Serous effusions of nine of 33 patients with systemic lupus erythematosus contained lupus erythematosus (LE) cells, identifiable in Papanicolaou-stained smears, wet films stained with toluidine blue, and cell blocks stained with haematoxylin and eosin. Specimens in which LE cells were found contained at least a moderate number of polymorphonuclear neutrophilic leucocytes. Most specimens containing LE cells also contained cells that resembled LE cells (tart cells), which appeared to be small macrophages that had phagocytosed a non-homogenized nucleus of a cell that had undergone degeneration. In 34 years of cytologic practice we have recognized LE cells in serous effusions only from patients who were already diagnosed as having systemic lupus erythematosus.  相似文献

Systemic lupus erythematosus (SLE) is a complex autoimmune disease and occurs worldwide in both children and adults. The estimated annual incidence among children is 2.22/100,000 and among adults is 23.2/100,000 in the United States. There is increasing understanding about differences in disease manifestations, medication use, and disease severity between those with childhood-onset SLE as compared with adult-onset SLE. Children have a more fulminant disease onset and course than adults with SLE, resulting in two to three times higher mortality. In future years, we anticipate more insight into the genetics between childhood-onset SLE and adult-onset SLE to help delineate the best therapies for both subsets of patients.  相似文献

目的通过对老龄系统性红斑狼疮小鼠(TC)皮肤症状的观察和鉴定,探讨其是否可以作为一种研究皮肤型狼疮的小鼠模型。方法观察TC小鼠出现皮肤症状的年龄,对比同龄小鼠与C57BL/6小鼠的皮肤症状,ELISA法检测小鼠血清抗dsDNA抗体及抗ANA抗体,HE染色检测小鼠皮肤病理学症状。结果 2/3的TC小鼠出现毛发脱失、皮肤溃烂等症状,其病变最早发生于40周龄,皮肤病变的狼疮小鼠血清抗dsDNA抗体和抗ANA抗体滴度明显高于C57BL/6小鼠(P<0.05);HE染色结果显示狼疮小鼠角质层缺失,上皮层连续性中断,真皮层大量淋巴细胞浸润。结论狼疮小鼠在40周后会出现皮肤病变,显示该小鼠可作为一种研究慢性皮肤型狼疮的疾病模型。  相似文献

Sera from patients with neuropsychiatric lupus (NP lupus) were screened for antibodies to mouse choroid plexus cell line ECPC-4 by Western blotting. A 29-kDa protein band detected in NP lupus sera was identified as triosephosphate isomerase (TPI). Using Western blotting with TPI, TPI was confirmed as the reactive molecule in sera (6 of 14 samples) and in cerebrospinal fluids (1 of 2 samples) of patients with NP lupus. Enzyme-linked immunosorbent assay with TPI showed that the serum anti-TPI antibody index of 89.8 (SD, 70.1) in NP lupus group was significantly higher than in systemic lupus erythematosus without NP manifestations, 34.6 (29.6); scleroderma, 38.2 (39.9); polymyositis/dermatomyositis, 42.1 (51.5); and control, 31.7 (27.4) groups (p<0.02). Sensitivity, specificity, and positive and negative likelihood ratios of serum anti-TPI antibody index for NP lupus were 42.9%, 94.7%, 8.1%, and 0.6%, respectively. These results suggest that anti-TPI antibodies are closely associated with NP lupus.  相似文献

目的探讨系统性红斑狼疮(system lupus erythematosus,SLE)患者外周血单个核细胞(peripheral blood monouuclear cells,PBMC)中细胞型Fas相关死亡域样白介素-1β转换酶抑制蛋白(cFLIP)表达的意义。方法应用半定量RT—PCR方法检测38例SLE患者和21名正常人PBMC中cFLIP—L mRNA和cFLIP—S mRNA的表达水平,并与SLE疾病活动指数(SLEDAI)评分进行相关性分析。结果①SLE患者PBMC中cFLIP—L mRNA和cFLIP—S mRNA表达水平均明显高于正常对照组(P〈0.01);SLE患者活动组cFLIP—L mRNA表达水平显著高于非活动组(P〈0.05),cFLIP—S mRNA表达水平在SLE患者活动组与非活动组之间没有显著性差异(P〉0.05)。②SLE患者cFLIP—L mRNA表达水平与SLEDAI评分呈正相关(r=0.423,P〈0.01);而eFLIP—S mRNA表达水平与SLEDAI评分无明显相关性(r=0.270,P〉0.05)。结论cFLIP—L mRNA和cFLIP—S mRNA可能在SLE发病机制中起重要作用。  相似文献

The association between systemic lupus erythematosus (SLE) and malignancy has been controversial in the literature. We report a case of lung cancer in a 50-year-old woman with a 4-year history of SLE. She underwent surgery at a pathological stage of T2N2M0, but she eventually died of rapid recurrence of the cancer in the abdomen resulting in massive haemorrhage from the inferior vena cava (IVC). Immunological disorders related to SLE are thought to contribute to rapid progression of the malignancy.  相似文献

IntroductionNeuropsychiatric systemic lupus erythematosus (NPSLE), a serious organ disorder with a variety of symptoms, has diverse therapeutic outcomes because of the variability of NPSLE manifestations. A comprehensive association study of NPSLE among clinical and immunopathogenic aspects and outcomes has not been conducted.MethodsWe analyzed the laboratory data, NPSLE symptoms, and clinical outcomes at 1 yr post-treatment and the profiles of 27 cytokines, chemokines and growth factors in cerebrospinal fluid (CSF) samples using the Bio-Plex Human 27-plex panel from 28 NPSLE patients. Univariate and multivariable competing risks regression analyses were used to determine the predictive factors of clinical response. We also tried to predict the outcome of NPSLE by the 27 cytokines/chemokines/growth factors using a weighted-voting (WV) algorithm.ResultsOf the two males and 26 females (92.9%), 16 were non-responders at 1 yr post-treatment; in the final model, the independent predictors of non-responders were longer disease durations of SLE (odds ratio [OR]: 1.490, 95% confidence interval [CI]: 1.143–2.461, p = 0.0003) and patients with more than one NPSLE symptom types (OR: 15.14, 95% CI: 1.227–452.1, p = 0.0334). The pretreatment CSF interleukin (IL)-6, IL-10, interferon-gamma (IFN-γ) and tumor necrosis factor-alpha (TNF-α) levels were significantly higher in the non-responders (p = 0.0207, p = 0.0054, p = 0.0242 and p = 0.0077, respectively). We identified six “minimum predictive markers:” IL-10, TNF-α, IL-6, IFN-γ, IL-4 and IL-13 by a WV algorithm that showed the highest accuracy (70.83%) and highest Matthews correlation coefficient (54.23%).ConclusionsWe have devised a numerical prediction scoring system that was able to separate the non-responders from responders. The patients with longer disease durations of SLE and those with more than one NPSLE symptom types had poorer outcomes. Our findings may indicate both the importance of making a diagnosis at an earlier phase for better therapeutic response and the usefulness of measuring multiple cytokines to predict NPSLE therapeutic outcomes.  相似文献

Chromatin, a huge polymer of nucleosomes, has been implicated as an important target of autoantibodies in idiopathic and drug-induced lupus for decades, but the antigenicity of chromatin has only recently been dissected. IgG reactivity with the (H2A-H2B)-DNA complex, a subunit of the nucleosome, is present in the majority of patients with systemic lupus erythematosus, in >90% of patients with lupus induced by procainamide and in individual patients with lupus induced by a variety of other drugs, but is not seen in people taking these medications who are clinically asymptomatic. Anti-[(H2A-H2B)-DNA] accounted for the bulk of the anti-chromatin activity in drug-induced lupus. The earliest detectable autoantibody in lupus-prone mice recognized similar epitopes in the (H2A-H2B)-DNA subnucleosome complex; as the immune response progressed, native DNA and other constituents of chromatin became antigenic. The importance of chromatin-reactive T cells in the anti-[(H2A-H2B)-DNA] response is suggested by the presence of somatic mutations in antibody VH and VL regions, their perdominant IgG isotype and the similarity in kinetics of their production to that of conventional T cell dependent antigens. Together with the serologic data from human lupus-like disease, these results are consistent with chromatin being a common stimulant for both B and T cells. The Toll-like receptor (TLR) family plays a fundamental role in host innate immunity by mounting a rapid and potent inflammatory response to pathogen infection. TLRs recognize distinct microbial components and activate intracellular signaling pathways that induce expression of host inflammatory genes. Several studies have indicated that TLRs are implicated in many inflammatory and immune disorders. Extensive research in the past decade to understand TLR-mediated mechanisms of innate immunity has enabled pharmaceutical companies to begin to develop novel therapeutics for the purpose of controlling an inflammatory disease. The roles of TLRs in the development of autoimmune diseases have been studied. TLR7 and TLR9 have key roles in production of autoantibodies and/or in development of systemic autoimmune disease. It remains to be determined their role in apoptosis, in the pathogenesis of RNA containing immune complexes, differential expression of TLRs by T regulatory cells.  相似文献

Objective. Proliferating cell nuclear antigen (PCNA), one of the target antigen recognized by lupus sera, has been reported to be present as a subnuclear multi-peptide complex. But autoantibodies reacting with components of PCNA complex are poorly understood. To study the specificity of those autoantibodies, immunoreactivities of autoimmune sera against purified PCNA antigen were studied. Methods. PCNA antigens were purified from rabbit thymus extract by affinity column using murine monoclonal antibodies (mAbs) to PCNA, TOB7, TO17 and TO30. Immunoreactivities of autoimmune sera against purified PCNA were analyzed by WB. Results. PCNA antigen purified by serum AK predominantly showed a 34 kD band specific for PCNA in SDS-PAGE. When antigens were purified by anti-PCNA mAb TOB7 and TO30 which are known to be targeting different epitopes on PCNA antigen, SDS-PAGE analysis showed various mol. wt of proteins in addition to the 34 kD PCNA while both AK and mAbs reacted only with 34 kD PCNA in WB. In WB using PCNA purified by TOB7, various immunoreactivities were observed at 150, 66, 58, 48, 45, 37, 32 and 16 kDa in sera from patients with connective tissue diseases. Conclusions. 目的:探讨系统性红斑狼疮(SLE)罕见的临床并发症——假性肠梗阻(IPO)的临床特点。方法:回顾性分析2例SLE合并IPO患者的发病情况、临床表现、实验室检查、病程、治疗及预后等临床特点。结果:两例均以肠梗阻为首发表症,均无SLE的特异性表现,且均有肾脏、血液系统的损害,抗抗核抗体、抗dsDNA均为阳性,病程较短,其中一例死于肾功能衰竭。结论:IPO是SLE的一个罕见但严重的并发症,以肠梗阻为首发表症的SLE易被误诊,合并IPO的SLE患者病情较重且常伴有其他脏器受累,病死率较高,免疫学检查有助于早期诊断,及早诊断及大剂量激素联合丙种球蛋白治疗对于缓解病情、改善预后意义重大。  相似文献

目的

建立基于胶体金标记米曲霉素(AOL)的快速筛查系统性红斑狼疮(SLE)的免疫层析试纸法。

方法

利用大肠杆菌BL21原核表达特异性识别核心岩藻糖基的AOL基因(FleA), 并进行纯化及胶体金标记。利用特异性识别IgG的Protein G以及胶体金标记的AOL共同建立快速筛查SLE的免疫层析试纸法(ICS)。

结果

成功表达并纯化AOL重组蛋白, 并进行胶体金标记; 应用胶体金标记AOL的ICS检测SLE患者血清呈阳性反应, 而健康对照及其他自身免疫性疾病(类风湿关节炎、IgA肾病和结缔组织病等)呈阴性反应。

结论

成功建立基于胶体金标记AOL的快速筛查SLE的ICS, 为SLE早期筛查提供了可靠依据。

探讨Th1、Th2和Th17型细胞在类风湿性关节炎(RA)和系统性红斑狼疮(SLE)发病机制中的作用。收集37例RA患者、25例SLE患者和34例健康人的抗凝血,应用ELISA检测血清中IFN-γ、IL-10和IL-17的水平。与健康对照组比较,RA和SLE患者血清中IFN-γ的水平均具有统计学意义(P<0.05);SLE患者IL-10水平出现有意义的升高(P<0.05);而RA患者IL-17的升高具有统计学意义(P<0.05)。由此提示Th1、Th2和Th17细胞在自身免疫性疾病中均发挥不同的重要作用。  相似文献

系统性红斑狼疮是一种涉及多系统损害的自身免疫性疾病,其主要并发症及死亡原因之一是狼疮性肾炎。研究表明,IL-17和产IL-17细胞可以浸润肾脏,与其他细胞因子协同作用,引起肾脏局部炎症反应。拮抗IL-17的生物制剂已应用于银屑病、强制性脊柱炎等免疫介导炎症性疾病,但在系统性红斑狼疮及狼疮性肾炎的研究尚少。本文对IL-17与狼疮性肾炎的关系进行综述,探讨IL-17及其抑制剂在狼疮性肾炎治疗的作用及发展前景。  相似文献

目的:提高对系统性红疯狼疮(SLE)伴发急腹症临床表现的认识,总结诊断和治疗此类病例的经验。方法:对18例SLE伴发急腹症的病例进行回顾性分析。结果:SLE并发急腹症临床表现多样化,可以表现为消化道出血,肠梗阻。肠穿孔,急性胃肠炎,急性胰腺炎,急性腹膜炎等。治疗后16例病情得到控制,2例死亡。结论:SLE伴发急腹症预示病情危重,对此应提高认识,尽早诊断;应用大剂量肾上腺皮质激素和免疫抑制剂有良好的疗效。  相似文献

利用近年来发展起来的代表性差异分析cDNA-RDA(cDNA-representational difference analysis)技术筛选BXSB红斑狼疮小鼠发病相关基因.发现了3个新的表达序列标签(EST)片段,在GenBank中的登录号分别为AF060113, AF060111, AF060110,同时发现了一些已知与自身免疫病相关的基因如逆转录病毒衣壳蛋白、Line-1逆转录酶等.通过RDA技术可能发现系统性红斑狼疮发病相关新基因,为自身免疫病的理论研究提供新的思路和方法.  相似文献
