Medical grand rounds: alive and well and living in Canada.

Grand rounds have long been the principal educational activity of departments of medicine at teaching hospitals. Several recent articles have suggested that there has been a general deterioration in the quality of grand rounds. To evaluate their status in Canada we mailed a questionnaire to the chairmen of the departments of medicine at the 53 Canadian teaching hospitals; of the 48 responses received (91%), 38 were from chairmen, 5 were from senior department members, and 5 were from chief residents. The results indicated that grand rounds continued to be the principal teaching exercise of the departments. Of the respondents 98% felt that the quality had improved or stayed the same. The overall attendance was considered to have improved or remained unchanged for the past decade by 75%; 25% thought that it had declined. The diminished emphasis on patient-related topics concerned 10%. The respondents gave numerous suggestions, which should help organizers to improve the impact of grand rounds on learning.     

Pediatric grand rounds: a tribute to Norman J. Siegel, M.D

At ceremonies held in Baltimore Maryland on May, 4, 2002, Dr. Howard A Pearson, Professor of Pediatrics, was awarded the John A. Howland award of the American Pediatric Society, probably the most prestigious award of American Pediatrics. Dr. Pearson had been nominated for the award by Dr. Norman Siegel. The Department of Pediatrics Grand Rounds on Wednesday noon, October 26, 2002 was originally scheduled as a repetition of the presentation by Dr. Siegel and the acceptance by Dr. Pearson for those who could not be in Baltimore. However; in a number of meetings, unknown to Dr. Siegel, it was unanimously decided that it would be very appropriate to instead honor him as he stepped down from his position as Vice- and Interim Chairman of Pediatrics, and to formally thank him for his long and faithful service to the Department of Pediatrics, the Yale University School of Medicine, and the Yale New Haven Hospital.     

Paradoxes in exogenous androgen treatments of bluegill

Three experiments were conducted to investigate the effect of oral administration of trenbolone acetate and 17a-methyltestosterone on sex determination in 28-day-old bluegill, Lepomis macrochirus Rafinesque, fry. Multiple androgen doses and various treatment durations were tested. All treatments produced fewer males and females than did the control group (P < 0.01). All concentrations of both androgens produced a high proportion of intersex fish (38–81%). The number of males and females declined with the increase in androgen dose or treatment duration. Sterile fish were found in treatments with a higher dose rate or a longer treatment period. The predominance of intersex fish and reductions of both males and females in the androgen treatments suggest that gonadal development of both genotypic male and female fish were being altered by the hormone treatment.     

Bedside haematology: new facility in general practice.

A technique has been developed for performing rapid white cell counts and differential white cell counts at the bedside which uses a pocket McArthur microscope and a development of Field''s staining technique. A fixed volume thick film technique is used for the total white cell count that uses the minimum of equipment, all of which is disposable. In a study of samples from 88 patients using the thick film technique and standard laboratory techniques the thick film counts fell within 1.1 X 10(9)/1 (mean) of the standard laboratory count. This compares with a value of 1.0 X 10(9)/1 (mean) for within laboratory variability, using different electronic counters on the same specimen.     

In search of ritual: tradition, outer world and bad manners in the Amazon

Ritual is currently a central issue in Indian politics in the Amazon. Ritual implies tradition, and thus ethnic legitimacy. But to some groups, such as the Panoan-speaking Yaminawa, political and social looseness are handicaps to the invention of shared performances that are recognizable as rituals. Leadership is based both on an outward policy and an inward search for patterned, collective actions that can designate a gathering of kin and followers as an indigenous community. This article focuses on such ritual-making efforts, describing two otherwise informal parties held on a Brazilian Yaminawa reservation where fieldwork was carried out in 1993.     

Bedside Diagnosis of Urinary Infection: A Pilot Study

Early recognition of catheter-associated urinary infection is essential to proper management. Daily bacteriological monitoring of indwelling catheters by routine urine culture is expensive and time consuming. A pilot study using liquid media at room temperature incubation showed a sensitivity approximating 90 percent in detecting bacteriuria. A simplified screening test can be carried out daily at the bedside; a positive screen would be followed by standard bacteriological studies. Such a method permits earlier diagnosis, and results in great saving in cost as well.     

Amino acid composition of protein termini are biased in different manners

An exhaustive statistical analysis of the amino acid sequences at the carboxyl (C) and amino (N) termini of proteins and of coding nucleic acid sequences at the 5' side of the stop codons was undertaken. At the N ends, Met and Ala residues are over-represented at the first (+1) position whereas at positions 2 and 5 Thr is preferred. These peculiarities at N-termini are most probably related to the mechanism of initiation of translation (for Met) and to the mechanisms governing the life-span of proteins via regulation of their degradation (for Ala and Thr). We assume that the C-terminal bias facilitates fixation of the C ends on the protein globule by a preference for charged and Cys residues. The terminal biases, a novel feature of protein structure, have to be taken into account when molecular evolution, three-dimensional structure, initiation and termination of translation, protein folding and life-span are concerned. In addition, the bias of protein termini composition is an important feature which should be considered in protein engineering experiments.

The grand game of metazoan phylogeny: rules and strategies

Many cladistic analyses of animal phylogeny have been published by authors arguing that their results are well supported. Comparison of these analyses indicates that there can be as yet no general consensus about the evolution of the animal phyla. We show that the various cladistic studies published to date differ significantly in methods of character selection, character coding, scoring and weighting, ground-pattern reconstructions, and taxa selection. These methodological differences are seldom made explicit, which hinders comparison of different studies and makes it impossible to assess a particular phylogeny outside its own scope. The effects of these methodological differences must be considered before we can hope to reach a morphological reference framework needed for effective comparison and combination with the evidence obtained from molecular and developmental genetic studies.
