The unpredictability of the occurrence of epileptic seizures makes it difficult to detect and treat this condition effectively. An automatic system that characterizes epileptic activities in EEG signals would allow patients or the people near them to take appropriate precautions, would allow clinicians to better manage the condition, and could provide more insight into these phenomena thereby revealing important clinical information. Various methods have been proposed to detect epileptic activity in EEG recordings. Because of the nonlinear and dynamic nature of EEG signals, the use of nonlinear Higher Order Spectra (HOS) features is a seemingly promising approach. This paper presents the methodology employed to extract HOS features (specifically, cumulants) from normal, interictal, and epileptic EEG segments and to use significant features in classifiers for the detection of these three classes. In this work, 300 sets of EEG data belonging to the three classes were used for feature extraction and classifier development and evaluation. The results show that the HOS based measures have unique ranges for the different classes with high confidence level (p-value < 0.0001). On evaluating several classifiers with the significant features, it was observed that the Support Vector Machine (SVM) presented a high detection accuracy of 98.5% thereby establishing the possibility of effective EEG segment classification using the proposed technique.

ObjectivesThe electroencephalogram (EEG) signal contains information about the state and condition of the brain. The aim of the study is to conduct a nonlinear analysis of the EEG signals and to compare the differences in the nonlinear characteristics of the EEG during normal state and the epileptic state.DataThe EEG data used for this study – which consisted of epileptic EEG and normal EEG – were obtained from the EEG database available with the Bonn University, Germany.ResultsThe attractors seen in normal and epileptic human brain dynamics were studied and compared. Surrogate data analyses were conducted on two nonlinear measures, namely the largest Lyapunov exponent and the correlation dimension, to test the hypothesis whether EEG signals were in accordance with linear stochastic models.DiscussionsThe existence of deterministic chaos in brain activity is confirmed by the existence of a chaotic attractor; also, saturation of the correlation dimension towards a definite value is the manifestation of a deterministic dynamics. Also a reduction is observed between the dimensionalities of the brain attractors from normal state to the epileptic state. The evaluation of the largest Lyapunov exponent also confirms the lowering of complexity during an episode of seizure.ConclusionIn case of Lyapunov exponent of EEG data, the change due to surrogating is small suggesting that it is not representing the system complexity properly but there is a marked change in the case of correlation dimension value due to surrogating.

The objective of this work is to identify similarities in the spatio-temporal dynamics of epileptic seizures, record with scalp EEG. A comprehensive method is proposed and applied in EEG of the patients who suffer from temporal lobe epilepsy. The method is based on the computation of the time-varying degree of non linear correlation between scalp electrodes at seizure onset and during seizure spread, determined by a nonlinear regression analysis. The quantification and coding of these similarity relations allow the comparison between two epileptic networks. Results show that reproducible patterns may be extracted from different seizures of the same patient and confirm the existence of different subtypes of temporal lobe epilepsy.

Simultaneous EEG-fMRI has proven to be useful in localizing interictal epileptic activity. However, the applicability of traditional GLM-based analysis is limited as interictal spikes are often not seen on the EEG inside the scanner. Therefore, we aim at extracting epileptic activity purely from the fMRI time series using independent component analysis (ICA). To our knowledge, we show for the first time that ICA can find sources related to epileptic activity in patients where no interictal spikes were recorded in the EEG. The epileptic components were identified retrospectively based on the known localization of the ictal onset zone (IOZ). We demonstrate that the selected components truly correspond to epileptic activity, as sources extracted from patients resemble significantly better the IOZ than sources found in healthy controls. Furthermore, we show that the epileptic components in patients with and without spikes recorded inside the scanner resemble the IOZ in the same degree. We conclude that ICA of fMRI has the potential to extend the applicability of EEG-fMRI for presurgical evaluation in epilepsy.

We report on the nonlinear analysis of electroencephalogram (EEG) recordings in the rabbit visual cortex. Epileptic seizures were induced by local penicillin application and triggered by visual stimulation. The analysis procedures for nonlinear signals have been developed over the past few years and applied primarily to physical systems. This is an early application to biological systems and the first to EEG data. We find that during epileptic activity, both global and local embedding dimensions are reduced with respect to nonepileptic activity. Interestingly, these values are very low () and do not change between preictal and tonic stages of epileptic activity, also the Lyapunov dimension remains constant. However, between these two stages the manifestations of the local dynamics change quite drastically, as can be seen, e.g., from the shape of the attractors. Furthermore, the largest Lyapunov exponent is reduced by a factor of about two in the second stage and characterizes the difference in dynamics. Thus, the occurrence of clinical symptoms associated with the tonic seizure activity seems to be mainly related to the local dynamics of the nonlinear system. These results thus seem to give a strong indication that the dynamics remains much the same in these stages of behavior, and changes are due to alterations in model parameters and consequent bifurcations of the observed orbits. Cortical epileptic focus was produced by an intracortical injection of FeCl3 in rat cerebral cortex using standard techniques. How after its onset in the cortical focus, the epileptiform activity evolved with time in the thalamus and substantia nigra has been determined. To study the propagation of the epileptiform activity, the local EEG and multiple unit action potentials were recorded from these structures simultaneously with the cortical epileptiform EEG. The results showed that in thalamus and substantia nigra epileptiform activity appeared simultaneously with that in the cortical focus. Intensity of epileptic activity in thalamus and substantia nigra on the whole increased in parallel with that in the cortical focus. The results suggest that the thalamic and nigral epileptiform activity may reinforce the cortical epileptiform activity.

Clinical electroencephalographic (EEG) recordings of the transition into generalised epileptic seizures show a sudden onset of spike-wave dynamics from a low-amplitude irregular background. In addition, non-trivial and variable spatio-temporal dynamics are widely reported in combined EEG/fMRI studies on the scale of the whole cortex. It is unknown whether these characteristics can be accounted for in a large-scale mathematical model with fixed heterogeneous long-range connectivities. Here, we develop a modelling framework with which to investigate such EEG features. We show that a neural field model composed of a few coupled compartments can serve as a low-dimensional prototype for the transition between irregular background dynamics and spike-wave activity. This prototype then serves as a node in a large-scale network with long-range connectivities derived from human diffusion-tensor imaging data. We examine multivariate properties in 42 clinical EEG seizure recordings from 10 patients diagnosed with typical absence epilepsy and 50 simulated seizures from the large-scale model using 10 DTI connectivity sets from humans. The model can reproduce the clinical feature of stereotypy where seizures are more similar within a patient than between patients, essentially creating a patient-specific fingerprint. We propose the approach as a feasible technique for the investigation of patient-specific large-scale epileptic features in space and time.

Multiway analysis of epilepsy tensors
MOTIVATION: The success or failure of an epilepsy surgery depends greatly on the localization of epileptic focus (origin of a seizure). We address the problem of identification of a seizure origin through an analysis of ictal electroencephalogram (EEG), which is proven to be an effective standard in epileptic focus localization. SUMMARY: With a goal of developing an automated and robust way of visual analysis of large amounts of EEG data, we propose a novel approach based on multiway models to study epilepsy seizure structure. Our contributions are 3-fold. First, we construct an Epilepsy Tensor with three modes, i.e. time samples, scales and electrodes, through wavelet analysis of multi-channel ictal EEG. Second, we demonstrate that multiway analysis techniques, in particular parallel factor analysis (PARAFAC), provide promising results in modeling the complex structure of an epilepsy seizure, localizing a seizure origin and extracting artifacts. Third, we introduce an approach for removing artifacts using multilinear subspace analysis and discuss its merits and drawbacks. RESULTS: Ictal EEG analysis of 10 seizures from 7 patients are included in this study. Our results for 8 seizures match with clinical observations in terms of seizure origin and extracted artifacts. On the other hand, for 2 of the seizures, seizure localization is not achieved using an initial trial of PARAFAC modeling. In these cases, first, we apply an artifact removal method and subsequently apply the PARAFAC model on the epilepsy tensor from which potential artifacts have been removed. This method successfully identifies the seizure origin in both cases.

ObjectiveEpileptic seizures are defined as manifest of excessive and hyper-synchronous activity of neurons in the cerebral cortex that cause frequent malfunction of the human central nervous system. Therefore, finding precursors and predictors of epileptic seizure is of utmost clinical relevance to reduce the epileptic seizure induced nervous system malfunction consequences. Researchers for this purpose may even guide us to a deep understanding of the seizure generating mechanisms. The goal of this paper is to predict epileptic seizures in epileptic rats.MethodsSeizures were induced in rats using pentylenetetrazole (PTZ) model. EEG signals in interictal, preictal, ictal and postictal periods were then recorded and analyzed to predict epileptic seizures. Epileptic seizures were predicted by calculating an index in consecutive windows of EEG signal and comparing the index with a threshold. In this work, a newly proposed dissimilarity index called Bhattacharyya Based Dissimilarity Index (BBDI), dynamical similarity index and fuzzy similarity index were investigated.ResultsBBDI, dynamical similarity index and fuzzy similarity index were examined on case and control groups and compared to each other. The results show that BBDI outperforms dynamical and fuzzy similarity indices. In order to improve the results, EEG sub-bands were also analyzed. The best result achieved when the proposed dissimilarity index was applied on Delta sub-band that predicts epileptic seizures in all rats with a mean of 299.5 s.ConclusionThe dissimilarity of neural network activity between reference window and present window of EEG signal has a significant increase prior to an epileptic seizure and the proposed dissimilarity index (BBDI) can reveal this variation to predict epileptic seizures. In addition, analyzing EEG sub-bands results in more accurate information about constituent neuronal activities underlying the EEG since certain changes in EEG signal may be amplified when each sub-band is analyzed separately.SignificanceThis paper presents application of a dissimilarity index (BBDI) on EEG signals and its sub-bands to predict PTZ-induced epileptic seizures in rats. Based on the results of this work, BBDI will predict epileptic seizures more accurately and more reliably compared to current indices that increases epileptic patient comfort and improves patient outcomes.

We present an empirical model of the electroencephalogram (EEG) signal based on the construction of a stochastic limit cycle oscillator using Itô calculus. This formulation, where the noise influences actually interact with the dynamics, is substantially different from the usual definition of measurement noise. Analysis of model data is compared with actual EEG data using both traditional methods and modern techniques from nonlinear time series analysis. The model demonstrates visually displayed patterns and statistics that are similar to actual EEG data. In addition, the nonlinear mechanisms underlying the dynamics of the model do not manifest themselves in nonlinear time series analysis, paralleling the situation with real, non-pathological EEG data. This modeling exercise suggests that the EEG is optimally described by stochastic limit cycle behavior.

An accurate epileptic seizure prediction algorithm can alleviate the problem and reduce risks in the life of a patient suffering from epilepsy. The main motive of this work is to propose a model which can predict seizures well in advance of its occurrence. Multivariate statistical process control (MSPC) has been used for seizure predictions in long-term scalp EEG signal. It has been observed that excessive neuronal activity in the preictal period of seizure changes the electrical characteristic from chaotic to rhythmic behavior. These changes have been utilized for prediction. Eight temporal based features are used for predicting the seizures by using multivariate statistical process control, which is widely known as an anomaly monitoring method. 90 seizures from the CHB-MIT EEG data of ten patients are analyzed.ResultThe results of the proposed method demonstrated that 80 seizures out of 90 in preictal period were correctly predicted prior to the seizure onset, thereby giving a sensitivity of 88.89%. The false positive rate is observed to 0.39 per hour.ConclusionThis study proposed a temporal based patient-specific epileptic seizure prediction method using MSPC in long-term scalp EEG signals. It also provides the possibility of realizing an EEG-based epileptic seizure prediction system which requires less computational power.SignificanceThe proposed method does not require preictal data for modeling. The extracted features are computationally easy. The tested result shows good accuracy on the CHB-MIT data base.

This paper proposes a new method for feature extraction and recognition of epileptiform activity in EEG signals. The method improves feature extraction speed of epileptiform activity without reducing recognition rate. Firstly, Principal component analysis (PCA) is applied to the original EEG for dimension reduction and to the decorrelation of epileptic EEG and normal EEG. Then discrete wavelet transform (DWT) combined with approximate entropy (ApEn) is performed on epileptic EEG and normal EEG, respectively. At last, Neyman–Pearson criteria are applied to classify epileptic EEG and normal ones. The main procedure is that the principle component of EEG after PCA is decomposed into several sub-band signals using DWT, and ApEn algorithm is applied to the sub-band signals at different wavelet scales. Distinct difference is found between the ApEn values of epileptic and normal EEG. The method allows recognition of epileptiform activities and discriminates them from the normal EEG. The algorithm performs well at epileptiform activity recognition in the clinic EEG data and offers a flexible tool that is intended to be generalized to the simultaneous recognition of many waveforms in EEG.

Electrophysiological and hemodynamic data can be integrated to accurately and precisely identify the generators of abnormal electrical activity in drug-resistant focal epilepsy. Arterial Spin Labeling (ASL), a magnetic resonance imaging (MRI) technique for quantitative noninvasive measurement of cerebral blood flow (CBF), can provide a direct measure of variations in cerebral perfusion associated with the epileptic focus. In this study, we aimed to confirm the ASL diagnostic value in the identification of the epileptogenic zone, as compared to electrical source imaging (ESI) results, and to apply a template-based approach to depict statistically significant CBF alterations. Standard video-electroencephalography (EEG), high-density EEG, and ASL were performed to identify clinical seizure semiology and noninvasively localize the epileptic focus in 12 drug-resistant focal epilepsy patients. The same ASL protocol was applied to a control group of 17 healthy volunteers from which a normal perfusion template was constructed using a mixed-effect approach. CBF maps of each patient were then statistically compared to the reference template to identify perfusion alterations. Significant hypo- and hyperperfused areas were identified in all cases, showing good agreement between ASL and ESI results. Interictal hypoperfusion was observed at the site of the seizure in 10/12 patients and early postictal hyperperfusion in 2/12. The epileptic focus was correctly identified within the surgical resection margins in the 5 patients who underwent lobectomy, all of which had good postsurgical outcomes. The combined use of ESI and ASL can aid in the noninvasive evaluation of drug-resistant epileptic patients.

Previous work has demonstrated that some dynamic properties of intracranial EEG signals are indicative of epileptic seizures and hence could be used for prediction in order to realize counter measures. However, most previous studies only investigated predictability via offline analysis of EEG signals as compared to actually predicting seizures in a setting applicable to implantable devices. Here we address this problem, which calls for simple and fast online methods, and based on previous offline analyses we hypothesize that prediction can be further improved when using multiple features to detect the preictal patterns. We propose a simple adaptive online method (an evolving neuro-fuzzy model) to adaptively learn such combined features. The classifier starts out with a simple structure and patient-independent parameters and then grows into a personal seizure predictor as recursive methods tune the model structure and parameters. We apply the adaptive classifier to a publicly available database of intracranial recordings from 21 patients and demonstrate that seizure prediction is improved with our online method as compared to offline non-adaptive techniques. We show that our method is robust with respect to those few model parameters, which are not adapted. Moreover, as we report the performance on data from a publicly available seizure database, our results can serve as a yardstick for future method developments.

Epilepsy is characterized by paradoxical patterns of neural activity. They may cause different types of electroencephalogram (EEG), which dynamically change in shape and frequency content during the temporal evolution of seizure. It is generally assumed that these epileptic patterns may originate in a network of strongly interconnected neurons, when excitation dominates over inhibition. The aim of this work is to use a neural network composed of 50 x 50 integrate-and-fire neurons to analyse which parameter alterations, at the level of synapse topology, may induce network instability and epileptic-like discharges, and to study the corresponding spatio-temporal characteristics of electrical activity in the network. We assume that a small group of central neurons is stimulated by a depolarizing current (epileptic focus) and that neurons are connected via a Mexican-hat topology of synapses. A signal representative of cortical EEG (ECoG) is simulated by summing the membrane potential changes of all neurons. A sensitivity analysis on the parameters describing the synapse topology shows that an increase in the strength and in spatial extension of excitatory vs. inhibitory synapses may cause the occurrence of travelling waves, which propagate along the network. These propagating waves may cause EEG patterns with different shape and frequency, depending on the particular parameter set used during the simulations. The resulting model EEG signals include irregular rhythms with large amplitude and a wide frequency content, low-amplitude high-frequency rapid discharges, isolated or repeated bursts, and low-frequency quasi-sinusoidal patterns. A slow progressive temporal variation in a single parameter may cause the transition from one pattern to another, thus generating a highly non-stationary signal which resembles that observed during ECoG measurements. These results may help to elucidate the mechanisms at the basis of some epileptic discharges, and to relate rapid changes in EEG patterns with the underlying alterations at the network level.
