When Muslims thought of establishing milk banks, religious reservations were raised. These reservations were based on the concept that women's milk creates 'milk kinship' believed to impede marriage in Islamic Law. This type of kinship is, however, a distinctive phenomenon of Arab tradition and relatively unknown in Western cultures. This article is a pioneer study which fathoms out the contemporary discussions of Muslim scholars on this issue. The main focus here is a religious guideline (fatwa) issued in 1983, referred to in this article as 'one text', by the Egyptian scholar Yūsuf al-Qaradāwī who saw no religious problem in establishing or using these banks. After a number of introductory remarks on the 'Western' phenomenon of milk banks and the 'Islamic' phenomenon of 'milk kinship', this article analyses the fatwa of al-Qaradāwī 'one text' and investigates the 'two contexts' in which this fatwa was discussed, namely, the context of the Muslim world and that of Muslim minorities living in the West. The first context led to rejecting the fatwa and refusing to introduce the milk banking system in the Muslim world. The second context led to accepting this system and thus allowing Muslims living in the West to donate and receive milk from these banks. Besides its relevance to specialists in the fields of Islamic studies, anthropology and medical ethics, this article will also be helpful to physicians and nurses who deal with patients of Islamic background.  相似文献

The use of contraceptives has become prevalent among females in Thailand in the past 20 years, and oral contraceptive use has been suggested to trigger changes in fat intake, energy expenditure, fat metabolism and blood pressure. Based on field investigations of 391 married women aged 20 years or over in Yasothon Province, North-east Thailand, this study aims to elucidate the effects of oral contraceptive use on body mass index (BMI: kg/m2) and blood pressure, taking into account reproductive histories and socioeconomic conditions. The proportion of obese (BMI > or = 25) subjects was high in the age groups 30-39, 40-49 and 50-59, accounting for, respectively, 39.4%, 51.1% and 48.5% of these populations. The proportion of women with hypertension (90/140 mmHg) was 23.7%, 18.5% and 26.2% in the 40-49, 50-59 and 60-69 age groups. Current contraceptive practices in the studied population included sterilization by operation, oral contraception and injection. These methods accounted for 43.0%, 12.8% and 8.2% of the population, respectively. Sociodemographic factors such as reproductive history, years of education and household income were not significantly related to BMI or to blood pressure (ANOVA with age adjustment). In contrast, oral contraceptive users had significantly higher BMIs and diastolic blood pressures (p<0.01, ANOVA with age adjustment). Multiple regression analysis also revealed that oral contraceptive use was a weak but significant contributing factor to both high BMI and blood pressure when sociodemographic factors were taken into account and controlled for statistically. It can thus be concluded that the use of contraceptive pills, which contain oestrogen and progestin and are provided free of charge to Thai women, tend to increase BMI and to elevate blood pressure.  相似文献

Many pregnant Muslim women fast during the Muslim holy month of Ramadan. A number of studies have reported negative life outcomes in adulthood for children who were prenatally exposed to Ramadan. However, other studies document minimal to no impact on neonatal indicators. Using data from the Indonesian Family Life Survey consisting of 45,246 observations of 21,723 children born to 9771 mothers, we contribute to the current discussion on prenatal exposure to Ramadan by examining the effects on stature (height-for-age Z-scores, weight-for-age Z-scores, and body-mass-index-for-age Z-scores: HAZ, WAZ, and BAZ, respectively) from early childhood to late adolescence (0–19 years of age). We introduce an objective mother’s religiosity indicator to improve the intention-to-treat estimations. Children were classified into three groups based on their mother’s religion-religiosity: religious Muslims, less-religious Muslims, and non-Muslims. Using cluster-robust mother fixed-effects, we found negative effects on stature for children born to religious Muslim mothers. The effects were age-dependent and timing-sensitive. For example, children born to religious Muslim mothers were shorter in late adolescence (15–19 years of age) compared to their unexposed siblings if they were prenatally exposed in the first trimester of pregnancy (HAZ difference = −0.105 SD; p-val. <0.05). Interestingly, we found positive effects on stature for exposed less-religious Muslim children that peak in early adolescence (10–14 years of age) and negative effects on stature for exposed non-Muslim children that occur only in early childhood (0–4 years of age). We nuance our discussion of health and socioeconomic factors to explain these surprising results.  相似文献

A multicenter survey of 400 married Filipino women 15-40 years of age conducted in 1986 by Family Health International and the International Health Foundation provided valuable information on contraceptive use in the Philippines, as well as factors influencing such practices. The respondents, who were drawn from 10 urban communities in Metro Manila and 10 rural communities in Nueva Ecija Province, averaged 31.5 years of age and had a mean number of 2.9 children. 32% of respondents indicated they desired another child. 225 women (56%) reported they were using no method of contraception. 59 (14%) were protected from pregnancy by tubal sterilization or vasectomy. 77 women (19%) were oral contraceptive (OC) users, and 8 (2%) were IUD users. Only 1 respondent was using a long-acting contraceptive. 3 women used spermicides, 21 (5%) used condoms, 25 (6%) practiced rhythm, and 30 (3%) of the women's husbands practiced withdrawal. Overall, 27% of contraceptive users in this sample used more than 1 method. 26% had been using a contraceptive method for 1-3 years and another 46% for more than 3 years, indicating consistent acceptance of family planning. A sequential trend of nonuse, OC use, and finally sterilization was observed. OC use was highest among women in their late 20s with 2-3 children. Urban-rural residence and socioeconomic factors had little impact on OC use. An encouraging finding was that only 25% of respondents believed that the pill poses important health risks and 61% were aware that pregnancy and childbirth involve greater health risks than OC use. This finding is in opposition to the extremely high levels of misinformation about the dangers of OC use identified in other surveys in developing countries and probably reflects the fact that 77% of women in the present study reported receiving advice on contraception from family planning professionals.  相似文献

Various national surveys suggest that the % of eligible couples in Bangladesh who are using traditional methods of contraception has been growing. This article presents detailed information on knowledge and use patterns of traditional methods and compares the use patterns of traditional and modern methods of contraception. The data are derived from the 1981 Contraceptive Prevalence Survey and information collected from a nationally representative sample of ever married women aged under 50, by way of field interviews using female interviewers. Quality of data was checked at different phases of the survey. Over 96% of the women reported that they knew at least 1 traditional method of contraception. The safe period was the most well known method (36.5%); about 30% reported knowledge of abstinence and 22% knew about withdrawal. Compared with modern methods, where knowledge declined with increased age, the knowledge of traditional methods shows no systematic pattern by age. Respondents with primary and higher education have higher knowledge of traditional methods than women who have never been to school. A similar pattern exists for employment status; non-Muslim women have consistently higher levels of knowledge than Muslim women. Data on patterns of use suggest that about 36% have used at least 1 contraceptive method, 15% having used traditional methods and 20.4% modern methods. The number of women who have used traditional methods increases from ages 20 to 44 and then declines. Use of traditional methods is relatively higher by the number of living children than by the number of children ever born. A similar pattern of association between background characteristics (education, employment and religion) and contraceptive knowledge is evident regarding level of use. Husband's level of education does not show any significant relationship with the use of traditional methods. The use rate of traditional methods was more than doubled among the women owning land compared to those having no land. The total current use rate of modern methods was higher among women aged 40-44 having relatively higher numbers of living children. As with current use, a positive relationship was observed between the socioeconomic variables and ever use. In general, it is concluded that socioeconomic variables played a dominant role in the decision of couples in choosing various methods of contraception. It is suggested that traditional methods still have an important role in family planning and that this should not be disregarded.  相似文献

This paper utilizes data from the 1977-78 Kenya Fertility Survey, 1 component of the World Fertility Survey, to analyze the determinants of breastfeeding durations for women 15 to 50 years old who had their last-but-1 live birth between 3 and 15 years prior to the interview. Comparisons are made with the findings fro m the World Fertility Surveys in 8 other developing countries in Asia and Latin America. Findings indicate that literacy, urban residence, secondary school education, and modern employment reduce the duration of breastfeeding in Kenya. In addition, the subgroups of women who appear to be curtailing breastfeeding are growing in proportional size or are composed of women may be innovators or leaders. A continuation of this pattern into the future may increase levels of infant morbidity and mortality and, in the absence of increased modern contraceptive practice, may increase the societal level of fertility. The death of the infant curtails the period of breastfeeding. Although there is a pronounced preference for male children in Kenya, this preference does not lead to differential durations of breastfeeding by sex of child. About 10% of Kenyan women used contraception in the last closed interval. Parity and age explain less than 1% of the variation in duration of breastfeeding in Kenya. Kenyan women are among the least likely to have attended secondary school, to have worked since marriage, and to have used modern contraception. The most traditional groups of Kenyan women, those who are Muslim or who are in polygamous unions, breastfeed for the longest durations. The Kenya Fertility Survey suggests that the subgroup of women with some secondary school education is growing considerably. Kikuyu women may be regarded as innovators in many respects. In addition to having shorter breastfeeding durations, they are the least likely to be in polygamous unions or to want more children, and they are the most likely to be using contraception.  相似文献

In Nigeria as in other African countries, population growth negatively affects economic development, and high parity affects maternal health. Breastfeeding, a common practice traditionally, is declining in some situations. This study was carried out in Ilorin, Nigeria. A sample population of 932 households stratified to represent different socioeconomic groups was used. 913 currently married women aged 15-35, who were in their prime childbearing ages, were interviewed on their contraceptive knowledge and on their attitudes towards modern contraception. In a bivariate statistical analysis, of 8 variables examined (i.e. ownership of a television, radio, religion, and other) only the woman's education, age, and area of residence within the city have significant independent effects on contraceptive knowledge. A linear logistic regression technic was also applied. 90% of the women interviewed thought that women should be free to practise family planning. Also, 95% of all the women believed that too frequent births could endanger the health of the mother and her children. Only the women with previous contraceptive knowledge overwhelmingly (80%) thought that the best way to prevent too frequent births is by family planning. 66.5% of those without previous contraceptive knowledge before this study suggested that traditional abstinence should be used and only 28.9% suggested family planning. Adequate awareness of the availability and usefulness of family planning methods can influence attitudes of women towards contraception and may also enhance contraceptive use. Better use can be made of broadcasting media, and efforts should be made to target younger, more fecund women, since there was evidence that more knowledge of family planning existed among women 30+ years old.  相似文献

PTC (phenylthiocarbamide) taste sensitivity was correlated with a measure of visual-motor maturation in children six to 15 years of age in two Ecuadorian Andean communities in which goiter (enlargement of the thyroid gland) and cretinism are endemic. The children in one population received injections of iodine in oil IM in 1966 while the children in the other population did not. The correlations between PTC taste sensitivity and visual-motor maturation were significant in both sex groups in the non-iodized population, but not in the iodized population. There was also a significant increase in taste sensitivity with age in the non-iodized, but not the iodized, population. The data support the hypothesis that the sensitive “tasters” of PTC limit their ingestion of naturally occurring goitrogens and are at an adaptive advantage with respect to the less sensitive “tasters” and “non-tasters” in this area in which iodine intake is low and the consumption of food crops containing naturally occurring goitrogens is high.  相似文献

Objective : Although obesity is typically associated with increased cardiovascular risk, a subset of obese individuals display a normal metabolic profile (“metabolically healthy obese,” MHO) and conversely, a subset of nonobese subjects present with obesity‐associated cardiometabolic abnormalities (“metabolically obese nonobese,” MONO). The aim of this cross‐sectional study was to identify the most important body composition determinants of metabolic phenotypes of obesity in nonobese and obese healthy postmenopausal women. Design and Methods : We studied a total of 150 postmenopausal women (age 54 ± 7 years, mean ± 1 SD). Based on a cardiometabolic risk score, nonobese (body mass index [BMI] ≤ 27) and obese women (BMI > 27) were classified into “metabolically healthy” and “unhealthy” phenotypes. Total and regional body composition was assessed with dual‐energy X‐ray absorptiometry (DXA). Results : In both obese and nonobese groups, the “unhealthy” phenotypes were characterized by frequent bodyweight fluctuations, higher biochemical markers of insulin resistance, hepatic steatosis and inflammation, and higher anthropometric and DXA‐derived indices of central adiposity, compared with “healthy” phenotypes. Indices of total adiposity, peripheral fat distribution and lean body mass were not significantly different between “healthy” and “unhealthy” phenotypes. Despite having increased fat mass, MHO women exhibited comparable cardiometabolic parameters with healthy nonobese, and better glucose and lipid levels than MONO. Two DXA‐derived indices, trunk‐to‐legs and abdominal‐to‐gluteofemoral fat ratio were the major independent determinants of the “unhealthy” phenotypes in our cohort. Conclusions : The “metabolically obese phenotype” is associated with bodyweight variability, multiple cardiometabolic abnormalities and an excess of central relative to peripheral fat in postmenopausal women. DXA‐derived centrality ratios can discriminate effectively between metabolic subtypes of obesity in menopause.  相似文献

This article explores the practical and theoretical significance and long-term consequences of the failure to incorporate women’s interests in post-conflict negotiations by examining the case of Muslim women in India. Analyses of deeply divided societies must recognize that political competition and political violence do not affect all citizens equally. Also, the “larger picture” depicted by inter-community conflicts should not overshadow the effects of intra-community conflicts, which are no less important. Evident within each community conflict are the winners and the losers of the political accommodation process, in which the marginalized and weaker sections of each “side” of the conflict may be the real “losers”. Gendered analysis of ethnic conflicts and ethnic conflict resolution demands a reorientation of the concepts of conflict and security – Whose conflict is being solved and who is being secured?  相似文献

Urban, white, ever-married women, aged 15-44, were sampled in low and high income census tracts in 16 U.S. cities, using a short adaptation of the Peabody Picture Vocabulary Test as a measure of IQ. After analysis of covariance, in which IQ was the independent variable and children ever born (CEB) was the dependent variable, it was found that there is .19 difference between the mean CEB of high- and low-IQ groups when covariables are controlled. It was shown that there was no significant difference in desired family size associated with IQ. But since the low-IQ women have more fertility than high-IQ women, effectiveness of contraceptive methods was examined. Examination showed that lower fertility was achieved by more effective use by the high-IQ women of methods of theoretically similar effectiveness. Also, those women not using a physician administered contraceptive (PAC) in the 3-year study period had unwanted births at about 3 times the rate of those who used PAC for the entire period. Thus, all American women will have to learn to realize their own fertility goals.  相似文献

We aimed to show the effect of osteoporosis on sleep quality in 59 postmenopausal women. The participants’ bone-mineral density levels were measured by dual-energy X-ray absorptiometry (DEXA). According to their DEXA results, participants were divided into two groups as osteoporotics and controls. The Pittsburgh Sleep Quality Index (PSQI) was used to evaluate sleep quality. Fourteen osteoporotic women (43.8%) and four controls (14.8%) were “poor” sleepers (p < 0.05). Postmeno-pausal women with osteoporosis scored greater on the “sleep latency” and “sleep duration” components of PSQI than controls. According to the findings of our study, osteoporosis is a risk factor for poor sleep quality in postmenopausal women.

Loss of libido associated with the use of oral contraceptives has been studied in 113 women of reproductive age who had taken a combined pill for periods ranging from 1 to 3 years. The patients were divided in four groups. In the first group (I) of 24 women oral contraceptive treatment was discontinued and all women received in intra-uterine contraceptive device (IUCD). The second group (II) of 36 patients, the brand of oral contraceptive was changed. Women in group (III) had their oral contraceptive maintained receiving in addition a mixture of an androgen and an estrogen (methyltestosterone 4 mg and ethynilestradiol 0.002 mg) daily. To women of group (IV) the oral contraceptive was discontinued but the androgen-estrogen mixture was given. All women in this group received an IUCD. Evaluation of the psyco-sexual parameters included changes in libido, time to reach an orgasm, duration and intensity or orgasms. Frequency of intercourse and response to autostimulation was also investigated. Libido was restored in 94.2% of patients in group II, in 97.3% of group III and in 92.8% of group IV. In group I only 55.6% of patients had libido fully restored. In view of the prompt restoration of libido in all groups except in patients discontinuing oral contraceptive therapy, it is suggested that loss of libido in oral contraceptive users has an important physological component which can be overcome probably by psychotherapy. Short term treatment with a mixture of methyltestosterone and ethynilestradiol seems to be highly effective in restoring libido in all patients.  相似文献

Introduction

Accessing family planning can reduce a significant proportion of maternal, infant, and childhood deaths. In Ethiopia, use of modern contraceptive methods is low but it is increasing. This study aimed to analyze the trends and determinants of changes in modern contraceptive use over time among young married women in Ethiopia.

Methods

The study used data from the three Demographic Health Surveys conducted in Ethiopia, in 2000, 2005, and 2011. Young married women age 15–24 years with sample sizes of 2,157 in 2000, 1,904 in 2005, and 2,146 in 2011 were included. Logit-based decomposition analysis technique was used for analysis of factors contributing to the recent changes. STATA 12 was employed for data management and analyses. All calculations presented in this paper were weighted for the sampling probabilities and non-response. Complex sampling procedures were also considered during testing of statistical significance.

Results

Among young married women, modern contraceptive prevalence increased from 6% in 2000 to 16% in 2005 and to 36% in 2011. The decomposition analysis indicated that 34% of the overall change in modern contraceptive use was due to difference in women’s characteristics. Changes in the composition of young women’s characteristics according to age, educational status, religion, couple concordance on family size, and fertility preference were the major sources of this increase. Two-thirds of the increase in modern contraceptive use was due to difference in coefficients. Most importantly, the increase was due to change in contraceptive use behavior among the rural population (33%) and among Orthodox Christians (16%) and Protestants (4%).

Conclusions

Modern contraceptive use among young married women has showed a remarkable increase over the last decade in Ethiopia. Programmatic interventions targeting poor, younger (adolescent), illiterate, and Muslim women would help to maintain the increasing trend in modern contraceptive use.  相似文献

Blurton Jones and Sibly (1978) developed a model of costs (weight of food and baby carried while foraging) of !Kung women's reproduction under ecological-economic constraints that were described by Lee (1972). Predictions are drawn from this model and tested on Howell's (1979) data from reproductive histories of 172 individual women.Women were rated on a scale of dependence on bush or cattlepost foods. The ratings were compared with what is known about the places at which the women gave birth to their children. Agreement was good and the population was divided into two groups for study: 65 women most dependent on bush foods and 70 women substantially dependent on agricultural produce. Thirty-seven women of uncertain or intermediate status were omitted.As predicted by the model, among the women who were dependent on bush foods: (1) first interbirth intervals (IBI) were shorter than were later IBI; (2) the survivorship of children in first IBIs was not strongly related to length of IBI, with shorter IBI giving as good survivorship as longer IBI; (3) IBIs lengthen as the number of surviving children increases, until the fourth child; (4) after the fourth child IBIs do not differ significantly although they tend to be shorter (contrary to the prediction, a null hypothesis whose statistical support is consequently poor); (5) for IBIs after the first IBI, mortality increased markedly as IBI decreased; (6) mortality was even more closely related to backloads entailed by each IBI, as calculated by Blurton Jones and Sibly (1978); (7) mortality was less closely related to backloads calculated from an alternative version of the model from which weight of food was excluded; (8) as reported by Howell (1979), a new pregnancy followed rapidly after the death of the preceeding child but (9) as predicted, a new pregnancy did not follow so fast after the death of older children; (10) for the cattlepost women IBIs were shorter than for women dependent mainly on bush foods; and (11) there was no significant relationship between IBI and mortality for the cattlepost women, and mortality at short IBI was lower than in bush women.The assumption that the benefit accruing from more births will be balanced against the costs (costs to survivorship assumed to result from the work entailed by caring for each child) was successful in giving many predictions confirmed by the data. The significance and limitations of this “optimization” study are discussed.  相似文献

Data from the 1982 Sri Lanka Contraceptive Prevalence Survey are used to identify women who wish to stop childbearing; they differ in socioeconomic status from their counterparts who want more children. Educated women are more likely to be motivated to cease childbearing than non-educated women; Christian or Sinhalese/Buddhist women are more willing to stop childbearing than Moor/Muslim or Tamil/Hindu women. The relationships between sex composition of existing children and women's fertility desires indicate that although moderate son preference exists it does not affect their contraceptive behaviour. Among those who want no more children, 15% are at risk of unwanted pregnancy because they do not practise contraception. Again better education and being Christian or Sinhalese/Buddhist reduced the risk of unwanted pregnancy. Women whose husbands disapproved of contraception had over four times higher risk of unwanted pregnancy than women whose husbands approved.  相似文献

The aim of the present study was to investigate the reading mechanisms in adults (27 subjects; mean age, 19.5 ± 0.8 [SD] years) with different levels of written text comprehension using fMRI. The main objective was to analyze the basic brain mechanisms of verbal stimuli perception with and without semantic component during reading discrimination tasks. The BOLD signal changes during WORD and PSEUDOWORD reading comparing to GAZE FIXATION state were estimated using both analysis of whole brain activation and ROIs (structures connected with the brain system providing reading) in two groups of subjects, “good” and “poor” readers. It was revealed that activations were higher in “poor” readers in lingual gyrus, SMG, STG compared to “good” readers during PSEUDOWORD reading. It was supposed, that the strategies of words and pseudowords recognition differed in two groups of readers: “good” readers identified words or pseudowords already at the stage of visual analysis of “word” structure and demonstrated attempts to decode pseudowords (i.e., language lexical zones were not activated); “poor” readers, apparently, tried to read pseudowords using the same strategy as for the words reading referring to the lexicon, and after failure identified pseudowords as meaningless concepts. In that case, activations of both lexical “language” zones and visual word form area (VWFA) were observed.  相似文献

Based on our interview survey of 574 randomly selected married women from a rural population in the South Ghor district, Jordan, where traditional Arab customs have been persistently maintained, in this paper we analyze the age-specific marital fertility rates (ASMFRs) and contraceptive practices, especially the prevalence and duration of contraceptive use. The ASMFRs fitted the natural fertility pattern proposed by Coale and Trussell's model, and the total fertility rate was estimated to be 7.2. Even though the prevalence rate of modern contraceptive methods has reached 14.3%, because of the recent increase among young women in particular, the users had larger numbers of children than the nonusers and the duration of contraceptive use was short, especially at young ages (e.g., about 90% 24-month discontinuation probabilities in 15-19- and 20-24-year-olds). These ineffective contraceptive uses were related to traditional Arab norms, represented by the pooled proportion of "as many as possible" and "up to God" answers to the ideal number of children (70% of men and 30% of women).  相似文献

Despite poor sensitivity and specificity, office blood pressure (BP) determinations are still the “gold standard” for diagnosing gestational hypertension. This prospective blind study evaluates the prognostic value of office values as compared with ambulatory monitoring in pregnancy. We analyzed 2175 BP series systematically sampled from 355 non-preeclamptic pregnant women for 48 h every 4 wks from the first hospital visit until delivery. Women were divided for comparative purposes into three groups: “detected” gestational hypertension, defined on the basis of casual clinical BP>140/90 mm Hg after 20 wks of gestation and hyperbaric index (area of BP excess above the upper limit of a time-specified tolerance interval adjusted for the circadian pattern of the reference population) consistently above the threshold for diagnosing hypertension in pregnancy; “undetected” gestational hypertension, women with office BP<140/90 mm Hg but hyperbaric index consistently above the threshold for diagnosis; and normotension, women with both office values and hyperbaric index below the respective thresholds for diagnosis. Small and insignificant differences in the 24 h mean BP between “detected” and “undetected” gestational hypertension is observed in all trimesters, in contrast with highly significant differences between these two groups and normotensive pregnancies. Normotensive women are characterized by highly significant lesser incidence by 60% in preterm delivery, 70% in intrauterine growth retardation, and 50% in delivery by cesarean section (P<0.001) compared with women with “detected” and “undetected” gestational hypertension (P>0.715). In pregnancy, the hyperbaric index is markedly superior to office BP measurements for diagnosis of what should be truly considered gestational hypertension, and for prediction of the outcome of pregnancy.  相似文献
