目的:探讨类风湿关节炎(RA)患者血清类风湿性因子(RF)、抗核抗体(ANA)、抗环瓜氨酸肽(CCP)抗体、免疫球蛋白(Ig)、补体(C3、C4)以及炎症因子的水平及临床意义。方法:收集2016年9月至2017年4月我院收治的165例RA患者为RA组,其中RA活动期患者93例(RA活动组),RA缓解期患者72例(RA缓解组),并于同期随机选取30例健康体检者为对照组。采用免疫散射比浊法检测各组血清RF、IgM、IgG、IgA、C3、C4水平,采用酶联免疫吸附法(ELISA)检测各组血清CCP抗体,电化学发光法检测白介素-6(IL-6)、化学发光法检测白介素-8(IL-8)。免疫荧光法检测ANA。比较不同组别各检测指标水平,并分析RA患者RF、ANA、CCP抗体、Ig、C3、C4与炎症因子的相关性。结果:RA活动组、RA缓解组血清RF、ANA、CCP抗体、IgM、IgG、IgA、IL-6、IL-8水平高于对照组,且RA活动组血清RF、ANA、CCP抗体、IgM、IgG、IgA、IL-6、IL-8水平高于RA缓解组,差异均有统计学意义(P0.05)。RA活动组、RA缓解组血清C3、C4水平低于对照组,且RA活动组血清C3、C4水平低于RA缓解组,差异均有统计学意义(P0.05)。经Pearson积矩相关分析,RA活动期和缓解期患者血清RF、ANA、CCP抗体、IgM、IgG、IgA与炎症因子IL-6、IL-8呈正相关关系(P0.05),血清C3、C4与炎症因子IL-6、IL-8呈负相关关系(P0.05)。结论:RA患者体内RF、ANA、CCP抗体、Ig及IL-6、IL-8水平明显较高,C3,C4水平明显较低,活动期RA患者更为显著,联合检测可早期辅助诊断RA及判断病情进展,在临床上有重要的参考意义。  相似文献

目的:探讨阿莫西林联合甲强龙治疗老年支气管肺炎患者的临床效果及其可能的作用机制。方法:选取我院呼吸科收治的 老年支气管肺炎患者84例,根据治疗方案不同分为常规组及试验组。比较两组患者治疗前后临床体征、IgA、IgM、IgG、CD4+及 CD8+T 淋巴细胞、NK 细胞及血清LDH3、LDH4、HBDH水平的变化情况。结果:治疗后,试验组咳喘、高热、肺内干湿罗音的恢复 时间明显短于常规组,IgA、IgM、IgG、LDH、HBDH、NK 细胞及CD8+T 淋巴细胞水平明显低于常规组,CD4+T 淋巴细胞水平明显 高于常规组,差异均具有统计学意义(P<0.05)。结论:阿莫西林联合甲强龙可有效快速改善老年支气管肺炎患者的临床症状,这可 能与其降低IgA、IgM、IgG、LDH、HBDHNK 细胞及CD8+T 淋巴细胞水平及提高CD4+T 淋巴细胞水平有关。  相似文献

Objectives

Fc receptors (FcR) interacting with immune complexes (ICs) is a central event in the immune pathogenesis of rheumatoid arthritis (RA). Here we asked if a specific FcR is linked to RA pathogenesis and if FcR activities relate to disease and treatment outcome in early RA.

Material and Methods

Twenty autoantibody-positive RA patients and 33 HC were included. The patients were evaluated before and after treatment with methotrexate and prednisolone. At follow-up, the EULAR response criteria were applied to determine the individual treatment outcomes. Serum immunoglobulin levels were measured and the expression of FcR for IgG (FcγR) and IgA (FcαR) on peripheral blood monocytes were determined by flow cytometry. The monocytic FcγR function was evaluated by human IgG1 and IgG3 IC-binding and TNFα stimulated release. Plasma levels of soluble FcRs (sFcRs) were determined with ELISA.

Results

The IgG1 and IgG3 levels were elevated in the RA sera. The RA monocytes expressed more CD64 and cell surface-bound IgG than HC monocytes, and showed an impaired FcγR function as reflected by changes in IC-binding and decreased IC-stimulated TNFα secretion. These findings correlated significantly with different disease activity markers. Furthermore, sFcRs were elevated in the patient plasma, and sCD64 was specific for RA (compared with a reference group of patients with active psoriatic arthritis). Following treatment, immunoglobulins and sFcR levels were reduced, whereas membrane CD64 was only decreased in patients with good response to treatment.

Conclusions

Early RA patients display increased membrane and soluble CD64 and an impaired FcγR function correlating with joint disease activity. Beneficial responses of anti-rheumatic treatment in patients reduce CD64. These data suggest sCD64 as an important objective biomarker in RA.  相似文献

目的

探讨肠道菌群与过敏性紫癜性肾炎患儿肾功能及免疫功能的相关性研究。

方法

选取2021年9月—2023年9月于本院治疗的85例过敏性紫癜性肾炎患儿,标记为研究组。并纳入同期进行健康体检的68例同年龄段健康儿童作为对照组。采集患者空腹静脉血和粪便,采用qRT-PCR法定量分析肠道中大肠杆菌和双歧杆菌的数量,计算双歧杆菌/大肠杆菌(B/E)比值。使用全自动特定蛋白分析系统ARRAY360和流式细胞仪检测IgM、IgG、IgA、外周血CD4+ T淋巴细胞和CD8+ T淋巴细胞,计算CD4+/CD8+比值。过敏性紫癜性肾炎患儿肠道菌群与肾功能及免疫功能之间的相关性分析采用Pearson分析。

结果

两组的性别、年龄、体质量指数差异均无统计学意义(P<0.05)。与对照组相比,研究组大肠杆菌数量增加(P<0.05),血肌酐、尿素氮、血清尿酸、IgM、IgG、IgA、CD8+ T细胞水平升高(P<0.05),但双歧杆菌数量降低(P<0.05),B/E比值、CD4+ T细胞、CD4+/CD8+比值显著降低(P<0.05)。过敏性紫癜性肾炎患儿肠道中的大肠杆菌数量分别与血肌酐、尿素氮、血清尿酸、IgM、IgG、IgA、CD8+ T细胞水平呈正相关(P<0.001),与CD4+ T细胞水平、CD4+/CD8+比值呈负相关(P<0.001);双歧杆菌、B/E比值分别与血肌酐、尿素氮、血清尿酸、IgM、IgG、IgA、CD8+ T细胞水平呈负相关(P<0.001),与CD4+ T细胞、CD4+/CD8+比值呈正相关(P<0.001)。

结论

过敏性紫癜性肾炎患儿肠道B/E比值显著降低,并与其肾功能及免疫功能指标存在相关性。

The possible role of abnormal T cell-dependent B-cell activation in Graves' disease was investigated by comparing lymphocyte subset distribution and the production of soluble CD8 (sCD8), sCD23, IL-10 and IL-12 by peripheral blood cells (PBMC) and thyroid-infiltrating lymphocytes (TL) in vitro. In TL, the percentage of CD8(+) cells was slightly higher and the sCD8 concentration was significantly higher than in PBMC. The ratio CD23(+) cells to CD20(+) cells (activated B/pan B cells) was increased in TL compared to PBMC from Graves' or normal controls, although the percentage of CD20(+) cells was decreased. Compared to PBMC in Graves' disease, the relative ratio of IL-10 to IL-12 release (IL-10/IL-12) by unstimulated TL was increased, despite a lack of significant difference between PBMC and TL in mean values for either IL-10 or IL-12 secretion. Incubating PBMC with a combination of anti-CD40 monoclonal antibodies and interleukin-4 (IL-4) resulted in B cell activation, reflected in an increase in the sCD23 level in both controls and Graves' patients, but especially prominent in the latter. Stimulation with anti-CD40 antibody and IL-4 also decreased the percentage of CD8(+) cells in PBMC but not TL from both Graves' disease and normal controls, and the percentage of CD8(+) cells in TL was higher than PBMC after the stimulation. The sCD23 concentration in TL was decreased compared to PBMC both in patients with Graves' disease and normal controls. However, in contrast to the increased responses observed in Graves' PBMC or normal controls after stimulation, sCD23 levels remained the same in stimulated TL from Graves' patients. This combination of B cell stimulants increased production of IL-10 in PBMC but not in TL obtained from patients with Graves' disease, and the increased IL-10/IL-12 ratio declined to a value no different from that in PBMC group after stimulation. Thus, T cell-dependent B-cell activation via a CD40 pathway may cause a shift in the Th(1)/Th(2) balance to Th(2) dominance in Graves' disease, while increased CD8(+) cells in TL may suppress sCD23 production and IL-10-producing Th(2) cells.  相似文献

The northern pig-tailed macaque(NPM,Macaca leonina) has become a widely used animal model in biomedical research. In this study, we measured serum immunoglobulin IgG, IgM, IgA, complement C3, C4 and CRP levels in 3-11 year old captive northern pig-tailed macaques using HITACHI 7600-20 automated chemistry analyzer in order to determine the influences of age and gender on these items. The results showed that serum IgA, IgM, C3 and C4 levels were not correlated with age(P0.05), while serum IgG levels increased progressively with age(r=0.202;P=0.045). Serum IgG, IgA, IgM and C3 levels were higher in females than in males(P0.05). Moreover, serum C3 concentration was both positively and strongly correlated with that of C4(r=0.700; P0.0001). This study provides basic serum immunoglobulin and complement data of captive northern pig-tailed macaques, which may prove useful for future breeding efforts and biomedical research.  相似文献

目的:分析氯雷他定对急性支气管炎患儿免疫功能及炎症反应的影响。方法:收集我院收治的急性支气管炎患儿100例,将其随机分为对照组与观察组,每组50例。对照组患儿给予常规治疗;观察组患儿给予常规治疗联合氯雷他定治疗。比较两组患儿治疗前后动脉氧分压(PaO_2)、二氧化碳分压(PaCO_2)、CD3~+、CD4~+、CD4~+/CD8~+、免疫球蛋白A(IgA)、免疫球蛋白G(IgG)、免疫球蛋白M(IgM)、血清降钙素原(PCT)、C反应蛋白(CRP)及白介素-6(IL-6)水平的变化。结果:治疗后,观察组患儿PaO_2水平明显高于对照组,PaCO_2水平明显低于对照组(P0.05);两组患儿细胞免疫及体液免疫水平较治疗前比较均显著升高(P0.05),且观察组患儿CD3~+、CD4~+、CD4~+/CD8~+、IgA、IgG及IgM水平明显高于对照组患儿(P0.05)。治疗后,两组患儿血清中PCT、CRP及IL-6水平均较治疗前显著降低,且观察组患儿上述炎症因子水平明显低于对照组患儿(P0.05)。结论:氯雷他定治疗小儿急性支气管炎可显著提高患儿的免疫功能,抑制炎症因子释放,缓解炎症反应,临床效果显著。  相似文献

We studied dipeptidyl peptidase IV (DPP-IV, CD26) expression in different T helper cells and serum soluble DPP-IV/sCD26 levels in rheumatoid arthritis (RA) patients, correlated these with disease activity score (DAS), and examined how they were affected by different therapies, conventional or biological (anti-TNF, anti-CD20 and anti-IL6R or Ig-CTLA4). The percentage of CD4+CD45R0+CD26- cells was greatly reduced in patients (up to 50%) when compared with healthy subjects. Three other subsets of CD4 cells, including a CD26high Th1-associated population, changed variably with therapies. Data from these subsets (frequency and staining density) significantly correlated with DAS28 or DAS28 components but different in each group of patients undergoing the different therapies. Th17 and Th22 subsets were implicated in RA as independent CCR4+ and CCR4- populations each, with distinct CD26 expression, and were targeted with varying efficiency by each therapy. Serum DPP-IV activity rather than sCD26 levels was lower in RA patients compared to healthy donors. DPP-IV and sCD26 serum levels were found related to specific T cell subsets but not to disease activity. We conclude that, according to their CD26 expression, different cell subsets could serve to monitor RA course, and an uncharacterized T helper CD26- subset, not targeted by therapies, should be monitored for early diagnosis.  相似文献

目的:研究糖皮质激素联合吗替麦考酚酯分散片治疗系统性红斑狼疮患者的临床疗效及对免疫功能的影响。方法:选取2014年9月至2015年8月本院收治的84例系统性红斑狼疮患者,根据投硬币法分为观察组和对照组,42例每组。对照组使用单纯的糖皮质激素,观察组在此基础上使用吗替麦考酚酯分散片。比较两组患者临床疗效,治疗前后免疫球蛋白A(IgA)、免疫球蛋白(IgG)、免疫球蛋白(IgM)、C反应蛋白(CRP)、血沉(ESR)的变化及不良反应的发生情况。结果:治疗后,观察组临床总有效率显著高于对照组(P0.05)。治疗前,两组患免疫球蛋白A(IgA)、免疫球蛋白(IgG)、免疫球蛋白(IgM)、C反应蛋白(CRP)、血沉(ESR)水平比较差异均无统计学意义(P0.05);治疗后,两组患者IgA、IgG、IgM、CRP、ESR水平均较治疗前显著降低(P0.05),与对照相比,观察组的IgA、IgG、IgM、CRP、ESR明显降低(P0.05)。两组组的不良反应率比较差异无统计学意义(P0.05)。结论:糖皮质激素联合吗替麦考酚酯分散片能有效改善系统性红斑狼疮患者的免疫功能,临床疗效良好,安全性高。  相似文献

The present study demonstrates that neonatal human lymphocytes that are incapable of producing IgG and IgA antibodies may differentiate into IgE-secreting cells under the influence of IL4. Indeed, the addition of recombinant IL4 to cultures of unfractionated umbilical cord blood mononuclear cells (CBMC) induces a dose-dependent synthesis of IgE but not of the other classes of Ig. Moreover, IgE-secreting B lymphoblastoid cell lines can be derived from neonatal lymphocytes costimulated with EBV and IL4. Comparison of the mechanisms regulating the in vitro IgE synthesis by adult and neonatal lymphocytes indicates that in most cases IFN-gamma markedly potentiates the IgE synthesis in CBMC cultures whereas it has a reverse effect on adult lymphocytes. These reciprocal effects of IFN-gamma are specifically blocked by a neutralizing mAb to IFN-gamma; they are dose-dependent and they are observed when IFN-gamma is added at the initiation of the culture or shortly thereafter. Moreover, in a small number of cases IFN-gamma may also potentiate IgE synthesis by adult lymphocytes. The potentiation or the suppression of IgE synthesis by IFN-gamma is not explained by a differential effect of IFN-gamma on the production of soluble CD23 (sCD23); indeed in both cases IFN-gamma slightly increases the IL4-induced production of sCD23. Moreover, the spontaneous and the IL4-induced production of sCD23 by CBMC is comparable to that of adult lymphocytes. The IgE response is dependent upon the expression of Fc epsilon RII (CD23) inasmuch as it is specifically blocked by anti-CD23 mAb.  相似文献

The aim of the present study was to compare, during the pollen season, serum levels of total IgE and soluble CD23 (sCD23) from patients with allergic bronchial asthma, with those from healthy subjects. Significantly higher levels of total IgE and sCD23 were found in patients with asthma compared to the control group. Both in normal controls and in asthmatic patients, a significant correlation was shown between the levels of these two molecules. In asthmatic patients, significant correlations were found for both total IgE and sCD23, with lung function measured as bronchial responsiveness to inhaled methacholine. These results suggest that in asthmatic patients, in addition to the study of total serum IgE levels, the assessment of sCD23 serum levels may be helpful in the evaluation of disease activity.  相似文献

The aim of the present study was to compare serum levels of soluble forms of interleukin-2 receptor, CD4 and CD8, released by lymphocytes during activation ofthe immune system, in patients with histologically verified chronic active hepatitis associated to hepatitis C virus infection, with those in healthy subjects. Significantly higher levels of soluble IL-2R and soluble CD8 were found in patients with chronic active hepatitis compared with controls. In contrast no difference was found for soluble CD4 values in the two groups. No correlations were found for both sIL-2R and sCD8 and these two molecules with other parameters of liver function. These results indicate that in these patients there is a general activation of the immune system, but the lack of correlation with parameters of liver function strengthens the suggestion that this activation does not play a role in the pathogenesis of chronic type C hepatitis.  相似文献

IntroductionRheumatoid arthritis–associated autoantibodies (RA-AAB) can be present in serum years before clinical onset of rheumatoid arthritis (RA). It has been hypothesized that initiation of RA-AAB generation occurs at inflamed mucosal surfaces, such as in the oral cavity or lungs. The aim of this study was to assess systemic presence of RA-AAB in patients without RA who had oral or lung mucosal inflammation.MethodsThe presence of RA-AAB (immunoglobulin A [IgA] and IgG anti-cyclic citrullinated peptide 2 antibodies (anti-CCP2), IgM and IgA rheumatoid factor (RF), IgG anti-carbamylated protein antibodies and IgG and IgA anti-citrullinated peptide antibodies against fibrinogen, vimentin and enolase) were determined in sera of non-RA patients with periodontitis (PD, n = 114), bronchiectasis (BR, n = 80) or cystic fibrosis (CF, n = 41). Serum RA-AAB levels were compared with those of periodontally healthy controls (n = 36). Patients with established RA (n = 86) served as a reference group. Association of the diseases with RA-AAB seropositivity was assessed with a logistic regression model, adjusted for age, sex and smoking.ResultsLogistic regression analysis revealed that IgG anti-CCP seropositivity was associated with BR and RA, whereas the association with PD was borderline significant. IgA anti-CCP seropositivity was associated with CF and RA. IgM RF seropositivity was associated with RA, whereas the association with BR was borderline significant. IgA RF seropositivity was associated with CF and RA. Apart from an influence of smoking on IgA RF in patients with RA, there was no influence of age, sex or smoking on the association of RA-AAB seropositivity with the diseases. Anti-CarP levels were increased only in patients with RA. The same held for IgG reactivity against all investigated citrullinated peptides.ConclusionAlthough overall levels were low, RA-AAB seropositivity was associated with lung mucosal inflammation (BR and CF) and may be associated with oral mucosal inflammation (PD). To further determine whether mucosal inflammation functions as a site for induction of RA-AAB and precedes RA, longitudinal studies are necessary in which RA-AAB of specifically the IgA isotype should be assessed in inflamed mucosal tissues and/or in their inflammatory exudates.  相似文献

IgE induction from human cells has generally been considered to be T cell dependent and to require at least two signals: IL-4 stimulation and T cell/B cell interaction. In the present study we report a human system of T cell-independent IgE production from highly purified B cells. When human cells were co-stimulated with a mAb directed against CD40 (mAb G28-5), there was induction of IgE secretion from purified blood and tonsil B cells as well as unfractionated lymphocytes. Anti-CD40 alone failed to induce IgE from blood mononuclear cells or purified B cells. The effect of the combination of anti-CD40 and IL-4 on IgE production was very IgE isotype specific as IgG, IgM, and IgA were not increased. Furthermore, anti-CD40 with IL-5 or PWM did not co-stimulate IgG, IgM, or IgA and in fact strongly inhibited PWM-stimulated IgG, IgM and IgA production from blood or tonsil cells. IgE synthesis induced by anti-CD40 plus IL-4 was IFN-gamma independent as is the in vivo production of IgE in humans; the doses of IFN-gamma that profoundly suppressed IgG synthesis induced by IL-4, or IL-4 plus IL-6, had no inhibitory effect on anti-CD40-induced IgE production. Anti-CD23 and anti-IL-6 also could not block anti-CD40 plus IL-4-induced IgE production, but anti-IL-4 totally blocked their effect. IgE production via CD40 was not due to IL-5, IL-6 or nerve growth factor as none of these synergized with IL-4 to induce IgE synthesis by purified B cells. Finally, we observed that CD40 stimulation alone could enhance IgE production from in vivo-driven IgE-producing cells from patients with very high IgE levels; cells that did not increase IgE production in response to IL-4. Taken together, our data suggest that the signals delivered for IgE production by IL-4 and CD40 stimulation may mimic the pathway for IgE production seen in vivo in human allergic disease.  相似文献

Epidemiological studies suggest various kinds of immune dysregulation in hemodialysis (HD) patients. The aim of this study was to investigate the relationship between immune functions and nutritional status of HD patients. We studied 54 patients with ESRD on chronic HD, included 34 females and 20 males with mean age 46.6 +/- 16.3 (18-77) years. We measured the height and dry weight of all patients. The BMI was calculated by dividing weight (kg) by height squared (m(2)). In all patients serum urea, creatinine, albumin, iron, cholesterol, triglyceride, CRP, IgG, IgM, IgA, CD4, CD8, CD19, CD16-56 lymphocytes were measured. Kt/V values were calculated according to DOQI guideline. In this study, a positive correlation between albumin, cholesterol, and triglyceride levels as nutritional parameters and immune functions in terms of total and subtype lymphocyte counts was observed. Further prospective studies are needed to determine the clinical importance of this finding and the appropriate means of measurement and effects of nutrition on immune function in hemodialysis patients.  相似文献

目的:探讨加速康复外科(ERAS)对腹腔镜胃癌根治术患者营养状态,免疫功能及炎性因子水平的影响。方法:将2016年1月~2020年1月于我院行腹腔镜胃癌根治术的胃癌患者166例纳入本研究,按照随机数字表法分为ERAS组(n=83)与对照组(n=83),对照组行常规处理,ERAS组以ERAS处理。观察两组术后1 d、7 d血红蛋白(Hb)、视黄醇结合蛋白(RbP)、转铁蛋白(TRF)、前白蛋白(PRE)、白蛋白(ALB)等营养指标,免疫球蛋白A(Ig A)、免疫球蛋白G(Ig G)、免疫球蛋白M(Ig M)等免疫球蛋白,CD3+、CD4+、CD8+、CD4+/CD8+等T细胞亚群,白介素-6(IL-6)、白介素-8(IL-8)、白介素-10(IL-10)等炎性因子及术后并发症等指标。结果:与术后1 d比较,两组术后7 d Hb、RbP、TRF、PRE、ALB、Ig A、Ig G、Ig M,CD3+、CD4+、CD4+/CD8+水平均升高,CD8+、IL-6、IL-8、IL-10水平均降低(P<0.05)。术后7 d,ERAS组Hb、RbP、TRF、PRE、ALB、Ig A、Ig G、Ig M、CD3+、CD4+、CD4+/CD8+水平均高于对照组,CD8+、IL-6、IL-8、IL-10低于对照组(P<0.05)。ERAS组术后并发症发病率(8.43%)低于对照组(22.89%),差异有统计学意义(P<0.05)。结论:ERAS应用于腹腔镜胃癌根治术中可有效降低患者机体应激反应,改善营养状态,解除免疫抑制,清除炎性因子,减少术后并发症,有助于患者康复。  相似文献
