Congenital coronary artery-cardiac chamber fistulae: Report of two cases

Two cases of coronary arteriovenous fistulae are reported. In the first case, the right coronary artery (RCA) drained into the right ventricular outflow tract, and the distal RCA filled through a branch of the left anterior descending coronary artery. In the second case, the RCA drained into the right atrium and filled through a branch of the left circumflex artery. The fistulae were closed with subcoronary mattress sutures that preserved the continuity of the native circulation.     

Blastomycosis: Report of three cases from Alberta with a review of Canadian cases

Approximately 120 cases of blastomycosis have been reported from Canada to-date. The great majority of these occurred in the Eastern provinces. Since 1970, three cases of blastomycosis have been seen in Alberta. The first case, with meningeal and pulmonary involvements, was diagnosed at post-mortem. The second case was that of a 75-year-old male with a history of pancytopenia, aortic arteriosclerosis, exposure to mercury, and fever. KOH and periodic-acid schiff (PAS) stained smears of the lung tissue, received after autopsy, showed numerous budding yeast cells of Blastomyces dermatitidis along with some hyphal filaments. Similarly, budding cells of B. dermatitidis and hyphal segments were observed in large numbers in the PAS and Gomori's methenamine-silver (GMS) stained sections made from adrenals, lung, kidney, and spleen tissues. Attempts to culture the fungus on a variety of selective and non-selective media were unsuccessful, due to heavy bacterial contamination. The indirect fluoroscent antibody results were 2+ with the B. dermatitidis conjugate. The third case was that of a 31-year-old male, who was admitted to the hospital with the chief complaint of chest pain. Biopsy tissue sections, stained with the GMS procedure revealed a few foci with B. dermatitidis yeast cells. The immunodiffusion and complement fixation (CF) tests gave positive results against B. dermatitidis antigen (titre, 116). The CF titre declined following treatment with amphotericin B and the immunodiffusion test became negative after the institution of antifungal therapy. Except for the last patient, the other two patients had no history of travel in any known endemic areas. In addition to these cases, a survey of blastomycosis occurring in this country has been presented along with on the disease in dogs and a cat.     

Animal rhinosporidiosis in Brazil. Report of three additional cases

Fifteen cases of animal rhinosporidiosis seen in Brazil are briefly reviewed. Additional cases of the disease in two horses and one mule are reported.     

Congenital Absence of the Gallbladder: Case Report

The virus of human poliomyelitis has been demonstrated in excretions before onset of the disease, during the disease, and in convalescence. It may be confused with different viruses likely to be found in the same sources in clinical conditions resembling poliomyelitis.Immunologic differences between strains of poliomyelitis virus have been detected so that three types are now evident. The distribution of these types and their importance as causes of epidemics are not known. This multiplicity of immunologic types is an important factor in considering immunization of humans. Commercial manufacture of vaccines faces many technical problems.Recently the Coxsackie virus has been demonstrated in humans with a disease closely resembling poliomyelitis.     

Distal duplication 14q: Report of three cases and further delineation of the syndrome

Summary Three cases of distal duplication 14q are presented. The first two cases are cousins in a kindred segregating a balanced translocation t(14;18)(q31;q23). The third case resulted from a maternal translocation t(14;18)(q24;p11). By review of these cases and those previously reported, a distal duplication 14q syndrome is further delineated. Common features include postnatal growth retardation, mental retardation, hypotonia, microcephaly, slanted palpebral fissures, ocular hypertelorism, sparse eyelashes and eyebrows, nasal dysmorphism, tented lip, micrognathia, posteriorly rotated ears, and minor skeletal anomalies.     

Fine needle aspiration cytology of supernumerary breasts. Report of three cases

Fine needle aspiration (FNA) was performed on small lumps in three female patients in the reproductive age group. One lump in the axilla and one on the chest wall were clinically suspected of being a lipoma or an angioma while another axillary lump was thought to be an enlarged lymph node. Cytologically, the aspirates revealed the presence of ectopic breast tissue. These cases show yet another instance in which FNA cytology can make the diagnosis of lumps of unknown origin, avoiding the need for open biopsy.     

Hypoglycemia in the elderly during chlorpropamide therapy. Report of three cases

The risk of the chronic neurologic glycopenia in the elderly with non-insulin dependent diabetes mellitus is higher than in middle- aged patients. Symptoms of hypoglycaemia may vary in patients over 70 years of life.     

Myxoid malignant melanoma. Report of three cases and review of the literature

Three cases of malignant myxoid melanomas are described. That feature may simulate other malignant tumors with myxoid changes, especially in amelanotic metastases. The problems of differential diagnosis are briefly discussed.     

先天性尿道下裂的矫治(附1 000例报告)

目的:总结先天性尿道下裂的矫治经验.方法:尿道下裂患者1000例,年龄1～26岁,平均4岁.冠状沟型118例,阴茎体型593例.阴茎阴囊型189例,会阴型100例.791例采用尿道板切开卷管成形法(TIP),117例采用Duckett+Duplay术,92例采用二期手术,一期行阴茎伸直及皮辩转移,二期行尿道成形术(TIP).术中遵循微创原则,使用显微器械,尽量保留原有的正常组织结构.结果:随访6月至2年,TIP术741例手术一次成功,Duckett+Duplay术99例一次成功,分期手术二期成功86例.并发尿道皮肤瘘51例,尿道狭窄17例,尿道憩室6例,均经再次手术治愈.结论:尿道下裂矫治手术中,首先保留尿道板手术,并发症少,成功率高,值得推广,阴茎严重弯曲者或重度尿道下裂患者可选Duckett+Duplay术或者分期手术.
