目的:评价机用ProTaper镍钛根管器械进行恒磨牙根管预备的临床疗效。方法:2007年7月至2010年12月期间就诊的需进行根管治疗的109患者,共计128颗磨牙,包括牙髓炎54颗牙,慢性根尖周炎74颗牙。按随机配对原则分入实验组和对照组,每组64颗牙。实验组用机用ProTaper镍钛根管锉预备根管,对照组用不锈钢K型锉预备根管。两组均采用侧向加压法充填根管,记录每颗患牙的单个根管预备所用时间、根管充填效果、术后不良反应及随访到的远期疗效。结果:机用ProTaper镍钛器械预备单根管所耗时间明显少于对照组,根管预备后疼痛发生率明显低于对照组,根管适充率实验组明显优于对照组,根管治疗术后1年疗效比较也优于对照组。结论:使用机用ProTaper器械进行恒磨牙根管预备快速有效,根充效果好,并发症发生率低,远期疗效确切,值得临床推广。  相似文献

目的:寻找一种预备C形根管的有效器械,提高下颌第二恒磨牙的治愈率。方法:选用80颗具有C形根管的下颌第二恒磨牙,选择四种器械进行根管预备,第一组用不锈钢K锉,第二组用不锈钢K锉和手用镍钛器械,第三组用机用镍钛器械,第四组用超声根管治疗仪,预备中均用3%双氧水和生理盐水交替反复冲洗根管,预备后用Vitapex糊剂和牙胶尖冷侧压方法充填根管,观察根充后X线片和根尖微渗漏情况,评价充填效果。结果:四种器械根充后微渗漏有明显差异(P<0.05)。结论:不锈钢K锉和手用镍钛器械联合应用及超声根管治疗仪为预备C形根管的有效器械。  相似文献

目的:比较新型旋转器械马尼GPR和ProTaper再治疗系统、手用H锉在根管再治疗中去除弯曲根管内牙胶的清理效果。方法:选取30个弯曲度为30°的透明树脂根管模块,根管长度为17mm。使用ProTaper Next机用镍钛器械预备至30/.06,热压胶垂直加压根管充填。样本随机分为3组(n=10),用以下方法配合丁克除溶剂去除根管内充填材料,A组手用H锉、B组ProTaper再治疗系统和C组马尼GPR,记录去除充填物所用总时间。从颊舌向和近远中向2个角度拍着数码X线片,使用Image J 2X图像分析软件分析根管内充填物残留量。用天平称量推出根尖孔碎屑量。结果:ProTaper再治疗系统组根管壁充填物残留量明显多于马尼GPR组和H锉组(P0.05)。H锉组操作时间明显高于ProTaper再治疗系统组和马尼GPR组(P0.05)。马尼GPR组推出根尖孔碎屑量明显少于ProTaper再治疗系统组(P0.05)。结论:马尼再治疗锉去除根管内充填物效率优于ProTaper再治疗系统和H锉,机用镍钛器械所用时间明显少于手用器械。  相似文献

Er,Cr:YSGG激光照射对牙根尖封闭效果影响的实验研究   总被引:3,自引:0,他引:3  
目的:深入探讨在根尖倒充填术中Er,Cr:YSGG激光照射对牙根尖的封闭效果,为此激光在口腔临床上的应用与推广提供重要的理论与实验依据。方法:将56颗新拔除的单根牙随机分成7组,每组8颗牙。去除牙冠,常规进行根管预备、根管充填。将倒充填的牙根尖进行预备,然后银汞合金倒充填。预备方法:1手机切除根尖后,制备成烧瓶状洞形;2手机处理方法同1,然后Er,Cr:YSGG激光于不同能量(1W、2W)及不同方式(喷水或无水)下对洞内壁进行照射。结合微渗漏检测与扫描电镜观察对Er,Cr:YSGG激光照射后的牙根尖封闭效果进行评估。结果:Er,Cr:YSGG激光可有效去除根切面上的玷污层及碎屑;与手机加银汞倒充填组相比,激光照射后的根尖染料微渗漏深度有所减少;但只有1WEr,Cr:YSGG激光于无水条件下照射时,根尖染料微渗漏深度显著减少(P<0.05)。结论:Er,Cr:YSGG激光应用于根尖倒充填术中可减少微渗漏的发生;促进根尖封闭;在特定的能量参数下可获得优于常规银汞倒充填术的治疗效果。因此我们推荐临床上将Er,Cr:YSGG激光照射与常规银汞倒充填术结合使用,这将有助于提高根尖倒充填术治疗的成功率。  相似文献

目的:比较热牙胶充填与冷牙胶侧方加压在根管充填中应用的临床效果。方法:选择62例急性牙髓炎和根尖周炎患者,均分后分别选择热牙胶充填与冷牙胶侧方加压的方法进行根管充填,比较两组患者根管充填时间、术后牙周疼痛病例和术后3个月后充填质量及临床疗效。结果:两组患者术后牙周疼痛病例比较,差异无统计学意义(P>0.05);其余各方面比较,差异均均有统计学意义(P>0.05),热牙胶组优于冷牙胶组。结论:临床进行根管充填治疗时,应首选热牙胶充填的方式,可显著提高临床疗效,且不增加患者术后牙周疼痛的发生。  相似文献

探讨常规根管预备后使用不同能量参数Er,C r:YSGG激光进行根管内照射对根管封闭效果的影响,为此激光在根管治疗领域的临床应用提供重要的实验依据。选择新拔除的单根管牙80颗,分成4组,每组20颗牙。常规根管预备后,分别使用1 W、2 W、3 W Er,C r:YSGG激光于有水条件下进行根管内照射,另选一组不做任何处理,作为阴性对照。扫描电子显微镜下观察根管内壁形态学改变后,常规根管充填,检测根管微渗漏情况。采用单因素方差分析及q检验进行统计学处理。结果可见:(1)形态学观察:与对照组相比,激光照射均可有效去除根管内壁的玷污层及碎屑;牙本质小管口清晰可见;未见熔融及炭化等热损伤改变;其中3 W激光处理后根管内壁形态最为理想。(2)微渗漏检测:与对照组相比,各激光组根管微渗漏深度均显著减少(P<0.05);但各激光组间比较无显著性差异(P>0.05)。综上,使用Er,C r:YSGG激光进行根管内照射可达到较为理想的根管清洁目的,显著改善根管充填后的封闭效果,因此有望在临床上应用提高根管治疗的成功率。  相似文献

目的:总结分析根管治疗期间急症的发生原因及其治疗方法。方法:对48例根管治疗期间急症的患牙进行临床分析。地塞米松和甲硝唑根管内封存,1周后用碘仿糊剂加牙胶尖行根管充填;术后随访一年,观察其疗效。结果:根管治疗期急症的发病率为11.62%(48/413),牙髓炎组4.6%(7/150)与牙髓坏死、根尖炎组15.59%(41/463)的急症发病率有显著差异(P<0.001),前牙和后牙发生率有显著差异(P<0.001),随访一年后疗效评定为成功率89.58%。结论:根管治疗期间急症的发生与术前患牙的牙髓状况、牙位等有关;用地塞米松和甲硝唑行根管内封存及碘仿糊剂加牙胶尖行根管充填,治疗效果可靠。  相似文献

目的:热垂直加压技术是目前应用广泛的根管充填技术.它利用牙胶加热后良好的流动性,严密的充填根管系统.成功的根管治疗需要性能良好的根管糊剂和规范的临床操作.基于以上特点,本实验探索两种不同根管糊剂及System B携热器工作尖与根尖孔的距离对热牙胶垂直加压充填根管根尖封闭能力的影响.方法:离体单根牙96颗,在釉牙骨质界处截去牙冠,采用冠向下法将根管预备至F3,冲洗后备用.将所有预备后的标本随机分为10组,其中实验组6组,对照组4组.分别采用美松和AH-Plus两种糊剂,System B携热器工作尖与根尖孔的距离分别为4mm、5mm、6mm.在根管壁上涂布少量的根管糊剂,插入预先试好的大锥度牙胶尖,用System B技术充填根管的根尖部分,冠方用Obtura Ⅱ技术充填.对标本进行染色及透明化处理后分析染料渗入长度进行分析,并通过扫描电镜观察根尖区根管充填材料与根管壁之间的结合形态.结果:AH-Plus组的染料渗入长度明显小于美松组,两者差距有统计学意义(P<0.05).System B携热器工作尖与根尖孔的距离为4mm时微渗漏值较小,且与其它两组比较有统计学差异(P<0.05).扫描电镜下观察各实验组的根管充填材料与根管壁之间都存在缝隙.结论:在临床操作中建议使用AH-Plus糊剂,并且将System B携热器工作尖与根尖孔的距离设置为4mm,以提高根管封闭性能、规范临床操作.  相似文献

目的:比较并评价干髓术及根管治疗术治疗乳磨牙牙髓炎的近远期疗效.方法:随机将231个患牙分为2组,分别用干髓术、根管治疗术治疗,记录治疗所需时间、复诊次数,术后随访3年,观察近远期疗效.结果:干髓术组平均操作时间为90分钟,根管治疗组平均操作时间为150分钟;干髓术组复诊一次完成治疗,根管治疗组复诊二次完成治疗;1年后,干髓术组与根管治疗组有效率比较无显著性差异(p>0.05),2年和3年后根管治疗有效率高于干髓治疗(p<0.05).结论:干髓术具有高效、省时、经济的优点,治疗牙髓炎有一定中、短期疗效,但远期疗效较差,应严格选择适应证;乳牙根管治疗是较理想的乳磨牙牙髓炎治疗方法,近远期疗效好,值得在临床上大力推广.  相似文献

目的:研究颌骨囊肿受累牙根管治疗中采用联合方法测量根管工作长度的效果。方法:选取36 例颌骨囊肿受累患牙需术前 根管治疗的患者,共135 颗患牙,186 个根管,随机分为两组,实验组(93 个根管)采用电测法联合纸尖法测量根管工作长度,对照 组(93 个根管)则采用电测法联合X 线法测量根管工作长度。对比两组根管充填后的充填效果。结果:实验组的根管恰填率为 90.32%,高于对照组的79.57%(P<0.05)。结论:采用电测法联合纸尖法测量颌骨囊肿受累牙根管工作长度,准确率高,根管充填 效果好且安全。  相似文献

目的 观察和探讨新型抗厌氧菌药物奥硝唑消毒感染根管的抗菌效果。方法 选择感染根管患牙98颗,常规根管预备后,随机分为试验组(奥硝唑组)和对照组(甲醛甲酚组)各4 9颗。分别在根管预备前、预备后及封药后进行根管内细菌标本采样、培养、分离与鉴定,测定根管内细菌变化及检出情况,并观察封药后临床症状和体征的变化。结果 2组封药后,根管内细菌数量及检出率均较封药前明显降低( P<0 .0 5 ) ,根管内细菌数量及检出率比较,差异无显著性( P>0 .0 5 ) ,奥硝唑的近期临床疗效优于甲醛甲酚。结论 奥硝唑是一种较安全、有效的根管消毒药物  相似文献

目的:比较普通冲洗和超声冲洗方法对瘘管型慢性根尖周炎一次性根管治疗疗效的影响,以期为超声冲洗方法在慢性根尖周炎临床治疗中的应用提供指导。方法:将2010年6月-2012年6月来我科就诊的166名瘘管型慢性根尖周炎患者随机分为2组,超声冲洗组和普通冲洗组,根据分组分别在一次性根管治疗术中的根管清理过程采用超声根管冲洗方法和普通注射器冲洗方法,术后1周和半年时复诊记录患者一次性根管治疗效果,通过以患者术后1周内的主观"疼痛程度"作为短期评价指标和术后半年治疗效果作为长期评价指标进行比较,采用X2检验进行统计分析。结果:与普通注射器冲洗方法相比,超声冲洗方法可以明显减低一次性根管治疗后短期不适症状的发生率(P0.05),特别是重度反应的发生(P0.05);而术后出现的轻度和中度疼痛反应无明显差异。同时,超声冲洗方法可以明显增加一次性根管治疗术的长期成功率(P0.05)。结论:一次性根管治疗术中使用超声冲洗方法替代常规注射器冲洗方法,能够明显减轻患者短期术后以"疼痛"为主要表现的不适症状,提高一次性根管治疗术在瘘管型根尖周治疗中的长期成功率,是一种较为理想的根管治疗冲洗方法。  相似文献

The goal of endodontic treatment is the debridement and removal of the microbial ecosystem associated with the disease process. The need for root canal disinfectants increases especially in those cases where infection is resistant to the regular treatment and the outcome of endodontic therapy is often compromised. Therefore, it is of interest to document the known effectiveness of silver nanoparticle based root canal disinfectants with other root canal disinfectants on microbial load reduction during root canal disinfection. Known data shows that the overall risk of bias for the selected studies was moderate. Silver nanoparticle based root canal disinfectants showed superior reduction of microbial counts in majority of the studies. This data is limited to vitro studies with no clinical information to validate the use of antimicrobial properties of silver nanoparticles used as root canal disinfectant.  相似文献

目的通过对临床病例的研究,比较减速镍钛机扩和普通不锈钢K型锉清洁根管的能力。方法选取临床需要根管治疗的患牙60例,随机分为2组,经开髓拨髓后分别用镍钛减速机扩和不锈钢K型及H型锉进行根管预备,预备完成后用灭菌棉捻擦拭管壁,作微生物检查。结果2组方法扩锉后对根管内微生物的变化做统计学分析,差异有显著性(P<0.05)。结论镍钛减速机扩对根管的清洁能力高于普通不锈钢扩锉,值得临床的推广。  相似文献

The purpose of this study was to conduct a cone-beam computed tomographic (CBCT) investigation on the root and canal configuration of the mandibular first molars, especially the morphology of the disto-lingual (DL) root, in a Chinese subpopulation. A total of 910 CBCT images of the mandibular first molars were collected from 455 patients who underwent CBCT examinations as a preoperative assessment for implants or orthodontic treatment. The following information was analyzed and evaluated: tooth position, gender, root and root canal number per tooth, root canal type of the mesial root(s) and distal root(s), angle of the DL root canal curvature, distance between two distal canal orifices in the teeth with DL root, and angle of disto-buccal canal orifice–disto-lingual canal orifice–mesio-lingual canal orifice (DB-DL-ML). Most of the mandibular first molars (64.9%, n = 591) had two roots with three root canals, and most of the mesial root canals (87.7%, n = 798) were type VI. The prevalence of the DL root was 22.1% (n = 201). The right side had a higher prevalence of DL root than the left side (p<0.05). Additionally, the curvature of the DL root canal were greater in the bucco-lingual (BL) orientation (30.10°±14.02°) than in the mesio-distal (MD) orientation (14.03°± 8.56°) (p<0.05). Overall there was a high prevalence of DL root in the mandibular first molars, and most of the DL roots were curved in different degrees. This study provided detailed information about the root canal morphology of the mandibular first molars in a Chinese subpopulation.  相似文献

Premolar root form remains an important taxonomic character in hominin alpha taxonomy. Variation in detailed aspects of root structure remains poorly characterized in extant apes. This limited comparative context hinders evaluations of the significance of root form variation in hominin systematics. Using micro‐computed tomography we examine morphological variation in premolars in 51 (n = 128 premolars) West African chimpanzees (Pan troglodytes verus). We categorize premolar root/canal form and number, based on the external root surface and pulp canal morphology, and test for a relationship between canal configuration and sex, jaw size, tooth/root size and cervix shape. Jaw size and root size/shape were quantified using standard metrics, and geometric morphometrics was used to examine root form and cervix shape. Our results confirm previous findings in external root form, but reveal previously undocumented variation in mandibular premolar canal number/form in this subspecies. The LP3 and UP4 exhibit variation in canal number/form, while the UP3 is restricted to external root configurations. The LP4 expresses only a single root/canal form. Generally, in LP3 and UP4 there is no correlation between canal form/number and sex, root size, and jaw size; UP4 canal variation covaried with cervix shape and size. Cervix size is significantly greater in three canal UP4s than two canal UP4s. Our results highlight canal form/number as an important aspect when characterizing root form. The implications of our results for the taxonomic utility of root form within the hominoid and hominin clades are discussed. Pulp regeneration using human dental pulp stem cells (hDPSCs) maintains tooth vitality compared with conventional root canal therapy. Our previous study demonstrated that preameloblast-conditioned medium (PA-CM) from murine apical bud cells induces the odontogenic differentiation of hDPSCs and promoted dentin formation in mouse subcutaneous tissue. The purpose of the present study is to evaluate the effects of PA-CM with human whole pulp cells on pulp regeneration in an empty root canal space. Human pulp cells were seeded in the pulp cavities of 5 mm-thick human tooth segments with or without PA-CM treatment, and then transplanted subcutaneously into immunocompromised mice. In the pulp cell-only group, skeletal muscle with pulp-like tissue was generated in the pulp cavity. A reparative dentin-like structure with entrapped cells lined the existing dentin wall. However, in the PA-CM-treated group, only pulp-like tissue was regenerated without muscle or a reparative dentin-like structure. Moreover, human odontoblast-like cells exhibited palisade arrangement around the pulp, and typical odontoblast processes elongated into dentinal tubules. The results suggest that PA-CM can induce pulp regeneration of human pulp cells with physiological structures in an empty root canal space.  相似文献
