We present in this paper the results of a study of the interdependence between signal characteristic of the central nervous system (electroencephalography) and the autonomic nervous system (heart rate and respiration) in human neonates during sleep. By using methods from nonlinear dynamical systems theory, we show that there exist significant differences in this interdependence with the sleep stage and the electrodes considered. This paves the way for the application of this methodology in clinical practice to study pathologies where this interdependence is altered, such as the sudden infant death syndrome.  相似文献

Moreover, non-REM/REM differences revealed a dominant role of parasympathetic activity during sleep stages as well as sleep cycles. Background

We previously suggested that abnormal sleep behaviors, i.e., as found in parasomnias, may often be the expression of increased activity of the reward system during sleep. Because nightmares and sleepwalking predominate during REM and NREM sleep respectively, we tested here whether exploratory excitability, a waking personality trait reflecting high activity within the mesolimbic dopaminergic (ML-DA) system, may be associated with specific changes in REM and NREM sleep patterns in these two sleep disorders.

Methods

Twenty-four unmedicated patients with parasomnia (12 with chronic sleepwalking and 12 with idiopathic nightmares) and no psychiatric comorbidities were studied. Each patient spent one night of sleep monitored by polysomnography. The Temperament and Character Inventory (TCI) was administered to all patients and healthy controls from the Geneva population (n = 293).

Results

Sleepwalkers were more anxious than patients with idiopathic nightmares (Spielberger Trait anxiety/STAI-T), but the patient groups did not differ on any personality dimension as estimated by the TCI. Compared to controls, parasomnia patients (sleepwalkers together with patients with idiopathic nightmares) scored higher on the Novelty Seeking (NS) TCI scale and in particular on the exploratory excitability/curiosity (NS1) subscale, and lower on the Self-directedness (SD) TCI scale, suggesting a general increase in reward sensitivity and impulsivity. Furthermore, parasomnia patients tended to worry about social separation persistently, as indicated by greater anticipatory worry (HA1) and dependence on social attachment (RD3). Moreover, exploratory excitability (NS1) correlated positively with the severity of parasomnia (i.e., the frequency of self-reported occurrences of nightmares and sleepwalking), and with time spent in REM sleep in patients with nightmares.

Conclusions

These results suggest that patients with parasomnia might share common waking personality traits associated to reward-related brain functions. They also provide further support to the notion that reward-seeking networks are active during human sleep.  相似文献

ARONNE, LOUIS J, RONALD MACKINTOSH, MICHAEL ROSENBAUM, RUDOLPH L LEIBEL, JULES HIRSCH. Cardiac autonomic nervous system activity in obese and never-obese young men. Autonomic nervous system (ANS) activity in age-matched, weight-stable, free-living, ad libitumfed, obese (OB) and never-obese (NO) young men (body mass index means [SD], 38.5 [3.9] and 22.0 [1.7], respectively) was evaluated by sequential blockade of cardiac autonomic innervation with weight-adjusted doses of parasympathetic (atropine) and sympathetic (esmolol) blockers so as to produce maximal effects on heart rate. Change in heart period (interbeat interval) from baseline, induced by atropine, defined parasympathetic control (PC), and the subsequent change, after esmolol administration, defined sympathetic control (SC). The heart period, after PC and SC blockade, defined intrinsic heart period (I). In the OB group, baseline heart period and PC were lower, and SC and I were higher, than in the NO group. The results in the OB, relative to the NO subjects, are similar to those reported in a previous study of NO subjects who had undergone a 10% weight gain by overfeeding. These findings suggest that the ANS of individuals with obesity is chronically altered in a way that would tend to oppose their excessive adiposity, and that these autonomic changes are more likely to be responses to other forces that induce obesity, rather than being primary agents in the production of the disease.  相似文献

We aimed at better understanding the brain mechanisms involved in the processing of alerting meaningful sounds during sleep, investigating alpha activity. During EEG acquisition, subjects were presented with a passive auditory oddball paradigm including rare complex sounds called Novels (the own first name - OWN, and an unfamiliar first name - OTHER) while they were watching a silent movie in the evening or sleeping at night. During the experimental night, the subjects’ quality of sleep was generally preserved. During wakefulness, the decrease in alpha power (8–12 Hz) induced by Novels was significantly larger for OWN than for OTHER at parietal electrodes, between 600 and 900 ms after stimulus onset. Conversely, during REM sleep, Novels induced an increase in alpha power (from 0 to 1200 ms at all electrodes), significantly larger for OWN than for OTHER at several parietal electrodes between 700 and 1200 ms after stimulus onset. These results show that complex sounds have a different effect on the alpha power during wakefulness (decrease) and during REM sleep (increase) and that OWN induce a specific effect in these two states. The increased alpha power induced by Novels during REM sleep may 1) correspond to a short and transient increase in arousal; in this case, our study provides an objective measure of the greater arousing power of OWN over OTHER, 2) indicate a cortical inhibition associated with sleep protection. These results suggest that alpha modulation could participate in the selection of stimuli to be further processed during sleep.  相似文献

目的:探讨兰索拉唑治疗胃食管反流病的临床疗效,并观察食管功能变化.方法:回顾性分析我院2009年10月-2012年5月收治的GERD患者500人,根据治疗方法不同分为分为兰索拉唑组和法莫替丁组,比较两组的治疗效果;并采用超声内镜测定食管运动功能的在服药前后的变化.结果:兰索拉唑组有效率为89.2%,法莫替丁组有效率为78.4%,二者进行比较,X2=16.850,P<0.05,差异有统计学意义.兰索拉唑组与法莫替丁组治疗Ⅰ级、Ⅱ级、Ⅲ级和Ⅳ级胃食管反流病效果比较,P均<0.05,差异有统计学意义(X2=3.943,X2=5.223,X2=4.028,X2=3.988).兰索拉唑组顶端收缩指数、收缩周期、顶端被动扩张管腔的最大截面积与法莫替丁组比较,P均<0.05,差异有统计学意义(t=4.271,t=3.982,t=4.028).兰索拉唑组的不良反应发生率为4.8%,法莫替丁组不良反应发生率为13.6%,两组不良反应发生率比较,X2=11.588,P<0.05,差异有统计学意义.结论:兰索拉唑治疗胃食管反流病效果良好,不良反应发生率低,安全性高,适应临床应用.  相似文献

The aim of this study was to compare actual versus mentally simulated preparation for a complex motor skill. Two behavioral periods are observed during weightlifting: (i) an initial phase in which the subject standing behind the bar is thought to focus his attention on forthcoming execution and (ii) a second phase between hands/bar contact and execution during which the subject is thought to increase activation. Such mental processes accompanying behavioral sequences are correlated with autonomic nervous system activity, phasic responses corresponding to allocation of attentional resources, and tonic variations related to increasing general activation. To study mental processes during preparation for action, 12 subjects performed actual and imagined preparation phases of execution. Six autonomic variables were measured continuously. Skin potential (2 = 0.16), skin temperature amplitude (Z = –0.66) and duration (Z = –1.78), skin blood flow amplitude (Z = –0.56) and duration (Z = –1.51), respiratory frequency amplitude (Z = –0.14) and duration (Z = –0.13), and duration of heart rate response (Z = –1.25) were shown to be comparable (p >.05), whatever the modality of preparation. However, during mentally simulated preparation, skin resistance response was shorter than in actual preparation (Z = –2.12, p <.05), thus attesting to a weaker load, whereas lower decrease in heart rate was elicited (Z = –1.96, p <.05). This may be explained by this particular experimental condition because mental preparation would not lead to actual action. Such autonomic variables could be used as feedback to improve performance.  相似文献

目的观察体位改变对Beagle犬心脏自主神经控制的影响。方法利用大动物无创生理遥测技术,监测清醒活动状态下雌性Beagle犬在静态姿势(lying、standing、sitting、hanging)和运动(walking)姿势下的心电图(ECG),并用HRV功率谱分析其自主神经功能。结果在静态姿势下,Beagle犬RR间期(RRI)、RR间期的标准差SDNN(SDNN)、相邻RR间期差值平方和的均方根RMSSD(RMSSD)、相邻R-R间期差值〉50 ms的窦性个数占心搏总数的百分比pNNabs(50)(pNNabs(50))、TP总功率(TP)、VLF极低频功率(VLF)、标准化高频功率(HFnorm)均明显高于运动状态(P〈0.05,P〈0.01),而心率(HR)、标准化低频功率(LFnorm)和低频功率/高频功率(LF/HF)平衡指数则明显低于运动状态(P〈0.05,P〈0.01)。结论不同体位姿势在静息状态下以迷走神经活动兴奋为主,相反,在运动状态下以交感神经活动兴奋为主;体位姿势改变能引起心率的变化,必然影响心脏自主神经控制能力,其主要取决于迷走神经活动强弱有关,且导致LF/HF均衡性的破坏。  相似文献

Fifteen subjects who lived singly in an isolation unit without temporal cues were asked to note every day after awakening how many hours they thought they had slept, and in the evening before retiring how many hours they had been awake. These estimates of the duration of sleep and wakefulness were compared with the intervals between two signals given by the subjects by pressing a button at the time of waking up and when turning off their bedside reading lamp. The results can be summarized as follows: (a) the daily estimated durations of sleep and wakefulness were positively correlated with the actual durations in all but one subject; (b) sleep and wake times were better estimated in the presence of a light-dark cycle even if the subjects were not entrained by the zeitgeber; (c) for both episodes, there was a consistent trend from an overestimation of relatively short to an underestimation of long durations; (d) with equal durations in the two episodes, sleep was estimated to be shorter than wake time; (e) the most accurate estimates centered around 10.5 h of sleep and 13.5 h of wake time; (f) the sleep and wake times added up to 24 h in subjects who did not deliberately “compensate” for relatively long sleep estimates with a short wake estimate, with the full cycle being adjusted to 24 h.  相似文献

The effect of nerve stimulation on inositol phospholipid hydrolysis in autonomic tissue was assessed by direct measurement of [3H]inositol phosphate production in ganglia that had been preincubated with [3H]inositol. Within minutes, stimulation of the preganglionic nerve increased the [3H]inositol phosphate content of the superior cervical sympathetic ganglion indicating increased hydrolysis of inositol phospholipids. This effect was blocked in a low Ca2+, high Mg2+ medium. It was also greatly reduced when nicotinic and muscarinic antagonists were present together in normal medium. However, neither the nicotinic antagonist nor the muscarinic antagonist alone appeared to be as effective as both in combination. In other experiments, stimulation of the vagus nerve caused dramatic increases in [3H]inositol phosphate in the nodose ganglion but did not increase [3H]inositol phosphate in the nerve itself. This effect was insensitive to the cholinergic antagonists. Thus, neuronal activity increased inositol phospholipid hydrolysis in a sympathetic ganglion rich in synapses, as well as in a sensory ganglion that contains few synapses. In the sympathetic ganglion, synaptic stimulation activated inositol phospholipid hydrolysis and this was primarily due to cholinergic transmission; both nicotinic and muscarinic pathways appeared to be involved.  相似文献

Genetic variations in the wild-derived inbred mouse strains are more diverse than that of classical laboratory inbred mouse strains, including C57BL/6J (B6). The sleep/wake and monoamine properties of six wild-derived inbred mouse strains (PGN2, NJL, BLG2, KJR, MSM, HMI) were characterized and compared with those of B6 mice. All examined mice were nocturnal and had a polyphasic sleep pattern with a “main sleep period” identified during the light period. However, there were three sleep/wake phenotypic differences between the wild-derived mouse strains and B6 strain. First, the amount of sleep during the dark phase was comparable with that of B6 mice. However, the amount of sleep during the light phase was more varied among strains, in particular, NJL and HMI had significantly less sleep compared with that of B6 mice. Second, PGN2, NJL, BLG2, and KJR mice showed a “highly awake period” (in which the hourly total sleep time was <10%) immediately after the onset of the dark period, which was not seen in B6 mice. Third, relative to that of B6 mice, PGN2 and KJR mice showed longer duration of wakefulness episodes during the 12-h dark phase. Differences in whole brain noradrenaline, dopamine, and 5-hydroxy-tryptamine contents between the wild-derived mouse strains and B6 strain were also found. These identified phenotypes might be potentially under strong genetic control. Hence, wild-derived inbred mice could be useful for identifying the genetic factors underlying the regulation of sleep and wakefulness.  相似文献

Background

The suprachiasmatic nucleus (SCN) may play an important role in central autonomic control, since its projections connect to (para)sympathetic relay stations in the brainstem and spinal cord. The cardiac autonomic modifications during nighttime may therefore not only result from direct effects of the sleep-related changes in the central autonomic network, but also from endogenous circadian factors as directed by the SCN. To explore the influence of the SCN on autonomic fluctuations during nighttime, we studied heart rate and its variability (HRV) in a clinical model of SCN damage.

Methods

Fifteen patients in follow-up after surgical treatment for nonfunctioning pituitary macroadenoma (NFMA) compressing the optic chiasm (8 females, 26–65 years old) and fifteen age-matched healthy controls (5 females, 30–63 years) underwent overnight ambulatory polysomnography. Eleven patients had hypopituitarism and received adequate replacement therapy. HRV was calculated for each 30-second epoch and corrected for sleep stage, arousals, and gender using mixed effect regression models.

Results

Compared to controls, patients spent more time awake after sleep onset and in NREM1-sleep, and less in REM-sleep. Heart rate, low (LF) and high frequency (HF) power components and the LF/HF ratio across sleep stages were not significantly different between groups.

Conclusions

These findings suggest that the SCN does not play a dominant role in cardiac autonomic control during sleep.  相似文献

目的:调查分析胃食管反流病患者自我管理行为依从性情况及其影响因素。方法:选取2010年2月-2012年6月来我院就诊治疗的胃食管反流患者150例,利用自我行为管理量表、自我效能量表、焦虑自评量表(SAs)及抑郁自评量表(SDS)进行调查,并采用单因素分析及多因素分析法分析其影响因素。结果:所有150例患者的自我行为管理平均得分为37.12+4.95分,处于中下游水平,其中治疗依从性较好而疾病知识认知方面较差;而从单因素及多因素分析中得知,胃食管反流病患者自我行为依从性的影响因素主要为自我效能、工作学习压力及文化程度(偏回顾系数=0.301、-2.264、1.403)。结论:胃食管返流病患者的自我管理行为依从性较差,这与其文化程度较低、工作学习压力较大有关,医务人员应指导患者减轻工作学习压力,改善生活方式以提高其自我管理行为依从性。  相似文献

目的:探讨硝酸酯类药物及钙离子拮抗剂对胃食管反流症状的影响.方法:选取2010年1月-2011年1月我院收治的心血管病患者99例,根据服用药物不同分为三组:单独使用硝酸酯类药物组35例;单独使用钙离子拮抗剂组31例;以及对照组(既无硝酸酯类药物也无钙离子拮抗剂)33例.比较三组患者胃食管反流症状积分.结果:单纯使用硝酸酯类药物组比其他药物组及钙离子拮抗剂组的病人在早饱、恶心和呕吐的症状积分上较高并存在明显差异(P<0.05).钙离子拮抗剂组与硝酸酯类药物组比较烧心症状积分较高并有统计学上明显差异(P<0.05).钙离子拮抗剂组与其他药物组比较烧心及返酸积分较高,但差异无统计学意义(P>0.05).结论:硝酸酯类药物在早饱、恶心、呕吐几方面比使用其他类药物更多地引起症状.钙离子拮抗剂似乎更易引起反酸、烧心等胃食管反流症状,但未得到证实.  相似文献

Objectives

Conventional scoring of sleep provides little information about the process of transitioning between vigilance states. We applied the state space technique (SST) using frequency band ratios to follow normal maturation of different sleep/wake states, velocities of movements, and transitions between states of juvenile (postnatal day 34, P34) and young adult rats (P71).

Design

24-h sleep recordings of eight P34 and nine P71 were analyzed using conventional scoring criteria and SST one week following implantation of telemetric transmitter. SST is a non-categorical approach that allows novel quantitative and unbiased examination of vigilance-states dynamics and state transitions. In this approach, behavioral changes are described in a 2-dimensional state space that is derived from spectral characteristics of the electroencephalography.

Measurements and Results

With maturation sleep intensity declines, the duration of deep slow wave sleep (DSWS) and light slow wave sleep (LSWS) decreases and increases, respectively. Vigilance state determination, as a function of frequency, is not constant; there is a substantial shift to higher ratio 1 in all vigilance states except DSWS. Deep slow wave sleep decreases in adult relative to juvenile animals at all frequencies. P71 animals have 400% more trajectories from Wake to LSWS (p = 0.005) and vice versa (p = 0.005), and 100% more micro-arousals (p = 0.021), while trajectories from LSWS to DSWS (p = 0.047) and vice versa (p = 0.033) were reduced by 60%. In both juvenile and adult animals, no significant changes were found in sleep velocity at all regions of the 2-dimensional state space plot; suggesting that maturation has a partial effect on sleep stability.

Conclusions

Here, we present novel and original evidence that SST enables visualization of vigilance-state intensity, transitions, and velocities that were not evident by traditional scoring methods. These observations provide new perspectives in sleep state dynamics and highlight the usefulness of this technique in exploring the development of sleep-wake activity.  相似文献

目的:对胃食管反流病(GERD)随机对照试验中的安慰剂反应率进行Meta分析并研究影响该反应率的因素。方法:检索EMBASE,CochraneControlledTrialRegister和Medline数据库中公开发表的关于双盲、随机、安慰荆对照治疗GERD的英文文献,所有文献均包括质子泵抑制剂/H2受体阻滞剂,治疗时间至少为2周。对文献试验数据进行Meta分析,绘制森林图,同时绘制漏斗图检查发表偏倚。结果:纳入24个研究,共包括8917名患者。有效治疗的反应率与安慰剂反应率相比的OR为3.70(95%CI:2.77~4.95)。所有的安慰剂反应率为18.84%(2.93%-47.05%)。运用质子泵抑制剂治疗的患者与H:受体阻滞剂治疗者相比,安慰剂反应率明显降低(14.50%vs.24.68%,P=0.05)。糜烂性食管炎患者的安慰剂反应率与非糜烂性食管炎者相比略低,两者差异无显著性(P〉0.05)。结论:在GERD随机对照试验中,安慰剂反应率确实存在。该反应率降低与质子泵抑制剂的运用相关,而与腐蚀性食管炎的存在与否无关。  相似文献
