022年11月徐州医科大学附属沭阳医院收治的163例OSAHS患者,根据是否合并高血压将其分为单纯OSAHS组(78例)及高血压组(85例),检测两组血脂、血小板参数、HOMA-IR、PSG参数;Pearson相关性分析血脂、血小板参数、HOMA-IR与PSG参数的相关性;多因素Logistic回归分析OSAHS合并高血压的危险因素;受试者工作特征(ROC)曲线分析血脂、血小板参数、HOMA-IR预测OSAHS合并高血压的价值。结果:高血压组甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、平均血小板体积(MPV)、HOMA-IR、微觉醒指数(MAI)、呼吸暂停低通气指数(AHI)、氧减指数(ODI)高于单纯OSAHS组(P<0.05),高密度脂蛋白胆固醇(HDL-C)水平低于单纯OSAHS组(P<0.05)。高血压组TG、TC、LDL-C、MPV、HOMA-IR与MAI、AHI、ODI呈正相关(P<0.05),HDL-C与MAI、AHI、ODI呈负相关(P<0.05)。高体质量指数、高HOMA-IR及TG、MPV水平升高是OSAHS患者合并高血压的危险因素(P<0.05)。联合TG、MVP、HOMA-IR预测OSAHS患者合并高血压的曲线下面积高于以上三指标单独预测。结论:OSAHS合并高血压患者TG、MPV水平及HOMA-IR显著增高,且与MAI、AHI、ODI呈正相关,TG、MPV、HOMA-IR联合检测对OSAHS患者合并高血压的预测价值较高。  相似文献

The aim of this study was to describe the polysomnographic features of childhood-onset narcolepsy. A retrospective review was performed on children with narcolepsy. The polysomnogram findings were compared with reference values obtained from normative data in the general population. Narcolepsy subjects had a mean initial sleep latency of 10.9 min on nocturnal PSG, which was shorter by a mean of 18 min than corresponding values from normative data (P < 0.001). Short initial REM latency on nocturnal PSG correlated with the number of SOREMPs on MSLT in subjects with narcolepsy (P = 0.007). These findings may assist in the clinical diagnosis of narcolepsy.

Daytime workers tend to have shorter sleep duration and earlier sleep onset during work days than on days off. Large individual differences in sleep onset and sleep duration may be observed on work days, but work usually synchronizes sleep offset to a similar time. The present study describes individual differences in sleep behaviour of 48 daytime workers (25 men, aged 20–58 years) from an iron ore mine in Northern Sweden. The aim of the study was to determine whether differences in sleep patterns during work days were associated with the outcomes of sleepiness and sleep complaints. Cluster analysis was used to group workers into two categories of sleep onset and sleep duration. The “Late Sleep Onset” cluster comprised workers who slept 1.30 h later than the “Early Sleep Onset” cluster (p < 0.0001 for all weekdays). The “Long Sleep Duration” cluster slept 1.10 h longer than the “Short Sleep Duration” cluster (p < 0.0002 for work nights). The “Late Sleep Onset” cluster reported less refreshing sleep (p < 0.01) and had lower sufficient sleep scores (p < 0.01) than the “Early Sleep Onset” cluster. The “Short Sleep Duration” cluster also reported lower scores for sufficient sleep (p < 0.04) than the “Long Sleep Duration” cluster. For combined characteristics (phase and duration), workers with a late phase and short sleep duration reported greater sleep debt and sleepiness than workers with an early phase and short sleep duration (p < 0.02). Work schedule and commuting time modulate both sleep phase and sleep duration independently. Workers, classified as having an intermediate sleep phase preference, can organize their sleep time in order to minimize sleep debt and sleepiness symptoms. Individual differences in sleep phase and duration should be considered when promoting well-being at work even among groups with similar sleep needs. In order to minimize sleep debt and sleepiness symptoms, successful sleep behaviour could be promoted involving extend use of flexitime arrangement (i.e. later starting times) and reduce use of alarm clocks.  相似文献

ABSTRACT

Approximately 50% to 80% of individuals with posttraumatic stress disorder (PTSD) also meet criteria for major depressive disorder (MDD). Sleep disturbance is a major concern in both PTSD and MDD, and is associated with poor treatment response, poor functional outcome and increased suicide risk. Social rhythm regularity, or the consistency of daily habitual behaviors, is theoretically linked to circadian rhythms and may be disturbed in both PTSD and MDD. The present study examined the relationship between social rhythm regularity, sleep disruption and MDD and PTSD symptoms in a sample of veterans with comorbid PTSD and MDD. Baseline data were obtained from 56 male veterans who met DSM-IV criteria for PTSD and MDD. Veterans completed the Social Rhythm Metric (SRM), a self-report questionnaire that assesses the regularity of routines by determining how regularly individuals completed 17 different types of activities. In a linear regression model, increased minutes awake after sleep onset (WASO) was a significant predictor of increased depression scores on the Hamilton Rating Scale for Depression (p < .05). SRM scores did not significantly predict depressive symptoms, however the interaction of WASO and SRM significantly predicted depressive symptoms (p = <.05), with significant relationships found at SRM scores less than 3.62. Neither minutes awake after sleep onset, SRM scores, nor their interaction was associated with PTSD symptom severity. Social and possibly circadian rhythm regularity may represent a risk or resilience factor for individuals with comorbid PTSD and MDD. Findings highlight the importance of exploring the interactions of sleep and social/circadian rhythms in depression in order to inform continued treatment development.  相似文献

The aim of this study was to evaluate daytime and nighttime sleep, as well as daytime and nighttime sleepiness of professional shift-working bus drivers. Thirty-two licensed bus drivers were assessed by nocturnal and diurnal polysomnography (PSG) recording and multiple sleep latency testing (MSLT) sessions. Sleep length was shorter and sleep efficiency reduced during daytime sleep compared with nighttime sleep. Thirty-eight percent of the drivers had indices of obstructive apnea and hypopnea syndrome (>5/h sleep) during nighttime and daytime sleep; more drivers snored during daytime than nighttime sleep (50% vs. 35%, p < 0.05), and 38% of the drivers evidenced periodic leg movements. The MSLT revealed that 42 and 38% of the bus drivers met the criteria for sleepiness when the test was conducted during the day and night, respectively. The daytime as compared to nighttime sleep of shift-working bus drivers was shorter and more fragmented and was associated in many with evidence of excessive sleepiness. Respiratory disorder was a common finding among the professional shift-working bus drivers. All these sleep deficiencies may adversely affect on the job driving performance.  相似文献

Our aim was to investigate the significance of neck circumference (NC) on the presence and severity of obstructive sleep apnea (OSA) syndrome independent of visceral fat (VF) obesity. A total of 219 subjects with suspected OSA underwent a complete polysomnography (PSG) study, along with the measurement of NC, and total body fat (TF) and VF levels (VFLs) measured by bioelectrical impedance analysis. We proposed NC divided by height (NC/H) as the simple index for height‐corrected NC in Japanese subjects. NC/H exhibited a significantly stronger correlation than NC per se with BMI (r = 0.781 vs. 0.675, P = 0.0178), TF (r = 0.531 vs. 0.156, P < 0.0001), and VF (r = 0.819 vs. 0.731, P = 0.0203), indicating that NC/H is a better indicator of visceral obesity than NC per se. Interestingly, despite the strong correlation between NC/H and VFL, VFL was significantly associated with the apnea‐hypopnea index (AHI) ≥5, ≥15, and ≥30, but not with ≥40 or ≥50, whereas NC/H was significantly associated with higher AHI values, i.e., AHI ≥50 but not with lower AHI value. Furthermore, multiple regression analyses revealed that VFL and NC/H were independently associated with the square root of AHI (AHI0.5) levels in obese and nonobese patients, respectively. In conclusion, NC is associated with the severity of OSA independently of visceral obesity, especially in nonobese patients.  相似文献

Elite soccer players are at risk of reduced recovery following periods of sleep disruption, particularly following late-night matches. It remains unknown whether improving sleep quality or quantity in such scenarios can improve post-match recovery. Therefore, the aim of this study was to investigate the effect of an acute sleep hygiene strategy (SHS) on physical and perceptual recovery of players following a late-night soccer match. In a randomised cross-over design, two highly-trained amateur teams (20 players) played two late-night (20:45) friendly matches against each other seven days apart. Players completed an SHS after the match or proceeded with their normal post-game routine (NSHS). Over the ensuing 48 h, objective sleep parameters (sleep duration, onset latency, efficiency, wake episodes), countermovement jump (CMJ; height, force production), YoYo Intermittent Recovery test (YYIR2; distance, maximum heart rate, lactate), venous blood (creatine kinase, urea and c-reactive protein) and perceived recovery and stress markers were collected. Sleep duration was significantly greater in SHS compared to NSHS on match night (P = 0.002, d = 1.50), with NSHS significantly less than baseline (P < 0.001, d = 1.95). Significant greater wake episodes occurred on match night for SHS (P = 0.04, d = 1.01), without significant differences between- or within-conditions for sleep onset latency (P = 0.12), efficiency (P = 0.39) or wake episode duration (P = 0.07). No significant differences were observed between conditions for any physical performance or venous blood marker (all P > 0.05); although maximum heart rate during the YYIR2 was significantly higher in NSHS than SHS at 36 h post-match (P = 0.01; d = 0.81). There were no significant differences between conditions for perceptual “overall recovery” (P = 0.47) or “overall stress” (P = 0.17). Overall, an acute SHS improved sleep quantity following a late-night soccer match; albeit without any improvement in physical performance, perceptual recovery or blood-borne markers of muscle damage and inflammation.  相似文献

ABSTRACT

Schedule changes associated with rotating shifts can interfere with the circadian rhythms of nurses and thereby affect their sleep duration, sleep quality, work efficiency, and work performance. The objectives of this study was to investigate differences in workday sleep fragmentation, rest-activity cycle, sleep quality, and activity level among nurses working different shifts. After filling out a basic information questionnaire and completing the Pittsburgh Sleep Quality Index (PSQI) questionnaire, participants were asked to wear an actigraph and keep sleep records for seven consecutive days. Data pertaining to wake after sleep onset (WASO), 24-hour autocorrelation coefficient (r24), and daytime activity mean was collected in order to investigate workday sleep fragmentation, rest-activity cycle, and daytime activity level. We obtained complete questionnaires and data from 191 nurses. Day- and evening-shift nurses had more regular workday rest-activity cycles than did night-shift nurses (F = 51.26, p < .001). After controlling for r24 coefficients, we determined that nurses who experienced greater workday sleep fragmentation had higher PSQI scores (β = .18, p = .008). After controlling for WASO times, we determined that nurses who had more regular rest-activity cycles on workdays had lower PSQI scores (β = – .16, p = .036). After controlling for shift type and WASO times, we determined that nurses with higher PSQI scores displayed lower activity levels (β = – .21, p = .015) and those with higher r24 coefficients displayed higher activity levels (β = .18, p = .040) on workdays. We then examined the causal path relationships. Among the shifts, only the day-shift nurses had a higher r24 (β = ?.59, p < .001) than did the night-shift nurses; WASO exerted a significant impact on PSQI scores (β = .20, p = .002); r24 had a significant and negative influence on PSQI scores (β = ?.38, p < .001), and PSQI scores significantly and negatively influenced workday activity levels (β = ?.20, p = .006). This study determined that day- and evening-shift nurses enjoyed more regular and consistent rest-activity cycles than did night-shift nurses; nurses with greater workday sleep fragmentation and/or more irregular rest-activity cycles experienced poorer sleep quality; and nurses suffering from poorer sleep quality displayed lower daytime activity levels on workdays.  相似文献

There is good evidence for cognitive and physiological arousal in chronic insomnia. Accordingly, clinical trial studies of insomnia treatments aimed at reducing arousal, including relaxation and meditation, have reported positive results. Yoga is a multicomponent practice that is also known to be effective in reducing arousal, although it has not been well evaluated as a treatment for insomnia. In this preliminary study, a simple daily yoga treatment was evaluated in a chronic insomnia population consisting of sleep-onset and/or sleep-maintenance insomnia and primary or secondary insomnia. Participants maintained sleep–wake diaries during a pretreatment 2-week baseline and a subsequent 8-week intervention, in which they practiced the treatment on their own following a single in-person training session with subsequent brief in-person and telephone follow-ups. Sleep efficiency (SE), total sleep time (TST), total wake time (TWT), sleep onset latency (SOL), wake time after sleep onset (WASO), number of awakenings, and sleep quality measures were derived from sleep–wake diary entries and were averaged in 2-week intervals. For 20 participants completing the protocol, statistically significant improvements were observed in SE, TST, TWT, SOL, and WASO at end-treatment as compared with pretreatment values.  相似文献

Obstructive sleep apnea syndrome (OSAS) is closely associated with hypertension. Activity of angiotensin II (Ang II) and non-dipping nocturnal blood pressure (BP) variability are implicated in hypertension-related target organ damage. We examined the correlation between OSAS with serum Ang II levels and evaluated the risk of non-dipping BP variability in 180 patients with essential hypertension (EHT). Eligible patients were divided into three subgroups based on their apnea-hypopnea index (AHI) evaluated by polysomnography. EHT alone, EHT with mild OSAS, and EHT with moderate/severe OSAS. Ambulatory BP monitoring was used to calculate mean BP over 24 h, as well as diurnal and nocturnal BP variability. Serum Ang II was determined with enzyme-linked immun-osorbent assay. EHT patients with OSAS had significantly higher systolic BP calculated either over 24 h, or by diurnal or nocturnal monitoring (P < 0.05). More EHT patients with OSAS showed non-dipping BP profiles than did EHT patients alone (P < 0.05). The number of patients with non-dipping BP increased with increasing OSAS severity. Surgical treatment alleviated OSAS and reduced AHI (P < 0.05). Preoperative serum Ang II in EHT patients with OSAS was significantly higher than that in those without OSAS (P < 0.05), and showed a rising trend with OSAS severity (P < 0.05). Postoperative serum Ang II, BP and the incidence of non-dipping BP were reduced by surgery to levels lower than preoperative values in patients with OSAS. We therefore conclude that OSAS leads to increased serum Ang II and increased risk of non-dipping BP in patients with EHT.

摘要 目的:探讨百乐眠胶囊联合艾司西酞普兰片对失眠伴抑郁焦虑患者睡眠质量、不良情绪以及神经递质水平的影响。方法:选取2017年7月~2019年12月期间我院收治的失眠伴抑郁焦虑患者117例,将上述患者根据随机数字表法分为对照组(n=58,艾司西酞普兰片治疗)和研究组(n=59,百乐眠胶囊联合艾司西酞普兰片治疗),比较两组患者睡眠质量、不良情绪、多导睡眠图(PSG)参数、神经递质水平及不良反应。结果:研究组治疗2个月后的临床总有效率为93.22%(55/59),高于对照组的79.31%(46/58)(P<0.05)。两组治疗2个月后汉密尔顿焦虑量表(HAMA)、汉密尔顿抑郁量表(HAMD)以及匹兹堡睡眠质量指数(PSQI)评分、睡眠潜伏期、P物质(SP)均较治疗前降低,且研究组低于对照组(P<0.05)。两组治疗2个月后睡眠总时间、睡眠效率、神经肽Y(NPY)、5-羟色胺(5-HT)升高,且研究组高于对照组(P<0.05)。治疗期间研究组不良反应发生率较对照组降低(P<0.05)。结论:失眠伴抑郁焦虑患者经百乐眠胶囊联合艾司西酞普兰片治疗后,睡眠质量、不良情绪得到显著改善,同时还可有效改善血清神经递质水平,减少不良反应,临床应用效果确切。  相似文献
