ObjectiveMalondialdehyde (MDA) is an end-product formed during lipid peroxidation, due to degradation of cellular membrane phospholipids. MDA is released into extracellular space and finally into the blood; it has been used as an effective biomarker of lipid oxidation. High circulating levels of MDA have been previously described in patients with ischemic stoke than in controls, and an association between circulating MDA levels and neurological functional outcome in patients with ischemic stoke. However, an association between serum MDA levels and mortality in patients with ischemic stroke has not been previously reported, and that was the objective of this study.MethodsObservational, prospective and multicenter study performed in six Intensive Care Units. We included patients with severe malignant middle cerebral artery infarction (MMCAI) defined as Glasgow Coma Scale (GCS) lower than 9. We measured serum MDA levels in 50 patients with severe MMCAI at the time of diagnosis and in 100 healthy subjects. Mortality at 30 days was the end point of the study.ResultsWe found that patients with severe MMCAI showed higher serum MDA levels than healthy subjects (p<0.001). We found higher serum MDA levels (p<0.001) in non-surviving MMCAI patients (n=26) than in survivors (n=24). The area under the curve for prediction of 30-day mortality for serum MDA levels was 0.77 (95% CI = 0.63-0.88; p<0.001). Serum MDA levels >2.27 nmol/mL were associated with 30-day mortality (OR=7.23; 95% CI=1.84-28.73; p=0.005) controlling for GCS and age on multiple binomial logistic regression analysis.ConclusionsTo our knowledge, this is the first study showing that serum malondialdehyde levels in patients with MMCAI are associated with early mortality.  相似文献

Background:Carcinoembryonic antigen (CEA) is a common gastrointestinal tumor biomarker. Irisin is adipo-myokines that has been suggested to have a potential role in cancer development. However, limited studies test irisin as biomarker in gastric and colorectal cancers. Therefore, this study aims to investigate whether CEA and irisin could be a potential diagnostic biomarker in gastric and colorectal cancer.Methods:A case-control study consists of 90 subjects, 21 gastric cancer patients, 49 colorectal cancer patients and 20 control. Serum CEA was detected by fluorescence immunoassay (FIA) kit. Serum irisin was determined by enzyme-linked immunosorbent assay (ELISA) kit.Results:Serum CEA increases significantly and serum irisin decreases significantly in gastric and colorectal cancer patients. According to Receiver Operating Characteristic (ROC) curve analysis, in gastric cancer, the area under curve of CEA is 1.00 (95% CI, 1.000-1.000, p< 0.0001). The diagnostic cut-off of CEA is< 3.08 ng/ml with %100 sensitivity and 100% specificity. The area under curve of irisin is 0.94 (95% CI, 0.8177-1.000, p< 0.0001). The cut-off of irisin is> 30.2 ng/ml with %90 sensitivity and 100%, specificity. In colorectal cancer, the area under curve of CEA is 0.99 (95% CI, 0.9866-1.000, p< 0.0001) and the diagnostic value< 2.6 ng/ml with %98 sensitivity and %100 specificity. The area under curve of irisin is 0.96 (95% CI, 0.9155-1.000, p< 0.0001). The diagnostic cut-off of irisin is> 41.9 ng/ml with 88.1sensitivity and 90.5 specificity.Conclusion:CEA and irisin could be powerful potential diagnostic biomarkers which would be use for early detection of gastric and colorectal cancers.Key Words: Biomarker, Carcinoembryonic antigen (CEA), Colorectal cancer, Gastric cancer, Irisin  相似文献

目的:探讨老年H型高血压合并急性缺血性脑卒中患者血清网膜素-1(omentin-1)、鸢尾素(Irisin)水平与病情及预后的关系。方法:选择2017年6月-2019年9月我院收治的老年H型高血压合并急性缺血性脑卒中患者92例,记作合并脑卒中组,根据美国国立卫生研究院卒中量表(NIHSS)评分将患者分为轻症组28例(NIHSS评分≤4分)、中症组39例(5分≤NIHSS评分≤20分)和重症组25例(NIHSS评分>20分),根据改良Rankin量表(mRS)评分将患者分为预后不良组28例(mRS评分>2分)和预后良好组64例(mRS评分≤2分)。另选择同期我院收治的单纯老年H型高血压患者90例作为单纯H型高血压组,分析合并脑卒中组患者血清omentin-1、Irisin水平及omentin-1、Irisin与NIHSS评分、mRS评分的相关性,并应用ROC曲线分析血清omentin-1、Irisin水平对患者预后的预测价值。结果:合并脑卒中组血清omentin-1、Irisin水平显著低于单纯H型高血压组(P<0.05),随脑卒中神经缺损严重程度的升高,H型高血压合并急性缺血性脑卒中患者血清omentin-1、Irisin水平逐渐降低(P<0.05),预后不良组血清omentin-1、Irisin水平显著低于预后良好组(P<0.05)。老年H型高血压合并急性缺血性脑卒中患者血清omentin-1、Irisin水平与NIHSS评分及mRS评分均呈负相关(P<0.05)。omentin-1最佳临界值为105.36 ng/ml,敏感度为78.23%,特异度为83.44%;Irisin最佳临界值为90.77 ng/L,敏感度为71.00%,特异度为61.43%。结论:老年H型高血压合并急性缺血性脑卒中患者血清omentin-1、Irisin水平异常降低,其水平与神经缺损程度和预后呈负相关,血清omentin-1、Irisin对老年H型高血压发生急性缺血性脑卒中的预后评估具有一定价值。  相似文献

目的:探讨缺血性脑卒中患者血清超敏C反应蛋白(hs-CRP)、糖化血红蛋白(HbA1c)水平及其与神经功能缺损的关系。方法:选择2016年10月~2017年9月我院接诊的123例缺血性脑卒中患者作为观察组及同期于我院进行体检的健康人群123例作为对照组,检测和比较两组血清hs-CRP、HbA1c水平的差异,并分析缺血性脑卒中患者血清hs-CRP、HbA1c水平与其美国国立卫生研究院卒中量表(NIHSS)评分的关系。结果:观察组患者血清hs-CRP、Hb A1c水平显著高于对照组[(6.23±1.97)mg/L、(7.96±0.65)%vs.(2.54±0.85)mg/L、(5.21±0.30)%],NIHSS评分明显高于对照组[(4.08±3.12)分vs. 8.62±3.25)分],差异具有统计学意义(P0.05);缺血性脑卒中患者血清hs-CRP、Hb A1c水平与NIHSS评分呈显著正相关(r=-0.465,-0.564,P0.05)。结论:缺血性脑卒中患者血清hs-CRP和HbA1c水平均明显上调,二者可以在一定程度上反映缺血性脑卒中患者神经功能缺损的严重程度。  相似文献

摘要 目的:探究急性缺血性脑卒中(AIS)患者血清视黄醇结合蛋白4(RBP4)、脂蛋白磷脂酶A2(Lp-PLA2)及单核细胞趋化蛋白-1(MCP-1)水平表达及其与患者病情及预后的关系。方法:选取2020年6月~2021年12月我院收治的130例初发AIS患者作为研究对象,根据美国国立卫生研究院卒中量表(NIHSS)评分分为轻度组、中度组、重度组,另取同期在我院体检的志愿者45例作为对照组。采用酶联免疫吸附测定(ELLSA)法检测并对比各组血清RBP4、Lp-PLA2、MCP-1水平差异。改良Rankin量表(MRS)评估AIS患者入院14 d预后,比较不同预后患者临床资料差异。采用Pearson相关系数分析血清RBP4、Lp-PLA2、MCP-1水平与NIHSS、MRS评分的相关性。受试者工作特征(ROC)曲线分析血清RBP4、Lp-PLA2、MCP-1对AIS患者预后的预测价值。结果:AIS患者的血清RBP4、Lp-PLA2、MCP-1水平均明显高于对照组,且随着病情程度的加重,血清RBP4、Lp-PLA2、MCP-1水平逐渐升高(P<0.05)。预后不良组年龄、血清RBP4、Lp-PLA2、MCP-1水平以及NIHSS评分均明显高于预后良好组(P<0.05)。AIS患者的血清RBP4、Lp-PLA2、MCP-1水平与NIHSS、MRS评分之间均呈正相关(P<0.05)。血清RBP4、Lp-PLA2、MCP-1三项联合检测预测AIS患者预后的ROC曲线下面积为0.957,明显高于各指标单独检测的0.775、0.799、0.781。结论:AIS患者血清RBP4、Lp-PLA2、MCP-1水平显著升高,升高程度与患者病情及预后密切相关,三项指标联合检测对AIS患者预后具有良好的预测价值。  相似文献

Background:Irisin as an exercise-induced myokine was proposed to improve bone health. This study investigated the role of serum irisin (s-irisin) in patients with osteoporosis (OP) through correlating to most biological bone markers and oxidative stress.Methods:A cross-sectional study recruited an eligible 175 postmenopausal women at Al-Hussien Teaching Hospital, Iraq. They were scanned by DEXA and stratified into two groups based on T-score; the first 95 patients as control group (GI) with −1 ≤ T-score and the second 80 patients as cases group (GII) with T-score ≤ −2.5. Demographic criteria were age, bone mineral density (BMD, g/cm2) and T-score. Serum irisin, total serum calcium (s-calcium), serum inorganic phosphate (s-phosphate), serum alkaline phosphatase (s-ALP), serum 25 [OH] vitamin D, the serum parathyroid hormone (s-PTH), serum Carboxy terminal collagen crosslinks (CTx), serum procollagen type I C-termidnal peptide (s-PICP), serum malondialdehyde (s-MDA) and serum superoxide dismutase (s-SOD) were collected from blood samples.Results:Serum irisin were 31.84 ± 2.65 vs. 20.88 ± 2.71 ng/mL for control and trial groups, respectively. Lower levels of BMD, T-score, 25 [OH] vitamin D, and s-irisin along with a higher serum levels of PTH, CTx, PICP, MDA and SOD were observed in patients with osteoporosis. All parameters were statistically meaningful upon correlation (p< 0.0001), except age and s-calcium (p= 0.0088 and p= 0.187, respectively).Conclusion:The results showed that, a significantly lower serum irisin levels among osteoporosis women, was intimately correlated to most bone turnover markers and it can be considered as encouraging results for clinical application in prediction and treatment of osteoporosis.Key Words: Bone turnover markers, DEXA scan, Irisin, T-score, BMD, osteoporosis, post-menopause  相似文献

Objectives:To evaluate the effects of different intensity exercises on maximal oxygen consumption (VO2max and levels of components, namely brain-derived neurotrophic factor (BDNF), tyrosine kinase receptor B (TrKB), vascular endothelial growth factor (VEGF), peroxisome proliferator activated receptor-gamma coactivator (PGC-1α), and irisin.Methods:Thirty-six male participants were divided into control (CNT), low-intensity (LIIT), and high-intensity interval training (HIIT) groups. LIIT and HIIT groups consisted of 8 exercises (20 s work and rest in each repetition, respectively) conducted for 4 weeks. VO2max and protein component levels were determined pre- and post-training. VO2max capacity was also determined using the Yo Yo Intermittent Recovery Test-1 (Yo Yo IR-1). Statistical analysis was conducted to determine significance of the differences observed.Results:According to the YoYo IR-1, VO2max, serum BDNF, VEGF, PGC1α, irisin, and TrkB data obtained in the study, a statistically significant difference between the groups was observed (p<0.05). While the interaction effect was found to be statistically significant in the study using PGC1α, VEGF, and TrkB data (p<0.05), it was not found to be statistically significant using YoYo IR-1, VO2max, serum BDNF, or irisin data (p>0.05).Conclusion:HIIT and LIIT improved all study parameters, while HIIT showed a greater effect than LIIT.  相似文献

Abstract

Background: Hyperthyroidism is associated with increased metabolic activity and thermogenesis. Irisin is a key molecule in thermogenesis and energy expenditure via adipose tissue browning. Epicardial fat was previously defined as brown-like fat. Thus, here we aimed to evaluate the association between serum irisin level and epicardial fat thickness (EFT) in patients with hyperthyroidism.

Methods: A total of 25 hyperthyroid patients and 24 age-, sex- and BMI-matched healthy controls were enrolled. Serum irisin levels, thyroid hormone levels, and body compositions were compared. EFT was measured via transthoracic echocardiography.

Results: Serum irisin level and EFT were significantly higher in the hyperthyroid group (p?<?0.001 and p?=?0.001, respectively). The distributions of fat-free mass, muscle mass and fat mass were similar between the study groups. Serum irisin level was negatively correlated with TSH (p?<?0.001) and positively correlated with fT3 (p?<?0.001), fT4 (p?<?0.001) and TSH receptor antibody (p?=?0.002) levels and EFT (p?=?0.001). In multivariate linear regression analysis, TSH (β?=??0.475, p?<?0.001) and EFT (β?=?0.290, p?=?0.023) levels were significantly associated with serum irisin levels.

Conclusions: An increased serum irisin level associated with EFT might contribute to metabolic derangement in hyperthyroidism. Further studies are needed to elucidate whether irisin levels and EFT are affected by hyperthyroidism or vice versa.  相似文献

Background and Purpose

YKL-40 is associated with various neurological disorders. However, circulatory YKL-40 levels early after onset of acute ischemic stroke (AIS) have not been systematically assessed. We aimed to identify the temporal changes and clinical usefulness of measuring serum YKL-40 immediately following AIS.

Methods

Serum YKL-40 and C-reactive protein (CRP) levels were monitored over time in AIS patients (n = 105) and compared with those of stroke-free controls (n = 34). Infarct volume and stroke severity (National Institutes of Health Stroke Scale; NIHSS) were measured within 48 hours of symptom onset, and functional outcome (modified Rankin Scale; mRS) was measured 3 months after AIS.

Results

Within 12 hours of symptom onset, levels of YKL-40 (251 vs. 41 ng/mL) and CRP (1.50 vs. 0.96 µg/mL) were elevated in AIS patients compared to controls. The power of YKL-40 for discriminating AIS patients from controls was superior to that of CRP (area under the curve 0.84 vs. 0.64) and YKL-40 (r = 0.26, P<0.001) but not CRP levels were correlated with mRS. On day 2 of admission (D2), YKL-40 levels correlated with infarct volume and NIHSS. High YKL-40 levels predicted poor functional outcome (odds ratio 5.73, P = 0.03). YKL-40 levels peaked on D2 and declined on D3, whereas CRP levels were highest on D3.

Conclusions

Our results demonstrate serial changes in serum YKL-40 levels immediately following AIS and provide the first evidence that it is a valid indicator of AIS extent and an early predictor of functional outcome.  相似文献

BackgroundCXC chemokine ligand 16 (CXCL16) is an inflammatory chemokine that mediates renal infiltration of macrophages and activated T cells. Aim: To investigate serum levels of CXCL16 in patients undergoing hemodialysis and their correlation with other inflammatory markers such as C-reactive protein (CRP) and intact parathyroid hormone (iPTH).MethodsThe study included 40 hemodialysis patients (22 males) and 40 age and gender-matched controls (24 males). Fasting blood sugar (FBS), urea, creatinine, calcium and inorganic phosphorous were assayed in participants using routine methods, glycosylated hemoglobin (HbA1c) by quantitative chromatographic spectrophotometry, iPTH by chemiluminescent microparticle immunoassay, CRP by nephelometry and CXCL16 by ELISA technique.ResultsSerum CXCL16, CRP, PTH, FBS, HbA1c, phosphorus, urea, and creatinine levels were significantly higher in hemodialysis patients compared to controls (p<0.00001). No statistically significant differences were observed between patients and controls for calcium. Serum CXCL16 levels correlated positively with CRP (r=0.956, p<0.00001) and iPTH (r=-0.403, p<0.001). Hemodialysis patients (diabetics or hypertensives) had significantly higher CXCL16 levels compared to non-diabetics or non-hypertensives.ConclusionsHigh levels of serum CXCL16, CRP and iPTH reflect the inflammatory status of hemodialysis patients and help avoid complications. Serum CXCL16 could be used as a biomarker together with CRP in these patients.  相似文献

Complement activation and inflammation have been suggested in the pathogenesis of stroke, mannose-binding lectin (MBL) were found to have roles during the process. The aim of this study was to investigate the relationship between acute ischemic stroke (AIS) and serum MBL levels in Chinese population. From January 1 to June 30 2013, all patients with first-ever AIS were recruited to participate in the study. Serum MBL levels and routine test were examined. The National Institutes of Health Stroke Scale (NIHSS) score was assessed on admission blinded to MBL levels. During the inclusion period, 148 patients with AIS were registered and completed study. The results indicated that the serum MBL levels were significantly (p < 0.0001) higher in acutely ischemic stroke patients as compared to normal controls [1,332; interquartile range (IQR) 996–2,134 μg/L and 897; IQR 678–1,100 μg/L, respectively]. There was a correlation between serum levels of MBL and NIHSS score [r (spearman) = 0.608, p < 0.0001). In multivariate analysis, serum MBL as a continuous variable was associated with an increased risk of AIS, after adjustment for above possible confounders (OR 1.002, 95 % CI 1.001–1.008; p < 0.0001). These results indicated that elevated MBL levels could be considered as an independent stroke risk factor in Chinese population, suggesting a role of MBL and the lectin pathway of complement activation in the pathogenesis of stroke.  相似文献

摘要 目的:探讨游离三碘甲腺原氨酸(FT3)/游离甲状腺素(FT4)比值、载脂蛋白B (Apo B)/载脂蛋白A1(Apo A1)比值与急性缺血性脑卒中(AIS)患者临床疗效和出院后短期预后的关系。方法:选取2020年2月至2022年2月安徽省第二人民医院收治的102例AIS患者,所有患者入院后接受血运重建、抗血小板等治疗,根据临床疗效将患者分为有效组(76例)和无效组(26例)。治疗后检测所有AIS患者血清FT3、FT4、Apo B、Apo A1水平,计算FT3/FT4比值、Apo B/Apo A1比值,所有AIS患者出院后随访3个月,根据AIS患者随访期间的预后情况将其分为预后不良组(31例)和预后良好组(71例)。比较不同组间血清FT3、FT4、Apo B、Apo A1水平以及FT3/FT4比值、Apo B/Apo A1比值差异,分析影响AIS患者出院后短期预后的因素。结果:无效组血清FT3水平、FT3/FT4比值低于有效组(P<0.05),血清Apo B水平、Apo B/Apo A1比值高于有效组(P<0.05)。预后不良组血清FT3水平、FT3/FT4比值低于预后良好组(P<0.05),血清Apo B水平、Apo B/Apo A1比值高于预后良好组(P<0.05)。美国国立卫生院神经功能缺损评分(NIHSS评分)高、Apo B/ApoA1比值增高是AIS患者出院后短期预后不良的危险因素(P<0.05),FT3/FT4比值增高是其保护因素(P<0.05)。结论:AIS临床治疗无效和预后不良患者FT3/ FT4比值降低,Apo B/ApoA1比值增高,低FT3/ FT4比值和高Apo B/ApoA1比值与AIS患者出院后短期预后不良有关。  相似文献

目的:探讨急性缺血性脑卒中(AIS)患者血清半乳糖凝集素3(GAL3)、血清趋化因子12(CXCL12)水平与病情严重程度和预后的关系。方法:选取成都市第五人民医院于2016年2月~2018年9月期间接收的AIS患者138例为观察组,另选取同期来该院行健康体检的志愿者60例为对照组。其中观察组根据美国国立卫生研究所中风量表(NIHSS)评分分为轻症组(n=42,4分),中症组(n=61,4~15分),重症组(n=35,15分),根据改良RABKIN量表(m RS)评分分为预后良好组(n=82)和预后不良组(n=56)。比较对照组、观察组的血清GAL3、CXCL12水平,分析不同NIHSS得分、不同预后的血清GAL3、CXCL12水平,采用Pearson相关性分析血清GAL3、CXCL12水平与NIHSS评分、m RS评分的相关性。结果:观察组血清GAL3、CXCL12水平均显著高于对照组,差异有统计学意义(P0.05)。重症组、中症组AIS患者血清GAL3、CXCL12水平高于轻症组,且重症组高于中症组,差异有统计学意义(P0.05)。预后不良组的AIS患者血清GAL3、CXCL12水平均高于预后良好组,差异有统计学意义(P0.05)。Pearson相关性分析结果可知,血清GAL3、CXCL12水平与NIHSS评分、m RS评分均呈正相关(P0.05)。结论:AIS患者的血清GAL3、CXCL12水平均异常升高,且其升高程度与AIS患者病情严重程度及预后息息相关。  相似文献

MethodsWe included all consecutive first-ever ischemic stroke patients, without hippocampal strokes or recurrent stroke/TIA, aged 18–50 years, admitted to our academic hospital between 1980 and 2010. One hundred and forty-six patients underwent T1 MPRAGE, DTI scanning and completed the Rey Auditory Verbal Learning Test and were compared with 84 stroke-free controls. After manual correction of hippocampal automatic segmentation, we calculated mean hippocampal fractional anisotropy (FA) and diffusivity (MD).ResultsOn average 10 years after ischemic stroke, lesion volume was associated with lower ipsilateral hippocampal integrity (p<0.05), independent of hippocampal volume. In patients with a normal ipsilateral hippocampal volume (volume is less than or equal to 1.5 SD below the mean volume of controls) significant differences in ipsilateral hippocampal MD were observed (p<0.0001). However, patients with a normal hippocampal volume and high hippocampal MD did not show a worse memory performance compared with patients with a normal volume and low hippocampal MD (p>0.05).ConclusionsPatients with average ipsilateral hippocampal volume could already have lower ipsilateral hippocampal integrity, although at present with no attendant worse memory performance compared with patients with high hippocampal integrity. Longitudinal studies are needed to investigate whether a low hippocampal integrity after stroke might lead to exacerbated memory decline with increasing age.  相似文献

The aim of this study was to evaluate the prognostic value of serum and cerebrospinal fluid (CSF) free fatty acid (FFA) levels in a cohort of patients with an acute ischemic stroke (AIS). In a prospective study, FFA levels were measured using an enzyme cycling method on admission in serum and CSF of 252 consecutive patients with AIS. The prognostic value of FFA to predict the functional outcome and mortality within 90-day was compared with the National Institutes of Health Stroke Scale score and with other known outcome predictors. Serum and CSF levels of FFA increased with increasing severity of stroke as defined by the NIHSS score (all P?<?0.001). Patients with an unfavorable outcomes and non-survivors had significantly increased FFA serum and CSF levels on admission (all P?<?0.0001). Multivariate logistic regression analysis adjusted for common risk factors showed that serum FFA ≥0.71 mmol/L (third quarters) was an independent predictor of functional outcome (odds ratios (OR)?=?4.86; 95 % confidence interval (CI) 2.26–10.48) and mortality (OR?=?7.72; 95 % CI 3.01–21.48). The area under the receiver operating characteristic curve of serum FFA was 0.79 (95 % CI, 0.72–0.86) for functional outcome and 0.86 (95 % CI, 0.78–0.94) for mortality. Similarly, CSF FFA level also was an indicator for predicting of functional outcome and mortality. FFA levels in serum and CSF may serve as independent biomarkers in addition of the traditional methods for assessing the functional outcome and mortality of AIS.  相似文献

摘要 目的:探讨不同病情急性缺血性脑卒中(AIS)患者血清过氧化还原蛋白1(PRDX1)、钙调蛋白(CAM)、触珠蛋白(HPT)水平与预后的关系。方法:收集2018年4月~2019年4月期间本院收治的127例AIS患者为研究对象,根据患者的病情分为轻度组(39例)、中度组(48例)、重度组(40例),根据患者的改良Rankin 量表( mRS)评分将患者分为预后良好组(73例)和预后不良组(54例),另选同期在我院进行健康检查的健康受试者50例为对照组。对比所有受试者PRDX1、CAM、HPT水平。对比不同预后患者的一般资料、PRDX1、CAM、HPT水平。分析PRDX1、CAM、HPT与NIHSS评分、mRS评分的关系。以多因素Logistic回归分析AIS患者预后的影响因素。结果:轻度组、中度组、中度组的PRDX1、CAM、HPT水平均高于对照组,且随着患者的病情加重,PRDX1、CAM、HPT水平依次升高,差异均有统计学意义(P<0.05)。预后不良组患者的年龄、PRDX1、CAM、HPT均高于预后良好组(P<0.05)。经Pearson检验,AIS患者的PRDX1、CAM、HPT与NIHSS、mRS评分均呈正相关(P<0.05)。经多因素Logistic回归分析可得,年龄较高、PRDX1水平升高、CAM水平升高、HPT水平升高是AIS患者预后不良的危险因素(P<0.05)。结论:AIS患者的PRDX1、CAM、HPT水平异常升高,且与其病情及预后呈现明显的相关性,年龄、PRDX1、CAM、HPT是患者预后的影响因素,对于病情的评估、预后的判断有一定的临床指导价值。  相似文献

Introduction

The capability of CT perfusion (CTP) Alberta Stroke Program Early CT Score (ASPECTS) to predict outcome and identify ischemia severity in acute ischemic stroke (AIS) patients is still questioned.

Methods

62 patients with AIS were imaged within 8 hours of symptom onset by non-contrast CT, CT angiography and CTP scans at admission and 24 hours. CTP ASPECTS was calculated on the affected hemisphere using cerebral blood flow (CBF), cerebral blood volume (CBV) and mean transit time (MTT) maps by subtracting 1 point for any abnormalities visually detected or measured within multiple cortical circular regions of interest according to previously established thresholds. MTT-CBV ASPECTS was considered as CTP ASPECTS mismatch. Hemorrhagic transformation (HT), recanalization status and reperfusion grade at 24 hours, final infarct volume at 7 days and modified Rankin scale (mRS) at 3 months after onset were recorded.

Results

Semi-quantitative and quantitative CTP ASPECTS were highly correlated (p<0.00001). CBF, CBV and MTT ASPECTS were higher in patients with no HT and mRS≤2 and inversely associated with final infarct volume and mRS (p values: from p<0.05 to p<0.00001). CTP ASPECTS mismatch was slightly associated with radiological and clinical outcomes (p values: from p<0.05 to p<0.02) only if evaluated quantitatively. A CBV ASPECTS of 9 was the optimal semi-quantitative value for predicting outcome.

Conclusions

Our findings suggest that visual inspection of CTP ASPECTS recognizes infarct and ischemic absolute values. Semi-quantitative CBV ASPECTS, but not CTP ASPECTS mismatch, represents a strong prognostic indicator, implying that core extent is the main determinant of outcome, irrespective of penumbra size.  相似文献

We aimed to evaluate the association between 25-hydroxyvitamin D [25(OH) D] levels and both clinical severity at admission and outcome at discharge in patients with acute ischemic stroke (AIS). From June 2012 to October 2013, consecutive first-ever AIS patients admitted to the Department of Emergency of The Fourth Affiliated Hospital of Harbin Medical University, China were identified. Clinical information was collected. Serum 25(OH) D levels were measured at baseline. Stroke severity was assessed at admission using the National Institutes of Health Stroke Scale (NIHSS) score. Functional outcome was evaluated at discharge using the modified Rankin scale (m-Rankin). Multivariate analyses were performed using logistic regression models. During the study period, 326 patients were diagnosed as AIS and were included in the analysis. Serum 25(OH) D levels reduced with increasing severity of stroke as defined by the NIHSS score. There was a negative correlation between levels of 25(OH) D and the NIHSS (r = ? 0.389, P = 0.000). In multivariate analyses, serum 25(OH) D level was an independent prognostic marker of discharge favorable functional outcome and survival [odds ratio 3.96 (2.85–7.87) and 3.36 (2.12–7.08), respectively, P = 0.000 for both, adjusted for NHISS, other predictors and vascular risk factors] in patients with AIS. Serum 25(OH) D levels are a predictor of both severity at admission and favorable functional outcome in patients with AIS. Additional research is needed on vitamin D supplementation to improve the outcome of post-stroke patients.  相似文献

QI软件分析Xevo~? G2-XS QTOF质谱系统MSE采集的子离子数据,精确定量到1054个脂质特征差异,准确定性得到368个脂质分子,多变量统计分析中差异脂质组成能将AIS患者和健康志愿者区分开来,通路富集分析图显示差异脂质主要参与甘油磷脂代谢的紊乱。结论:AIS患者血浆脂质组成与健康志愿者存在显著差异,差异表达的脂质可能与AIS发生有关。这些发现有助于开发新的诊断标志物和AIS治疗靶点。  相似文献

Objective

Evaluate the predictive value of Boston Acute Stroke Imaging Scale (BASIS) in acute ischemic stroke in Chinese population.

Methods

This was a retrospective study. 566 patients of acute ischemic stroke were classified as having a major stroke or minor stroke based on BASIS. We compared short-term outcome (death, occurrence of complications, admission to intensive care unit [ICU] or neurological intensive care unit [NICU]), long-term outcome (death, recurrence of stroke, myocardial infarction, modified Rankin scale) and economic index including in-hospital cost and length of hospitalization. Continuous variables were compared by using the Student t test or Kruskal-Wallis test. Categorical variables were tested with the Chisquare test. Cox regression analysis was applied to identify whether BASIS was the independent predictive variable of death.

Results

During hospitalization, 9 patients (4.6%) died in major stroke group while no patients died in minor stroke group (p<0.001), 12 patients in the major stroke group and 5 patients in minor stroke group were admitted to ICU/NICU (p = 0.001). There were more complications (cerebral hernia, pneumonia, urinary tract infection) in major stroke group than minor stroke group (p<0.05). Meanwhile, the average cost of hospitalization in major stroke group was 3,100 US$ and 1,740 US$ in minor stroke group (p<0.001); the average length of stay in major and minor stroke group was 21.3 days and 17.3 days respectively (p<0.001). Results of the follow-up showed that 52 patients (26.7%) died in major stroke group while 56 patients (15.1%) died in minor stroke group (P<0.001). 62.2% of the patients in major stroke group and 80.4% of the patients in minor stroke group were able to live independently (P = 0.002). The survival analysis showed that patients with major stroke had 80% higher of risk of death than patients with minor stroke even after adjusting traditional atherosclerotic factors and NIHSS at baseline (HR = 1.8, 95% CI: 1.1–3.1).

Conclusion

BASIS can predict in-hospital mortality, occurrence of complication, length of stay and hospitalization cost of the acute ischemic stroke patients and can also estimate the long term outcome (death and the dependency). BASIS could and should be used as a dichotomous stroke classification system in the daily practice.  相似文献
