Epidemiological evidence indicates that sudden loading of the torso is a risk factor for low back injury. Accurately quantifying the time-varying loading of the spine during sudden loading events and how these loading profiles are affected by workplace factors such as fatigue, expectation, and training can potentially lead to intervention strategies that can reduce these risks. Electromyographic and trunk motion data were collected from six male participants who performed a series of sudden loading trials with varying levels of expectation (no preview, 300-ms audible preview), fatigue (no fatiguing exertion preceding sudden load, short duration/high intensity fatiguing exertion preceding sudden load), and training (untrained, trained). These data were used as inputs to an adaptive system identification model wherein time-varying lower back stiffness, torque, work, and impulse magnitudes were calculated. Results indicated that expectation significantly increased peak and average stiffness by 70% and 113%, respectively, and significantly decreased peak torque, work, and impulse magnitudes by 36%, 50%, and 45%, respectively. Training significantly decreased peak torque and work by 25% and 34%, respectively. The results also showed a significant interaction between expectation and training wherein training had a positive effect during the trials with preview but no effect during the trials with no preview (increased peak stiffness by 17% and decreased impulse magnitude by 43%). 相似文献

The effects of whole body vibration exposure on the neuromuscular responses following inertial-based trunk perturbations were examined. Kinematic and surface EMG (sEMG) data were collected while subjects were securely seated on a robotic platform. Participants were either exposed to 10 min of vibration or not, which was followed by sudden inertial trunk perturbations with and without timing and direction knowledge. Amplitude of sEMG was analyzed for data collected during the vibration protocol, whereas the onset of sEMG activity and lumbar spine angle were analyzed for the perturbation protocol. Data from the vibration protocol did not show a difference in amplitude of sEMG for participants exposed to vibration and those not. The perturbation protocol data showed that those not exposed to vibration had a 14% faster muscle onset, despite data showing no difference in fatigue level. 相似文献

Much discussion exists about which is the most effective technique to improve spine stability. The purpose of this study was to evaluate the effectiveness of abdominal bracing and abdominal hollowing maneuvers to control spine motion and stability against rapid perturbations. Eleven healthy males were posteriorly loaded in different experimental conditions: resting with no knowledge of the perturbation timing; performing each of the stabilization maneuvers at 10%, 15% and 20% of internal oblique maximum voluntary contraction with no knowledge of the perturbation timing; and naturally coactivating the trunk muscles when perturbation timing was known. An EMG biofeedback system was used to control the pattern and intensity of abdominal coactivation. The muscular preactivation of seven trunk muscles (bilaterally registered), the applied force, and the torso muscular and kinematic responses to loading were measured; and the spine stability and compression were modeled. The hollowing maneuver was not effective for reducing the kinematic response to sudden perturbation. On the contrary, the bracing maneuver fostered torso cocontraction, reduced lumbar displacement, and increased trunk stability, but at the cost of increasing spinal compression. When the timing of the perturbation was known, the participants were able to stabilize the trunk while imposing smaller spine compressive loads. 相似文献

Background
The abdominal drawing-in maneuver (ADIM) is used to prevent abnormal movements of the lumbar spine and pelvis during therapeutic exercises. This study compared the effects of ADIM on the muscle onset time of the hamstring, gluteus maximus, and erector spinae muscles during prone hip extension exercise in subjects with or without hyperlordotic lumbar angle. Forty healthy adults (18 male, 22 female) were recruited for this study.Methods
The lumbar lordotic angles and pelvic tilt angles of the subjects were measured using the Avaliação postural analysis software. The subjects were divided into two groups: the lumbar hyperlordotic angle (LHLA) and lumbar normal lordotic angle (LNLA) groups. The muscle contraction onset time of the hamstring, gluteus maximus, and erector spinae were assessed using surface electromyography.Results
During ADIM application, the muscle contraction onset time of the gluteus maximus was significantly increased in the LHLA group compared with the LNLA group.Conclusions
ADIM application during prone hip extension was more effective for gluteus maximus onset time in the LHLA group. Therefore, ADIM during prone hip extension may be useful for gluteus maximus training in individuals with lumbar hyperlordosis. 相似文献5.
目的观察舒芬太尼用于经腹子宫全切术后静脉镇痛效果、临床副作用,并与等效价的芬太尼比较有无优越性。方法选择择期硬膜外阻滞麻醉下经腹行子宫全切术患者80例,随机分为A组(芬太尼组)40例,B组(舒芬太尼组)40例。A组给予负荷量芬太尼40μg;B组给予负荷量舒芬太尼4μg。两组均采用珠海福尼亚电子微量泵(CPE-101—200型)行自控静脉镇痛。观察术后4、8、16、24h的疼痛、镇静、恶心呕吐的评分,记录血氧饱和度、镇痛液消耗量。疼痛情况用水平视觉模拟评分法(VAS)评估,镇静程度按Ramsay评分法评估。结果与A组比较,B组各时点VAS评分普遍低于A组,但仅术后24h差异有显著统计学意义(P〈0.05)。两组患者镇静评分均无达到3分者,B组各时点Ramsay评分普遍高于A组,其中术后16、24h差异有显著统计学意义(P〈0.05)。B组患者术后各时点镇痛液消耗量普遍低于A组,其中术后16、24h差异有显著统计学意义(P〈0.05)。两组术后恶心呕吐发生率均低,其中术后4h差异有统计学意义(P〈0.05)。结论经腹子宫全切术后舒芬太尼组的静脉镇痛效果略优于芬太尼组,镇静作用亦明显高于芬太尼组,而且减少恶心呕吐等副作用的发生率。 相似文献

Animal test systems are reviewed that have relevance to sudden infant death syndrome (SIDS) are reviewed. These test interactions between infectious agents (or their toxins) and products of cigarette smoke. Infectious agents implicated in SIDS include members of the enterobacteria and clostridia, Staphylococcus aureus and Streptococcus pyogenes. Smoking is thought to be the single most preventable cause of SIDS. Tobacco smoke contains many extremely toxic products including cyanide and nicotine. Many animal test systems are available to examine the potency of bacterial toxins and smoke-derived components. These include mice, hamsters, rats and chick embryos. Such systems reveal synergy between bacterial toxins, especially endotoxin and superantigens. They have also demonstrated potentiation of low levels of bacterial toxin by low levels of both nicotine and its primary metabolite, cotinine. These findings suggest a possible causal explanation for the fact that passive exposure to cigarette smoke is a risk factor in sudden infant death syndrome. 相似文献

Mechanical loading of the spine is a major causative factor of degenerative changes and causes molecular and structural changes in the intervertebral disc (IVD) and the vertebrae end plate (EP). Pleiotrophin (PTN) is a growth factor with a putative role in bone remodeling through its receptor protein tyrosine phosphatase beta/zeta (RPTPβ/ζ). The present study investigates the effects of strain on PTN and RPTPβ/ζ protein expression in vivo. Tails of eight weeks old Sprague-Dawley rats were subjected to mechanical loading using a mini Ilizarov external apparatus. Rat tails untreated (control) or after 0 degrees of compression and 10°, 30° and 50° of angulation (groups 0, I, II and III respectively) were studied. PTN and RPTPβ/ζ expression were evaluated using immunohistochemistry and Western blot analysis. In the control group, PTN was mostly expressed by the EP hypertrophic chondrocytes. In groups 0 to II, PTN expression was increased in the chondrocytes of hypertrophic and proliferating zones, as well as in osteocytes and osteoblast-like cells of the ossification zone. In group III, only limited PTN expression was observed in osteocytes. RPTPβ/ζ expression was increased mainly in group 0, but also in group I, in all types of cells. Low intensity RPTPβ/ζ immunostaining was observed in groups II and III. Collectively, PTN and RPTPβ/ζ are expressed in spinal deformities caused by mechanical loading, and their expression depends on the type and severity of the applied strain. 相似文献

Abstract: The activity of 2',3'-cyclic nucleotide 3'-phos-phohydrolase (CNPase) has been determined in corpus callosum, subcortical white matter, and spinal cord of infants whose death was attributed to the sudden infant death syndrome (SIDS), and compared with enzyme activity in other cases in which the cause of death was not associated with respiratory distress. In nearly half the SIDS cases, CNPase activity and oligodendroglial cell numbers were reduced before the onset of myelination, but only in the corpus callosum. In other SIDS cases, enzyme activity and cell numbers were the same as in non-SIDS cases. If the expression of CNPase activity reflects glioblast differentiation to oligodendrocytes with myelinating potential, then this transformation is abnormal in certain SIDS cases, as also evidenced in cases of prolonged neonatal respiratory insufficiency and gives rise to a subsequent deficit of myelin in the corpus callosum. 相似文献

Objectives:The study aims to investigate the relationship between abdominal muscle activity and the cross-sectional area (CSA) of the lumbar muscles and assess their role in the functional assessment of patients with chronic non-specific low back pain (CNSLBP).Methods:142 patients with CNSLBP were included in this study. Disability levels were evaluated with the Roland-Morris Low Back Pain and Disability Questionnaire. The functional assessments of the participants were evaluated with a 6-minute walk test. Abdominal muscle activity was measured using a pressure biofeedback unit. The CSA of the bilateral multifidus, erector spinae, and psoas muscles were measured T2-weighted MRI images at the L2-L5 levels.Results:Significant correlations were found between the abdominal muscle activity during the posterior pelvic tilt movement and the CSA of the erector spinae muscle at the L4 and L5 levels, and the psoas muscle at the L2–L5 levels (correlation coefficient range from 0.32 to 0.48). Abdominal muscle activity yielded a significant additional contribution to the variance on the functional assessment (R2 change=0.101).Conclusions:The relationship of abdominal muscle activity with lumbar muscles and the contribution of muscle activities to functional assessment should be considered in the management of patients with CNSLBP. 相似文献
