There are few formally documented proficiency testing programs for cytology laboratories, and those that have been documented are not entirely comparable in format. The first of three papers documenting a mandatory universal proficiency testing program for cytology laboratories in the Province of Ontario, Canada, presents data on the structure and function of the participating laboratories (including a comparison of the data for 1974 and 1980) and on the organization of the testing model (including selection of terminology, construction and use of the survey and assessment of responses). In 1980, of the 463 medical laboratories in Ontario, 91 of 222 hospital laboratories and 65 of 216 nonhospital laboratories were licensed in cytology. In that year, the 156 cytology laboratories processed 1.48 million cytology specimens, 92% of which were gynecologic. Hospital laboratories processed 87.5% of the nongynecologic cytology specimens and 30% of the gynecologic cytology specimens. These proportions have been virtually constant for several years. Between 1974 and 1980, there was a trend in Ontario to fewer laboratories processing less than 5,000 cytology specimens per annum. Subsequent papers in this series describe the results of the initial surveys in this program and a precision study to evaluate the consistency of reporting by individual laboratories.  相似文献

Leishmania parasites are eukaryotic protozoans responsible for a variety of human diseases known as leishmaniasis, which ranges from skin lesions to fatal visceral infections. Leishmania is transmitted by the bite of an infected sandfly where it exists as promastigotes and, upon entry into a mammalian host, differentiates into amastigotes, which replicate exclusively in macro-phages. The biochemical pathways enabling Leishmania to differentiate and survive in the mammalian host are poorly defined. We have therefore examined the role of protein-tyrosine phosphorylation, which is essential in regulating cell function in higher eukaryotes. Using the recently completed Leishmania genome, we have identified and cloned a Leishmania protein-tyrosine phosphatase (PTP) gene (LPTP1) by virtue of its homology with the human protein-tyrosine phosphatase 1B gene (hPTP1B). The enzyme activity of recombinant LPTP1 was confirmed using a combination of PTP-specific substrates and inhibitors. We further demonstrate, by creating LPTP1 null mutants through gene targeting, that LPTP1 is necessary for survival as amastigotes in mice, but it is dispensable for survival as promastigotes in culture. Human PTPs, including the PTP1B enzyme, are actively pursued drug targets for a variety of diseases. The observations with the LPTP1 mutants in mice suggest that it may also represent a drug target against the mammalian amastigote stage. However, in silico structure analysis of LPTP1 revealed a striking similarity with hPTP1B in the active site suggesting that, although this is an attractive drug target, it may be difficult to develop an inhibitor specific for the Leishmania LPTP1.  相似文献

The objectives of this study were to establish a profile of cervical cytology laboratories in Washington State, identify quality assurance problems amenable to correction through education or legislation, and describe differences between large and small cytology laboratories. All 43 Washington laboratories that perform cervical cytology were surveyed by mail during 1989. Completed surveys were returned by 37 (86%) of the laboratories. Nearly half (43%) of the respondents reported processing less than 10,000 Papanicolaou smears annually. Only one-third (35%) of the respondents reported participating in relevant proficiency programs. A proportion of smaller cytology laboratories were compensating their cytotechnologists on the basis of the number of slides read and allowing Papanicolaou smears to be read outside the confines of the laboratory. The results of this study suggest that cytotechnologists in some larger Washington laboratories have been exceeding work load limits recommended by professional associations. Recent legislation includes regulations that address cervical cytology quality assurance. However, continued efforts will need to be made to encourage voluntary adoption of quality control measures not addressed by this legislation.  相似文献

In the olfactory bulb (OB) of the pike, phenomena of long posttetanic potentiation (LPTP) and long posttetanic depression (LPTD) are reproduced. Probability of induction of LPTP and level of its expression are shown to depend direct on parameters of density of axo-dendritic synapses in the glomerular neuropil and size of its active zones (AZ). In pikes with the high level of these parameters (the synapse density 5.2 per 20 µm2, the mean length of AZ cross-section 293 nm), potentiation appear in 95% of cases, while in pikes with low values (the density 3.1, the AZ length 239 nm) in as low as 30%, with LPTD developing rather often instead of LPTP. The LPTP in the phase of its maximal development (60 min after tetanus) corresponds to an increase of the mean length of AZ cross-section of axodendritic synapses by 13.6%, while the LPTD—to its decrease by 16.5% as compared with control. The LPTP induction phase (5–15 min after tetanus) is not associated with any ultrastructural transformations of postsynaptic AZ of axo-dendritic synapses. By the morphometric method, it has been established that LPTP in the pike OB is of the homosynaptic nature, i.e., is due to an enhancement of synaptic efficiency of the excitatory afferent input just subjected to tetanization. In rat hippocampal slices, synaptic transmission from Schaffer collaterals to CA1 neurons was studied by assessment of the effects of cessation of low-frequency testing stimulation to the population spike amplitude. A positive correlation between the duration of the stimulation interruption period (from 10 to 120 min) and the increment of the population spike amplitude (from 30 to 100% above baseline level). The "deprivational" potentiation (DP) offers a property of summation and is able to persist for long (testing was continued up to one hour). Prior induction of the long-term posttetanic potentiation (LPTP) that for 1-3 hours prevents the development of the late rather than the short-term LPTP phase, in our experiments also suppressed the DP induction after 60-min stimulation interruption interval. The same results were obtained in the presence of polymixin B (20 mcM), which is an inhibitor of protein kinase C (PKC) and PKC-dependent phase of the LPTP. This finding provides support for the earlier assumption about the similarity of the mechanisms responsible for the development of the DP and PKC-dependent LPTP phase related with protein phosphorylation.  相似文献

OBJECTIVE: To develop a local quality assurance program in cytopathology based on circulation of patient specimens on glass slides, with limited resources. STUDY DESIGN: A working group was set up for design and running of the program. Participation is on a laboratory basis. The scope and frequency of testing are defined. Well-documented cases (including gynecologic, nongynecologic and fine needle aspiration cytology) with commonly encountered diagnoses are collected. Consensus concerning the diagnosis, interpretive menu and scoring system is sought before the actual slide circulations using express mail. After returning their answers to the program organizer, the participating laboratories receive immediate feedback on their scores, with reference answers, explanatory notes, "whole-mount" images of glass slides and cumulative responses of peer laboratories for on-site checking. At the end of each year, an electronic file containing representative photomicrographs of all cases examined is provided to individual laboratories for their permanent records and training purposes. RESULTS: The program was launched in mid-2003. There were 24 and 27 participating laboratories from Hong Kong (and Macau) in 2003 and 2004, respectively. To date, >150 well-documented cytology cases are available in the slide pool and ready for circulation. As the revenue is mainly to cover the expenses of express mail, the program can be carried out at a relatively low cost. CONCLUSION: In order to have any cytology quality assurance program accepted by local laboratories, it has to be fair and practical. Strict confidentiality needs to be observed throughout the process. This program emphasizes both performance assessment and educational value. Adequate representation from experienced local cytology workers, detailed documentation support from authorities and assistance from dedicated staff are essential to the success of any external proficiency testing scheme. Regular review and evaluation are also necessary for continuous improvement. The Hong Kong experience can serve as an example of running a glass slide-based cytology quality assurance program in a small region with limited resources.  相似文献

There has been a reduction in inadequate rate and some data to suggest an increase in sensitivity for dyskaryosis. Training for LBC has focused on differences in distribution of abnormal cells, but in most cases the morphological appearance of the dyskaryotic cells themselves is similar to that seen in conventional cytology. We are describing a new presentation of dyskaryosis which may be a cause of false negative cytology. We have referred to this as 'Bland dyskaryosis' because cells appear deceptively bland on low power examination, and can be misinterpreted as metaplastic or endocervical cells. Bland dyskaryosis cells are seen in groups. The architecture of the group is very disorganized, and adjacent cells show variation in size. Cells have a high nuclear/cytoplasmic ratio and smooth nuclear membranes. Chromatin is finely granular and evenly distributed. This is an unusual presentation of high-grade dyskaryosis and we feel that there is a learning curve in laboratories converting to liquid based cytology. The spectrum of appearances of squamous dyskaryosis needs to be delineated to allow further increases in sensitivity for dyskaryosis.  相似文献

In hippocampal slices of rats was studied the influence of different gangliosides on the dynamics of development of long-term post-tetanic potentiation (LPTP) in the pyramidal cell layer of the CA3 area at stimulation of the mossy fibers. Each ganglioside was shown to change synaptic efficiency specifically after the tetanic stimulation. Incubation of hippocampal slices with monosialoganglioside GM1 induced the rapid increase of population spike amplitude in the pyramidal neurons being of higher level in comparison to that of the active control up to the end of the experiment. Disialoganglioside GD1b increased the amplitude of the summary cellular response to a lesser degree than GM1, but contributed to its conservation up the control level in the course of the whole recording period. Gangliosides GD1a and GT1b induced inhibitory action on LPTP decreasing population spike value lower than that of both the control and initial levels, GT1b causing more rapid decrease of amplitudes of cellular responses than GD1a. A conclusion was drawn on the participation of gangliosides in the mechanisms of synaptic plasticity. The differential influence of various kinds of gangliosides on the LPTP dynamics was found out. The possible mechanisms of these reactions are discussed.  相似文献

OBJECTIVE: To compare the use of Papanicolaou and Wright-Giemsa stains for the evaluation of body fluids in cytology and hematology laboratories and determine whether other factors account for discrepancies in diagnosis. STUDY DESIGN: We retrospectively reviewed cytopathology reports of peritoneal, pleural, and cerebrospinal fluids received by hematology and cytology laboratories for 1 year. Cases were divided into 3 categories-benign, atypical, and malignant--and slides of discrepant diagnoses were reviewed. RESULTS: During this period, 198 of 3212 (0.61%) cases received by the hematology laboratory and 252 of 4402 (0.57%) cases received by the cytology laboratory were diagnosed as malignant or atypical. Of 3212 cases simultaneously received by the cytology and hematology laboratories, 17 diagnosed as malignant by hematology were diagnosed benign by cytology (sensitivity 96%). Sixteen cases diagnosed as malignant by cytology were diagnosed as benign by hematology (sensitivity 97%). No benign cases were diagnosed as malignant (specificity 100%). Review of the glass slides of the discrepant cases revealed 8 cases undercalled by hematology and 7 cases undercalled by cytology. CONCLUSION: Papanicolaou stain is superior for carcinoma and Wright-Giemsa stain for hematopoietic disorders, but used together they may reduce false negative results. Delays in processing, staining technique, and interobserver variability contribute to discrepancy.  相似文献

Comparison of cytology and histology results in English cervical screening laboratories before and after liquid‐based cytology conversion: do the data provide evidence for a single category of high‐grade dyskaryosis? Objective: To determine whether the difference between the positive predictive value (PPV) for cervical intraepithelial neoplasia (CIN) grade 2 or worse (CIN2+) of referral from moderate dyskaryosis and from severe dyskaryosis was reduced after laboratories converted from conventional to liquid‐based cytology (LBC). Furthermore, to explore the cytology/histology agreement after LBC conversion, and to determine post‐LBC whether there was increased support for the use of one single category of high‐grade dyskaryosis (equivalent to high‐grade squamous intraepithelial lesion). Methods: The association between cytology and histology has been examined using annual Korner return data (KC61 returns) collected by laboratories from the English National Health Service cervical screening programme. The study compares return data before and after LBC conversion. Results: The study examined data from 102 laboratories that converted from conventional cytology to LBC. Before conversion the PPV for CIN2+ of severe dyskaryosis was 88% and after increased to 90% (P = 0.003). For moderate dyskaryosis the PPV for CIN2+ increased from 70% to 72% (P = 0.06). The absolute difference of 18% between severe and moderate dyskaryosis was therefore the same pre‐ and post‐LBC conversion. The PPV of mild dyskaryosis for CIN2+ before and after conversion reduced from 23% to 19% (P < 0.001). The agreement between cytology and histology measured using a weighted Kappa statistic increased from 0.52 to 0.60 after conversion to LBC because of small increases in the proportions of severe dyskaryosis or worse with CIN3+ outcomes and mild dyskaryosis with CIN1 or less outcomes. Conclusions: Following LBC conversion there was evidence of a modest increase in the agreement between cytology and histology but no evidence of a change in the absolute difference in PPV for CIN2+ between moderate and severe dyskaryosis. The data support the conclusion that women referred with moderate dyskaryosis will on average have a lower risk of progression to invasive cancer than women referred with severe dyskaryosis. If the data were considered to support the categories of high‐grade dyskaryosis (moderate) and high‐grade dyskaryosis (severe) before LBC conversion then it can be strongly argued that they also support these categories after conversion.  相似文献

O’Rourke Breast fine needle aspiration cytology: a review of current practice in Australasia Aims: To investigate breast fine needle aspiration (FNA) cytology in Australasia, in terms of laboratory demographics, specimens received and quality control (QC). Methods: A questionnaire was sent to all laboratories enrolled in the FNA module of the Royal College of Pathologists of Australasia Cytopathology Quality Assurance Program (QAP), requesting information about specimens received in a 3‐month period in 2001 and in 2008. Results: Responses were received from 81/180 laboratories in 2001 and from 94/200 in 2008. The mean number of cases per 3 months was 137 in 2001 and 141 in 2008 and for the 42 laboratories responding on both occasions, the mean number of cases declined from 191 to 149 (P = 0.001). The mean percentage of malignant cases was 11.7% in 2001 and 10.5% in 2008 and the mean percentages of unsatisfactory rates were 21.7% and 25.2%, respectively; 43.2% of laboratories in 2001 and 40.4% in 2008 reported fewer than 50 cases for the 3‐month period. The unsatisfactory rate was inversely proportional to the number of cases received. Most QC (69.1% in 2001, 71.3% in 2008) was carried out by correlation with any later histology. With no histology available, 35.8% of laboratories in 2001 and 48.9% in 2008 did no further follow‐up. Follow‐up of all diagnostic categories increased from 30.9% in 2001 to 44% in 2008. Conclusions: Breast FNA cytology is still actively undertaken in Australasia, but numbers have declined. Unsatisfactory rates have reached the Australian recommended upper limit and are inversely proportional to the total number of cases received. Overall QC measures are unchanged and consideration of a review of breast FNA guidelines is suggested.  相似文献

Cytopathology started in Slovenia in the early 50s with exfoliative cytology, while fine needle aspiration biopsy (FNAB) was introduced some 10 years later. Today cytopathology is a well accepted diagnostic method in Slovenia and there are currently 20 cytopathological laboratories and 17 cytopathologists. The number of specimens examined in 2001 was 26 230 FNABs, 13 355 exfoliative non cervical and 323 888 cervical smears. FNABs are performed by cytopathologists, by clinical doctors and by radiologists. So far only the cytopathologists have a supervised training period in performing biopsies. In future the same requirement will be obligatory for non pathologists. In four laboratories immunocytochemistry is used as an ancillary technique to morphology and one laboratory is using also flow cytometry for immunophenotyping of lymphomas. The classification system used in Slovenia for reporting the findings in cervical cytology is a combination of Papanicolaou's classification and assessment of dyskariosis. In spite of a long tradition in opportunistic screening for cervical cancer (Cca) an organised screening programme was started late. A four year pilot study, which included one third of the women population of Slovenia, was concluded in 2002 and an organised screening programme was introduced to the whole country in 2003. The incidence rate of Cca in Slovenia has been rising slowly since 1994 and it reached 19.6/100 000 in the year 2000. The mortality rate has remained roughly constant at 5-7/100 000 for the last 20 years. During the last few years quality assurance measures have been taken for improving the performance in cervical cytology.  相似文献

Given interest from the professionals concerned, an external quality assurance scheme for cervical cytology can successfully be introduced in developing countries. This is a very important precondition if screening programs are to be expanded and decreases in mortality from cervical cancer are to occur in developing countries. Nicaragua and Peru have been experimenting with an external quality assurance system adapted from the Scottish and Northern Ireland scheme. It has been received with enthusiasm and acceptance and has helped cytology laboratories in these countries focusing on quality issues. Nevertheless, a successful quality control scheme that is to result in improvements in the quality of professionals' diagnostic skills needs to be accompanied by a remedial program for subperformers.  相似文献

OBJECTIVE: When a laboratory has a low reporting rate of high-grade abnormality for its cervical cytology specimens, the question arises whether this is due to the laboratory screening a low-risk group of women. This study was undertaken to explore the hypothesis that a low-risk group of women were screened by Victorian laboratories not meeting the recommended minimum standard for the detection of high-grade abnormalities. METHODS: A cohort of 28 094 women was established comprising women whose cervical cytology was reported in 2000 by laboratories not meeting the recommended standard of 0.5% high-grade abnormalities in their reporting of community smears. Outcome measures included the prevalence of high-grade abnormality on the next cytology test for the women, the positive predictive value of the cytology reports of high-grade abnormality, and the standardized incidence ratio for a later diagnosis of cervical cancer. RESULTS: The prevalence of high-grade abnormality was 0.36% on the index cytology compared with 0.68% on the first subsequent cytology report. Sixty-nine per cent (60/87) of the index cytology reports of high-grade abnormality were confirmed as high-grade abnormalities on histology, compared with 70.8% (121/171) for the first subsequent reports of high-grade abnormality. During 70 015 person-years at risk, nine cases of cervical cancer were observed compared with 5.21 expected cases, giving a standardized incidence ratio of 1.73 (95% CI 0.79-3.28). CONCLUSIONS: These results do not support a hypothesis that the low detection rate for high-grade abnormalities is due to the women comprising a low-risk group for cervical neoplasia.  相似文献

OBJECTIVE: To validate the use of a Web-based tutorial to introduce the Bethesda System (TBS) to Mainland Chinese laboratories. STUDY DESIGN: Digitized color images of the diagnostic features in 20 Pap smears were displayed on a Web page. Participants were asked to give each smear a diagnosis using the reporting nomenclature employed in their laboratory or one that was familiar to them. This was followed by teaching images of each smear accompanied by text in English and Chinese that highlighted important features for making a diagnosis using TBS. Participants then reviewed the 20 original Pap smears and rendered a diagnosis using TBS. RESULTS: Pathologists and cytotechnologists at 17 laboratories located in 10 cities completed the exercise. The average diagnostic accuracy for the 20 Pap smears before and after the tutorial was 76% and 88%, respectively. CONCLUSION: Web-based tutorials can be used for disseminating cytologic information to widely dispersed laboratories in China and help enhance the practice of cytology, currently an underutilized diagnostic technique. However, such difficulties as lack of Internet connections in the laboratory, outdated computers and a lack of interest in cytology need to be overcome to ensure success.  相似文献

Regulatory controls of quality assurance in veterinary laboratories are less common than in human reproduction laboratories and the intra- and inter-technician variation in the assessment of canine vaginal cytology has not been reported. This study was designed to determine whether variation in classification of vaginal epithelial cells and interpretation of vaginal cytology smears existed within and between technicians in a canine reproductive laboratory.Sixteen vaginal cytology smears representing different known stages of the oestrous cycle were examined twice by one experienced technician and three inexperienced technicians in a blinded random order study design. Seven assessments were made; counting and classifying one hundred vaginal epithelial cells into four morphological classifications and assessment of three cellular categories. Technicians also interpreted their results and reported the stage of the cycle they thought each slide represented. In addition, selected samples were sent to four external commercial laboratories for interpretation.For the experienced technician, intra-technician variation was low for the morphological classifications and cellular assessments (r = 0.69-0.95). There was more intra-technician variation between results from Examination One and Examination Two for the inexperienced technicians (r = 0.53-0.92 where correlations were found). When inexperienced technicians' results were compared to results from the experienced technician, the inter-technician variation was low; results were correlated for 17 of the 21 observations (four morphological classifications and three cellular assessments across the three technicians) (r = 0.38-0.87). When technician interpretations of stage of the oestrous cycle were compared to the known stage of the cycle for each smear, the experienced technician correctly interpreted 19 of the 32 smears, whilst the three inexperienced technicians correctly interpreted 14, 16, and 18 of the 32 smears. The interpretation of vaginal smears by external laboratories was varied and sometimes inconclusive; 50% of laboratories incorrectly identified metoestrus smears as proestrus and 25% of the laboratories incorrectly identified an oestrus smear as proestrus.The results of this study are highly important for clinicians undertaking canine reproductive assessments since they demonstrate the potential for variability of results. While the greatest precision was found when vaginal smears were examined by an experienced technician (who, on a daily basis, examines many smears), more variability in both the reporting of different cell types and interpretation of the smears was observed by inexperienced technicians and when samples were sent to external commercial laboratories. These findings suggest that suitable quality control programmes should be implemented for laboratories that are undertaking routine assessments of canine reproductive function.  相似文献

ABC3 Part II: a review of the new criteria for evaluating cervical cytology in England The new Achievable Standards, Benchmarks for Reporting, and Criteria for Evaluating Cervical Cytopathology, 3rd edn (ABC3) includes radical changes in the criteria for evaluating cervical cytology. First, they include a new mission statement the objective of cervical screening is to reduce cervical cancer incidence and mortality by screening with a high sensitivity for the detection of CIN2 or worse, whilst maintaining a high specificity’. Second, the original four performance measurement criteria where laboratories were examined further if they were below the 10th or above the 90th percentile has been changed to three and laboratories are only mandatorily examined if they fall below the 5th or above the 95th percentile. The old criteria related to the percentage of samples that were inadequate, the percentage of all adequate samples reported as moderate dyskaryosis or worse (equivalent to high‐grade squamous intraepithelial lesion or cancer), the percentage of adequate samples reported as mild dyskaryosis or borderline (equivalent to low‐grade squamous intraepithelial lesion or atypical squamous/glandular cells) and the positive predictive value. The new criteria are percentage of inadequate samples, positive predictive value and a new measure termed referral value. These changes mean that far fewer laboratories will require mandatory examination. Third, a raft of optional performance measures have been introduced to help laboratories examine their annual statistical return to the Department of Health in comparison with other laboratories. These measures have been designed to produce a more uniform national programme, and to help laboratories decide whether they are maximizing the benefit of screening while minimizing the harm, which is the goal of all screening programmes. This review examines in detail the new criteria and explains in more detail some of the thinking behind them.  相似文献

A review of the first 14 years of the New York State Proficiency Testing Program in cytology shows the value of external quality-control evaluation based upon an established standard of performance as a prerequisite for licensure by a regulatory agency. Performance by participating laboratories has improved over the 14-year period. Hospital and independent laboratories had a higher pass rate than did state-run laboratories. Laboratories that did not employ cytotechnologists and performed fewer than 1,000 tests annually tended to have the lowest pass rates and consequently to undergo the most training before retesting and licensing. Overall, the predominant type of error, regardless of specimen type, was false negatives (underreads). Cervicovaginal false-negative errors by cytotechnologists ranged from 2% to 8%; such errors by cytopathologists ranged from 2.1% to 6.9%. Deficiencies and weaknesses detected by proficiency testing were remedied through a combination of surveillance and education.  相似文献

OBJECTIVE: Cervical screening programmes in England are in transition as the liquid-based cytology (LBC) method replaces conventional Papanicolaou screening and staff in NHS laboratories are trained to analyse LBC smears. Cytoscreeners and biomedical scientists undertake routine microscopy of slides, but the scientists usually have a wider professional role. Attitudinal surveys were carried out in laboratories where LBC was partially introduced. METHODS: Staff in two cytology laboratories in Greater Manchester were surveyed twice over 6 months. The questionnaire assessed work pressures using scales from the Measures of Work Characteristics instrument, work-related stress using the General Survey version of the Maslach Burnout Inventory, job intentions and job satisfaction. RESULTS: Cytoscreeners, many aged over 50 years, formed over 60% of respondents in both surveys (27/42 in the first survey), and biomedical scientists and doctors, 30%. Both groups were under moderate pressure from work demands in each survey, but cytoscreeners had significantly less autonomy over their working methods (P < 0.001). Although both groups experienced similar levels of exhaustion, cytoscreeners were much more cynical or indifferent towards work in the second survey (P = 0.008) and had lower expectations of being effective (P < 0.001). For the cytoscreeners, there were strong negative correlations in both surveys between cynicism and the work characteristics of influencing decisions and autonomy/control. CONCLUSIONS: The strength of the relationship between work performance and wellbeing serves to emphasize the importance of the new LBC technology in ameliorating low morale where it exists. Further attitudinal research involving larger samples of laboratories is warranted to assess the full impact of this innovation.  相似文献
