In some patients with antibodies against LPS antigen of Chlamydia, specific immunoglobulins G are not present. The findings of isolated anti-LPS IgA/IgM antibodies are to be considered as possibly non-specifically or "false" positive. The aim of this study was to investigate if there is any difference in the level of total immunoglobulins of isotypes IgG and IgG2 in probands with isolated positivity anti-LPS IgA (i.e. without simultaneous presence of specific IgG; n = 34) as compared with a control group of subjects presenting positive anti-LPS IgA and IgG antibodies (n = 44). Antibodies against LPS antigen of Chlamydia were determined by ELISA method ("Chlamydien--rELISA", medac, Germany). Total immunoglobulin levels were determined by nephelometry using the following kits: "Immunoglobulin G Reagent, ARRAY Systems", Beckman Coulter, USA and "Human IgG2 Subclass Beckman ARRAY Kits", The Binding Site, UK. The measured values were related to the age-dependent laboratory standard values and the differences between the tested groups were statistically evaluated (chi(2) test). Decreased total IgG have been demonstrated in 4 (11.8 %) probands and in 5 (11.4 %) subjects of the control group; increased total IgG were found in 2 (5.9 %) probands and in 1 (2.3 %) subject of the control group. Decreased levels of total IgG2 were not determined in any proband and were found in 1 (2.3 %) subject of the control group. Increased levels of IgG2 were registered in 12 (35.3 %) probands and in 15 (34.1 %) control subjects. No statistically significant differences were found between the two groups. It can be concluded that no relationship was proved between the levels of total IgG and IgG2 and the absence of formation of specific anti-Chlamydia-LPS IgG. Further research will be necessary for the elucidation of this phenomenon (e.g. the presence of specific anti-LPS IgG in immunocomplexes).  相似文献

Background

Research indicates that certain personality traits relate to performance in the medical profession. Yet, personality testing during selection seems ineffective. In this study, we examine the extent to which different medical school selection processes call upon desirable personality characteristics in applicants.

Methods

1019 of all 1055 students who entered the Dutch Bachelor of Medicine at University of Groningen, the Netherlands in 2009, 2010 and 2011 were included in this study. Students were admitted based on either top pre-university grades (n = 139), acceptance in a voluntary multifaceted selection process (n = 286), or lottery weighted for pre-university GPA. Within the lottery group, we distinguished between students who had not participated (n = 284) and students who were initially rejected (n = 310) in the voluntary selection process. Two months after admission, personality was assessed with the NEO-FFI, a measure of the five factor model of personality. We performed ANCOVA modelling with gender as a covariate to examine personality differences between the four groups.

Results

The multifaceted selection group scored higher on extraversion than all other groups(p<0.01), higher on conscientiousness than both lottery-admitted groups(p<0.01), and lower on neuroticism than the lottery-admitted group that had not participated in the voluntary selection process. The latter group scored lower on conscientiousness than all other groups(p<0.05) and lower on agreeableness than the multifaceted selection group and the top pre-university group(p<0.01).

Conclusions

Differences between the four admission groups, though statistically significant, were relatively small. Personality scores in the group admitted through the voluntary multifaceted selection process seemed most fit for the medical profession. Personality scores in the lottery-admitted group that had not participated in this process seemed least fit for the medical profession. It seems that in order to select applicants with suitable personalities, an admission process that calls upon desirable personality characteristics is beneficial.  相似文献

The aim of the study was to investigate the range of mandibular movements and to analyze the difference in range of mouth opening, right and left lateral movements, and protrusive movement between patients with clinical diagnoses of temporomandibular disorders and asymptomatic subjects (control group) in a young male population. A total of 240 subjects, aged 19-28, were included in the study. The TMD sample comprised 180 patients (60 patients with muscle disorders; 60 patients with disc displacement with reduction; and 60 patients with muscle disorders and disc displacement with reduction) and was compared with 60 healthy control subjects. All participants were evaluated by the attending dentists at baseline by means of a physical examination of the masticatory system and a history questionnaire which included the Research Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD) Axis I measures. Analysis of variance (ANOVA) with the post hoc Bonferroni criteria showed significant difference in ranges of mandibular movements between and within the groups of asymptomatic subjects and TMD patients for active mouth opening (p = 0.001), right lateral movement (p = 0.002), left lateral movement (p = 0.006), and protrusive movement (p = 0.05). It has been found that there are statistically significant differences in the range of mandibular movements that separate asymptomatic subjects and patients with muscle disorders and disc displacements with reduction in this young male population. However, we cannot conclude that measurements of active mandibular movements can discriminate one group (TMD patients) from the other (asymptomatic subjects), because the mean ranges of these active movements between the groups were measured in clinically "normal" values.  相似文献

目的:探讨案例式教学(CBL)教学模式在细胞研究进展教学中的应用效果。方法:选择我校2013~2015级选修"细胞研究进展"的学生80人为研究对象,按选修课程时间的不同分为实验组及常规组各40例。常规组实施传统的教学模式(LBL),实验组实施CBL教学模式,调查两组学生的教学效果并比较两组学生考试得分情况。结果:实验组学生分析问题的能力、临床思维的建立、课堂积极性及教学满意度得分均高于常规组,差异均有统计学意义(P0.05)。实验组学生临床案例分析及临床操作能力得分均高于常规组,差异均有统计学意义(P0.05)。结论:对选修细胞研究进展课程的学生采用CBL模式进行教学,有助于其建立临床思维,提高临床操作能力,提高学生对教学的满意度。  相似文献

目的:探讨目标教学法在护理教学查房中的临床应用价值,以培养护生的临床思维能力,并且提高实践工作能力。方法:选取我院2014年5月~2014年9月在急诊科实习的护生120人为研究对象,在实习之初,按照随机的原则将其分为观察组与对照组,每组60人。在查房中,观察组实施目标教学法,对照组实施常规护理教学法。实习结束后,调查两组护生对查房时教学与课堂效果的满意程度,记录两组护生病历书写的质量以及在实习结束后,通过考核,统计两组护生的在理论成绩、基础护理成绩、专科护理成绩以及护理综合技能方面的成绩,然后对所得到的数据进行统计,以探讨目标教学法在护理教学查房中的临床应用价值。结果:从查房时教学的满意程度及对课堂效果的满意程度来看,观察组都高于对照组,差异具有统计学意义(P0.05);观察组病历书写优良的人数为48,优良率为80%,高于对照组,差异具有统计学意义(P0.05);观察组护生的理论成绩、基础护理成绩、专科护理成绩及护理综合技能的成绩均高于对照组,差异具有统计学意义(P0.05)。结论:目标教学法是一种科学、合理并且适合时代发展要求的新型教学法,应用于护理教学查房中,能提高护生的主动性,激发护生的学习兴趣,充分发掘其潜能,做到理论与实际的结合,从而提高学习者的自身能力,值得临床推广应用。  相似文献

World Wide Web (Web)-based learning (WBL), problem-based learning (PBL), and collaborative learning are at present the most powerful educational options in higher education. A blended (hybrid) course combines traditional face-to-face and WBL approaches in an educational environment that is nonspecific as to time and place. To provide educational services for an undergraduate second-year elective course in acid-base physiology, a rich, student-centered educational Web-environment designed to support PBL was created by using Web Course Tools courseware. The course is designed to require students to work in small collaborative groups using problem solving activities to develop topic understanding. The aim of the study was to identify the impact of the blended WBL-PBL-collaborative learning environment on student learning outcomes. Student test scores and satisfaction survey results from a blended WBL-PBL-based test group (n = 37) were compared with a control group whose instructional opportunities were from a traditional in-class PBL model (n = 84). WBL students scored significantly (t = 3.3952; P = 0.0009) better on the final acid-base physiology examination and expressed a positive attitude to the new learning environment in the satisfaction survey. Expressed in terms of a difference effect, the mean of the treated group (WBL) is at the 76th percentile of the untreated (face-to-face) group, which stands for a "medium" effect size. Thus student progress in the blended WBL-PBL collaborative environment was positively affected by the use of technology.  相似文献

Background

Depression is common among diabetic subjects. We conducted the present study to estimate the prevalence of depression in subjects with type 2 diabetes (T2D) in Greece.

Methods

The study sample consisted of 320 T2D subjects without overt macrovascular disease attending the diabetes outpatient clinic of our hospital, from June 2007 to December 2007. Depressive symptoms were measured using the 21-item Beck Depression Inventory, modified for use in diabetic subjects.

Results

Of the study subjects 107 (33.4%) reported elevated depressive symptoms. More women than men with diabetes reported symptoms of depression (48.4% vs. 12.7%, P < 0.001). In the female study group, depressive symptoms were correlated with HbA1c (P = 0.04), and duration of diabetes (P = 0.004). In the male study group, univariate linear regression analysis showed no significant relationships between depressive symptoms and the testing variables.

Conclusion

The prevalence of depression in Greek T2D subjects is high. Diabetic female subjects showed increased levels of depressive symptoms compared with male subjects. Independent risk factors of depressive symptoms in diabetic female subjects were diabetes duration and glycemic control.  相似文献

The effect of the variables "stimulus intensity", "residual effect of previous stimulus intensity", "period effect" and "subject effect" on the amplitudes and on the peak latencies of the N1 and P1 components of the acoustical EEG evoked potentials was studied in human subjects with the use of an orthogonal experimental design. The stimulus intensity (30, 50 and 80 dB) accounts for the greatest part of the total variance of the amplitudes and of the peak latencies of N1.-The existence of a totalvariance component due to differences between the subjects is also statistically significant (P equals 5%), As far as the period effect (habituation effect) is concerned, only its effect on the amplitude of N1 - P2 is statistically significant. - The residual effect of the stimulus intensity used in the previous series was not significant.  相似文献

目的:研究全日制专升本医学微生物学教学改革方法及效果。方法:选取滨州医学院药学院2010年入学全日制药学专升本学生140人,随机分为2组。实验组实施教学模式改革,对照组采取传统教授法教学模式。问卷调查对比两组学生对教师授课的满意度,统计分析两组学生理论考试成绩。结果:实验组学生对教师授课满意度高于对照组,实验组考试成绩高于对照组,差异有显著性。结论:对全日制专升本学生实施教学模式改革,做到了因材施教,促进学生对知识的融会贯通,改革后教学模式的教学效果优于传统教学模式。  相似文献

目的:探讨医学模拟教学结合以问题为导向(PBL)的教学模式在重症医学教学中的应用价值。方法:选取2015年1月~2017年1月在我院重症医学科轮转实习的五年制医学生64人作为研究对象。按随机数字表法将64名医学生分为PBL组(n=32)和结合教学组(n=32)。PBL组采用PBL教学,结合教学组采用医学模拟结合PBL教学。分别在入科时和轮转实习结束时对两组学员进行理论考试和技能操作考核,记录成绩并比较。理论考试和技能操作考核后采取发放问卷进行调查的形式获得学员对教学效果的主观评价。结果:轮转实习结束时,两组理论考试分数差异无统计学意义(P0.05);而结合教学组技能操作考核成分数显著高于PBL组,差异具有统计学意义(P0.05)。两组学员在教学方法接受度高、学习兴趣提升、自学能力提升和临床诊疗水平提高所占比例差异无统计学意义(P0.05);结合教学组学员在团队协作能力提高、沟通能力与人文关怀提高和技能操作水平提高所占比例高于PBL组,差异有统计学意义(P0.05)。结论:与单独PBL教学相比较,医学模拟教学结合PBL的教学模式应用于重症医学教学对学员技能操作的掌握有更好的效果,同时能提高学员团队协作能力、沟通能力以及对患者的人文关怀,且受到学员的认同与喜爱,应在重症医学的教学实践中逐步完善并进一步推广应用。  相似文献

The aim of this research was to investigate the opinions and attitudes of medical staff towards schizophrenic patients. The research included three groups of examinees, 200 physicians of various specialties, 200 nurses and technicians working in Zagreb city hospitals, and 200 3rd and 4th year students of the School of Medicine in Zagreb. Previously validated anti-stigma questionnaire was used, consisting of 25 questions divided into three thematic groups, structured and adapted to the specific requirements of this study. The results were mutually compared and statistically analyzed by applying the chi 2-test. Significant difference (p < 0.01) between the answers of physicians and those of medical students was found in questions 2, 4, 5, 6, 11, 13, 15, 16, 18, 22, 23, 25, and between physicians and nurses/technicians in answers to questions 4, 15, 22, 23. Significant difference (p < 0.01) between the answers given by nurses/technicians and medical students was found in questions 10, 13, 22, 23. The results point to the existence of prejudices and stigmatizing attitudes in all three investigated groups. The most frequent reasons for stigmatizing attitude of students are based on fear and insufficient knowledge about mental patients and schizophrenia as a disease, while there are a high percentage of positive answers to the questions on rehabilitation and resocialization. The nurses/technicians also show a high degree of mistrust towards schizophrenic patients and mostly answer with "I don't know", thus presenting insufficiently formed attitudes about the mentioned problems. The physicians in their answers confirm fear, mistrust and stigmatizing attitudes towards schizophrenic patients found in general population in Croatia. The consequences of such attitudes are the low quality of life of schizophrenic patients, and slow, often incomplete, resocialization.  相似文献

Two groups of consecutive patients from two different plastic surgical practice populations were evaluated to determine psychosocial differences between those who underwent immediate (n = 25) versus delayed (n = 38) breast reconstruction. Psychological assessment consisted of a standardized symptom inventory (BSI) and a specially designed self-report questionnaire investigating reactions unique to mastectomy and reconstruction. Both groups were extremely equivalent with regard to sociodemographic data, with the typical subject being a well-educated and employed Caucasian wife. Verbal reports of physical complaints revealed no significant differences between the two groups except for difficulty with arm movement, which was statistically higher for the immediate group (p = 0.006.). This difference most likely was due to the axillary dissection being performed simultaneously at the time of reconstruction. The relationship between timing of reconstruction and self-reported distress over the mastectomy experience revealed that only 25 percent of the women who underwent immediate repair reported "high distress" in recalling their mastectomy surgery compared with 60 percent of the delayed reconstruction group (p = 0.02). In reference to the two scales measuring psychological symptoms, a general trend was present, with the delayed group scoring higher (although not statistically significantly) on 9 of our 12 scales. 目的:测量医学生学习激情现状,探索相关人口学变量差异及影响因素作用机制。方法:通过网络问卷对1793名在校医学生进行问卷调查,回收有效问卷1218份,应用SPSS22.0软件对数据进行统计处理。结果:测量对象的学习激情处于中等偏上水平(4.37±0.91);学习激情平均得分在性别(F=9.097,P<0.05)、生源地(F=14.850,P<0.05)、独生子女情况(F=5.470,P<0.05)方面的差异有统计学意义;成绩水平(β=0.094,P<0.01)、发展规划(β=0.105,P<0.01)、生活满意度(β=0.213,P<0.01)和活力(β=0.253,P<0.01)对医学生学习激情的影响有统计学意义。结论:医学生学习激情水平在性别、生源地、独生与否方面存在显著差异;成绩水平、发展规划、生活满意度、活力是影响医学生学习激情的因素。  相似文献

The aim of this study was to assess the functional outcome of a population of lower limb amputees supplied with prosthesis. The research was conducted from June to September of 2010 at the Center for Physical and Rehabilitation Medicine, of the Clinical Hospital Center Rijeka, Croatia. The study included 50 adult subjects of both genders with a unilateral transtibial or transfemoral lower limb amputations. The 2-minute walk test (2MWT) was used to assess the functional outcome of these individuals. Data were statistically analyzed. Subjects were divided into groups according to age. The best results were obtained by subjects between the age of 45 and 59 years. The difference between groups was statistically significant (p < 0.001). Taking into account the cause of amputation, there was a statistically significant difference in the results of the 2 MWT between subjects in whom the cause of amputation was circulatory and those where the cause of the amputation was not due to circulatory problems. The best results were obtained in subjects in whom the cause of amputation was not circulatory (p = 0.009). Considering the level of amputation there was a statistically significant difference in the results of the 2MWT between subjects with transtibial and those with transfemoral amputations. Better results were obtained in transtibial amputees (p = 0.039). Considering the first prosthetic supply, better results were obtained in subjects using prosthetic devices over 9 years (p = 0.031). Our research confirmed that age, gender, level and cause of amputation, including the time from the first prosthetic supply have an effect on the 2MWT results.  相似文献

目的:探讨多媒体技术应用于在医学高等院校人体解剖学专业课程中的意义,以及该教学策略对生物医学教学实践的作用。方法:选取我院校本部学生80人,随机分为实验组和对照组,每组40人。实验组采取多媒体教学法,对照组则采用传统教学法。比较两组学生的期末考试成绩,并在假期对两组学生进行问卷调查,分析调查结果。结果:①期末测试结果:实验组学生的期末考试平均分为88.3,对照组学生的期末考试平均分为68.9,实验组明显优于对照组,差异具有统计学意义(P〈0.05);②问卷调查结果:实验组92.28%的学生认为教学能够提高学习主动性,而对照组为69.36%;实验组88.61%的学生认为教学有助于提升对课程内容的理解,而对照组为66.19%;实验组93.45%的学生认为教学促进师生互动,而对照组为67.64%;实验组72.75%的学生认为教学可增强团队合作精神,而对照组为46.73%。实验组的调查结果明显优于对照组,差异具有统计学意义(P〈0.05)。结论:将多媒体技术引入人体解剖学课程的教学实践中,能够更好的帮助学生掌握人体解剖学的理论体系,提升教学质量,提高学生的专业知识水平。  相似文献

OBJECTIVE: To examine the cytopathological changes in the conjunctiva of patients with active Kawasaki disease. STUDY DESIGN: Case-control prospective study. Bilateral conjunctival swabs were obtained from 3 groups of children: patients with acute KD (11), age-matched controls (7), and patients with inactive KD (9). The ThinPrep prepared and Papanicolaou stained smears were examined blindly by 2 cytopathologists. The cell count differential of cells was performed and recorded quantitatively and comparisons between the 3 patient groups were made. RESULTS: Only neutrophil counts showed a significant difference among the 3 groups. The average scores for the acute KD, control group, and patients with inactive KD were 3.5, 1.6 and 1.3, respectively. Using the Pearson chi2 test, the difference between the acute KD and the inactive group was statistically significant for both eyes (right p = 0.049, left p = 0.004). Samples from acute KD patients were more cellular. Neutrophils surrounding conjunctival epithelial cells, or "neutrophilic rosetting", were seen in 4 (36%) cases of the active disease group but not in the other groups. CONCLUSION: "Neutrophilic conjunctivitis" is characteristic in patients of acute KD that may be of value in the initial evaluation and subsequent follow-up of KD patients.  相似文献

The aim of the study was to determine the effect of football school program and physical education curriculum on changes in the motor abilities of 7- and 8-year-old boys. The study included a sample of 180 boys divided into group 1 (7-year-old boys), subdivided to experimental (n = 40) and control (n = 50) groups, and group 2 (8-year-old boys), subdivided to experimental (n = 40) and control (n = 50) groups. Experimental groups included children attending three training units of football training over a 9-month period, in addition to the conventional physical education curriculum. Control groups included children attending only conventional physical education curriculum. All study subjects underwent testing with a battery of 12 motor tests at the beginning and at the end of the study. Results obtained by discriminative canonic analysis showed no statistically significant between-group difference in motor abilities at the beginning of the study. However, significant differences in favor of experimental groups were recorded at the end of the study. Favorable changes in all motor variables were observed in both experimental and control groups of children from the initial through the final state. These changes were more pronounced in experimental groups. Analysis of variance for difference variables (final to initial measurement) indicated programmed education in the form of football training in addition to regular physical education curriculum to predominantly influence the development of aerobic endurance, agility, speed and flexibility in 7-year-old boys, and of explosive strength, aerobic endurance, flexibility and speed in 8-year-old boys. In the latter, football training led to the formation of a motor complex integrating explosiveness, speed, coordination, endurance and flexibility as a general motor factor determining future quality development in football.  相似文献

The aim of this study was to determine the different levels of depression, hopelessness and post-traumatic stress disorder between two groups of adolescents, those who returned to Baranja and those from Osijek. The first group consisted of 57 adolescents (32 female and 25 male) with the mean age of 17.36, who were grammar school students in Beli Manastir (Gimnazija, Beli Manastir). The mean duration of displacement period was 7 years. The second group consisted of 58 adolescents of grammar school students in Osijek (III. gimnazija, Osijek) (32 female and 26 male) with the mean age of 17.28. All examinees filled in the Croatian version of the Children's Depression Inventory (CDI), the Hopelessness Scale for Children (HSC) and Children's Post-Traumatic Stress Disorder (PTSD) Inventory. The analysis of the results did not show any statistically significant difference between these two groups regarding levels of depression, hopelessness and post-traumatic stress disorder. The results point out rather good psychosocial adjustment of adolescents upon their return home after 4 years.  相似文献

目的:比较尼可地尔与单硝酸异山梨酯对冠心病合并2型糖尿病患者不完全血运重建术后的疗效。方法:入选112例经皮冠状动脉介入治疗(PCI)部分血运重建的冠心病合并2型糖尿病患者。随机分为2组:尼可地尔组(5 mg,3次/d,口服)60例,单硝酸异山梨酯组(50 mg,1次/d,口服)52例,两组患者均给予常规冠心病及糖尿病药物治疗。术后4周末行运动负荷试验,观察总运动时间,从开始运动到出现ST段压低1.0 mv和出现心绞痛的时间(s),以及最大ST段压低幅度,同时记录每周心绞痛发作次数及硝酸甘油用量。结果:4周后两组患者同服药前比较,从开始运动到出现ST段压低1.0 mv的时间延长,总运动时间延长,从开始运动到出现心绞痛的时间延长,最大ST段压低幅度降低(P0.01),但尼可地尔组与单硝酸异山梨酯组比较无明显统计学差异(P0.05);尼可地尔组每周心绞痛发作次数及硝酸甘油用量明显少于单硝酸异山梨酯组(P0.05)。结论:尼可地尔可增加冠心病合并糖尿病患者行不完全血运重建术后患者运动耐量,在减少心绞痛方面作用优于单硝酸异山梨酯。  相似文献

目的:探讨核医学肾动态显像和血清胱抑素C(Cys C)在评价肾积水患者肾功能中的临床应用。方法:选择2014年1月至2015年1月我院收治的肾积水患者70例,依据肾积水程度将患者分为轻度积水组(21例)、中度积水组(26例)、重度积水组(23例)三组,选择同期我院体检中心的健康成人20例作为对照组,所有研究对象分别通过肾动态显像和双血浆法测量肾小球率过滤(GFR),分别记为I-GFR和rGFR,采用酶联免疫吸附法和血生化分析仪分别检测血清Cys C和尿素氮(BUN)、血肌酐(SCr)。结果:各组各指标比较差异均有统计学意义(均P0.05),肾积水患者r-GFR、I-GFR显著低于对照组患者,Cys C、BUN、SCr均显著高于对照组患者,差异有统计学意义(均P0.05);不同程度患者r-GFR、Cys C和SCr差异有统计学意义(均P0.05),其中,重度积水组中度积水组轻度积水组,差异有统计学意义(均P0.05);重度积水组I-GFR和BUN均高于轻度积水组和中度积水组,差异有统计学意义(均P0.05);rGFR和I-GFR与肾积水程度呈负相关(r=-0.967、-0.754,P=0.000、0.000),Cys C、BUN和SCr与肾积水程度呈正相关(r=0.883、0.627、0.671,P=0.000、0.002、0.001)。结论:核医学肾动态显像在评价轻度肾积水患者肾功能时更敏感,而血清Cys C在区分中、重度肾积水患者时更加准确,上述两指标作为替代双血浆法的检测方法在临床上应该互为补充,方能使诊断更加准确。  相似文献
