The Automated External Defibrillation is the key link of the chain of survival for patients in cardiac arrest. A lot of case series and trials have shown the effectiveness of early defibrillation by first rescuers and trained lay persons. The earlier the defibrillation is performed, the better is the rate of survival to hospital discharge. To increase the survival rate healthcare providers, first rescuer citizens at worksites and trained lay rescuers should be authorized, equipped and encouraged to perform early defibrillation combined with effective cardiopulmonary resuscitation (CPR). The new generation of Automated External Defibrillators (AED) are sophisticated, computerized devices that are reliable and simple to operate, enabling also lay rescuers to administer this lifesaving intervention to victims of cardiac arrest. For the concept of recurrent adequate and qualified training in the use of the AED integrated in effective DPR is recommended.  相似文献

Effective immobilization and transportation are vital to the life-saving acute medical care needed when treating critically injured people. However, the most common types of stretchers used today are wrought with problems that can lead to further medical complications, difficulty in employment and rescue, and ineffective transitions to hospital treatment. Here we report a novel first aid stretcher called the "emergency carpet", which solves these problems with a unique design for spine injured patients. Polyurethane composite material, obtained by a novel process of manually mixing isocyanate and additives, can be poured into a specially designed fabric bag and allowed to harden to form a rigid human-shaped stretcher. The effectiveness of the emergency carpet was examined in the pre-hospital management of victims with spinal fractures. Additionally, it was tested on flat ground and complex terrain as well as in the sea and air. We demonstrated that the emergency carpet can be assembled and solidified on the scene in 5 minutes, providing effective immobilization to the entire injured body. With the protection of the emergency carpet, none of the 20 patients, who were finally confirmed to have spinal column fracture or dislocation, had any neurological deterioration during transportation. Furthermore, the carpet can be handled and transported by multiple means under differing conditions, without compromising immobilization. Finally, the emergency carpet allows the critically injured patient to receive multiple examinations such as X-ray, CT, and MRI without being removed from the carpet. Our results demonstrate that the emergency carpet has ideal capabilities for immobilization, extrication, and transportation of the spine injured patients. Compared with other stretchers, it allows for better mobility, effective immobilization, remarkable conformity to the body, and various means for transportation. The emergency carpet is promising for its intrinsic advantages in the pre-hospital management of accident victims.  相似文献

目的探讨毒蛇咬伤患者院前早期程序化急救处理方案的应用价值。方法回顾性分析我院58例毒蛇咬伤患者应用院前早期程序化急救处理方案的临床资料。结果 58例毒蛇咬伤患者经院前早期程序化急救处理后,痊愈出院53例,好转3例,自动出院1例,院前死亡2例(眼镜王蛇咬伤1例,银环蛇咬伤1例)。结论院前急救是蛇伤治疗前期的一个关键性步骤,院前早期程序化急救处理方案应用于蛇伤患者,有利于提高抢救成功率。  相似文献

Snake venom toxins first transit the lymphatic system before entering the bloodstream. Ointment containing a nitric oxide donor, which impedes the intrinsic lymphatic pump, prolonged lymph transit time in rats and humans and also increased rat survival time after injection of venom. This pharmacological approach should give snakebite victims more time to obtain medical care and antivenom treatment.  相似文献

Tetrapolar rheography was used to investigate central hemodynamics parameters in a group of 80 healthy subjects aged 25-68. Hyperkinetic, eukinetic and hypokinetic types of circulation were determined by interval distribution according to cardiac index. The proportion of the examinees by their hemodynamic type was: 16.3:63.7:20.0%. In hyperkinetic type the optimal level of average blood pressure was provided by a relatively high cardiac output (CO) and low total and specific peripheral resistance (TPR, SPR). The hypokinetic type of hemodynamics was characterized by a low level of CO and a high value of TPR and SPR. Subjects with eukinetic hemodynamic type were in an intermediate position. It is suggested to take into consideration the hemodynamic type in hypertensive subjects for drug selection and treatment control.  相似文献

Background  

The species is a fundamental unit of biological pattern and process, but its delimitation has proven a ready source of argument and disagreement. Here, we discuss four key steps that utilize statistical thresholds to describe the morphological variability within a sample and hence assess whether there is evidence for one or multiple species. Once the initial set of biologically relevant traits on comparable individuals has been identified, there is no need for the investigator to hypothesise how specimens might be divided among groups, nor the traits on which groups might be separated.  相似文献

A bioenergetic conception of insulin-independent diabetes mellitus (IIDM) is put forward. Molecular mechanisms of carbohydrate and fat metabolism are discussed which result in a vicious regulatory cycle to provoke a transition to stable pathology. Previously unknown role of human saliva in oral cavity in this regulatory cycle is revealed. New diagnostic tests are developed on the basis of the conception on IIDM pathogenesis. The results of clinical trials are presented which have been obtained during population screening.  相似文献

Rates of allergic diseases such as asthma and rhinitis are on the rise as important health problems in every country of the world. Allergen specific immunotherapy with natural allergenic extracts is a treatment directed to changing the natural course of these diseases, and is a treatment that has reported beneficial effects in a majority of allergic patients. However, this treatment is difficult because of the complex composition of the extracts. The composition is difficult to standardize and, consequently, the risk of anaphylactic shock is increased; furthermore, sensitization can occur to other antigens present in the extract. Therefore, new allergen specific immunotherapy approaches are needed. Chemically defined and standardized antigens are more easily managed and provide a safer and more efficient treatment. Vaccines for immunotherapy have already been designed, based on recombinant allergens, variants (or peptides derived from them), that can be administrated alone or in combination with adjutants. Some of these preparations are indicated for facilitating the uptake and antigenic presentation by dendritic cells, or by targeting the mast cells and basophiles. Studies in vitro, in animal models and clinical trials in allergic patients, indicate that these preparations may provide protection against the allergen exposure and improve the symptoms by inducing the production of blocking antibodies of the IgE mediated response, production of regulator T cells and cytokines of Th1 profile.  相似文献

First aid, as a profession in its own right, has a history of only 120 years. It evolved from the teachings of the Royal Humane Society and military surgeons, who saw the wisdom of training in splinting and bandaging for battlefield wounds. In 1878 two Aberdeenshire military officers, Surgeon-Major Peter Shepherd of the Royal Herbert Military Hospital, Woolwich, London, and Colonel Francis Duncan established the concept of teaching first aid skills to civilians. This radical new enterprise, conducted under the auspices of the newly formed St John Ambulance Association, was a natural evolution from the body''s philanthropic and ambulance transport work. Shepherd conducted the first class in the hall of the Presbyterian school in Woolwich using a comprehensive first aid curriculum that he had developed. Within months of that first class, local Woolwich civilians used their skills when the pleasure boat Princess Alice sank in the Thames at Woolwich, killing 600 people. Within a decade, the new discipline of first aid spread rapidly throughout the world, and by the end of the 19th century, hundreds of thousands of St John first aid certificates had been awarded in four continents. Shepherd''s pioneering classes changed the world''s concept of the need for the provision of skilled prehospital care.  相似文献

Despite many shortcomings, liver biopsy is regarded as the gold standard for assessing liver fibrosis. A less invasive and equally or more reliable approach would constitute a major advancement in the field. Proteomics can aid discovery of novel serological markers and these proteins can be measured in patient blood. A major challenge of discovering biomarkers in serum is the presence of highly abundant serum proteins, which restricts the levels of total protein loaded onto gels and limits the detection of low abundance features. To overcome this problem, we used two-dimensional gel electrophoresis (2-DE) over a narrow pH 3-5.6 range since this lies outside the range of highly abundant albumin, transferrin and immunoglobulins. In addition, we used in-solution isoelectric focusing followed by SDS-PAGE to find biomarkers in hepatitis C induced liver cirrhosis. Using the pH 3-5.6 range for 2-DE, we achieved improved representation of low abundance features and enhanced separation. We found in-solution isoelectric focusing to be beneficial for analyzing basic, high molecular weight proteins. Using this method, the beta chains of both complement C3 and C4 were found to decrease in serum from hepatitis C patients with cirrhosis, a change not observed previously by 2-DE. We present two proteomics approaches that can aid in the discovery of clinical biomarkers in various diseases and discuss how these approaches have helped to identify 23 novel biomarkers for hepatic fibrosis.  相似文献

Vaccines have had a tremendous impact on the control of infectious diseases. Not only are vaccines potentially the least expensive mechanism to combat infectious diseases, under optimal conditions, widespread vaccination can result in disease eradication - as in the case of smallpox. Despite this great potential, vaccines have had little impact on human parasitic infections. The reasons for this are many - these eukaryotic pathogens are genetically and biologically complex organisms, some with elaborate life cycles and well-honed immune evasion mechanisms. Additionally, our understanding of the mechanisms of immune control of many parasitic infections -- of what constitutes an effective immune response and of how to induce high-quality immunological memory -- is not fully developed. This review attempts to highlight recent advances that could impact vaccine discovery and development in parasitic infections and proposes areas where future studies may lead to breakthroughs in vaccines for the agents of parasitic diseases. There are several other recent reviews highlighting the results of vaccine trials, specifically in the malaria field.  相似文献
