目的:探讨肿瘤标志物癌胚抗原(CEA)、糖链抗原19-9(CA19-9)、糖链抗原15-3(CA15-3)联合检测在乳腺癌早期诊断中的应用价值。方法:检测87例乳腺癌患者,55例乳腺良性肿瘤患者和35例健康人血清中CEA、CA19-9、CA153等肿瘤标志物的水平及3种标志物不同组合对乳腺癌的阳性检出率。结果:乳腺癌患者3种肿瘤标志物显著高于正常对照组及乳腺良性肿瘤组(P〈0.01)。3项标志物不同组合对不同分期乳腺癌检出的敏感性均高于单项标志物。其中CEA+CA199+CA153组合的检出敏感性较其他组合均高,特别是对早期患者检出率明显提高。结论:CEA+CA199+CA153联合检测能提高乳腺癌的早期诊断率。  相似文献

目的:探讨CA125、CA199、CEA联合检测在妇科肿瘤临床诊断中的应用价值。方法:选择妇科恶性肿瘤组65例、妇科良性肿瘤组55例、健康对照组70例,采用自动荧光磁微粒酶免分析法检测血清中CA199、CA125的含量,采用光激化学发光法检测血清中CEA的含量。结果:妇科恶性肿瘤组血清中CA125、CA199、CEA的水平明显高于妇科良性肿瘤组及对照组,差异有统计学意义(P〈0.01);妇科良性肿瘤组血清中CA125、CA199、CEA的水平高于对照组,差异有统计学意义(P〈0.01);CA125、CA199、CEA联合检测对早期妇科肿瘤诊断的阳性率较单独检测高,差异有统计学意义(P〈O.05);良性组、恶性组联合检测ROC曲线下面积大于单项检测曲线下面积。结论:联合检测可提高对妇科肿瘤早期诊断的阳性率,对妇科良恶性的诊断及鉴别诊断有重要价值。  相似文献

目的:探讨胰腺癌患者血清癌胚抗原(CEA)、糖类抗原242(CA242)、糖类抗原199(CA199)水平变化,并分析上述指标对胰腺癌的联合诊断价值,为胰腺癌的临床诊断提供参考。方法:选择2014年2月至2018年2月我院收治的186例胰腺癌患者(胰腺癌组)、89例胰腺炎患者(胰腺炎组)作为研究对象,并取同期来我院检查的268例健康人作为对照组。比较三组受试者的血清CEA、CA242、CA199水平变化,对比分析血清CEA、CA242、CA199的单一以及联合诊断的准确度、特异度以及灵敏度,并绘制ROC曲线以分析上述指标的诊断价值。结果:三组受试者血清CEA、CA242、CA199水平差异具有统计学意义(P<0.05)。且胰腺炎组和胰腺癌组的血清CEA、CA242、CA199水平明显高于对照组,胰腺癌组患者的血清CEA、CA242、CA199水平明显高于胰腺炎组,差异均有统计学意义(P<0.05)。ROC曲线结果显示,CEA诊断价值最大,CA199诊断价值最小。CEA是胰腺癌单项肿瘤标志物中敏感度最高的,为85.48%;特异度最高的为CA242(96.72%);三项肿瘤标志物联合诊断的准确度增加至92.27%,敏感度增加至95.16%,特异度相比略有下降。结论:与单一肿瘤标记物诊断胰腺癌相比,CEA、CA242、CA199联合诊断的敏感度和准确度均明显升高,可以明显改善胰腺癌的漏诊率,提高患者的生存率,具有较好的临床应用价值。  相似文献

目的:探究类风湿性关节炎伴间质性肺病中肿瘤标志物对肺功能的影响。方法:选取2011年3月至2013年3月我院收治的类风湿性关节炎患者88例,根据其是否伴有ILD,分为RA组53例和RA-ILD组35例。检测两组肺功能指标用力肺活量(FVC)、1s用力呼气容积(FEV1)、最大通气量百分比(MVV)、一氧化碳弥散量(DLCO),肿瘤标志物癌胚抗原CEA、癌抗原125(CA125)、癌抗原199(CA199)及抗环胍氨酸肽抗体(抗CCP抗体)的值,并利用统计学方法分析肿瘤标记物与肺功能指标、抗CCP抗体的相关性。结果:RA.ILD组FVC、FEV1、MVV、DLCO均比RA组低,CEA、CA125、CA199均比RA组高,结果比较差异显著具有统计学意义(P〈0.05)。但两组抗CCP抗体含量相比却无明显差异,不具有统计学意义(P〉0.05)。相关性分析显示,CEA与FVC、FEV1、MVV、DLCO呈负相关(P〈0.05),而CA125、CA199却与肺功能指标无相关性,CEA、CA125、CA199与抗CCP抗体均无相关性(P〉0.05)。结论:RA-ILD的肺功能指标均有明显下降,而肿瘤标志物表达水平却明显增高,CEA对肺功能损害影响较大,却与关节损害的关系不大,这为临床对类风湿性关节炎伴间质性肺病早诊断、早治疗提供了依据。  相似文献

目的:分析血清CA199、CYFR21-1、CEA、NSE 水平检测在肺癌诊断及疗效评估中的临床价值,探讨其与肺癌的相关性。方 法:采用化学发光免疫法检测78 例肺癌患者、50 例肺部良性疾病及50 例健康体检者血清CA199、CYFR21-1、CEA、NSE 水平,比 较治疗前后肺癌患者肿瘤标志物水平的变化。结果:肺癌组血清CA199、CYFR21-1、CEA、NSE 水平较对照组、肺良性疾病组均显 著升高(P<0.05);Ⅲ-Ⅳ期肺癌患者各肿瘤标志物水平均较Ⅰ-Ⅱ期患者明显升高(P<0.05);四项联合检测的敏感性为79.5 %,明显 高于CA199、CYFR21-1、CEA、NSE 任一单项肿瘤标志物的敏感性(P<0.05);随着疗效的下降,血清CA199、CYFR21-1、CEA、NSE 水平逐渐升高(P<0.05)。结论:肺癌患者血清CA199、CYFR21-1、CEA、NSE 水平呈高表达状态,且其表达水平与肺癌病情程度密 切相关,联合检测有助于提高肺癌诊断的敏感度及指导治疗方案选择。  相似文献

摘要 目的:研究血清甲状旁腺激素(PTH)和肿瘤标志物对类风湿性关节炎合并骨质疏松症(RAOP)的诊断价值。方法:选取2019年1月-2021年12月在我院就诊的60例RAOP患者为研究对象,并选取同期在我院就诊的类风湿性关节炎(RA)患者作为对照。比较两组患者血清PTH、甲胎蛋白(AFP)、癌胚抗原(CEA)、癌抗原125(CA125)、癌抗原199(CA199)和癌抗原724(CA724)。通过pearsonr相关系数分析各指标的相关性,通过Logistic回归分析RAOP的影响因素和通过受试者工作特征(ROC)曲线分析各指标对RAOP的诊断价值。结果:(1)RAOP患者血清PTH、CA125和CA199水平均显著高于RA患者(P<0.05),而血清CA724水平显著低于RA患者(P<0.05),并且血清AFP和CEA水平与RA患者比较无差异(P>0.05);(2)RAOP患者血清PTH与血清PTH水平与血清CA125和CA199水平呈正相关(P<0.05),与血清CA724水平呈负相关(P<0.05),与血清AFP和CEA水平不相关(P>0.05);(3)Logistic回归分析显示:血清PTH、CA125、CA199、CA724是影响类风湿性关节炎合并骨质疏松症的独立影响因素(P<0.05);(4)ROC曲线分析显示:血清PTH、CA125、CA199对RAOP具有诊断价值,诊断敏感性和特异性分别为90.00%和86.97%、80.36 %和78.97 %、75.62 %和75.12 %。结论:血清PTH、CA125和CA199在RAOP患者含量升高,是影响RAOP的独立危险因素,可作为诊断RAOP的指标。  相似文献

目的:研究骨桥蛋白OPN在胰腺癌中的表达水平及其与临床病理特征的关系。方法:采用RT-PCR和免疫组织化学方法分别检测50例胰腺癌手术切除标本和20例癌旁正常胰腺组织中OPNmRNA及其蛋白水平的表达。结果:胰腺癌组织中OPNmRNA高表达率为90%(45/50)明显高于其在癌旁正常胰腺组织中的表达15%(3/20),差异有统计学意义(P<0.01);OPN蛋白的阳性率为86.0%(43/50),明显高于癌旁正常胰腺组织中的表达30%(6/20),差异有统计学意义(P<0.01);OPN在胰腺癌组织中表达与其淋巴结转移明显相关(P<0.05)。结论:OPN在胰腺癌中的过表达对胰腺癌的生长、浸润及转移起重要作用。  相似文献

目的:探讨ADAM10在胰腺癌患者外周血中的表达及其临床意义。方法:应用酶联免疫吸附试验法(ELISA)检测40例胰腺癌患者和20例健康体检者的外周血ADAM10的表达水平,分析其与临床病理特征的关系。结果:胰腺癌患者血清ADAM10水平显著高于正常对照组(P〈0.01);胰腺癌患者血清中ADAM10的表达水平与胰腺癌淋巴结转移、远处转移及TNM分期密切相关(P〈0.05),且行根治性手术切除的胰腺癌患者ADAM10表达水平低于姑息性手术切除的患者(P〈0.05);ADAM10对胰腺癌诊断的敏感度、特异性分别为51.7%、76.9%,联合检测CA19-9有助于提高胰腺癌诊断敏感度,但特异性有所下降;根治性切除后胰腺癌患者血清中ADAM10水平明显下降。结论:胰腺癌癌患者血清中ADAM10水平明显增高,检测血清ADAM10有助于胰腺癌的诊断和治疗。  相似文献

目的 探讨肿瘤干细胞标志物CD133和maspin在肝细胞性肝癌(hepatocellular carcinoma HCC)中的表达情况及其临床病理意义.方法 采用免疫组织化学ElivisionTM plus法检测100例HCC组织和20例癌旁肝组织中CD133和maspin蛋白的表达情况.结果 在癌旁肝组织中CD133和maspin蛋白的阳性表达率分别为5.0%和100%,而在HCC组中其阳性表达率分别为51.0%和48.0%,两组之间差异有显著性(P〈0.01).CD133蛋白的表达水平与肿瘤的Edmondson分级、淋巴结转移、肿瘤的数目和有无血管侵犯有关(全部P〈0.05);maspin蛋白的表达水平与肿瘤的Edmondson分级、肿瘤的数目和有无血管侵犯有关(全部P〈0.05).且CD133蛋白的表达与maspin蛋白的表达呈负相关(P〈0.01).结论 CD133和maspin蛋白的表达与HCC组织的Edmondson分级、肿瘤数目和血管侵犯有关;CD133和maspin的联合检测对HCC的进展及预后判断有重要意义.  相似文献

摘要 目的:探讨与分析血清CEA、CA19-9联合CRP在消化道恶性肿瘤的诊断价值。方法:2019年8月到2022年5月选择在本院诊治的消化道恶性肿瘤患者150例作为消化道恶性肿瘤组,同期选择在本院体检的健康人群150例作为健康组。采集两组人群的血清癌胚抗原(CEA)、糖类抗原19-9(CA19-9)、C-反应蛋白(CRP)含量,调查患者的病理特征并判断诊断价值。结果:消化道恶性肿瘤组的血清CEA、CA19-9、CRP含量都高于健康组(P<0.05)。消化道恶性肿瘤组的CEA、CA19-9、CRP阳性率为54.7 %、58.7 %、60.7 %,高于健康组的3.3 %、4.0 %、4.7 %(P<0.05)。在消化道恶性肿瘤组中,不同组织学分化、临床分期、淋巴结转移患者的血清CEA、CA19-9、CRP含量对比有差异(P<0.05)。血清CEA、CA19-9联合CRP诊断为阳性113例,在健康组中诊断为阳性3例,血清CEA、CA19-9联合CRP在消化道恶性肿瘤的诊断敏感性与特异性分别为75.3 %(113/150)和98.0 %(147/150)。结论:消化道恶性肿瘤患者多伴随有血清CEA、CA19-9、CRP的高表达,病理特征与血清CEA、CA19-9、CRP含量存在相关性,血清CEA、CA19-9联合CRP在消化道恶性肿瘤的诊断敏感性与特异性都比较好。  相似文献

Summary A panel of human colon carcinoma cell lines were characterized regarding both antigenic heterogeneity and variations in radiosensitivity. Monoclonal antibodies were used to study the expression of carcinoembryonic antigen (CEA), gastrointestinal cancer antigen (GICA or CA 19-9) and carcinoma-associated antigen (CA-50). Radiosensitivity was studied with the clonogenic survival technique. Three cell lines, LS 174T, HCTC, and SW 1116 stained positive for all three antigens. HT-29 was positive for CA 19-9 and CA-50 whereas Caco-2 was positive for CEA and CA 19-9. The cell lines SW 620 and LIM 1215 only stained positive for one of the antigens, CA-50 and CEA, respectively. In nearly all positive cases the stainings were very heterogeneous with mixtures of positive and negative cells. One exception was the HCTC cells which stained homogeneously for the CA 19-9 and CA-50 antigens. The neuroendocrinelike COLO 320 cells were negative in all cases. The radiosensitivity varied strongly between the cell lines with Dq-values between 0.8 and 1.9, extrapolation numbers between 2.0 and 4.7, Do-values between 1.1 and 2.8. The surviving fraction at 2 Gy varied between 0.3 and 0.7 with HCTC as the most radiosensitive and HT-29 as the most radioresistant cell line. Thus, there were differences in antigenic expression and intrinsic radiosensitivity between the cell-lines and antigenic heterogeneities within each cell line. The analyzed panel of cell lines will be valuable in studies of dose-effect relations for monoclonal antibodies labeled with toxic radionuclides simulating both antigenic heterogeneity and variations in radiosensitivity.  相似文献

The concentration of serum CA19-9TM in 101 patients with colorectal adenocarcinoma (CRC), and 109 patients with carcinomas of lung, breast, stomach and pancreas and hepatoma, and 40 normal healthy controls including an equal number of smokers and nonsmokers were determined by solid phase radioimmunoassay of CA19-9 assay kits (Centocor). Of the normal sera, only 1 out of 40 (2.5%) was over 37.6 U/ml. No significant difference of CA19-9 levels was found between smokers (14.4 +/- 9.0 U/ml) and non-smokers (16.0 +/- 10.2 U/ml) of normal control. In patients sera, the mean value of CA19-9 levels was significantly higher in patients with Dukes B (P less than 0.05) and in patients with Dukes C and D (P less than 0.001) than the normal healthy control (15.2 +/- 10.2 U/ml). Analysis of serum CEA concentrations has shown a similar result in patients with all Dukes staged CRC. The CA19-9 levels was also significantly elevated in patients with gastric carcinoma, lung carcinoma, hepatoma, and especially in patients with pancreatic carcinoma (P less than 0.0001). The levels of CA19-9 elevated in 50% (22/44) of patients with advanced CRC while the elevation was 8 of 43 (18.6%) patients with localized CRC. A comparison of CA19-9 and CEA assays showed no correlation (r = 0.125) between the two assays. Although the CA19-9 assay (26.4%) was less sensitive than the CEA assay (51.7%), the specificity of CA19-9 assay (97.5%) was better than that of CEA assay (87.5%).(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

摘要 目的:研究磁共振胰胆管成像(MRCP)联合血清糖类抗原125(CA125)、糖类抗原19-9(CA19-9)、癌胚抗原(CEA)对良恶性梗阻性黄疸的诊断价值。方法:将医院从2018年1月~2020年2月期间收治的90例良恶性梗阻性黄疸患者纳入研究。将其按照良恶性的差异分为良性梗阻性黄疸51例以及恶性梗阻性黄疸39例。分别对所有患者进行MRCP检测,并分析良恶性梗阻性黄疸MRCP影像学表现特征的差异。此外,采集所有患者清晨空腹静脉血,检测血清CA125、CA19-9、CEA水平并进行对比。通过受试者工作特征(ROC)曲线分析明确MRCP联合血清CA125、CA19-9、CEA对良恶性梗阻性黄疸的诊断价值。结果:恶性梗阻性黄疸部位为十二指肠乳头区人数占比明显高于良性梗阻性黄疸,而胰头上区、胰头区人数占比均明显低于良性梗阻性黄疸;且恶性梗阻性黄疸梗阻重度扩张人数占比明显高于良性梗阻性黄疸,而梗阻轻度扩张人数占比明显低于良性梗阻性黄疸,差异均有统计学意义(均P<0.05)。恶性梗阻性黄疸患者血清CA125、CEA水平均明显高于良性梗阻性黄疸患者(均P<0.05);而两组血清CA19-9水平对比不明显(P>0.05)。MRCP联合血清CA125、CA19-9、CEA诊断良恶性梗阻性黄疸的曲线下面积、灵敏度、特异度、约登指数均明显高于MRCP和血清CA125、CA19-9、CEA单独诊断。结论:MRCP联合血清CA125、CA19-9、CEA对良恶性梗阻性黄疸的诊断价值较高,值得临床推广应用。  相似文献

Abstract

Purpose: It is suggested that tumour markers carcinoembryonic antigen (CEA) and carbohydrate antigen 19-9 (CA19-9) could be used to predict the stage of pancreatic cancer. However, optimal cut-off values for CEA and CA19-9 are disputable. This study aimed to assess the value of CEA and CA19-9 serum levels at diagnosis of pancreatic ductal adenocarcinoma (PDAC) as predictors for the advanced stage of PDAC in patients discussed at pancreatic multidisciplinary team (MDT) meetings.

Methods: Patients with suspected PDAC discussed at MDT meetings from 2013 to 2017 were reviewed, in order to determine optimal cut-off values of both CEA and CA19-9.

Results: In total, 375 patients were included. Optimal cut-off values for predicting advanced PDAC were 7.0?ng/ml for CEA and 305.0?U/ml for CA19-9, resulting in positive predictive values of 83.3%, 73.6%, and 91.4% for CEA, CA19-9 and combined, respectively. Both tumour markers were independent predictors of advanced PDAC, demonstrated by an odds ratio of 4.21 (95% CI:1.85–9.56; p?=?0.001) for CEA and 2.58 for CA19-9 (95% CI:1.30–5.14; p?=?0.007).

Conclusions: CEA appears to be a more robust predictor of advanced PDAC than CA19-9. Implementing CEA and CA19-9 serum levels during MDT meetings as an additional tool for establishing tumour resectability is worthwhile for tailored diagnostics.  相似文献

A new tumor cell line derived from a human pancreatic exocrine adenocarcinoma was established in tissue culture and was transplantable in a nude mouse. In tissue culture, the neoplastic cells grew as epithelial-like, mucin-producing cells with a population doubling time of 50-70 hrs. Chromosomes ranged from 63 to 186 with a modal number of 77. Subcutaneous injection of 1 x 10(6) cultured neoplastic cells into nude mice resulted in tumor formation histologically closely resembling the original neoplasm. Ultrastructurally, the cell line showed characteristic ductal epithelium. Immunohistochemically, carcinoembryonic antigen (CEA). Carbohydrate Antigen 19-9 (CA19-9) and DU-PAN-2 antigen were demonstrated in the original tumor, the culture cells and the transplanted tumor. The cells secreted CEA (48.7 ng/1 x 10(5) cells/24 hrs) and CA19-9 (325 U/1 x 10(5) cells/24 hrs) in spent medium as well as sera of the nude mouse. This cell line has been passaged 30 times in vitro and maintained for more than one year. These characteristics will make the cell line SOJ a valuable tool in studying various aspects of biology of human pancreatic cancer.  相似文献

目的:探讨血清miR-92a在胰腺癌诊断和预后分析中的价值,为胰腺癌早诊断以及预后评估提供潜在的分子标志物。方法:回顾性分析我院及重庆医科大学附属第一、第二医院2014年8月~2016年12月收治的30例胰腺癌未转移患者、30例胰腺癌转移患者和30例慢性胰腺炎患者的临床资料,另选择同期在我院进行健康体检的30例健康人作为健康组。收集血清,应用定量PCR法检测各组血清miR-92a的表达水平,利用化学发光法检测各组血清中的糖蛋白抗原19-9(CA-19-9)含量。以ROC分析比较血清miR-92a与CA 19-9在胰腺癌诊断中的特异度、敏感性。结果:胰腺癌未转移组患者和胰腺癌转移组患者血清中miR-92a水平显著高于健康组和慢性胰腺炎组(P0.05),胰腺癌转移组患者血清中miR-92a水平显著高于胰腺癌未转移组患者(P0.05)。胰腺癌未转移组患者和胰腺癌转移组患者血清中CA19-9水平显著高于健康组和慢性胰腺炎组(P0.05)。miR-92a诊断胰腺癌的敏感度高于CA19-9和miR-92a+CA 19-9,而miR-92a+CA 19-9诊断胰腺癌的特异度显著高于miR-92a和CA19-9(P0.05),且有较高的胰腺癌转移预测应用价值。结论:血清miR-92a联合CA 19-9检测能够诊断胰腺癌,具有良好的敏感度和特异度,miR-92a还具有较好的胰腺癌转移预测价值,可作为胰腺癌早期无创筛查方法加以应用。  相似文献

目的:探究血清癌胚抗原(carcinoembryonic antigen,CEA)、糖类抗原19-9、细胞角蛋白19片段(cytokeratin19 fragements,CYFRA21-1)与结直肠腺癌的病理相关性。方法:选择于我院接受治疗的80例结直肠腺癌患者为病例组,选择同期于我院接受治疗的50例良性结直肠病变患者为良性对照组,选择我院体格检查的50例健康个体为对照组,分别采集三组个体的血样并进行CEA、CA19-9以及CYFRA21-1水平的检测,并比对三组个体上述因子阳性表达率、因子水平,同时分析三种因子同结直肠腺癌患者TNM分期相关性,最后探究三种因子对结直肠腺癌的诊断价值。结果:(1)以CEA≥2.805μg/L、CA19-9≥39 U/m L、CYFR21-1≥3.3 ng/mL为临界值,结果显示病例组CEA阳性率51.25%,CA19-9阳性率31.25%,CYFR21-1阳性率40.00%,明显高于良性组的10.00%、20.00%和10.00%,高于对照组的8.00%、12.00%和2.00%(P<0.05);(2)比较显示病例组患者的CEA、CA19-9以及CYFR21-1水平明显高于良性对照组以及对照组(P<0.05),良性对照组CEA、CA19-9以及CYFR21-1水平明显高于对照组(P<0.05);(3)比较显示IV期结直肠腺癌患者CEA、CA19-9以及CYFRA21-1水平明显高于III期以及I+II期(P<0.05),III期三种因子水平明显高于I+II期(P<0.05);(4)CEA对结直肠腺癌诊断一致性71.25%,灵敏度65.00%,特异度90.00%;CA19-9诊断一致性46.25%,灵敏度35.00%,特异度80.00%;CYFRA21-1诊断一致性55.00%,灵敏度46.67%,特异度80.00%;联合诊断一致性95.00%,灵敏度95.00%,特异度95.00%。结论:血清CEA、CA19-9以及CYFRA21-1对结直肠腺癌具有较明确的诊断价值,不同病理分期患者中表达差异明显,可以考虑将联合诊断作为结直肠腺癌鉴别方式之一,推广于临床中。  相似文献

The levels of carcinoembryonic antigeny (CEA), tissue polypeptide antigeny (TPA), CanAg 50, neuron specific enolase (NSE) and ferritin were determined in bronchial secretion and serum of patients with neoplastic and non-neoplastic lung diseases. Simultaneous determination of two or three markers in the serum and in bronchoalveolar lavage (BAL) may be clinically useful for the diagnosis of lung cancer and even for the type of tumor. The positivity of CEA determined simultaneously in serum and in BAL of patients with lung cancer is higher than 80% whereas in patients with benign lung disease it is lower than 40%. The simultaneous assay of TPA in serum and in BAL showed 100% positivity in patients with oat-cell carcinoma, the frequencies of positivity were similar in patients with non-oat-cell carcinoma. For NSE and CanAg CA-50 patients with oat-cell carcinoma showed 100% positivity. Simultaneous assay of ferritin in serum and in BAL gave 85% positivity in patients with oat-cell carcinoma and only 23% in patients with non-oat-cell carcinoma. We conclude that the simultaneous determination of CEA and CanAg CA-50 or NSE in serum and in BAL is a useful aid in the diagnosis of lung malignancy.  相似文献
