Values of peak expiratory flow (PEF) in 142 current smokers (116 men, 26 women) and 108 ex-smokers (88 men, 20 women) aged 55 or over were compared with the predicted values obtained in lifelong nonsmokers of the same age range. None of the subjects had been liable during childhood or subsequently to expectoration, lower respiratory tract infection, wheeze, or shortness of breath. Observed values of PEF were expressed as differences from predicted. Analysis of the relation between smoking state and ventilatory function in the men disclosed significant reductions of PEF in current smokers, the deficits increasing with the amount smoked from a mean of 48.1 l/min in those smoking fewer than 20 cigarettes a day to 73.3 l/min in smokers of 20 or more a day. Significant reductions of PEF were also found in women who were currently smoking (mean 47.4 l/min) and in male ex-smokers of 20 or more cigarettes a day (mean 27.8 l/min). There was no significant reduction of PEF in male or female ex-smokers of fewer than 20 cigarettes a day. These findings suggest that factors besides smoking are concerned in the development of irreversible airflow obstruction.  相似文献

Data from a hospital based case-control study of lung cancer in Western Europe were used to examine changes in the risk of developing lung cancer after changes in habits of cigarette smoking. Only data for subjects who had smoked regularly at some time in their lives were included. The large size of the study population (7181 patients and 11 006 controls) permitted precise estimates of the effect of giving up smoking. Risks of developing lung cancer for people who had given up smoking 10 or more years before interview were less than half of those for people who continued to smoke. The reduction in risk was seen in men and women and in former smokers of both filter and non-filter cigarettes but varied by duration of smoking habit before giving up. The protective effect of giving up became progressively greater with shorter duration of smoking habit. The risks after not smoking for 10 years for both men and women who had previously smoked for less than 20 years were roughly the same as those for lifelong non-smokers. Reducing the number of cigarettes smoked a day or switching from non-filter to filter cigarettes also lowered the risk of developing lung cancer but not to the extent associated with giving up smoking.  相似文献

The smoking habits of 48 patients with malignant hypertension were compared with those of 92 consecutive patients with non-malignant hypertension. Thirty-three of the patients with malignant and 34 of the patients with non-malignant hypertension were smokers when first diagnosed. This difference was significant, and remained so when only men or black and white patients were considered separately. Results suggest that malignant hypertension is yet another disease related to cigarette smoking.  相似文献

Single-breath oxygen (SBO2) tests at expiratory flow rates of 0.2, 0.5, and 1.01/s were performed by 10 normal subjects in a body plethysmograph. Closing capacity (CC)--the absolute lung volume at which phase IV began--increased significantly with increases in flow. Five subjects were restudied with a 200-ml bolus of 100% N2 inspired from residual volume after N2 washout by breathing 100% O2 and similar results were obtained. An additional five subjects performed SBO2 tests in the standing, supine, and prone positions; closing volume (CV)--the lung volume above residual volume at which phase IV began--also increased with increases of expiratory flow. The observed increase in CC with increasing flow did not appear to result from dependent lung regions reaching some critical "closing volume" at a higher overall lung volume. In normal subjects, the phase IV increase in NI concentration may be caused by the asynchronous onset of flow limitation occurring initially in dependent regions.  相似文献

The purpose of this study was to develop and validate a regression equation to estimate peak power (PP) using a large sample of athletic youths and young adults. Anthropometric and vertical jump ground reaction forces were collected from 460 male volunteers (age: 12-24 years). Of these 460 volunteers, a stratified random sample of 45 subjects representing 3 different age groups (12-15 years [n = 15], 16-18 years [n = 15], and 19-24 years [n = 15]) was selected as a validation sample. Data from the remaining 415 subjects were used to develop a new equation ("Novel") to estimate PP using age, body mass (BM), and vertical jump height (VJH) via backward stepwise regression. Independently, age (r = 0.57), BM (r = 0.83), and VJ (r = 0.65) were significantly (p < 0.05) correlated with PP. However, age did not significantly (p = 0.53) contribute to the final prediction equation (Novel): PP (watts) = 63.6 × VJH (centimeters) + 42.7 × BM (kilograms) - 1,846.5 (r = 0.96; standard error of the estimate = 250.7 W). For each age group, there were no differences between actual PP (overall group mean ± SD: 3,244 ± 991 W) and PP estimated using Novel (3,253 ± 1,037 W). Conversely, other previously published equations produced PP estimates that were significantly different than actual PP. The large sample size used in this study (n = 415) likely explains the greater accuracy of the reported Novel equation compared with previously developed equations (n = 17-161). Although this Novel equation can accurately estimate PP values for a group of subjects, between-subject comparisons estimating PP using Novel or any other previously published equations should be interpreted with caution because of large intersubject error (± >600 W) associated with predictions.  相似文献

The effects of smoking cigarettes with 30% of the tobacco replaced by NSM tobacco substitute, which lowered their tar and nicotine delivery, were studied by comparing them with the effects of conventional cigarettes in a controlled crossover trial lasting 20 months. Chest symptoms, cigarette consumption, and forced expiratory volume in one second (FEV1) were measured each month. Two-hundred men began the trial and 159 completed it. The test cigarettes were acceptable to all but one of the men. In a subsample of 35 men estimates of nicotine intake were obtained from monthly analyses of cigarette stubs. On changing from NSM to control cigarettes six of the 17 men, who were accustomed to low nicotine, kept their nicotine intake down by some change in smoking habit. Before the crossover and this change in smoking habit the men smoking NSM cigarettes had a small but significant reduction of cough. Cigarettes containing 30% NSM and delivering only 1 mg of nicotine are likely to be acceptable to smokers and may reduce coughing. Further trials are needed to confirm these findings and establish what long-term effects such cigarettes may have on smokers'' health.  相似文献

Most hospital policies place little or no restriction on patients'' smoking in hospital. In this study patients were surveyed to determine if they smoked and if their doctors advised or ordered them to stop smoking in hospital. As well, the smoking habits and attitudes towards smoking of the medical staff and other hospital workers were explored. Of 741 patients 37% were smokers, and those who responded fully to a questionnaire 86% continued to smoke in hospital. Patients who were advised or ordered not to smoke (59%) were no more likely to stop smoking than those who were not so advised or ordered. Physicians were less likely to smoke than other hospital staff, and those who did smoke were much more likely not to smoke while in the hospital. Physicians appear to have a reasonable appreciation of the health hazards of smoking, and almost two thirds are in favour of stricter restrictions on patients'' smoking in hospital. The ineffectiveness of their efforts is primarily due to hospital policies that are not in keeping with physicians'' standards of practice and with established knowledge of the deleterious effects of smoking on health.  相似文献

Gastric emptying after a test meal was studied in 17 normal volunteers--10 habitual smokers and seven non-smokers. The solid component of the test meal was labelled with technetium and the liquid component with indium. After one meal the habitual smokers smoked two cigarettes. Emptying curves were produced for both technetium and indium, and the differences between curves for meals with and without cigarettes were analysed. Cigarette smoking accelerated the rate at which the liquid component of a meal left the stomach. This may be important in the pathogenesis of duodneal ulcer and the delay in healing caused by cigarette smoking.  相似文献
