摘要 目的:探讨老年男性腹股沟疝与肌肉质量、肌力及腹内压力(IAP)的相关性。方法:选取2021年1月-2023年1月在眉山市人民医院普外一科就诊的 86例老年男性腹股沟疝患者作为研究组,选取同期100名老年体检者作为对照组,对两组研究对象的腹部肌肉质量、肌力及IAP进行检测,并探讨其与腹股沟疝发病风险的相关性。结果:研究组患者腹部骨骼肌肉面积值(SMA)、骨骼肌指数(SMI)水平及右手握力、左手握力均低于对照组,腹内脂肪面积、SMI异常比例及直立加压时腹内压(OVIAP)、直立加压前后腹内压差(OVIAPD)、平卧与直立加压时腹内压差(OSVIAPD)均高于对照组,差异均有统计学意义(P<0.05)。Logistic多元回归模型分析结果显示,老年腹股沟疝的发生与SMI水平、SMI异常、右手握力、OVIAPD、OSVIAPD具有相关性(P<0.05)。结论:老年男性腹股沟疝患者存在腹部肌肉质量和肌力的下降,患者在直立做加压动作时可出现IAP水平的升高,上述指标均与腹股沟疝的发生具有相关性,临床可采用针对性的综合干预措施以降低老年男性人群腹股沟疝的发病风险。  相似文献

Objectives

We aimed to assess the correlation between muscle strength and muscle mass based on sex and age, and their association with walking speed, which is a health predictor for independent living, in elderly Japanese individuals.

Methods

The participants included 318 (111 men, 207 women) community-dwelling elderly Japanese individuals aged ≥65 years. Knee extension strength was assessed as an indicator of muscle strength, and bioelectrical impedance analysis was used to measure muscle mass. The maximum walking speed of participants was recorded. All measurements were categorized based on sex and age groups as follows: young-old (age, 65–74 years) and old-old (age, ≥75 years).

Results

Appendicular muscle mass and knee extension strength decreased with age in both men and women. In men, knee extension strength showed significant positive correlations with leg and appendicular muscle mass in both young-old and old-old age groups. However, in women, only the old-old age group showed significant positive correlations between knee extension strength and leg and appendicular muscle mass. Muscle strength was significantly positively correlated with maximum walking speed in all groups, whereas muscle mass was not significantly correlated with maximum walking speed in men and women.

Conclusions

Muscle strength was significantly correlated with muscle mass in both age groups in men. However, in women, the correlation between muscle strength and muscle mass differed according to age. This finding suggests that the relationship between muscle strength and muscle mass differs according to sex and age. Muscle strength showed significant correlation with walking speed in both men and women in both age groups. These findings suggest that it is necessary to recognize that muscle strength is different from muscle mass, and that an individualized approach to prevent decline of muscle strength and muscle mass is necessary for health promotion in elderly.  相似文献

Objective

To investigate the associations of body mass index (BMI) and grip strength with objective measures of physical performance (chair rise time, walking speed and balance) including an assessment of sex differences and non-linearity.

Methods

Cross-sectional data from eight UK cohort studies (total N = 16 444) participating in the Healthy Ageing across the Life Course (HALCyon) research programme, ranging in age from 50 to 90+ years at the time of physical capability assessment, were used. Regression models were fitted within each study and meta-analysis methods used to pool regression coefficients across studies and to assess the extent of heterogeneity between studies.

Results

Higher BMI was associated with poorer performance on chair rise (N = 10 773), walking speed (N = 9 761) and standing balance (N = 13 921) tests. Higher BMI was associated with stronger grip strength in men only. Stronger grip strength was associated with better performance on all tests with a tendency for the associations to be stronger in women than men; for example, walking speed was higher by 0.43 cm/s (0.14, 0.71) more per kg in women than men. Both BMI and grip strength remained independently related with performance after mutual adjustment, but there was no evidence of effect modification. Both BMI and grip strength exhibited non-linear relations with performance; those in the lowest fifth of grip strength and highest fifth of BMI having particularly poor performance. Findings were similar when waist circumference was examined in place of BMI.

Conclusion

Older men and women with weak muscle strength and high BMI have considerably poorer performance than others and associations were observed even in the youngest cohort (age 53). Although causality cannot be inferred from observational cross-sectional studies, our findings suggest the likely benefit of early assessment and interventions to reduce fat mass and improve muscle strength in the prevention of future functional limitations.  相似文献

The apoB RNA-editing enzyme, catalytic polypeptide-like (APOBEC) family of proteins includes APOBEC1, APOBEC3, and activation-induced deaminase, all of which are zinc-dependent cytidine deaminases active on polynucleotides and involved in RNA editing or DNA mutation. In contrast, the biochemical and physiological functions of APOBEC2, a muscle-specific member of the family, are unknown, although it has been speculated, like APOBEC1, to be an RNA-editing enzyme. Here, we show that, although expressed widely in striated muscle (with levels peaking late during myoblast differentiation), APOBEC2 is preferentially associated with slow-twitch muscle, with its abundance being considerably greater in soleus compared with gastrocnemius muscle and, within soleus muscle, in slow as opposed to fast muscle fibers. Its abundance also decreases following muscle denervation. We further show that APOBEC2-deficient mice harbor a markedly increased ratio of slow to fast fibers in soleus muscle and exhibit an ∼15–20% reduction in body mass from birth onwards, with elderly mutant animals revealing clear histological evidence of a mild myopathy. Thus, APOBEC2 is essential for normal muscle development and maintenance of fiber-type ratios; although its molecular function remains to be identified, biochemical analyses do not especially argue for any role in RNA editing.  相似文献

Since it has been suggested that lipid peroxidation following free radical overproduction may be one of the causes of physical exercise-induced myopathies and hemolysis in horses, we looked for the possible relationships between these phenomena and muscle fiber damage. We use a homogeneous group of Maremmana stallions which, after a 3-month training period, underwent a series of physical exercises of increasing intensity. We determined the contents of malondialdehyde (MDA), one of the main lipid peroxidation end-products, and glutathione the substrate of one of the most important free radical scavenger enzymes. We also measured creatine phosphokinase and serum lactate dehydrogenase isoenzyme activities whose modification may be indicative of muscle fiber damage. The results obtained indicated that the physical exercise we adopted was able to modify both MDA and glutathione contents in blood. However, its effect on some LDH isoenzyme activities suggested possible damage to tissues other than muscle.  相似文献

Cachexia occurs in patients with advanced cancers. Despite the adverse clinical impact of cancer-induced muscle wasting, pathways causing cachexia are controversial, and clinically reliable therapies are not available. A trigger of muscle protein loss is the Jak/Stat pathway, and indeed, we found that conditioned medium from C26 colon carcinoma (C26) or Lewis lung carcinoma cells activates Stat3 (p-Stat3) in C2C12 myotubes. We identified two proteolytic pathways that are activated in muscle by p-Stat3; one is activation of caspase-3, and the other is p-Stat3 to myostatin, MAFbx/Atrogin-1, and MuRF-1 via CAAT/enhancer-binding protein δ (C/EBPδ). Using sequential deletions of the caspase-3 promoter and CHIP assays, we determined that Stat3 activation increases caspase-3 expression in C2C12 cells. Caspase-3 expression and proteolytic activity were stimulated by p-Stat3 in muscles of tumor-bearing mice. In mice with cachexia caused by Lewis lung carcinoma or C26 tumors, knock-out of p-Stat3 in muscle or with a small chemical inhibitor of p-Stat3 suppressed muscle mass losses, improved protein synthesis and degradation in muscle, and increased body weight and grip strength. Activation of p-Stat3 stimulates a pathway from C/EBPδ to myostatin and expression of MAFbx/Atrogin-1 and increases the ubiquitin-proteasome system. Indeed, C/EBPδ KO decreases the expression of MAFbx/Atrogin-1 and myostatin, while increasing muscle mass and grip strength. In conclusion, cancer stimulates p-Stat3 in muscle, activating protein loss by stimulating caspase-3, myostatin, and the ubiquitin-proteasome system. These results could lead to novel strategies for preventing cancer-induced muscle wasting.  相似文献

Eleven healthy males were studied twice. On one occasion (control, C), they slept (night 1) and then underwent a battery of tests at 4h intervals from 06: 00 day 1 to 02: 00 day 2; then, after a normal sleep (night 2), they were tested from 10: 00 to 22: 00 on day 2. On the second occasion (sleep deprivation, SD), the subjects remained awake during night 1. Each battery of tests consisted of measurements of tympanic membrane temperature, profile of mood states (POMS), muscle strength, self-chosen work rate (SCWR), perceived exertion, and heart rate (HR) while exercising on a stationary cycle ergometer. Subjects also kept a diary of their activities during the two days and answered a questionnaire about their habitual physical activity. Results showed a significant negative effect of sleep deprivation on most mood states on day 1, but no effect on the other variables. By day 2, mood had tended to recover, though muscle strength tended to be worse in both control and sleep-deprivation experiments. There was also a more general tendency for negative effects to be present at the end of day 1 (02: 00) or at the beginning of day 2 (10: 00). There was limited support for the view that subjects who were habitually more active showed less negative effects after sleep deprivation and responded less adversely to the poor sleep achieved on the university premises (night 2). These results stress the considerable interindividual variation in the responses to sleep loss and, therefore, the difficulty associated with giving general advice to individuals about work or training capability after sleep loss.  相似文献

目的:探讨体外冲击波(ESWT)联合钻孔减压术对早期股骨头坏死患者关节功能、肌力及生活质量的影响。方法:回顾性分析2017年9月~2018年12月期间收治的80例早期股骨头坏死患者的临床资料,依据治疗方式将其分为A组(n=37,钻孔减压治疗)和B组(n=43,钻孔减压联合ESWT治疗),对比两组患者关节功能、肌力、生活质量及并发症情况。结果:两组治疗后3、6、9个月的视觉模拟评分(VAS)评分较治疗前降低(P<0.05),髋关节Harris评分较治疗前升高(P<0.05);B组治疗后3、6、9个月VAS评分低于A组,髋关节Harris评分则高于A组(P<0.05)。两组患者治疗后9个月生理功能、情感职能、躯体疼痛、生理职能、活力、社会功能、总体健康、精神健康这8个维度评分均较治疗前升高,且B组高于A组(P<0.05)。B组患者治疗后9个月肌力3级者少于治疗前及A组同时间点,肌力4级者多于治疗前及A组同时间点(P<0.05)。两组并发症发生率比较差异无统计学意义(P>0.05)。结论:相比于单独采用钻孔减压术,ESWT联合钻孔减压术治疗早期股骨头坏死,可有效减轻患者疼痛症状,改善患者关节功能、肌力及生活质量,且不增加并发症发生率。  相似文献

摘要 目的:观察Crede手法联合凯格尔运动对产后尿潴留产妇膀胱功能、盆底肌力及生活质量的影响。方法:选择2021年3月~2022年3月期间我院收治的产后尿潴留产妇92例,按照随机数字表法分为对照组(凯格尔运动,n=46)和研究组(Crede手法联合凯格尔运动,n=46)。对比两组临床疗效、膀胱功能、盆底肌力、临床指标及生活质量变化情况。结果:与对照组(73.91%)相比,研究组(93.48%)的临床总有效率更高(P<0.05)。干预4周后,研究组膀胱初感容积、最大膀胱容量、最大尿流率均高于对照组(P<0.05)。干预4周后,研究组0~2级例数低于对照组,3级及其以上例数高于对照组(P<0.05)。干预4周后,研究组健康调查量表(SF-36)各维度评分均高于对照组(P<0.05)。研究组的残余尿量少于对照组,首次排尿时间短于对照组(P<0.05)。结论:Crede手法联合凯格尔运动应用于产后尿潴留产妇,可促进膀胱功能、盆底肌力恢复,提高产妇生活质量,具有较好的临床应用价值。  相似文献

摘要 目的:探讨重复经颅磁刺激(TMS)联合等速肌力训练对不完全性脊髓损伤(SCI)患者神经电生理指标、下肢肌力和脊髓功能独立性的影响。方法:选取2018年3月~2019年12月期间我院收治87例不完全性SCI患者,根据入院奇偶顺序分为观察组(n=44)和对照组(n=43),两组均给予常规康复训练,对照组在此基础上联合等速肌力训练,观察组在对照组基础上联合TMS,对比两组神经电生理指标[静息运动阈值(RMT)和运动诱发电位(MEP)]、下肢肌力指标[屈、伸肌群的峰力矩(PT)、力矩加速能(TAE)以及胭绳肌与股四头肌肌力比率(H/Q)]、功能独立性评定(FIM)量表、疼痛简化McGill疼痛问卷(SF-MPQ)、Barthel指数评定量表(BI)评分。结果:治疗4周后,观察组RMT较治疗前降低,且低于对照组(P<0.05);MEP较治疗前升高,且高于对照组(P<0.05)。治疗4周后,两组屈肌群PT、屈肌群TAE、伸肌群PT、伸肌群TAE、H/Q、FIM、BI评分均较治疗前升高,且观察组高于对照组(P<0.05);两组SF-MPQ评分均较治疗前下降(P<0.05),且观察组低于对照组(P<0.05)。结论:TMS联合等速肌力训练治疗不完全性SCI患者可刺激患者神经功能恢复,提高患者脊髓功能独立性,改善下肢肌力,减轻患者的神经性疼痛。  相似文献

摘要 目的:探讨佐金平木联合健脾化痰法对多发性抽动症(TS)患儿肌肉功能、肾脏损害及血清γ-氨基丁酸(GABA)、去甲肾上腺素(NE)的影响。方法:选取2017年2月~2019年2月期间我院收治的TS患儿83例,根据随机数字表法将患儿分为对照组(n=41)和研究组(n=42),对照组患儿给予氟哌啶醇治疗,研究组在对照组的基础上给予佐金平木联合健脾化痰法治疗,比较两组患儿临床疗效、肌肉功能、肾脏损害及GABA、NE水平,记录两组治疗期间不良反应情况。结果:研究组治疗3个月后的总有效率为90.48%,高于对照组的73.17%(P<0.05)。两组治疗3个月后耶鲁综合抽动严重程度量表(YGTSS)评分均下降,且研究组低于对照组(P<0.05)。两组患儿治疗3个月后血清尿素氮(BUN)、血肌酐(Scr)水平均降低,但研究组高于对照组(P<0.05)。两组患儿治疗3个月后血清GABA、NE水平均升高,且研究组高于对照组(P<0.05)。研究组、对照组不良反应发生率分别为16.67%(7/42)、24.39%(10/41),差异无统计学意义(P>0.05)。结论:佐金平木联合健脾化痰法治疗TS患儿,可有效改善患儿肌肉功能及血清GABA、NE水平,同时可减轻肾脏损害,用药安全性较好,临床应用价值较高。  相似文献

Objective: To examine rates of reported childhood maltreatment in binge eating disorder (BED), and to explore associations with obesity, gender, eating disorder features, and associated functioning. Research Methods and Procedures: Subjects were 145 consecutive outpatients with BED as defined in the Diagnostic and Statistical Manual of Mental Disorders, 4th edition. Subjects were interviewed and they completed questionnaires to assess eating disorder features and functioning. The Childhood Trauma Questionnaire was given to assess childhood maltreatment in five domains (emotional abuse, physical abuse, sexual abuse, emotional neglect, and physical neglect). Results: A total of 83% of BED patients reported some form of childhood maltreatment. A total of 59% of BED patients reported emotional abuse, 36% reported physical abuse, 30% reported sexual abuse, 69% reported emotional neglect, and 49% reported physical neglect. There were no differences in the distribution of any form of childhood maltreatment by gender or by obesity status. The different forms of maltreatment were not associated with variability in current body mass index, binge eating, or in the attitudinal features of eating disorders. Only one of the five forms of maltreatment (physical neglect) was associated with dietary restraint in women. Emotional abuse was significantly associated with greater body dissatisfaction, higher depression, and lower self‐esteem in men and women and sexual abuse was associated with greater body dissatisfaction in men. The different forms of maltreatment were unrelated to the age at onset of overweight, dieting, or binge eating. Discussion: BED outpatients reported a wide range of childhood experiences of maltreatment that do not differ by gender or obesity status. Different forms of maltreatment were not associated with the onset of overweight, dieting, or binge eating, or with variability in current body mass index or eating disorder features (except for one association between physical neglect and dietary restraint). Reports of emotional abuse were associated with greater body dissatisfaction and depression and lower self‐esteem in men and women and sexual abuse with greater body dissatisfaction in men.  相似文献

The red-striped soft scale insect Pulvinaria tenuivalvata (Newstead) (Hemiptera: Coccidae) started to infest sugarcane plants (Saccharum officinarum L.) in different districts in Egypt during the last decade. The percentage of infestation was recorded in El-Wakf area, Qena Governorate (Naghhamadi mill zone) Upper Egypt in some fields. There are three levels of infestation, low, intermediate and high. From these fields, samples were selected for physical and chemical studies. The results obtained show that the stalks of infested plants decreased in weight, the sugar content (glucose and sucrose) drastically reduced and as the percentage of infestation increased the percentage of glucose and sucrose content significantly decreased. The primary and secondary humidity and the cellulose content also increased in the healthy plants compared to the infested ones. All the physical character of the infested plants was significantly affected in comparison with the healthy ones.  相似文献

The effect of greater trochanteric pain syndrome (GTPS) on gluteus medius (GMed) and minimus (GMin) activation in post-menopausal women is unknown. The aim of this study was to compare segmental muscle activation and variability of the GMed, GMin and tensor fascia latae (TFL) during gait in post-menopausal women with and without GTPS.Intramuscular electrodes were inserted into segments of GMin (x2) and GMed (x3) and a surface electrode placed on TFL. Ten control participants and 8 with GTPS completed six walking trials. Peak amplitude, average amplitude and time to peak from each phase of the gait cycle (0–30%, 30%- toe off (TO), total stance and swing) were compared between groups using independent t-tests and effect-size (ES) calculations. Variability of muscle activation was calculated using the mean coefficient of variation (CV). Reversal of anterior GMin electromyographic burst pattern and greater average muscle activity was found in the GTPS group compared to controls: 0-TO for anterior GMin (p < 0.05), anterior and middle GMed (p < 0.01); 0–30% for posterior GMin (p < 0.01) and GMed (p < 0.05). No significant differences were identified in TFL. Overall, this study found increased segmental gluteal muscle activation, decreased hip abduction strength, and reduced variability in muscle activation in post-menopausal women with GTPS, compared with controls.  相似文献

摘要 目的:探讨调督理筋针法联合腰部核心肌力训练对腰椎间盘突出症患者腰椎功能、腰背肌力学效应和血清炎性因子的影响。方法:选取湖南中医药大学第二附属医院于2020年6月-2022年3月期间收治的120例腰椎间盘突出症(LDH)患者,按照随机数字表法将患者分为对照组(腰部核心肌力训练,60例)和观察组(调督理筋针法联合腰部核心肌力训练,60例)。对比两组疗效、疼痛及腰椎功能指标、腰背肌力学效应和血清炎性因子的情况。结果:与对照组相比,观察组临床总有效率更高(P<0.05)。观察组治疗15 d后日本矫形外科学会(JOA)评分高于对照组,疼痛视觉模拟评分(VAS)评分低于对照组(P<0.05)。观察组治疗15 d后屈曲过程表面肌电值(AEMG)、屈伸松弛比(FRR)低于对照组,伸直过程AEMG高于对照组(P<0.05)。观察组治疗15 d后白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、白介素-1β(IL-1β)、C反应蛋白(CRP)低于对照组(P<0.05)。结论:腰部核心肌力训练联合调督理筋针法治疗LDH,可减轻疼痛症状,改善腰背肌力学效应并降低血清炎性因子水平,促进腰椎功能恢复。  相似文献

摘要 目的:观察MOTOmed下肢运动训练联合等速肌力训练在脑卒中偏瘫患者的应用价值。方法:根据随机数字表法将2019年5月-2022年12月期间南京医科大学附属脑科医院收治的158例脑卒中偏瘫患者分为对照组(n=79,接受等速肌力训练)和观察组(n=79,对照组基础上接受MOTOmed下肢运动训练)。对比两组下肢运动功能、下肢肌张力、步行步态功能。结果:干预12周后,两组Fugl-Meyer运动功能评定量表(FMA)、Berg平衡量表(BBS)评分升高,且观察组高于对照组同期(P<0.05)。干预12周后,两组膝关节后伸(FKE)、前屈(FKF)、髋关节后伸(FHE)、前屈(FHF) 肌张力升高,且观察组高于对照组同期(P<0.05)。干预12周后,两组步速、步频、步长、功能性步行分级量表(FAC)评分升高,且观察组高于对照组同期(P<0.05)。结论:MOTOmed下肢运动训练联合等速肌力训练治疗脑卒中偏瘫患者,可有效改善下肢运动功能、下肢肌张力以及步行步态功能。  相似文献

Asia, which has the highest increase in dementia prevalence, is unfortunately lacking recent up-to-date research, with regions of Southeast Asia being the most inadequate. Preventive approaches, such as the understanding of Mild Cognitive Impairment (MCI), are currently the most effective approach in reducing the risk or delaying the onset of dementia but are not adequately understood. Additionally, there is a paucity of research examining lifestyle and sociodemographic correlates of MCI that are relevant to the local population of Singapore. To address these gaps, this study aimed to explore: 1) the prevalence of MCI and Amnestic Mild Cognitive Impairment (aMCI), 2) the psychosocial and lifestyle correlates of MCI and aMCI. Data were drawn from the Well-being of the Singapore Elderly (WiSE) population study, which is a single-phase cross-sectional household survey conducted among older adult residents aged 60 years and above. Analyses revealed that the weighted MCI prevalence (1.2%) was lower than global figures. Few sociodemographic and lifestyle habits were related to MCI prevalence, as only age and physical activeness emerged as significant correlates. Despite the low prevalence of MCI, individuals with MCI experienced marked disability, clinical levels of depression and anxiety, which are all concerning finds. Due to the exploratory and cross-sectional nature of the study, future longitudinal research could further refine our understanding of MCI and confirm the present findings.  相似文献
