Surgical method for treatment of syndactyly with osseous fusion of the distal phalanges.

For the interdigital space plasty to correct a syndactyly with fusion of the distal phalanges, we adopted a new operative procedure, covering the exposed distal bone with local rectangular flaps designed transversely on both dorsal and volar surfaces. A full-thickness skin graft from the groin region covers the remaining raw surface. The procedure was performed in six patients, and the flaps measured 6 mm (width) x 12 to 16 mm (length) in four patients, 5 x 15 mm in one patient, and 5 x 18 mm in one patient. Partial necrosis was observed in only one patient, in whom a narrow flap of 5 x 18 mm was used. In the other five patients, however, the results were satisfactory on both external appearance and function during the follow-up period of 3 to 6 years.     

Surgical treatment of pulmonary tuberculosis; a decade of change

To observe trends in the surgical therapy of pulmonary tuberculosis, the records of patients treated during the last ten years at the Tuberculosis Division of the San Diego County General Hospital (Vauclain Home) were reviewed. In this decade, a chemotherapeutic revolution permitted more patients to be treated with fewer beds, lower mortality and shorter hospitalization. Pneumoperitoneum has replaced other forms of temporary collapse. Pneumothorax, phrenic nerve interruption and pneumonolysis have been abandoned in favor of extraperiosteal plombage, particularly in older, poor risk patients. The use of permanent collapse measures as definitive treatment has decreased, thoracoplasty and extrapleural pneumothorax having been virtually abandoned. The use of resection in patients with permanent collapse failure, residual cavities, bronchostenosis and destroyed lobes or lungs has become common, and good results have been obtained.     

Surgical treatment of osteomyelitis

Chronic osteomyelitis is refractory to nonsurgical treatment due to a resilient, infective nidus that harbors sessile, matrix-protected pathogens bound to substrate surfaces within the wound. Curative treatment mandates physical (surgical) removal of the biofilm colony, adjunctive use of antibiotics to eliminate residual phenotypes, and efforts to optimize the host response throughout therapy. Patient selection, therapeutic options, and the treatment format are determined by the Cierny/Mader staging system, while reconstruction is governed by the integrity/stability of the affected bone(s) and quality/quantity parameters of the soft-tissue envelope.     

桡骨远端骨折治疗的选择

目的:探讨不同类型的桡骨远端骨折的有效治疗方法。方法:分析106例桡骨远端骨折,分别采用闭合手法复位,切开复位或有限切开复位内固定方法,分别对骨折复位比较及功能评分。结果:完整随访106例桡骨远端骨折病例,随访时间3～21个月。对保守治疗组与手术治疗组进行骨折复位测定及改良Garland和Werley评分,A,B型骨折无显著性差异;C型骨折中,手术组明显优于保守治疗组。结论:对于C型骨折,建议行切开复位内固定治疗;对于A,B型需根据实际情况选择治疗方式。
