<正>背景:鉴于婴儿麻疹死亡率高,故人们考虑在12月龄就开始接种是否能提供持久的免疫力。方法:70名儿童在6、9或12月龄初次接种麻疹疫苗,在5~10岁时对麻疹特异性免疫应答进行了评估。结果:分别在第6、9或12月龄时接种麻疹疫苗第一剂次,在5~10岁麻疹T细胞增殖的刺激指数为11.4(SE1.3)、10.9(SE,1.5)、14.4(SE2.1)。其中在6月龄时接种疫苗的儿童,存在被动抗体(PAS)的那些儿童体内中和抗体浓度[几何平均滴度  相似文献

本文旨在探讨当前我国麻疹疫苗接种程序下的婴儿麻疹流行特征,为改进麻疹疫苗接种策略提供科学依据。利用麻疹监测系统,对杭州市2004~2007年婴儿麻疹发病的分布和疫苗免疫状况进行分析,发现婴儿麻疹发病率无明显城乡差异,发病季节主要集中在3~5月,发病年龄主要集中在5~9月龄。≤8月龄病例以常住儿童为主,≥9月龄病例以流动儿童为主。≥9月龄麻疹病例中,常住儿童所占比例低于流动儿童,而其麻疹疫苗接种率(47.5%)明显高于流动儿童(14.3%),两者有显著差异(χ2=11.75,P0.01)。因此,为进一步降低婴儿麻疹发病率,需加强流动儿童的麻疹疫苗补种;另外,建议将麻疹疫苗初次免疫时间提前至6月龄,这可能有助于降低婴儿麻疹发病率。  相似文献

沪191麻疹疫苗免疫持久性和影响因素的评价   总被引:11,自引:0,他引:11  
1991~1998年,我们对荆州区川店镇503名6~15月龄儿童进行了现行沪191麻疹疫苗血清流行病学效果观察,结果表明,初次免疫后1个月麻疹IgG抗体阳转率为9165%,GMT为1∶26674,达保护滴度者比例为465%。随着时间的推移,第4年上述指标迅速下降到4686%、1∶1274和185%,第6年时低至2943%、1∶489和136%。02ml、03ml和05ml麻疹疫苗组的近期和远期效果是类似的,初免后1个月时IgG滴度越高,其免疫持久性越好;初免月龄是影响麻苗免疫效果的主要原因,6月龄初免组的免疫效果明显低于≥8月龄组。结果提示麻苗8月龄初免是可行的。  相似文献

为了了解2月龄婴儿中针对脊髓灰质炎病毒的中和抗体水平,并探讨母传抗体对脊髓灰质炎减毒活疫苗(OPV)和灭活疫苗(IPV)免疫效果的影响。对416名2月龄婴儿分别接种OPV和IPV,采集免疫前后血清,用微量中和法检测血清中Ⅰ、Ⅱ、Ⅲ型脊髓灰质炎病毒中和抗体滴度,评价抗体GMT水平及4倍增长情况。检测结果显示,2月龄婴儿母传抗体Ⅰ、Ⅱ、Ⅲ型阳性率分别为45%、38.2%和17.5%,抗体GMT水平为9.0、8.1和5.2。经接种两组疫苗后,母传抗体阳性者与阴性者免后抗体GMT水平相比,OPV组无明显差异,IPV组阳性者略低于阴性者。在免前抗体滴度<1∶32人群中,OPV组免后抗体滴度4倍增长率及几何滴度增长倍数分别为:Ⅰ型93.6%、71.2;Ⅱ型98.2%、43.7;Ⅲ型91.7%、47.9;IPV组免后抗体滴度4倍增长率及几何滴度增长倍数分别为:Ⅰ型82%、9.4;Ⅱ型62.8%、5.1;Ⅲ型95.6%、11.7;在免前抗体滴度1∶32~1∶128人群中,OPV组Ⅰ型92.3%、23;Ⅱ型86.4%、13.9;Ⅲ型55.6%、4.1;IPV组Ⅰ型48%、2.5;Ⅱ型15%、0.9;Ⅲ型55.6%、2.7。目前中国2月龄婴儿免前脊灰抗体阳性率较高,尤其是Ⅰ、Ⅱ型。脊灰母传抗体对两种疫苗免疫效果有一定干扰,对IPV疫苗的影响较为明显。  相似文献

目的评价麻疹减毒活疫苗(Measles Attenuated Live Vaccine,MV)不同接种剂量在初免及复种后的免疫效果及差异性。方法在同一观察区域、不同时段对适龄儿童分成两组采用MV0.2 ml与0.5 ml接种剂量分别在初免和复种后进行麻疹IgG抗体滴度测定。结果初免后,0.5 ml组较0.2 ml组IgG抗体阳性率、保护率分别提高5.28%与9.79%,抗体几何平均滴度(GMT)由1:543.03提高至1:813.51;初免加复种后,0.5 ml组较0.2 ml组阳性率、保护率分别提高5.32%与3.98%,GMT由1:740.49提高至1:1098.30;初免加复种较单一初免的阳性率、GMT差异有统计学意义。结论初免接种剂量0.5 ml可提高IgG抗体阳转率;复种0.5 ml与0.2 ml组的保护率差异无统计学意义,但0.5 ml组抗体阳性率、GMT较高。  相似文献

本文对比观察了213例2月龄、3月龄婴儿接种DTP后百日咳抗体水平的变化,探讨了母传抗体对免后抗体增长的影响。结果表明2月龄、3月龄婴儿DTP免疫后1个月和3个月血清中百日咳抗体达保护水平的百分率无显著性差异(x ̄2=0.036,p>0.9;x ̄2=0.327,p>0.5),免后1个月抗体GMT无显著性差异(t=0.17,p>0.5),免后3个月抗体GMT3月龄组高于2月龄组(t=2.22,p<0.05)。我们还发现免前抗体水平与免后1个月GMT虽有负相关(r=—0,754)的倾向,但总体上对抑制免后抗体应答不明显,因而建议将儿童DTP基础免疫的起始月龄提前至2月龄进行。  相似文献

麻疹是由麻疹病毒引起的严重危害儿童健康和生命的传染病。预防接种是控制麻疹最有效的措施。麻疹减毒活疫苗在全世界广泛使用30多年来,麻疹的发病率和死亡率大幅度下降,对控制全球麻疹流行发挥了重大的作用。由于母传抗体的干扰作用,该疫苗不适合早龄接种。疫苗不耐热,需要完整的冷链系统。麻疹病毒疫苗株和野毒株免疫原性的变化也有可能使麻疹减毒活疫苗的有效率降低。发展适合早龄婴儿接种的、耐热的、免疫原性更强的新一代麻疹疫苗、配合最佳的免疫策略,将会加速实现消灭麻疹的目标。  相似文献

目的:了解兰州生物制品研究所研制的b型流感嗜血杆菌结合疫苗的免疫原性。方法:采用单纯随机抽样方法对柳州市709名3~59月龄婴幼儿按照规定的免疫程序接种兰州所(或巴斯德)Hib疫苗,2年内在不同时段采集血清样本,采用ELISA方法检测Hib-PRP抗体滴度。结果:基础免疫后血清Hib-PRP抗体含量平均水平为18.043μg/ml,抗体阳转率为97.19%,1年后血清Hib-PRP抗体含量为7.575μg/ml,抗体阳转率为93.75%。加强免疫后血清Hib-PRP抗体含量为130.330μg/ml,抗体阳转率为100.00%,加强免疫1年后血清Hib-PRP抗体含量为51.723μg/ml,抗体阳转率为100.00%。基础免疫后接种巴斯德Hib疫苗组血清Hib抗体含量高于接种兰州所Hib疫苗组,抗体阳转率则没有差别;免后1年,接种兰州所Hib疫苗儿童体内Hib抗体含量平均水平高于接种法国巴斯德Hib疫苗儿童;其余各时段两组儿童血清Hib-PRP抗体含量及抗体阳转率均无差别。结论:兰州所Hib疫苗有较好的免疫原性和免疫持久性。  相似文献

目的:对2012年广州市越秀区疑似麻疹风疹的血清学结果进行整理分析,以了解麻疹风疹病例的流行特点。方法:选取2012年1月-2012年12月的166例疑似麻疹风疹患者的血清作为研究对象,统计血清中IgM阳性抗体的检出情况,以及各年龄段、月份IgM抗体阳性的例数及构成比。结果:麻疹风疹疑似病例中有36.1%的患者检测出IgM抗体阳性,其中麻疹IgM阳性以0-1岁婴幼儿的检出率较高,可达44.4%。从月份来看,12月、8月和5月是麻疹的好发时期。风疹IgM阳性以20岁以上中老年患者检出率最高,为75.0%,7月、4月和5月是风疹的易感季节。结论:疑似麻疹风疹病例有较高的IgM抗体阳性检出率,0-1岁婴幼儿和20岁以上中老年人是麻疹风疹的易感人群,4-5、7-8和12月是发病的高峰时期,临床应注意加强防范。  相似文献

A study has been carried out in the Ivory Coast to assess the efficacy of a combined vaccine against yellow fever and measles relative to that of each vaccine administered separately. Healthy children aged six to nine months were recruited and divided into two age groups: less than seven months (group I) and more than eight months (group II). In each group, they were randomly assigned to receive either yellow fever vaccine only (A), measles vaccine only (B), or the combined vaccine (C). The serological responses to measles and yellow fever were assessed in 219 initially seronegative children 45 days after immunization. More than 90% of the children developed yellow fever haemagglutination inhibiting antibodies. Neither age nor combination with measles vaccine influenced the responses to yellow fever vaccine. Measles haemagglutinational inhibiting antibodies were found in 97% of the children and the seroconversion rate was influenced neither by age nor by combination with yellow fever vaccine. Younger infants had lower titres of measles antibody. No particular adverse reactions were notified during the follow up. This study shows that combined yellow fever and measles vaccines are immunogenic in infants from the age of six months. Controlling yellow fever in endemic areas and the prevention of measles in young infants may greatly benefit by this combination.  相似文献

A single dilution technique has been used for the determination of antimeasles antibody titer. The method involved the plotting of the calibration curve and the characterization of the serum by arbitrary "evaluation units" in comparison with the specially selected positive serum whose titer was taken to be equal to 100 "evaluation units". By means of this method 57 sera obtained from children immunized against measles and 118 sera from non-vaccinated adults aged 18-22 years were examined. The values of the calculated titers were similar to those determined experimentally. This recommends this method for seroepidemiological investigations aimed at determining the level of herd immunity to measles.  相似文献

Many disease pathogens stimulate immunity in their hosts, which then wanes over time. To better understand the impact of this immunity on epidemiological dynamics, we propose an epidemic model structured according to immunity level that can be applied in many different settings. Under biologically realistic hypotheses, we find that immunity alone never creates a backward bifurcation of the disease-free steady state. This does not rule out the possibility of multiple stable equilibria, but we provide two sufficient conditions for the uniqueness of the endemic equilibrium, and show that these conditions ensure uniqueness in several common special cases. Our results indicate that the within-host dynamics of immunity can, in principle, have important consequences for population-level dynamics, but also suggest that this would require strong non-monotone effects in the immune response to infection. Neutralizing antibody titer data for measles are used to demonstrate the biological application of our theory.  相似文献

目的了解潍坊市2007-2012年麻疹疫情流行规律,为消除麻疹策略提供科学依据。方法采用描述性流行病学方法,研究探讨潍坊市2007-2012年麻疹流行规律。结果2007-2012年潍坊市报告麻疹确诊病例385例,均为散发病例,无死亡病例,年均发病率为0.72/10万,其中2008、2010年出现两个发病高峰,发病率分别为1.49/10万、1.17/10万,2012年发病率降至0.033/10万。3-5月为发病高峰,病例主要集中在寿光、诸城、安丘、青州四个市,占72.73%(280/385)。发病年龄最小2个月,最大63岁,病例年龄以〈1岁婴儿和20-44岁成年人为主。职业以民工、工人、农民和散居儿童为主,民工、工人和农民所占比例平均为52.47%(202/385);散居儿童所占比例平均为22.08%(85/385);民工、工人、农民和散居儿童发病人群所占比例逐渐升高,托幼儿童和学生逐渐降低。≥8月龄病例中有明确麻疹疫苗免疫史者96例,占26.74%;其中8月龄~14岁有明确麻疹免疫史者66例,占该年龄组病例的66%。结论潍坊市麻疹发病年龄构成以小于1岁婴儿和20~44岁成年人为主,出现向两极移动现象,1岁以下婴幼儿发病呈上升趋势。今后要提高常规免疫接种率和及时接种率,降低小年龄儿童麻疹发病率,适时对重点地区、重点人群开展麻疹疫苗强化免疫,消除免疫空白。  相似文献

IL-12, IFN-gamma, and T cell proliferation to measles in immunized infants   总被引:3,自引:0,他引:3  
Measles infection in infants is associated with severe complications, and secondary infections are attributed to generalized immunosuppression. Measles binding to its monocyte receptor down-regulates IL-12 which is expected to diminish Th1-like cytokine responses, including IFN-gamma. Whether young infants can be immunized effectively against measles is an important public health issue. We evaluated Ag-specific IL-12, IFN-gamma, and T cell responses of infants at 6 (n = 60), 9 (n = 46), or 12 mo (n = 56) of age and 29 vaccinated adults. IL-12 and IFN-gamma release by PBMC stimulated with measles Ag increased significantly after measles immunization in infants. IL-12 and IFN-gamma concentrations were equivalent in younger and older infants, but IL-12 concentrations were significantly lower in infants than in adults (p = 0.04). IL-12 production by monocytes was down-regulated by measles; the addition of recombinant human IL-12 enhanced IFN-gamma production by PBMC stimulated with measles Ag, but infant T cells released significantly less IFN-gamma than adult T cells under this condition. Of particular interest, the presence of passive Abs to measles had no effect on the specific T cell proliferation or IFN-gamma production after measles stimulation. Cellular immunity to measles infection and vaccination may be limited in infants compared with adults as a result of less effective IFN-gamma and IL-12 production in response to measles Ags. These effects were not exaggerated in younger infants compared with effects in infants who were immunized at 12 mo. In summary, infant T cells were primed with measles Ag despite the presence of passive Abs, but their adaptive immune responses were limited compared with those of adults.  相似文献

The results of 5-year observations on the duration of immunity to measles virus in persons vaccinated and revaccinated against measles, as well as in persons having had this infection, are presented. The intensity of immunity was determined in the same persons with the use of the passive hemagglutination test. The study revealed differences in the formation, intensity and duration of postvaccinal immunity. A significant decrease in the concentration of antibodies over the period of 5 years was established in 50.0-52.3% of vaccines. Revaccination with live measles vaccine is an effective measure for enhancing immunity to measles virus in persons with initial antibody titers less than 1:10-1:20, but revaccination made in a single injection is not sufficient for the stable maintenance of measles morbidity at the sporadic level. Postinfectious immunity is characterized by stability and has no tendency towards decrease. Persons having had measles have no need in additional measures irrespective of the time elapsed after the disease.  相似文献

A trivalent measles-mumps-rubella live virus vaccine, containing measles AIK-C strain, mumps Hoshino strain, and rubella Takahashi strain, was evaluated in 229 children, aged 1 to 5 years. The vaccine induced a high seroconversion rate: 221 (98.7%) out of 224 subjects initially seronegative for measles virus, 167 (93.3%) out of 179 initially seronegative for mumps virus, and 212 (99.1%) out of 214 initially seronegative for rubella virus. It also induced a sufficient cellular immunity against each of the three viruses in over 90% of the subjects, as judged by virus-specific interferon-gamma (IFN-gamma) production. Virus-specific IFN-gamma production was observed 10 days after vaccination by stimulation with measles virus and rubella virus and 14 days after vaccination by stimulation with mumps virus. Mumps-virus-specific IFN-gamma production was observed in 7 out of 12 recipients without seroconversion for mumps virus. And measles-virus-specific IFN-gamma production was demonstrated in one out of three recipients without seroconversion for measles virus. A significant correlation was observed between the serum antibody and IFN-gamma production six weeks after vaccination for measles virus (r = 0.201, P less than 0.01) and for mumps virus (r = 0.174, P less than 0.05) but not for rubella virus (r = -0.045, P less than 0.05). The incidence of febrile reactions of greater than or equal to 37.5 C was quite low, 14.4%, and that of greater than or equal to 39 C occurred in only 1.3% of the recipients. These results suggested that the trivalent vaccine induced sufficient humoral and cellular immunity and yet resulted in no more untoward reaction than observed from the measles vaccine alone.  相似文献

Antibody persistence was measured in 39 children in an open community 12-13 years after immunization against measles with further attenuated live vaccine, Biken CAM. Serum samples of the children taken every two or three years after vaccination had higher, lower, or the same HI antibody titers as those in samples taken 6 weeks after vaccination. These differences reflected a decrease in the titer in some children and subclinical natural reinfection in others. However, all the children still retained detectable antibody in 12 or 13 years after vaccination, indicating long-term persistence of immunity after immunization with Biken CAM vaccine. For evaluation of the protective efficacy of the vaccine, matched controls were studied during the same period. Serological examination revealed that 97.5% of the controls were infected with measles and contracted the disease. In contrast, none of the vaccinees developed clinical infection after close contact with measles patients.  相似文献
