目的:研究不同术前皮肤准备方案与手术切口感染(SSI)的关系,为降低临床SSI发生率提供参考。方法:选择自2015年1月~2019年12月在医院行手术治疗的患者1810例为本次研究对象。根据析因设计表,将因素A:是否剃毛(1不剃毛;2剃毛),B:清洁方式(1清水清洁;2肥皂水清洁),C:术前备皮时间(1术前1 d;2术前2 h)配对分为8个组:A1B1C1组226例,A1B2C1组229例,A1B1C2组216例,A1B2C2组232例,A2B1C1组221例,A2B2C1组241例,A2B1C2组221例,A2B2C2组224例,比较各组手术部位及切口类型分布、术后SSI发生率,并采用析因分析法分析术前皮肤准备后各组菌落计数的相关性及交互作用。结果:各组患者的手术部位及切口类型之间的差异不存在统计学意义(P0.05)。A1B1C1组及A2B1C1组的SSI发生率较高,分别为12.83%和14.48%。A1水平的SSI发生率是8.75%,与A2水平的8.27%相比,差异不存在统计学意义(P0.05)。B1、C1水平的SSI发生率分别是11.31%、10.03%,明显高于B2、C2水平的5.83%、6.94%,差异均存在统计学意义(P0.05)。各组术前皮肤准备后的菌落计数差异存在统计学意义(P0.05),析因分析结果显示,B、C单因素分析差异存在统计学意义(P0.05),且A与C,B与C间具有交互作用,而A、B、C间具有二级交互作用(P0.05)。结论:术前皮肤准备对降低SSI发生具有重要作用,实际操作时,建议在较短的时间内利用肥皂水或其他消毒水进行皮肤清洗并完成备皮。  相似文献

探讨感染控制与抗菌药物管理措施干预对骨科手术部位感染(surgical site infection, SSI)的影响,为有效降低SSI的发生提供循证依据。以2013—2018年在上海市杨浦区中心医院骨科住院的 8 916 例手术患者作为研究对象,其中干预前 2 462 例,干预后 4 636 例,强化干预 1 818 例。采取综合性感染控制集束化管理与抗菌药物管理干预措施,分为干预前(2013—2014年)、干预后(2015—2017年)和强化干预(2018年)3个阶段,主要措施包括术前手卫生及患者准备、术中保温及围术期合理用药等。对骨科SSI情况及干预效果进行评估。结果显示,实施感染控制的集束化措施与有效的抗菌药物管理措施后,手术患者SSI发生率由干预前的1.58%下降至0.61%(P<0.05)。干预前、后及强化干预Ⅰ、Ⅱ类切口预防用抗菌药物的品种合理率分别为54.83%、79.82%和99.04%,时机合理率分别为51.99%、78.53%和89.97%,疗程合理率分别为52.98%、82.19%和91.97%,结果显示抗菌药物品种选择、给药时机及疗程的合理率明显改善(P<0.05),且平均住院天数及抗菌药物占住院总费用的比例明显下降(P<0.05),表明长期连续的综合性感染控制与抗菌药物管理干预可有效降低骨科手术患者SSI的发生。  相似文献

目的:探讨腹腔镜胆囊切除术治疗高龄患者急性胆囊炎的应用价值。方法:回顾性分析2005年1月.2011年12月我院收治的210例65岁以上因急性胆囊炎实施胆囊切除术的老年患者的临床资料,按手术方式分为腹腔镜组(LC组)和剖腹胆囊切除术组(OC组),分析和比较两组患者的手术时间、术后肠功能恢复时间及住院时间,术中出血、腹腔引流量和术后并发症的发生情况。结果:与OC组比较,LC组的手术时间、术后肠功能恢复时间及住院时间均显著缩短,差异有统计学意义(P〈0.01);但两组之间术中出血、腹腔引流量和术后并发症的发生率差异均无统计学意义(P〉0.05)。LC组中转开腹10例,占7_35%;其中粘连严重导致胆囊三角解剖不清6例,无法控制的出血2例,结石嵌顿胆囊管2例。结论:老年急性胆囊炎患者在条件合适的情况下行腹腔镜胆囊切除术治疗有助于患者更快地恢复.具有较强的临床应用价值。  相似文献

BACKGROUND: Risk factors associated with surgical site infection (SSI) and the development of short-term complications in macaques undergoing vascular access port (VAP) placement are evaluated in this study. METHODS: Records from 80 macaques with VAPs were retrospectively reviewed. Logistic regression was used to identify factors associated with short-term post-operative complications. RESULTS: The primary outcome was SSI, which occurred in 21.6% (52.6% in the first 12 months vs. 13% thereafter) of procedures. SSI was associated with major secondary complications including VAP removal (11.4%), wound dehiscence (5.7%), and mechanical catheter occlusion (5.7%). In multivariate modeling, only surgical program progress was a statistically significant predictor of SSI, while animal compliance had a slightly protective effect. CONCLUSIONS: Vascular access ports have a moderate risk of complications, provided the surgical program optimizes best practices. Under complex experimental conditions, VAPs represent an important refinement, both improving animals' overall well-being and environment and reducing stress.  相似文献

摘要 目的:了解新型冠状病毒(以下简称"新冠")肺炎定点医院病区终末消毒控制措施并监测其消毒效果。方法:对中山大学附属第五医院隔离病区实施终末消毒,并对终末消毒前后空气和物表进行采样检测及终末消毒后环境进行采样检测,评估消毒效果。结果:对14个隔离病区开展了终末消毒,终末消毒前后均采集空气沉降菌样品156份、物表样品254份,终末消毒后采集环境核酸样品209份进行消毒效果评价。各隔离病区经终末消毒后空气消毒效果合格率均为100.00%,高于终末消毒前空气消毒效果合格率76.20%(P<0.05);终末消毒后物表消毒效果为94.88%,高于终末消毒前物表消毒效果合格率71.65%(P<0.05);终末消毒后环境核酸检测结果均为阴性。结论:中山大学附属第五医院各隔离病区经终末消毒后各项指标均符合终末消毒评价要求。  相似文献

Procedural and surgical site infections create difficult and complex clinical scenarios. A source for pathogens is often thought to be the skin surface, making skin preparation at the time of the procedure critical. The most common skin preparation agents used today include products containing iodophors or chlorhexidine gluconate. Agents are further classified by whether they are aqueous-based or alcohol-based solutions. Traditional aqueous-based iodophors, such as povidone-iodine, are one of the few products that can be safely used on mucous membrane surfaces. Alcohol-based solutions are quick, sustained, and durable, with broader spectrum antimicrobial activity. These agents seem ideal for longer open surgeries with the potential for irrigation or surgical spillage, such as cystoprostatectomy, radical prostatectomy, and retroperitoneal lymph node dissection.Key words: Skin pathogens, Procedural and surgical site infection, Skin preparation solutionsSurgical site infection (SSI) complicates an estimated 5% of all clean-contaminated operations performed annually in US hospitals and accounts for the most common nosocomial infection in surgical patients.1 Patients who develop SSI have longer and costlier hospitalizations and are more likely to spend time in an intensive care unit (ICU), are 5 times more likely to be readmitted, and are twice as likely to die.2Recognizing this substantial morbidity and economic burden, in 1999 the Centers for Disease Control (CDC) issued standardized guidelines for the prevention of surgical infections. These included making specific evidence-based recommendations for modifying patient factors that may predispose to infection, for the use of antimicrobial prophylaxis, for optimizing sterility in the operating room, and for the use of antiseptic agents for skin preparation.The choice of which specific agent to use for skin preparation was not addressed due to the diversity of sites and approaches in surgery, as well as the absence of data on SSI risk in well-controlled, operation-specific studies.1 Therefore, the choice of agent should be based primarily on the surgeon’s knowledge of the product’s efficacy, cost, and ease of use. Urologic surgeons have the additional challenge of choosing the best agent for the variety of procedures that they perform, including intraperitoneal and extraperitoneal surgery; scrotal, perineal, and vaginal operations; endoscopy; and percutaneous renal surgery. Each of these operative sites has different endogenous flora, body contours, and skin types, all factors that influence the risk of SSI and, therefore, the best type of antiseptic skin agent to use. This article focuses on skin preparation for the prevention of SSI with an assessment of currently available antiseptic products and their application to urologic surgery.  相似文献

摘要 目的:探究一步法腹腔镜胆囊切术(LC)联合胆总管探查取石术(LCBDE)在慢性胆囊炎胆囊结石合并胆总管结石治疗中的有效性及安全性。方法:纳入2018年6月至2021年9月行一步法LC+LCBDE治疗的慢性胆囊炎胆囊结石合并胆总管结石患者49例(观察组),并以行开腹胆囊切除术+胆总管切开取石治疗的慢性胆囊炎胆囊结石合并胆总管结石患者43例为对照组,比较两组手术疗效及手术相关指标;观察患者手术前后肝功能指标、胆红素水平及免疫功能变化,并统计患者术后并发症发生情况。结果:观察组及对照组手术成功率均为100%,两组对比无明显差异(P>0.05);观察组手术时间、肠鸣音恢复时间、肛门恢复排气时间及住院时间短于对照组,术中出血量少于对照组(P<0.05);观察组术后丙氨酸氨基转移酶(ALT)、γ-谷氨酰转肽酶(GGT)、天冬氨酸氨基转移酶(AST)及总胆红素(TBIL)、直接胆红素(DBIL)、间接胆红素(IBIL)水平均低于对照组(P<0.05);观察组术后7 d的免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)水平均高于对照组(P<0.05);观察组术后并发症发生率低于对照组(P<0.05)。结论:一步法LC+LCBDE治疗慢性胆囊炎胆囊结石合并胆总管结石的成功率高,可促进术后胃肠功能及肝功能恢复,提高机体免疫力,并能降低术后并发症发生率。  相似文献

Swabbing skin to collect bacteria for enumeration revealed that a single washing or rinsing of the swab in buffer removed between 90 and 95% of the bacteria collected. Further removal of the remaining bacteria from the initial swab by repeated washings of the swab produced plate counts that showed nearly proportional decreases in the numbers and types of bacteria retained or eluted from the swab. None of the commonly isolated cutaneous bacteria was retained or eluted from the single swab in numbers that misrepresented measurement of their proportions present in the sample. Populations of nonlipophilic bacteria on skin when present were mainly gram-positive catalase-producing cocci. Diphtheroids when present were chiefly lipophilic. Media containing furoxone were selective for cutaneous diphtheroids. The concentration of furoxone used affected isolation and enumeration of the diphtheroids. Lipophilic diphtheroids from the stratum corneum were basically aerobic and anaerobic incubation reduced or eliminated this group from enumeration studies. Lipophilic furoxone-resistant aerobic diphtheroids from several areas, particularly the nasal passages, occurred in numbers inversely proportional to the numbers of staphylococci for periods up to 11 months. The cocci-diphtheroid relationships occurred independently of the total number of aerobic bacteria present, the transient appearance of yeasts or gram-negative bacilli, the required use of certain antibiotics by the subjects, climate, age, and sex.  相似文献

Although widely used, the radial forearm flap has been criticized for the poor quality of its donor site. Attempts to avoid donor-site problems have concentrated on the elaboration of the split-thickness and full-thickness skin graft methods of reconstruction. Skin grafts frequently fail over the flexor carpi radialis tendon, leading to chronic skin breakdown or, at best, tendon adhesion. Tissue expansion appears to be a good alternative that allows the use of local tissues to ultimately improve the forearm donor-site appearance. To avoid the disadvantages of traditional silicone balloon expanders (such as pressure peaks, infection, the valve at a distance from the expander, postoperative fillings), an osmotically active system was used. In an 18-month prospective study, 10 osmotically active hydrogel tissue expanders were placed on the forearms of 10 patients. The radial forearm flap was performed for intraoral reconstruction after surgical resection of oral cavity malignancies. The study showed that, in nine out of 10 patients, the expanded skin achieved was sufficient to cover the donor site after raising the forearm flap. Additionally, the expansion-related swelling pressure was well tolerated by the patients, the cosmetic results were very satisfactory, and the incidence of complications was very low. By using osmotically active hydrogel tissue expanders, there is no postoperative filling and no risk of complications arising from defective balloon expanders, filling valves, or missing ports.  相似文献

目的:探讨喉癌患者手术部位发生感染(Surgical site infection,SSI)的相关危险因素及干预对策,为临床预防提供参考。方法:回顾性分析2012年7月-2013年11月因喉癌而在我院行喉切除术的83例患者的临床资料,根据术后是否发生SSI将患者分为感染组(n=12例)和非感染组(n=71例)。分析两组患者术后SSI的危险因素,并提出相应护理对策。结果:83例患者中,12例术后发生SSI,发生率为14.46%;单因素分析显示,感染组和非感染组在临床III期以上、术前气管切开、手术时间4 h、全喉切除等方面存在显著差异性(P0.05);多因素回归分析显示:患者的临床分期、术前气管是否切开、手术时间及手术切除方式是术后发生SSI的独立危险因素(P0.05)。结论:喉癌术后感染与临床分期、手术时间及切除方式等因素有关,采取有效的干预对策可减少术后SSI的发生率。  相似文献

Skin bacteria at peripheral intravenous catheter (PIVC) insertion sites pose a serious risk of microbial migration and subsequent colonisation of PIVCs, and the development of catheter related bloodstream infections (CRBSIs). Common skin bacteria are often associated with CRBSIs, therefore the bacterial communities at PIVC skin sites are likely to have major implications for PIVC colonisation. This study aimed to determine the bacterial community structures on skin at PIVC insertion sites and to compare the diversity with associated PIVCs. A total of 10 PIVC skin site swabs and matching PIVC tips were collected by a research nurse from 10 hospitalised medical/surgical patients at catheter removal. All swabs and PIVCs underwent traditional culture and high-throughput sequencing. The bacterial communities on PIVC skin swabs and matching PIVCs were diverse and significantly associated (correlation coefficient = 0.7, p<0.001). Methylobacterium spp. was the dominant genus in all PIVC tip samples, but not so for skin swabs. Sixty-one percent of all reads from the PIVC tips and 36% of all reads from the skin swabs belonged to this genus. Staphylococcus spp., (26%), Pseudomonas spp., (10%) and Acinetobacter spp. (10%) were detected from skin swabs but not from PIVC tips. Most skin associated bacteria commonly associated with CRBSIs were observed on skin sites, but not on PIVCs. Diverse bacterial communities were observed at skin sites despite skin decolonization at PIVC insertion. The positive association of skin and PIVC tip communities provides further evidence that skin is a major source of PIVC colonisation via bacterial migration but microbes present may be different to those traditionally identified via culture methods. The results provide new insights into the colonisation of catheters and potential pathogenesis of bacteria associated with CRBSI, and may assist in developing new strategies designed to reduce the risk of CRBSI.  相似文献

目的:探讨腹腔镜与十二指肠镜联合治疗胆囊结石合并胆总管结石的临床应用价值。方法:74例胆囊结合并胆总管结石患者随机分为EST+LC组(44例)和OC+OCHTD组(30例),其中EST+LC组行十二指肠镜Oddi括约肌切开术(EST)加腹腔镜下胆囊切除术(LC),OC+OCHTD组行开腹胆囊切除术(OC)加胆总管切开取石T管引流术(OCHTD),比较两组手术过程、术后恢复、并发症情况等。结果:①EST+LC组手术时间长于OC+OCHTD组,切口长度与术中出血量少于OC+OCHTD组(P〈0.01或0.05)。②EST+LC组手术成功率90.91%,取石成功率100%,OC+OCHTD组手术成功率100%,取石成功率93.33%,两组间差异无显著性(P〉0.05)。③EST+LC组镇痛药使用率低于OC+OCHTD组,术后引流时间、肠鸣音恢复时间、肛门排气时间和平均住院时间短于OC+OCHTD组(P〈0.01或0.05)。④EST+LC组与OC+OCHTD组均并发症少,无结石复发。结论:十二指肠镜Oddi括约肌切开术联合加腹腔镜下胆囊切除术治疗胆囊结石和胆总管结石具有创伤小、恢复快、并发症少、住院时间短等优势,值得进一步临床推广。  相似文献

OBJECTIVE--To determine whether percutaneous transluminal coronary angioplasty may be safely performed in cardiology centres in the United Kingdom without immediate on site cardiac surgical cover for complications arising at angioplasty. DESIGN--Retrospective review of coronary angioplasties and complications in a hospital without on site cardiac surgical cover. SETTING--All angioplasties were performed in the catheterisation laboratory of the Belfast City Hospital. Revascularisation surgery for complicated coronary angioplasty was performed in the cardiac surgical unit of the Royal Victoria Hospital, 2.4 km away from the catheterisation laboratory. PATIENTS--540 Coronary angioplasties were performed on 512 patients between late 1982 and November 1988. Indications included stable angina, unstable rest angina, and suitable coronary disease at coronary arteriography after myocardial infarction. MAIN OUTCOME MEASURES--In hospital mortality after complicated coronary angioplasty and delay to surgical revascularisation after acute coronary occlusion at angioplasty. RESULTS--Coronary angioplasty was successful in 444 cases (82%). Acute coronary occlusion occurred in 35 cases (6.5%). Twelve patients required urgent revascularisation surgery and were transferred safely to the surgical unit; none of these patients died. A mean delay of 268 minutes (range 180-390 minutes) occurred before revascularisation compared with 273 minutes (range 108-420 minutes) in the Royal Victoria Hospital, where on site surgical cover was available. The principal cause of delay was the wait for a cardiac operating theatre to become available and not the transfer time between hospitals. Five deaths occurred after coronary angioplasty, a mortality of 0.9%. Three deaths were related to acute coronary occlusion. The absence of immediate surgical help did not influence the outcome in any patient. CONCLUSION--With careful selection of patients coronary angioplasty may be safely performed in a hospital without on site cardiac surgical facilities, provided that these are available at a nearby centre.  相似文献

OBJECTIVE: To examine the effect of the introduction of laparoscopic cholecystectomy (LC) on patterns of practice (number of cholecystectomy procedures, case-mix and length of hospital stay) and patient outcomes in Ontario. DESIGN: Cross-sectional population-based time trends using hospital discharge data. SETTING: All acute care hospitals in Ontario where cholecystectomy was provided. PATIENTS: All 119,821 Ontario residents who underwent cholecystectomy between 1989-90 and 1993-94. After exclusions (initial bile duct exploration, cancer, incidental cholecystectomy, or missing codes for age, sex or residence) 108,442 patients remained. OUTCOME MEASURES: Number of cholecystectomy procedures, proportion of patients with acute or chronic gallstone disease, length of hospital stay, and rates of death, readmission, and bile duct injury and other in-hospital complications after cholecystectomy by year. RESULTS: The number of cholecystectomy procedures increased by 30.4% between 1989-90 and 1993-94. The number of patients with chronic gallstone disease increased by 33.6%, and the number who underwent elective surgery increased by 48.3%. The proportion of procedures performed as LC increased from 1.0% in 1990-91 to 85.6% in 1993-94. Patients who received LC tended to be younger female patients with chronic gallstone disease with no coexisting conditions undergoing elective operations. The mean length of stay, adjusted for case-mix differences, was significantly lower in 1993-94 than in 1989-90 (2.6 days v. 7.5 days) (p < 0.05); the values for LC and open cholecystectomy in 1993-94 were 1.8 days and 7.3 days respectively. The decrease in the crude death rate over the study period (0.3% to 0.2%) was not significant (relative odds 1.10, 95% confidence interval [CI] 0.72 to 1.69). In 1993-94 the adjusted risk of readmission to hospital within 30 days was 1.38 (95% CI 1.19 to 1.58) as compared with 1989-90. Over the 5 years the rate of bile duct injuries tripled (0.3% in 1989-90 v. 0.9% in 1993-94). The adjusted risk of having at least one complication after cholecystectomy in 1993-94 was 1.90 (95% CI 1.75 to 2.07) as compared with 1989-90. CONCLUSIONS: LC has had a substantial effect on the number of cholecystectomy procedures performed, the type of patient having the gallbladder removed and the length of hospital stay. Death rates are unchanged, but the odds of readmission and in-hospital complications are both increased. Future research should be directed toward determining the reasons for the overall increase in rates, developing methods to reduce bile duct injuries and identifying other relevant outcomes, such as patient satisfaction with the procedure.  相似文献

Cutaneous leishmaniasis (CL) is a parasitic disease characterized by single or multiple ulcerations. Secondary bacterial infections are one of the complications that can increase the tissue destruction and the resulting scar. To better determine the incidence of real secondary bacterial infections in CL, we designed the current study. This was a cross-sectional study performed in Skin Diseases and Leishmaniasis Research Centre, Isfahan, Iran. A total of 1,255 patients with confirmed CL enrolled in the study. Sterile swaps were achieved for ulcer exudates and scraping was used for non-ulcerated lesions. All samples were transferred to tryptic soy broth medium. After 24 hr of incubation at 37 degrees C they were transferred to eosin methylene blue agar (EMB) and blood agar. Laboratory tests were used to determine the species of bacteria. Among 1,255 confirmed CL patients, 274 (21.8%) had positive cultures for secondary bacterial infections. The bacteria isolated from the lesions were Staphylococcus aureus in 190 cases (69.3%), coagulase negative Staphylococcus in 63 cases (23.0%), E. coli in 10 cases (3.6%), Proteus sp. in 6 cases (2.2%), and Klebsiella sp. in 5 cases (1.9%). The results show that the overall incidence of secondary bacterial infections in the lesions of CL was 21.8%, considerably high. The incidence of secondary bacterial infections was significantly higher in ulcerated lesions compared with non-ulcerated lesions.  相似文献

Ventriculostomy-related infection (VRI) is a serious complication of external ventricular drain (EVD) but its natural history is poorly studied. We prospectively tracked the bacteria pathways from skin towards ventricles to identify the infectious process resulting in ventriculostomy-related colonization (VRC), and VRI. We systematically sampled cerebrospinal fluid (CSF) on a daily basis and collected swabs from both the skin and stopcock every 3.0 days for microbiological analysis including in 101 neurosurgical patient. Risk factors for positive event defined as either VRC or VRI were recorded and related to our microbiological findings. A total of 1261 CSF samples, 473 skin swabs, and 450 stopcock swabs were collected. Skin site was more frequently colonized than stopcock (70 (60%) vs 34 (29%), p = 0.023), and earlier (14 ±1.4 vs 24 ±1.5 days, p<0.0001). Sixty-one (52%) and 32 (27%) skin and stopcock sites were colonized with commensal bacteria, 1 (1%) and 1 (1%) with pathogens, 8 (7%) and 1 (1%) with combined pathogens and commensal bacteria, respectively. Sixteen positive events were diagnosed; a cutaneous origin was identified in 69% of cases. The presence of a pathogen at skin site (6/16 vs 4/85, OR: 11.8, [2.5–56.8], p = 0.002) and CSF leakage (7/16 vs 6/85, OR 10 [2.4–41.2], p = 0.001)) were the two independent significant risk factors statistically linked to positive events occurrence. Background

Surgical Site Infections (SSI) are relatively frequent complications after colorectal surgery and are associated with substantial morbidity and mortality.

Objective

Implementing a bundle of care and measuring the effects on the SSI rate.

Design

Prospective quasi experimental cohort study.

Methods

A prospective surveillance for SSI after colorectal surgery was performed in the Amphia Hospital, Breda, from January 1, 2008 until January 1, 2012. As part of a National patient safety initiative, a bundle of care consisting of 4 elements covering the surgical process was introduced in 2009. The elements of the bundle were perioperative antibiotic prophylaxis, hair removal before surgery, perioperative normothermia and discipline in the operating room. Bundle compliance was measured every 3 months in a random sample of surgical procedures.

Results

Bundle compliance improved significantly from an average of 10% in 2009 to 60% in 2011. 1537 colorectal procedures were performed during the study period and 300 SSI (19.5%) occurred. SSI were associated with a prolonged length of stay (mean additional length of stay 18 days) and a significantly higher 6 months mortality (Adjusted OR: 2.71, 95% confidence interval 1.76–4.18). Logistic regression showed a significant decrease of the SSI rate that paralleled the introduction of the bundle. The adjusted Odds ratio of the SSI rate was 36% lower in 2011 compared to 2008.

Conclusion

The implementation of the bundle was associated with improved compliance over time and a 36% reduction of the SSI rate after adjustment for confounders. This makes the bundle an important tool to improve patient safety.  相似文献

Background

Post-discharge surgical site infections (SSI) are a major source of morbidity, expense and anxiety for patients. However, patient perceptions about barriers experienced while seeking care for post-discharge SSI have not been assessed in depth. We explored patient experience of SSI and openness to a mobile health (mHealth) wound monitoring “app” as a novel solution to address this problem.

Methods

Mixed method design with semi-structured interviews and surveys. Participants were patients who had post-discharge surgical wound complications after undergoing operations with high risk of SSI, including open colorectal or ventral hernia repair surgery. The study was conducted at two affiliated teaching hospitals, including an academic medical center and a level 1 trauma center.

Results

From interviews with 13 patients, we identified 3 major challenges that impact patients'' ability to manage post-discharge surgical wound complications, including required knowledge for wound monitoring from discharge teaching, self-efficacy for wound monitoring at home, and accessible communication with their providers about wound concerns. Patients found an mHealth wound monitoring application highly acceptable and articulated its potential to provide more frequent, thorough, and convenient follow-up that could reduce post-discharge anxiety compared to the current practice. Major concerns with mHealth wound monitoring were lack of timely response from providers and inaccessibility due to either lack of an appropriate device or usability challenges.

Conclusions

Our findings reveal gaps and frustrations with post-discharge care after surgery which could negatively impact clinical outcomes and quality of life. To address these issues, we are developing mPOWEr, a patient-centered mHealth wound monitoring application for patients and providers to collaboratively bridge the care transition between hospital and home.  相似文献
