Telescopic crowns: extra‐oral and intra‐oral retention force measurement –in vitro/in vivo correlation Objective: This study deals with the determination of the retentive forces of telescopic crowns measured extra‐ and intra‐orally and the correlation of these values. Background: The telescopic denture is a well‐documented solution for prosthetic rehabilitation for a partially edentulous jaw. Acceptable retention forces are needed to avoid inadvertent removal of the denture during movement of the jaws. Recent literature suggests 3–7 N per attachment to be acceptable. These values are only supported by in vitro studies. In vivo data are scarce, and a correlation of the in vitro and in vivo values is lacking. Material and Methods: Twenty‐five combined fixed‐removable prostheses with a total of 72 double crowns were used for extra‐oral retention force measurement prior to cementation (in vitro). The intra‐oral measurement was performed at 72 defined measuring points of the dentures 4–6 weeks after prosthetic rehabilitation (in vivo). A specifically designed measuring device was used. Results: The rank correlation showed that the in vitro and in vivo values correlate with each other (Spearman’s ρ = 0.5052). Additionally, it was found that the median values measured before (1.97N) insertion of the dentures were significantly lower than after (4.70N) insertion (Mann–Whitney test, p ≤ 0.0001). Conclusion: The retention forces measured before and after insertion of the denture correlate with each other although their dimension is significantly higher after insertion. The reason for this behaviour might be some tilting during removal. Nevertheless, the correlation allows a prediction of the clinically relevant forces by a measurement of the extra‐orally measured retentive values.  相似文献

Telescopic overdentures supported by the combination of natural teeth and implants have been thought a valuable treatment for the severely compromised partially edentulous patients. But the combination of teeth and implants involves highly complex biomechanical problems. This study is to evaluate biomechanical behaviors of the natural abutment teeth with the treatment of combined tooth-implant supported telescopic crown prostheses in mandible through 3D FEA. According to this study, the prosthetic option supported by a combination of teeth and implants and retained by double crowns could protect teeth and their periodontal support tissues acting as a rigid splint, and may be a valuable treatment option for partially edentulous patients with severely reduced remaining teeth in mandible.  相似文献

doi: 10.1111/j.1741‐2358.2011.00572.x Compressive strains and displacement in a partially dentate lower jaw rehabilitated with two different treatment modalities Background: Understanding of the biomechanical consequences of the stresses generated to the supporting bone during occlusal loading is significant for improving the design and clinical planning process in partial edentulism therapy. Objective: The aim of this study was to analyse the distribution of strain and displacement on the partially dentate lower jaw rehabilitated with an partial denture (RPD) and to compare it to the strain and displacement distribution on a partially dentate lower jaw rehabilitated with a cantilever fixed partial denture (FPD). Material and methods: The experimental models were a partially dentate mandible with full‐arch PFM crowns and RPD and a partially dentate mandible rehabilitated with a full‐arch cantilever FPD. Strains and displacement were measured using the Digital Image Correlation Method. Results: Displacement values of the first experimental model ranged from 0.31 to 0.54 mm with strains from 1.35 to 2.34%. Analysis of the second experimental model results showed displacement values from 0 to 0.34 mm, while strains were in the range of 0–1.40%. Conclusion: Higher displacements and strains of bone tissue were observed below the RPD, especially in the region of the distal abutment and distal portion of the free‐end saddle. Strains within bone and the bone‐denture contact area were mostly influenced by the teeth and denture vertical displacement.  相似文献

In order to explore the application of the precision extracoronal attachment based on three-dimensional finite element stress model in the repair of unilateral mandibular dissociation and loss, the stress distribution of the butment and periodontal tissues when the precision extracoronal attachment was used in unilateral mandibular dissociation and loss was analyzed, so as to provide reference for the application of materials in clinical repair. In this study, the biomechanics of oral cavity and its attachment were analyzed to provide theoretical basis for model construction. Because the degree of periodontal tissue absorption and the number of abutment teeth affected the restoration effect of the coronal precision attachment, the models were established from these two perspectives. The results showed that when the periodontal tissue absorption of both abutment teeth exceeded 1/3, the method of double abutment restoration was not recommended. When three abutments were used, there was a significant decrease compared with the absorption of the periodontal tissue of the double abutment. In clinic, the stress distribution of the distal abutment teeth and its periodontal tissues can be improved by increasing the number of abutment tooth. However, since increasing the number of abutments required an increase in the amount of healthy tooth tissue, it couldn’t be blindly selected in the clinic. After comprehensive consideration, it is appropriate to choose two abutment teeth for restoration.  相似文献

Few studies are performed to evaluate the influence of connection type on the stress distribution of distal extension mandibular removable partial dentures (RPDs) supported by both implants and natural teeth. In this study, five three-dimensional finite element models were prepared to simulate mandibular bilateral partially edentulous arches. Four were RPDs supported by both implants and natural teeth, and the other one was RPDs supported only by natural teeth. The maximum equivalent (EQV) stress values of bone around implants, the abutments, and the mucosa displacements of the related supporting structures were measured. It was found that a non-rigid telescopic coping was more favorable to protect the implant than a rigid telescopic coping. Compared with other connection types, the easy resilient attachment (ERA) system seemed to be effective to associate implant without complications. However, the results obtained in the present study should be cautiously interpreted in the clinic.  相似文献

Although immediate implantation has not been previously recommended in infected sites, it is now becoming a procedure of choice in modern implant dentistry. We report a case of a 65-year-old male patient, who required multiple tooth extractions in the lower jaw and fabrication of a new overdenture in the lower jaw and a complete denture in the upper jaw. Immediate implantation in infected tooth sockets followed extraction. Two NobelReplace Tapered implants (Nobel BioCare, Zürich, Switzerland), one 13 mm long and 4.3 mm wide and the other 13 mm long and 5.0 mm wide, were placed in the position 33 and 43. The site was sutured and a temporary denture was fabricated by adjusting the old denture. After a period of three months the implant site was reopened and healing abutments were placed. Impressions were made using the Impregum Penta Soft (3M ESPE, St. Paul, Minn, USA) polyether material. Ball Abutment Titanium" was used as a patrix attachment and a matrix was inserted into the denture. Clinical examination and x-ray analysis after six months showed no significant changes of the implants. Bone resorption was within standard values. Although it is still a controversial subject, immediate implantation in infected sites can be a safe and predictable procedure if surgical protocols are followed. However, further research is needed to draw firm conclusions.  相似文献

To enable attritional studies on telescopic crowns, a specimen holder has been constructed that is intended to simulate, in zero approximation, the biomechanical properties of abutments bearing telescopic crowns. The specimen holder consists essentially of a system with two metal "concertinas" with different elasticity constants reflecting the different biomechanical parameters for periodontium and bone. The biomechanical processes in the periodontium are too complex to be simulated accurately in vitro. Nevertheless, the specimen holder described here enables the forces acting during insertion and removal of a partial denture to be accommodated elastically in the angular and axial directions, to simulate the possible degrees of freedom of the abutments in situ. In this way, the unavoidable errors of specimen fixation are rendered tolerable, so that local preferential wear is decisively reduced.  相似文献

This cross-sectional study aimed to identify and quantify up to 42 target species colonizing the early biofilm of dental implants restored with titanium or zirconia abutments. A total of 720 samples from 20 healthy individuals were investigated. Biofilm samples were collected from the peri-implant sulci, inner parts of implants, abutment surfaces and prosthetic crowns over a functioning period of 30 days. Checkerboard DNA–DNA hybridization was used for microbial detection and quantitation. Clinical characteristics (probing depth, bleeding on probing, clinical attachment level and marginal bone loss) were also investigated during the monitoring period. Genome counts were low at the implant loading time point for both the abutment materials, and increased over time. Both the titanium and the zirconia groups presented similar microbial counts and diversity over time, and the microbiota was very similar to that colonizing the remaining teeth. Clinical findings were consistent with a healthy condition with no significant difference regarding marginal bone loss between the two materials.  相似文献

The aim of this study was to screen patients with fixed prosthodontic appliances that were in oral cavity for a period of 5 years or more and to assess clinically and radiologically root caries, gingival recession, pocket formation, alveolar ridge resorption, as well as gingival (GI) and plaque index (PI). The aim also was to find out the differences between materials and constructions, between abutment and non-abutment teeth, and to find out the need for replacement. A total of 260 patients and their orthopantomograms were examined, with a total of 2,265 teeth, 610 being bridge abutments and 246 being crowns. The most frequent were metal+ acrylic veneer crowns or bridges. Root caries was found under the abutments in 10-20%; however abutments with ceramic crowns had the lowest percentage of caries (p < 0.01). Alveolar ridge resorption, pocket formation deeper than 3 mm and gingival recession of various degree was found in 50% of the cases, again with the lowest percentage of ceramic-fused-to-metal appliances (p < 0.01). Pocket depth was registered in significantly higher percentage in metal-acrylic veneer appliances compared to natural teeth (p < 0.01), while there was no significant difference between metal-ceramic appliances and natural teeth (p > 0.05). Although the worst findings were recorded for metal- + acrylic veneer crowns for PI, no significant difference existed between crowns of different material or non-abutment teeth (p > 0.05). There was statistically significant difference between abutments with metal + acrylic veneer crowns, full metal crowns, metal ceramic crowns and non-abutments for GI scores. Higher percentage of scores 0 and 1 was recorded for metal ceramic crowns and non-abutments and significantly higher percentage of scores 2 and 3 was recorded for metal + acrylic veneer crowns and full metallic crowns. Almost 50% of metal-ceramic abutments had no pathologic findings. Almost 30% of the patients needed replacement, or even some abutments to be extracted and therefore a new prosthodontic appliance.  相似文献

It is difficult to make direct measurements of the displacement of a partial denture as a function of the magnitude and direction of a force applied to the occulusive surface of the denture teeth during chewing. An indirect approach is possible, using a theoretical model in conjunction with some particular experimental measurements that are easier to make. The purpose of this paper is to present a theoretical linear model.

The origin of the coordinate system is chosen at the tissue level under the center of the lower first molar. We first compute the total depth to which an arbitrary point on the ridge crest line will go when the denture is depressed into the ridge tissue during mastication. Using the approximation of linear depth force relationship, we find:

Objective: Stabilisation of partial dentures is an important part of prosthodontic treatment, particularly in patients with an excessive reduction in alveolar bone supporting their abutment teeth. In these cases, overdenture prostheses are one possible treatment choice. Background: The aim of this study was to evaluate the effects of overdentures on the condition of alveolar bone of the remaining teeth, and to evaluate the overdenture concept in the prosthetic treatment of partially edentulous patients. Materials and methods: The investigation was conducted on 22 patients during observation periods of 6 and 10 years. After preparation, all abutment teeth were provided with either amalgam restorations or gold copings. An overdenture in the lower jaw was fabricated for every patient. A special film holder was used to repeat the X‐ray procedure in every patient in the same manner. The radiographs were evaluated using the grid scale. The alveolar bone level of all remaining teeth was evaluated on dental films by measuring the distance between the root apex and coronal border of the alveolar bone. Results: A statistically significant difference (p < 0.01) was calculated 6 years after delivery stage between distal alveolar bone levels of experimental abutments under overdentures and the remaining teeth of the control group. Alveolar bone of abutment teeth was considerably preserved, particularly in seven patients observed 10 years after provision of their dentures. Conclusion: Within the limitation of this study, the validity of the overdenture treatment in partially edentulous patients is concluded. Good oral hygiene was crucial for the success of treatment with overdentures.  相似文献

Background: The effect of different mucosa characteristics on stress distribution of complete dentures and overdentures remains unknown. Objective: The aim of this study was to evaluate the effect of different mucosa thickness and resiliency on the stress distribution of complete dentures and implant‐retained overdentures using a two‐dimensional finite element analysis. Material and methods: Representative models of the edentulous mandible were constructed on AutoCAD software according to the groups’ characteristics. In group CD, a model of the edentulous mandible supporting a complete denture was obtained while in group IO, a model of edentulous mandible supporting an overdenture over two unsplinted implants with an o’ ring system was constructed. In each group, mucosa assumed three characteristics of thickness (1, 3 and 5 mm) corresponding to the resiliencies hard, resilient and soft respectively. Evaluation was performed on Ansys software with 100N vertical load applied on central incisor teeth. The principal stress was used as analysis criteria. Results: Group IO showed higher stress values than group CD regardless of mucosal thickness and resiliency. Stress decreased at the supporting tissues in both groups as the thickness and resiliency of mucosa increased. In relation to the supporting tissues, cortical bone showed the highest stress values. Conclusion: It was concluded that the use of an attachment system increases stress values and the thickness and resiliency of mucosa influence more on these values.  相似文献

A novel measuring set-up based on a hexapod system for use in dental biomechanics is described. It was specially developed to measure force/deflection characteristics of different dental materials and devices. The functionability and suitability of the system for use in experimental biomechanics were investigated in two different studies. In a first study the micro mobility of prosthetic telescopic crowns prior to and after simulated wear was determined to investigate the influence of wear processes on the stability of the anchorage elements and thus of prostheses. This study investigated the ability of the setup to load a specimen with high forces or torques of up to 100 Newton. The second study looked at the force/deflection characteristics of orthodontic anchorage pins used in orthodontics to additionally stabilize the anchorage unit, for example during molar movement. In this study specimens were loaded with small forces of less than 10 Newton, as are typically used in orthodontics. Using the setup, the deflection behaviour of these devices under high and low loading was measured at a resolution of approximately one micrometer or one angular second.  相似文献

目的探讨精密附着体固位体对基牙牙周组织的影响,为精密附着体固位体的临床应用提供理论基础。方法记录了在戴牙时,修复后1个月,修复后3个月,修复后6个月四个时段,基牙的菌斑指数(PLI),牙龈指数(GI),牙龈出血指数(GBI),探诊深度(PD)。结果精密附着体基牙牙周各指数四个时段纵向比较,牙龈出血指数、牙周袋深度在四个时段与对照组差异均无显著性;基牙菌斑指数和牙龈指数在修复后3个月与戴牙前差异有显著性(P〈0.05);6个月后差异有非常显著性(P〈0.01)。结论精密附着体对基牙牙周有一定的影响,应强化口腔卫生宣传教育。  相似文献

目的将隐形义齿与传统卡环固位式可摘局部义齿进行对比,探寻隐形义齿对基牙龈沟内菌群的影响,从而指导活动义齿的选择和设计。方法下颌第一磨牙缺失的患者随机分成3组。分别戴用弯制卡环式可摘局部义齿、铸造卡环式可摘局部义齿和隐形义齿。定时对基牙龈沟进行取样并行细菌培养,并统计细菌总量、专性厌氧菌检出率以及G-菌检出率。结果戴牙后同期相比,隐形义齿组与传统卡环固位式可摘局部义齿相比,基牙龈沟内细菌总量更大,专性厌氧菌检出率及G-菌检出率更高,差异具有统计学意义。结论上述研究结果说明:相较于传统卡环固位式可摘局部义齿,隐形义齿对基牙龈沟内菌群的影响更大。对于牙周状况不佳的患者应当谨慎使用隐形义齿作为修复方式。  相似文献

Effect of pre‐processing methods on bond strength between acrylic resin teeth and acrylic denture base resin Objectives: This study evaluated the effects of various pre‐processing methods on the bond strength between resin and denture teeth. Backgrounds: Debonding of acrylic resin teeth from denture base material is a problem for patients wearing complete dentures. Materials and Methods: Four experimental groups (n = 30) were investigated by subjecting tooth–resin bonding to tensile loading. Specimens were prepared and tested according to the methods of the International Standards Organization (ISO 22112:2005) using a special assembly. Four pre‐processing surface treatments of teeth were applied: (i) ST1, no treatment applied (control); (ii) ST2, wax solvent (Dewaks, Faber Kimya & Ilaç, Turkey); (iii) ST3, boiling water followed by conditioning with methyl methacrylate (MMA) monomer (Meliodent, Bayer Dental, Germany); (iv) ST4, boiling water followed by wax solvent agent and finally MMA monomer application. Bond strength test was performed using a universal testing machine. Results: All the strength values of the test groups were within clinically acceptable limits. The lowest values were from the ST1 group and the highest values were in the ST4 group. Conclusions: Wax elimination methods affected bonding strength. Application of wax solvent and MMA monomer to the ridge lap surfaces of the teeth gave the best results. In clinical practice, this application procedure may decrease the bonding failure of denture teeth.  相似文献

Aspects of mosasaur dental ontogeny are well preserved in many fossils of these giant marine squamates. Replacement teeth on the tooth-bearing elements (TBEs) first appear as small enamel crowns positioned posterolingual to the attached tooth (posterolabial for the pterygoid). Several developing crowns, of progressively larger size, are aligned in rows relating to a specific tooth position. The crowns rest in a dental groove that varies in width and depth depending on the TBE. The crown closest to the attached tooth is always the largest and is found in a small resorption pit. As resorption proceeds, the pit expands in volume (cementum and alveolar bone), and the crown increases in size and settles into the pit. Once mature crown size is achieved, the dentine root and cementum portion of the root develop rapidly, the attached tooth is lost and the replacement tooth erupts out of the alveolus. Mosasaurid teeth develop along a 'zig-zag'-shaped movement path: horizontally along the dental groove, down into the alveolus, and up and out of the alveolus prior to attachment to the alveolar wall. At no point in mosasaurid tooth development are the crowns observed in a horizontal position. The mosasaurid dental lamina appears to have been a continuous strip of dental epithelium as it is in other squamates. Mosasaurid tooth attachment is thecodont (histologically and geometrically) not subpleurodont. Objectives: Wear of attachments leads to a loss of retention and potentially reduces the function of complete dentures. This study evaluated the retention force changes of different prefabricated attachment systems for implant‐supported overdentures to estimate the wear constancy and applicability in clinical practice. Methods: Four prefabricated attachment systems were tested [Group SG: retentive ball attachment (Straumann, Switzerland) with gold matrix, Group ST: retentive ball attachment (Straumann, Switzerland) with titanium spring matrix, Group IB: UNOR i‐Ball with Ecco matrix (UNOR, Switzerland) and Group IMZ: IMZ®‐TwinPlus ball attachment with gold matrix (DENTSPLY Friadent, Germany)]. Ten samples of each system were subjected to 10 000 insertion‐separation cycles. Results: Results showed that all types of attachments showed wear, which led to a loss of retention force after an initial increase at the beginning of the wear simulation. Attachments with a plastic retention insert or gold matrices underwent the smallest changes in retention force. The titanium spring system showed the largest changes in retention force and a greater variation between the different cycles and specimen. This behaviour is probably caused by a large fitting tolerance of the titanium spring. Conclusions: Attachment systems which possess a male and female component of different material composition are preferable. They show smaller changes in the retention force. For retention force increase and wear compensation, an attachment system should be adjustable.  相似文献

Use of resin‐bonded extracoronal attachments with removable partial denture: case report Introduction: Resin‐bonded extracoronal attachments may be indicated for the abutment teeth of removable partial dentures, especially for anterior teeth when a cingulum rest must be provided. This type of treatment has a series of advantages such as minimal tooth reduction, supragingival margins, favourable stress distribution, and improved aesthetic appearance. Objective: To report a clinical case of oral rehabilitation using a combination of resin‐bonded extracoronal attachments joined by a Dolder bar with a removable partial denture. Case report: A 60‐year‐old male patient with only the canines in the maxillary arch was restored with a combination of resin‐bonded extracoronal attachments joined by a Dolder bar and a removable partial denture. Conclusion: Dentures with resin‐bonded extracoronal attachments can have a number of advantages over traditional clasp‐retained removable partial dentures.  相似文献
