Evidence of smolting was studied in Danish hatchery-reared brown trout Salmo trutta L. Twenty-four hour seawater (SW) challenge tests (28‰, 10°C) at regular intervals showed that maximal hypo-osmoregulatory ability developed within a 3–4-week period in March and April. The improved ability to regulate plasma osmolality, muscle water content and plasma total [Mg] developed asynchronously, indicating that developmental changes in the gill, the gastrointestinal system and the kidney may not necessarily concur during smolting. Gill Na+, K+-ATPase activity peaked in April at the time of optimal hypo-osmoregulatory ability. Na+, K+-ATPase a -subunit mRNA level in gills was unchanged from January until April, but decreased in May in parallel with a decrease in the activity of the enzyme. In the middle region of the intestine, Na+, K+-ATPase activity increased in February and remained high until April. In the posterior region of the intestine, the activity was stable from January until April after which it decreased. In vitro fluid transport capacitity, Jv, in the middle intestine fluctuated throughout the spring. In the posterior intestine, Jv was low until late March, when it increased fivefold until early May. Drinking rate in fish transferred to SW for 24 h surged during spring. Na+, K+-ATPase activity in the pyloric caeca was elevated from March until May, and increased in response to SW transfer in June, suggesting a hypo-osmoregulatory function of the pyloric caeca. Plasma GH levels surged in FW trout during spring, concurring with the increase in gill Na+, K+-ATPase activity and SW tolerance, but peaked in May when gill Na+, K+-ATPase activity and SW tolerance were regressing. GH levels were generally low in SW-challenged fish, and there was no consistent effect of 24-h SW exposure on GH levels. In wild anadromous trout, gill Na+, K+-ATPase activity varied seasonally as in hatchery-reared fish, but peaked at higher levels suggesting a more intense smolting in fish living in their natural environment.  相似文献

With an emphasis on the tight junction protein occludin, the response of goldfish following abrupt exposure (0–120 h) as well as long-term acclimation (14 and 28 days) to ion-poor water (IPW) was examined. Both abrupt and long-term exposure to IPW lowered serum osmolality, [Na+] and [Cl], and elevated serum glucose. After abrupt exposure to IPW, gill tissue exhibited a prompt and sustained decrease in Na+–K+–ATPase activity, and a transient increase in occludin expression that returned to control levels by 6 h. Following 14 and 28 days in IPW, gill occludin expression was markedly elevated, while Na+–K+–ATPase activity was only significantly different (elevated) at day 14. Kidney tissue exhibited an elevation in both Na+–K+–ATPase activity and occludin expression after 28 days; however, in the intestine, occludin expression declined at day 14 but did not differ from FW fish at day 28. These studies demonstrate that goldfish can tolerate abrupt as well as sustained exposure to ion-poor surroundings. Data also suggests that occludin may play an adaptive role in fishes acclimated to ion-poor conditions by contributing to the modulation of epithelial barrier properties in ionoregulatory tissues. Rainbow trout Oncorhynchus mykiss were exposed to 0, 100, 300 and 800 μgl−1 ambient Cu in brackish water (BW) for 4 days at 13 °C and subsequently transferred to either clean BW, clean fresh water (FW) or clean seawater (SW) at 16 °C. After incubation with 32P-phosphate and 14C-acetate added as precursors to the water the fish showed a degradation, depending on previous [CU], of 32P- and 14C-labelled gill membrane phospholipids if they had been transferred to SW or remained in BW. Corresponding experiments where the fish were exposed to Cu in BW for 12 days showed a similar subsequent degradation in SW and BW of both gill and esophagus membrane phospholipids, however to a much lesser degree in gill tissue than after 4 days. Plasma Na+ was similarly reduced by up to 8%, depending on previous ambient Cu, but in this case only after transfer to FW and only after 4 days of exposure. Both the effect on membrane lipid metabolism and plasma Na+ thus showed acclimation to ambient Cu but there was apparently no direct correlation between the two different types of observed changes in membrane function.  相似文献

We assessed the effects of dietary fatty acid composition on sodium–potassium ATPase (Na+/K+-ATPase) activity and isoform expression in the gills of juvenile fall chinook salmon, Oncorhynchus tshawytscha by supplementing diets with either anchovy oil (AO) or AO blended with canola oil (CO) so that CO comprised 0% (0CO), 11% (11CO), 22% (22CO), 33% (33CO), 43% (43CO), or 54% (54CO) of the measured dietary lipid content. The effects of diet were assessed in freshwater (FW) following 104 days of diet manipulation, in response to 24-h seawater (SW) transfer at this time, and following an additional 35 days of SW acclimation. Gill Na+/K+-ATPase activity was not significantly affected by diet at any sampling time, and there were no consistent effects of diet on the expression of the Na+/K+-ATPase α1a isoform. As dietary CO increased, Na+/K+-ATPase α1b mRNA decreased in fish held in FW, with the 43CO and 54CO diet groups having significantly lower levels than fish fed the 0CO and 11CO diets. Twenty-four-hour SW challenge did not affect the expression of the Na+/K+-ATPase α1a isoform in any diet group, but this isoform was down-regulated in all diet groups following 35 days of SW acclimation. Na+/K+-ATPase α1b expression levels increased in response to 24-h SW transfer and SW acclimation only in fish fed the 54CO diet. The effects of the two extreme diets (0CO and 54CO) were also assessed at various time points during 104 days of rearing in FW. Na+/K+-ATPase α1b mRNA levels were greater in fish fed diet 0CO versus those fed diet 54CO at all times during the FW culture period. These data demonstrate that dietary fatty acid composition can influence the gill Na+/K+-ATPase isoform physiology of juvenile fall-run chinook salmon prior to SW transfer.  相似文献

Salinity tolerance in wild (Glendale) and hatchery (Quinsam) pink salmon Oncorhynchus gorbuscha (average mass 0·2 g) was assessed by measuring whole body [Na+] and [Cl?] after 24 or 72 h exposures to fresh water (FW) and 33, 66 or 100% sea water (SW). Gill Na+, K+‐ATPase activity was measured following exposure to FW and 100% SW and increased significantly in both populations after a 24 h exposure to 100% SW. Whole body [Na+] and whole body [Cl?] increased significantly in both populations after 24 h in 33, 66 and 100% SW, where whole body [Cl?] differed significantly between Quinsam and Glendale populations. Extending the seawater exposure to 72 h resulted in no further increases in whole body [Na+] and whole body [Cl?] at any salinity, but there was more variability among the responses of the two populations. Per cent whole body water (c. 81%) was maintained in all groups of fish regardless of salinity exposure or population, indicating that the increase in whole body ion levels may have been related to maintaining water balance as no mortality was observed in this study. Thus, both wild and hatchery juvenile O. gorbuscha tolerated abrupt salinity changes, which triggered an increase in gill Na+, K+‐ATPase within 24 h. These results are discussed in terms of the preparedness of emerging O. gorbuscha for the marine phase of their life cycle.  相似文献

The influence of acclimation to different environmental salinities (low salinity water, LSW; seawater, SW; and hyper saline water, HSW) and feeding conditions (fed and food deprived) for 14 days was assessed on osmoregulation and energy metabolism of several tissues of gilthead sea bream Sparus auratus. Fish were randomly assigned to one of six treatments: fed fish in LSW, SW, and HSW, and food-deprived fish in LSW, SW, and HSW. After 14 days, plasma, liver, gills, kidney and brain were taken for the assessment of plasma osmolality, plasma cortisol, metabolites and the activity of several enzymes involved in energy metabolism. Food deprivation abolished or attenuated the increase in gill Na+,K+-ATPase activity observed in LSW- and HSW-acclimated fish, respectively. In addition, a linear relationship between renal Na+,K+-ATPase activity and environmental salinity was observed after food deprivation, but values decreased with respect to fed fish. Food-deprived fish acclimated to extreme salinities increased production of glucose through hepatic gluconeogenesis, and the glucose produced was apparently exported to other tissues and served to sustain plasma glucose levels. Salinity acclimation to extreme salinities enhanced activity of osmoregulatory organs, which is probably sustained by higher glucose use in fed fish but by increased use of other fuels, such as lactate and amino acids in food-deprived fish.  相似文献

Chalcalburnus tarichi is an anadromous cyprinid fish that has adapted to extreme conditions (salinity 22 ‰, pH 9.8 and alkalinity 153 mEq × l?1) in Lake Van in eastern Turkey. Changes in immunoreactivity of Na+/K+-ATPase in gill tissue and osmolarity and ion levels in plasma were investigated in C. tarichi during reproductive migration. Physicochemical characteristics and ion levels in Lake Van were high compared freshwater. Plasma osmolality and plasma ion concentrations ([Na+], [K+] and [Cl?]) increased after transfer from freshwater to Lake Van. The mitochondria-rich (MR) cells of the gill were stained in both filament and lamellar epithelia of C. tarichi by immunocytochemistry with a specific antiserum for Na+/K+-ATPase in river fish samples. Density and area of MR cells were decreased in lake-adapted fishes. These results indicated that freshwater acclimation capacity is correlated with the size and distribution of MR cells in C. tarichi, in contrast to many teleost fishes.  相似文献

Human umbilical CD34+ immature haematopoietic cells were rapidly and efficiently obtained from light density MNC (mononuclear cells) by MACS (magnetic cell sorting). An ex vivo expanded population of CD34+ was cultured in serum‐free medium supplemented with cytokines FL (flt3 ligand), SCF (stem cell factor) and TPO (thrombopoietin) in order to obtain a sufficient number of CD34+ cells. CD34+ cells expanded from cord blood for 7 days were demonstrated to increase in the absolute number of CD34+ cells by 5.12±2.47‐fold (mean±S.D., n=3). Flow cytometric analysis demonstrated that the percentage of CD34 antigen expression after expansion of the culture was 97.81±1.07%, whereas it was 69.39±10.37% in none‐expanded CD34+ cells (mean±S.D., n=3), thus defining a system that allowed extensive amplification accompanied by no maturation. MTs (metallothioneins), low molecular weight, cysteine‐rich metal‐binding proteins, exhibit various functions, including metal detoxification and homoeostasis. We here examined the expression pattern of functional members of the MT gene family in immature CD34+ cells and compared it with more mature CD34? cells in order to strengthen the proposed function of MT in differentiation. Cells were cultured in RPMI 1640 medium, with or without different zinc supplements for 24 h. Relative quantitative expression of MT isogenes in the mature CD34? cells was higher than in the immature CD34+ cells. IHC (immunohistochemical staining) revealed an increased MT protein biosynthesis in CD34? cells, greater than in CD34+ cells. Therefore, the role of MT in differentiation of human haematopoietic progenitor cells from human cord blood is reported for the first time.  相似文献

Summary
  • 1 About 25 % of juvenile Atlantic salmon (Salmo salar) migrating downstream in the River Frome in southern England do so in the autumn rather than in the spring. Here, we examine the physiological status of these fish with regard to those features that adapt them to sea water during the parr–smolt transformation (i.e. gill Na+K+ ATPase activity; the number, size and type of chloride cells on the gill lamellae; salinity tolerance and relative plasma thyroid levels).
  • 2 Autumn migrants, and those fish which subsequently reside in the tidal reaches during the winter, are not sufficiently physiologically adapted to permit permanent or early, entry into the marine environment.
  • 3 It is not known what proportion of autumn migrating fish survive and return to spawn as adults. If significant numbers do return, however, the production from tidal reach habitats must be taken into account in the development of salmon stock management strategies, especially monitoring and assessment programmes, and in the evaluation of factors affecting stocks.

Summary Daily intramuscular injection of cortisol (4 mg kg–1 body weight) in rainbow trout,Salmo gairdneri, for 10 days caused significant increases in the number and individual apical surface area of gill chloride cells per mm2 of filament epithelium. Concomitantly, whole body influxes of sodium (Na+) and chloride (Cl) increased. Acute (3 h) intra-arterial infusion of cortisol did not affect whole body Na+ or Cl influx. A significant correlation was observed between both Na+ and Cl influxes and the fractional apical surface area of filament chloride cells in control, sham (saline-injected) and experimental (cortisol-injected) fish. The chloride cells displayed similar ultrastructural modifications in trout undergoing cortisol treatment as in trout transferred to ion-deficient water. These findings suggest the existence of structure/function relationships in which branchial chloride cell morphology is an important determinant of Na+ and Cl transport capacity. We conclude that chronic cortisol treatment enhances whole body Na+ and Cl influxes by promoting proliferation of branchial chloride cells. The results of correlation analysis indicate that the chloride cell is an important site of NaCl uptake in freshwater rainbow trout.  相似文献

CD8+ T cells play a central role in antitumour immunity, which often exhibit ‘exhaustion’ in the setting of malignancy and chronic viral infection due to T cell immunoglobulin and mucin domain 3 (TIM3) and myeloid‐derived suppressor cells (MDSCs). Our team previously found that overactive MDSCs and exhausted TIM3+CD8+ T cells were observed in myelodysplastic syndromes (MDS) patients. However, it is not obvious whether MDSCs suppress CD8+ T cells through TIM3/Gal‐9 pathway. Here, Gal‐9, as the ligand of TIM3, was overexpressed in MDSCs. The levels of Gal‐9 in bone marrow supernatants, serum and culture supernatants of MDSCs from MDS patients were elevated. CD8+ T cells from MDS or normal controls produced less perforin and granzyme B and exhibited increased early apoptosis after co‐culture with MDSCs from MDS. Meanwhile, the cytokines produced by CD8+ T cells could be partially restored by TIM3/Gal‐9 pathway inhibitors. Furthermore, CD8+ T cells produced less perforin and granzyme B after co‐culture with excess exogenous Gal‐9, and the function of CD8+ T cells was similarly restored by TIM3/Gal‐9 pathway inhibitors. Expression of Notch1, EOMES (associated with perforin and granzyme B secretion), p‐mTOR and p‐AKT (associated with cell proliferation) was decreased in CD8+ T cells from MDS after co‐culture with excess exogenous Gal‐9. These suggested that MDSCs might be the donor of Gal‐9, and TIM3/Gal‐9 pathway might be involved in CD8+ T cells exhaustion in MDS, and that TIM3/Gal‐9 pathway inhibitor might be the promising candidate for target therapy of MDS in the future.  相似文献

Summary Carbonic anhydrase (CA) activities in gills and venous blood, acid-base balance, and haematological variables were studied during environmental hypercapnia in rainbow trout (Salmo gairdneri). Batches of 8–10 fish were exposed to about 3 or 13 mmHg in flow-through tests of various duration from 4 h to 80 days.After initial acidosis, blood pH rose above pre-experimental values. At 3 mmHg it became normal again within 21 days, while at 13 mmHg the overshoot lasted for 80 days. In fish acclimated for 3 weeks or more to 13 mmHg , blood HCO 3 increased four to five times while plasma Cl levels were lower and K+ higher. Na+ levels did not show any consistent trend associated with exposure to hypercapnia. After an initial acidaemia, Hct, Hb, and RBC remained relatively constant.Patterns of change in CA activity differed between gills and erythrocytes. Initially, blood CA decreased at both levels. It then began rising after about 3 weeks and tended to reach pre-experimental values by 80 day's hypercapnia. At 13 mmHg , gill CA increased to twice the pre-experimental level. Compared with blood CA, gill CA appeared to be more specifically involved in fish acclimation to hypercapnia, which demands an increase in blood bicarbonate to provide a sufficient buffering capacity. Low-salinity adaptability was investigated in a flatfish spotted halibut Verasper variegatus during the period from late metamorphic larvae to early juveniles by a 20-day rearing experiment under different salinity regimes (1, 4, 8, 16 and 32 ppt). Effects of low-salinity on growth and development were examined and the changes in the prolactin (PRL) production level in the pituitary and the gill chloride cell morphology were examined as physiological backgrounds for low salinity adaptation. PRL cells and chloride cells were identified by immunocytochemistry with a specific antiserum for PRL188 and Na+,K+-ATPase. Most of the fish exposed to over 4 ppt survived for 20 days, but all the fish exposed to 1 ppt died within 5 days. Fish kept in intermediate salinities (8, 16 ppt) grew significantly better than those in the control group (32 ppt). Fish exposed to 4 ppt attained almost the same body length as the control group at 20 days after transfer, although these fish showed an abnormally dark body color as well as delayed development. These results suggested that spotted halibut has a high-adaptability to low-salinity environments and prefers an intermediate salinity near iso-osmolality (about 12 ppt) from the late metamorphic larval stage, but does not completely adapt to a hypoosmotic of 4 ppt salinity or less than half of the osmolality. The percentage of PRL-cell volume to pituitary volume was significantly higher at 4 ppt than in the control group. The chloride cells in gill filaments were significantly larger at 4 ppt than in the control group. These results suggest that juveniles could adapt to a low-salinity environment due to the activation of PRL production and enlargement of chloride cells. These laboratory findings suggest that late metamorphic larvae and early juveniles of spotted halibut may utilize a low salinity environment such as estuarine tidal flats or very shallow coastal areas as their nursery grounds in the sea.  相似文献

To examine osmotic regulation during long-term acclimation to a hyperosmotic medium, hemolymph osmolality, [Na+] and total protein, tissue hydration, and free amino acid (FAA) pools in abdominal muscle, gills, central nervous tissue and hemolymph were quantified in the diadromous freshwater (FW) shrimp, Macrobrachium olfersii, during direct exposure to 21‰S seawater over a 20-day period. Hemolymph osmolality and [Na+] reach stable maxima within 24?h while total protein is unchanged. Muscle and nerve tissues rapidly lose water while gills hydrate; all tissues attain maximum hydration (+5%) by 5 days, declining to FW values except for gills. Total FAA are highest in muscle, reach a maximum by 2 days (+64%), declining to FW values. Gill FAA increase by 110% after 24?h, diminishing to FW values. Nerve FAA increase 187% within 24?h, and remain elevated. Hemolymph FAA decrease (?75%) after 24?h, stabilizing well below the FW concentration. During acclimation, muscle glycine (+247%), gill taurine (+253%) and proline (+150%), and nerve proline (+426%), glycine (+415%) and alanine (+139%) increase, while hemolymph leucine (?70%) decreases. Total FAA pools contribute 10–20% to intracellular (22–70?mmol/kg) and 0.5–2.4% to hemolymph (3–7?mOsm/kg) osmolalities during direct acclimation from FW. These data emphasize the modest participation of FAA pools in intracellular osmotic regulation during physiological adaptation by M. olfersii to osmotic challenge, accentuating the role of anisosmotic extracellular regulation, suggesting that, during the invasion of freshwater by the Crustacea, dependence on intracellular adjustment employing FAA as osmotic effectors, has become progressively reduced.  相似文献

The metabolic aspects of ionic and osmotic regulation in fish are not well understood. The objective of this study was to examine changes in carbohydrate metabolism during seawater (SW) acclimation in the euryhaline tilapia (Oreochromis mossambicus). Hepatic activities of three key enzymes of the intermediary metabolism, phosphofructokinase, glycogen phosphorylase and glucose 6-phosphate dehydrogenase, together with glycogen content and plasma glucose concentration were measured at 0, 0.5, 1, 2, 3, 6, 12, 24, 48 and 96 h after the direct transfer of tilapia from fresh water (FW) to 70% SW. Plasma growth hormone, prolactin177 and prolactin188, Na+ and Cl concentrations were also measured. Plasma Na+ and Cl levels were highest at 12 h, but returned to FW levels at 24 h after transfer, suggesting the tilapia were able to osmoregulate within 24 h after transfer. Plasma glucose levels were significantly higher in 70% SW than in FW during the course of acclimation, especially in the early stages. Hepatic enzyme activities and glycogen content did not change significantly during the acclimation period. Our results suggest the possibility that glucose is an important energy source for osmoregulation during the acclimation to hyperosmotic environments in O. mossambicus.  相似文献

The biological response after exposure to a high‐strength static magnetic field (SMF) has recently been widely discussed from the perspective of possible health benefits as well as potential adverse effects. To clarify this issue, CD34+ cells from human placental and umbilical cord blood were exposed under conditions of high‐strength SMF in vitro. The high‐strength SMF exposure system was comprised of a magnetic field generator with a helium‐free superconducting magnet with built‐in CO2 incubator. Freshly prepared CD34+ cells were exposed to a 5 tesla (T) SMF with the strongest magnetic field gradient (41.7 T/m) or a 10 T SMF without magnetic field gradient for 4 or 16 h. In the harvested cells after exposure to 10 T SMF for 16 h, a significant increase of hematopoietic progenitors in the total burst‐forming unit erythroid‐ and megakaryocytic progenitor cells‐derived colony formation was observed, thus producing 1.72‐ and 1.77‐fold higher than the control, respectively. Furthermore, early hematopoiesis‐related and cell cycle‐related genes were found to be significantly up‐regulated by exposure to SMF. These results suggest that the 10 T SMF exposure may change gene expressions and result in the specific enhancement of megakaryocytic/erythroid progenitor (MEP) differentiation from pluripotent hematopoietic stem cells and/or the proliferation of bipotent MEP. Climate change affects organisms that inhabit not only in aerial but also in aquatic environments by making water more hypoxic and acidic. In the past, we evaluated morphological and functional variations in the gills of 12 species of aquatic air-breathing fishes. The aim of the present study is to examine the degree of gill modification in the aquatic air-breathing fish, Trichogaster lalius, in response to acidic stress. This provides a link between the ecological and physiological studies. We evaluated the changes in morphology and function of the gills, labyrinth organ, and kidney when the fish were subjected to acidic water and deionized water (DW). In the first experiment, fish were sampled at 1, 2, 4, and 7 days after acidic treatment. Apparent morphological modification was observed on day 4 and recovery was noted on day 7. Protein expression and enzyme activity of vacuolar-type H+-ATPase (VHA) and the protein expression of the proliferating cell nuclear antigen (PCNA) of the 1st and 4th gill arches both increased in the 4-day and 7-day acidic groups while the enzyme activity of Na+/K+-ATPase (NKA) decreased. In the second experiment, fish were tested for changes in the 1st and 4th gill arches and kidney after exposure to DW and acidic water for 4 days. The gill structure of the fish in the DW was not different from that of the control group (fresh water). The protein expression and enzyme activity of the VHA of the 1st and 4th gill arches increased in both the DW and acidic groups for 4 days. We found a decrease in the protein expression of NKA in the kidney and in the enzyme activity of NKA in the 1st and 4th gill arches in the DW and acidic groups. From these results, we suggest that T. lalius exhibited significantly different ionic regulation and acid-base regulatory abilities in the DW and acidic groups in the 1st and 4th gill arches and kidney. The responses of the gills in T. lalius were different from those fish that show apparent morphological variations between the 1st and 4th gill arches.  相似文献

Because the ghrelinergic system in teleost fishes is broadly expressed in organs that regulate appetite as well as those that contribute to the regulation of salt and water balance, we hypothesized that manipulating salt and water balance in goldfish (Carassius auratus) would modulate the ghrelinergic system. Goldfish were acclimated to either freshwater (FW) or ion-poor FW (IPW) and were fed either a control diet containing 1% NaCl or low-salt diet containing 0.1% NaCl. Endpoints of salt and water balance, i.e., serum Na+ and Cl levels, muscle moisture content and organ-specific Na+-K+-ATPase (NKA) activity, were examined in conjunction with brain, gill and gut mRNA abundance of preproghrelin and its receptor, growth hormone secretagogue receptor (ghs-r). Acclimation of fish to IPW reduced serum osmolality and Cl levels and elevated kidney NKA activity, while FW fish fed a low NaCl diet exhibited a modest reduction in muscle moisture content but otherwise no apparent osmoregulatory disturbance. In contrast, a combined treatment of IPW acclimation and low dietary NaCl content reduced serum osmolality and Cl levels, elevated muscle moisture content and increased gill, kidney and intestinal NKA activity. This intensified response to the combined effects of water and dietary ion deprivation is consistent with an increased effort to enhance ion acquisition. In association with these latter observations, a significant upregulation of preproghrelin mRNA expression in brain and gut was observed. A significant increase in ghs-r mRNAs was also observed in the gill of goldfish acclimated to IPW alone but a reduction in dietary NaCl content did not impact the ghrelinergic system of goldfish in FW. Background

We evaluated maternal CD4+ cell count (CD4+) decline after PMTCT prophylaxis in a multi-country HIV care program.

Methods

Analysis was restricted to antiretroviral therapy (ART)-naive, HIV-infected pregnant women with CD4+ ≥250 cells/mm3 at enrollment. Single-dose nevirapine (sd-NVP) or short-course antiretroviral prophylaxis (sc-ARVp) with zidovudine (AZT) or AZT + lamivudine (3TC) was initiated in 11 programs while 2 programs offered triple-drug antiretroviral prophylaxis (tARVp) (AZT+3TC+ NVP or nelfinavir). All regimens were stopped at delivery. CD4+ decline was defined as proportion of women who declined to CD4+ <350 cells/mm3 or <200 cells/mm3 at 24 months. Weibull regression was used for multivariable analysis.

Findings

A total of 1,393 women with enrollment CD4+ ≥250 cells/mm3 initiated tARVp (172; 12%) or sc-ARVp (532; 38%) during pregnancy or received intrapartum sd-NVP (689; 50%). At enrollment, maternal median age was 27 years (interquartile range (IQR) 23–30), median CD4+ was 469 cells/mm3 (IQR: 363–613). At 24 months post-delivery, the cumulative probability of CD4+ decline to <200 cells/mm3 was 12% (95% CI: 10–14). Among a subgroup of 903 women with CD4+ ≥400 cells at enrollment, the 24 month cumulative probability of decline to CD4+ <350 cells/mm3 was 28%; (95% CI: 25–32). Lower antepartum CD4+ was associated with higher probability of CD4+ decline to <350 cells/mm3: 46% (CD4+400–499 cells/mm3) vs. 19% (CD4+ ≥500 cells/mm3). After adjusting for age, enrollment CD4+ and WHO stage, women who received tARVp or sd-NVP were twice as likely to experience CD4+ decline to <350 cells/mm3 within 24 months than women receiving sc-ARVp (adjusted hazard ratio: 2.2; 95% CI: 1.5–3.2, p<0.0001).

Conclusion

Decline in CD4+ cell count to ART eligibility thresholds by 24 months postpartum was common among women receiving PMTCT prophylaxis during pregnancy and/or delivery.  相似文献
