BackgroundThe indications for operative treatment of scapula fractures have been debated over the past decade. Our purpose was to determine 1) the incidence and trends in the operative treatment of scapula fractures, 2) the incidence of conversion from operative fixation to total or hemi-shoulder arthroplasty (THSA) and 3) rates of associated injuries in scapula fractures. We hypothesized that the operative treatment of scapula fractures is increasing over time and that scapula fractures treated with open reduction and internal fixation (ORIF) would have increased risk for conversion to THSA.MethodsThe Humana Inc. administrative claims database was queried from 2008 to 2015. Patients with any scapular fracture, ORIF of scapula fracture, total or hemi-shoulder arthroplasty, and associated injuries were identified by ICD-9 and CPT codes. Analysis was performed for 1) all patients with a scapula fracture undergoing operative fixation (i.e. ORIF and THSA), 2) all scapular fractures treated with ORIF with subsequent conversion to ipsilateral THSA, and 3) all associated injuries.ResultsThere were 10,097 scapula fractures (28.4% glenoid, 48% female). 60% occurred in patients 65 years and older. There were 198 (1.96%) fractures (70% glenoid) treated with ORIF. There were 287 (2.84%) fractures (45% glenoid) treated with THSA (76% total shoulder). The rate of ORIF of scapular fractures did not significantly increase (RR=0.87, p=0.58). There was a significant increase in THSA as primary treatment of scapula fractures in 2015 compared to 2007 (RR=0.43, p=0.0016). Conversion from ORIF to THSA was 12.6% (25/198). Scapula fractures treated with ORIF were at significant risk for conversion to THSA (RR=4.77, p<0.0001). Associated injuries occurred in nearly 50% of scapula fractures—other fractures, lung contusion and pneumothorax/hemothorax ranking the highest, accounting for 37%, 14.5% and 8.3% of all associated injuries, respectively.ConclusionThe incidence of operative treatment of scapula fractures was 1.96% and 2.84% for ORIF and THSA, respectively. Scapular fractures previously treated with ORIF were at significant risk for conversion to THSA. Although ORIF in scapular fractures did not significantly increase over time, both THSA and overall (ORIF+THSA) operative treatment of scapula fractures increased significantly. ObjectiveMost patients do not receive osteoporosis treatment after osteoporotic fracture. This study reviewed osteoporosis treatment after osteoporotic fractures in a center without a Fracture Liaison Service.MethodsWe identified all patients with hip, vertebral, humeral or radial fractures, evaluated in Meir Medical Center, in 2017. The exclusion criteria were not a Clalit Health Services member, high-energy fracture or 30-day postoperative mortality. The primary endpoint was osteoporosis drugs issued within 12 months of fracture. Secondary endpoints included bone densitometry and 1-year mortality.ResultsFive-hundred-eighty-two patients (average age 78.6 ± 11.1 years, 75.8% women) were included. There were 321 (55.5%) hip, 84 (14.1%) humeral, 33 (5.6%) vertebral, and 144 (24.7%) radial fractures. Osteoporosis drugs were issued to 26.5% of the patients; those with humeral fractures received the least (21.4%) and vertebral, the most (30.3%; P = .51). Bone densitometry was performed in 23.2% of patients. One-year mortality after hip fracture was 12.1%, followed by humeral (3.6%; P < .05). Logistic regression showed that previous treatment (odds ratio [OR] = 7.4; 95% confidence interval [CI] 3.6–15.2), bone densitometry (OR = 4.4; 95% CI 2.6–7.4) and endocrinology visit (OR = 2.6; 95% CI, 1.4–4.6) were the most important factors associated with treatment.ConclusionFewer than one third of patients received pharmacotherapy within 1 year after fracture. Because pharmacotherapy reduces future fractures and mortality, we recommend that medical staff who care for patients with fracture adopt practical and effective strategies to increase treatment rates among patients with osteoporotic fractures.  相似文献

Objective:This study aimed to investigate the relationship between the psoas major muscle area as a risk factor and subsequent contralateral hip fractures in patients with initial intertrochanteric fractures.Methods:Of 136 treated for intertrochanteric fractures, 104 female patients had computed tomography done to assess their fractures at initial stage and had been followed up for more than 2 years. These patients were then divided into 2 groups: i.e. those who had a contralateral hip fracture (CF) (n=16) and those who did not (NF) (n=88) groups. We mainly assessed the relationship between the corrected psoas major muscle area (CPMA) at initial fracture and the occurrence of contralateral hip fracture.Results:The CF group had significantly lower CPMA than the NF group (p=0.001). There was positive correlation between the CPMA and the period from the initial to the contralateral hip fracture in the CF group. The CPMA cutoff value of 480.98 mm2/m2, was showed sensitivity of 63.6% and specificity of 87.5% in receiver operating characteristic curve analysis for all patients.Conclusions:The lower CPMA was associated with the contralateral hip fracture within 2 years from initial intertrochanteric fracture. The low CPMA would be a risk factor for contralateral hip fracture.  相似文献

BackgroundOpen reduction and internal fixation (ORIF) of proximal humerus fractures in elderly individuals (age >70) carries a relatively high short-term complication and reoperation rate but is generally durable once healed. Reverse total shoulder arthroplasty (RTSA) for fractures may be associated with superior short-term quality of life but carries the lifelong liabilities of joint replacement. The tradeoff between short and long-term risks, coupled with disparities in quality of life and cost, makes this clinical decision amenable to cost-effectiveness analysis.Methods A Markov state-transition model was constructed with a base case of a 75 year-old patient. Reoperation rates, quality of life values, mortality rates, and costs were based upon published literature. The model was run until all patients had died to simulate the accumulated costs and benefits.ResultsRTSA was associated with greater quality of life (7.11 QALYs) than ORIF (6.22 QALYs). RTSA was cost-effective with an incremental cost-effectiveness ratio of $3,945/QALY and $27,299/ QALY from payor and hospital perspectives, respectively. RTSA was favored and cost-effective at any age above 65 and any Charlson Score. The model was sensitive to the utility of both proceduresConclusionRTSA resulted in a higher quality of life and was cost-effective in comparison to ORIF for elderly patients.Level of Evidence: III  相似文献

Objective To compare the functional results after displaced fractures of the femoral neck treated with internal fixation or hemiarthroplasty.Design Randomised trial with blinding of assessments of functional results.Setting University hospital.Participants 222 patients; 165 (74%) women, mean age 83 years. Inclusion criteria were age above 60, ability to walk before the fracture, and no major hip pathology, regardless of cognitive function.Interventions Closed reduction and two parallel screws (112 patients) and bipolar cemented hemiarthroplasty (110 patients). Follow-up at 4, 12, and 24 months.Main outcome measures Hip function (Harris hip score), health related quality of life (Eq-5d), activities of daily living (Barthel index). In all cases high scores indicate better function.Results Mean Harris hip score in the hemiarthroplasty group was 8.2 points higher (95% confidence interval 2.8 to 13.5 points, P=0.003) at four months and 6.7 points (1.5 to 11.9 points, P=0.01) higher at 12 months. Mean Eq-5d index score at 24 months was 0.13 higher in the hemiarthroplasty group (0.01 to 0.25, P=0.03). The Eq-5d visual analogue scale was 8.7 points higher in the hemiarthroplasty group after 4 months (1.9 to 15.6, P=0.01). After 12 and 24 months the percentage scoring 95 or 100 on the Barthel index was higher in the hemiarthroplasty group (relative risk 0.67, 0.47 to 0.95, P=0.02. and 0.63, 0.42 to 0.94, P=0.02, respectively). Complications occurred in 56 (50%) patients in the internal fixation group and 16 (15%) in the hemiarthroplasty group (3.44, 2.11 to 5.60, P<0.001). In each group 39 patients (35%) died within 24 months (0.98, 0.69 to 1.40, P=0.92)Conclusions Hemiarthroplasty is associated with better functional outcome than internal fixation in treatment of displaced fractures of the femoral neck in elderly patients.Trial registration NCT00464230.  相似文献

BackgroundPostoperative imaging protocols for common hand procedures typically consist of 2-3 plain radiographs at defined intervals dependent on the surgery. Radiographs typically verify reduction, hardware position, and/or evaluate for complications, but also generate costs and alterations in clinic flow. We hypothesize the use of mini-fluoroscopic images will provide comparable clinical data with less cost and improved clinic flow. The objectives of the study were to determine if clinic-based fluoroscopic imaging is feasible for post-operative hand patients and if fluoroscopic imaging results in improved clinic flow (less waiting and more time with provider) and theoretical cost savings using mini-fluoroscopy instead of traditional radiographs.MethodsWith institutional review board exemption, the prospective use of mini-fluoroscopic evaluation of post-operative hand surgery patients was compared to traditional radiographs using time-based cohort analysis. Patients who underwent percutaneous pinning of phalanges/metacarpals, ORIF (open reduction and internal fixation) of phalanges/metacarpals or ORIF of distal radius fractures were included to evaluate common hand surgery patients. Each cohort consisted of a 3-month data collection period with prospective measures of clinic flow (wait times, appointment times, time with provider) and estimated cost compared between the groups.Results24 patients met inclusion criteria for data analysis; 11 patients in the mini-fluoroscopy group and 13 patients in the traditional radiograph group. Appointments using mini-fluoroscopy were 24 minutes shorter (53 vs 77 minutes) from patient check-in to check out time. Check-in time to rooming was 10 minutes quicker (9 vs. 19 minutes) using mini-fluoroscopy. Traditional hand radiographs have a face-value of $734 per appointment/series for a total theoretical savings of $9540 (n=13).ConclusionThe use of mini-fluoroscopy during orthopedic hand surgery post-operative visits provides a feasible, quick, cost effective way to evaluate patients postoperatively, ultimately, resulting in quicker patient appointments and greater time spent with providers. BackgroundPatients with psychiatric comorbidities represent a significant subset of those sustaining pilon fractures. The purpose of this study is to examine the association of psychiatric comorbidities (PC) in patients with pilon fractures and clinical outcomes.MethodsA multi-institution, retrospective review was conducted. Inclusion/exclusion criteria were skeletally mature patients with a tibia pilon fracture (OTA Type 43B/C) who underwent definitive fracture fixation utilizing open reduction internal fixation (ORIF) with a minimum of 24 weeks of follow-up. Patients were stratified into two groups for comparison: PC group and no PC group.ResultsThere were 103 patients with pilon fractures that met the inclusion/exclusion criteria of this study. Of these patients, 22 (21.4%) had at least one psychiatric comorbidity (PC) and 81 (78.6%) did not have psychiatric comorbidities (no PC). There was a higher percentage of female patients (PC: 59.1% vs no PC: 25.9%, p=0.0.005), smokers (PC: 40.9% vs no PC: 16.0%, p=0.02), and drug users (PC: 22.7% vs no PC: 8.6%, p=0.08) amongst PC patients. Fracture comminution (PC: 54.5% vs no PC: 32.1%, p=0.05) occurred more frequently in PC patients. The PC group had a higher incidence of weightbearing noncompliance (22.7% vs 7.5%, p=0.04) and reoperation (PC: 54.5% vs no PC: 29.6%, p=0.03).ConclusionPatients with psychiatric comorbidities represent a significant percentage of pilon fracture patients and appear to be at higher risk for postoperative complication. Risk factors that may predispose patients in the PC group include smoking/substance use, weightbearing noncompliance, and fracture comminution. Level of Evidence: III  相似文献

目的:探讨右美托咪定和丙泊酚对髋部骨折手术患者术后镇静效果及谵妄改善效果的影响。方法:选取2016年4月-2018年3月于我院行髋部骨折手术的108例患者作为研究对象,按随机数字表法分为观察组(n=54)和对照组(n=54),两组患者术中均采用全身静脉麻醉,观察组患者给予右美托咪定进行镇静诱导,对照组患者给予丙泊酚进行镇静诱导。术后24h,采用Ramsay镇静评分评价两组患者术后的镇静效果,采用视觉模拟量表(VAS)评分评价术后镇痛效果,术后1周,对两组患者术后谵妄发生率、谵妄评定量表(CAM)评分、简易智能精神状态检查量表(MMSE)评分进行比较,记录不良反应发生情况。结果:术后24h,观察组患者的Ramsay镇静评分高于对照组,VAS评分及镇痛药追加量低于对照组,差异有统计学意义(P0.05)。术后1周,观察组患者谵妄发生率、CAM评分低于对照组,MMSE评分高于对照组,差异有统计意义(P0.05)。观察组不良反应发生率为9.26%,与对照组的14.81%比较,差异无统计学意义(P0.05)。结论:与丙泊酚相比,髋部骨折手术患者应用右美托咪定可获得更好的术后镇静、镇痛效果,能够降低谵妄的发生率,且无严重不良反应发生,有较高的临床应用价值。  相似文献

ObjectiveTo investigate the vitamin D sufficiency status and the relationships among serum 25-hydroxyvitamin D [25(OH)D] levels, intact parathyroid hormone (iPTH) levels, and bone mineral density (BMD) in patients attending an osteoporosis clinic in Singapore.MethodsIn total, 193 adults with or without prevalent fragility fractures and with low BMD at the femoral neck, total hip, or lumbar spine underwent assessment. Multivariate regression models were used to investigate the relationships among serum 25(OH)D, iPTH, and BMD.ResultsThe mean values (standard deviation) for age of the patients and serum 25(OH)D level were 61 (14) years and 26.05 (7.97) ng/mL, respectively. In 72% of patients, serum 25(OH)D levels were below 30 ng/mL. There was no association between 25(OH)D levels and BMD at the femoral neck, total hip, or lumbar spine(P = .568, .461, and .312, respectively). Serum iPTH levels were negatively associated with BMD at the total hip(P = .035) and the lumbar spine (P = .019). At levels < 30 ng/mL, 25(OH)D was negatively associated with iPTH (P = .036).ConclusionAmong this Southeast Asian population of patients with low BMD, no direct relationship between serum 25(OH)D levels and BMD was observed. A negative correlation existed, however, between iPTH and 25(OH)D at serum 25(OH)D concentrations < 30 ng/mL, and serum iPTH levels showed a significant negative association with BMD at the total hip and lumbar spine. These significant negative associations between iPTH levels and BMD at the total hip and lumbar spine underscore the critical role of this hormone in bone metabolism and health. BackgroundThere is limited literature evaluating the impact of isolated cannabis use on outcomes for patients following spinal surgery. This study sought to compare 90-day complication, 90-day readmission, as well as 2-year revision rates between baseline cannabis users and non-users following thoracolumbar spinal fusion (TLF) for adult spinal deformity (ASD).MethodsThe New York Statewide Planning and Research Cooperative System (SPARCS) database was queried between January 2009 and September 2013 to identify all patients who underwent TLF for ASD. Inclusion criteria were age ≥18 years and either minimum 90-day (for complications and readmissions) or 2-year (for revisions) follow-up surveillance. Cohorts were created and propensity score-matched based on presence or absence of isolated baseline cannabis use. Baseline demographics, hospital-related parameters, 90-day complications and readmissions, and two-year revisions were retrieved. Multivariate binary stepwise logistic regression identified independent outcome predictors.Results704 patients were identified (n=352 each), with comparable age, sex, race, primary insurance, Charlson/Deyo scores, surgical approach, and levels fused between cohorts (all, p>0.05). Cannabis users (versus non-users) incurred lower 90-day overall and medical complication rates (2.4% vs. 4.8%, p=0.013; 2.0% vs. 4.1%, p=0.018). Cohorts had otherwise comparable complication, revision, and readmission rates (p>0.05). Baseline cannabis use was associated with a lower risk of 90-day medical complications (OR=0.47, p=0.005). Isolated baseline cannabis use was not associated with 90-day surgical complications and readmissions, or two-year revisions.ConclusionIsolated baseline cannabis use, in the absence of any other diagnosed substance abuse disorders, was not associated with increased odds of 90-day surgical complications or readmissions or two-year revisions, though its use was associated with reduced odds of 90-day medical complications when compared to non-users undergoing TLF for ASD. ObjectiveTo determine whether patients with fragility hip fractures underwent assessment and treatment of osteoporosis during initial hospitalization or recommendations for such intervention were made to the primary care provider (PCP) at the time of hospital dismissal.MethodsA review of medical records of patients admitted with a low-impact hip fracture to the Royal University Hospital in Saskatoon, Saskatchewan, Canada, was performed to determine whether recommendations were made to evaluate for or treat osteoporosis. In addition, a questionnaire was sent to the orthopedic surgeons practicing at the hospital to help identify barriers to widespread diagnosis and treatment of osteoporosis in such patients.ResultsBetween January and December 2004, 174 patients with fragility hip fractures were admitted to the Royal University Hospital. The mean age of these patients was 82.5 ± 9.8 years. Evaluation for treatment of osteoporosis was recommended in only 9 patients (5%). We found no significant differences in the intervention rates between male and female patients, between patients with and those without a prior history of osteoporosis or fracture, between patients who were previously taking osteoporosis medications and those who were not, and between patients who were seen by a medical consultant and those who were not. Most orthopedic surgeons believed that they were primarily responsible for the surgical care of these patients, and because they did not see these patients in regular follow-up, the management of osteoporosis was considered the responsibility of the PCP.Conclusion: The results of this study indicate that only a small number of patients with fragility hip fractures receive appropriate evaluation or treatment for underlying osteoporosis either during initial hospitalization or at the time of dismissal from the hospital. In this study, most orthopedic surgeons believed that evaluation and treatment of osteoporosis were the responsibility of the PCP. Because these patients have an increased risk for future fractures, barriers to the diagnosis and treatment of osteoporosis need to be removed, and health-care professionals need to be educated about appropriate risk factor modification in these patients. 目的:探讨股骨近端防旋髓内钉、SL-R柄人工股骨头置换术两种术式治疗老年不稳定型骨质疏松性股骨粗隆间骨折的应用效果。方法:选取2015年1月-2017年6月间于河北医科大学附属秦皇岛市第一医院行手术治疗的老年不稳定型骨质疏松性股骨粗隆间骨折患者98例,以数表法将患者随机均分为对照组(n=49)和观察组(n=49)。其中对照组采用股骨近端防旋髓内钉,观察组采用SL-R柄人工股骨头置换术。比较两组患者临床各项指标、手术前后髋关节功能评分以及术后并发症发生情况。结果:观察组手术时间、术中失血量均高于对照组,但住院时间、开始负重时间则低于对照组(P0.05)。观察组术后并发症发生率为4.08%(2/49),低于对照组的18.37%(9/49)(P0.05)。术后1个月、3个月时患者髋关节各项评分相比术前均有升高,术后3个月的各项评分高于术后1个月(P0.05);观察组术后1个月、3个月各项评分均高于对照组(P0.05)。结论:在老年不稳定型骨质疏松性股骨粗隆间骨折患者治疗过程当中,采用SL-R柄人工股骨头置换术可减少住院时间,疗效确切,无严重并发症,且可改善髋关节各项功能评分。  相似文献

目的:研究分析急诊闭合复位和切开复位空心钉治疗股骨颈骨折的疗效。方法:选取2013年6月至2014年6月期间于我院接受治疗的92例急诊股骨颈骨折患者,将所有患者(按随机数字表法)均分为观察组(n=46)和对照组(n=46)。观察组患者采取切开复位空心钉治疗法进行治疗,对照组则采取闭合复位空心钉治疗法进行诊疗。比较两组患者术中术后情况、随访中髋关节功能Harris评分以及相关术后并发症情况。结果:观察组患者所需的手术时间、术中出血量、伤口长度明显多于对照组[(61.43±5.34)min、(30.54±4.27)m L、(6.43±0.56)cm比(46.54±3.26)min、(16.43±2.43)m L、(1.83±0.44)cm](P0.05)。观察组的髋关节功能Harris评分优良率显著高于对照组[82.61%(38/46)比50.00%(23/46)](P0.05)。两组患者的骨折不愈合率比较无显著性差异(P0.05),观察组的股骨头坏死率显著低于对照组[2.17%(1/46)比13.04%(6/46)](P0.05)。结论:应用切开复位空心钉治疗股骨颈骨折患者的方法更佳,其临床治疗效果更好且患者股骨头发生缺血坏死的几率也更低。  相似文献

BackgroundPostoperative delirium is one of the most common postoperative complications in geriatric patients. Mini-mental state examination (MMSE) assesses cognitive function in patients and is associated with postoperative delirium. However, whether there is an age-dependent relationship between preoperative MMSE score and postoperative delirium remains unknown.MethodsWe therefore set out to investigate the association between preoperative MMSE score and postoperative delirium in young-old (≤80 year-old, 75.46±4.69 years, 27.0% male, n = 63) and old-old (>80 year-old, 84.51±3.46 years, 20.9% male, n = 67) participants, who had repairs of hip fractures under general anesthesia. The Confusion Assessment Method and Memorial Delirium Assessment Scale were administrated before surgery, and on the first, second and fourth days after surgery, to assess the incidence and severity of the delirium, respectively. A receiver operating characteristic curve analysis was used to calculate the optimal cutoff score of MMSE in predicting postoperative delirium.ResultsThirty-four (26.2%) of 130 patients (80.12±6.12 years, 23.8% male) developed postoperative delirium. Preoperative MMSE scores were negatively associated with higher incidences and greater severity of postoperative delirium. The optimal cutoff scores of MMSE associated with postoperative delirium for young-old and old-old participants were 18.4 and 21.4, with a sensitivity of 60% and 83.8%, and a specificity of 92.5% and 62.8%, respectively.ConclusionThe data demonstrated the optimal cutoff score of MMSE associated with postoperative delirium in young-old adults might be lower than that in old-old adults. Pending further investigation, these findings suggest that the association between preoperative MMSE score and postoperative delirium is age-dependent.  相似文献

目的:比较切开复位钢板内固定及闭合复位外固定架固定治疗不稳定型桡骨远端骨折的临床疗效,为临床治疗方式的选择提供参考。方法:选择2014年1月至2016年1月间于我院骨科就诊的桡骨远端骨折患者102例作为研究对象。按患者的治疗方式分为内固定组(n=52)和外固定组(n=50)。内固定组患者采用切开复位钢板内固定进行治疗,外固定组患者采用闭合复位外固定架固定进行治疗。术后3个月及术后1年通过影像学测量两组患者的关节面台阶、掌倾角和尺偏角,同时测量患者腕部功能情况。随访1年,比较两组患者疗效,并观察患者并发症发生情况。结果:术后3个月及术后1年,内固定组患者关节面台阶、掌倾角和尺偏角均低于外固定组患者,差异均具有统计学意义(P0.05)。术后3个月,内固定组的旋前旋后活动度、屈伸活动度、桡尺偏活动度、捏力、握力均明显高于外固定组(P0.05),但两组的上述指标在术后1年比较无统计学差异(P0.05)。内固定组患者治疗有效率为86.54%,外固定组患者治疗有效率为86.00%,差异无统计学意义(P0.05)。内固定组并发症发生率为15.38%,外固定组并发症发生率为18.00%,差异无统计学意义(P0.05)。结论:与闭合复位外固定架固定相比,切开复位钢板内固定治疗对患者关节面台阶、掌倾角和尺偏角的恢复效果较好,同时患者能及早进行腕部功能锻炼,近期疗效更加明显。  相似文献

ObjectiveTo investigate the added value of 1/3 radius (1/3R) for the diagnosis of osteoporosis by spine and hip sites and its correlation with prevalent fractures and predicted fracture risk.MethodsFracture Risk Assessment Tool (FRAX) scores for hip and major osteoporotic fractures (MOF) with/without trabecular bone score were considered proxy for fracture risk. The contribution of 1/3R to risk prediction was depicted via linear regression models with FRAX score as the dependent variable—first only with central and then with radius T-score as an additional covariate. Significance of change in the explained variance was compared by F-test.ResultsThe study included 1453 patients, 86% women, aged 66 ± 10 years. A total of 32% (n = 471) were osteoporotic by spine/hip and 8% (n = 115) by radius only, constituting a 24.4% increase in the number of subjects defined as osteoporotic (n = 586, 40%). Prior fracture prevalence was similar among patients with osteoporosis by spine/hip (17.4%) and radius only (19.1%) (P = .77).FRAX prediction by a regression model using spine/hip T-score yielded explained variance of 51.8% and 49.9% for MOF and 39.8% and 36.4% for hip (with/without trabecular bone score adjustment, respectively). The contribution of 1/3R was statistically significant (P < .001) and slightly increased the explained variance to 52.3% and 50.4% for MOF and 40.9% and 37.4% for hip, respectively.ConclusionReclassification of BMD results according to radius measurements results in higher diagnostic output. Prior fractures were equally prevalent among patients with radius-only and classic-site osteoporosis. FRAX tool performance slightly improved by incorporating radius BMD. Whether this approach may lead to a better fracture prediction warrants further prospective evaluation.  相似文献

Background and objectiveDuring the COVID-19 pandemic, a strict population confinement was decreed in Spain. In a situation of health crisis and health system saturation, knowledge of pathologies whose treatment cannot be delayed is of great importance for the correct management of resources and processes. The objective was to determine the influence of the COVID-19 pandemic and the strict population confinement on hip fracture incidence in Spain.Material and methodSystematic review following the PRISMA guidelines. Search in Pubmed and Chocrane Library on 11/11/2022: hip fracture and COVID-19 and Spain.ResultsSix studies met the inclusion criteria, 50% showed a decrease in the hip fractures incidence during strict population confinement (all in Comunidad de Madrid hospitals), and in the other 50% there were no differences (all in other region hospitals). It was constant that hip fractures, either did not decrease, or decreased much less than the rest of fractures during the period of strict population confinement, increasing their relative number with respect to the total admissions in orthopedic surgery and traumatology.ConclusionsThe behavior of the hip fractures number during the strict population confinement decreed by the COVID-19 pandemic has not been symmetrical in all regions. To design and implement contingency plans in the event of a pandemic situation, these data must be taken into account in order to properly plan material and human resources.  相似文献

目的:探讨不同剂量利伐沙班对老年髋关节置换术患者凝血功能及下肢深静脉血栓(DVT)的影响。方法:选择2013年6月~2016年6月期间我院收治的200例老年髋关节置换术患者,按随机数字表法分为两组各100例,术后分别予利伐沙班5 mg、10mg口服以预防DVT。术前及术后1d、7d检测纤维蛋白原(Fbg)、凝血酶原时间(PT)、凝血酶时间(TT)、活化部分凝血酶原时间(APTT)及D-二聚体(D-D),同时行下肢彩色多普勒超声检查判断DVT发生情况,观察临床出血事件。结果:术后1d两组Fbg水平明显降低,PT明显延长(均P0.05);术后7d两组Fbg水平与术后1d相比明显回升,PT明显缩短(均P0.05)。两组各时期的Fbg与PT均无明显差异(P0.05)。术后1d两组D-D水平均明显升高(P0.05),且利伐沙班5 mg组明显高于利伐沙班10 mg组(P0.05);术后7d两组D-D水平均较术后1d明显降低(P0.05),且利伐沙班10 mg组明显低于利伐沙班5 mg组(P0.05)。利伐沙班10 mg组术后早期DVT发生率明显低于利伐沙班5 mg组(P0.05)。两组预防性抗凝后临床出血事件的发生率比较无统计学差异(P0.05)。结论:老年髋关节置换术后口服大剂量(10 mg)利伐沙班,可有效改善血液高凝状态,降低DVT发生率,且不会明显增加临床出血事件,值得推广。  相似文献
