目的:探讨脂代谢紊乱在妊娠期糖尿病(GDM)中的作用,为妊娠期糖尿病的预防及指导临床干预提供理论依据。方法:观察妊娠期糖尿病患者和糖耐量正常孕妇血脂水平及胰岛素抵抗程度差异,分析妊娠期糖尿病患者饮食治疗前后的血脂及炎症标志物的动态变化,于孕12W、24W及36W分别抽取两组孕妇空腹静脉血,测定糖、脂代谢指标及炎症标志物水平,计算血浆致动脉粥样硬化指数(atherogenic index of the plasma,AIP);应用稳态模型胰岛素抵抗指数(HOMA-IR)及胰岛分泌功能指数(HBCI),评价胰岛素抵抗指数(IR)程度及胰岛功能。结果:(1)GDM组的C肽、FINS、HOMA-IR明显高于糖耐量正常组(normal glucose tolerance,NGT)组(p0.05),GDM组HBCI指数低于NGT组(p0.05)。(2)干预组与对照组比较,12W时,TC、TG、HDL、LDL差异均无统计学意义(p0.05);24W及32W差异均有统计学意义(p0.05),均较对照组高。(3)对GDM组中TC、TG、HDL、LDL、AIP、hs-CRP、N及WBC值进行分析,TG、TC、LDL、AIP、hs-CRP、N及WBC值24W较36W及12W高(p0.05);HDL水平24W较36W及12W低(p0.05)。(4)NGT组中TG、TC、LDL、AIP、hs-CRP、N及WBC值36W较24W及12W高(p0.05);HDL水平36W较24W及12W高(p0.05)。结论:GDM孕妇存在着明显的胰岛素抵抗和胰岛β细胞分泌功能受损。GDM孕妇合并较正常妊娠更为严重的炎症反应,血脂水平明显升高,饮食治疗后对改善IR有益,提示在妊娠期糖尿病患者中,通过适当的饮食治疗进而对血糖及血脂的调整可以显著减少母儿并发症。  相似文献

目的:为明确妊娠期糖尿病(gestational diabetes mellitus,GDM)患者血清糖化血红蛋白(glycosylated hemoglobin,Hb A1c)与C反应蛋白(C-reactive protein,CRP)的关系,本研究检测了GDM患者血糖、血清Hb A1c与CRP水平,并对Hb A1c与CRP的相关性进行了分析。方法:以68例2015年4月至2017年4月于我院诊治的GDM孕妇为研究对象,所有患者均符合《妇产科学》中关于GDM的诊断标准,另选取68例正常孕妇为对照组。采用葡萄糖氧化酶法检测血糖水平(空腹血糖及餐后2 h血糖水平),采用免疫凝集法检测和比较两组血清糖化血红蛋白(HbA1c)水平,采用免疫透射比浊法检测血清C反应蛋白(CRP)水平,并分析HbA1c与CRP的相关性。结果:GDM组患者空腹血糖(fasting plasma glucose,FPG)、2 h血糖(plasma glucose,PG)、血清HbA1c和CRP水平均显著高于正常组(P0.05),GDM患者血清Hb A1c和CRP水平呈显著正相关关系(r=0.654,P0.05)。结论:GDM患者血清HbAlC和CRP水平相较于正常孕妇有显著提高,且二者呈显著的正相关关系,二者联合检测可能作为GDM早期诊断的筛查的重要参考指标。  相似文献

Oxidized low-density lipoprotein (ox-LDL) is involved in the pathogenesis of atherosclerosis and atherosclerotic plaque rupture by promoting lipid accumulation, proinflammatory responses, and cell death. LDL is mainly oxidized in the subendothelial layer of the vascular wall and then can be taken up by vascular endothelial cells. However, little is known about the intracellular processing of the damaged LDL. Previous studies found that autophagy is involved in degrading oxidized proteins under oxidative stress conditions in Arabidopsis thaliana, while ox-LDL can activate autophagy in EA.hy926 endothelial cells, suggesting a possible role of autophagy in the degradation of ox-LDL by endothelial cells. The present study showed that ox-LDL aggregated in human umbilical vein endothelial cells (HUVECs) and brought about an increase in the formation of autophagosomes and autolysosomes. Ox-LDL-induced increase in the autophagic level was blocked by treatment with the autophagy inhibitor 3-methyladenine and increased by the autophagy inducer rapamycin, while the aggregation of Dil-labled ox-LDL was increased by 3-methyladenine and decreased by rapamycin. In addition, Dil-labeled ox-LDL colocalized with the autophagy marker MDC, microtubule-associated protein light chain 3 (MAP1-LC3), and lysosome-associated membrane protein 2a (lamp2a). HUVECs treated with Dil-labeled-ox-LDL showed a much greater degree of overlap of MAP1-LC3 and Lamp2a than control. The results suggest that ox-LDL activates the autophagic lysosome pathway in HUVECs through the LC3/beclin1 pathway, leading to the degradation of ox-LDL.  相似文献

ObjectiveTo estimate the association of free triiodothyronine (FT3) and total triiodothyronine (TT3) in early pregnancy and subsequent gestational diabetes mellitus (GDM) risk and define appropriate TT3 thresholds for GDM screening.MethodsThis investigation is a hospital-based cohort study of pregnant women submitted to a universal thyroid function test before 24 weeks of gestation. GDM was diagnosed according to a 75-g oral glucose tolerance test. The association of maternal high FT3 and TT3 levels in early pregnancy with the risk of GDM was estimated using logistic regression. The potential nonlinear association was probed by the restricted cubic spline curve method.ResultsA total of 27 184 pregnant women and 3073 GDM cases were included in the analysis. FT3 and TT3 were associated with an increased subsequent risk of GDM in a nonlinear fashion. The adjusted odds ratios were 1.59 (95% confidence interval, 1.50-1.68) and 2.80 (95% confidence interval, 2.46-3.18) for FT3 and TT3 continuous levels, respectively. Associations were strong in euthyroid women, showed heterogeneity in women with mild thyroid dysfunction, and lacked in patients with overt hypothyroidism and hyperthyroidism. The TT3 thresholds of 1.5 and 2.0 ng/mL between 7 and 12 weeks of gestation and 1.6 and 2.1 ng/mL for 13 to 23 weeks of gestation effectively distinguished the subsequent risk of GDM.ConclusionThe increased FT3 and TT3 levels in early pregnancy were associated with a subsequent higher risk of GDM. These findings provide measures for early detection and potential prevention of GDM.  相似文献

Abstract

There is increasing evidence that oxidative stress may play a role in the pathophysiology of hyperemesis gravidarum. Serum paraoxonase-1 (PON-1) is a high density lipoprotein (HDL)-associated enzyme that prevents oxidative modification of low density lipoprotein. The aim of the study was to measure the serum levels of PON-1 activity in women with hyperemesis gravidarum. Thirty-four women with hyperemesis gravidarum and 31 healthy pregnant women were enrolled in the study. Serum PON-1 activity was measured spectrophotometrically. Lipid hydroperoxide (LOOH) levels were measured by iodometric assay. PON-1 activity was significantly lower and LOOH levels were significantly higher in pregnant women with hyperemesis gravidarum than in healthy pregnant women (P < 0.0001, for all). There were significant correlations between PON-1 and LOOH, triglyceride, total cholesterol, HDL, low density lipoprotein (LDL) and high sensitive C-reactive protein (HSCRP; P < 0.0001, for all). By using multiple regression analysis LDL, HDL, HSCRP and LOOH were independent determinants of serum PON-1 activity in the study. Decreased PON-1 activity might be related to increased oxidative stress and inflammation in pregnant women with hyperemesis gravidarum. Subjects with hyperemesis gravidarum might be more prone to the development of atherogenesis due to low serum PON-1 activity.  相似文献

ObjectiveTo estimate and report the prevalence of gestational diabetes mellitus (GDM) in pregnant women of Bandar Abbas, a city in southern Iran.MethodsFrom March 2002 to March 2004, 800 pregnant women underwent assessment for GDM in obstetrics clinics in Bandar Abbas. The medical history and risk factors for GDM were recorded, and the weight, height, and blood pressure were measured. All the women were screened for GDM by a 1-hour, 50-g oral glucose tolerance test (OGTT), with a cutoff point of 130 mg/dL. All patients with a “positive” screening test result underwent a 3-hour OGTT with 100 g of glucose.ResultsThe prevalence rate of GDM in our study was 6.3% (95% confidence interval, 4.7% to 8.4%) and 8.9% (95% confidence interval, 6.9% to 11.3%) with use of the National Diabetes Data Group and the Carpenter and Coustan criteria, respectively. The patients with GDM were significantly older than the normal group of pregnant women (28.2 ± 5.6 years versus 24.6 ± 5.2 years, respectively) (P < 0.001). The mean number of pregnancies, systolic blood pressure, and body mass index (BMI) were significantly higher in the GDM group than in the normal pregnant women (P < 0.05). Among the recorded risk factors, only age ≥ 25 years, history of macrosomia in previous newborns, and BMI ≥ 25 kg/m2 were significantly more prevalent in the GDM group than in the normal group (P < 0.05). If selective screening criteria for GDM had been applied, 11.3% of patients with GDM would have been missed.ConclusionThis study showed a higher prevalence of GDM in Bandar Abbas in comparison with other parts of Iran. Most people are aware of gestational diabetes mellitus (GDM), a dangerous pregnancy complication in which pregnant women who have never been diagnosed with diabetes develop chronic hyperglycaemia. Exosomal microRNA (miRNA) dysregulation has been shown to be a key player in the pathophysiology of GDM. In this study, we looked into how placental exosomes and their miRNAs may contribute to GDM. When compared to exosomes from healthy pregnant women, it was discovered that miR-135a-5p was elevated in placenta-derived exosomes that were isolated from the maternal peripheral plasma of GDM women. Additionally, we discovered that miR-135a-5p encouraged HTR-8/SVneo cell growth, invasion and migration. Further research revealed that miR-135a-5p activates HTR-8/SVneo cells' proliferation, invasion and migration by promoting PI3K/AKT pathway activity via Sirtuin 1 (SIRT1). The transfer of exosomal miR-135a-5p generated from the placenta could be viewed as a promising agent for targeting genes and pertinent pathways involved in GDM, according to our findings.  相似文献

Poliovirus (PV), like many positive-strand RNA viruses, subverts the macroautophagy/autophagy pathway to promote its own replication. Here, we investigate whether the virus uses the canonical autophagic signaling complex, consisting of the ULK1/2 kinases, ATG13, RB1CC1, and ATG101, to activate autophagy. We find that the virus sends autophagic signals independent of the ULK1 complex, and that the members of the autophagic complex are not required for normal levels of viral replication. We also show that the SQSTM1/p62 receptor protein is not degraded in a conventional manner during infection, but is likely cleaved in a manner similar to that shown for coxsackievirus B3. This means that SQSTM1, normally used to monitor autophagic degradation, cannot be used to accurately monitor degradation during poliovirus infection. In fact, autophagic degradation may be affected by the loss of SQSTM1 at the same time as autophagic signals are being sent. Finally, we demonstrate that ULK1 and ULK2 protein levels are greatly reduced during PV infection, and ATG13, RB1CC1, and ATG101 protein levels are reduced as well. Surprisingly, autophagic signaling appears to increase as ULK1 levels decrease. Overexpression of wild-type or dominant-negative ULK1 constructs does not affect virus replication, indicating that ULK1 degradation may be a side effect of the ULK1-independent signaling mechanism used by PV, inducing complex instability. This demonstration of ULK1-independent autophagic signaling is novel and leads to a model by which the virus is signaling to generate autophagosomes downstream of ULK1, while at the same time, cleaving cargo receptors, which may affect cargo loading and autophagic degradative flux. Our data suggest that PV has a finely-tuned relationship with the autophagic machinery, generating autophagosomes without using the primary autophagy signaling pathway.

ObjectiveThrough metabolomics method, the objective of the paper is to differentially screen serum metabolites of GDM patients and healthy pregnant women, to explore potential biomarkers of GDM and analyze related pathways, and to explain the potential mechanism and biological significance of GDM.MethodsThe serum samples from 30 GDM patients and 30 healthy pregnant women were selected to conduct non-targeted metabolomics study by liquid chromatography-mass spectrometry. The differential metabolites between the two groups were searched and the metabolic pathway was analyzed by KEGG database.ResultsMultivariate statistical analysis found that serum metabolism in GDM patients was different significantly from healthy pregnant women, 36 differential metabolites and corresponding metabolic pathways were identified in serum, which involved several metabolic ways like, fatty acid metabolism, butyric acid metabolism, bile secretion, and amino acid metabolism.ConclusionThe discovery of these biomarkers provided a new theoretical basis and experimental basis for further study of the early diagnosis and pathogenesis of GDM. At the same time, LC-MS-based serum metabolomics methods also showed great application values in disease diagnosis and mechanism research.  相似文献

A postembedding labeling technique was employed to visualize human native low density lipoproteins (LDL) during transcytosis in rat arterial endothelium. For this purpose human LDL was perfused through rat vasculature before fixation and processing for immunoelectron microscopy. The LDL particles were located on sections by anti-human apolipoprotein B-100 (LDL) antibodies and secondary antibodies or protein-A conjugated to 10-nm colloidal gold. LDL molecules were seen in plasmalemmal vesicles as well as in the subendothelial space. No colloidal gold was found in the intercellular junctions. Perfusion with reductively methylated LDL, which cannot bind to the LDL receptor, gave a similar labeling pattern, indicating that transcytosis of LDL via plasmalemmal vesicles is most likely receptor independent. Furthermore, the passage of LDL through intact vascular endothelium is a vesicular transport rather than an intercellular diffusion process.  相似文献

Gestational diabetes mellitus (GDM) leads to poor pregnancy outcomes. The methods for GDM early diagnosis and treatment are still unknown. This study aimed to investigate the expression and diagnostic potential of miR-34b-3p in GDM patients and further analyzed the effects of miR-34b-3p on HUVECs viability and migration. The expression of miR-34b-3p was detected in HUVECs of GDM and normal pregnant women by qRT-PCR. Then the HUVECs were isolated from normal pregnant women. High glucose (HG) was used to treat the HUVECs to mimic the GDM in vitro. The cell viability and migration were determined by MTT, wound healing assay, and transwell assay. The interaction between miR-34b-3p and PDK1 was evaluated by luciferase activity assay. Our results showed that miR-34b-3p was up-regulated in HUVECs of GDM patients. Then the HUVECs were isolated from normal pregnant women and they were treated with HG to mimic the GDM in vitro. Interestingly, knockdown of miR-34b-3p restored the impairment of HG treatment-induced effects in HUVECs. More importantly, PDK1 was proved to be a potential target of miR-34b-3p. Finally, the rescue experiments confirmed that miR-34b-3p impaired cell viability and migration ability in HUVECs by targeting PDK1. These findings concluded that miR-34b-3p impaired HUVECs viability and migration in GDM by targeting PDK1, which might provide a novel perspective for the pathogenesis and underlying therapeutic target for GDM.

ObjectiveThe association between subclinical hypothyroidism (SCH) and gestational diabetes mellitus (GDM) is controversial. This review evaluates whether the risk of GDM is different in pregnant women with SCH compared to euthyroid pregnant women.MethodsA computerized search of the MEDLINE and EMBASE databases was conducted from their inceptions to July 2013 and was complemented with the perusal of the reference sections of the retrieved articles. Prespecified criteria were applied to assess eligibility, and standard meta-analytic methodology was employed for evidence synthesis.ResultsSix cohort studies, reporting data on 35,350 pregnant women (1,216 women with SCH), were identified. The risk of GDM in pregnant women with SCH was found to be substantially higher compared to euthyroid pregnant women (5 studies, pooled unadjusted odds ratio [OR]: 1.35, 95% confidence interval [CI]: 1.05-1.75, I2: 41%, Harbord test P = .44). Similarly, the risk of GDM was estimated to be significantly higher in pregnant women with SCH when using adjusted estimates (3 studies, pooled adjusted OR: 1.39, 95% CI: 1.07-1.79, I2: 0%). Neither finding remained significant in sensitivity analyses.ConclusionA modestly increased risk of GDM might be present in pregnant women with SCH compared to euthyroid pregnant women. Assuming a 5% baseline risk of GDM and that SCH increases the risk of GDM by 50% (in odds) compared to a euthyroid population, then there would be 1 extra case of GDM in every 43 pregnant women with SCH. BackgroundArsenic (As) exposure is one of the risk factors for gestational diabetes mellitus (GDM). This study aimed to explore the effect of As-exposure on DNA methylation in GDM and to establish a risk assessment model of GDM in As exposed pregnant women.MethodWe collected elbow vein blood of pregnant women before delivery to measure As concentration and DNA methylation data. Then compared the DNA methylation data and established a nomogram.ResultWe identified a total of 10 key differentially methylated CpGs (DMCs) and found 6 corresponding genes. Functions were enriched in Hippo signaling pathway, cell tight junction, prophetic acid metabolism, ketone body metabolic process, and antigen processing and presentation. A nomogram was established that can predict GDM risks (c-index = 0.595, s:p = 0.973).ConclusionWe found 6 genes associated with GDM with high As exposure. The prediction of the nomograms has been proven to be effective.  相似文献

Monocyte chemoattractant protein 1 (MCP-1) has been implicated as a key factor in the recruitment and activation of peripheral blood leukocytes in atherosclerotic lesions and adipose tissue. Elevated levels of circulating MCP-1 have been found in patients with type 1 and type 2 diabetes, as well as with coronary artery disease. In this study we compared serum MCP-1 concentrations between pregnant women with normal glucose tolerance (NGT), gestational diabetes mellitus (GDM) and non-pregnant healthy women. The group studied consisted of 62 patients with GDM (mean age 30.1 +/- 5.0 years) at 29.0 +/- 3.5 week of gestation, 64 pregnant women with NGT (mean age 30.0 +/- 4.7 years) at 29.2 +/- 2.9 week of gestation and 34 non-pregnant healthy women (mean age 29.8 +/- 4.7 years). Serum MCP-1 concentration was measured using an enzyme - linked immunosorbent assay. Median MCP-1 concentrations did not differ significantly between women with GDM (median 342.3 [interquartile range 267.9-424.4] pg/ml) and NGT (338.0 [274.7-408.2] pg/ml), but were markedly lower than those found in non-pregnant women (485.2 [409.6-642.4] pg/ml, p<0.0001). After adjusting for glucose, the difference between pregnant and non-pregnant women remained highly significant (p<0.0001). In GDM patients MCP-1 levels correlated significantly with fasting glucose (r=0.2665, p=0.0363), insulin (r=0.4330, p=0.0004), HOMA-IR (r=0.4402, p=0.0003), ISQUICKI (r=-0.4402, p=0.0003), HbA1c (r=0.2724, p=0.0322), as well as with prepregnancy and current BMI (r=0.3501, p=0.0057 and r=0.3250, p=0.0106, respectively). Multiple regression analysis revealed that MCP1 concentrations were significantly predicted only by plasma glucose ( beta=0.3489, p=0.00004). Our results suggest that MCP1 levels are decreased in pregnant women, irrespective of their glucose tolerance status.  相似文献

目的研究孕妇肠道微生物组成与孕妇以及新生儿糖脂代谢的相关性。方法选择2017年6月至2018年9月在大连市妇幼保健院定期产检的诊断为妊娠期糖尿病(GDM)的孕妇89例为GDM组,血糖正常孕妇96例为对照组。收集孕妇一般资料,孕妇和新生儿糖脂代谢物、孕妇粪便进行微生物检测。结果 GDM组孕妇空腹血糖、甘油三酯明显高于对照组,高密度脂蛋白水平明显低于对照组(均P<0.05)。GDM新生儿脂联素和血糖明显低于对照组,胰岛素和糖化血红蛋白明显高于对照组(均P<0.05)。对照组孕妇肠道微生物中拟杆菌(Bacteroides)丰富度最高(36.6%),其次是普雷沃菌(Prevotella)(15.3%)、柔嫩梭菌(Faecalibacterium)(10.2%)、考拉杆菌(Phascolarctobacterium)(7.2%)。GDM组孕妇肠道微生物中拟杆菌(Bacteroides)丰富度最高(29.4%),其次是柔嫩梭菌(Faecalibacterium)(19.7%)、普雷沃菌(Prevotella)(11.1%)、考拉杆菌(Phascolarctobacterium)(9.3%)。柔嫩梭菌(Faecalibacterium)与孕妇空腹血糖呈正相关(r=0.377 6,P=0.027 6),拟杆菌(Bacteroides)、埃希菌(Escherichia)与孕妇甘油三酯呈负相关(r=-0.027 5,-0.585 8,P=0.023 7,0.041 3)。柔嫩梭菌(Faecalibacterium)与新生儿空腹血糖呈负相关(r=-0.397 6,P=0.027 6)。结论 GDM孕妇肠道微生物构成与血糖正常孕妇不同,菌群丰富度和多样性较低,并且孕妇肠道菌群与其及新生儿的糖脂代谢相关,调节孕妇肠道菌群或有利于改善其与新生儿的糖脂代谢功能。  相似文献

Objective

Recent genetic studies have shown that potassium voltage-gated channel, KQT-like subfamily, member1 (KCNQ1) gene is related to gestational diabetes mellitus (GDM). However, studies for the rs2237892 polymorphism in KCNQ1 and GDM remain conflicting in Asians. Furthermore, associations of this polymorphism with glucose levels during oral glucose tolerance test (OGTT) have not been described in Chinese pregnant women. The present study aimed to provide evidence for the associations of rs2237892 in KCNQ1 with GDM and glucose levels, and to systematically evaluate the effect of rs2237892 on GDM in Asians.

Methods

A case-control study on 562 women with GDM and 453 controls was conducted in Beijing, China. The association of rs2237892 with risk of GDM was analyzed using logistic regression. The associations with quantitative glucose levels were assessed using linear regression models. A meta-analysis including the present case-control study and four previously published reports in Asians was conducted.

Results

The rs2237892 polymorphism in KCNQ1 was associated with GDM (OR (95%CI) =1.99(1.26-3.15)). Additionally, the polymorphism was associated with levels of 1h and 2h glucose during OGTT. The pre-pregnancy BMI, age and genotypes of KCNQ1 polymorphism were independent risk factors of GDM. Subsequently, we performed a meta-analysis in Asians. In total, C-allele carriers of rs2237892 polymorphism had a 50% higher risk for GDM (OR (95%CI) =1.50(1.15-1.78)).

Conclusion

The study demonstrated for the first time that the KCNQ1 rs2237892 polymorphism was associated with GDM and glucose levels in Chinese women. The study provides systematic evidence for the association between this polymorphism and GDM in Asians.  相似文献

目的:探讨组织多普勒Tei指数对妊娠期糖尿病(GDM)孕妇胎儿心功能的评估价值,并分析母婴预后与血糖控制水平的关系。方法:选取2017年1月到2018年6月期间在我院行产前检查的GDM孕妇80例,根据血糖控制情况将其分为良好组(48例)和不良组(32例)。另选取同期在我院行产前检查的并在我院生产的健康孕妇50例作为对照组。比较三组研究对象的等容收缩时间(ICT)、等容舒张时间(IRT)、射血时间(ET)、Tei指数、妊娠结局以及新生儿并发症。结果:不良组的ET小于良好组和对照组,Tei指数大于良好组和对照组,差异均有统计学意义(P0.05),不良组的巨大儿、早产、羊水过多、胎儿窘迫的发生率高于良好组和对照组,差异均有统计学意义(P0.05),不良组的新生儿低血糖、高胆红素血症的发生率高于良好组和对照组,差异均有统计学意义(P0.05)。结论:组织多普勒Tei指数对GDM孕妇胎儿心功能具有较高的评估价值,可敏感地检测出胎儿心功能异常,血糖控制不良会明显增加不良妊娠结局和新生儿并发症的发生率。  相似文献

ObjectiveTo explore the prevalence of gestational diabetes mellitus (GDM), defined by the previous criteria of the American Diabetes Association (ADA), as well as the criteria suggested by the International Association of Diabetes and Pregnancy Study Groups (IADPSG), in an unselected group of urban Mexican pregnant women and to analyze the frequency of large for gestational age (LGA) newborns in this same group of women with use of both diagnostic criteria.MethodsA cross-sectional study included 803 consecutive Mexican urban women with a singleton pregnancy, without concomitant diseases and no prior history of GDM, who underwent a 2-step screening protocol for diagnosis of GDM at admission to prenatal care.ResultsThe ADA criteria identified 83 women (10.3%) whereas the IADPSG criteria diagnosed 242 women (30.1%) having GDM (P = .0001). Fasting glucose concentrations during the 100-g 3-hour oral glucose tolerance test were abnormal in 116 women (14.4%) and in 160 women (19.9%) on the basis of ADA and IADPSG criteria, respectively (P = .004). The frequency of LGA newborns was 7.4% based on IADPSG criteria and 6.0% based on ADA criteria—no significant difference (P = .64).ConclusionWith use of the IADPSG criteria, the prevalence of GDM increased almost 3-fold in comparison with that for the ADA criteria. Efforts to determine a link between diabetes and atherosclerosis have involved examining the effect of high glucose levels on the adhesion and migration of circulating leukocytes, mostly monocytes and T lymphocytes. Leukocyte differentiation and proliferation within the subendothelial space can also be investigated by the use of a 3D in vitro human vascular tissue model. This model was used to study the effect of short-term, high glucose concentration on certain cell behavior associated with the early stages of atherosclerosis. Samples were exposed to either a 30- or 5.6-mM glucose concentration for 9 h to represent either hyperglycemic or normoglycemic conditions, respectively. There was a significant increase in vascular cell adhesion molecule-1 expression on the endothelial cells exposed to a 30-mM compared to a 5.6-mM glucose concentration. There was no significant difference in either intercellular adhesion molecule-1 or E-selectin expression on the endothelial cells exposed to a 30-mM compared to a 5.6-mM glucose concentration. After the endothelium was exposed to 30 mM glucose concentration, there was a 70% increase in the number of monocytes (CD14+) migrating across the endothelium and a 28% increase in the number of these monocytes differentiating into macrophages, compared to cell migration and differentiation across the endothelium exposed to 5.6 mM glucose concentration. Also, for the endothelium exposed to 30 mM glucose concentration, there were nearly 2.5 times more T lymphocytes that migrated across the endothelium, along with significant cell proliferation, compared to cell migration across the endothelium exposed to 5.6 mM glucose concentration.  相似文献

目的:探讨妊娠期糖尿病(GDM)孕妇血清皮质醇激素抑制素(CST)、humanin、血管黏附蛋白-1(VAP-1)与糖脂代谢及胰岛素抵抗的关系。方法:选择2017年1月至2019年10月我院妇产科门诊收治的79例GDM患者(GDM组),另选取同期到我院行产检的52例正常妊娠孕妇(NGT组)。检测并比较血清CST、humanin、VAP-1水平,分析CST、humanin、VAP-1与GDM患者糖脂代谢、胰岛素抵抗的相关性,并作二元Logistic回归分析探讨GDM发病的危险因素。结果:GDM组血清CST、humanin、空腹C肽(FC-P)、C-P峰值/FC-P、胰岛素β细胞功能指数[HOMA-β(C-P)]水平低于NGT组(P<0.05),VAP-1、空腹血糖(FPG)、胰岛素抵抗指数[HOMA-IR(C-P)]、甘油三酯(TG)、低密度脂蛋白胆固醇(LDL-C)水平高于NGT组(P<0.05)。Pearson相关性分析结果显示血清CST水平与FPG、HOMA-IR(C-P)呈负相关(P<0.05),与FC-P、C-P峰值/FC-P、HOMA-β(C-P)呈正相关(P<0.05)。humanin水平与TG、FPG、HOMA-IR(C-P)呈负相关,与FC-P、C-P峰值/FC-P、HOMA-β(C-P)呈正相关(P<0.05)。VAP-1与TG、FPG、HOMA-IR(C-P)呈正相关(P<0.05),与FC-P、C-P峰值/FC-P、HOMA-β(C-P)呈负相关(P<0.05)。二元Logistic回归分析结果显示CST、humanin、HOMA-β(C-P)水平降低,年龄、BMI、LDL-C、VAP-1、HOMA-IR(C-P)水平升高是GDM发病的危险因素(P<0.05)。结论:GDM患者血清CST、humanin水平降低,VAP-1水平升高,三者均参与GDM发病和胰岛素抵抗。CST与糖代谢紊乱有关,humanin、VAP-1与糖脂代谢异常有关。  相似文献
