目的:观察强化阿托伐他汀治疗对急性冠脉综合征(ACS)患者血脂水平的短期影响。方法:收集100例ACS患者,入院当天(the day of admission,D0)、入院第一天(the first day,D1)及入院第二天(The Second Day,D2)分别给予阿托伐他汀80mg治疗,入院D0立即采血化验血脂参数包括总胆固醇(total cholesterol,TC)、低度脂蛋白(LDL-cholesterol,LDL-C)、高密度脂蛋白(HDL-cholesterol,HDL-C)、甘油三酯(triglycerides,TG),分别在D1和D2晨起空腹复查。结果:总胆固醇的平均基线水平为5.24±0.07(D0),低密度脂蛋白为3.26±0.07,高密度脂蛋白胆固醇为1.07±0.07,甘油三酯为1.31±0.07。口服阿托伐他汀80毫克后第一天早晨,TC水平下降6.1%(D1)(与D0相比P0.001),第二日下降13.2%(D2)(与D0相比P0.001),LDL-C下降5.8%(D1)(DO与D1相比,P0.001)和15.6%(D2)(DO与D2相比,P0.001);HDL-C下降7.5%(D1)(DO与D1相比,P0.001)和12.1(D2)(DO与D2相比,P0.001);相反TG水平升高20.6%(D1)(DO与D1相比,P0.001)和25.5%(D2)(DO与D2相比,P0.001)。结论:强化他汀治疗对ACS患者血脂短期的影响与长期的影响是不同的,TC,LDL和HDL在短期内是下降的,而TG是升高的。  相似文献

BACKGROUND: Activation of inflammatory pathways plays an important contributory role in coronary plaque instability and subsequent rupture, which can lead to the development of acute coronary syndrome (ACS). Elevated levels of serum inflammatory markers such as C-reactive protein (CRP) represent independent risk factors for further cardiovascular events. Recent evidence indicates that in addition to lowering cholesterol levels, statins also decrease levels of inflammatory markers. Previous controlled clinical trials reporting the positive effects of statins in participants with ACS were designed for very early secondary prevention. To our knowledge, no controlled trials have evaluated the potential benefits of statin therapy, beginning immediately at the time of hospital admission. A previous pilot study performed by our group focused on early initiation of cerivastatin therapy. We demonstrated a highly significant reduction in levels of inflammatory markers (CRP and interleukin-6). Based on these preliminary findings, we are conducting a clinical trial to evaluate the efficacy of another statin, fluvastatin, as an early intervention in patients with ACS. METHODS: The FACS-trial (Fluvastatin in the therapy of Acute Coronary Syndrome) is a multicenter, randomized, double-blind, placebo-controlled study evaluating the effects of fluvastatin therapy initiated at the time of hospital admission. The study will enroll 1,000 participants admitted to hospital for ACS (both with and without ST elevation). The primary endpoint for the study is the influence of fluvastatin therapy on levels of inflammatory markers (CRP and interleukin-6) and on pregnancy associated plasma protein A (PAPP-A). A combined secondary endpoint is 30-day and one-year occurrence of death, nonfatal myocardial infarction, recurrent symptomatic ischemia, urgent revascularization, and cardiac arrest. CONCLUSION: The primary objective of the FACS trial is to demonstrate that statin therapy, when started immediately after hospital admission for ACS, results in reduction of inflammation and improvement of prognosis. This study may contribute to new knowledge regarding therapeutic strategies for patients suffering from ACS and may offer additional clinical indications for the use of statins.  相似文献

To identify genetic loci influencing blood lipid levels in Caribbean Hispanics, we first conducted a genome-wide linkage scan in 1,211 subjects from 100 Dominican families on five lipid quantitative traits: total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C), triglycerides (TG), and LDL-C/HDL-C ratio. We then investigated the association between blood lipid levels and 21,361 single nucleotide polymorphisms (SNP) under the 1-logarithm of odds (LOD) unit down regions of linkage peaks in an independent community-based subcohort (N = 814, 42% Dominican) from the Northern Manhattan Study (NOMAS). We found significant linkage evidence for LDL-C/HDL-C on 7p12 (multipoint LOD = 3.91) and for TC on 16q23 (LOD = 3.35). In addition, we identified suggestive linkage evidence of LOD > 2.0 on 15q23 for TG, 16q23 for LDL-C, 19q12 for TC and LDL-C, and 20p12 for LDL-C. In the association analysis of the linkage peaks, we found that seven SNPs near FLJ45974 were associated with LDL-C/HDL-C with a nominal P < 3.5 × 10(-5), in addition to associations (P < 0.0001) for other lipid traits with SNPs in or near CDH13, SUMF2, TLE3, FAH, ARNT2, TSHZ3, ZNF343, RPL7AL2, and TMC3. Further studies are warranted to perform in-depth investigations of functional genetic variants in these regions.  相似文献

目的:研究成人原发肾病综合征(PNS)肾脏病理改变与血脂代谢紊乱的关系。方法:选取成人PNS患者109 例为研究对象, 分析不同病理类型PNS 患者血脂及血清脂蛋白代谢异常的发生率、代谢水平及胆固醇代谢异常的相关因素。结果:成人PNS 患 者血清总胆固醇(TC)升高、低密度脂蛋白胆固醇(LDL-C)升高、高密度脂蛋白胆固醇(HDL-C)降低、甘油三酯(TG)升高的发生率分 别为89.90%、69.72%、9.17%、41.28%,高胆固醇血症占51.37%,混合型高脂血症占39.45%;微小病变型肾病(MCD)、膜性肾病 (MN)、系膜增生性肾小球肾炎(MsPGN)、局灶阶段性肾小球硬化(FSGS)组间TC、TG、LDL-C、HDL-C 代谢异常发生率及代谢水平 比较均无明显差别(P>0.05);病理类型为MCD 及n-MCD 的PNS 患者,胆固醇水平均与24 小时尿蛋白定量呈正相关(r=0.440, P=0.036;r=0.361,P=0.001),与血白蛋白水平无明显相关性(P>0.05)。结论:成人PNS患者血脂代谢异常以TC 及LDL-C 升高为 主,临床分型中以高胆固醇血症及混合型高脂血症为主;病理类型为MCD及n-MCD 的PNS 患者,胆固醇水平均与24 小时尿蛋 白呈正相关,与血白蛋白水平无明显相关性。  相似文献

Aim

To explore the association of lipid ratios and triglyceride (TG) with insulin resistance (IR) in a Chinese population. We also provide the clinical utility of lipid ratios to identify men and women with IR.

Methods

This cross-sectional study included 614 men and 1055 women without diabetes. Insulin resistance was defined by homeostatic model assessment of IR > 2.69. Lipid ratios included the TG/ high density lipoprotein cholesterol (HDL-C), the total cholesterol (TC)/HDL-C and the low density lipoprotein cholesterol (LDL-C)/HDL –C. Logistic regression models and accurate estimates of the area under the receiver operating characteristic (AUROC) curves were obtained.

Results

In normal-weight men, none of lipid ratios nor TG was associated with IR. In overweight/obese men, normal-weight women and overweight/obese women, the TG/HDL-C, the TC/HDL-C and TG were significantly associated with IR, and the associations were independent of waist circumference. All of the AUROCs for the TG/HDL-C and TG were > 0.7. The AUROCs for TC/HDL-C ratio were 0.69–0.77. The optimal cut-offs for TG/HDL-C were 1.51 in men and 0.84 in women. The optimal cut-offs for TG were 1.78 mmol/L in men and 1.49 mmol/L in women, respectively. In men, the optimal cut-off for LDL-C/HDL-C is 3.80. In women, the optimal cut-off for LDL-C/HDL-C is 3.82.

Conclusion

The TG/HDL-C, the TC/HDL-C and TG are associated with IR in overweight/obese men, normal-weight and overweight/obese women. The LDL-C/HDL-C is only associated with IR in normal-weight women. The TG/HDL-C and TG might be used as surrogate markers for assessing IR.  相似文献

目的:观察短期阿托伐他汀治疗对高胆固醇血症的冠心病患者血管内皮功能的影响。方法:78例高胆固醇血症患者每日口服阿托伐他汀共8周,服药前后测量患者血清的总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(IDIrC)、高密度脂蛋白胆固醇(HDL-C)和氧化低密度脂蛋白(ox-LDL)以及NO值,并用彩色多普勒超声测定反应性充血时肱动脉内径的变化。结果:高胆固醇血症患者经阿托伐他汀8周治疗后,血清的TC、TG、LDL-C和ox-LDL明显下降,血清的HDL-C以及NO值明显增加,反应性充血时肱动脉内径扩张程度明显增加,这些与治疗前相比有明显差异。结论:阿托伐他汀治疗能使高胆固醇血症的冠心病患者血脂改变,NO值增加,血管内皮功能改善。  相似文献

Serum lipid changes during infection may be associated with atherogenesis. No data are available on the effect of Brucellosis on lipids. Lipid parameters were determined in 28 patients with Brucellosis on admission and 4 months following treatment and were compared with 24 matched controls. Fasting levels of total cholesterol (TC), HDL-cholesterol (HDL-C), triglycerides, apolipoproteins (Apo) A, B, E CII, and CIII, and oxidized LDL (oxLDL) were measured. Activities of serum cholesterol ester transfer protein (CETP), paraoxonase 1 (PON1), and lipoprotein-associated phospholipase A2 (Lp-PLA2) and levels of cytokines [interleukins (IL)-1β, IL-6, and tumor necrosis factor (TNFa)] were also determined. On admission, patients compared with controls had 1) lower levels of TC, HDL-C, LDL-cholesterol (LDL-C), ApoB, ApoAI, and ApoCIII and higher LDL-C/HDL-C and ApoB/ApoAI ratios; 2) higher levels of IL-1b, IL-6, and TNFa; 3) similar ApoCII and oxLDL levels and Lp-PLA2 activity, lower PON1, and higher CETP activity; and 4) higher small dense LDL-C concentration. Four months later, increases in TC, HDL-C, LDL-C, ApoB, ApoAI, and ApoCIII levels, ApoB/ApoAI ratio, and PON1 activity were noticed compared with baseline, whereas CETP activity decreased. LDL-C/HDL-C ratio, ApoCII, and oxLDL levels, Lp-PLA2 activity, and small dense LDL-C concentration were not altered. Brucella infection is associated with an atherogenic lipid profile that is not fully restored 4 months following treatment.  相似文献

The effects of long-term low intensity aerobic training and detraining on serum lipid and lipoprotein concentrations were examined in 30 elderly men and women. These subjects were randomly divided into two groups. The training group [n=15; 7 men and 8 women; mean age 75.5 (SD 5.6) years] agreed to take part in physical training using a treadmill with an exercise intensity at the blood lactate concentration threshold for 30 min 3–6 times a week for 9 months. The other group [n=15; 7 men and 8 women; mean age 73.7 (SD 4.4) years] did not perform any particular physical training and was followed as the control. Following this training period the high density lipoprotein-cholesterol (HDL-C) had increased significantly (P<0.01) while the total cholesterol (TC) : HDL-C ratio had decreased significantly (P<0.01) in the training group after 9 months but had not changed in the control group. The TC, triglyceride (TG) and low density lipoprotein-cholesterol (LDL-C) had not changed significantly in either group. No significant difference was seen between the groups throughout the period for TC, LDLC or TG. There was, however, a significant correlation between the initial TC:HDL-C ratio and the change in the TC:HDL-C ratio following 3 months of training (P <0.05). After 1 month of detraining in 5 patients, the HDL-C had decreased significantly (P < 0.05) while the TC:HDL-C had increased significantly in the training group (P<0.01). These results suggested that long-term low intensity aerobic training improved the profile of serum lipid and lipoprotein concentrations, while detraining returned the profile to that of the pretraining levels in elderly persons.  相似文献

目的:探讨健康体检人群血糖与血脂现况及两者之间的相关性。方法:采集我院9758例健康体检者的空腹静脉血,检测其空腹血糖(FPG)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL-C)和低密度脂蛋白(LDL-C)水平,比较不同FPG水平组间各血脂水平,分析FPG水平与血脂的相关性。结果:各年龄组FPG、TC、TG、HDL-C和LDL-C水平差异均具有统计学意义(P0.05);FPG均与TC、TG和LDL-C水平呈现正相关(r=0.127,0.189,0.141,P0.005),而与HDL-C呈现负相关(r=-0.112,P0.005);根据FPG水平由高到低分为糖尿病(DM)组,血糖调节受损(IFG)组及血糖正常组,其中对TC和LDL-C水平异常率而言,IFG组DM组血糖正常组;对TG、HDL-C水平异常率而言,DM组IFG组血糖正常组。结论:健康人群中血糖和血脂水平呈现一定的相关性,两者的水平异常会增加慢性疾病的发生风险。  相似文献

目的:分析肾移植术后患者血清的同型半胱氨酸(Hcy)、肾功能和血脂水平的变化和相关性,探讨其在肾移植术后评价肾功能的应用价值。方法:将2013年10月~2016年9月就诊于我院确诊慢性肾衰并进行肾移植手术的300例术后随访患者作为观察组,选择同期健康志愿者100例作为对照组。检测并比较两组Hcy、总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)水平。根据观察组患者Hcy水平的不同将其分成Hcy正常组与Hcy异常组,并对比两组患者的血脂指标水平;测定半胱氨酸蛋白抑制剂C(CysC)的水平并计算肾小球滤过率(eGFR);对观察组血清Hcy与eGFR值、血脂指标水平进行相关性分析,并采用Logistic回归分析分析观察组肾移植术后eGFR下降的影响因素。结果:观察组患者的血清Hcy、TC、TG、HDL-C、LDL-C水平均明显高于对照组(P0.05)。Hcy异常组血清LDL-C水平明显高于Hcy正常组,而HDL-C水平明显低于Hcy正常组(P0.05)。观察组患者血清Hcy与eGFR、HDL-C水平呈负相关关系(r=-0.573、-0.414,P0.05);与TG水平呈正相关(r=0.432,P0.05),与TC、LDL-C无相关(P0.05)。多元Logistic回归分析显示,Hcy、TG、LDL-C水平均与患者eGFR下降有关(P0.05)。结论:在肾移植术后,慢性肾衰患者的TG、LDL-C、Hcy水平均升高,且伴有eGFR水平的降低;肾移植术后肾功能的改变与血清TG、LDL-C、Hcy水平相关;检测肾移植患者血脂指标、Hcy的水平可以评估移植肾功能受损情况。  相似文献

ABSTRACT: BACKGROUND: An increasing number of authors employing intravascular ultrasound (IVUS) and virtual histology (VH-IVUS) have investigated the effect of statin use on plaque volume (PV) and plaque composition. However, inconsistent results have been reported. Therefore, we conducted a meta-analysis to determine the appropriate regimen of statins to effectively stabilize vulnerable coronary plaques. METHODS: Online electronic databases were carefully searched for all relevant studies. We compared mean values of PV and plaque composition between baseline and follow-up in patients receiving statin therapy. We pooled treatment effects and calculated mean differences (MD) with the 95% confidence interval (CI) using a random-effects model. By stratified analyses, we explored the influence of clinical presentation, dose and duration of statin treatment, and low-density lipoprotein-cholesterol (LDL-C) levels on the effects of statins. RESULTS: Seventeen studies involving 2,171 patients were analyzed. Statin therapy significantly decreased PV (-5.3 mm3; 95% CI: --3.3 mm3 to -7.2 mm3; P < 0.001), without heterogeneity. When considering the dose and duration of statins used, only subgroups employing a high dose and long duration demonstrated a significant reduction in PV (p < 0.001). A significant decrease in PV was noted if achieved LDL-C levels were <100 mg/dL (p < 0.001). Statin treatment could induce a twofold decrease in PV in patients with acute coronary syndrome (ACS) compared with that observed in patients with stable angina pectoris (SAP). A regressive trend was seen for necrotic core volume (MD: --2.1 mm3; 95% CI: --4.7 mm3 to 0.5 mm3, P = 0.11). However, statin use did not induce a significant change for fibrotic, fibro-fatty, or dense calcium compositions. CONCLUSIONS: Our meta-analysis demonstrated that statin therapy (especially that involving a high dose and long duration and achieving <100 mg/dL LDL-C levels) can significantly decrease PV in patients with SAP or ACS. These data suggested that statins can be used to reduce the atheroma burden for secondary prevention by appropriately selecting the statin regimen. No significant change in plaque composition was seen after statin therapy.  相似文献

目的:探讨甲状腺功能减退患者血清同型半胱氨酸(Hcy)、甲状腺激素(TH)及血脂水平测定的临床意义。方法:选取2016年2月-2017年2月期间我院收治的甲状腺功能减退患者101例为观察组,选取同期于我院体检的健康志愿者80例为对照组。检测所有研究对象甲状腺素(FT4)和三碘甲状腺原氨酸(FT3)、Hcy及血脂[甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)]变化水平。比较观察组治疗前后、对照组与观察组治疗前FT3、FT4、Hcy及血脂水平,采用Pearson相关性分析血清Hcy与FT3、FT4、血脂水平的相关性。结果:观察组患者治疗后FT3、FT4均较治疗前升高,Hcy、TC、LDL-C均较治疗前降低(P0.05),而观察组患者治疗前后TG、HDL-C比较差异无统计学意义(P0.05)。观察组治疗前FT3、FT4、HDL-C水平明显低于对照组,而Hcy、TG、TC、LDL-C则明显高于对照组(P0.05)。经Pearson相关性分析显示,甲状腺功能减退患者Hcy与FT3、FT4呈负相关(P0.05),与TC、TG呈正相关(P0.05),而与LDL-C、HDL-C无相关性(P0.05)。结论:甲状腺功能减退患者的FT3、FT4、Hcy及血脂水平表达异常,且Hcy与FT3、FT4、血脂水平密切相关,临床上可通过监测甲状腺功能减退患者的上述指标,有助于评估患者的病情程度。  相似文献

目的探讨建立兔饵食性高脂血症模型的方法,观察高脂饮食对兔体重、死亡率及血脂变化情况。方法取普通级雄性新西兰大耳白兔,其中随机抽取10只作为普通饮食组,喂以普通饮食;其他动物给予高脂饮食,4周后,根据血清TC水平分为高脂饮食敏感组和高脂饮食非敏感组,继续喂养9周,观察三组间兔血清总胆固醇(TC)、甘油三脂(TG)、高密度脂蛋白胆固醇(HDL-C)和低密度脂蛋白胆固醇(LDL-C)的变化。结果与普通饮食组相比,高脂饮食敏感组在第4~13周时血清TC、TG、HDL-C、LDL-C水平显著升高(P〈0.01),高脂饮食非敏感组在第7~13周时,动物血清TC、HDL-C、LDL-C水平显著升高(P〈0.01),而高脂饮食非敏感组血清TG水平的改变无统计学意义(P〉0.05)。与高脂饮食敏感组相比,高脂饮食非敏感组家兔血清TC、TG、HDL-C和LDL-C水平的上升程度均显著低于高脂饮食敏感组动物(P〈0.01)。结论首次发现兔对高脂饮食敏感性存在差异,高脂饮食非敏感组家兔抗病能力、对环境的适应能力和耐受性均高于高脂饮食敏感组兔。与高脂饮食敏感组家兔相比,高脂饮食非敏感组家兔对高脂饮食耐受性强。  相似文献

目的:探讨格列美脲联合艾塞那肽治疗肥胖2型糖尿病患者的临床效果。方法:选择2016年1月到2019年1月我院收治的82例肥胖2型糖尿病患者作为本次研究的对象,并将其随机的分为研究组和对照组,每组41例。研究组患者给予格列美脲联合艾塞那肽进行治疗,对照组患者给予格列美脲治疗,观察和比较两组患者治疗前后空腹血糖、餐后2 h血糖、空腹和餐后2 h血清C肽、身体质量指数(body mass index, BMI)、总胆固醇(total cholesterol, TC)、甘油三酯(triglyceride, TG)、低密度脂蛋白胆固醇(low density lipoprotein cholesterol, LDL-C)、高密度脂蛋白胆固醇(high density lipoprotein cholesterol, HDL-C)和糖化血红蛋白(glycosylated hemoglobin, HbAlC)水平的变化。结果:治疗后,两组患者空腹血糖、餐后2 h血糖水平均较治疗前明显降低,餐后2 h血清C肽水平均较治疗前明显升高,且研究组以上指标的改善程度较对照组更明显(P0.05)。两组患者治疗前后空腹血清C肽水平比较差异无统计学意义(P0.05);治疗后,两组患者BMI、TC、TG、LDL-C和HbAlC水平均较治疗前显著降低,HDL-C水平明显升高,而研究组BMI、TC、TG、LDL-C和HbAlC水平显著低于对照组(P0.05),HDL-C水平明显高于对照组(P0.05)。结论:格列美脲片联合艾塞那肽治疗肥胖2型糖尿病可有效的控制患者血糖,降低BMI,改善血脂水平。  相似文献

目的:探讨血浆同型半胱氨酸(Homocysteine,Hcy)、高敏C反应蛋白(Hypersensitive C-reactive protein,hs-CRP)在家族性高胆固醇血症(FH)的纯合子和杂合子患者中的水平及其与临床生化指标的相关性。方法:入选在2013.10~2015.7期间在动脉硬化门诊随访、确诊的家族性高胆固醇患者34人(纯合子14例,杂合子20例)。根据FH纯合子、杂合子、健康体检者分成三组,分别测定其总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、空腹血糖(Glu)、Hcy、hs-CRP水平,并比较FH患者血浆LDL-C、非高密度脂蛋白(non-HDL)水平Hcy、hs-CRP水平的相关性。结果:FH组的TC、LDL-C、non-HDL、Hcy水平显著高于正常对照组(P0.05)。FH根据基因型亚组分析中,青少年纯合子组HDL-C[(0.78±0.21)vs(1.25±0.40)mmol/L]小于杂合子组(P0.001),而TC[(16.11±2.66)vs(7.30±2.54)mmol/L]、LDL-C[(14.06±2.22)vs(5.25±2.16)mmol/L]、non-HDL[(15.33±2.60)vs(6.05±2.61)mmol/L]、Hcy[(19.3±11.58)vs(11.29±3.42)μmol/L]水平显著高于杂合子组(P0.05)。Pearson相关性分析显示,FH患者血浆LDL-C、non-HDL水平与Hcy水平呈正相关(P0.05),而与hs-CRP无相关性。结论:FH患者,特别是青少年的纯合子患者,与对照组相较,具有较高的TC、LDL-C和Hcy水平,并且与血浆LDL-C、non-HDL水平呈正相关。FH患者hs-CRP的水平高于健康人,但差异无统计学意义,与血浆LDL-C、non-HDL水平也无明显相关性。  相似文献

BackgroundObservational studies have suggested that selenium levels might associate with the risk of cardio-metabolic diseases, but how circulating selenium is related to dyslipidemia remains inconclusive.ObjectivesTo investigate the association of circulating selenium levels with lipid profiles and dyslipidemia among US adults.MethodsUsing the data collected from the National Health and Nutrition Examination Survey (NHANES 1999–2006), we performed multivariate logistic regression to examine the association of circulating selenium levels (in quartiles) with total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), non-HDL-C, and atherogenic index (AI).ResultsWe included 2903 adults (49.3 % male) (average age: 61.9) for analysis. Circulating selenium had non-linear association with TC, LDL-C, HDL-C, and AI (all p < 0.05). When comparing with the lowest quartile, subjects with the highest quartile of circulating selenium (>147.00 μg/L) had the higher odds of elevated TG (OR: 1.75, 95% CI = 1.14, 2.68), TC (OR: 2.47, 95% CI = 1.62, 3.76), LDL-C (OR: 2.52, 95% CI = 1.60, 3.96), non-HDL-C (OR: 2.17, 95% CI = 1.41, 3.33), AI (OR: 1.20, 95% CI = 0.73, 1.97) and low-HDL-C (OR: 2.10, 95% CI = 1.19, 3.72). Similar patterns were observed in subgroup analysis.ConclusionsHigher circulating selenium levels had non-linear association with lipid profiles and the increased odds of dyslipidemia.  相似文献

目的:对比研究不同年龄急性冠脉综合征患者急性期血脂代谢特点及相关因素变化,探讨血脂代谢在不同年龄急性冠脉综合征发生过程中的作用及临床意义。方法:222例急性冠脉综合征(ACS)患者及119例稳定型心绞痛(SAP)患者共341例按照年龄分为3组,<60岁为非老年组共68人,其中ACS组33人,SAP组35人;60-74岁为普通老年组共67人,其中ACS组34人,SAP组33人;≥75岁为高龄老年组共206人,其中ACS组155人,SAP组51人。详细记录其基线资料并测定其血脂各项指标(包括TC、TG、LDL-C、HDL-C)及CRP。结果:与SAP患者比较,非老年人组及普通老年人组ACS患者LDL-C、CRP均显著升高;高龄老年人ACS患者LDL-C水平无显著升高,但HDL-C显著降低,CRP显著升高,差异均具有统计学意义(P<0.05),LDL-C虽无显著升高,但未达指南要求水平。各组之间TC、TG之间差异无统计学意义(P>0.05)。结论:脂代谢异常在不同年龄ACS患者的发生过程中均有重要作用,老年ACS患者中,除了应关注脂代谢紊乱外,还应该关注高血压、糖尿病等危险因素。  相似文献

目的:研究肾病综合征(NS)患者血脂代谢紊乱与血胆红素和蛋白代谢指标的相关性。方法:纳入我院从2017年5月~2019年5月收治的NS患者50例进行研究,记作病变组,另取同期于我院进行体检的健康人员50例作为健康组。检测两组血脂代谢相关指标[甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、载脂蛋白A1(ApoA1)、载脂蛋白B(ApoB)]、血胆红素相关指标[总胆红素(TBIL)、直接胆红素(DBIL)以及间接胆红素(IBIL)]以及蛋白代谢相关指标[血清总蛋白(TP)、血清清蛋白(ALB)、尿微量清蛋白(UALB)]水平并进行对比,予以Pearson相关性分析各指标之间的相关性。结果:病变组TG、TC、LDL-C、ApoA1、ApoB水平均显著高于健康组,而HDL-C水平显著低于健康组(均P0.05)。病变组TBIL、DBIL、IBIL水平均显著低于健康组(均P0.05)。病变组TP、ALB水平均显著低于健康组,而UALB水平显著高于健康组(均P0.05)。经Pearson相关性分析可得:NS患者TG、TC、LDL-C、ApoA1、ApoB与TBIL、DBIL、IBIL、TP、ALB均呈负相关(均P0.05);TG、TC、LDL-C、ApoA1、ApoB与UALB均呈正相关(均P0.05);HDL-C与TBIL、DBIL、IBIL、TP、ALB均呈正相关(均P0.05);HDL-C与UALB呈负相关(均P0.05)。结论:NS患者血脂代谢紊乱和血胆红素、蛋白代谢指标密切相关,血胆红素、蛋白代谢指标可能参与了NS患者血脂代谢紊乱的机制。  相似文献

Background

Diets are the important players in regulating plasma lipid profiles. And the R219K polymorphism at the gene of ATP-binding cassette transporter 1(ABCA1) was reported to be associated with the profiles. However, no efforts have been made to investigate the changes of lipid profiles after a high-carbohydrate and low-fat diet in different subjects with different genotypes of this polymorphism. This study was to evaluate the effects of ABCA1 R219K polymorphism on serum lipid and apolipoprotein (apo) ratios induced by a high-carbohydrate/low-fat (high-CHO) diet. After a washout diet of 54.1% carbohydrate for 7 days, 56 healthy young subjects (22.89 ± 1.80 years old) were given a high-CHO diet of 70.1% carbohydrate for 6 days. Height, weight, waist circumference, hip circumference, glucose (Glu), triglyceride (TG), total cholesterol (TC), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), apoA-1 and apoB-100 were measured on the 1st, 8th and 14th days of this study. Body mass index (BMI), waist-to-hip ratios (WHR), log(TG/HDL-C), TC/HDL-C, LDL-C/HDL-C and apoA-1/apoB-100 were calculated. ABCA1 R219K was analyzed by a PCR-RFLP method.

Results

The results indicate that the male subjects of all the genotypes had higher WHR than their female counterparts on the 1st, 8th and 14th days of this study. The male K carriers had higher log(TG/HDL-C) and TC/HDL-C than the female carriers on the 1st and 14th days, and higher LDL-C/HDL-C on the 14th day. When compared with that on the 8th day, TC/HDL-C was decreased regardless of the genotypes and genders on the 14th day. Log(TG/HDL-C) was increased in the males with the RR genotype and the female K carriers. Lowered BMI, Glu and LDL-C/HDL-C were found in the male K carriers, but only lowered BMI in the female K carriers and only lowered LDL-C/HDL-C in the females with the RR genotype.

Conclusions

These results suggest that ABCA1 R219K polymorphism is associated differently in males and females with elevated log(TG/HDL-C) and decreased LDL-C/HDL-C induced by the high-CHO diet.  相似文献
