A case of Aspergillus niger fungal colonization associated with bilateral inflatable silicone mammary implants is reported. Painful fibrous capsular contractures without clinical evidence of infection or inflammation characterized the presenting symptoms. Operative findings included a cheesy-white exudate that surrounded the implants and turbid fluid within the implants. All specimens yielded a heavy growth of Aspergillus niger. Special stains of the fibrous capsules were negative for fungal invasion. The etiology and pathogenesis of Aspergillus colonization in this patient are postulated.  相似文献

Pneumococcal intra-abdominal infections of female genital tract origin are rare. A 33-year-old woman with peritonitis due to Streptococcus pneumoniae serotype 3 was reported. The patient did not have any of the known predisposing conditions for intra-abdominal pneumococcal infection. The clinical presentation included signs of multiorgan failure. Peritoneal toilette, bilateral salpingectomy and antibiotic treatment were promptly administered. The patient remained febrile, developed respiratory failure and required a repeated laparotomy and a prolonged antimicrobial treatment. Penicillin susceptible S. pneumoniae was isolated from the pus collected at surgery. Thus, culturing of intra-operative specimens should never be neglected to establish the correct microbiologic diagnosis.  相似文献

Two of five women with mixed mesodermal tumor of the uterus are well more than ten years after therapy. A third was well when last seen two years after therapy and died at age 84 but autopsy was not done.Review of the literature and the reported experience indicates that this diagnosis should be suspected more often. Since histologic features may vary from one place to another within the lesion, accurate assessment requires examination of adequate specimens of the tumor. The mere presence of heterologous elements in an endometrial carcinoma changes the five-year survival rate from 85 percent to less than 35 percent. While total abdominal hysterectomy with bilateral salpingo-oophorectomy is mandatory, radiation therapy should be added more often than in the past, as it has been clearly shown that many of these tumors are radiosensitive.  相似文献

Sangiran hominid 2 (S-2), Gibraltar hominid 1 (G-1), and Shanidar hominid 5 (SH-5) exhibit previously undescribed bilateral, paramedian hyperostosis of the endocranial frontal squama that spares the frontal crest, sagittal sinus, and ectocranial surface. The hyperostosis is localized to the frontal (usually the middle third) and parietal and is consistent with a diagnosis of hyperostosis calvariae interna (HCI), inclusive of hyperostosis frontalis interna. The hyperostosis in these specimens is compared to fossil hominids from Indonesia and Europe and to modern human cases of HCI. The three cases of HCI reported here document the existence and frequency of HCI in fossil hominids and push the antiquity of the disease back to nearly 1.5 millin years. The relatively great incidence of HCI in fossil hominids adds another confounding factor to the problematical issue of the taxonomic significance of cranial vault thickness. Previous investigations revealed that fish inner ear otolith growth depends on the amplitude and the direction of gravity. Both otolith total size, otolith bilateral size-asymmetry and the total and bilateral calcium-incorporation are also affected by gravity. Hypergravity, e.g., slows down otolith growth and diminishes bilateral otolith asymmetry as compared to 1 g control specimens raised in parallel. Since the enzyme carbonic anhydrase (CA) plays a prominent role in otolithic calcification, the reactivity of inner ear CA during otolith growth under hypergravity was investigated. CA-reactivity was demonstrated histochemically and densitometrically on sections of inner ear maculae of larval cichlid fish (Oreochromis mossambicus), that were kept for 6 hrs in a 3 g hypergravity centrifuge. The total unilateral macular CA-reactivity and the bilateral difference in CA between the left and the right maculae were significantly lower in 3 g animals than in 1g controls. The result is in complete agreement with previous studies indicating that a regulatory mechanism, which adjusts otolith size and asymmetry towards the gravity vector, acts via activation/deactivation of macular CA.  相似文献

Observations on 56 specimens of Aratinga pertinax when bringing food to the beak prove that 28 birds were right-handed while the other 28 were left-handed. A biometric analysis reveals a slight departure from bilateral symmetry in hindlimb bones in close relationship with handedness. In the right-handed birds, there is a significant predominance in the length of the right hindlimb as a whole and all right limb segments. The opposite holds true in left-handed parrots (except for the femur). Right-handed birds are in general more highly asymmetrical than left-handed ones. There is a positive correlation between asymmetry of homologous segments of a pair of limbs and the asymmetry of the other segments of the same pair of limbs. The absence of significant negative correlations between bilateral differences of the different segments of a limb indicates that departures from bilateral symmetry tend to affect the limb as a whole, and thus is not in agreement with the rule of compensating variations.  相似文献

The cytohistologic and ultrastructural findings in a case of bilateral renal oncocytoma diagnosed by needle aspiration cytology are presented. This case appears to be the first reported example in which the preoperative needle aspiration cytologic diagnosis of bilateral renal oncocytoma was made due to the presence of characteristic tumor cells that were seen singly or in small clusters with rounded polygonal shapes and abundant eosinophilic granular cytoplasm. The preoperative diagnosis of bilateral oncocytoma permitted a timely decision as to the appropriate management in this case.  相似文献

In a previous paper (Hiraba and Sato ) we reported that an accurate mastication might be executed by the cortical processing in bilateral masticatory area (MA)and motor cortices. The aim of this study was to determine if cats with lesion of either unilateral or bilateral MA showed changes in mastication. After exploring mechanoreceptive fields and motor effects of mastication-related neurons (MRNs) in MA using the single unit recording and intracortical microstimulation methods, we made various lesions in MAs with injections of kainic acid (0.1%, 2.0?µl). Since the MA was divided into facial (F) and intraoral (I) projection areas as reported in the previous paper, cats with the unilateral lesion in F or I, and with the bilateral lesion in F & F, I & I or F & I (F on one side and I on other side) were prepared. Cats with unilateral lesion in F or I and with bilateral lesion in F & I showed no changes in mastication except for prolongation of the food intake and masticatory periods. Cats with bilateral lesion into F & F, or I & I showed wider jaw-opening during mastication. Particularly, the latter group showed enormous jaw-opening, delay in the start of mastication and difficulty in manipulating food on the tongue. In all cats with lesions of each type, masticatory and swallowing rhythms remained normal. These findings suggest that accurate mastication is executed by the close integration between F & F and I & I of the bilateral MA.  相似文献

Inattention to current activity is ubiquitous in everyday situations. Mind wandering is an example of such a state, and its related brain areas have been examined in the literature. However, there is no clear evidence regarding neural rhythmic activities linked to mind wandering. Using a vigilance task with thought sampling and electroencephalography recording, the current study simultaneously examined neural oscillatory activities related to subjectively reported and behaviorally indexed mind wandering. By implementing time-frequency analysis, we found that subjectively reported mind wandering, relative to behaviorally indexed, showed increased gamma band activity at bilateral frontal-central areas. By means of beamformer source imaging, we found subjectively reported mind wandering within the gamma band to be characterized by increased activation in bilateral frontal cortices, supplemental motor area, paracentral cortex and right inferior temporal cortex in comparison to behaviorally indexed mind wandering. These findings dissociate subjectively reported and behaviorally indexed mind wandering and suggest that a higher degree of executive control processes are engaged in subjectively reported mind wandering.  相似文献

Although congenital digital defects, particularly polydactyly, have been reported frequently in humans, their occurrence in rhesus macaques is relatively rare. We observed two cases of spontaneous digital defects in male rhesus monkey infants recently born at the California Regional Primate Research Center. One infant exhibited bilateral postaxial polydactyly and the other infant had bilateral oligodactyly with unilateral phalangeal duplication. In this report, we present the clinical/pathological details of these cases as well as discuss the embryology of normal and abnormal limb development. We will also summarize a variety of spontaneous and experimentally induced digital defects that have been reported in several nonhuman primate species.  相似文献

BACKGROUND: Primary papillary serous carcinoma (PPSC) of the peritoneum is a rare neoplasm, histologically indistinguishable from papillary serous carcinoma of the ovary, which diffusely involves the peritoneum but spares or minimally invades the ovaries. To the best of our knowledge, the preoperative and the fine needle aspiration diagnosis of this disorder have not been reported before. CASE: A woman developed an extensive peritoneal neoplasm 4 years after hysterectomy and bilateral salpingo-oophorectomy for benign disease. Fine needle aspiration of the tumor was performed, and the cytologic and immunocytochemical findings were consistent with papillary serous carcinoma. A diagnosis of PPSC of the peritoneum was rendered because review of all slides from previous surgical specimens showed no evidence of carcinoma and no other primary tumors were found elsewhere. CONCLUSION: Fine needle aspiration cytology coupled with immunocytochemical and clinical data allows an unequivocal preoperative diagnosis of papillary serous carcinoma (primary peritoneal or with an ovarian origin). The sole limitation to establish a primary peritoneal origin before surgery is the requirement to histologically study the ovaries. Based on this fact, the preoperative fine needle aspiration cytology diagnosis of PSCP should be restricted to oophorectomized patients.  相似文献

Although muscles are assumed to be capable of stabilizing the spinal column in vivo, they have only rarely been simulated in vitro. Their effect might be of particular importance in unstable segments. The present study therefore tests the hypothesis that mechanically simulated muscle forces stabilize intact and injured cervical spine specimens. In the first step, six human occipito-cervical spine specimens were loaded intact in a spine tester with pure moments in lateral bending (+/- 1.5 N m), flexion-extension (+/- 1.5 N m) and axial rotation (+/- 0.5 N m). In the second step, identical flexibility tests were carried out during constant traction of three mechanically simulated muscle pairs: splenius capitits (5 N), semispinalis capitis (5 N) and longus colli (15 N). Both steps were repeated after unilateral and bilateral transection of the alar ligaments. The muscle forces strongly stabilized C0-C2 in all loading and injury states. This was most obvious in axial rotation, where a reduction of range of motion (ROM) and neutral zone to <50% (without muscles=100%) was observed. With increasing injury the normalized ROM (intact condition=100%) increased with and without muscles approximately to the same extend. With bilateral injury this increase was 125-132% in lateral bending, 112%-119% in flexion-extension and 103-116% in axial rotation. Mechanically simulated cervical spine muscles strongly stabilized intact and injured cervical spine specimens. Nevertheless, it could be shown that in vitro flexibility tests without muscle force simulation do not necessarily lead to an overestimation of spinal instability if the results are normalized to the intact state.  相似文献

Glomerulonephritis associated with antibody to glomerular basement membrane, shown by linear staining of the glomerular basement membrane with fluoresceinated anti-IgG antisera, was found in only 10 out of 400 (2·5%) renal biopsy specimens studied by immunofluorescence. Seven of these cases had rapidly progressive glomerulonephritis, five with lung haemorrhage (Goodpasture''s syndrome) and two without, and three had less severe nephritis without lung haemorrhage. Circulating antibody to glomerular basement membrane, measured by a passive haemagglutination technique and by indirect immunofluorescence, was detected in the serum of all patients with rapidly progressive glomerulonephritis by both techniques but only by the passive haemagglutination method in two of the other three patients. Two patients died of their lung haemorrhage, one despite bilateral nephrectomy, and lung haemorrhage and circulating antibody to glomerular basement membrane persisted after bilateral nephrectomy in another patient.  相似文献

Biopsy specimens from 38 "de-epithelized" dermal pedicles were examined microscopically. There was considerable variation in the depth of the plane among surgeons, procedures, and even between specimens from two sides of a bilateral procedure done by the same surgeon. Usually, the "de-epithelization" removed all the epidermis plus the upper layer of dermis containing the pilosebaceous apparatus. The significance of this finding as related to the future development of epidermal inclusion cysts is uncertain. Also, it brings into question the importance of "the dermal plexus circulation," which many have thought to be critical for viability of the nipple.  相似文献

Abstract: A rare case of bilateral adrenal myelolipomas in a female cotton-top tamarin is reported. Large bilateral masses in the adrenal glands were composed of mature adipose cells containing varying amounts of hematopoietic cells of the myeloid, erythroid, and megakaryocyte series. Ten survivors of sporadic bilateral retinoblastoma had 14 offspring, of whom eight were affected, seven of them in both eyes. Other reports from the literature raise the total of similar unselected cases to 19 survivors with a total of 39 offspring, of whom 17 were affected in both eyes and three in one eye.The high incidence of the bilateral affection in dominantly inherited retinoblastoma—as recorded in the literature—and in the offspring of survivors from sporadic bilateral retinoblastoma, as reported in the present study, establish all cases of bilateral retinoblastoma as a dominant disorder either in transmission or as a new mutation. This disorder, though fully or almost fully penetrant, is not always fully expressed. A small proportion, probably about 5 to 10% of all cases of the much more common sporadic unilateral affection, are in fact incompletely expressed germinal mutations for bilateral retinoblastoma. There is some evidence that histological appearances may distinguish these potentially transmissible unilateral tumours from the mass of unilateral retinoblastoma which have no genetic significance.  相似文献
