Background

The non-invasive diagnosis of cardiac sarcoidosis (CS) is difficult. Cardiovascular magnetic resonance (CMR) has become a very valuable diagnostic tool in patients with suspected CS, but usually a combination of different tests is used. Oxygen uptake efficiency slope (OUES) is a parameter of cardiopulmonary exercise testing (CPET), which is used as an indicator for cardiovascular impairment. We investigated the predictive value of OUES for the diagnosis of myocardial involvement in sarcoid patients.

Methods

Retrospectively 37 consecutive patients (44.9±13.8 years) with histologically confirmed sarcoidosis and clinical suspicion of heart involvement underwent noninvasive diagnostic testing including CMR. CS was diagnosed according to the guidelines from the Japanese Society of Sarcoidosis and other Granulomatous Disorders with additional consideration of CMR findings. Furthermore, CPET with calculation of predicted OUES according to equations by Hollenberg et al. was carried out.

Results

Patients with CS (11/37; 30%) had a worse cardiovascular response to exercise. OUES was significantly lower in CS-group compared to non-CS-group (59.3±19.1 vs 88.0±15.4%pred., p<0.0001). ROC curve method identified 70%pred. as the OUES cut-off point, which maximized sensitivity and specificity for detection of CS (96% sensitivity, 82% specificity, 89% overall accuracy). OUES <70%pred. was the single best predictor of CS (Odds ratio: 100.43, 95% CI: 1.99 to 5064, p<0.001) even in multivariate analyses.

Conclusion

OUES assessed in CPET may be helpful in identifying patient with cardiac involvement of sarcoidosis. Patient selection for CMR may be assisted by CPET findings in patients with sarcoidosis.  相似文献

Regulation of Cardiac Muscle Contractility   总被引:2,自引:0,他引:2  
The heart's physiological performance, unlike that of skeletal muscle, is regulated primarily by variations in the contractile force developed by the individual myocardial fibers. In an attempt to identify the basis for the characteristic properties of myocardial contraction, the individual cardiac contractile proteins and their behavior in contractile models in vitro have been examined. The low shortening velocity of heart muscle appears to reflect the weak ATPase activity of cardiac myosin, but this enzymatic activity probably does not determine active state intensity. Quantification of the effects of Ca++ upon cardiac actomyosin supports the view that myocardial contractility can be modified by changes in the amount of calcium released during excitation-contraction coupling. Exchange of intracellular K+ with Na+ derived from the extracellular space also could enhance myocardial contractility directly, as highly purified cardiac actomyosin is stimulated when K+ is replaced by an equimolar amount of Na+. On the other hand, cardiac glycosides and catecholamines, agents which greatly increase the contractility of the intact heart, were found to be without significant actions upon highly purified reconstituted cardiac actomyosin.  相似文献

Auxin-mediated elongation growth of maize coleoptile segments is inhibited by reducing the O2 concentration in the incubation medium to GT 100 μmol . 1?1. The half-maximal elongation rate is reached at 40 μmol . 1?1 O2, i.e. about two orders of magnitude higher than with mitochondrial respiration. O2 uptake of the segments measured under similar conditions with an O2 electrode shows a very similar dependence on O2 concentration. Auxin increases O2 uptake by 5–10% when it induces growth. About 40% of the O2 uptake is insensitive to inhibition by KCN. Auxin has no effect on O2 uptake in the presence of KCN. The possibility that auxin-mediated elongation growth depends on a KCN-sensitive oxidative process, other than cytochrome c oxidase-catalyzed respiration, is discussed.  相似文献

An in vitro O2 assay was used to measure early response of wheat (Triticum aestivum L.) etioplasts to light. A transient photoinducible O2 uptake occurred when dark-grown etioplasts were initially exposed to light. The rate of inducible O2 consumption was dependent on both the intensity of light and the quantity of organelle protein present. Higher light intensities resulted in greater O2 utilization per minute, and a greater quantity of organelle protein in the sample resulted in an increased rate of O2 uptake under the same light intensity conditions. Experiments with various plant tissues as well as with mitochondrial respiratory inhibitors indicated that etioplasts are the organelles responsible for the photoinduced O2 uptake phenomenon. A preliminary action spectrum study revealed that wavelengths 640 to 680 nm resulted in maximum O2 uptake. This indicated the presence of an etioplast red light receptor pigment which induces O2 uptake in etioplasts.  相似文献

Rice seedlings (Oryza saliva L.) were grown in the dark forup to 4 d in solutions containing various concentrations ofO2. Compared with seedlings grown at 0·250 mol O2 m–3,the dry weight of the growing seedling was 14% lower at 0·110mol O2 m–3 and 60% lower at 0 mol O2 m–3. Decreasesin fresh weight were similar but not identical to decreasesin dry weight, possibly because leaf growth was suppressed evenabove 0·110 mol O2 m–3. Oxygen deficiency inhibitedroot growth more severely than coleoptile growth. Coleoptiles from seedlings grown in aerated solution were exposedto an atmosphere of pure N2 for 30 min. Anoxia caused a declinein ATP content and energy charge, suggestive of decreased oxidativephosphorylation. It is not clear whether the decline in oxidativephosphorylation was solely responsible for impaired growth inhypoxia. In seedlings growing at O2 concentrations less than 0·110mol O2 m–3, significant amounts of ethanol were synthesized.The rate of O2 uptake decreased markedly below 0·06 molO2 m–3; this was presumably near the external O2 concentrationat which oxidative phosphorylation became limited by the supplyof O2. The stage of development of the seedlings appeared toinfluence O2 uptake, possibly through changes in conductanceof the tissue to O2. Uncouplers were used to confirm that thecritical O2 concentration was dependent on O2 diffusion ratherthan enzyme kinetics. Impaired growth above 0·110 molO2 m–3 may have been due to a decreased activity of oxygenasesof relatively low affinity for O2, which in turn altered cellmetabolism. Oxygen Free-Radical Reduction of Brain Capillary Rubidium Uptake   总被引:1,自引:1,他引:1  
Free radicals are proposed to play a role in the injury following cerebral ischemia in which cerebral edema is a prominent feature. To determine whether free radicals might alter the movement of ions and water across the blood-brain barrier, we examined their effect on brain capillary transport. Rat brain capillaries were isolated, incubated with a system that generates free radicals, and various capillary transport systems were studied. Rubidium uptake was reduced 74% whereas rubidium efflux, glucose transport, and capillary water space were unchanged. The results following the addition of radical scavengers indicated that hydrogen peroxide or a related free radical was the toxic species. These data suggest that free radicals can impair capillary endothelial cell mechanisms that help maintain homeostasis of electrolytes and water in brain.  相似文献

Objectives

We assessed the prospective association of resting heart rate (RHR) at baseline with peak oxygen uptake (VO2peak) 23 years later, and evaluated whether physical activity (PA) could modify this association.

Background

Both RHR and VO2peak are strong and independent predictors of cardiovascular morbidity and mortality. However, the association of RHR with VO2peak and modifying effect of PA have not been prospectively assessed in population studies.

Methods

In 807 men and 810 women free from cardiovascular disease both at baseline (1984–86) and follow-up 23 years later, RHR was recorded at both occasions, and VO2peak was measured by ergospirometry at follow-up. We used Generalized Linear Models to assess the association of baseline RHR with VO2peak, and to study combined effects of RHR and self-reported PA on later VO2peak.

Results

There was an inverse association of RHR at baseline with VO2peak (p<0.01). Men and women with baseline RHR greater than 80 bpm had 4.6 mL·kg−1·min−1 (95% confidence interval [CI], 2.8 to 6.3) and 1.4 mL·kg−1·min−1 (95% CI, −0.4 to 3.1) lower VO2peak at follow-up compared with men and women with RHR below 60 bpm at baseline. We found a linear association of change in RHR with VO2peak (p = 0.03), suggesting that a decrease in RHR over time is likely to be beneficial for cardiovascular fitness. Participants with low RHR and high PA at baseline had higher VO2peak than inactive people with relatively high RHR. However, among participants with relatively high RHR and high PA at baseline, VO2peak was similar to inactive people with relatively low RHR.

Conclusion

RHR is an important predictor of VO2peak, and serial assessments of RHR may provide useful and inexpensive information on cardiovascular fitness. The results suggest that high levels of PA may compensate for the lower VO2peak associated with a high RHR.  相似文献

Background

Exercise capacity is a strong predictor of survival in patients with coronary artery disease (CAD). Exercise capacity improves after cardiac rehabilitation exercise training, but previous studies have demonstrated a decline in peak oxygen uptake after ending a formal rehabilitation program. There is a lack of knowledge on how long-term exercise adherence can be achieved in CAD patients. We therefore assessed if a 12-month maintenance program following cardiac rehabilitation would lead to increased adherence to exercise and increased exercise capacity compared to usual care.

Materials and Methods

Two-centre, open, parallel randomized controlled trial with 12 months follow-up comparing usual care to a maintenance program. The maintenance program consisted of one monthly supervised high intensity interval training session, a written exercise program and exercise diary, and a maximum exercise test every third month during follow-up. Forty-nine patients (15 women) on optimal medical treatment were included following discharge from cardiac rehabilitation. The primary endpoint was change in peak oxygen uptake at follow-up; secondary endpoints were physical activity level, quality of life and blood markers of cardiovascular risk.

Results

There was no change in peak oxygen uptake from baseline to follow-up in either group (intervention group 27.9 (±4.7) to 28.8 (±5.6) mL·kg (-1) min (−1), control group 32.0 (±6.2) to 32.8 (±5.8) mL·kg (−1) min (−1), with no between-group difference, p = 0.22). Quality of life and blood biomarkers remained essentially unchanged, and both self-reported and measured physical activity levels were similar between groups after 12 months.

Conclusions

A maintenance exercise program for 12 months did not improve adherence to exercise or peak oxygen uptake in CAD patients after discharge from cardiac rehabilitation compared to usual care. This suggests that infrequent supervised high intensity interval training sessions are inadequate to improve peak oxygen uptake in this patient group.

The isolated rat diaphragm was used to study the effects of 17β-estradiol on basal and insulin-mediated glucose uptake. Rats were injected with estradiol for 2 wk in daily doses of 10 μg/100 g of body weight and were compared to untreated control animals. Estrogen treatment resulted in a 16% decrease in basal glucose uptake by diaphragm muscle as compared to controls. In contrast, in the presence of insulin, glucose uptake by muscle increased 103% above basal in estradiol-treated animals as compared to a 38% rise in the control group. The absolute rate of glucose uptake induced by insulin in the estradiol treated animals (5.8 mg/g/hr) was 22% higher than in controls. These findings were not accompanied by changes in weight gain, plasma glucose and plasma immunoreactive insulin concentrations in the treated animals. In vitro incubation of diaphragm muscle with estradiol did not have an effect on basal or insulin-mediated glucose uptake.  相似文献
