目的:探讨慢性阻塞性肺疾病急性加重期(AECOPD)患者血清淀粉样蛋白A(SAA)水平与肺功能及炎性因子的相关性,并分析其诊断价值。方法:选取2013年6月-2018年6月中国人民解放军第970医院收治的204例慢性阻塞性肺疾病(COPD)患者作为研究对象,其中COPD稳定期患者132例作为COPD稳定组,AECOPD患者72例作为AECOPD组。另选取同期于中国人民解放军第970医院进行健康体检的50例健康体检者作为健康组。比较三组受试者血清SAA水平、白细胞介素-8(IL-8)、白细胞介素-6(IL-6)、C反应蛋白(CRP)、降钙素原(PCT)水平及肺功能,采用Pearson相关性分析AECOPD患者血清SAA水平与肺功能及炎性因子的相关性,并分析SAA对AECOPD的诊断价值。结果:AECOPD组患者血清SAA、PCT、CRP、IL-6、IL-8水平较COPD稳定组及健康组升高(P0.05),COPD稳定组患者血清SAA、IL-6、IL-8、PCT、CRP水平均高于健康组,差异有统计学意义(P0.05);AECOPD组患者第一秒用力呼气容积(FEV1)、用力肺活量(FVC)、第一秒用力呼吸容积占用力肺活量的百分比(FEV1/FVC%)、第一秒用力呼气容积占预计值百分比(FEV1%)低于COPD稳定组及健康组,差异有统计学意义(P0.05),COPD稳定组患者FEV1、FVC、FEV1/FVC%、FEV1%低于健康组,差异有统计学意义(P0.05)。Pearson相关性分析显示,AECOPD患者血清SAA水平与IL-8、IL-6、CRP、PCT呈正相关,与FEV1%、FEV1/FVC%、FEV1、FVC呈负相关(P0.05)。受试者工作特征(ROC)曲线结果显示,SAA对AECOPD诊断的敏感度为80.85%,特异度为80.07%,曲线下面积为0.832。结论:AECOPD患者血清SAA水平明显升高,其与患者肺功能及炎症因子存在相关性,具有较高的诊断价值,可用于AECOPD患者病情的评估。  相似文献

摘要 目的:探讨慢性阻塞性肺疾病(COPD)患者中血清亲环素A(CyPA)、趋化因子CX3CL1以及其他炎性指标的表达水平及其临床意义。方法:选取2019年1月-2021年6月本院收治的COPD患者120例作为研究对象,其中68例患者为急性加重期组,52例患者为稳定期组;另选取体检健康者45例作为对照组。收集所有受试者的临床资料,检测其血清中CyPA、CX3CL1、C反应蛋白(CRP)、白细胞介素-6(IL-6)以及基质金属蛋白酶-9(MMP-9)水平,比较3组患者各参数的差异,并与肺功能进行相关性分析,比较急性加重期患者治疗前后各指标的差异。结果:急性加重期组患者的血清 CyPA、CX3CL1、CRP、IL-6、MMP-9水平均明显高于稳定期组和对照组患者,稳定期组患者的血清 CyPA、CX3CL1、CRP、IL-6、MMP-9水平均明显高于对照组患者(均P<0.05);而急性加重期组患者的第1s用力呼气量(FEV1)、用力肺活量(FVC)、第1s用力呼气量(FEV1)与用力肺活量(FVC)的比值(FEV1%)、最大呼气峰流速(PEF)以及最大呼气中期流速(MMEF)明显低于稳定期组和对照组患者,稳定期组患者的FEV1、FVC、FEV1/FVC、PEF、MMEF明显低于对照组患者(均P<0.05)。COPD患者的血清CyPA、CX3CL1、CRP、IL-6、MMP-9水平与FEV1、FVC、FEV1/FVC、PEF、MMEF呈负相关(P<0.05)。与治疗前相比较,急性加重期组患者在治疗后血清CyPA、CX3CL1、CRP、IL-6、MMP-9水平明显下降,而FEV1、FVC、FEV1/FVC、PEF、MMEF明显上升(均P<0.05)。结论:COPD患者的血清CyPA、CX3CL1、CRP、IL-6、MMP-9水平可在一定程度上预测患者的严重程度,同时也可以作为急性加重期治疗后效果的评价指标。  相似文献

摘要 目的:探讨血清钙结合蛋白S100A12、脂联素及白细胞介素-17(Interleukin-17,IL-17)水平与稳定期慢性阻塞性肺疾病(COPD)病情严重程度和肺功能的关系。方法:选择稳定期COPD患者(稳定期COPD组)和健康体检者(对照组)各80例,稳定期COPD患者分为四级:I级轻度(22例),Ⅱ级中度(24例),Ⅲ级重度(19例),Ⅳ级极重度(15例)。使用肺功能仪对所有研究对象的肺功能进行检查,采用血气分析仪检测动脉血中的氧分压 (PaO2) 和二氧化碳分压 (PaCO2),采用酶联免疫吸附试验检测血清中钙结合蛋白S100A12、脂联素和IL-17水平,采用 Pearson相关分析各指标之间的相关性。结果:稳定期COPD组肺功能[第一秒用力呼出气量容积占预计值百分比(FEV1%)和第一秒用力呼出气量容积/用力肺活量(FEV1/FVC%)]和PaO2显著低于对照组(P<0.05),并随着COPD严重程度增加而降低(P<0.05);稳定期COPD组血清钙结合蛋白S100A12、脂联素、IL-17水平和PaCO2均高于对照组(P<0.05),并随COPD严重程度增加而升高(P <0.05)。Pearson相关分析显示:稳定期COPD患者血清钙结合蛋白S100A12、脂联素和IL-17水平与FEV1%、FEV1/FVC%和PaO2呈负相关(P<0.05),与PaCO2呈正相关(P<0.05)。结论:钙结合蛋白S100A12、脂联素和IL-17可能共同参与了稳定期COPD慢性炎症过程,引起气流受限,影响肺通气功能,可以辅助评估稳定期COPD病情的严重程度。  相似文献

Abstract

Oxidative stress and inflammation are hallmarks of chronic obstructive pulmonary disease (COPD). A critical byproduct of oxidative damage is the introduction of carbonyl groups into amino acid residues. We hypothesize that plasma carbonyl content is inversely correlated with lung function and computed tomography (CT) measures of lung density among smokers and is elevated in COPD. Carbonyl was measured in plasma of participants aged 60 years and older by ELISA. Generalized linear and additive models were used to adjust for potential confounders. Among 541 participants (52% male, mean age 67 years, 41% current smokers), mean plasma carbonyl content was 17.9±2.9 nmol ml?1 and mean forced expiratory volume in one second (FEV1) was 80.7±20.9% of predicted. Plasma carbonyl content was inversely associated with FEV1, but this relationship was largely explained by age. Multivariate analyses ruled out clinically meaningful associations of plasma carbonyl content with FEV1, FEV1/FVC (forced vital capacity) ratio, severity of airflow obstruction, and CT lung density. Plasma carbonyl content is a poor biomarker of oxidative stress in COPD and emphysema.  相似文献

Cigarette smoking is the main risk factor for developing the inflammatory lung disease chronic obstructive pulmonary disease (COPD). Differences in susceptibility among smokers have been attributed to a genetic predisposition. A recent publication on the Framingham Heart Study found a strong association of the Asn142Asp SNP in Glutatthione-S-transferase Omega (GSTO) 2 with forced expiratory volume in the first second (FEV1) and forced vital capacity (FVC). FEV1 is the main parameter reflecting the degree of airflow limitation in patients with COPD. Therefore the present study was undertaken to investigate whether the Asn142Asp polymorphism in GSTO2 occurs more frequently in patients with COPD than healthy subjects and to replicate the finding that it strongly correlates with FEV1. Furthermore, the Ala140Asp substitution in GSTO1 was examined. Genotyping was carried out in 195 healthy controls and 355 patients with COPD. The results demonstrate that the Asn142Asp polymorphism in GSTO2 and the GSTO1140Asp/GSTO2142Asp haplotype were associated with increased risk of COPD. However, single-marker and haplotype-based analyses failed to reveal an association between lung function parameters and investigated non-synonymous coding SNPs in the GSTO genes. In conclusion, GSTO2 is a candidate gene for COPD, but is not associated with FEV1.  相似文献

摘要 目的:探讨慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)不同表型评估测试问卷(COPD assessment test,CAT)评分与肺功能及预后的关系。方法:收集361例COPD患者临床资料、CAT评分、肺功能检查结果及肺外合并症、肺内并发症等情况,按临床表型分为肺气肿组(n=200)和支气管炎组(n=161),分析肺气肿组200例和支气管炎组161例COPD患者CAT评分与肺功能及预后的关系。结果:肺气肿组CAT评分高于支气管炎组(P<0.05),一秒用力呼气容积(FEV1)占预计值百分比(FEV1%)、FEV1/用力肺活量(FVC)低于支气管炎组(P<0.05),吸气分数(IC/TLC)低于支气管炎组,残总比(RV/TLC)高于支气管炎组(P<0.05);肺气肿组肺间质性病变、肺动脉高压发生率均高于支气管炎组(P<0.05);支气管炎、肺气肿组CAT评分均与FEV1%、FEV1/FVC、IC/TLC呈负相关(P<0.05),与RV/TLC呈正相关(P<0.05),肺气肿各参数相关度更高(P<0.05);肺气肿组不同CAT评分患者肺间质性病变、肺动脉高压发生率比较差异有统计学意义(P<0.05),支气管炎组不同CAT评分肺动脉高压发生率比较差异有统计学意义(P<0.05),随CAT评分的升高,肺气肿组肺间质性病变、肺动脉高压发生率上升,支气管炎组肺动脉高压发生率上升。结论: COPD肺气肿表型CAT评分较支气管炎表型高,肺功能降低更明显,呈现肺过度通气,气流受限特点,更易并发肺间质纤维化、肺动脉高压,且与CAT评分变化密切相关。  相似文献

摘要 目的:探讨布地格福与布地奈德福莫特罗粉对慢性阻塞性肺疾病(COPD)缓解期患者肺功能、运动耐力的影响。方法:选取2020年1月至2022年12月本院收治的300例COPD缓解期患者,将其随机分为A组(n=150)和B组(n=150),A组予以布地格福吸入治疗,B组予以布地奈德莫特罗粉吸入治疗。连续治疗3个月后,比较两组疗效及用药安全性;比较两组肺功能、血气水平、运动耐力和生活质量、炎症指标变化。结果:连续治疗3个月后,A组治疗总有效率为96.00 %,高于对照组的90.00 %(P<0.05);治疗后两组第1 s用力呼气容积(FEV1)、FEV1/用力肺活量、FEV1占预计值百分数、血氧分压、6 min步行距离较治疗前均有提升,且A组高于B组(P<0.05);治疗后两组残气容积/肺总量、二氧化碳分压、白细胞介素-17、基质金属蛋白酶-9、圣乔治呼吸问卷评分较治疗前均有降低,且A组低于B组(P<0.05);治疗期间两组均未发生严重的不良反应(P>0.05)。结论:相较于布地奈德福莫特罗,布地格福对COPD缓解期的治疗效果更佳,能有效减低气道炎症损伤,改善其肺功能及血气水平,提升患者运动耐力及生活质量。  相似文献

摘要 目的:观察肺功能康复训练联合八段锦对慢性阻塞性肺病(COPD)稳定期患者运动耐力、肺功能及生活质量的影响。方法:选取80例COPD稳定期患者,纳入病例时间:2018年1月~2020年7月,采用随机数字表法分为观察组(肺功能康复训练联合八段锦,40例)、对照组(肺功能康复训练,40例),干预6个月。对比两组干预前、干预6个月后的肺功能[用力肺活量(FVC)、1秒用力呼气容积(FEV1)]、运动耐力[6 min步行距离(6MWD)]及生活质量[世界卫生组织生存质量量表简表(WHOQOL-BREF)评分]、症状评分[改良英国医学研究学会呼吸困难量表(mMRC)评分、慢阻肺综合评估测试(CAT)评分]。结果:两组干预6个月后FVC、6MWD、FEV1均较干预前升高,且观察组高于对照组(P<0.05)。两组干预6个月后WHOQOL-BREF各维度评分较干预前升高,且观察组高于对照组(P<0.05)。干预6个月后, 两组mMRC评分、CAT评分较干预前下降,且观察组低于对照组(P<0.05)。结论:肺功能康复训练联合八段锦干预COPD稳定期患者,可缓解其临床症状,提高肺部适应性,进而改善患者运动耐力及生活质量。  相似文献

QUESTION

A 6-second spirometry test is easier than full exhalations. We compared the reliability of the ratio of the Forced expiratory volume in 1 second/Forced expiratory volume in 6 seconds (FEV1/FEV6) to the ratio of the FEV1/Forced vital capacity (FEV1/FVC) for the detection of airway obstruction.

METHODS

The PLATINO population-based survey in individuals aged 40 years and over designed to estimate the prevalence of post-Bronchodilator airway obstruction repeated for the same study participants after 5–9 years in three Latin-American cities.

RESULTS

Using the FEV1/FVC<Lower limit of normal (LLN) index, COPD prevalence apparently changed from 9.8 to 13.2% in Montevideo, from 9.7 to 6.0% in São Paulo and from 8.5 to 6.6% in Santiago, despite only slight declines in smoking prevalence (from 30.8% to 24.3%). These changes were associated with differences in Forced expiratory time (FET) between the two surveys. In contrast, by using the FEV1/FEV6 to define airway obstruction, the changes in prevalence were smaller: 9.7 to 10.6% in Montevideo, 8.6 to 9.0% in São Paulo, and 7.5 to 7.9% in Santiago. Changes in the prevalence of COPD with criteria based on FEV1/FVC correlated strongly with changes in the FET of the tests (R2 0.92) unlike the prevalence based on a low FEV1/FEV6 (R2 = 0.40).

CONCLUSION

The FEV1/FEV6 is a more reliable index than FEV1/FVC because FVC varies with the duration of the forced exhalation. Reporting FET and FEV1/FEV6<LLN helps to understand differences in prevalence of COPD obtained from FEV1/FVC-derived indices.  相似文献

Background

The assessment of bronchodilator-induced change in forced vital capacity (FVC) is dependent on forced expiratory time (FET) in subjects with airflow limitation. Limited information is available on the concurrent responses of FVC, forced expiratory volume in six seconds (FEV6), and FET in the bronchodilation test among patients with obstructive airways disease or in the general population. The aim of this study was to assess the changes in FEV6, FVC, and FET, and their relationships in a standardized bronchodilation test in the general population.

Methods

We studied bronchodilation response in a general adult population sample of 628 individuals (260 men, 368 women) with flow-volume spirometry. The largest FVC, the corresponding FET and the largest FEV6 both at the baseline and after 0.4 mg of inhaled salbutamol were selected for analysis.

Results

After administration of salbutamol FEV6 decreased on average -13.4 (95% CI -22.3 to -4.5) ml or -0.2% (-0.4% to 0.0%) from the baseline. The 95th percentile of change in FEV6 was 169.1 ml and 5.0%. FVC decreased on average -42.8 (-52.4 to -33.3) ml or -1.0% (-1.2% to -0.7%). Concurrently FET changed on average -0.2 (-0.4 to 0.0) seconds or 0.4% (-1.4% to 2.3%). There were four subjects with an increase of FVC over 12% and only one of these was associated with prolonged FET after salbutamol. Changes in FEV6 and FVC were more frequently positive in subjects with reduced FEV1/FVC in baseline spirometry.

Conclusion

In general adult population, both FEV6 and FVC tended to decrease, but FET remained almost unchanged, in the bronchodilation test. However, those subjects with signs of airflow limitation at the baseline showed frequently some increase of FEV6 and FVC in the bronchodilation test without change in FET. We suggest that FEV6 could be used in assessment of bronchodilation response in lieu of FVC removing the need for regulation of FET during bronchodilation testing.  相似文献

目的:本文旨在推广高才达主任医师治疗慢性阻塞性肺病稳定期的临床经验,以及观察五子参药汤治疗慢性阻塞性肺疾病稳定期属肺脾两虚、痰浊内蕴症的临床疗效。方法:随机选取我院慢性阻塞性肺疾病(稳定期)患者90例分为对照组(n=45)和治疗组(n=45),对照组吸入沙美特罗替卡松粉气雾剂进行治疗,治疗组在对照组基础上加服五子参药汤,两组疗程均为12 w。对比两组治疗前后临床症状、肺功能、慢阻肺患者自我评估测试(CAT量表)、改良版英国医学研究委员会呼吸问卷(m MRC)量表及治疗后6个月内因COPD急性加重的再次住院次数。结果:治疗组的有效率优于对照组(91.11%vs 82.45%,P<0.05);治疗后,两组第1秒用力呼气容积(FEV1)、FEV1占预计值百分比(FEV1%)和FEV1/FVC较治疗前均有增加,且治疗组高于对照组(P<0.05);两组m MRC量表、CAT量表评分较治疗前均有下降,且治疗组低于对照组(P<0.05)。结论:五子参药汤可有效治疗肺脾两虚、痰浊内蕴型稳定期慢性阻塞性肺疾病,值得临床推广。  相似文献

Free iron in lung can cause the generation of reactive oxygen species, an important factor in chronic obstructive pulmonary disease (COPD) pathogenesis. Iron accumulation has been implicated in oxidative stress in other diseases, such as Alzheimer’s and Parkinson’s diseases, but little is known about iron accumulation in COPD. We sought to determine if iron content and the expression of iron transport and/or storage genes in lung differ between controls and COPD subjects, and whether changes in these correlate with airway obstruction. Explanted lung tissue was obtained from transplant donors, GOLD 2–3 COPD subjects, and GOLD 4 lung transplant recipients, and bronchoalveolar lavage (BAL) cells were obtained from non-smokers, healthy smokers, and GOLD 1–3 COPD subjects. Iron-positive cells were quantified histologically, and the expression of iron uptake (transferrin and transferrin receptor), storage (ferritin) and export (ferroportin) genes was examined by real-time RT-PCR assay. Percentage of iron-positive cells and expression levels of iron metabolism genes were examined for correlations with airflow limitation indices (forced expiratory volume in the first second (FEV1) and the ratio between FEV1 and forced vital capacity (FEV1/FVC)). The alveolar macrophage was identified as the predominant iron-positive cell type in lung tissues. Futhermore, the quantity of iron deposit and the percentage of iron positive macrophages were increased with COPD and emphysema severity. The mRNA expression of iron uptake and storage genes transferrin and ferritin were significantly increased in GOLD 4 COPD lungs compared to donors (6.9 and 3.22 fold increase, respectively). In BAL cells, the mRNA expression of transferrin, transferrin receptor and ferritin correlated with airway obstruction. These results support activation of an iron sequestration mechanism by alveolar macrophages in COPD, which we postulate is a protective mechanism against iron induced oxidative stress.  相似文献

The purpose of this study was to assess the effects of a particular breathing pattern training (BPT) on forced expiratory volume during the first second (FEV1) and forced vital capacity (FVC) in patients with chronic obstructive pulmonary disease (COPD). The subjects adjusted each breath to a target breath displayed on a video screen, by using visual feedback. This target was chosen in an individual sample recorded at rest. We used a randomized, controlled group design. Twenty patients with stable COPD, FEV1 less than 1.5 liters, undergoing a traditional rehabilitation program were randomly assigned to the BPT group or to the control group. Each BPT subject underwent 30–35 training sessions spread out over four weeks, in addition to the traditional program. FEV1 and FVC were performed before and after this program. ANOVAs showed that FEV1 and FVC significantly improved in BPT subjects, with a mean percent increase of 22% and 19%, respectively. Corresponding changes in controls were not significant. This study showed short-term increases in FEV1 and FVC in COPD patients practicing BPT in addition to respiratory rehabilitation, in comparison with controls. 目的:探讨沙丁胺醇、布地奈德不同雾化吸入联合头孢哌酮钠舒巴坦钠对慢性阻塞性肺疾病急性加重期(AECOPD)患者血气指标和肺功能的影响。方法:选取本院2015年1月至2018年10月期间收治的210例AECOPD住院患者作为研究对象,随机分为对照组(头孢哌酮钠舒巴坦钠治疗)、持续雾化组(沙丁胺醇、布地奈德持续雾化治疗+头孢哌酮钠舒巴坦钠)、先后雾化组(先给予沙丁胺醇雾化、后给予布地奈德雾化治疗+头孢哌酮钠舒巴坦钠),每组各70例,对比三组的临床疗效、血气指标、肺功能及不良反应。结果:先后雾化组总有效率为91.43%,高于持续雾化组的78.57%及对照组的67.14%(P0.05)。先后雾化组患者治疗5天后氧分压(PaO_2)、氧饱和度(SaO_2)高于持续雾化组及对照组,且持续雾化组高于对照组(P0.05);先后雾化组患者治疗5天后二氧化碳分压(PaCO_2)低于持续雾化组及对照组,且持续雾化组低于对照组(P0.05)。先后雾化组患者第一秒用力呼气容积(FEV_1)、用力肺活量(FVC)、第一秒用力呼吸容积占用力肺活量的百分比(FEV_1/FVC%)、第一秒用力呼气容积占预计值百分比(FEV_1%)高于持续雾化组及对照组,且持续雾化组高于对照组(P0.05)。三组不良反应总发生率比较无统计学差异(P0.05)。结论:沙丁胺醇、布地奈德先后雾化吸入联合头孢哌酮钠舒巴坦钠治疗AECOPD患者疗效确切,可提高患者肺功能,改善血气相关指标。  相似文献

Background

Chronic obstructive pulmonary disease (COPD) is characterized by expiratory flow limitation, causing air trapping and lung hyperinflation. Hyperinflation leads to reduced exercise tolerance and poor quality of life in COPD patients. Total lung capacity (TLC) is an indicator of hyperinflation particularly in subjects with moderate-to-severe airflow obstruction. The aim of our study was to identify genetic variants associated with TLC in COPD.

Methods

We performed genome-wide association studies (GWASs) in white subjects from three cohorts: the COPDGene Study; the Evaluation of COPD Longitudinally to Identify Predictive Surrogate Endpoints (ECLIPSE); and GenKOLS (Bergen, Norway). All subjects were current or ex-smokers with at least moderate airflow obstruction, defined by a ratio of forced expiratory volume in 1 second to forced vital capacity (FEV1/FVC) <0.7 and FEV1 < 80% predicted on post-bronchodilator spirometry. TLC was calculated by using volumetric computed tomography scans at full inspiration (TLCCT). Genotyping in each cohort was completed, with statistical imputation of additional markers. To find genetic variants associated with TLCCT, linear regression models were used, with adjustment for age, sex, pack-years of smoking, height, and principal components for genetic ancestry. Results were summarized using fixed-effect meta-analysis.

Results

Analysis of a total of 4,543 COPD subjects identified one genome-wide significant locus on chromosome 5p15.2 (rs114929486, β = 0.42L, P = 4.66 × 10−8).

Conclusions

In COPD, TLCCT was associated with a SNP in dynein, axonemal, heavy chain 5 (DNAH5), a gene in which genetic variants can cause primary ciliary dyskinesia. DNAH5 could have an effect on hyperinflation in COPD.

摘要 目的:探讨益肺胶囊联合乌美溴铵维兰特罗吸入粉雾剂对慢性阻塞性肺疾病(COPD)疾病稳定期患者肺功能、细胞免疫功能和血清碱性成纤维细胞生长因子(bFGF)、沉默信息调节因子1(SIRT1)的影响。方法:纳入我院2021年3月~2022年6月期间收治的COPD稳定期患者104例,按照随机数字表法分为对照组(乌美溴铵维兰特罗吸入粉雾剂治疗,n=52)和观察组(益肺胶囊联合乌美溴铵维兰特罗吸入粉雾剂治疗,n=52)。对比两组临床症状恢复时间、6 min步行试验(6MWT)和COPD评估测试(CAT)评分、肺功能[第1秒用力呼气容积(FEV1)、用力肺活量(FVC)、每分钟最大通气量(MVV)]、细胞免疫功能和血清bFGF、SIRT1水平,观察两组不良反应发生情况。结果:与对照组相比,观察组的肺啰音、咳嗽、咯痰消失时间缩短(P<0.05)。与对照组相比,观察组治疗14 d后6MWT更远,CAT评分更低(P<0.05)。与对照组相比,观察组治疗14 d后FVC、FEV1、MVV更高(P<0.05)。与对照组相比,观察组治疗14 d后CD3+、CD4+、CD4+/CD8+更高,CD8+更低(P<0.05)。与对照组相比,观察组治疗14 d后bFGF更低,SIRT1更高(P<0.05)。两组不良反应总发生率组间对比无差异(P>0.05)。结论:益肺胶囊联合乌美溴铵维兰特罗吸入粉雾剂有助于调节COPD稳定期患者血清bFGF、SIRT1水平,改善肺功能和细胞免疫功能。  相似文献

Objective

To determine whether the presence of chronic obstructive lung disease (COPD) and reduction of lung function parameters were predictors of mortality in a cohort.

Materials/Patients and Methods

Population based cohorts were followed in Montevideo, Santiago and Sao Paulo during 5, 6 and 9 years, respectively. Outcomes included all-cause, cardiovascular, respiratory and cancer mortality; exposures were COPD, forced expiratory volume in one second (FEV1) and forced vital capacity (FVC). Cox regression was used for analyses. Sensitivity, specificity, positive and negative predictive values, receiver operator characteristics curves and Youden''s index were calculated.

Results

Main causes of death were cardiovascular, respiratory and cancer. Baseline COPD was associated with overall mortality (HR = 1.43 for FEV1/FVC<LLN; 2.01 for GOLD 2-4; 1.46 for GOLD 1-4; 1.50 for FEV1/FEV6 <LLN). For cardiovascular mortality, significant associations were found with GOLD 2-4 (HR = 2.68) and with GOLD 1-4 (HR = 1.78) for both genders together (not among women). Low FEV1 was risk for overall and respiratory mortality (both genders combined). FVC was not associated with overall mortality. For most COPD criteria sensitivity was low and specificity high. The area under the curve for FEV1 was greater than for FVC for overall and cardiovascular mortality.

Background

Osteoporosis is common in patients with COPD but the likely multi-factorial causes contributing to this condition (e.g. sex, age, smoking, therapy) mask the potential contribution from elements related to COPD. In order to study osteoporosis and bone mineral density (BMD) related to COPD, we studied a well-defined group of patients and controls.

Methods

BMD, forced expiratory volume in one second (FEV1), circulating bone biomarkers and biochemistry were determined in 30 clinically stable male ex-smokers with confirmed COPD and 15 age matched "ex-smoker" male controls. None of the patients were on inhaled corticosteroids or received more than one short course of steroids.

Results

Mean (SD) FEV1% predicted of patients was 64(6)%, the majority having Global Initiative for Chronic Obstructive Lung Disease (GOLD) II airflow obstruction. There were 5/30 patients and 1/15 controls who were osteoporotic, while a further 17 patients and 5 controls were osteopenic. The BMD at the hip was lower in patients than controls, but not at the lumbar spine. Mean values of procollagen type 1 amino-terminal propeptide and osteocalcin, both markers of bone formation, and Type 1 collagen β C-telopeptide, a marker of bone resorption, were similar between patients and controls. However, all bone biomarkers were inversely related to hip BMD in patients (r = -0.51, r = -0.67, r = -0.57, p < 0.05) but did not relate to lumbar spine BMD. 25-OH Vitamin D was lower in patients.

Conclusions

Men with COPD had a greater prevalence of osteoporosis and osteopenia than age matched male controls, with a marked difference in BMD at the hip. Bone biomarkers suggest increased bone turnover.  相似文献

An imbalance between oxidative stress and antioxidative capacity may play an important role in the development and progression of bronchial asthma (BA) and chronic obstructive pulmonary disease (COPD). We carried out a study to assess the systemic oxidant–antioxidant status during the exacerbation and the stable period in patients with BA and COPD. A total of 33 patients, 16 with BA and 17 with COPD were included in the study. During the exacerbation and the stable periods, levels of malondialdehyde (MDA), activities of superoxide dismutase (SOD), glutathione peroxidase (GSH‐Px), glutathione reductase (GRd), and catalase (CAT) in erythrocytes and serum melatonin concentrations were investigated. Blood counts, respiratory functions, and blood gases of the patients were also performed. During an exacerbation period of BA, despite the decreases in GSH‐Px, GRd and melatonin levels, MDA and CAT levels, and the white blood cell count, the percentage of eosinophils were significantly higher than in the stable period. Also, it was found that FEV1/L (where FEV1 is the forced expiratory volume in 1 s), FVC/L (where FVC is forced vital capacity), PEF/L/s (where PEF is peak expiratory flow), pO2 (where pO2 is oxygen pressure) levels increased during the stable period in patients with BA. MDA and SOD values were higher in the exacerbation period than in the stable period although GSH‐Px, GRd, melatonin, pH, and pO2 values were lower in the exacerbation period than in the stable period. The blood counts and the respiratory function tests did not change between the exacerbation and the stable period of patients with COPD significantly. In conclusion, we observed that oxidative stress in the exacerbation period of patients with BA and COPD increased whereas the antioxidant enzymes and melatonin values reduced. 摘要 目的:探讨腹式呼吸训练法对慢性阻塞性肺疾病(COPD)伴Ⅱ型呼吸衰竭患者肺通气状态、血气指标及运动耐力的影响。方法:选择我院2020年07月2022年12月期间收治的100例COPD伴Ⅱ型呼吸衰竭患者,根据随机数字表法将患者分为对照组[常规治疗基础上接受双水平气道正压(BIPAP)辅助通气,n=50]和研究组(对照组的基础上接受腹式呼吸训练法干预,n=50)。对比两组临床相关指标、肺通气状态、血气指标及运动耐力指标。结果:研究组的喘憋消失时间、体温恢复正常时间、住院时间、肺部啰音消失时间短于对照组(P<0.05)。两组干预1周后第1秒呼气的最大容积(FEV1)、最大自主分钟通气量(MVV)、用力肺活量(FVC)均升高,且研究组高于对照组(P<0.05)。两组干预1周后氧分压(PaO22)、血氧饱和度(SpO2)均升高,且研究组高于对照组;二氧化碳分压(PaCO2)下降,且研究组低于对照组(P<0.05)。两组干预1周后6 min步行距离(6MWT)升高,且研究组高于对照组(P<0.05)。结论:腹式呼吸训练法有助于改善COPD伴Ⅱ型呼吸衰竭患者的临床症状,调节肺通气状态、血气指标,提高运动耐力。  相似文献
