目的调查产后42天产妇阴道感染发生情况。方法选取2013年1月至2013年3月期间大兴区医院产后42天复检产妇188例,采用白带常规镜检方法对产后42天复检产妇进行常规筛查。结果检出:需氧菌阴道炎患者21例(占受检人数的11.17%),细菌性阴道病患者29例(占受检人数的15.43%),滴虫性阴道炎患者7例(占受检人数的3.72%),外阴阴道假丝酵母菌病患者5例(占受检人数的2.67%),共计62例(占受检人数的32.98%)。结论产后42天产妇阴道感染以细菌性阴道感染为主。  相似文献

目的 调查晋城地区需氧菌阴道炎(AV)的发病情况.方法 采用需氧菌阴道炎/细菌性阴道病联合测定技术对阴道分泌物进行快速检测.结果 1936例受检者中,AV患者242人,总体发病率为12.50%(242/1936).其中单纯AV感染93例,占38.43%,AV合并其他感染患者149人,占61.57%.70 ~81岁年龄组的AV发病率最高,达到了20.53%(8/39).其次为50 ~59岁年龄组和30~ 39岁年龄组,AV发病率分别为16.73%(46/275)和15.29%(98/641).40 ~49岁年龄组,AV的发病率最低,为6.63%(39/588).结论 AV易合并其他感染,应引起临床足够的重视.  相似文献

This study investigated whether a correlation exists between predisposition to candidal vaginitis and adherence of Candida albicans to vaginal epithelial cells in vitro. Vaginal epithelial cells from 120 fecund women who were pregnant and/or diabetic had a greater propensity to bind C. albicans than did 71 oral contraceptive users and 75 non-pregnant, non-diabetic controls. The highest level of adherence occurred in pregnant diabetic women. Among 48 non-diabetic postmenopausal females, C. albicans adherence was lower than for fecund controls, but it was higher for cells from 33 postmenopausal diabetic women. The hormonal status of the fecund and postmenopausal women was assayed cytologically by the Karyopyknotic and Maturation Indices, which determine the ratios of superficial, intermediate and parabasal vaginal epithelial cells. Our findings point to increased C. albicans adherence in situations where there is an increase in the number of intermediate epithelial cells: pregnancy, the first or fourth weeks of the menstrual cycle, or diabetes. The adherence of 41 C. albicans isolates from patients with vaginitis was significantly higher than that of 36 isolates from asymptomatic carriers.  相似文献

健康育龄妇女的阴道微生态环境呈现多样性,阴道内微生物菌群以乳杆菌为主。乳杆菌可维持妇女阴道微环境平衡和保持阴道酸性环境以抑制致病性病原体生长。当乳杆菌比例低至致病阈值以下或无乳杆菌存在时,阴道微环境的菌群组成转变成以兼性需氧及厌氧菌为主。越来越多的证据表明,怀孕期间阴道菌群的异常与早产、及因早产和围产期结局不佳导致的潜在新生儿后遗症有关。本文综述了妊娠期阴道菌群的变化、细菌性阴道炎与早产发生风险等方面的问题,目的是帮助临床医生确定怀孕期间有阴道炎症状女性是否可接受治疗,从而降低早产、早产胎膜早破、绒毛膜羊膜炎和母胎传染病的发病率。  相似文献

目的了解人流后阴道微生态的改变,探讨定君生调整人流后阴道微生态失衡,减少阴道炎的临床效果。方法分别对正常体检妇女、早期妊娠妇女和人流后妇女各60例行阴道分泌物pH测定和乳酸杆菌培养;另外,选择无阴道感染的80例人流后妇女随机分为2组,定君生组40例,连续应用定君生10 d,对照组40例,30 d后对其白带清洁度、pH、阴道炎患病率进行比较。结果人流后阴道乳酸杆菌检出率明显低于孕前组和妊娠组(P<0.01),阴道pH显著高于孕前组和妊娠组(P<0.01)。人流后40 d随访阴道pH均有下降,应用定君生组明显低于对照组(P<0.01),清洁度I、II度比例较对照组升高(P<0.05),细菌性阴道病的发生率明显低于对照组(P<0.05)。结论人流可导致阴道微生态改变,使人流后妇女易患阴道感染,应用定君生可迅速恢复阴道内乳酸杆菌的优势地位,恢复阴道微生态平衡,减少阴道炎的发生。  相似文献

The aim of the study was to determine the prevalence of vaginal group B streptococcus (GBS) colonization in pregnant women from Osijek area, the possible effect of GBS colonization on pregnancy outcome and neonatal complications and the role of intrapartum prophylaxis in this context. This retrospective case-control study took place at the Department of Gynecology and Obstetrics, Osijek University Hospital Center from December 2003 to June 2006. A total of 118 pregnant women was enrolled in study and divided into two groups: 59 women in 35th-37th week of gestation, free from risk factors for infection (control group); and 59 women in 25th-41st week of gestation with risk factors for infection. Low vaginal swab for GBS isolation and identification on selective and enriched medium was obtained from each woman. GBS colonization was recorded in 29 (24.6%) women: 12 (20.3%) control and 17 (28.8%) women at risk of infection, yielding a statistically non-significant difference (Chi2 = 1.480489; p < 0.48). Early neonatal infection was observed in six (20.7%) neonates born to 29 mothers with GBS colonization, pointing to a correlation between vaginal GBS colonization and early neonatal infection (r(s) = 0.99). Early perinatal infection was found in 22 (18.6%) neonates, including 17 (28.8%) pregnancies with risk factors, pointing to a significant correlation between vaginal GBS colonization, risk factors and early perinatal infection (Chi2 = 88.68; p < 0.001); however, gestational age and pregnancy outcome were not influenced by GBS colonization. In eight (36.4%) newborns, early neonatal infection developed in spite of intrapartum administration of antibiotics; three of these children were born to GBS positive mothers, and perinatal GBS infection was demonstrated in one (0.84%) child. Study results revealed a relatively high rate of GBS colonization in the population of pregnant women in Croatia, occasionally leading to early neonatal infection. Large studies are needed to develop national strategy for the prevention of GBS infection in Croatia.  相似文献

Infectious vaginitis occurs when the normal vaginal flora is disrupted; it may arise when saprophytes overwhelm the host immune response, when pathogenic organisms are introduced into the vagina or when changes in substrate allow an imbalance of microorganisms to develop. Examples of these types of vaginitis include the presence of chronic fungal infection in women with an inadequate cellular immune response to the yeast, the introduction of trichomonads into vaginal epithelium that has a sufficient supply of glycogen, and the alteration in bacterial flora, normally dominated by Lactobacillus spp., and its metabolites that is characteristic of "nonspecific vaginitis". The authors review microbiologic and clinical aspects of the fungal, protozoal and bacterial infections, including the interactions of bacteria thought to produce nonspecific vaginitis, that are now recognized as causing vaginitis. 目的 探讨生化快速检测法在诊断需氧菌阴道炎中的临床价值.方法 分别使用镜检法和生化快速检测法对320例疑似需氧菌阴道炎患者的阴道分泌物进行检查.结果 在320例就诊者中,生化快速检测法检出阳性患者286例,镜检法检出阳性患者283例.两种方法的符合率为96.56%[ (279+ 30)/320],二者呈高度一致性( Kappa=0.823,P<0.05).以镜检法为诊断标准,生化快速检测法的敏感度为98.59%( 279/283)、特异度为81.08%(30/37)、阳性预测值为97.55%(279/286)、阴性预测值为88.24%(30/34).结论 生化快速检测法对需氧菌阴道炎的诊断具有较高的敏感度和特异度,值得临床推广使用.  相似文献

The Group B Streptococcus (GBS, Streptococcus agalactiae) is an important cause of neonatal and maternal infection. GBS is a commensal organism of the lower gastrointestinal and vaginal tract. A frequent mode of neonatal infection is vertical transmission from pregnant women to their foetus or neonate. The aim of this study was to evaluate the survival and biofilm production of 10 GBS strains in simulated vaginal fluid at pH 4.2, 5.5 and 6.5. GBS survived longer at higher pH than at normal vaginal pH. At pH 4.2, with the exception of two isolates that were recovered up to 48 and 72 h, viable GBS numbers declined below the limit of detection by 24 h. At higher pH, GBS survived between 3 and 15 days. All isolates investigated were biofilm producers but biofilm production was greater in tryptone soy broth compared to simulated vaginal fluid. The quantity of biofilm produced increased with the rise in the pH. This study suggests that high vaginal pH may influence both GBS survival and biofilm production and thus could be a risk factor for GBS infection.  相似文献

用阴道分泌物直接涂片、革兰染色、选择性培养分离和细菌预成酶谱分析等微生物学方法和生物化学方法,分析了正常女性、AV、BV患者的阴道菌群。正常女性阴道中以乳酸杆菌为主,产H2O2乳酸杆菌的检出率89%,AV的致病菌主要是革兰阳性率需氧菌、检出率可达80%,其中金黄色葡萄球菌,粪肠球菌和埃希氏大肠菌较为常见,B族链球菌的检出率较低;BV的致病菌主要是厌氧菌,以革兰阴性厌氧菌的检出率最高,加德纳菌的检出率不足40%。48例BV患者中有8例合并AV感染,31例AV患者中有6例合并BV感染。细菌谱分析发现,AV患者易感染消化链球菌、普雷沃菌等厌氧菌和加德纳菌,BV患者易感染粪肠球菌、大肠埃希菌等需氧菌。  相似文献

目的:探讨21143例不同年龄段职业女性阴道炎病原菌的分布及特征。方法:对2009年1月至2010年1月参加健康体检的21143例不同年龄段职业女性的阴道分泌物进行常规湿片法和革兰染色法联合检查。结果:发现阴道炎病原菌检出率为44.96%,其中细菌性阴道病检出率最高。在20岁以下和59岁以上阴道炎患者数量太少,未作统计。各年龄组间滴虫性检出率差异无显著性(P〉0.05),而细菌性阴道病和念珠茵性阴道炎的检出率差异均有显著性(P〈0.05)。结论:在参加健康体检的21143例不同年龄段职业女性中,细菌性阴道病是最常见的阴道炎类型,其次是念珠茵性阴道炎,滴虫感染占第三位,且细菌性阴道病和念珠茵性阴道炎的检出率在不同年龄段的差异有统计学意义(P〈0.05)。  相似文献

目的调查海宁地区女性阴道感染情况。方法采用白带常规镜检联合需氧菌阴道炎/细菌性阴道病联合测定技术对1260例阴道分泌物进行检测。结果在1260例就诊患者中,清洁度I~II度526例(41.75%),清洁度III~IV度734例(58.25%),检出阴道感染患者912例,单一性感染487例(占患病人数的53.4%),混合感染患者425例(占患病人数的46.6%)。在487例单一性感染患者中,需氧菌阴道炎(AV)患者123例,细菌性阴道病(BV)患者178例,胞溶性阴道病(CV)患者8例,外阴阴道假丝酵母菌病(VVC)患者146例,滴虫性阴道炎(TV)患者32例;在425例混合感染患者中,AV+BV患者136例,AV+VVC患者56例,AV+TV患者62例,BV+VVC患者103例,BV+TV患者48例,AV+BV+VVC患者13例,AV+BV+TV患者7例。结论阴道混合感染及AV发病率较高,值得临床高度关注。  相似文献

The human vagina is known to be heavily innervated by vasoactive intestinal polypeptide (VIP) immunoreactive nerve fibres. In the present study we have examined the effect of VIP (900 pmol x kg-1 x h-1, IV during 30 min) on vaginal lubrication and blood flow in fourteen normal non-pregnant women. Vaginal blood flow was measured by the heat clearance technique and the vaginal lubrication quantified by the weight gain of preweighed filter papers placed on the surface of the vaginal wall for 30 min. Arterial blood pressure, pulse frequency and the concentration of VIP in peripheral blood were monitored. VIP (median concentrations of 200-300 pmol x l-1) induced a significant increase in vaginal blood flow accompanied by a 100% increase in vaginal lubrication (from 27 mg/cm2 to 53 mg/cm2). The VIP infusion lead to a significant increase in pulse frequency and a significant fall in diastolic arterial blood pressure. The findings suggest that VIP may participate in the control of the local physiological changes observed during sexual arousal: genital vasodilation and increase in vaginal lubrication.  相似文献

本文对31例非特异性阴道炎病人和31例正常人阴道微生物群进行定性、定量分析.结果表明,阴道炎病人的阴道乳杆菌的平均浓度明显低于正常人,而肠杆菌科、葡萄球菌、棒状杆菌、类杆菌属、支原体却明显增多,阴道炎病人未发现特异性病原体,菌群失调可能是其发病因素。乳杆菌为阴道正常优势菌,对改善阴道的微环境,防止条件致病菌引起的内源性感染具有重要的生理作用。  相似文献

Streptococcus agalactiae (group B streptococcus [GBS]) is a Gram-positive bacterium found in the female rectovaginal tract and is capable of producing severe disease in susceptible hosts, including newborns and pregnant women. The vaginal tract is considered a major reservoir for GBS, and maternal vaginal colonization poses a significant risk to the newborn; however, little is known about the specific bacterial factors that promote GBS colonization and persistence in the female reproductive tract. We have developed in vitro models of GBS interaction with the human female cervicovaginal tract using human vaginal and cervical epithelial cell lines. Analysis of isogenic mutant GBS strains deficient in cell surface organelles such as pili and serine-rich repeat (Srr) proteins shows that these factors contribute to host cell attachment. As Srr proteins are heavily glycosylated, we confirmed that carbohydrate moieties contribute to the effective interaction of Srr-1 with vaginal epithelial cells. Antibody inhibition assays identified keratin 4 as a possible host receptor for Srr-1. Our findings were further substantiated in an in vivo mouse model of GBS vaginal colonization, where mice inoculated with an Srr-1-deficient mutant exhibited decreased GBS vaginal persistence compared to those inoculated with the wild-type (WT) parental strain. Furthermore, competition experiments in mice showed that WT GBS exhibited a significant survival advantage over the ΔpilA or Δsrr-1 mutant in the vaginal tract. Our results suggest that these GBS surface proteins contribute to vaginal colonization and may offer new insights into the mechanisms of vaginal niche establishment.  相似文献

The present study evaluated the secretions of interleukin (IL)-1beta and tumor necrosis factor (TNF) alpha by fetal membranes stimulated with group B streptococci (GBS) and lipopolysaccharide (LPS). The aim was to evaluate the initial response of full-thickness membranes to the microbial insult using an in vitro experimental model that allowed testing of the individual contributions of amnion and choriodecidua to stimulation. Full-thickness membranes were obtained after delivery by elective cesarean section from women at 37-40 wk of gestation without evidence of active labor. The membranes were mounted in Transwell devices, physically separating the upper and lower chambers. The LPS (500 ng/ml) or GBS (1 x 10(6) colony-forming units/ml) was added to either the amniotic or choriodecidual surface, and accumulation of IL-1beta and TNFalpha were measured in both compartments using a specific ELISA. Fetal membranes followed different patterns of secretion of proinflammatory cytokines that depended on the side to which the stimulus was added or the nature of the stimulus itself. The TNFalpha was secreted by amnion and choriodecidua in the presence of LPS or GBS, and stimulation with GBS induced a greater synthesis of IL-1beta than did stimulation with LPS. Choriodecidual tissue was more responsive than amniotic tissue, and this response tended to be higher even when the stimulation was only on the amniotic side. However, the amnion plays an active role in recognizing LPS or GBS, contributing a significant amount of TNFalpha. Thus, cooperative and bidirectional communications occur between amnion and choriodecidua in response to bacterial products, which include intermembranous cytokine traffic and signaling between tissues.  相似文献

Vaginal candidiasis continues to be a common cause of vaginal discharge, pruritus and other local complaints in women worldwide. Although numerous antimycotic agents are available for the treatment of yeast vaginitis there is little comparative data on the in vitro activity of these drugs. The objectives of this study were to isolate and identify the Candida species in the vagina and anus of patients treated in a gynaecology clinic, as well as determine the susceptibility to azolic compounds measured by the E-test method. Vaginal and rectal swabs were collected from 80 adult non-pregnant patients, seen at a gynaecological clinic, aged 18–59 years, with sexual activity, with and without vaginitis. The swabs were processed by methods routinely used for the detection of pathogenic yeasts. The susceptibility of the isolates to fluconazole, ketoconazole and itraconazole, was measured by the agar diffusion method (E-test), using RPMI 1640 medium with 2% glucose and phosphate buffer. Candida species (33) strains were isolated from 17 patients at similar proportions from both anatomical sites, and 12 patients harboured 24 strains of C. albicans in the vaginal and rectal tracts. Twenty one percent of the strains of C. albicans were resistant to ketoconazole, 54% were resistant to itraconazole and 0% were resistant to fluconazole. The sensitivity of strains isolated from the two sites were similar, indicating that these are strains of the same phenotype. Group B streptococci (GBS) are the leading cause of neonatal meningitis and sepsis worldwide. Intrapartum antibiotic prophylaxis (IAP) is the current prevention strategy given to pregnant women with confirmed vaginal GBS colonization. Due to antibiotic resistance identified in GBS, we previously developed another strategy using a bacteriophage lytic enzyme, PlyGBS, to reduce vaginal GBS colonization. In this study, various DNA mutagenesis methods were explored to produce PlyGBS mutants with increased lytic activity against GBS. Several hyperactive mutants were identified that contain only the endopeptidase domain found in the N-terminal region of PlyGBS and represent only about one-third of the wild-type PlyGBS in length. Significantly, these mutants not only have 18–28-fold increases in specific activities compared to PlyGBS, but they also have a similar activity spectrum against several streptococcal species. One of the hyperactive mutants, PlyGBS90-1, reduced the GBS colonization from >5 logs of growth per mouse to <50 colony-forming units (cfu) 4 h post treatment (∼4-log reduction) using a single dose in a mouse vaginal model. A reduction in GBS colonization before delivery should significantly reduce neonatal GBS infection providing a safe alternative to IAP.  相似文献

目的探讨微生物制剂(阴道乳杆菌胶囊)对产后6周妇女特异性阴道炎(细菌性阴道病、滴虫性阴道炎、外阴阴道念珠菌病)的防治作用。方法采用前瞻性研究方法。自2009年6月至2011年8月先后收集895例产后6周特异性阴道炎妇女,包括细菌性阴道病535例、外阴阴道念珠菌病339例和滴虫性阴道炎21例,随机分成三组,第一组(n=200)阴道局部用乳杆菌胶囊(定君生)250 mg/d,连用10 d;第二组(n=200)阴道局部用抗菌药(细菌性阴道病或滴虫性阴道炎:甲硝唑栓500 mg/d,外阴阴道念珠菌病:克霉唑栓150 mg/d,连用10 d);第三组(n=200)阴道局部用抗菌药+乳杆菌胶囊(细菌性阴道病或滴虫性阴道炎:甲硝唑栓500 mg/d+定君生250 mg/d,外阴阴道念珠菌病:克霉唑栓150 mg/d+定君生250mg/d,连用10 d),三组均于停药1周后复查阴道分泌物以了解疗效,以及产后3、6、9、12个月分别复查阴道分泌物,以了解三组产后1年内阴道pH、乳杆菌数量变化情况及阴道炎复发率情况。结果 600例产后6周特异性阴道炎妇女中,单用定君生治疗组总有效率为76.5%;用甲硝唑栓或克霉唑栓治疗组总有效率为77.5%;两组总疗效比较差异无统计学意义(P〉0.05)。用抗菌药(甲硝唑栓或克霉唑栓)+定君生治疗组总有效率为95.0%,与前两组比较差异有统计学意义(P〈0.05)。应用定君生治疗两组1年内阴道pH明显降低,阴道乳酸杆菌数量明显增多,以+++~++++比例为主,且追踪1年阴道炎的发生率均明显降低,与只用抗生素治疗组相比差异有统计学意义(P〈0.05)。结论阴道乳杆菌胶囊(定君生)是治疗特异性阴道炎的有效药物,与传统抗生素相比疗效差异无统计学意义,且定君生联合抗生素治疗特异性阴道炎,疗效更为显著,这不失为一个新的治疗途径。产后6周妇女特异性阴道炎选用微生态制剂(阴道乳?  相似文献
