Assessment of higher mental functions, objective detection of cognitive impairments, and investigation of pathophysiological mechanisms underlying these impairments in various neuropsychological diseases are of great importance for neuropsychophysiology. The endogenous event-related potential (ERP) approach is one of the instrumental neurophysiological methods that are currently used for assessing these complicated processes because recorded potentials reflect the intrinsic brain activity and changes in these potentials are caused by endogenous factors of the brain activity. The P300 cognitive evoked potential, induced by selective attention to a stimulus, has been the most widely used endogenous ERP. This potential may be helpful for studying mechanisms of mental disturbances, as it reflects neuronal processes connected with nonspecific activating reticulothalamic systems, as well as with limbic and neocortical mechanisms of selective attention and short-term memory.  相似文献

Fluorodeoxyglucose (FDG) Positron Emission Topography (PET) brain hypometabolism (HM) correlates with diminished cognitive capacity and risk of developing dementia. However, because clinical utility of PET is limited by cost, we sought to determine whether a less costly electrophysiological measure, the P300 evoked potential, in combination with neuropsychological test performance, would validate PET HM in neuropsychiatric patients. We found that patients with amnestic and non-amnestic cognitive impairment and HM (n = 43) evidenced significantly reduced P300 amplitudes, delayed latencies, and neuropsychological deficits, compared to patients with normal brain metabolism (NM; n = 187). Data from patients with missing cognitive test scores (n = 57) were removed from the final sample, and logistic regression modeling was performed on the modified sample (n = 173, p = .000004). The logistic regression modeling, based on P300 and neuropsychological measures, was used to validate membership in the HM vs. NM groups. It showed classification validation in 13/25 HM subjects (52.0%) and in 125/148 NM subjects (84.5%), correlating with total classification accuracy of 79.8%. In this paper, abnormal P300 evoked potentials coupled with cognitive test impairment validates brain metabolism and mild/moderate cognitive impairment (MCI). To this end, we cautiously propose incorporating electrophysiological and neuropsychological assessments as cost-effective brain metabolism and MCI indicators in primary care. Final interpretation of these results must await required additional studies confirming these interesting results.  相似文献

The aim of the study was to trace the cumulative changes in the amplitude-time parameters of the AEP (acoustic evoked potential) components N1, N2, and P300 in the experimental situations of different complexities (counting and listening to sounds) and to compare the differences between young and mature subjects. The AEP was recorded in 12 healthy subjects aged 18 to 22 years and in 12 subjects aged 32 to 59 years. It was revealed that the components N1, N2, and P300 recorded in the situation of listening without any preliminary instruction did not differ in young and mature individuals. In the younger subjects, a higher N1 amplitude was revealed when they counted sounds, compared to listening, whereas the latency values of this component did not differ, irrespective of the complexity of the task. The component N2 was shown to demonstrate latency stability in relation to the age and the experimental situation. The N2 amplitude was higher in the situation of counting sounds in both age groups. The amplitude-time parameters of P300 did not differ in subjects of different ages in the listening task. The features of the components N1, N2, and P300 revealed in young and mature subjects in the experimental situations differing in complexity lead us to suggest that, with age, humans develop adaptive mechanisms contributing to successful task performance.  相似文献

Participants of the Chernobyl clean-up (n = 145) teams exposed to radiation doses from 0.05 to 3.5 Gy who had for the first time complained of pathologic somatosensory sensations (ostealgic syndrome), 20 healthy subjects, and 50 veterans of the war in Afghanistan with posttraumatic stress disorder (PTSD) were examined by a neuropsychiatrist and presented with the MMPI test. Somatosensory evoked potentials (SSEPs) were recorded. Paresthesia and cenesthopathy were characteristic of the participants of the Chernobyl clean-up. Sensation disorders of the cerebral type, kinesthetic illusions, cenesthopathic hypochondriac disorders, and paroxysmal psychosensory states predominated in this group of subjects. They differed significantly from the veterans with PTSD in markedly increased scores on MMPI scales (hypochondriasis, schizophrenia, pure hypochondriasis, pure schizophrenia, emotional exclusion, and perception oddity), which closely correlated with clinical somatosensory symptoms. In clean-up workers, somatosensory disorders were significantly associated with hypochondriac and schizophrenic-like symptoms. The latencies (LPs) of main SSEP components—N 20, P 25, N 140, P 300, and N 400—were increased and their amplitudes decreased in subjects exposed to radiation. Their SSEPs had significant topographical deviations in the left temporoparietal area: the contralateral LPs were increased, whereas the contralateral amplitudes of the thalamocortical N 20 component and the cortical P 25 component were decreased as compared to normal values. Somatosensory disorders and hypochondriac and schizophrenic symptoms were significantly correlated with changes in the SSEPs. The decrease in the N 20 amplitude and increase in the P 25 latency in the left temporoparietal area were dose-dependent. The results suggest cerebral rather than peripheral origin of ostealgic syndrome and other somatosensory disorders in the participants of the Chernobyl clean-up. These disorders are associated with radiation-induced dysfunction of the corticolimbic structures of the left—dominant—hemisphere. It is suggested that somatosensory disorders in patients exposed to low doses of radiation can be considered as manifestations of chronic fatigue syndrome /fibromyalgia, whereas schizoform organic brain lesions manifest themselves after exposure to a radiation dose of 0.3–0.5 Gy.  相似文献

The correlations between the volumes of the caudate nucleus, putamen, amygdala, and hippocampus, on the one hand, and the P300 amplitude and latency of auditory evoked potentials, on the other hand, were studied in 14 schizophrenics. Significant positive correlations were found between the parameters of the late cognitive potential P300 (predominantly in the left hemisphere) and the caudate nucleus and putamen volumes, as well as between the right amygdala volume and the P300 amplitude in the left temporal region. The parameters of an event-related EEG potential (ERP), P300 wave, are now extensively used as objective neurophysiological indices of the state of cognitive functions. At the same time, information on the effects of the autonomic nervous system on the parameters of P300 is limited. In Parkinson’s disease clinics, in addition to the leading motor disorders, more or less clear psychoemotional, cognitive, and autonomic (in particular cardiovascular) impairments are usually observed. This allows one to study the dependence between the cardiovascular dysfunction and intensity of cognitive disorders in Parkinsonian patients. In our study on this contingent, we analyzed correlations between the parameters of P300 potential, indices of the state of the cognitive sphere (determined using a questionnaire, Mini Mental State Examination, MMSE, and a Luriya’s test), and indices of variational pulsometry. Thirty-five Parkinsonian patients (49 to 74 years, severity of disease 1.5 to 3.0 by the international classification) were examined. We found a negative influence of excessive sympathetic tonus in cardiovascular control on the state of cognitive functions. The latency of P300 potential was longer in patients with greater intensities of sympathetic influences on the cardiovascular system. The coefficients of correlation of the latency of P300 with the amplitude of mode of R-R intervals (AMo), index of tension in the regulatory systems by Baevskii (IT), and index of autonomic balance by Baevskii (IAB) were 0.52 (P < 0.01), 0.36 (P < 0.05), and 0.37 (P < 0.05), respectively. The above autonomic indices demonstrated significant negative correlations with the volume of short-term memory measured by Luriya’s test. The P300 latency, in turn, showed negative correlations with the memory volume estimated by the MMSE scale and Luriya’s test. With increase in the age of patients, the degree of the above-mentioned correlations between the P300 latency, memory volume (by Luriya’s test), and parameters of variational pulsometry increased. Our data emphasize the expedience of “routine” studies of the balance of sympathetic and parasympathetic control in pathological states accompanied by clear or subclinical cognitive disorders. Early recognition of cardiovascular dysfunction and its corresponding therapeutic correction should improve the state of brain functions and quality of life in patients suffering from neurodegenerative diseases, in particular from Parkinson’s disease. Background

Deficit schizophrenia (DS) has been proposed as a pathophysiologically distinct subgroup within schizophrenia. Earlier studies focusing on neurocognitive function of DS patients have yielded inconsistent findings ranging from substantial deficits to no significant difference relative to non-deficit schizophrenia patients (NDS). The present study investigated the severity and characteristic patterns of neurocognitive impairments in DS and NDS patients and their relationships with clinical variables.

Methods

Attention, ideation fluency, cognitive flexibility and visuospatial memory function were assessed in 40 DS patients, 57 NDS patients, and 52 healthy controls by a comprehensive neuropsychological battery.

Results

Both schizophrenia subgroups had overall more severe cognitive impairments than controls while DS performed worse on every neuropsychological measure except the Stroop interference than the NDS patients with age and education as the covariates. Profile analysis found significantly different patterns of cognitive profiles between two patients group mainly due to their differences in attention and cognitive flexibility functions. Age, education, illness duration and negative symptoms were found to have the correlations with cognitive impairments in the NDS group, while only age and the negative symptoms were correlated with the cognitive impairments in the DS group. Multiple regression analyses revealed that sustained attention and cognitive flexibility were the core impaired cognitive domains mediating other cognitive functions in DS and NDS patients respectively.

Conclusions

DS patients exemplified worse in almost all cognitive domains than NDS patients. Sustained attention and cognitive flexibility might be the key impaired cognitive domains for DS and NDS patients respectively. The present study suggested the DS as a specific subgroup of schizophrenia.  相似文献

Obstructive sleep apnea syndrome (OSAS) is a sleep disorder characterized with upper airway obstructions. Some studies showed cognitive and electrophysiological changes in patients with OSAS; however, contradictory results were also reported. The purpose of the present study was twofold: (1) to investigate cognitive changes in severe OSAS patients by using neuropsychological tests and electrophysiological methods together, (2) to investigate influence of hypoxemia levels on cognition. Fifty-four severe OSAS patients and 34 age-, gender- and education matched healthy subjects were participated. OSAS patients were further divided into two subgroups according to minimum oxygen saturation levels. All participants underwent a detailed neuropsychological test battery. A classical visual oddball task was used to elicit ERP P300 and mean P300 amplitudes were measured from Fz, Cz and Pz electrode sites. OSAS patients showed reduced mean P300 amplitudes up to 43–51% on all electrode sites compared to healthy controls. Subgroup analysis revealed significant differences in neuropsychological test scores between healthy controls and high hypoxemia OSAS group, as well as between low and high hypoxemia groups. Moreover, both low and high hypoxemia OSAS groups had lower P300 amplitudes compared with healthy controls. P300 amplitudes showed a gradual decline in parallel with increasing hypoxemia severity; however, the difference between high and low hypoxemia OSAS groups did not reach significance. Moderate correlations were found between sleep parameters, neuropsychological test scores and P300 amplitudes. These results suggest that electrophysiological measures could be better indicators of cognitive changes than neuropsychological tests in OSAS, particularly in mildly affected patients.  相似文献

With age regular changes take place in the latent periods of spikes of the slow auditory evoked potential. In particular, the latencies of the comparatively early waves (P1, N1, and P2) become progressively shortened. Between 3–7 and 8–13 years the decrease is 50–60 msec, and later it is 25–35 msec. The latencies of the latest waves, especially P3, N3, and P4, increase from 3–7 to 8–13 years by 35–65 msec. Later the latent period of the P3 spike remains unchanged but the N3 and P4 waves disappear completely. Of all the components of the slow auditory evoked potential the most stable is the N2 wave, the latent period of which decreases only very slightly with age. In children aged 3–7 years two wave complexes (P1N1P2 and P2N2P3) overlap frequently to form a single undifferentiated wave. This splits up into its components by 8 years of age. Long age changes in the shape and parameters of the slow auditory evoked potential are examined from the standpoint of the predominantly extralemniscal origin of this potential. On the basis of correlation discovered between the late waves of the evoked potential and the level of EEG synchronization it is postulated that the late waves of the slow evoked potential are formed with the participation of the nonspecific synchronizing system.Tbilisi State Postgraduate Medical Institute. The visual evoked potentials (EPs) in response to lateralized and central visual symbols under the conditions of involuntary (passive viewing) and selective attention (when one of the symbols was a target and required a rapid and precise motor reaction) are considered. The evoked potentials in the occipital, parietal, and frontal derivations were recorded in 20 healthy subjects. It was shown that the EP during selective attention are most pronounced and more alike in the parietal derivations. A strong positive correlation was revealed between the EP amplitude ([N1–P3] component) and the EP stability (correlation between the repeated EP). The involuntary and voluntary forms of attention supplement each other: the more expressed the involuntary attention (assessed by the [N1–P3] component) the higher the EP to target stimuli during voluntary attention and the shorter the reaction time. It is suggested that the role of visual attention consists in the increase and stabilization of cortical activity (primarily, the parietal regions) engaged in solving a visual task.  相似文献

To justify neurophysiological correlates of depressive disorders, the spetral parameters of EEG, peak latencies of the “late” components of auditory cognitive evoked potentials, and latencies of sensorimotor reactions in middle age and elderly patients (aged 53–72 years) during therapy of prolonged psychogenic depressive reaction (F43.21 according to ICD-10) have been studied. Initial depression severity was associated to the EEG signs of decreased functional state of the anterior areas of the left hemisphere and increased activation of the right hemisphere (especially, its temporal areas). Pronounced improvement of clinical state under the affect of psychopharmacotherapy was accompanied by acceleration of the sensorimotor reactions, a decrease in peak latencies of the “late” components (P2, N2, and P3) of auditory cognitive evoked potentials and associated with the EEG signs of improvement of functional state of the posterior areas of the brain, an enforcement of inhibitory processes in the right hemisphere (especially, in its frontal, central, and temporal areas) and more pronounced activation of frontal areas of the left hemisphere. The data are in good agreement with the concept on the systemic character of impairments of brain functioning in depression, as well as on the preferential role of the left hemisphere in control of positive emotions and the right one, of negative emotions.  相似文献

Background

Apolipoprotein C3 (APOC3) modulates triglyceride metabolism through inhibition of lipoprotein lipase, but is itself regulated by insulin, so that APOC3 represents a potential mechanism by which glucose metabolism may affect lipid metabolism. Unfavorable lipoprotein profiles and impaired glucose metabolism are linked to cognitive decline, and all three conditions may decrease lifespan. Associations between apolipoprotein C3 (APOC3) gene polymorphisms and impaired lipid and glucose metabolism are well-established, but potential connections between APOC3 polymorphisms, cognitive decline and diabetes deserve further attention.

Methods

We examined whether APOC3 single nucleotide polymorphisms (SNPs) m482 (rs2854117) and 3u386 (rs5128) were related to cognitive measures, whether the associations between cognitive differences and genotype were related to metabolic differences, and how diabetes status affected these associations. Study subjects were Hispanics of Caribbean origin (n = 991, aged 45–74) living in the Boston metropolitan area.

Results

Cognitive and metabolic measures differed substantially by type II diabetes status. In multivariate regression models, APOC3 m482 AA subjects with diabetes exhibited lower executive function (P = 0.009), Stroop color naming score (P = 0.014) and Stroop color-word score (P = 0.022) compared to AG/GG subjects. APOC3 m482 AA subjects with diabetes exhibited significantly higher glucose (P = 0.032) and total cholesterol (P = 0.028) compared to AG/GG subjects. APOC3 3u386 GC/GG subjects with diabetes exhibited significantly higher triglyceride (P = 0.004), total cholesterol (P = 0.003) and glucose (P = 0.016) compared to CC subjects.

Conclusions

In summary, we identified significant associations between APOC3 polymorphisms, impaired cognition and metabolic dysregulation in Caribbean Hispanics with diabetes. Further research investigating these relationships in other populations is warranted.  相似文献

An integrated neuropsychological study and recording of auditory cognitive evoked potentials (EPs) using the three-stimulus oddball paradigm was performed in groups of ten subjects who participated in the liquidation of the consequences of the Chernobyl accident aged 50.5 ± 4.0 years and ten healthy subjects aged 47.0 ± 6.0 years. The neuropsychological study showed impairment of higher mental functions, including aspontaneity, fatigability, a decrease in the auditory-verbal and visual memories, and higher motor function deficiency, in cleanup workers (usually referred to as liquidators). Analysis of the amplitude and time characteristics of the P300 component of the auditory cognitive EP showed a decrease in the amplitude of this component in all cortical areas of liquidators in both experimental situations and for all stimuli compared to healthy subjects of the same age. Analysis of the latent period (LP) of P300 in liquidators showed the most distinct increase in the situation of passive audition for all types of stimuli. Analysis of the patterns of reactive rearrangements in different experimental situations showed that the P300 LPs of healthy subjects were similar for all types of stimuli during passive audition and were higher for the significant stimulus in the situation of counting. Liquidators were characterized by an type of response, with the LPs of responses to most stimuli increased in both experimental situations. Analysis of regional changes in the P300 LP showed that the differences in the P300 LP between the groups were the largest in the frontal areas of the left hemisphere. In addition, liquidators exhibited “uneconomical, excessive” responses as compared to normal responses, which suggests a decrease in the attention and memory capacities and could contribute to the impairment of higher mental functions. The changes in the amplitude and time characteristics of the P300 component of auditory EPs suggest deceleration of perception, processing, and analysis of information combined with weakened inhibition. The changes found in liquidators are similar to those observed in elderly people, which supports the hypotheses on accelerated brain aging and on pathological aging caused by low-dose irradiation.  相似文献

Objectives

Bipolar disorder is accompanied by cognitive impairments, which persists during euthymic phases. The purpose of the present study was to identify those neuropsychological tests that most reliably tell euthymic bipolar patients and controls apart, and to clarify the extent to which these cognitive impairments are clinically significant as judged from neuropsychological norms.

Methods

Patients with bipolar disorder (type I: n = 64; type II: n = 44) and controls (n = 86) were examined with a comprehensive neuropsychological test battery yielding 47 measures of executive functioning, speed, memory, and verbal skills. Multivariate analysis was used to build a model of cognitive performance with the ability to expose underlying trends in data and to reveal cognitive differences between patients and controls.

Results

Patients with bipolar disorder and controls were partially separated by one predictive component of cognitive performance. Additionally, the relative relevance of each cognitive measure for such separation was decided. Cognitive tests measuring set shifting, inhibition, fluency, and searching (e.g., Trail Making Test, Color-Word) had strongest discriminating ability and most reliably detected cognitive impairments in the patient group.

Conclusions

Both bipolar disorder type I and type II were associated with cognitive impairment that for a sizeable minority is significant in a clinical neuropsychological sense. We demonstrate a combination of neuropsychological tests that reliably detect cognitive impairment in bipolar disorder.  相似文献

The aim of this study was to investigate the effects of smoking, polymorphisms of XRCC1 codons 194 and 399, and age on levels of basal DNA damage (as measured by an alkaline comet assay) on mononuclear cells in 122 healthy Japanese workers. In the whole group of 122 individuals, the tail moment (TM) values of current smokers (P < 0.001) or former smokers (P = 0.03) were significantly higher than those of nonsmokers. Individuals bearing the XRCC1 399Gln variant allele showed significant increases in TM values in all subjects or in referent subgroups stratified by age or smoking status except in the current smokers group; in contrast, the TM values of individuals bearing the XRCC1 194Trp variant allele were significantly lower than those of individuals bearing wild-type Arg/Arg genotypes. Furthermore, older subjects (≥47 years old) had significantly higher TM values than younger subjects (<47 years old) in all subjects (P = 0.008). Multiple regression analysis indicated that smoking habits, polymorphisms of XRCC1 codons 194 and 399, and age were important variables affecting individuals basal DNA damage.  相似文献

The changes in the parameters of P300 cognitive evoked potentials, the psychological and emotional state, and heart rate variability parameters reflecting the characteristics of autonomic regulation during polysensory physiotherapeutic recovery treatment were studied. A single treatment with combined polysensory stimuli resulted in substantial activation of sympathetic regulation of the heart rate, improvement of the emotional state, and an increase in the P300 amplitude in the central frontal areas, which indicates activation of cognitive processes.  相似文献

The localization and the order of triggering of the equivalent dipole sources of the auditory evoked potential (AEP) P300 component were analyzed in healthy subjects performing tasks that required different degrees of concentration. The AEP P300 was isolated and analyzed in 12 subjects aged 18–23 years. The P300 was recorded via 19 channels in mono- and bimodal tasks using two- and three-stimulus methods of the oddball paradigm. Analysis of the equivalent dipole sources of the P300 component upon hearing sounds showed simultaneous activity of dipole sources located in the temporal areas of both hemispheres and in the right frontal area, as well as in the brainstem, irrespective of the task’s complexity. The counting of sounds in the monomodal task was accompanied by consecutive involvement of the left temporal, right temporal, and left frontal sources in the P300 generation; in the bimodal task, these were, respectively, the left parietal, right temporal and hippocampal, and left frontal sources.  相似文献

The responses of the cortical secondary auditory area (AII) to the non-acoustic stimuli (electrical stimulation of the skin in the vibrissae area and light flash) and their combination with acoustic stimulation (sound click or tone) were studied in experiments on cats anesthetized by kalipsol using extra- and intracellular recording. Of the total number of neurons, 69% of the units generating spike responses to the acoustic stimulation responded to the non-acoustic stimulation too. The responses to the modal-nonspecific stimulation, as a rule, were weak and variable; they were mostly represented by a tonic change in the neuronal discharge frequency. The nonspecific stimulation evoked primary excitatory and inhibitory postsynaptic potentials in 77% and 20% of the examined neurons, respectively. We found that synaptic effects of the nonspecific and specific stimulations interact with each other, ensuring considerable modulation of the latter (mostly a significant facilitation resulting from the EPSP summation and suppression of an inhibitory component of the response to acoustic stimulation). Cognitive development of children is influenced by different environmental factors like nutritional and socio‐economic status. The objectives of the present study were to determine the influence of grades of undernutrition and socio‐economic status (SES) on the cognitive development of school children of Kolkata. Five hundred sixty six (566) school children having 5–12 years of age were selected from different schools of Kolkata. The cognitive development was measured by the scores of Raven's colored progressive matrices (RCPM). The chronic and acute nutritional statuses were measured from height‐for‐age (HAZ) and weight‐for‐age (WAZ) Z scores respectively with reference to the values of WHO. SES was determined by updated Kuppuswamy scale. The prevalences of undernutrition in the observed children were 57.95% (according to HAZ) and 52.82% (according to WAZ). The age dependent growth curve of RCPM scores of the observed children remains in between the 10th and 25th centile of British children. The children belonging to superior and intellectual deficit IQ classes were 21.55 and 36.40%, respectively of the total subjects. Most of the subjects belong to lower middle (39.93%) and upper middle (36.40%) class of SES. RCPM scores of school children were gradually decreased with the grades of undernutrition and SES. RCPM scores were significantly correlated with HAZ, WAZ, SES, age, and sex (P < 0.001) and strongly associated with HAZ, SES, age, and sex (P < 0.001, P < 0.05). CEREBRAL evoked potentials can be monitored by averaging scalp records in subjects performing psychological tasks. The waveform is affected not only by the physical features of the sensory stimulus but also by cognitive and motivational parameters1–7. These can be manipulated through verbal instructions given to the subject and several studies have shown that a slow positive component of about 300 ms peak latency (P300) can be elicited by stimuli to which perceptual significance is attached2,5–8. We have examined the evoked potential correlates of sensory overload in forced-paced auditory tasks and find that the cerebral “decision” potential can reveal intermittency in the perceptual channel.  相似文献
