Pupillary block is considered as an important cause of primary angle-closure glaucoma (PACG). In order to investigate the effect of pupillary block on the hydrodynamics of aqueous humor (AH) in anterior chamber (AC) and potential risks, a 3D printed eye model was developed to mimic the AH flow driven by fluid generation, the differential pressure between AC and posterior chambers (PC) and pupillary block. Particle image velocimetry technology was applied to visualize flow distribution. The results demonstrated obvious differences in AH flow with and without pupillary block. Under the normal condition (without pupillary block), the flow filed of AH was nearly symmetric in the AC. The highest flow velocity located at the central of AC when the differential pressure between AC and PC was under 5.83 Pa, while it appeared near the cornea and iris surface when the differential pressure was greater than 33.6 Pa. Once pupillary block occurred, two asymmetric vortices with different sizes were observed and the shear stress in the paracentral cornea and iris epithelium increased greatly. It can be concluded that the pupillary block and the elevated differential pressure between AC and PC could change the flow distribution and thus increase the risk of corneal endothelial cells detachment. This study could make a further understanding of the pathogenesis of PACG.  相似文献

To guide the development of improved procedures for selecting meniscal allografts, the objective of this study was to identify which cross-sectional parameters of a lateral meniscal allograft predict the contact pressure of the articular surface of the tibia. To meet the objective, the contact pressure of the articular surface of the tibia was measured with a lateral meniscal autograft and a lateral meniscal allograft using pressure sensitive film in 15 fresh-frozen human cadaveric knees. Allografts were matched only in transverse dimensions to the autograft but not in cross-sectional dimensions. Knees were loaded to 1200 N in compression at flexion angles of 0, 15, 30 and 45 degrees using a load application system that allowed unconstrained motion in the remaining degrees of freedom. Five cross-sectional parameters for both of the grafts in each of the anterior, middle, and posterior regions were derived from measurements obtained using a laser-based non-contacting three-dimensional coordinate digitizing system (3-DCDS) (Haut et al., J. Orthop Res, 2000). Five contact variables (i.e. the maximum pressure, mean pressure, contact area, and anterior-posterior and medial-lateral locations of the centroid of contact area) were determined from the pressure sensitive film. When each allograft was paired with the corresponding autograft, the root mean squared percent differences for the cross-sectional parameters ranged from a minimum of 28% for the width of the posterior region to 572% for the height of the posterior region. The root mean squared percent differences between the contact variables for paired grafts were 29% for the maximum pressure, 19% for the mean pressure, and 24% for the contact area. Differences in the cross-sectional parameters between the grafts were related to differences in the contact variables using regression analysis. Difference in the width was most often a predictor variable in the regression models with R2 values > or = 0.45. Differences in all of the four remaining cross-sectional parameters were also important predictor variables. Because failure to match cross-sectional parameters causes substantial difference in contact variables between an allograft and autograft and because cross-sectional parameters predict the contact pressure on the tibial plateau, protocols used to prospectively select allografts should concentrate on matching cross-sectional parameters and particularly the width to those of the original meniscus.  相似文献

The anterior-inferior glenohumeral capsule is the primary passive stabilizer to the glenohumeral joint during anterior dislocation. Physical examinations following dislocation are crucial for proper diagnosis of capsule pathology; however, they are not standardized for joint position which may lead to misdiagnoses and poor outcomes. To suggest joint positions for physical examinations where the stability provided by the capsule may be consistent among patients, the objective of this study was to evaluate the distribution of maximum principal strain on the anterior-inferior capsule using two validated subject-specific finite element models of the glenohumeral joint at clinically relevant joint positions. The joint positions with 25 N anterior load applied at 60° of glenohumeral abduction and 10°, 20°, 30° and 40° of external rotation resulted in distributions of strain that were similar between shoulders (r2 ≥ 0.7). Furthermore, those positions with 20-40° of external rotation resulted in capsule strains on the glenoid side of the anterior band of the inferior glenohumeral ligament that were significantly greater than in all other capsule regions. These findings suggest that anterior stability provided by the anterior-inferior capsule may be consistent among subjects at joint positions with 60° of glenohumeral abduction and a mid-range (20-40°) of external rotation, and that the glenoid side has the greatest contribution to stability at these joint positions. Therefore, it may be possible to establish standard joint positions for physical examinations that clinicians can use to effectively diagnose pathology in the anterior-inferior capsule following dislocation and lead to improved outcomes.  相似文献

 This report describes the distribution and localization of thrombomodulin (TM) in the rat eye by light and electron microscopic immunocytochemistry. In addition to the endothelium of the entire vasculature, TM was found on the non-vascular structures lining the cavities of the posterior and anterior chambers and the limbus. TM was localized on the basal, lateral, and apical plasma membranes of the inner and outer ciliary epithelium, and the posterior iris epithelium in which there was no polarized expression of TM. In the anterior chamber, TM was localized on the luminal surface of the corneal endothelium, but was negative on the anterior border layer of the iris, which is composed of a discontinuous layer of fibroblasts and collagen fibers. Thus, TM was present at sites of cell-to-cell contact. TM was also present on the endothelia of the trabecular meshwork and the Schlemm’s canal in the limbus. TM was localized not only on the luminal plasma membrane, but also on the cytoplasmic giant vacuoles in the endothelial cells of the Schlemm’s canal. These findings extend the importance of anticoagulant mechanisms to the systems of secretion, circulation, and drainage of the aqueous humor. Accepted: 18 March 1997  相似文献

目的:探究一期经后路固定联合侧前方入路病灶清除植骨融合术治疗老年下腰椎结核临床效果。方法:回顾性分析我院2007年1月-2010年12月收治下腰椎结核老年患者共84例,分别设立实验组和对照组。实验组42例采用后路固定联合侧前方入路病灶清除植骨融合术治疗老年下腰椎结核,对照组42例采用前路病灶清除。分别对两组一期治疗后临床效果进行观察,对患者术后腰椎局部前凸角、Oswestry障碍指数(ODI)及脊髓损伤神经功能分级标准(ASIA)分级进行客观评定。结果:实验组一期经后路固定联合侧前方入路病灶清除植骨融合术治疗老年下腰椎结核,术后腰椎局部前凸角为(8.22±1.67)°,明显优于对照组患者术后腰椎局部前凸角为(5.02±1.93)°,(P〉0.05);实验组患者术后ODI为(21.25±3.75)%,对照组ODI为(35.72±4.15)%,实验组疗效优于对照组(P〈0.05);实验组患者术后ASIA分级全部转化为E级。而对照组仍有7人为D级。结论:一期经后路固定联合侧前方入路病灶清除植骨融合术治疗老年下腰椎结核具有显著的临床效果,应进一步临床推广。  相似文献

A ruby laser has been used to produce a permeable lesion in the iris to establish a communication between the anterior and posterior chambers. Pigmentary glaucoma is one of the more common forms of secondary open angle glaucoma. It typically affects younger individuals, being more common among myopes, Caucasian men and persons with a family history of glaucoma. It is characterized by pigment dispersion throughout the anterior segment. The biomicroscopic signs of this condition include iris transillumination defects, Krukenberg spindles, and a dense uniform trabecular meshwork pigment band, oftentimes with a discernable pigmented Schwalbe's line (Sampoalesi line). In addition, pigment deposition may be observed on the anterior iris surface, zonules and the crystalline lens. The development of pigmentary glaucoma moves through a continuum of stages. It is advocated that posterior bowing of the iris (reverse pupillary block) results in iridozonular contact, causing release of iris pigment, i.e. pigmentary dispersion syndrome. Pigment accumulation within the trabecular meshwork leads to endothelial cell damage and loss, resulting in trabecular collapse. The trabecular meshwork support beams collapse and fuse, leading to decreased outflow facility, elevated IOP and glaucoma. Present day management includes antiglaucoma medications, trabeculoplasty, laser iridotomy for those patients with concave iris contour and filtering surgery.  相似文献

Relatively high rates of loosening and implant failure have been reported after total ankle arthroplasty. Abnormal kinematics and incongruency of the articular surface may cause increased contact pressure and rotational torque applied to the implant, leading to loosening and implant failure. We measured in vivo kinematics of two-component total ankle arthroplasty (TNK ankle), and assessed congruency of the articular surface during the stance phase of gait. Eighteen ankles of 15 patients with a mean age of 75±6 years (mean±standard deviation) and follow-up of 44±38 months were enrolled. Lateral fluoroscopic images were taken during the stance phase of gait. 3D-2D model-image registration was performed using the fluoroscopic image and the implant models, and three-dimensional kinematics of the implant and incongruency of the articular surface were determined. The mean ranges of motion were 11.1±4.6°, 0.8±0.4°, and 2.6±1.5° for dorsi-/plantarflexion, inversion/eversion, and internal/external rotation, respectively. At least one type of incongruency of the articular surface occurred in eight of 18 ankles, including anterior hinging in one ankle, medial or lateral lift off in four ankles, and excessive axial rotation in five ankles. Among the four ankles in which lift off occurred during gait, only one ankle showed lift off in the static weightbearing radiograph. Our observations will provide useful data against which kinematics of other implant designs, such as three-component total ankle arthroplasty, can be compared. Our results also showed that evaluation of lift off in the standard weightbearing radiograph may not predict its occurrence during gait.  相似文献

The facet joint contributes to the normal biomechanical function of the spine by transmitting loads and limiting motions via articular contact. However, little is known about the contact pressure response for this joint. Such information can provide a quantitative measure of the facet joint's local environment. The objective of this study was to measure facet pressure during physiologic bending in the cervical spine, using a joint capsule-sparing technique. Flexion and extension bending moments were applied to six human cadaveric cervical spines. Global motions (C2-T1) were defined using infra-red cameras to track markers on each vertebra. Contact pressure in the C5-C6 facet was also measured using a tip-mounted pressure transducer inserted into the joint space through a hole in the postero-inferior region of the C5 lateral mass. Facet contact pressure increased by 67.6 ± 26.9 kPa under a 2.4 Nm extension moment and decreased by 10.3 ± 9.7 kPa under a 2.7 Nm flexion moment. The mean rotation of the overall cervical specimen motion segments was 9.6 ± 0.8° and was 1.6 ± 0.7° for the C5-C6 joint, respectively, for extension. The change in pressure during extension was linearly related to both the change in moment (51.4 ± 42.6 kPa/Nm) and the change in C5-C6 angle (18.0 ± 108.9 kPa/deg). Contact pressure in the inferior region of the cervical facet joint increases during extension as the articular surfaces come in contact, and decreases in flexion as the joint opens, similar to reports in the lumbar spine despite the difference in facet orientation in those spinal regions. Joint contact pressure is linearly related to both sagittal moment and spinal rotation. Cartilage degeneration and the presence of meniscoids may account for the variation in the pressure profiles measured during physiologic sagittal bending. This study shows that cervical facet contact pressure can be directly measured with minimal disruption to the joint and is the first to provide local pressure values for the cervical joint in a cadaveric model.  相似文献

The accuracy of estimating the relative pose between knee replacement components, in terms of clinical motion, is important in the study of knee joint kinematics. The objective of this study was to determine the accuracy of the single-plane fluoroscopy method in calculating the relative pose between the femoral component and the tibial component, along knee motion axes, while the components were in motion relative to one another. The kinematics of total knee replacement components were determined in vitro using two simultaneous methods: single-plane fluoroscopic shape matching and an optoelectronic motion tracking system. The largest mean differences in relative pose between the two methods for any testing condition were 2.1°, 0.3°, and 1.1° in extension, abduction, and internal rotation respectively, and 1.3, 0.9, and 1.9 mm in anterior, distal, and lateral translations, respectively. For the optimized position of the components during dynamic trials, the limits of agreement, between which 95% of differences can be expected to fall, were -2.9 to 4.5° in flexion, -0.9 to 1.5° in abduction, -2.4 to 2.1° in external rotation, -2.0 to 3.9 mm in anterior-posterior translation, -2.2 to 0.4mm in distal-proximal translation and -7.2 to 8.6mm in medial-lateral translation. These mean accuracy values and limits of agreement can be used to determine whether the shape-matching approach using single-plane fluoroscopic images is sufficiently accurate for an intended motion tracking application.  相似文献

The purpose of this study was to investigate the hemodynamic effect of variations in the angulations of the left coronary artery, based on simulated and realistic coronary artery models. Twelve models consisting of four realistic and eight simulated coronary artery geometries were generated with the inclusion of left main stem, left anterior descending and left circumflex branches. The simulated models included various coronary artery angulations, namely, 15°, 30°, 45°, 60°, 75°, 90°, 105° and 120°. The realistic coronary angulations were based on selected patient's data with angles ranging from narrow angles of 58° and 73° to wide angles of 110° and 120°. Computational fluid dynamics analysis was performed to simulate realistic physiological conditions that reflect the in vivo cardiac hemodynamics. The wall shear stress, wall shear stress gradient, velocity flow patterns and wall pressure were measured in simulated and realistic models during the cardiac cycle. Our results showed that a disturbed flow pattern was observed in models with wider angulations, and wall pressure was found to reduce when the flow changed from the left main stem to the bifurcated regions, based on simulated and realistic models. A low wall shear stress gradient was demonstrated at left bifurcations with wide angles. There is a direct correlation between coronary angulations and subsequent hemodynamic changes, based on realistic and simulated models. Further studies based on patients with different severities of coronary artery disease are required to verify our results.  相似文献

Frontal plane mandibular movements during mastication and the associated electromyographic (EMG) activity for left and right superficial masseter, posterior temporalis, anterior temporalis, and anterior belly of the digastric (ABD) were studied for two adult male Macaca mulatta by the new technique of “contour” analysis. Contour analysis allowed graphic and quantitative portrayal of multiple chew cycle patterns of mandibular movement and EMG activity during active mastication. A series of computer programs (ATS, ATSED, ATSXYZ) facilitated the collection, editing and definition, and finally processing of these masticatory data into contour plots. These preliminary data indicated the essential symmetry of mandibular movement patterns, high chew cycle variability inferior to occlusion, multiple centers of intense EMG activity for balancing-side superficial masseter, and no difference between working-side anterior and posterior temporalis EMG patterns. Maximum EMG amplitude was found in the area of buccal phase power stroke (BPS). Maximum EMG amplitude for ABD was located medial and inferior to occlusion; all other muscle maximum amplitudes were buccal and inferior to occlusion. The location of maximum EMG amplitudes for superficial masseter and ABD were closer to occlusion (more superior) during mastication of carrot than were maximum amplitudes during biscuit mastication. The absence of any detectable shift of EMG maximum amplitude location between biscuit and carrot for posterior and anterior temporalis suggested, along with the continuous EMG activity of working-side posterior temporalis, a secondary role for the temporalis (compensation for superficial masseter activity) during active mastication.  相似文献

The aim of the present study was to evaluate retropatellar contact characteristics at different angles of flexion of the knee joint. To this end, 6 cadaveric legs were examined using pressure sensitive film (Fuji Prescale type "super low") at angles of flexion of 45 degrees, 60 degrees, 90 degrees and 120 degrees both in neutral rotation and 10 degrees internal and external rotation of the femur in the same knee joints. A force of 140 N was applied to both the vastus medialis and lateralis, and a comparison made with a medially and a laterally dominating muscle force. The contact areas decreased with increasing angles of flexion. The medially dominating muscle traction increased the contact area. Comparison between internal and external rotation revealed a decrease in contact area on internal rotation. The pressure measurements were comparable in all loading situations. Comparison between neutral and medial traction revealed significant differences in contact area, pressure and force. The influence of femoral rotation showed no significant difference. A comparison of the different angles of flexion revealed only few significant differences. To prevent the development of retropatellar arthrosis, maximum contact areas are necessary. The study has shown an advantage for medially dominating muscle traction, and external rotation of the femur.  相似文献

To understand the role seating plays in the support of posture and spinal articulation, it is necessary to study the interface between a human and the seat. However, a method to quantify lumbar curvature in commercially available unmodified seats does not currently exist. This work sought to determine if the lumbar curvature for normal ranges of seated posture could be documented by using body landmarks located on the anterior portion of the body. The development of such a methodology will allow researchers to evaluate spinal articulation of a seated subject while in standard, commercially available seats and chairs. Anterior measurements of boney landmarks were used to quantify the relative positions of the ribcage and pelvis while simultaneous posterior measurements were made of lumbar curvature. The relationship between the anterior and the posterior measures was compared. The predictive capacity of this approach was evaluated by determining linear and second-order regressions for each of the four postures across all subjects and conducting a leave-one-out cross validation. The relationships between the anterior and posterior measures were approximated by linear and second-order polynomial regressions (r(2?) = 0.829, 0.935 respectively) across all postures. The quantitative analysis showed that openness had a significant relationship with lumbar curvature, and a first-order regression was superior to a second-order regression. Average standard errors in the prediction were 5.9° for the maximum kyphotic posture, 9.9° for the comfortable posture, 12.8° for the straight and tall, and 22.2° for the maximum lordotic posture. These results show predictions of lumbar curvature are possible in seated postures by using a motion capture system and anterior measures. This method of lumbar curvature prediction shows potential for use in the assessment of seated spinal curvatures and the corresponding design of seating to accommodate those curvatures; however, additional inputs will be necessary to better predict the postures as lordosis is increased.  相似文献

Measurements were made of the osmotic pressure of plasma, and of aqueous humor taken from the anterior chamber of the right and left eyes and from the posterior chamber of unanesthetized rabbits. Aqueous humor from the anterior chamber was found to be hypertonic to the plasma by approximately 3 mM/liter equivalent of sodium chloride. The aqueous humor from the anterior and posterior chambers of the right and left eyes was isotonic. The concentration of chloride in the anterior and posterior chambers was the same. The concentration of all the major components of the aqueous humor and plasma has been determined by chemical analysis on fluid samples obtained from unanesthetized rabbits at approximately the same time. The calculated osmotic pressure of the total of these substances in terms of sodium chloride equivalent agrees to within better than 1 per cent of the total osmotic pressure as measured experimentally. The distribution of some individual anions and cations of the aqueous humor and plasma was determined. This distribution is widely different from that which would obtain at a state of equilibrium. The positive and negative charges carried by the ions in the aqueous humor were approximately equal. Sources of error in the experiments are discussed.  相似文献

To study changes in survival, in biological activities and behavior of planorbids submitted to increased hydrostatic pressure, we developed a technique using two transparent chambers and a hydraulic piston. The apparatus permitted renewal of the liquid medium without substantial variations in pressure, thus eliminating excretion products and maintaining the desired O2 level and thereby permitting us to evaluate the effects of pressure independently of the occurrence of anoxia. Pressure was maintained without any contact of the liquid medium with compressed air, a situation which reproduced with relative fidelity what occurs in nature and assured the presence of the same amounts of gases in the two observation chambers (Control and Experimental). Biomphalaria glabrata was found to be able to survive at least 48 hours when submitted to 49.02 x 10(4) Pa (equivalent to a water depth of 48.8 m), continuing to lay egg masses and showing few behavioral changes when compared with the control group.  相似文献

During a population survey in Eskimos family resemblance with respect to anterior chamber depth (ACD) was studied. Heritability estimates of about 0.7 against a marital correlation of zero were obtained. No correlation between ACD and parity, but a significant correlation with stature was found which possibly accounts for the sex difference in ACD. Significant associations between ACD, iris colour, facial appearance, and corneal diameter were observed. West Greenlanders with a predominantly Eskimoan physiognomy showed smaller anterior chambers than unmixed East Greenland Eskimos and Eskimo-Caucasian hybrids.  相似文献

The concept of the blood-aqueous barrier is largely based on the use of horseradish peroxidase (HRP). The present investigation was designed to check its reliability as a macromolecular tracer, especially with regard to the transport of plasma proteins. Rabbits were killed 5 min to 24 h after being intravenously injected with HRP. The tracer diffused rapidly, reaching the aqueous humor of the eye in 3 min or less and was detected at high concentration in the narrow space between the outer epithelial layer of the ciliary epithelium and the wall of the pervious capillaries in the stroma of the processes. HRP appeared to migrate from the blood to the posterior chamber, permeating the tight junctions, viz., the anatomical basis of the blood-aqueous barrier. It was detected at higher concentration at the anterior surface of the iris, at short time intervals; this was interpreted as penetration of the tracer from the aqueous humor of the anterior chamber. The choroid was also labeled in continuation with the reaction in the stroma of the pars plana of the ciliary body which, in turn, sometimes reached the iris root. Therefore, the pervious blood vessels of the choroid could be a source of macromolecules for the iris root. HRP also induced the formation of lysosomes in the ciliary epithelium. This can hardly be accepted as the way in which plasma proteins are physiologically transported to the aqueous humor. However, the pathway of HRP migration over short time intervals seems to be in agreement with previous research indicating that the entrance of serum albumin into the posterior chamber is the first step of its incorporation into the aqueous humor. The ontogeny of pigment cells in the eyes of rhesus monkeys was studied by electron microscopy and histochemistry.In 60- to 80-day-old fetuses, the pigment epithelium of the iris and retina has already differentiated whereas stromal melanocytes of the uveal tract differentiate much later. The morphological and histochemical difference between melanocytes of the iris stroma and the choroid suggests that during embryonic development melanocytes migrate from the iris toward the ciliary body and choroid.Similarly, melanosomes of pigmented epithelial cells may have their origin in the epithelium of the anterior layer of the iris, which is metabolically more active than both the posterior layer and the pigment epithelium of the ciliary body and retina.  相似文献
